Write-up: Four hours following the vaccination the patient developed Numbness; Tingling of the back of her neck with extension to the back of her head; Headache; Dizziness; Fatigue; Arm pain; Rash at the injection site; Blurry vision for one day. All symptoms except the rash and the blurry vision were still present after one week when seen in the Medical Centers ER.

Diagnostic Lab Data: MRI, multiple antibody studies that are positive for antinuclear antibodies n Her 2, positive to dilution 1:40 and 1:160 on 4/6 and 1:40 on 4/9, elevated CDG protein and albumin.

Write-up: Patient had an initial response 2 days after the vaccine, with general malaise, dry mouth, headache and itching hands as well as local discomfort at the injection site that seemed to resolve spontaneously. On April 3rd he had non-specific changes in his hands that he attributed to cold weather that led to discomfort when running them under warm water. By monday APril 5th he was having worsening sensitivity in his hands and feet to heat, touch and experienced numbness. he also began to have difficulty walking. He went to the ER April 6th and was discharged April 7th after undergoing MRI of the brain and spinal cord, labs and lumbar puncture, with a presumptive diagnosis of early Guillain-barre Syndrome. His symptoms continued to worse, loss of gait was more pronounced as well as loss of balance and new off sensations in his tongue. He was admitted to the hospital, continued to undergo diagnostic workout. Results from tests on APril 6th came back positive for Anti nuclear antibodies and high protein in his CSF. He was treated with 3 days of infused Immunoglobolin therapy and was discharged on April 11th with a diagnosis of likely GBS. His symptoms continue to worsen slightly, but with less day to day decline

Write-up: 7 days after receiving J & J vaccine, patient developed a severe headache at approximately 1am. He had periods of time that he couldn''t recall. He experienced vertigo as well as nausea. He tried to text and it came out as a jumble of misspelled words and letters. He called me, his mother, and he was extremely hard to understand and couldn''t say a complete sentence without using words that made no sense. Him being in college I was 1.5 hours away. We were able to contact a friend of his and she was able to go to his dorm room and they called the campus on call nurse line. After speaking with her she instructed them to call 911 because she thought he could possibly be having a stroke. Patient was transported by ambulance to ER.

Write-up: One week after my vaccine, on Saturday morning 4/10th I woke up and saw red in the outer white part of my left eye . I looked closer and it looked like blood, a burst/popped blood vessel. I felt no pain or irritation in my eye. My eye and surrounding eye area were not swollen or inflamed. There was no fluid or substance leaking from my eye or tear ducts. My eyes were not watery.

Write-up: Later on that evening around 10:30pm I started having chills. Around 1am I woke up with high fever of 103 my body was very hot. By 5am the fever broke I was at normal temperature I felt better and was able to sleep. On 04/8/2021 around 6pm I went to reach out for something & I couldn''t feel my arm my legs then I felt that my face was numb as well It was only left side. I felt tingling . It lasted for like 4 or 5 seconds. I went to the ER they did some lab work & a CT scan and everything came back normal. I will be scheduling an MRI as well.

Write-up: April 2nd had stomach pain around noontime. At about 9 pm, I felt chills and fever and thought I was just cold. Fever the entire night until the following day. Went to urgent care for COVID tests to be sure. Fever and headaches April 3 and 4, April 5 started to have diarrhea still with really painful headache. COVID tests came out negative April 6th. Write-up: by 10-11 p.m. that night severe chills. did not sleep the entire night due to extreme headache, chills, fever (102+), body aches. felt truly awful. by 6 am Saturday feeling nauseated and wound up vomiting. Saturday night/Sunday -- not 100%, no appetite, intermittent nausea, but able to work/do normal activities. Monday 4/5--nausea at an 8-9 in the morning and kept a trash can next to me during virtual hearings I had that day. Intermittent nausea throughout the week and not much appetite but made sure I was eating/drinking despite that. Wednesday night 4/7 -- I went to use a PT stick on my left leg b/c I had felt a little pain in my left knee and figured my quadriceps were fibrotic. I could barely put any pressure on my left thigh. I had never experienced anything like this. As soon as I started rolling the stick on my leg, my skin was immediately red and warm and my leg was so sore and tender I had to stop. Saturday 4/10 -- woke up with severe headache and later that morning felt very weird in my head/heart, like heart racing and my head felt "spinny." Felt like I might pass out at one point so I had one of my kids sit on my bed with me until I felt a little better. Sunday 4/11 -- had another episode like Saturday (headache and lightheaded/spinny/pass out feeling/slow speech) and asked neighbor (an RN) to take my BP. her at-home device said 147 over something and she suggested I go to urgent care just to double check. About three hours later, I was seen at urgent care. BP was normal, heart rate normal, oxygen was 96. 4/13--followed up with primary doctor. Send me for blood work to check platelets, etc. Feeling pretty good today but nervous after the news and the pause.

