Write-up: Around 1730: Patient was found unresponsive in the bathroom by a patron who alerted PHN. Patient was unresponsive, breathing, and had a pulse. 911 was called for EMS and further medical staff came and patient''s head and neck was stabilized. Patient woke up and was A&Ox1, to self. EMS then arrived and transported patient to local emergency department.

Write-up: Horrible side effects within 9 hours and had to be admitted to emergency room. Husband and son also got shot at same location with similar side effects but not requiring hospitalization. Massive body shakes, chills, slurred speech, accelerated heart rate, high blood pressure, shaking so hard could get EKG

Write-up: Approximately 2 hours following vaccine administration, patient began to feel short of breath and had chest pain, so they returned to the vaccine clinic. From there, she was taken to the ED. Was observed for 45 minutes following the vaccine without any signs or symptoms of reaction. On arrival to the ED, she had redness without hives on the anterior portion of her neck. At that time, patient denied shortness of breath or chest pain. Given 25 mg diphenhydramine po in the ED. Discharged home with instructions for use of Benadryl if needed.

Diagnostic Lab Data: That morning of 03/27, in order to evaluate the extent of the injuries and rule out contributing factors, I received the following lab tests: - Acetaminophen level - Alcohol - CBC with differential - Cell count, CSF (performed 2 times) - Comprehensive metabolic panel - Covid-19 PCR - Culture, CDF with gram stain - Glucose, CSF - Lactate - Magnesium - Protein, CSF - Prothrombin Time/INR - Salicylate - Troponin - Urinalysis with Microscopy - Culture, blood, bacteria and yest (performed 2 times) - Drug screen, urine - CT cervical spine W/O IV contrast -CT Head W/O IV contrast - ECG - 12 Lead - X-ray chest single view - Lumbar Puncture

Write-up: Only minor chills before bed, 11pm. Muscle aches, felt as tightness in IT bands, made remaining asleep difficult. Got up to use the bathroom around 5am. Upon finishing peeing while standing, I lost consciousness, falling. As a result of the fall, I fractured my cheek bone, cut open my ear, later requiring three stitches, and was concussed, resulting in a loss of memory. I was nonresponsive when my fiance found me immediately following the crash, and was rushed to the hospital via ambulance. Upon arrival to the hospital, my fever was found to be 102 F. As a result of the fall, I have a cheek bone fracture, three stiches in my ear, and a concussion.

Write-up: A vaccinator came to PHN to report a woman in the bathroom who was throwing up. PHN and vaccinator came to attend to the client. Client said she was feeling warm and her head was throbbing. Client said her name. Client was eventually able to sit up in a chair and said she was feeling hot and lightheaded. Client asked to lay on the floor and legs were placed on a chair. Client said she was feeling better after this. PHN came back to the bathroom with ice pack, fruit strip, water, and apple juice. Client seen throwing up again, vaccinator was standing by with client. Vital signs taken after episode of emesis. BP: 113/84 and HR: 75. Client asked to take deep breaths and slow breathing, which client was able to do. Client said she was still feeling hot so client was given another ice pack. Client felt cold after some time so both ice packs removed. PHN asked another PHN to come assess client and Dr. called in to assess client also. Client was throwing up for the third time, and Dr. suggested we call 911 to get her checked out. EMS called at 1808 by other PHN. Client said she is only allergic to an antidepressant but did not recall the name. Client continued to talk and be responsive throughout entirety of event. Client called someone she knew to pick up her truck as she knew she would be going to the hospital. PHN took down name of friend, his number, and client?s keys. PHN gave keys and information to client flow lead, who gave items to an employee to ensure keys were returned to trusted entity. EMS arrived. Client denied any swelling, throat closing sensation, sensation of a thick tongue, or itching. Client taken on stretcher to Hospital.

Diagnostic Lab Data: EMT was on site and administered EKG, and O2 meter. I didn''t feel anxious, and have had allergic reactions before- but this one was pretty bad. I wasn''t thinking clearly, as I know now I should have done my epipen. I did do my inhaler 3 puffs, along with the benadryl. Not anxious, and was able to drive home after I waited awhile.

