Write-up: Refusal to eat solid foods started in July 2000-June 2001. Unable to sleep for more than 3-4 hrs at a time (previously slept throught he night) this began in July 2000 until February 2002. Chronic diarrhea began in October 2000 and lasted until June 2001 when we began a Gluten and casein free diet. Fine motor skills ceased to continue to develop beginning in July of 2000 until September 2001. Occupational therapy for treatment. In July 2000 speech ceased to continue to develop. Speech problems continue today. Speech therapy for treatment. Sensor issues became obvious in August 2000 and continue to a mild degree today. Occupational therapy for treatment. In April 2002 the measles virus was found in pt''s spinal fluid. No treatment known for this. The 60 day follow up received on 7/24/03 states seizures, chronic measles virus in brain, continued behavioral problems, GI problems. This is a chronic life long illnes. No cure. Life expectancy could be compromised due to MMR.

Write-up: Five days after receiving his MMR, (the child) had a seizure. He did not have a fever. He had a CT + bloodwork- Normal. He had an EEG which showed "spikes". He had more seizures. He had a 48 hour EEG with video monitoring on 11/10/02 -11/11/02. At this time, he was diagnosed with epilepsy. He is currently on Phenobarbitol, which has controlled his seizures. He does, however, continue to have "episodes" where his eyes rolled back for a few seconds and he collapses. (The child) has also been diagnosed with a developmental delay. He was not walking at 15 months and was refereed to a physical therapist. He is still not walking at 20.5 months although he is talking some steps. He had no developmental delays prior to receiving the MMR shot As per follow up letter; the pt has not recovered fron vaccination. Pt has seizures daily. He was hospitalized in August and he was specifically diagnosed with complex partial seizures. Although pt began pulling up to standing at 12 months, he just started walking at 22 1/2 months. The annual follow up received on 8/5/03 states pt was averaging 40 seizures per day until 7/14/03. He is now on the ketogenic diet and seizure-free for 2 weeks. He will have to remain on this diet for at least 2 years. Pt still has developmental delays. He receives speech, OT and PT. He will have to start pre-school in October for physically challenged children. He had no delays prior to the MMR and varicella vaccines.

Write-up: At age two years realized severe learning difficulties, to this day at age 8 has ADHD and has still been unable to learn to read or do math. Had hep B at birth, 2+4, had multiple vaccines on 08/09/2000. On 08/28/2000 had complex partial seizure turned "deadly stop breathing." Med-flighted to hospital. Told them about the vaccinations but is on anti-seizure medicine. Follow up information received 09/25/02,stated "She continues to have seizures and is on medication to prevent them. She also has learning problems." An annual follow up report received 7/31/03 adds: She still suffers learning delays; she still suffers seizures.

Write-up: Vaccines were given on 12/19/2000. Cold symptoms on 12/29/2000 were the first signs of a reaction. Doctor was notified of cold symptoms on 01/02/2001. Fussiness, sore throat, and 100+ temperature had been going on for three weeks by 01/26/2001. Atypical varicella suspected on 02/20/2001. Skin and sinus conditions rapidly deteriorating as well as evident behavior changes by 03/01. Skin problems are out of control by 04/16/2001. Referred to a dermatologist. Open skin sores four months old by 07/16/2001 will not heal. Hearing test requested by us on 08/07/01 due to obvious developmental regression. Continued sinus and skin problems occasionally acoompanied by diarrhea 08/01 thru 12/01. Autism is suspected. Pediatric neurology referral given on 12/21/01. First neurologist gives autism as diagnosis and orders testing on 01/17/02. A second neurologist makes diagnosis of autism on 04/29/02. ABA (Applied Behavior Analysis) intensive behavioral intervention treatment program through CARD (the Center for Autism and Related Disorders) was began on 06/17/02. Pt is making some progress but the program is so expensive we will not be able to carry it on for very long. The estimated annual cost of implementing the program correctly is $35000-$55000. Social security, Medicaid, nor insurance will help with expenses. The 60 day follow-up states that, "I have been advised by my attorneys not to submit any additional info concerning this adverse event." Annual follow up on 07/28/2003: "Patient did not recover from adverse event. ''Patient has autism spectrum disorder. He has not recovered. I feel that vaccines are the leading cause of regressive autism.''"

