Write-up: Was on restraints 2 days after vaccination; She was talking all over the place; This spontaneous case was reported by a consumer and describes the occurrence of SOLILOQUY (She was talking all over the place) and PATIENT RESTRAINT (Was on restraints 2 days after vaccination) in a 39-year-old female patient who received mRNA-1273 (Moderna COVID-19 Vaccine) for COVID-19 vaccination. Concurrent medical conditions included Behaviour disorder. On an unknown date, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) 1 dosage form. On 31-Dec-2021, the patient experienced SOLILOQUY (She was talking all over the place). On an unknown date, the patient experienced PATIENT RESTRAINT (Was on restraints 2 days after vaccination). At the time of the report, SOLILOQUY (She was talking all over the place) and PATIENT RESTRAINT (Was on restraints 2 days after vaccination) outcome was unknown. The action taken with mRNA-1273 (Moderna COVID-19 Vaccine) (Unknown) was unknown. No concomitant medications were provided by the reporter. Patient had mentioned about treatment medication as Zyprexa but she refused to take, however, she was given 2 injections in her legs, which she does not know what it was. Patient had reported that 2 days after vaccination she was on restraints at a behavioral health facility, and already went to court. The facility made her to get the vaccine. She was calling today to say that the Facility violated her rights.

Write-up: On 12/31 woke with fatigue and lymph node pain under left arm. Driving in a car later in the day I noticed pressure/a tone in my ears. Went away after leaving vehicle. Comes back with every enclosed space where their is a pressure issue and continues for hours after. Ibuprofen helps with some of the pain but no all of it. Working in a positive pressure lab causes ear pain/pressure/ringing in the ears.

Write-up: Per pt. and older sister, she got her first covid vaccine (pfizer) on last Wednesday the 29th, Friday night the 31st she started having sore throat, then rash and swelling with itching, rash to face, lips swollen, puffy face and little rash to left hand and right arm, all itchy. No Benadryl has been given and her epi-pen has expired. No wheezing, No headache, No N/V/D. No fever. No contact with allergens, new foods, new products or new environment.

Write-up: Approximately 8 hrs after the Booster I started having the chills, shaking and my teeth were chattering. I took my temperature and it was 100.3 and my heart rate was 118. I had lymphedema after my first vaccine under my arm. It went down under my arm. But, not completely. This time the lymphedema under my arm showed up with in 8 hrs. The last two times it showed up the next day. I continued to have fever, chills, and rapid heart the next day. Two days later I felt better. Four days later I still have lymphedema under arm.

Write-up: 11 year old child experienced a sudden, severe abnormal onset of her menstrual cycle 2 days post vaccine. She has had light regular cycles before but this one came 2 weeks early. This period has been abnormally heavy in that she is soaking a pad every hour, has severe menstrual cramps, nausea, and difficulty eating. She is also experiencing light headedness and headache. Write-up: CHIEF COMPLAINT: Pt states, "I got up too fast out of bed and slid. I had a hard time getting up myself. My son helped me." Denies hitting head or LOC. Denies blood thinners. HISTORY OF PRESENT ILLNESS: 63-year-old gentleman who was recently diagnosed with B-cell lymphoma involving his sigmoid colon, right ureter, lung presenting for the evaluation of generalized weakness. Write-up: I received the Pfizer booster along with the flu shot in the same left arm (bicep location) on 12/30. I had the usual soreness in the arm on 12/30. However on 12/31 when I awoke, my entire collarbone was swollen, lumpy and extremely painful. The swollen supraclavicular lymph nodes were inflamed and at least 1-2 inches. The entire left side of the collarbone to the neck was swollen and extremely sore. Anything touching the collarbone & lymph nodes and area near the carotid artery resulted in discomfort and extreme pain. I was more alarmed on how huge my lymph nodes had swelled to. It was impossible to get any rest the night of 12/31-1/1 since the arm and lymph nodes were giving me so much pain. I was extremely nervous that this could be a clot but after checking out the collarbone and touching it in various areas, I felt it was lymphnodes. I did an internet search and saw that this was a normal effect response so didn''t seek urgent care. It is 1/4 and my collarbone is still swollen but not painful. The lymphnodes are not as huge as they were on 12/31 through 1/3. I feel you should know about this reaction I had. I didn''t have any adverse extreme side effect with my Pfizer vaccines1 & 2. Only the booster.