Write-up: Covid shots administered at airport for employees. I received shot, and within a minute or two- my lips and tongue started to tingle, swell and go numb. My throat started to swell too. Had a hard time starting to breathe and took my xopenex inhaler. I had taken a benadryl beforehand, and ended up taking two more as EMT came over to help me there at the site. She kept an eye on me. My A Fib was speeding up and getting irregular too. After about half an hour, it stopped getting worse and seemed to level off. They offered to take me to ER, and had me promise to call 911 if it got bad again. Still felt bad the rest of the day. Felt a little better later the next day, and a little better (today is day 3), but still not 100%. I did contact my doctor to check in

Write-up: Chest heaviness and Extremely bad leg pain; lower leg (calves) hurt the worse. I have been feeling the pain since Monday, March 15. I went to the Urgent Care on March 16; I was sent to the ER where I stayed overnight. They ruled out heart related issues and blood clots. I had a follow up appointment on March 26; he said that he had seen patients who experienced the same, and it was due to the vaccine. Unlike them, my pain remained. He thinks that the vaccine triggered something else in me. After his appointment, I saw my orthopedist who prescribed meds to help with leg pain. If pain is not better in a month, then he will order a MRI. I saw my rheumatologist Monday, April 5. She is going to work with my orthopedists to treat me. Currently, I am doing the exercises that were prescribed for me to do at home, and I am on pain meds.

Write-up: 0213A21A -(The lot number is hard to read and it could be this number or the one above.) Wednesday, Thursday and Friday - I had headaches and I had extra fatigue (I already do have fatigue because I have insomnia). My arm soreness was on the other arm not on the one that I had an injection. Friday and up until yesterday, I had nausea. Saturday, I had headache and being tired. Sunday, I had chest pain (woke up with it) like a burning in my chest. My left arm, I felt a tingling down my arm to my legs - it almost felt like a blood clot was there in my arm. I had to put heat on and then ice. I couldn''t extend arm because it hurt. Midnight on Monday, the burning and the chest pain was too much and I was getting worried. The only thing I have is asthma, otherwise I''m healthy. I went to ER - they gave a Cocktail of Lidocaine; Malex and something else to see if it would help with the burning in my chest. Monday, I took Tylenol. Tuesday, the chest pains were more intense. 03:00 on Wednesday am - I went back to ER. They brought me to a full chest x-ray - front, side and back and a CAT SCAN and gave me another Cocktail and also put something in my IV. This time, they sent me home with four different medicines. The meds are: Generic of Nexium; Aspirin low dose of 81 mg - bayers children - one a day; Generic version of Bentil Triacetin - I take this when I want to be still; Generic version of Valium - they gave me four of these - and I have one left that I will take tonight so I can sleep. I am taking Vit D. I haven''t taken Tylenol since getting on these. I try to stay hydrated. I still have a really really funny feeling inside me. Last night, I was very restless and couldn''t sleep. It makes me on borderline and dizzy and my heart starts palpitating. It''s a scary feeling in my chest.

Write-up: Felt hot within a few minutes after the vaccine, then passed out. The NP on site said she was unable to find a pulse so did 10 chest compressions before I woke up. Called the rescue squad who did an EKG on route to Hospital and once I was in the ER another EKG was performed along with blood work and chest X-ray. Entry was noted as allergic reaction but diagnosis was noted as Vasovagal Syncope, Adult. EKG?s performed along with labs were normal. I have pulled muscles, tendons and ligaments from the chest compressions which were questionable but since that day have experienced tingling in upper body and arms and have felt bad. Went back to my Primary Care Doctor for follow up on March 29 for another EKG. Have had panic attacks from the situation and per Doctor will take 2 months for my chest, muscles, tendons and ligaments to heal. Hard to determine if the way I have felt is from the vaccine or what transacted when I fainted. Today which is April 9, I still feel weak, normally I am very energetic. Just wanted to report as when I saw the news that others had some situations thought best to let you know.

