Write-up: On 3/8/21 65 y/o M received his first Covid-19 vaccine (Moderna) and states on 4/2/2021 he had a myocardial infarction with 2 stents placed. He discharged to home and was doing very well. Tuesday, 4/6/2021, he received the second Covid vaccine. The following morning, he had pain in his right ankle. It progressed and involved his left foot and ankle. Over the next few days his pain progressed to involve his legs, but stopped at the knees. He had leg weakness and associated diarrhea, fever and chills. He thought his symptoms would pass and tried to wait it out over the weekend. When he awoke on Monday morning, 4/12/21, he could not move his feed. He went to the ER and was diagnosed with Guillain Barre Syndrome.. He received 5 doses of IVIG with very good response. Patient states his leg strength is improving, but is not back to baseline. His arms are unaffected by his Guillain-Barre. He is in Rehabilitation now.

Write-up: Patient developed upper extremity weakness starting two days after first dose of pfizer vaccine progressing to the point of being unable to lift arms. Was hospitalized and apparently treated with Prednisone and then re-hospitalized on 4-22-21. He has absent reflexes in upper extremities with 1/5 shoulder strength bilaterally, normal strength and reflexes in lower extremities. Seen by neurology who feels presentation consistent with an atypical form of Guillen Barre or Acute Inflammatory Demyelninating Polyneuropathy.

Write-up: Guillain Barre syndrome; shortness of breath; tingling in her hands; legs were numb and tingling; feet were swollen; This spontaneous case was reported by a consumer (subsequently medically confirmed) and describes the occurrence of GUILLAIN-BARRE SYNDROME (Guillain Barre syndrome) in a 61-year-old female patient who received mRNA-1273 (Moderna COVID-19 Vaccine) (batch no. 020B21A) for COVID-19 vaccination. The occurrence of additional non-serious events is detailed below. The patient''s past medical history included No adverse event (no medical history reported). Concomitant products included AMLODIPINE and BISACODYL (LAXATIVE [BISACODYL]) for an unknown indication. On 30-Mar-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. On 31-Mar-2021, the patient experienced PERIPHERAL SWELLING (feet were swollen). On 01-Apr-2021, the patient experienced HYPOAESTHESIA (legs were numb and tingling). On 02-Apr-2021, the patient experienced GUILLAIN-BARRE SYNDROME (Guillain Barre syndrome) (seriousness criteria hospitalization and medically significant). On 19-Apr-2021, the patient experienced DYSPNOEA (shortness of breath) and PARAESTHESIA (tingling in her hands). The patient was hospitalized from 02-Apr-2021 to 06-Apr-2021 due to GUILLAIN-BARRE SYNDROME. At the time of the report, GUILLAIN-BARRE SYNDROME (Guillain Barre syndrome), DYSPNOEA (shortness of breath), PERIPHERAL SWELLING (feet were swollen), HYPOAESTHESIA (legs were numb and tingling) and PARAESTHESIA (tingling in her hands) outcome was unknown. Write-up: Diagnosed as Guillain-Barre syndrome; This is a spontaneous report from a contactable consumer (patient himself). A 50-year-old male patient received bnt162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE), dose 2 via an unspecified route of administration, administered in arm left on 16Feb2021 12:00 (Batch/Lot number was not reported) at the age of 50-year-old as single dose for covid-19 immunisation. The patient was health and had no medical history. The patient had no known allergy. Concomitant medication included finasteride (PROSCAR); lorazepam; hydroxyzine. There was no other vaccine in four weeks. The patient was diagnosed as Guillain-Barre syndrome on 24Feb2021 16:00. The event resulted in doctor or other healthcare professional office/clinic visit, Emergency room/department or urgent care, hospitalization, life threatening illness (immediate risk of death from the event), disability or permanent damage. The patient was in hospitalization for 8 days. The patient had no COVID prior vaccination and had been tested post vaccination. The patient underwent lab tests and procedures which included covid (blood test): negative on 01Mar2021. Outcome of event was recovered with sequel in 2021. The patient received IVIG Infusion therapy for 5 days for event. Information on the lot/ batch number has been requested.

Write-up: 2/20 received first shop and within 6 days awakened with vertigo. After speaking to doctor went to PT. Was relieved of symptoms for 8 days. Then on 3/19 received 2nd dose and shortly after began feeling dizzy and experienced severe head aches. Trying for 2 weeks to work through symptoms but was admitted to the hospital with extreme dizziness and unable to walk. While in I was diagnosed with Guillain-Barre'' Syndrome. It has completely debilitated me causing extreme unsteady and uncoordinated body functions that could possibly last for the remainder of my life. Since I am an active gym athlete, this is devastating.

Write-up: I was informed by CPT, that a patient who received the vaccine was in critical care and diagnosed with Guillian Barre Syndrome and is now on ventilator. Attempts to reach the DR or patient have been unsuccesful at this point but we will continue to try. I have no PMHx of the patient or current medications