Write-up: Approximately 10 days after receiving my booster shot I got full-body hives located in most of the folds of my skin (under the belly, near the groin, under the armpits, behind the ears). The hives have been incredibly itchy. I have been taking Zyrtec and Benadryl since 12/31 (it is now 1/4) as well as topical Benadryl anti-itch cream. The majority of the hives have went away but some still remain on my face, scalp, palms of my hands, top of my feet, inner thigh, elbows, and side of torso. I hope that all of the remaining hives disappear by the end of the week (1/8).

CT chest CLINICAL HISTORY: Dyspnea. Elevated D-dimer. Elevated WBC count. Abdominal pain. Elevated liver function test. COMPARISON: CT chest 08/14/2021. Today''s chest x-ray. LABS: None. CONTRAST: Exam performed without IV contrast. All CT scans at this facility are performed using dose modulation techniques including the following: automated exposure control, adjustment of the mA and/or kV according to patient size, and/or use of iterative reconstruction technique as applicable. FINDINGS: CT CHEST: There is extensive infiltrate seen throughout much of the right lung. These have shown significant progression compared to prior exam and presumably secondary to pneumonia. There are mild-to-moderate patchy infiltrates involving the left upper and lower lobes. There is more focal consolidation near the costophrenic angle in the left lower lobe. The asymmetry in distribution pattern is atypical for COVID-19. Other etiologies of pneumonia should be considered. Aspiration remains a consideration. Small- to moderate-sized right pleural effusion is new. Small left pleural effusion which may be somewhat loculated (on prior CT, there was a moderate-to-large left pleural effusion). Mild-to-moderate cardiomegaly. Mildly enlarged pretracheal lymph node, image 33, measuring 1.4 cm is new. This could be reactive in nature. No axillary adenopathy. No pericardial effusion. Calcification of the aortic valve can be associated with aortic valvular stenosis. Nodular and heterogeneous appearance of the thyroid gland. Mild degenerative changes in the spine. There is somewhat nonspecific edematous change involving the subcutaneous fat/breast tissue bilaterally. IMPRESSION: 1. There are extensive infiltrates throughout the right lung. Mild-to-moderate infiltrates involving the left lung. Small- to moderate-sized right pleural effusion. Small left pleural effusion which appears to be somewhat loculated. 2. Mildly enlarged pretracheal lymph node may be reactive in nature. Cardiomegaly. FINDINGS CT ABDOMEN AND PELVIS: Motion degrades image quality. Write-up: Delayed Reaction to the Covid-19 Moderna Booster shot - 12/28/21 - vomiting and diarrhea - resolved within 24 hours; tiredness12/31/21 - developed rash on arm1/1/22 - woke up with rash and welts on hips, arms, neck, stomach; complained of a ''rock'' feeling in his upper stomach which resolved in 48 hours; took Maalox tablet for the "rock" feeling in stomach at bedtime; took one dose of Benadryl for the rash - didn''t help. 1/2/22 - woke up with joint pain in shoulders, elbows, hips and back really hurt; Rash got worse. Determined via home test that patient likely has dermatographia - ''skin writing'' reactions. He cannot place any pressure on his skin or scratch in any way or welts develop within minutes - anywhere on his body. In the evening, developed fever 100.2 by mouth, chills, muscle aches, sore throat that felt like it was ''closing'' although he did not have any difficulty breathing; took 1 Allegra dose for dermatographia - didn''t help; took two ibuprofen for pain. 1/3/22 - Dermatographia still a big problem. Sore throat was better. Fever broke in afternoon. Still had major muscle aches at bedtime, but joint pain had resolved. PA advised to take 2 Claritin daily for 7-10 days; also took two Tylenol for pain at bedtime. 1/4/22 - Dermatographia still a big problem. So uncomfortable and cannot do daily activities - must wear loose clothing and not touch his skin or welts develop; took 2 Claritin for dermatographia/rash - seems to help a little with the itchiness/

