Preexisting Conditions: No major. I have been prone to fever blisters on or around my mouth about 3 x a year since I was a little girl. Usually brought on by sunburn or stress. What I am now is 2 days after Moderna Booster shot, a rash that started around the left bridge of my nose and continued downward toward my chin and underneath mouth. This rash looked like poison ivy with little blisters. I also developed a very bad fever blister on my lower lip around Dec 26th, and I feel rough patches and blisters inside the left cheek, and inside cheek feels swollen because I keep biting it, which is unusual.

Write-up: Rash on face with blisters starting left side at nose, continued to chin 2 days after injection. Rash has crusted over and has bled a little and is healing as of today 12/29. But I feel like I have blisters or rough patches inside mouth at left cheek. I developed a fever blister at mouth 2 days after shot, which is one of the larger ones I have ever had over my lifetime. The rash on my face does not look like fever blisters, but more like poison ivy rash.

Write-up: Case was vaccinated with 3 Pfizer vaccines, then hospitalized for Covid-19 3 months following the booster shot. Hospitalized at local Hospital. 1, with MD discharging physician. Patient is a pleasant 78 yo female with pmh CLL, htn, hld, CKD III, OSA on CPAP, osteoporosis, GERD. She presents to ED with shaking chills and cough. She has Covid-19 pneumonia. She was vaccinated with Pfizer x2 and boosted against Covid-19. She had a fever in ED. She was given Decadron and remdesivir. She has had symptoms for two weeks which worsened in the past 3 days prior to admission. She has no antibodies against Covid on testing. Dr. was consulted with ID and has prescribed casirivimab and imdevimab which bind to SARS-CoV-2 spike protein, on 24/Dec. * Pneumonia due to COVID-19 virus Assessment & Plan Fully vaccinated with 3 mRNA shots but likely had poor response in the setting of immunosuppression. Presents with sepsis, F and unrelenting cough as well as chills today. N/V- Symptom onset about 7 days ago. Will treat with Decadron and remdesivir. Would d/w ID tomorrow to see if any additional changes considering her current treatment for CLL. CXR with only small changes to RLL. CRP is 1.9. Write-up: Patient completed two doses of Moderna COVID vaccines in June 2021. She presented at the ED c/o vomiting and diarrhea after taking Linzess on 12/24/21. She was found out of altered mental status and chest pain and admitted to the hospital. She also had low-grade fever and cough, and her COVID PCR was positive on 12/26/21. She has received IV steroids, but she was not required supplemental oxygen. She is going to be discharged home on 12/29/21.

Write-up: On 12/24/2021 I had a dry cough, congestion, coughing a fever. My fever ended on 12/27/2021. The highest fever I had was 100.7. I took Tylenol and Nyquil. On 12/27/2021 I talked to my PCP and they advised me to get tested since my husband tested positive a few days prior. I took a test that afternoon and it came back positive. I am feeling much better today.

Write-up: 11 Days after the booster shot on the 12/24/2021, I had itchy hives all over my body. This includes my face/legs/arms/body. I also had a very strong fever and extreme tiredness. I started taking anti histamines such as bennadryl. On the 27th it was still bad so I went to the doctors and they prescribed me Prednisone for a week. I have continued using Prednisone and bennadryl and currently it is helping my symptoms.

Write-up: It started with the body aching on Friday (12/24/21)and on Saturday my lower back was hurting so bad I couldn?t sit, stand or do anything and then on Sunday it was my upper back by my right shoulder, like a baseball had grown there and then Sunday night I started getting dizzy so dizzy I was nauseous and then Monday very sick to my stomach and no appetite. Write-up: On 12/22/2021 I started feeling fatigued and had a slight sore throat. The symptoms worsened the following day and I developed a headache and cough, as well. On 12/24/2021, I got the over the counter COVID test and tested myself at 11am and 4pm, and both were positive. Write-up: Sudden breakout in hives on the evening of 12/24 followed by worsening symptoms on 12/25 and 12/26 leading to an ER visit. Rash/Hives move based on wherever patient scratches or touches. Given Benadryl, Famotidine, and Oral Steroid at ER on 12/26. Currently taking 7 day prednisolone taper pack, with 1x Zyrtec in the morning, and Benadryl as needed every 4-6 hours. Itching goes down with Benadryl, otherwise symptoms continue.

Write-up: Chest pain (central and left started Dec 24th and still experiencing slight central pain), left arm numbness and tingling on the inside (Dec 25- 26th lasted 2 days), swollen lymph nodes in left armpit, shortness of breath when sharp chest pains (24th-26th). I went to ER on Dec 26th for symptoms. They did an EKG, said everything looked normal and told it was likely a reaction to booster. It was effecting my musculoskeletal system. I was told to take ibuprofen and keep an eye out. Called PCP to follow-up with persistent chest tightness. Scheduled to be seen tomorrow, December 31st, to get a second look.

