Write-up: reported feeling "foggy" immediately following vaccination about 10 mins. in observation lot. told observer she was having tingling lips & had broken out in a rash with increased " foggy feeling". seen at 11:35- full facial hives & neck & chest hives with associated felling of throat tightening & dysphagia. Write-up: my skin on my body began to become sore and agitated easily to touch within 6 hours of shot. from 9pm to 6am I had chills and a high fever. for the next 24-48 hour I was lethargic, with a headache. I have since developed 2 small cold sores even with taking Valtrex daily, kanker sors inside of mouth, daily headache as well as pain in my right ear.

Write-up: Friday 4/9/2021- Nausea, Upper Body Pains (arms, chest, upper back, neck, stomach), Dizziness, unable to walk, fatigue, headache. Saturday 4/10- Same symptoms with little to no improvement Sunday 4/11- Same symptoms with minor improvement Monday 4/12- Same symptoms with no improvement, increased pains, dizziness, fatigue with headache and facial pain in eyes, cheeks, jaw, ear, nose, and back of neck, primarily on left side of face. Tuesday 4/13- Same symptoms, No improvements, increased nausea with severe headaches and facial pains Wednesday 4/14- Same symptoms, with diarrhea. Thursday 4/15- Ongoing same symptoms, little improvement with Tylenol. Less upper body pains, and nausea. Off and on facial pain and headaches but with facial numbness when without the pain. Dizziness while standing. Diarrhea has ceased. Still weak/fatigue.

Write-up: Patient received Janssen( Johnson and Johnson ) Covid vaccine on Friday 4/9/2021. Patient attended work that evening . Mother reports patient complained of feeling dizzy weak fatigued and generally not well when got home that evening at 10pm . Redness pain and swelling noted ot injection site on left arm noted at that time also. Nausea and vomiting began on saturday 4/10/2021. Nausea vomiting dizziness malaise fatigue continued through Tuesday 4/13/2021. Redness and swelling and pain to injections arm. Consulted verbally with family practice physician on Monday 4/12/2021 Benadryl and anti nausea medications used with some improvement . Injection arm visually inspected non-visit . patient missed work and school Saturday 4/10/2021 through Tuesday 4/13/2021 . report received on Thursday 4/15/2021 that patient''s nausea vomiting has resolved but continues with dizziness headache and injection site with redness, swelling . Swelling has not increased but has not decreased

Write-up: Prior to immunization the patient was extremely nervous and stated he did not like blood and was also nervous about potential side effects. The pharmacist addressed his concerns and proceeded with the immunization. While the patient was in the waiting area and decided to get up and walked over to report he was not feeling well and proceeded to faint. 911 was called and the patient refused treatment

Write-up: I did not receive treatment other than a prescription for a headaches. I ended up having a bad headache after my J&J vaccine. My headache developed at the base of my skull and radiated to the top of my neck. I had another headache above my eye as well. I also had intense joint pain from my upper leg all the way to my right ankle. I also experienced nausea along with my headache. An ultrasound of the right groin was performed to check any clots in my right leg. The doctor who saw me prescribed Emetrex for the headaches and there has been no follow up since then. I still have a headache today but is a lot less in intensity.

CONTRAST: 7 mL Gadavist. FINDINGS: BRAIN:Normal parenchymal signal intensity. No diffusion abnormality. No pathologicgradient susceptibility. Normal parenchymal volume and morphology.There is asymmetric enhancement of the right cisternal 6th cranial nerve and thevery proximal left sixth cranial nerve (series 22 image 3) as well as of thecisternal portion of the right 3rd cranial nerve (series 22 image 6). Urine drug screen was obtained and negative. RPR negative. TSH WNL at 3.61. Vitamin D low at 18.6 and started on supplementation. COVID IgG Nonreactive, patient received Pfizer vaccine first dose 1 week prior to admission. Lumbar Puncture obtained on 4/11/21. CSF studies remarkable for elevated protein 45 with normal glucose and normal WBC 1. Multiple sclerosis panel sent on CSF fluid and pending. EBV panel, Mycoplasma serology, Cocci serology, EBV serology, ANA with reflex to Lupus panel, antiGQ1B, anti-GAD, Aquaporin 4, Anti MOG sent and pending.

Write-up: Admitted to hospital on 4/10 with 2 day history of mydriasis, diplopia with blurry vision, dizziness and balance difficulty. Found to have enhancement of 6th and 3rd CNs on MRI. Patient continued to have blurry and double vision during her stay and mild gait instability. She developed mild frontal headache managed with Tylenol. Her exam was remarkable for mydriasis, mild lateral gaze limitation R$gL, mild dysmetria, dizziness and unsteady gait without notable ataxia. No papilledema. Her reflexes were 2+ in her BL UE and LEs. Pediatric Neurology and ophthalmology consulted and followed patient. Eye exam remarkable for bilateral cranial nerve 6 palsy but no other element of CN3 palsy and bilateral dilated but unreactive pupils. Patient treated with IVIG 1mg/kg/day x2 days for likely miller fisher syndrome variant of Guillain-Barre.

