Write-up: after receiving vaccine, felt tired, had runny nose on 4/5/21 visited patient around 10am, told us she was going to see the doctor next day 4/6/21 called patient 3x and no one picked up learned that patient suffered from stroke sometime prior night, learned that from sister (approximately 12 hours before patient was discovered in her home) - 911 called and rushed patient to ER patient spent time in ICU, conditioned stabled - continues to stay in hospital, condition continues to be monitored

Diagnostic Lab Data: 3:54 PM - Chest XR appears like severe COPD. No obvious acute infiltrate. 5:34 PM - Procalcitonin is slightly elevated but labs are otherwise normal. COVID test is negative. She is on 3.5 liters of oxygen and sats are at 95%. She has been having purulent sputum. We will treat with antibiotics. I will send him home with a prescription for Adoxa and Deltasone. I advised her to follow up with her PCP for further evaluation. Strict return precautions were given prior to discharge.

Write-up: The patient is a 59 y.o. male with a hx of COPD who presents to the Emergency Department with SOB. He received the COVID vaccine on 3/18/2021 and has felt bad since then. Patient has experienced increased SOB and a productive cough with yellow sputum.. He experienced fevers for 2 days after receiving the vaccine but has not had any since then. He is on 4-5 liters oxygen at a time. He experienced some diarrhea today but took a laxative. He denies blood in stool, dysuria, or urinary symptoms. Patient reports some body aches. He is on chronic pain medications for back and leg pain. He has not been on any steroids or antibiotics recently.

Diagnostic Lab Data: :45 AM Today''s work-up is overall reassuring. Findings were discussed with the patient. Upon reevaluation, the patient is feeling fatigued with body aches, but does report feeling better than before. She is declining nausea medication. Patient reports she has valium at home which she plans to take at home so that she can sleep and feel better. I feel that she is appropriate for discharge at this time and the patient is comfortable with this plan. The patient has also been advised to follow up with her PCP for further evaluation. Home care instructions and return precautions were discussed prior to discharge.

Write-up: The patient is a 35 y.o. female with history of POTS, Chiari malformation type I, and mast cell disorder, BIBM to the Emergency Department with flu-like symptoms following the Johnson&Johnson Covid vaccination. Patient received her vaccination mid-day yesterday and by yesterday evening she developed generalized myalgias, palpitations, HA, and chest pressure "like something is sitting on my chest." She specifies though that her chest pressure is "not bad enough that I need an inhaler." She notes that throughout the night she had a fever of 102 and woke up this morning with a heart rate of 148 prompting her ED visit. Patient is also nauseated, but denies any vomiting. She reports taking zyrtec, ketotifen, and cromolyn sodium at 0600 this morning. Patient is allergic to Benadryl. Patient notes that she received saline infusions in home a couple times a week for her history of POTS.

Write-up: Background: With Chrohn''s and with the Erythromycin - I had some fatigue; stomach upset; and joint pain. I didn''t go into it with no symptoms. These that I am reporting would be increased. I could feel injection go into the arm. By the morning of the 15th, I think, increase joint pain and fatigue from my baseline. My knees; feet hands - achy in the joints themselves. Fatigue: wishing I could have a nap throughout the day. These lasted for about a week. I had stomach upset that I believe may have been from the Erythromycin - but that didn''t change that stomach upset - still had it... it may have gotten a little worse (the nausea. By morning of the 23rd, I took 2 mg Ondansetron for that nausea. That helped a lot with the nausea. Around 2 am on the 24th, I woke up from a very bad nightmare and I woke up with a pounding heartrate. My chest was really thumping. I got up to the bathroom. My heart kept pounding harder and faster and didn''t come down. My watch said tit started going up to: 122 to 144 to 154 and it kept gradually going up. I was petting the dog and sitting in the recliner. I phone my Dr''s on-call number and spoke to him on the phone. I called 9-1-1 by his advice. Paramedics came - Heartrate stayed up. I was still able to walk around the house. I followed up to their truck and they checked me over. By that time as they were taking my vitals my heart rate started to calm down. (It was elevated about 15 minutes about all together) My blood pressure - 160 over 110 when the paramedics checked me but that was coming down as they had me at the truck and the heart rate went down to 80-90 and was continuing to go down. It hasn''t come back. They listened to my heart. Never had that happen to me before like that. Didn''t have to go to ER. I do have the apple records of my heart rate, etc. if needed. The joint pain and fatigue faded back to my baseline.

Write-up: On the evening of 4/7, very achy and sore arm and mild fever. Recovered and felt fine by the evening of 4/8. On 4/12 Began feeling nauseous, and was throwing up all night, along with diarrhea. Severe lower back, knee, hip and outer thigh pain. On morning of 4/13 had a fever of 102?, very nauseous. Still very sore. On 4/14 still achy in lower back and hips and thighs, temperature is closer to normal.

