Write-up: Within one month began severe abdominal pain, abnormal behavior (no eye contact, fecal smearing, lost speech), regression in speech and motor skills. Developed sensory problems. Night waking and screaming. Overactive thyroid and yeast. Nurse follow up on 08/31/04 states: "PDD-pervasive developmental delay; chronic encephalopathy; chronic diarrhea."

Write-up: Distant, cranky, and aloof. Crying a lot and very clingy to me. Sleeping a lot more than usual after the vaccinations. Rash on face at 6 days after vaccines. Bright redness at site of vaccinations and hot to the touch. Large lumps in legs at site of vaccinations. Fever spiked to 103.3 6 days after vaccines. Follow up on 06/25/04 states: "Additional symptoms/illnesses after vaccination were chronic URI, vomiting and headache. Final diagnosis are autism, GERD, ADHD, gastroparesis." Nurse follow up on 07/06/04 states: "Per review of other correspondence from reporter, the following symptoms are added: moodiness, loss of appetite, sleeping poorly, nasal discharge, and speech loss." Nurse follow up on 08/23/04 states: "night terrors and very hyperactive." Nurse follow up on 09/20/04 states: "No new information."

Write-up: Information has been received from a physician concerning an 18 month old male patient, who uses albuterol nebulizer PRN for asthma and with a possible milk sensitivity, who on 4/13/03 was vaccinated with a dose of MMR. Concomitant therapy on 4/13/03 included a dose of varicella virus vaccine live. On 4/13/03, shortly after the patient received the vaccines, he experienced an anaphylactic reaction. The patient developed sneezing, coughing, vomiting, hives, facial edema and turned purple. The patient was taken to the emergency room and was admitted overnight and then discharged. Therapy included IV diphenhydramine and dexamethasone sodium phosphate. The patient was seen by an allergist, who was consulted while the patient was in the hospital, on 4/24/03 and was OK. The reporter indicated that the physician is planning on doing allergy testing on both vaccines in order to determine which vaccine the child is allergic to in the near future. The reporter did not have vaccination records for the lots of the vaccines. It was noted that the patient was first seen by the allergist on 5/6/02 due to a rash that developed around his mouth after drinking Enfamil. The child''s prick test was positive and the child was switched to a soy formula at that time. Subsequently, the patient recovered from anaphylactic reaction. It was reported that the patient''s experiences were considered to be immediately life-threatening and disabling. Additional information has been requested.

Write-up: Developmental delays, poor eye contact, speech delays, agitated when held occurring between 1 to 6 months after MMR. Food allergies, diarrhea, weight loss, looking through peripheral vision between 6-12 months. Nurse follow up on 07/20/04 states: "Complete." Per annual report-He has an official "autism" diagnosis from AGRE 2005. He still continues to have unformed stools. He has sensoriy processing problems. He does not like to touch most foods, he has obsessive compulsive issue of needing to touch you last. He is currently recieving early education services with autism consultant and speech therapist. His speech is improving but still delayed for his age. Mom would like more research on : "If I received vaccines as a child what if any immunity was passed to my child and now their receiving vaccines affects them.

Write-up: Patient stopped progressing after 18 month shots. he has been diagnosed autistic. He is lacking in many areas, i.e. potty training, language, social skills, behavior. His behavior is volatile. He bites, head slams, and is very physical with my 5.5 year old girl. I noticed all of these things within days. Follow up:Fever developed after vaccination. Mom treated it with alternating doses of Tylenol and Motrin x 2 days. Nurse follow up on 10/20/04 states: "Medical records review confirms diagnosis of autism." Per the follow up report-Patient is delayed in so many areas except motor skills. He is not wanting to potty train. He eats non-edible items such as pencils, chalk, porcelain and wood. He is self injuting when angry or upset. His language is less than 24 month. He is very destructive in the home such as breaking glass and flipping furniture. On occasion, this school year, he has shown some aggression towards classmates. He cannot sleep more than 2-3 hours per night without medication. He is currently taking Clonidine 0.1M, Trazodone 50 mg, Seroquel 25 mg, once daily at bedtime. He is obsessive with man things like certain foods, certain movies, toys and closing doors. He is also obsessed with water and is unaware of danger about water. He does not understand about fear like fire, cars in the road, climbing, breaking glass with his bare hands. He bites often and sometimes so severe he can break the skin through clothing hisself and daughter as well. I feel like this is a pretty good picture of patient. I could go on and on.