Write-up: I had been experiencing slight intestinal/abdominal discomfort for a week, continuing to increase during the few days prior to the acute event. In the middle of the night, while trying to sleep, I was woken by intense and painful intestinal pain and cramping, followed by severe diarrhea and nausea and vomiting, I also had a temperature of 100.5. My chest and torso broke out in a red rash. The temperature subsided after a day, and the severe diarrhea and vomiting subsided; yet the diarrhea continued. The red rash continued for three days. I had not eaten any different foods, nor had I gone anywhere out of my house , and did not have any visitors, since I had the J&J vaccine.

Diagnostic Lab Data: My doctor did run a CBC, INR and PTT on me on Tuesday, April 13, 2021. My CBC was normal but INR and PTT were abnormal.

Write-up: My arm was immediately sore and was sore for a week. Within a few hours from the vaccine being administered I was tired, hands and feet very cold, chills, headache and a fever. This lasted for 3 days. I was and am still fatigued and it has been 2 weeks since my vaccine. On Friday, April 9, 2021 (8 days after vaccine) I woke up with a sore throat and a swollen gland in my neck on the right side (side of my injection). The sore throat and my swollen gland is starting to subside on Day 14. Throughout these whole 14 days I have had hot and cold "flashes" and while I already have anxiety my anxiety has been much worse (feeling of a pounding heart) since the vaccine. It is not my normal anxiety. On Day 11, Tuesday April 13, 2021 I woke up with a black spot on the right side of my chin closer to my bottom lip. It does not hurt, not raised and it is just a black spot. It is still there on Day 14 and slowly starting to fade but not by much.

Write-up: Immediately after shot ...probably within 10 minutes or so I had vasodilation of my sinuses & headache. It came together & was quick like feeling inside of my nose swell & slight headache. I WAS VACCINATED WITH MY DAUGHTER 18 YRS OLD WITH TYPE 1 DIABETES AND SON 20 WITH PRIOR HISTORY OF DENGUE IN PHILIPPINES. At night as it was getting dark,severe symptoms hit us at home all around the same time. Severe excruciating pain in the back of our head and it ran down into our necks & jaw. I have had migraines & it was worse pain in the back of the head. My son said it felt like dengue when he spent a week in the hospital,which comes with low platelets count. To me it felt like when I had infection in my left mound after double mastectomy and spent five days in hospital hooked to Ivs with 2 antibiotics & they thought I had staff & going septic. I had horrible headache with that. My daughter reacted the worse,she was crying with head pain & shivering non-stop & her body aches hit all over first. She slept with a hot pad on her head & had ringing in the ears & if she tried to get up she heard rushing water sound in ears & felt like going to pass out. My body aches hit as I was trying to get strength to drive to ER if needs be,but if felt like crippling arthritis in arms wrists jaw legs knees & ankles. Write-up: Symptoms occurred 12 hours after shot - approx 2 am. Chills all over body - hands freezing, body aches- whole body, heart racing. After approx 6 hours - developed additional symptoms of a headache - location was back of head and severe teeth and jaw pain on right side of face. Unable to sleep all night. Write-up: pt says that she had a low grade temp, body aches and pains. On 4/11/2021 she was walking across the room and her knee dislocated then when back into place. Her shoulders, ankles, wrist and joints in her spine go in and out. Her CSF leak has gotten much worse. She cannot turn her head without having a problem. Her tremors from EDS are getting worse. Write-up: Patient developed hives from upper thigh up to her neck bilaterally. These were more localized in the torso area, to the point they overlapped. These lasted for 3 days heavily but decreased in size and quantity over time. They continue but are now much smaller and less frequent. Claritan has helped to calm the hives. Patient also noticed minor swelling of lips two days after vaccine. No swelling of throat and no shortness of breath per patient. The lip swelling would come and go and has disappeared.

Write-up: About 7 hours after receiving the vaccine I developed a painful migraine. The pain was very intense behind my left eye like there was a needle being poked in from behind. Some nights the pain is so bad it hurts to lay my head on a pillow or I wake up all through the night because of it. I took tylenol the first 4 days then stopped. Write-up: 35 y/o and 25 weeks pregnant at the time of vaccination, first time pregnancy, EDD 7/22/201. Received vaccine on Thus 4/8/21 during 9am appointment and felt arm/muscle pain immediately after, slight throat tightness. Waited for 30 minutes due to previous history of allergies and cleared to leave. After first 12 hours, had initial onset of feverish, chills, moderate headache, and fatigue that lasted through 48 hours after vaccine. Friday 4/9/21 evening ~7pm, almost 36 hours after vaccine, woke up from brief nap with profuse sweating, chills, and severe, constant abdominal pain. Symptom was new and never felt previously throughout entire pregnancy. Consulted with L&D who advised to test, hydrate, and monitor symptoms. Severe abdominal pain lasted over 4 hours, only began to subside around midnight. Next morning, pain was not as severe, but was still persisted through Sunday, and began to slowly subside with each day. Telemedicine consult with doctor on Tues 4/13/21 to review symptoms, follow up tests ordered.