Write-up: About two weeks after the moderna booster shot (3rd shot) I started breaking out in hives on my thighs, feeling pressure on my esophagus and also having a weird feeling in my bowel movements. Initially thought I had a food allergy but after 3 days of no relief with OTC meds, I decided to look up possible names for what I was experiencing. I came across a medical board discussion where various people like me had similar issue after their booster shots. Write-up: On the afternoon of December 31st a rash broke out on my forehead. When I woke up on January 1st it had started changing to blisters and the whole right side of my face hurt and started swelling. I thought it was just a contact dermatitis. Woke up the next day and it had gotten worse. Finally, on the morning of the 3rd I realized it was something else and went to the urgent care. I was immediately diagnosed with Shingles and was prescribed and anti-viral medication. Because of the location I immediately had to go to the Opthamologist to make sure it was not in my eye. Write-up: Moderna Booster RT deltoid @ 2:30 pm on thurs 12/30/2021 NO PRIOR ADVERSE RXN TO VACCINATION #1 05/12/2021 #2 06/08/2021. First symptom 0330 on 12/31- generalized aching, severe upper body, And headache Legs better by mid day; upper Body myalgia persisted----" felt like hit with baseball bat, never felt like that" 11 am Friday developed " instant chest pain over left breast about 2 in medial /superior to nipple with a palpable nodule size of "walnut" 1840 pm developed pain in right axilla, HAD A tender & LARGE "SOFBALL SIZE" lump WITH ERYTHEMA, then to ER ON Saturday 1/1. Erythematous rash with tiny papules developed in a large area to back and along the anterior axillary area. At ER MASS MEASURED 3 IN w x 4 IN Length. Rash was well beyond those parameters. Mild Tenderness in infra-axillary region persists. Sunday 1/2 developed erythema at injection site and axillary rash worsened. LFTS ELEVATED BEYOND BASELINE This has been followed up by Dr, ER physician by phone.

Write-up: It was a booster shot. First two shots were Pfizer. Two week since the shot, my left scalp started feel tingly and tender to touch. Pulsating headache started that evening; feels like electric shock sporadically. Next day, the headache became more of a throbbing pain and started making its way down to the left ear area and down my neck (on the left side). Spoke to the naturopath, wondering if it may be the supplements (all of the supplements I have been on for more than 3 months+, and nothing new was added). Measured blood pressure and it was 119/75 normal. Took 2 Advil to manage the pain. Couldn''t sleep that night. Next day, noticed rash/hive near the forehead/hair line (close proximity to the tender spots). Headache remains to be rhythmic throbbing. It goes away if I close my eyes and sleep face down on my pillow. Today is day 4 and hive is worse and skins in the area is starting to flake.

Write-up: I got the booster on Dec. 30, 2021 at 10:50 a.m. Around 6:30 p.m. the next day, I had a sharp, shooting pain down my leg while passively standing. (I was not hinging, lifting, etc). I have "thrown my back out" approx. 3 times now, but the last time was about 18 months prior to this incident on 12/31/21. Also, the other two times, I was hinging or exercising. This seemingly happened completely untriggered. While I have not seen Dr. about this incident on 12/31/21, I have seen him ~18 months ago for the same. Dr. recommended I work on my core and I have been doing so pain-free in classes and private sessions 4x/week. This seemingly came out of nowhere, and I have read in an online thread that people who were doing fine with longer-term, chronic back issues had experienced "flare ups" after getting the booster, hence my report. It has been 1 week since my booster, and 6 days since I threw my back out, and I am still experiencing pain, however I know the kinds of stretches I need to do in order to heal based on past experience, thus I have not contacted Dr. I would be open to doing so, if necessary.

Write-up: That night, after receiving the vaccine I developed severe body aches and joint pain, and a mild headache. I took gabapentin and advil for the pain, and used iced packs, which only seemed to take the edge off. On the evening of 01/01/2022 I developed itching on my face. So, I went to the mirror to look at my face and saw the left half of it was as red as a raspberry. Also, I had developed a rash across my neck, chest, arms and back. I called the COVID line and they told me to go to the ER. I wanted to avoid that so I could avoid COVID exposure, so I told them I took allegra and benadryl, and had an epipen for safety, they said it was okay, but still urged an ER visit. I waited until that Monday 01/03/2022 to call my doctor''s office when it opened. They told me to continue the allegra and benadryl, and to increase the NSAIDS along with taking pantoprazole to coat my stomach. If I did not improve, I would need to start steroids, but I did not want to because I didn''t want to lessen the effectiveness of the vaccine. As of yesterday 01/05/2022 my rash has subsided and the body aches have subsided, but the joint pain and the mild headache still remain.

Write-up: 10 days after my booster shot (3rd), I broke out in full-body itching and hives that started on my scalp and then moved to every part of my body. I have never had an allergic reaction to anything in my life, including previous vaccinations. I am now on day 6 and the itchy rash comes and goes all over my body. I have been prescribed Methylprednisolone and I take Claritin every morning to try to counteract the itch. I am reporting this because I have not seen any guidance or a reason for full-body itching allergic reaction in response to the vaccine.