Write-up: Information has been received from a physician concerning an 18 month old previously healthy Polynesian female with no known drug allergies and no birth defects or medical conditions who on 07/12/04 was vaccinated SC in the left arm with a 0.5mL dose of varicella virus vaccine live (Lot # 645290/0326N). There was no illness at the time of vaccination. There was no concomitant medication. On 07/23/04 the pt developed a rash, described as scattered over her trunk and legs, not clustered, some lesions vesicular, some scratched open. The pt also developed ataxia at that time. On 07/23/04 the pt visited the pediatrician and on the evening of 07/27/04, the pt was admitted to the hospital. A CT scan without contrast was done, and a spinal tap was done. Both were negative. Spinal fluid for VZV was pending. The physician indicated that the pt was alert and awake when admitted to the hospital. Interventions included acyclovir (Zovirax) and varicella zoster immune globulin. The outcome was reported as not recovered. There was no product quality complaint. Follow up information received from the medical record, the pt''s family, and the physician indicated that on 07/23/04, the pt was noted to walk funny and it was noted to be getting worse. She also started to develop lesions on her skin. The family indicated that the child had not had a fever. The pt was seen by the physician on 07/27/04, and he was concerned about vaccine reaction and sent the pt for further evaluation. The physical examination revealed an alert child who did not appear toxic at all. She had some scattered lesions on her trunk, legs, and arms. Some of them were excoriated from scratching, but some of them did look like vesicular lesions on an erythematous base. Examination shows the CSF with 3+ red cells, 1+ white cells, and no organisms in the Gram''s stain. The cell count is 2 white cells, 1 seg, 70 lymphs, 20 monocytes, glucose 51, and protein 15. The CT scan done without contrast was unremarkable. The child was admitted to the hospital for acyclovir. After presenting 8-week previously for acute ataxia after receiving a dose of varicella virus vaccine live, she has had an MRI of her complete neuraxis, which was normal, she has had a spinal tap, which was normal, and her ataxia had been relatively persistent, although there had been mild improvement. On 09/15/04, the pt presented with fever and left esotropia. She was admitted to the hospital for further evaluation. On physical examination, the pt was resting comfortably. She had a temperature overnight of 38.2. She was noted to have a poor neurologic prognosis for a full recovery. The plan was to treat with IV gamma-globulin, 2 grams per kilogram divided over 4 days over 6 hours, and EEG, an MRI of the brain as an outpatient in 6 months, or sooner if necessary, and consider pediatric opthalmology consultation. The physician felt that the pt had a "probable adverse reaction to the varicella vaccine". The reporter indicated that the ataxia, fever, vesicular rash, and esotropia were considered to be disabling. Additional information has been requested. Medical records are attached. Follow up 03/03/05: Patient has not recovered from the adverse event(s).

Current Illness: Don''t Know. On Christmas Eve 2003 Patient and Mom were both exposed to Chicken Pox. I was diagnosed in mid January 2004 with Chickenpox at age 30 and had to be hospitalized. Pediatric Care would not see my daughter to see if the bump on her scalp was chickenpox because they said she would be highly contagious and If I have it then as primary caretaker then she does have it and the only treatment for her is rest and Tylenol.
Preexisting Conditions: She had a real bad upper respiratory infection the winter of 2003/2004 with a lot of congestion with antibiotic and decongest/allergy med she got well. She is also allergic to cow''s milk

Diagnostic Lab Data: Testing determined normal:ABR,EKG,short and long term EEG,MRI of ears and part of brain, CatScan of Brain Blood testing determined to be normal: CBC, Many genetic tests(Rett''s/Fragile X) more that I can not think of right now all normal. Throid/hormonal/metabolic tests normal.

Write-up: My daughter was administered multiple immunizations on 3/9/04. That night she became very, very, fussy. Even with TYLENOL she had a temp of 99. Before the vaccinations she was a Healthy, Happy, Friendly, Bright, Brilliant and Smart child. Within a week my daughter went into what I call a FOG. She would no longer respond to her name and would not even respond to most sounds. She quickly and drastically began to lose her many skills. She could no longer talk (she had been saying mama, dada, lala, bye bye). She began this constant deep wining, moaning sound that at times she would make all day and nothing would comfort her. She was friendly to family and strangers and then all of a sudden she would scream and cry if we were in a room with people or family members that would try to interact with her. Her family gave her a nickname was because she could mimick anything that her mother did. She could wave, play patty cake, dance, jump, stick her tongue out, pose with smiles for pictures. Anything I taught her she learned almost instantly. Since 3/9/04 we cannot attend a family function longer than 5-10 minutes because my daughter becomes hysterical. This extreme, traumatic fog lasted from March to Mid August when she began showing slow, small improvements. She no longer pushes her mommy away but will now let me hold her or kiss her. Other family members still can''t hold her but they can wave or talk to her. She has slowly re-learned to clap but she still cannot speak or wave bye bye. Her arlertness to sounds has improved but she still usually won''t respond to her name. She has every test imaginable done to determine what is wrong with my baby and all the results come back fine or negative. Please, Please, Please help me to understand what happened to my daughter after she received those shots and what I can do to help her. She is my whold world. I cry myself to bed everynight because I don''t have the answers needed to help her. She and I are currently in speech therapy, occupational therapy but my daughter is still only 25% of the child she once was. Please, Please, Please help me to help her.

Write-up: Information has been received from a pharmacist and a mother concerning her 25 month old son with food allergies who on 8/29/02 and 10/9/03 was vaccinated by injection with the first and second doses of varicella virus vaccine live. Concomitant therapy included HIB conjugate vaccine and hepatitis B vaccine, recomb. It was noted that the three year old child is at 2 year old level mentally and has been placed on autistic spectrum. Unspecified medical attention was sought. There was no product quality complaint. The reporter considered the three year old child is at 21 year old level mentally and has been placed on autistic spectrum to be disabling. Additional information has been requested. Follow up 03/03/05: Patient''s recovery status is unknown. Followup report received 06/14/2005: Patient''s recovery status still unknown.