Write-up: Patient was given the vaccine at 12:26 pm and was advised to wait for observation. While waiting, patient started to complain of some anxiety and lightheadedness. We advised patient to wait a little longer for further observation. A few minutes later, at 12:36 pm patient said the lightheadness was getting worse and then passed out for a few seconds while waiting sitting in our observation area. The pharmacy technician was in the area to hold up the patient and keep her calm while patients mother requested that EMS/911 be called. EMS came minutes later and patient was coherent and talking to the technician and her mother. EMS asked patient if she wanted to be taken to the hospital and patient agreed. Patient called a few hours later to follow up with pharmacy staff and informed us that she was discharged and feeling better.

Write-up: On 04/04/21, around 11 AM, the customer walked in for his COVID vaccine appointment. After registering for the vaccine , the patient sat in the designated area and one of our immunizing technicians administered the vaccine while I was observing and I left the immunization area while my technician was finishing up cleaning. When the patient asked the immunizing technician a question about side effects, she offered him to talk to the pharmacist at the consultation window. While I was assisting the next customer, I heard a loud sound and saw the customer collapsing on the floor. I ran outside and checked his pulse, and asked to call 911. He was conscious and breathing. I went inside the pharmacy to get EpiPen and informed the customer that I will be administering EpiPen, but he denied it. In the meantime, the immunizing technician was already on hold with 911 and informed that they are on the way. The customers in the waiting area also kept him engaged and were watching him if he is alert. The store manager on duty was also in the pharmacy area. I came inside the pharmacy and offered the patient a chair to sit in. Paramedics arrived in a few minutes to transport him to the hospital. The patient came back to the pharmacy around 2:30PM to collect his immunization card and when I inquired about how he is doing, the patient informed me that he is feeling more better and the ER determined that there was no allergic reaction to the vaccine but the patient most likely stood up too quickly after the vaccine was administered.

Write-up: ATRIAL FIBRILLATION; VASOVAGAL EPISODE; This spontaneous report received from a patient concerned a patient of unspecified age and sex. The patient''s weight, height, and medical history were not reported. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, intramuscular, batch number: UNKNOWN) .5 ml, administered on 08-MAR-2021 14:45 for prophylactic vaccination. The batch number was not reported and has been requested. No concomitant medications were reported. On an unspecified date, the patient experienced a very adverse reaction. According to the physicians at the site, it was a vasovagal episode. Ultimately, the patient had to go to the ER because of atrial fibrillation. Write-up: Waiting 15 minutes with no issues. Got in car to drive home and after about 5 minutes I got extremely flush and hot. I then began shaking. got very dizzy and light headed. My throat began to get very tight and hard to swallow. Got to friends house in about 15 minutes. Took 30 mg of prednisone dissolved in water. Symptoms did not subside and went to ER. ER gave me IV benadryl, steroid, and antacid med in IV with full bag of fluids. Sent home with prednisone prescription sent home.

The patient''s weight, height, and medical history were not reported. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 1805022, expiry: UNKNOWN) dose was not reported, administered on 19-MAR-2021 approximately at 10:30 am for prophylactic vaccination. No concomitant medications were reported. On 19-MAR-2021, within 6 hours from vaccination, the patient developed fever, chills, fatigue, which she treated with rest and acetaminophen. At 24 hours post vaccine, on 20-MAR-2021, patient experienced hyperactivity, flight of ideas, emotional lability, and visual and auditory hallucinations. At approximately 50 hours post vaccination, the symptoms worsened and patient was brought to the emergency room on 21-Mar-2021. Hospital Providers found lab work: CBC (complete blood count), BGM (Blood glucose meter), metabolic panel, urine analysis, EKG (electrocardiogram), chest x-ray, CT (computerized tomography)of the head all to be within normal limits. Patient was admitted to the hospital for altered mental status. On 22-Mar-2021, providers ordered EEG (electroencephalogram )showing irregularity in the temporal lobe, MRI(Magnetic resonance imaging ) of the brain within normal limits, Lumbar puncture negative for bacteria, HSV, normal glucose/protein. ANA blood test positive. Due to persistent psychological symptoms and all medical tests coming back essentially normal, on the 4th day of her hospital stay, patient was admitted to the psychiatric unit for question delirium vs. late onset bipolar disorder. The environment on the psychiatric floor was worsening her symptoms ,and family and hospital providers agreed that she would do better in her home environment and she was discharged on 30-Mar-2021. Tested neg. for strep. 3/29 - Went to PCP for continues pain in head and starting to have pain in lower back and down both legs. Some pain in arms too. 3/31 - Went to PCP for increased pain, weakness and numbness in legs and arms. Took blood for tests. COvid test 4/3 - Went to ER with intense pain/numbness/weakness/ restlessness in legs and some in arms/hands. Head still hurts too. More blood work, given Hydrocodone/Ibuprofin to help manage pain. 4/5 - Returned to PCP. Was given Gabapentin to help with the pain. Was referred to neurologist. 4/8 - Initial exam at neurologist. Plan to complete MRI and Nerve Conduction studies. 4/9 - Had MRI completed of brain and upper spinal cord.