Write-up: He developed Superficial Phlebitis in the left lower leg on 3/31/21, and was seen in the ER. He reports concern to me on 4/14/21 that due to reports of blood clots with the Janssen vaccine, that he is concerned that his vaccine on 3/9/21 may have provoked the blood clot. He still has some phlebitis in the lower left calf. Note, he is on self given testosterone, which may also contribute.

Write-up: Patient received vaccine and about 2-3 minutes after vaccination began sweating profusely. His wife was with him and alerted our staff that something was wrong. Our staff pharmacist, immediately attended to him and noticed that his tongue looked enlarged. He was not speaking for himself and was very disorientated. , RPh administered and Epi-Pen 0.3mg injection in the patient''s thigh. The patient quickly stopped sweating, became more oriented and began talking. EMS was called and came to the pharmacy. EMS observed him for a short time then took him to the ERD

At the hospital in the ER they had to stop my heart something called Cardiac Aversion I believe. Diagnostic Lab Data: I went to the ER on 4/13 because my abdominal pain became severe. I was told by the healthcare professional that they could not report a VAERS for me and that all they could do was run a few tests. The hospital was packed and everything was a bit slow- I left without many answers but my symptoms have not subsided and I was told to fill out a VAERS on my own.

Write-up: About 10 hours after receiving my vaccine- I became extremely ill. I had migraines, fever of 101, chills, nausea, tightness of chest, abdominal pain, difficulty with my stool / bowl movements, and body aches. I could not move, and most effects subsided after laying in bed for two/three days straight. However, continuing up to today (three weeks later) I have still had tightness of chest/throat, bowl difficulties, and abdominal pain. In discussing history, denies known or suspected exposure to SARS-CoV-2 within the past 14 days has not had recent upper respiratory infection, denies seasonal allergies, does report recently started on ACE inhibitor (lisinopril). Client reports persistent dry cough. In addition, client is an active smoker he reports 5 cigarettes daily. On assessment, lungs noted is having coarse wheezing, . Denies dyspnea, shortness of breath. Given new onset cough elected for SARS-CoV-2 POC; final negative. At this time discussed cough likely associated with ACE inhibitor difficult to say given presence of wheezing. Explained he should continue to stop taking medication explained he needs to follow-up with primary care provider in order to elect a different medication.

Write-up: The night I got the vaccine 3/10 my arm was sore just like any other vaccine I?ve received. Two days later, I woke up and noticed a welt on my arm that was about two inches in diameter. It was itchy and painful. I could not lift my arms up. By noon it had doubled, and the slightest touch was unbearable. The ?Covid arm? was also hot in temperature. That night, it was at its worst at 5 inches in diameter and it was even more swollen. It stayed that way for three more days before it started to go down. Write-up: I experienced the normal flu like symptoms. Fever, chills, body aches along with that I had chest pain. My chest pain was sharp, stabbing pain that comes and goes, intermittently. I am on day 24 with chest pain. I was referred to the heart doctor, they did an EKG which was normal. MY PCP did a tri-triponent blood test was normal, an also ordered a D-dimmer and it was elevated, i was sent to ER to check for pulmonary embolism which was clear. Another EKG was done and it was normal. My electrolytes were normal. I had stress test this morning 04-14-2021. My PCP ordered platelets due to my left leg pain and heavy feeling and burning feeling that comes and goes. My platelets were normal. My WBC is very low and I have an echocardiogram scheduled on Monday. The ER doctor said that the vaccine caused the inflammation on my body and my chest, having an auto-immune reaction. I still have chest pain.

Diagnostic Lab Data: WBC was normal 6.18k/uL, with slight monocytosis 1.24(20%) and 32 bands present. CRP was elevated at 4.89mg/dL. This led to CT which revealed significant inflammation of the distal jejunum without appendicitis, no abscess, and no other infectious suggestions. Covid was negative same day, a DNA GI panel covering campylobacter, Cdiff, plesiomonas, salmonella, vibrio, yersinia enterocolitica, EAEC, EPEC, ETEC, STEC, EIEC, cryptosporidium, cyclospora, entamoeba, giardia, adenovirus, astrovirus, norovirus, rotavirus, was negative. The patient reported no history of colitis or similar symptoms, reported no family history of autoimmune disease. Pt was considered for admission but was watched closely via multiple ED and outpatient visits during her recovery.

Write-up: 4 days after vaccination pt experienced severe abdominal pain the RLQ and diarrhea. Pt was sent from village to emergency room where i evaluated, exam with significant RLQ tenderness but no rebound or guarding, some LLQ tenderness as well. With exception of mild tachycardia and slight hypertension associated with pain, vitals were normal.