Write-up: Patient was administered first dose of Johnson and Johnson vaccine. She actively carries an epi pen and was directed to self time for 30 minutes of observation. Pt felt better and discharged from vaccination clinic but about an hour later patient called to report dizziness and persistent headache and returned to vaccination clinic for re-evaluation. EMS on scene administered 2L o2 via NC for comfort as pt highly anxious and tearful reporting feeling dizzy and frontal HA. Is not with Shortness of breath or chest pain. Advised transport to ED for further evaluation and management

Write-up: Had vaccine 7-March at 4:15 PM. One week later, 14-March, woke up at 8:15 AM with severe BPPV. Went to doctor and had Epley Maneuver performed a few days later. Then on 23-March woke up at 4:15 AM with another BPPV event. Again, returned to doctor for Epley Maneuver. Then on 2-April went to ENT Doctor to have ears checked. She performed the Epley Maneuver and everything was fine; there was no nystagmus. Then she laid me down on my back and tilted my head off the table and I had the worse dizziness ever, but no nystagmus. She sat me up. My BP jumped to 190/100; pulse 100. I couldn''t stop being dizzy. Paramedics took me to emergency room. After 2 hours the doctor performed the Epley Maneuver, but there was no nystagmus so it wasn''t BPPV. Had CT scan and MRI and everything was clear. What changed and what may have caused all this was that I had the J&J vaccine prior to all the dizziness.

Write-up: I had a rash on my arm that spread to my chest and other arm. By Saturday night, the rash had spread everywhere. By Sunday, I was wheezing and had to use and inhaler. Write-up: Patient received Janssen COVID 19 vaccine and within a few minutes became very lightheaded and dizzy. Patient sat down to try and relieve symptoms and after sitting a minute proceeded to feint. She regained consciousness within a few moments and was then disoriented for a short time. EMS arrived and evaluated her and since she was pregnant she agreed to go with them for further evaluation. We followed up with her the next day, she was feeling better having recovered fully and was told at hospital everything was fine medically with her health and her pregnancy.

Write-up: Reaction started with cough and increase phlegm in throat and quickly progressed to swollen throat, dry tongue, unquenchable thirst, shakiness, weakness, ears feeling full, headache, increased blood pressure and increased pulse. Called personal physician who recommended going to the Emergency Department. Took one 25-mg benedryl and went to ED where physician confirmed allergic reaction to the vaccine and administered 20 mg prednisone and pepcid. Symptoms improved over two-hour observation and was released. Continued 20 mg prednisone, pepcid (twice daily) and benedryl (every 6 hrs) daily for 4 days, followed by Allegra and Zyrtec once daily each until 4/13/21. Swollen throat, dry tongue, excess thirst, shakiness, weakness, ears full, headache, and increased heart rate/palpitations persist 7 days post-vaccination. Had in-person visit with primary care physician on 4/13/2021 who recommended Nasocort twice daily for two weeks for fluid behind ears and antihistamine once daily for 5 additional days, and will refer to local allergist for work-up.

He summoned the paramedics who loaded me in an ambulance soon after I came to. They checked my vitals. My heart rate was 47 bpm and blood pressure was 80/40. They also checked my blood sugar. It was 116. They decided I needed to go to the hospital and began giving me iv fluids in route. At the hospital an EKG was taken, which came out normal, they continued iv fluids, and did a urinalysis on me. All test indicated I was healthy. It about three hours before my blood pressure and heart rate stabilized. Just as long for me to feel like I would be ok. They discharged me at this point. Since they I have just been extra tired and have had muscle discomfort.

