Write-up: Burning pain started in right leg and thighs on 5/6. On 5/8 he fell from his legs giving out. Pain continued to worsen and both legs began the burning. From 5/8-5/26 he fell 8 times from his legs giving out. On 5/26 he was admitted to hospital and was released on 5/28. He had bloodwork, CT scan, MRI?s and a brain scan and no cause was discovered. The pain and his legs usage continued to worsen. He had in-house physical therapy from hospital discharge. By 7/4 the pain was horrendous and he went to the emergency room at a different hospital. He was admitted and they did bloodwork, MRI?s, and a spinal tap. On 7/6 he was diagnosed with Guillain Barre Syndrome. He immediately started treatment which consisted of infusions for 5 days. After the infusion treatment it has been the task of pain control. By this time his muscles have deteriorated and walking has become a challenge and struggle. On 7/24 he was discharged from the hospital to a rehab for physical therapy and occupational therapy.

Write-up: Noticed tingling and numbness in both my feet first, then it moved into both my hands. best way to describe it would be if your hands and feet fell asleep then you start to use them for the first time as all the blood starts to flow. I thought i might of had a pinched nerve so i started chiropractic treatments. after two weeks with no relief i went to my primary care doctor. He referred me to a neurologist. by the time i was able to see a neurologist my strength in my hands has diminished pretty rapidly as well as it was difficult to walk as my legs were weakening as well. After initial visit with Neurologist the Dr. suspected a neuropathy and called it GBS. Guillian-Barre Syndrome. She scheduled a nerve test and after the nerve test she confirmed the GBS and scheduled me for IGIV treatment. Treatment was about 4hours per day for 5 straight days. Since treatment i have seen some improvement in strength, but feet and hands are still tingling and numb. Time Frame. June 7th noticed symptoms, June 23rd started Chiro Treatments, Saw Primary Care Doc June June 28th, Saw Neurologist July 13, Performed Nerve Test July 14, Started IGIV July 19 and finished IGIV on July 23.

Write-up: 7/8/2021-numbness started on left two fingers then numbness progressed to the right hand within a day.7/19/2021-started with upper back pain, throbbing in nature started at the spine and moved outward,took Aleve and muscle relaxer without relief . 7/21/2021 started with weakness all over body. 7/22/2021 went to ER was admitted with a diagnosis of Guillain-Barre given IV IG x 5 units with relief, but continues with numbness in bilateral hands and feet L$gR. Now has high blood pressure running 150-160 over 100-106 and believe she is prescribed lisinopril 5mg, taking Aleve for pain and percocet at h.s. Transferred to rehabilitation unit on 7/28/2021.

Write-up: Symptoms began roughly one month after administration. Symptoms began with numbness and tingling in the feet, moving to the calves, upper legs, arms, hands, and face. Weakness developed in the extremities shortly thereafter. Patient consulted with primary care provider (?PCP?) on July 21, 2021. PCP ordered various laboratory blood tests and recommended patient see a rheumatologist. On July 27, 2021, patient contacted PCP for a neurology referral. Upon receiving referral, Patient contacted Neurology for a consult. Patient described symptoms to via a web portal. On July 28, 2021, ,clinic advised patient to go to the emergency room. Patient visited emergency room at Hospital and Health center on July 28, 2021. Patient was admitted to the hospital with a diagnosis of Guillain-Barr? syndrome on the same day after a clinical examination, MRI of the entire spine, and a lumbar puncture showing elevated protein levels. Patient lost almost all strength in legs and developed saddle paresthesia shortly thereafter. Patient was treated with Immunoglobulin therapy over the course of five days. Patient was discharged from the hospital on August 4, 2021.