Write-up: Immediately following the vaccine administering I stood up and felt very sick and nauseated. I began to sit down again and was brought water. Write-up: Patient came today to receive his Janssen Covid vaccine and experienced feeling faint and light headedness few minutes after administering the vaccine. He has no history of allergic reactions to a vaccine, medication or food. There was no visible swelling or localized reactions to the injection site. I cleared the table next to him and had the patient lie flat on the floor with feet elevated for several minutes. Diagnostic Lab Data: 4/9/21- went to the urgent care but they did not do any tests, and told me they couldn?t verify that I had an allergic reaction, even though my tongue and throat were still mildly swollen and sore.

Write-up: Received vaccine at 4:10pm on Thursday 4/8/21. 2 hours later, at 6:15pm, I noticed mild tongue swelling which slowly continued to worsen. I pulled out my Epipen but was not having any difficulties breathing and my tongue was only mildly swollen still, so I took Zyrtec at 8pm (which I usually do), and continued to monitor. Zyrtec seemed to slow the progression of tongue swelling slightly but at 10pm, I started having a dry cough and wheezing. I took Albuterol and Benadryl and that worked to stop the tongue swelling. Please note, the reason I don?t use Epi immediately is because I have Reynaud?s and it is contraindicated. So I use other rescue meds first if I can. My O2 sats remained above 95%, so I was not in full anaphylaxis. I did not eat anything or come in contact with anything within those 2 hours that would have caused the allergic reaction, so I am positive it was the vaccine. I also have a red, raised itchy bump at the site.

Write-up: I went to bed that night 6 hours after vaccine, and woke up with extreme flu 8 and half hours after vaccine. High fever chills, vomiting, body aches. The next morning 15 hours after vaccine my arm started to hurt so badly I could barely move. The nerves in the injected arm running up and down with shooting and pulsing, lack of feeling in hands and extreme pain through the the arm and shoulder on the injected side. The pain was extreme for 13 days I lost work time. I saw several doctors who sent me on my way. My arm still hurts right now 24 days since vaccine. The pain is not extreme but the nerves still shoot pain and oils and there is often extreme pain by the end of the day.

Write-up: Within minutes of getting the injection, became faint with a severe drop in blood pressure. Under observation at the site by Health professionals for 50 minutes. On leaving the facility, on the 15 minute drive home, had some tongue swelling and other indications of anaphlyaxsis, so drove directly Hospital ER. They suggested fluids and prescribed Benedryl and waiting nearby. When symptoms did not worsen, left and arrived at home around 2 p.m. Now 24 hours later, still some swelling of tongue and tinnitus, along with site soreness and dizziness.

Write-up: Standard side effects occurred within 24 hours, shivering, chills, body aches, fever of 100.3. Fever would not break until later that evening even when taking one Tylenol every four hours. Day two I couldn''t tell if I was actually looking at myself in the mirror. I was dizzy and felt high. I could not remember simple tasks I just did like washing my face in the shower. I felt like I was hallucinating. I went to urgent care and they had no recommendations. I was to take the LSAT today, Saturday, April 9. I had to cancel it because I''m still not fully coherent. I still feel dazed and out of it. I tried to go to work yesterday and old lasted an hour. I had a COVID test performed yesterday on my own accord, not per the request of urgent care, just in case I caught it prior to the vaccine without knowing, and it was negative. I can remember things now at least but I''m still not mentally coherent. It''s a scary feeling. I am about three and a half days post vaccine and slowly becoming myself again. I still feel dazed but I am able to have a conversation and remember it.