Write-up: Information has been received from a nurse practitioner concerning a 17 month old female who on 06/08/98 was vaccinated with MMRII. On 08/06/98, the patient began to lose her hearing. **Follow-up report on 9/26/02: In follow-up, the MD reported that the PT''s mother would "scream at her at times" and nothing happened. The PT was talking more in the first yr. of life, which seemed to decrease during each ear infection episode. There was no family history of hearing loss, but this was only known on the father''s side in that the PT''s mother was adopted. The mother''s pregnancy, labor, and perinatal period were unremarkable, and the PT had no history of any head trauma. In Oct. 98, the PT underwent tympanograms, which were noted to be "almost flat". The MD felt the PT might be a candidate for myringotomy, and consulted an ear, nose, and throat (ENT) specialist for evaluation. On 12/11/98, the PT was seen by an ENT, who documented the following: "Physical examination revealed a retracted ear drum on the right, and the left drum was mobile. The rest of the head and neck exam was unremarkable and tonsils were 2+ in size". "Tympanograms revealed a normal curve on the left and a Type C negative pressure curve on the right. Impression: Eustachian tube dysfunction, rule out congenital deafness". The PT was diagnosed w/sensory neural hearing loss, moderate to severe, bilaterally symmetric, "cause undetermined", and hearing aids were recommended. On 2/3/99, the PT was evaluated by speech pathology, w/the following documentation noted: "At present" the PT "demonstrates a severe communication deficit 2ndary to moderate to severe hearing impairment. Total language score on the Preschool Language Scale-3 was obtained at 66 which converted to a Percentile of 1 and an age-equivalent of 1 yr 2 mts (chronological age: 2-0). This total score falls 2 standard deviations below the mean. Speech therapy is necessary for this child in order to develop a functional communication system by which wants, needs and ideas may be expressed". On 6/9/99, the MD reported that the PT was seen in his office and the PT was very active, bright, alert, and verbalizing. The PT''s submaxillary glands were normal. On physical examination the postauricular areas and external auditory canals were normal. The tympanic membranes computed axial tomography (CAT scan) for cochleovestibular abnormality, which revealed the following: "vestibules were slightly enlarged bilaterally, but structurally the semicircular canals and cochleas appear otherwise normal". On 6/23/99, the MD reported that the PT''s review of systems was unchanged and that the PT was being followed by speech and hearing therapy on a weekly basis, w/the use of hearing aid amplification.** The patient was diagnosed with sensory neural hearing loss, and as of 07/01/02, it was reported that the patient had lost 80% of her hearing. It was reported that the patient sought unspecified medical attention. Upon internal review, it has been determined that the patient''s sensory neural hearing loss to be an other important medical event (OMIC). The reporter felt that the PT''s sensory neural hearing loss was a serious event which resulted in a persistent or significant disability that required medical treatment. Additional information is not expected.