Write-up: Heart attack after J&J vaccine - My husband, complained of on and off chest/lung discomfort after two days getting the vaccine. This continued for about 3-3.5 weeks. On the night of Apr 16, he went to bed early not feeling well. Next morning he complained of heaviness in the chest area. We immediately consulted Dr., and mentioned that he got the vaccine. Write-up: 6hrs after the vaccine I started having chills, body aches, light headed, & headache. Also at the same time I started spotting which I found out it was the symptoms of a miscarriage. On 04/11/2021 I continued with the same symptoms then later on that night i started feeling dizzy the bleeding continued. Around 2am on 4/12/2021 I was bleeding a lot & at 7am I went to the ER and they confirmed I had a miscarriage. I was 11 weeks pregnant. My estimated delivery date was 10/28/2021. Diagnostic Lab Data: On arrival to the ED on 4/12 at around 11:00 AM, patient under went a non-contrast CT of his head and his cervical spine, both of which were negative for acute traumatic injuries. He also head a chest x-ray and pelvis X-ray, which were also unremarkable. Patient had a BMP which showed a mild AKI (Creatinine 1.37), but was otherwise unremarkable. His blood alcohol level was undetectable. His CBC was unremarkable. His PT/INR was 1.1/11.2.

An hour later, he was driving on a highway, had a syncopal event and was involved in a single-vehicle MVC where his vehicle struck a guardrail and rolled twice. Patient was brought to the ED at Hospital where he was evaluated by me in the ED. Patient had a normal mental status (GCS 15) and was alert and oriented on exam. He did have some bruising to his forehead from the airbag. Patient denies feeling ill or lightheaded after receiving the vaccine and reports the syncopal event came on very suddenly.

Write-up: About 3 hours after getting the vaccine I started to feel short of breath. About 5 hours after that I got chills that made me shake all over because I got a fever of 103. Then 2 hours later my heart started to beat very rapidly and got up to 126 beats per minute, my heart was also not only beating vast but very very hard. Then throughout the night and all of the next day, I had pain in my chest on the left side and also pain in my rib area on the left side. The pain lasted for over 24 hours and because of that I went to the emergency room. It turned out they didn''t find anything life threatening at the emergency room but all the pain I had was very scary and very uncomfortable. It was a terrible experience and because of it, I am telling people not to get the Johnson and Johnson vaccine.

Write-up: With prior vaccines and flu shots, I have either had no side effects or mild. But with the COVID vaccine, I was sicker than sick! I wasn''t expecting to miss as much work as I did, nor go to the doctor three times in a week''s time. I received it around 11:30am on Thursday, March 18. Around 1pm, my mild headache started to intensify and had ringing in my ears that would repeatedly come and go (still is doing so today, just not as frequent). Around 4pm is when all of the "normal" side effects started?chills, hot flashes, sore arm, and super sleepy. I woke up a couple hours later, with a headache that was WAY worse and was super nauseous. This was close to 10pm. Then, around 11pm that same night, I developed HORRID shooting ear pains and took my first Tylenol. The ear/jaw pain was more than I could stand - had it in both ears, but the right ear was more intense. It felt like I had a lighting rod between both ears and had these intense "zings" that kept going back and forth, like they were playing ping-pong. The painful "zings" reminded me of those felt during an ear infection but WAY MORE INTENSE. My jaw was super tight, as well, with a horribly intense headache. I spent the entire night night doing warm compresses on my ears, taking Tylenol every 4-6 hours, staying in the shower, and lots of crying. It was so bad that I felt the need to go to the ER but didn''t feel comfortable driving and didn''t want to go the ambulance route?and thought it was pointless because going to the ER for ear pain would leave me there for hours and hours since I didn''t feel it would be viewed an an urgent emergency. My goal was to make it through the night and see if I could get into my PCP doctor the following morning. I took another Tylenol around 5am, and I thankfully cried myself to sleep for a couple of hours. When I woke up, I called my PCP but couldn''t get in, so I did an online doctor visit through my insurance company. My fever was about 103.06 at its highest during the night but was gone by 3:34pm Friday afternoon (assuming because I was taking more Tylenol than I had been post-online doctor''s appointment). Around 7pm Friday evening, I knew I needed to go to the ER or urgent care. I got a ride and was taken to urgent care for my intense ear pain. They gave me a Toradol injection and Tramadol prescription. By bedtime Friday night, the injection was doing its job and made the ear pain and zings were much less intense (didn''t get the prescription until a couple days later). On March 20, Saturday night, is when I developed a red, itchy rash on my back and tummy. Over the next couple of days, it was on my chest, arms, legs?pretty much come and was in various places. I called my PCP Monday morning (March 22), left a message, and they called me back to tell me to restart taking my Zyrtec/HCL to see if it would help the itching. It has and am just mildly breaking out at this time. The stinkin'' ear/jaw pain returned on Tuesday the 23rd, so I loaded up on OTC ear drops and made an appointment with my PCP for Wednesday, March 24. She saw no ear infection and essentially told me to keep taking the pain meds. I received the vaccine on March 18 and did not return to work until the following Thursday. In retrospect, I should have just missed the remainder of that week, too.