Said that her fingers felt numb and tingly (could not pick up water or orange juice...we had to put water and juice to her lips to drink). She said that she needed to lay down. Per patient she had oatmeal with very little sugar for breakfast and coffee. Believe the reaction was a vasovagal response.

Diagnostic Lab Data: The EKG report on April 9, 2021 found that there is a slight delay in blood flow to ventricles and the electrical current for the heart is not as strong as it should be. The Plan is weight loss, exercise, and follow-up with PCP. Although I am overweight for my age and height (within last year weight gain), I am concerned how sporadic my BP has become since receiving the vaccine on March 12, 2021.

Write-up: Approximated 12-13 minutes after receiving the shot; my respiration increased, heart rate, and my vision became blurred. I reported the blurred vision to the Pharmacist and she said they have not heard reports forth this symptom. Based upon events following the injection relative to my blood pressure; it is likely that my blood pressure became elevated following the injection, resulting in blurred vision. My blurred vision resolved several minutes later. However, my heart rate increased later that evening to the point I slept upright in my recliner that evening. During sleep, I awoke from my sleep gasping because my heart felt like it was beating too slowly. It has not been several weeks since receiving the injection, and during this time period, my Blood Pressure (BP) has been fluctuating from "very high" to normal. Yesterday, I presented to the ER due to High Blood Pressure (145/100), by the recommendation of a NP. I have NEVER had any history of high BP prior to the injection.

He was seen in ED and 03/07/21, given IV Solu-Medrol and prescriptions for Medrol taper plus Pepcid. Reportedly he is seen his primary care provider several times in the interim but presented back to the ED on 3/29/21 for a pruritic rash. He was admitted for 48 hours during which time he underwent nondiagnostic skin biopsy, and was discharged home on steroids and the proton pump inhibitor. Within 48 hours of discharge she was readmitted with a mild lactic acidosis and persistent rash.

Paramedics noted that patient was ?very pale? and was still light headed 30 minutes after receiving the vaccine. After consultation with paramedics and patient, it was agreed to transport because of low vitals and no change in status. Write-up: Patient arrived to Public Health Vaccination POD at the Fairgrounds for a Covid-19 vaccine on 4/10/2021. He received a dose of Janssen, Lot #1808982, in his left deltoid at 11:37am. In the car with him were 3 other adults/relatives who also received the vaccine and an infant in a car seat; Patient was the driver. After receiving the vaccine, patient pulled away from the table, reportedly became faint, briefly lost consciousness, and crashed his Jeep into the barricade at the side of the large garage door where he was attempting to exit. The Jeep sustained damage to the front passenger side. Medics were called to the scene. The first medic on the scene noted that Patient was diaphoretic and a noted an irregular pulse. Patients seat was reclined. Additional medics from ambulances in the parking lot arrived to the scene. Patient was put on a stretcher and taken to the ambulance. An IV was started and glucose level checked (result 98). Medics continued evaluation and patient was ultimately transported to Medical Center for further evaluation.

Write-up: I am well managed on 20 mg Olmesartan Medoxomil daily and BP generally runs about 130/80. The day following the vaccine, I experienced a faster than normal heart rate. I checked my BP about 4 PM and found it to be 180/94. I took a second Olmesartan and waited 2 hours. 6 PM my BP was 190/96 - I took a diuretic. 9 pm rechecked my bp but machine only showed an error. I took a second diuretic and went to the ED. Upon arrival my BP was 230/110. I was in the ED for 3 hours and during that time my BP slowly lower to 166/90 and discharged without treatment.

Diagnostic Lab Data: The EMTs tested my blood sugar, pulse, oxygen, temperature, and gave me an EKG. All of my numbers were perfect.

Write-up: I suddenly became lightheaded and had extreme weakness, then collapsed, then immediately began having chest pain and pressure, shortness of breath, arm numbness, and uncontrollable shaking. All of these symptoms happened at the same time. We called the paramedics but all my vitals were totally normal. The severe symptoms passed after about three hours. I continued to have chest pain and pressure, but was able to breathe normally and had 100% oxygen saturation. I am not a smoker and don''t have asthma or any breathing issues so the sudden inability to breathe coupled with the other symptoms all at once was very alarming.