The occurrence of additional non-serious events is detailed below. The patient''s past medical history included COVID-19 in August 2021. On 23-Dec-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 24-Dec-2021, the patient experienced VACCINATION SITE INDURATION (the injection site was hard), VACCINATION SITE RASH (had a rash from the injection site to her elbow), MYALGIA (body ached) and VACCINATION SITE SWELLING (the injection site swelled up really big). On 25-Dec-2021, the patient experienced VACCINATION SITE WARMTH (the swelling moved down and was a puffy bubble on the top of her elbow that was hot) and VACCINATION SITE MASS (the swelling moved down and was a puffy bubble on the top of her elbow that was hot). The patient was treated with IBUPROFEN for Adverse event, at an unspecified dose and frequency. Write-up: Received J&J vaccine on 04/28/2021. She had no adverse reaction. Approximately 48 hours post Moderna booster on 12/22/2021, skin began to tingle and itch, developing into hives. Upon administration of diphenhydramine, symptoms subsided only to return hours later. It persisted for 6 days, accompanied by joint pain/swelling of fingers and toes.

Write-up: Received vaccine on Thursday 12/23/2021 she vomited at 0300 12/24/2021 . I provided here Zofran 2 mg she continued to have severe nausea and vomiting (x 4) over the course of 4 days. My parents attempted to take her somewhere and she could not get 10 miles down the road before having to turn around due to severe nausea. She was taken to medical facility on 12/27/2021 for follow up as she was not eating much and having so much nausea. She had no other symptoms, however, out of abundance of caution flu and COVID swabs were done and were negative. She was sent home with refill of Zofran. Today (12/31/2021) is the first day she has not complained of nausea requiring treatment with Zofran. She is only 35 pounds part of me is wondering if she needs to be vaccinated with the 4 year old dosing as she is so small.

Write-up: Migraine from 12/24/21 through evening of 12/26/21. The migraine was throbbing and spread across a headband like area in front of my ears. The location of pain was consistent across all three days. I have experienced migraines before but never lasting multiple days or occuring multiples days in a row. During that time period nothing happen that would typically trigger a migraine (no menstruation, alcohol, or insufficient sleep). Over the counter painkillers did provide temporary relief from pain.

Write-up: Day of vaccine: flu like symptoms with chest tightness, pain, severe shortness of breath and 101 fever 6-8 hours after receiving. December 24th delayed reaction began. Day 1-3 hives jumping from one place to another randomly all over body. Zyrtec calmed it down. Day 4 low grade fever, chest tightness, racing heart, chest pain, shortness of breath, sore throat, large joint inflammation, blood pressure low, bpm 100-110 resting and 110-147 upon standing in place. Nasal drainage and fluid in ears. This went on until day 7 of delayed reaction. Cardiac symptoms manageable by day 7. Days 7-10 ears very full feeling with pressure. Awful ear ache. Decongestants not working, prednisone helping, but not working well, pulsating tinnitus causing insomnia, Tylenol for intense ear pain. Left eye very red without irritation. Asthma attacks began day 9-10. Using albuterol inhaler prescribed for exercise induced asthma. Lymph nodes in neck and jaw very large and full. Very painful.

Write-up: 12/24/2021 11AM ET Vaccination. I started to experience some arm pain within a 2 hour window. It took forever to fall asleep, tossing and turning and I''m not sleeping. I am getting MAYBE 3-4 hours total of disturbed, interrupted sleep. I have not had a full night sleep since I had the booster. The lymph nodes were swollen the following morning for a total of 3 days. Called neurologist cause I was afraid it would trigger migraines. He recommended the Tylenol PM. It helped arm and lymph nodes, nothing for the sleep. I have not called him back since. Experiencing insomnia.