Write-up: Tachycardia. I had the expected side effects of headache, chills, body aches, fatigue and fever (102.6 F) but began to experience symptoms of pounding/racing heart that worsened through the evening. By 10:00 pm my heart rate was bouncing around between 130 bpm and 155 bpm, and I was beginning to have pain in my lower legs/ankles. Was told by a on call nurse for my clinic to go to the ER. I was in the ER for about 5 hours, roomed the whole time as they worked to get my heart rate down and checked to make sure I didn''t have a blood clot. Write-up: The day after the vaccine was administered (April 5, 2021) I woke up with headache, fatigue and I developed ringing in my ears. I went to urgent care, as I had pain in my ears. Recipient was lying on side and assisted to supine position; assessed skin pallor, bilateral hands cool to the touch; denied SOB, denied chest pain; no swelling noted; alert & conversing; stated "I can''t hear from my right ear"; d/t fall skull examined and small raised area noted with no skin break. Write-up: Got heaviness in chest and ring finger turned white and numb around 8:30 a.m. Got heaviness in chest again around 6:00 p.m. and it would not subside so I went to the ER and they gave me 3 nitroglycerin and did several tests to check my heart but could not find anything wrong. Write-up: Had chest pain on and off for 5 days (started Thursday 3/25), which I thought was related to acid reflux. Write-up: Warm flush in neck and head a few minutes after injection. Six hours after injection, headache with mild fever, chills and join aches. Nine days after start of symptoms, severe headache with neck pain. Was prescribed antibiotics for sinus infection symptoms and given a chest X-ray. On March 26th was instructed to go to ER by primary but I refused at the time. On 12th day after vaccination vision in left eye became blurry with swelling. Also had numbness in lower back radiating to the front. On 16th day, 03/29/2021, primary care physician again instructed me to go to ER. I was admitted to the hospital. Released 03/31/2021. Was treated for head and neck pain and given several tests.

Write-up: days 1-3 the expected fever (101), headache, dizziness, extremely sore arm and body aches, tiredness, difficulty falling asleep Day 4 swelling in left armpit about half a golf ball size with pain and soreness into left upper chest, called family drs office they said it?s a listed possible effect of the vaccine Day 7 vomiting, diarrhea, stomach pain, dizziness, flu-like symptoms. Urgent care visit - test for covid came back negative, given medication help keep fluids down. Severe symptoms lasted for 3 days. Diarrhea persisted for 8 days. Followed up with family dr. Write-up: About 10 to minutes after vaccine I had a strange wave warm sensation running through my body and tingling. I got this sensation about 4 times that day. Twice at vaccine facility and 2 more times at home that same day. Then in the past couple of weeks I?ve had numbness in the right side of my face, arm/hand and leg/foot. Sharp pains in my head, PVC, eye pressure, vision issues, shortness of breath, electric shock sensations through my body, head tingling. Not sure the cause of these symptoms but the only different/new thing was the vaccine.