Write-up: On Day 3 I started to develop severe coughing and sinuses problems, and a bad cold. The cough is so extreme that I keep vomiting and at times cannot breathe properly and have missed 4 days of work to date. A video appointment with my Dr suspected sinusitis and bronchitis and received antibiotics and cough syrup. As it didn''t help and the cough became so severe, I sought urgent care on the evening of 01/04/22. An x-ray showed no abnormalities and the covid and flu tests were negative. I was given Prednisone and stronger cough suppressant medication. I still suffer from severe cough and flu symptoms and I can only eat liquid food. I strongly suspect an allergic reaction to my 3rd dose, though I took the same (Moderna) as previously given. Hence, I''m reporting this as I''m very concerned for my well-being as well as for others.

Write-up: Patient hospitalized for 2 days. 83 y/o male HX of COPD on 2L at baseline who presented with acute hypoxic respiratory failure due to COVID-19. He had symptoms for about 2 weeks prior to presentation. He ultimately presented to urgent care and was sent to the ER. He required 4 L of oxygen to maintain his saturations in the ER and was started on Decadron due to his hypoxia. CT thorax with contrast showed no PE. He was weaned to his home baseline 2L easily. He was able to ambulate with minor desaturations but recovered quite quickly. Physical therapy and occupational therapy cleared him for home without services. He was discharged to home in stable condition. He was found to be hyperkalemic on admission so his potassium supplements were stopped. He remained mildly hyperkalemic, will need this rechecked outpatient.

Case was discussed with on-call stroke neurologist who advised multifactorial encephalopathy less likely CVA with no further neuro workup indicated. Patient with chronic dysphagia was evaluated by SLP who advised dietary modifications. Given that patient COVID-19 positive with no evidence of hypoxia and numerous comorbidities monoclonal antibody infusion clinic was contacted. Patient provided fact sheet and discussed risks and benefits of medical antibodies which patient ultimately declined. She was also unable to be evaluated with esophagram and VFSS while under COVID isolation. She continued to have significantly positive orthostatic vital signs in seated as well as standing positions. She was initiated on midodrine and Florinef therapies with improvement of blood pressures, improved dizziness. 2D TTE Limited without significant abnormalities. PT/OT evaluated patient and advised home with assist and outpatient therapy services PT/OT. Patient clinically improved and hemodynamically stable for discharge. Urine cultures with E coli and E faecalis both sensitive to ampicillin. She was transitioned from IV to PO Augmentin without any side effect. She will complete 5 additional days of antibiotics. Discussed plan of care discharge with patient including outpatient follow-up PCP within 7 days. He was recommended to patient follow-up outpatient Neurology and neuropsychiatry for further evaluation. He was also advised patient follow-up outpatient psychiatry to review medications as they may be contributing to orthostatic hypotension. She was also advised to continue outpatient GI and SLP evaluations for ongoing dysphagia. Patient voiced understanding was agreeable with plan of care discharge

She has a known history of chronic obstructive pulmonary disease and chronic hypoxic respiratory failure requiring 3lpm NC of O2 at baseline. Write-up: Pt presented to the ED on 12/31/21 Patient is a 52-year-old Caucasian female who presents to the emergency room with chief complaint of nausea and vomiting. She states that her whole body hurts. She states generalized weakness and fatigue. For the past 2-3 days she''s had several episodes of nonbloody emesis to the point that she is not even able to keep down her usual home medications. Write-up: Pt presented to the ED on 12/31/21 Patient is a 52-year-old female who presents to the emergency room with chief complaint of nausea and vomiting. She states that her whole body hurts. She states generalized weakness and fatigue. For the past 2-3 days she''s had several episodes of nonbloody emesis to the point that she is not even able to keep down her usual home medications. She states that she has abdominal pain with shortness of breath. No chest pain. She has a reactive cough. She is fully vaccinated for coronavirus and received her second shot in April. Has not received a booster shot. She doesn''t smoke. No alcohol or illicit drug use. Denies history of previous heart attacks... Pt presented to the ED on 12/31/21. Write-up: I have an IUD and do not have periods. I have had a period since 4 days post injection. It has been 7 days and I am still bleeding which is longer than I have ever gone before. The discharge is light and dark brown. Today on day 7 I have been cramping and the flow is heavier than on the previous days.