Write-up: Hospitalized 12/24/2021; COVID-19 positive 12/24/2021; fully vaccinated BRIEF OVERVIEW: Admission Date: 12/24/2021 Discharge Date: Dec 26, 2021 DETAILS OF HOSPITAL STAY: PRESENTING PROBLEM: Mania (HCC) [F30.9] Cocaine abuse (HCC) [F14.10] Hypoxia [R09.02] Acute renal failure, unspecified acute renal failure type (HCC) [N17.9] COVID-19 [U07.1] HOSPITAL COURSE: 61 year old male w/PMH including CKD3, polysubstance abuse and opiate use disorder, peripheral vascular disease with recent femoral endarterectomy, HTN, T2DM, who presented with mania and altered mental status, reportedly moving his arms erratically, yelling, and raising his voice. He was found to have COVID-19 positivity, a drug screen containing nicotine, cocaine and fentanyl, and an AKI. He was admitted for further management. Mania/AMS -His mania and altered mental status did resolve and are thought to be secondary to illicit drug use. He returned to his neurologic baseline. He was counseled against illicit drug use and evaluated by social work. He was seen by Psychiatry who did not appreciated any psychiatric or active thought disorder. Of note, he has not had suboxone in over a month and is without a prescriber. Psych recommended discontinuation of suboxone at this time and for patient to re-establish with provider in future if desired. COVID-19 infection - Patient had only mild URI symptoms during admission and did not become hypoxic or require treatment for COVID. Recommended continued 10-day isolation from 12/24/21. He was counseled on signs and symptoms of worsening COVID PNA and when to return to the ED. He is therapeutically anticoagulated with warfarin so DVT/PE is unlikely. No signs or symptoms of myocarditis. Elevated procalcitonin - Patient had elevated procalcitonin with negative culture data and no clear bacterial focus of infection. Observed on antibiotics however no clear infectious focus was identified and he had no further s/sx of infection. Antibiotics were discontinued. Hypertension -remained stable inpatient on current antihypertensives. One of his antihypertensives is clonidine. Outpatient consideration of switching this agent to another if compliance is an issue but would defer to PCP. AKI - likely prerenal. This resolved with IV fluids. Also noted was a mild nontraumatic rhabdomyolysis which improved with IV fluids. Peripheral vascular disease, h/o endarterectomy - remained stable. Patient is to continue on ASA/statin/cilostazol. Pharmacy dosed warfarin while inpatient and he should continue on this until otherwise instructed. Counseled on need to make follow up appoints with Vascular surgery and counseled against nicotine/tobacco use. At time of discharge, patient is hemodynamically and neurologically stable, tolerating diet. He declines need for social work/substance abuse evaluation. No current signs of intoxication or withdrawal for past two days. Discharged in stable condition. Recommended outpatient PCP and vascular follow-ups.

Write-up: Patient still hospitalized at 10 days admission. Patient is a 64 y.o. female with history of oxygen dependent COPD, chronic pain syndrome on chronic opioids, depression with anxiety, and former tobacco use who presents to the ER with worsening shortness of breath with cough in setting of COVID-19 infection. The patient lives at home with her daughter and grandson who tested positive for COVID on 12/21. The patient developed symptoms shortly after in tested positive on 12/22. She was fully vaccinated with Pfizer vaccine and booster on 10/27/2021. Patient complains of fevers, chills, generalized weakness with fatigue, worsening cough and shortness of breath. Patient reports she last had a short course of antibiotics about 1 month ago for bronchitis. She does has significant chronic obstructive pulmonary disease, on 2 L nasal cannula O2 at baseline, and former tobacco use. Patient reports chronic steroid use, typically on prednisone 7.5 mg daily, although patient self increased prednisone to 10 mg daily 3 days prior to arrival. Patient also has chronic pain, follows with pain clinic, and takes methadone 10 mg 4 times daily as well as Norco 10 daily. In the ER, patient remained afebrile, normotensive, borderline tachycardic, not tachypneic. Patient did desat to 88% on her baseline 2 L nasal cannula, bumped up to 3 L. chest x-ray showed significant emphysema with superimposed multi focal faint opacities. Labs unremarkable with normal renal function, no leukocytosis, normal procalcitonin. Patient received Solu-Medrol and remdesivir in the ER. The patient was admitted to Internal Medicine for further evaluation and treatment of acute on chronic hypoxic respiratory failure due to COVID-19 pneumonia.

Write-up: Starts by feeling heat in the face, head and all over the body, then itchiness develops and then redness and hives across the body. The skin turns red and bumpy and it''s impossible to stop scratching because is needed to relieve itchiness. It started on 12/24. Had never this in the past. It''s impossible to sleep or do anything during the day if left untreated. The only allergy med that has worked well is Zyrtec (10mg). After one week (12/30) I tried stopping Zyrtec. After 24 hrs when the med wore out hives started appearing again and in the timeframe of 32-48hrs after the last med was taken hives become so intense that was impossible to sustain it anymore. Doc appointment is scheduled for 1/11 because I am currently not in my home area where my insurance covers me but I am taking Zyrtec daily. It''s impact usually starts wearing off around 18-20hrs

