Preexisting Conditions: Currently unable to walk - neuropathy Dec of 2019 - flair-up of neuropathy, was walking with assistance before this, unable to walk afterward.

Diagnostic Lab Data: No hx of positive covid test. Client has been extremely cautious, minimal risk factors for covid-19 infection.
CDC Split Type:

Write-up: Approximately 3 hours after injection, client developed bright red, "cherry red" toes on left foot. No reported changes in sensation or circulation. Toes remained bright red for about half an hour and then returned to normal. Episodes of this are ongoing, reoccurring approximately daily. If leg is elevated for more than 5-10 minutes, symptoms resolve.

Diagnostic Lab Data: Primary care physician prescribed Metronidazole gel us 0.75% on 04/29/21. The dx was rosacea. This is not like the rosacea that has occurred in the past. Medication did not help. A Dermatologist was seen on 05/24/21. She stated it is an autoimmune reaction to the covid-injection. It could be Lupus. A steroid cream was prescribed. If results are not as anticipated, blood-work will be done in 2 months. Result is pending treatment and reaction to treatment.

Write-up: Rash on forehead, nose, cheeks of face. Skin is very dry, itching, red, irritated; large scaling skin flakes are peeling. One area to the right of right cheek has a very large scale peeled area, very noticeable, and very deep. .

Write-up: Syncopal episode 10 min after vaccination which lasted 1-2 min. Regained consciousness when laid flat on the floor without any complications. VS 10:12 122/70, HR 88, RR 16, Temp 98.6, O2 sat 99% RA, BG 100, PERRL, EOMI. Recheck at 10:38 110/66, HR 86, RR 16, Temp 98.2, O2sat 98% RA Denied any complications.

Write-up: Within about 5-6 hours of receiving the vaccine, the patient began to develop nausea, vomiting and diarrhea. He took OTC pepto bismol for relief of symptoms which lasted about 6-8 hours. The next day patient was weak and stayed home from work. Fully recovered by day 2.

Write-up: no energy; Sore arm; spike of asthma; chills; tiredness; fever; body ache; headache; nausea; This is a spontaneous report from a contactable consumer, the patient. A 60-year-old non pregnant female patient received BNT162B2 (PFIZER BIONTECH COVID-19 mRNA VACCINE Lot /Batch Number: EN6205), Dose 2 via an unspecified route of administration on left arm on 04Mar2021 at 10:15 (at the age of 60 years) as a single dose for COVID-19 immunization. Medical history included heart attack, diabetes, obesity, asthma and Sulfa allergic. The patient''s past medical history included BNT162B2 (PFIZER BIONTECH COVID-19 mRNA VACCINE; Lot/Batch Number: EN6201) dose 1 on 13Feb2021 at 10:00 as a single dose for COVID-19 immunization. Prior to the vaccination, the patient was not diagnosed with COVID-19. The patient took Nasal swab test for COVID-19 and was tested negative at 04Feb2021. Concomitant medications include Meloxicam ER (Manufacturer Unknown), Metformin (Manufacturer Unknown), Spironolac (Manufacturer Unknown), Pantoprazole (Manufacturer Unknown), Metoprolol tartrate (MANUFACTURER UNKNOWN), Atorvastatin (Manufacturer Unknown), Clopidogrel (Manufacturer Unknown), Losartan (Manufacturer Unknown), Montelukast (Manufacturer Unknown). The patient experienced no other adverse event except for sore arm until 11Mar2021. On 11Mar2021 around 03:30am it started with chills but moved on and would cycle through between chills, fevers up to 102.4, headaches, body aches, tiredness, spikes in asthma, nausea, and just an overall feeling of feeling bad and 6 days later the patient feels ok, just no energy. Patient not taken any treatment for reported events. Therapeutic outcome of reported events was recovering at the time of report. No follow-up attempts are needed. No further information is expected.

Write-up: 14 hrs after the vaccine, developed flu symptoms. beginning at 14 hrs, started taking temperature and did so every half-hour. At hour 18, temp was 100.9, 30 min later, 101.5, 30 min later, 102.0, 30 min later, 102.7. when temp reached 1019, spouse began to administer cold-wet cloth to heated areas, e.g., stomach and lower back, spouse continued until for 45 min, until temp dropped to 101.5. checked temp every hour after that, until dropped below 100.0. Temp returned to normal roughly 25 hrs after vaccine administration. 68 hrs after vaccine administration, developed cold sores. Normally, the cold sore would manifest in an area no more than 5% of upper lip area, After the COVID vaccine, the cold sore covered 35% of upper lip area, and lasted about 24 hours longer than it normally would. lasted 5 days vice 3 - 4 days, normally.

Write-up: Patient passed out a couple minutes after vaccine was administered. She was disoriented when she came to. I give her some Gatorade and she seemed ok for a couple minutes then she passed out again for 30 or 40 seconds. She was disoriented and confused when she came to. She laid on floor for 15 minutes with her elevated and then sat in waiting area with her dad for 20 minutes. She said she felt fine and color returned to her face.

Write-up: Armpit lymph nodes swelled up for about a week and a half. Along with the usual nausea and sluggishness of the the vaccine.
