He stayed for 30 minutes further observation, then left feeling very normal

Write-up: Received shot in upper right arm and it was painful. My arm grew more painful throughout the day. I became very tired as the day went on. Went to bed for the evening, Monday 4/26/2021 and woke up at 1:50am on Tuesday morning, 4/27/2021 in full body sweats, dripping with sweat. So weak I couldn''t get up or lift my head. My husband took my temperature which was 103. Massive headaches, full body aches and pains, chills which made my body shake uncontrollably. My whole body hurt. Could not get up to use the restroom, tried and fainted back into the bed. Heart felt like it was beating out of my chest. My husband wanted to call 911 but I didn''t want to. Felt like I had COVID again.

Write-up: Vaccine was received at 11:30 am, 4/27/21. Very minor headache started a few hours later. Fatigue, aches and chills started around 6pm. At 8pm, temp was normal at 97.7. Around 11pm, while sleeping, heart rate increased to above 100, ranging from 105-140. At 12:30am, after no decrease in heart rate, I went to ER. After receiving 2 bags of IV fluids and multiple tests later, heart rate was down to around 105 and I was discharged at 2am. Heart rate returned to normal (about 61 for me) around 3pm on 4/28/21

Write-up: Patient is a 46 year old female who was admitted on 4/27/2021 for ACUTE DVT, UNSPECIFIED VEIN. 46 year old female with past medical history of POTS and on chronic hormonal oral contraceptive medication, and reported history of recurrent renal stones. Patient presented to ED after several days of left upper quadrant pain and tenderness, that worsened and became associated with nausea and vomiting. Initial evaluation by imaging demonstrated left renal stone in left renal pelvis with associated mild hydronephrosis. Incidentally on imaging, patient had DVT noted in right illiac vein with continuation into IVC. Subsequently admitted and started on heparin drip and pain control.

Write-up: Pt called call center regarding symptoms after getting J&J vaccine on 4/10. Pt was upset that she had to repeat her symptoms when she was transferred to advice nurse. Pt stated multiple times that her symptoms were only on both arms. She denied any emergency symptoms and declined to answer the rest of call center staff questions. I advised the patient that I need to ask screening questions prior to making a telephone MD appt. Pt stated again, that her symptoms were localized only to both arms and had some pain. Staff explained to the interpreter, at this time there are no urgent care appts available, pt would need to go to Urgent Care or ED to be evaluated. Translator repeated once more, and no response from pt. Pt disconnected from the call. called patient back, no answer.

Write-up: Started with a low fever 6 hours after vaccination, with slight muscle ache symptoms. About 4-5 hours later it evolved into a high fever (104 at peak), muscle aches on the whole body, and ringing in my right ear. The next day the symptoms were more mild, but still present (other than ringing in ear) with a lower fever, and the day after the symptoms were gone except for tenderness at injection site.

Write-up: This is a 59 year old male that presented to the emergency department at approximately 1447 on 04/27/2021 with sudden onset of left upper and lower extremity weakness and left-sided numbness of his face. The patient stated he may have had some weakness the previous night (04/26/2021), but mostly noticed it when he woke up around 0900 on 04/27/2021. The patient stated he may have also had some transient speech changes which he felt were improved upon emergency department presentation. The patient stated he took an 81 mg aspirin prior to hospital arrival. The patient was overall hemodynamically stable in the emergency department. Past medical history is only significant for benign prostatic hyperplasia. This patient has no history of previous CVA or TIA, no smoking history, no history of clotting disorders, no recent trauma or other injury. CT head revealed a small right frontal subarachnoid hemorrhage with no midline shift and concern for clot. CTA head and neck perfusion study was significant for ?perfusion abnormality involving the right frontal lobe in the region of known subarachnoid hemorrhage, likely related to dural venous sinus thrombosis involving the posterior aspect of the superior sagittal sinus, left greater than right transverse sinus, and left sigmoid sinus.? This patient is noted to have recently received his Johnson and Johnson vaccine on 03/20/2021. The patient was admitted to the intensive care unit. Neurology and hematology were consulted.

Write-up: A 40 year old man with no apparent risk factors for current disease presents with an anterior myocardial infarction. At acute in far related angioplasty is LAD is found to be completely included by fresh red clot that was removed using an instruction catheter. Imaging of the interior endothelial surface of the left anterior descending which was occluded by clot did not reveal any ruptured plaque or abnormality in the endothelium. He experience extensive anterior myocardial infarction and now has a left ventricular ejection fraction between 35 and 40%. Write-up: Caller''s husband called the call center on the morning of 4/26 to ask about symptoms that his wife (the patient), who was 24 days post vaccination with Johnson & Johnson COVID-19 vaccine, had experienced the evening of 4/25. He reported that she had developed a severe headache and vomited. She c/o leg and chest pain as well and he reports that she has a history of blood clots but no h/o migraine headaches. At the time of the call his wife was feeling fine. I advised that they discuss with PCP. Having none I advised that she discuss with Urgent Care. I called him back about 6 hours later. They had decided that since since she continued asymptomatic they were not going to seek clinical evaluation. Write-up: Within 10 minutes of getting the vaccine I started having hay fever- like symptoms. Sinus drainage, itchy, burning eyes, runny nose, scratchy throat, hoarse voice. This lasted for several hours. It again worsened during the night and next day I had bouts of nausea and some head ache.

Write-up: By 6pm I noticed something wasn?t right. I could t get warm, even when sitting in the sunshine. By 9pm the chills had turned to fever. My temperature at 9pm was 101.2F. I also had a headache and started to feel a bit shaky. By 10pm I had body aches and was in bed with 3 blankets on me and still could not get warm. My temp was 102.7 at 10pm. I tried to sleep. At midnight I awoke to a fever of 104.8 and could only cry. The worst body aches, headache, racing heart and dizziness, nausea and vomiting (after additional aspirin were taken to relieve the fever). I was very weak in an alarmingly rapid period. I stayed in this condition until approximately 6:30am on 4/26. At 7am my temp was 102.5 and I found it hard to make it back and forth to the bathroom, due to dizziness and weakness. By noon, the temp was back up to 103.1 and I could barely cry, as I suspected I was then close to dehydration. I focused on hydration and slept as much as possible. The fever slowly came down to its current 100.4 at 11pm on 4/27. This has been an alarming reaction to a vaccine. I have had serum sickness previously, so maybe that is a factor that should also be considered. I hope this is helpful.