Patient first developed MIS-C symptoms on 12/24/2021, became febrile on 12/27/2021, and was subsequently hospitalized in the ICU on 12/29/2021. Patient experienced mild perihilar peribronchial cuffing, volume loss at lung bases, mildly suppressed systolic function,abnormal LV longitudinal strain. On presentation to ER he had CT abdomen/pelvis which was negative for any acute process. He was found to be COVID-19 positive but this was felt to be residual from recent past positive test in October. His chest x-ray on admission showed mild bibasilar atelectasis without any consolidation. He did have hypoxia in the ER which was initially felt to be secondary to IV narcotics given. He was admitted to Medicine. After admission, he did increase in O2 needs and was placed on 3-4 liters nasal cannula. He developed a fever meeting SIRS/sepsis criteria and was found to be RSV positive on film array. Given that his COVID positive test was felt to be residual from prior positive test in October he was not treated with Remdesivir. He was started on IV steroid for 5 day course for RSV pneumonia (finished with prednisone) and hypoxia resolved. During his stay he developed acute encephalopathy, and he had to be placed in soft bilateral wrist restraints. His ammonia level was found to be elevated at 146. Both Psychiatry and Neurology Service were consulted. Psychiatry assisted with agitation while hospitalized. Ritalin was stopped. Neurology recommended decrease of home Depakote and addition of Vimpat given high ammonia level felt to be secondary to Depakote. EEG showed background slowing without seizures. Ammonia level trended down. Patient was discharged back to his facility in stable condition. Patient is unable to contribute to history r/t drowsiness likely 2/2 recent dose of morphine along with developmental delay. Per caregiver at bedside, she is aware that he had an episode of emesis last night. This morning he seemed in normal state of health and was able to go out to lunch and participated in opening Christmas gifts. This afternoon he reported abdominal pain and had an episode of emesis, prompting presentation. His caregiver reports him having a cough today but otherwise they have not noticed any abnormalities. In the ED, patient was tachycardic with VS otherwise stable. He did have transient hypoxia with spO2 85% which was felt to be 2/2 IV narcotics. Write-up: Event occurred after 2nd vaccine. Patient is an 86-year-old female presents to ED on 12/24/21 secondary to falls and generalized weakness. Patient''s son is Covid positive. Patient denies shortness of breath pain in the abdomen or chest, denies nausea vomiting or diarrhea. Denies Symptoms: no abdominal pain/ no chest pain/ no chills/ no cough/ no discharge/ no fever/ no headache/ no nausea/ no palpitations/ no rash/ no shortness of breath/ no URI symptoms/ no vomiting. In the ED patient was tachycardic and tachypneic. Patient was placed on 2L NC. CXR showed Tiny left pleural effusion versus scarring in this region. Lactic acid was elevated. Write-up: My daughter started feeling off around 8 p.m the evening of her 2nd covid vaccine. She woke up around 3 a.m with sever nausea and a fever 0f 102.5. Shortly after she began vomiting which continued throughout the night. By morning she still had a fever of about 102 and could not hold down fluids. Write-up: I had sore throat, congestive, coughing, loss of voice, headache, and low-grade fever. I also had nausea and vomiting. My ears also hurt as well. Write-up: When I woke on 12/24 I had hives on Legs, Torso, Stomach, Arms. Took a benadryl but by that evening could not function. 12/25 By Christmas day my hives was everywhere on my body and tried to stay home. 12/26 Went to see a doctor thru urgent care telehealth and was advised if got worse go to hospital but try to stay home and take benadryl with rest. Write-up: Received Moderna booster on 12/14. Noticed that I felt itchy at the base of my scalp along my hairline and in my groin the afternoon of 12/24 after showering. Itching resolved on its own. After washing my face before bed I noticed that my eyelids were red and swollen. I went to bed and the itching returned. It began in my armpits and spread all over my back then to my groin. My hands felt like they were burning and my forearms began to ache. Didn''t sleep well. Woke up the next morning slightly itchy but it wasn''t until after I showered that morning that I developed intensely itchy hives. I noticed these in my armpits, along my neck, chest, and along the skin folds of my groin. I took a zyrtec and applied anti histamine cream which resolved the itching. The itching returned in the evening most significantly in my groin and armpits and the hives had turned into large angry red rashes in those areas. I also developed heartburn which was not responsive to tums, bicarbonate, apple cider vinegar or pepcid (all of which typically resolve these symptoms for me). I took 2 benadryl at bedtime and slept well. I awoke on 12/25 with continued heartburn and pain in my esophagus when swallowing. I took another zyrtec that morning. The rash in my armpits remained angry red. The itching returned in the afternoon. I also felt as if I had a fever/chills but temp was lower than usual at 96.9. The itching became a painful burning and along with heartburn, I developed 6/10 aching in both forearms. I took 2 ibuprofen and used a heat pack to relieve the symptoms. 12/27 I felt better. I have continued with zyrtec daily and protonix along with tums. The rash flares along my hairline/neck after I shower and tonight (1/4) I noticed my face was red and slightly swollen. My hands and forearms are itchy today as well. Heartburn has been present nightly since 12/25 but the pain in my esophagus is gone. I have managed the symptoms at home with otc medications but will be seeing my PCP if they persist. I have no known history of rash, skin, seasonal or food allergies, urticaria, dermatitis etc.