Write-up: Information has been received from a physician concerning a 15 month old pt who was vaccinated with varicella virus vaccine live. Concomitant suspect therapy included MMR and other unspecified vaccines. Subsequently, one month post-vaccination, the pt developed type I diabetes mellitus. The pt''s type I diabetes mellitus persisted. The physician reported that he felt that the type I diabetes mellitus may have to do with one of the vaccines the pt was given. Upon internal review, the type I diabetes mellitus was considered to be an other important medical event. Additional info has been requested. Additional info has been received from the physician. The physician reported that child''s mother had a scar from thyroid surgery years ago and that her pregnancy was uneventful without any complications or gestational diabetes. The physician reported that diabetes was not in the family''s history and that the child''s father was healthy. The child was born a normal healthy baby weighing 8lbs. 8oz. and was breast and bottle fed. The physician reported that the child''s first visit with him was on 01/22/01 as a new pt. The child was a normal healthy infant and he received his 2 month, 4 month and 8 month vaccinations as recommended. The child did not have any problems with these vaccines or any allergies. The child was only in the office for well visits and immunizations. The physician reported that after the 8 month visit the child was lost to follow-up. On 04/19/02 the child was seen again when he was vaccinated with varicella virus vaccine live, MMR vaccine and Haemophilus B conjugate vaccine+hepatitis B vaccine recombinant. On 05/31/02 the child was back in the office with rapid breathing, decreased appetite and a red throat. The child was diagnosed with tonsillitis and was prescribed Augmentin. On 06/02/02 the child became ill with vomiting and was taken to the ER. In the ER the child was found to have an elevated blood sugar. The child was hospitalized for seven days and was diagnosed with type 1 diabetes mellitus. The physician reported that the child was now doing better since he has been on insulin. The reporting physician felt that the type 1 diabetes mellitus required hospitalization, was a life-threatening, and was a disabling event. Additional info has been requested. Additional information has been received from the physician. The physician reproted that the child had also been treated with Ibuprofen. Additional information is not expected.

Write-up: 10/26/01-Prescription for Amoxicillin for ear infection; 10/31/01-1 year old vaccinations; 11/6/01 to 11/10/01-Rash from Amoxicillin (?); 11-14-01-Seen by doctor, losing weight, not eating, prescribed Polycose and sent home, vomiting and said it was flu. On 11/17/01, dx''d with diabetes by ER staff. Follow up information received 10/18/02 stated, "She has Type 1 diabetes." Annual follow up received on 08/20/2003: "Vaccine recipient did not recover from the adverse event that was reported for this vaccination. COMMENT: Type I Diabetes."

Write-up: In the months following the child''s MMR, he developed an involuntary twitching of the left side of his face. He was seen by his pediatrician and a neurologist. Jan.2001 an MRI showed no abnormality.The cause for the twitching remained unknown. The neurologist said it was most likely viral in nature and would heal. This was puzzling as our son had had no serious illness. The 7th cranial nerve did heal, but we began to notice a lag in his beginning speech and increased "babbling". We thought perhaps it was due to his pacifier,but later when we took it away we saw no improvement. Winter 2002 he had a middle right ear infection. It became apparent he was hearing very little. We treated the infection, waited for the infection to clear and still noticed problems,then he was seen by his Pediatrician, ENT and an Audiologist. His middle ear was perfectly healthy, but there was concern about sensorineural hearing loss due to lack of OAE''s. An Auditory Brainstem Response test was done and he was diagnosed in June 2002 of having bilateral sensorineural mild- moderate hearing loss. He now has hearing aids and is in speech therapy for the sounds he has not heard. I have no proof the MMR caused this loss, but I do know something caused damage to both the 7th and 8th cranial nerve and the first symptoms were noted shortly after his immunizations. also he has had no serious illness. I hesitate to repeat the MMR before kindergarten as I feel there is a greater risk to my son if he gets this injection than if he does not.I have not yet discussed this with my physician, but he is aware of th child''s problems. 60 day follow-up state patient did recover from damage done to the facial nerve, but did not regain his hearing. The annual follow up received on 9/9/03 states he has moderate hearing loss and hearing aids both ears. He is in speech therapy.

Write-up: One day after vaccine, had hives, seizure and stomach pain. Two weeks later, had a viral infection with pinpoint rash and fever. Eyes get fixed. Body goes limp. Last episode 9/20/02. The annual follow up received on 9/10/03 states still has episode every 3 to 4 months. Eyes roll back and he goes limp. Last one in June. Some delay in speech noted. He has allergies to milk and dairy products.