I was told if the symptoms worsen to come back but that I can be feeling these side effects for up to 3 weeks after receiving this vaccine. Write-up: Day 1 - Immediate small hives all over body for 1 hour, flush, fever, shaking, nausea, then hives lef. Day 1 evening - Fever 103, blurry eyes, severe headache, swollen tongue, severe pain all over, all major surgery sites spasming, vomit and diarrhea , hard to breath and blackout. They admitted me and gave me different combos of steroids, pain killer, lorazepam, etc. to figure out the right combination to keep me steady. During the rest of this time - Severe pain throughout body, fever, shortness of breath. The hives are now just my my skin flushing. Heat flashes. My fever has just gone down to normal degrees today. My Blood Pressure has gone from 175/111 back and forth to normal. My headache is still there and body pain. Hives are now just flushed skin. Swollen Lymph nodes in throat. No more vomit or diarrhea.

Diagnostic Lab Data: I have extensive medical records. I was hospitalized for 8 days and have not been able to return to work. I am currently under care of my PCP, allergy/immunologist, and a pulmonologist to try to figure out why I am still having breathing difficulties. While hospitalized, I had 2 positive D-dimer tests, was in lactic acidosis, low hemoglobin, high white blood counts, low kidney and liver function, oxygen by nasal cannula, and many other blood chemistries that were out of range. Write-up: Day two after getting the vaccine, I started feeling vertigo and nauseous. I hydrated extensively, and day three are started to feel a little bit better. Day four I took a turn for the worse, and the vertigo and nausea intensified. In the middle of the night, the vertigo was extreme. I almost checked myself into the ER, but I decided to try to continue sleeping and hopefully feel better in the morning. In the morning, day five, I went to my primary care doctor. I told her my symptoms, she said possibly due to the vaccine. She prescribed me meclizine. I took it in the evening, and did not feel any symptom relief. In the middle of the night, extreme vertigo again, it was atrocious. Day six (Saturday), I checked myself into the ER. They upped my dose of meclizine (to 50 mg). This new dose also had zero effect on my vertigo/nausea. I felt vertigo/nausea all day. Speaking to some doctors in the family, they suggested to me that I had developed BPPV. Day seven, woke up again with vertigo/nausea, and it didn?t subside at all as the day went on. I spoke with my father, and he told me that he had BPPV before apparently. He drove to my house and did the Eply maneuver on me 10 times throughout the day. Felt a little bit better. Scheduled an appt with ENT for following day. Day eight, feeling a little bit better (after eply maneuvers, possibly related). Visited ENT, he reassured me that the vestibular system is very efficient and I will eventually get better. Scheduled appt for me for neurologist and MRI for later that day. Went to see neuro, did MRI. Day nine, feeling even more a little bit better. Day ten, which is today, feeling even more a little bit better. Saw neurologist for follow-up, he told me MRI turned out fine, everything looks good with brain. Just stay very hydrated and relax, I should get better in a week. I really went through hell, it was atrocious. My doctors said could be vaccine-related, impossible to prove one way or the other. I figured I?d report it, just in case there might be others in the country who went through the same thing.

Write-up: Pt states that she experienced low back pain and extreme fatigue after imz but resolved in 24 hours. On Sunday, (4/11/21) which is roughly 3 weeks later pt noticed pain in lower right leg (calf) which was warm and tender to touch. Pt went to urgent care and was immediately sent to ER. Diagnosis was DVT in right lower leg . Pt was administered 2 shots of heparin injection in abdomen and discharged on Eliquis. Pt followed up with PCP on today 4/14/21 and was told to continue Eliquis and to follow up in 1 week for another ultrasound to ensure medication is working.

Write-up: Immediate dizziness and nausea. Became tachycardic within an hour. This is still an issue 3 weeks out. Day after vaccine I felt mild flu symptoms all day and was struck with a severe chest pain and intense nausea in the afternoon. I went to the hospital emergency room and had multiple tests run. No cause was found for the chest pain. It began in the center of my chest and moved to the right side of my back. I have been dizzy, nauseous and extremely fatigued as well as a headache for the last three weeks. The doctor didn''t know what caused the pain. Write-up: I had chills - severe- all night long. Couldn''t sleep. Muscle joints and muscles were hurting me and fatigue. Next day, fatigue and muscle and joint soreness. It continued for almost three weeks. The third week I got calf pain that was very painful. I went to Urgent Care. It lasted until the next day (so for two days total). When I woke up on the 6th it was gone like it wasn''t even there. The third day after the vaccine, the injection site started hurting - but not swollen or anything. Diagnostic Lab Data: Patient went to the Hospital. She was seen by Doctor on April 13th at 2:30 pm. Patient received a "Migraine Cocktail", a COVID 19 test (negative), and numerous other medications administered. She had blood work sent to the lab, and a CT scan with and without iodine contrast. She also had an IV. The only thing reported that was abnormal was high blood pressure, and inability to find the source of migraines, as the scans came back normal. As of this time, results of blood work are unknown.

Write-up: On April 10th, 12 days after vaccination, at approximately 6pm, patient started having explosive diarrhea and uncontrollable gas. This subsided by the morning of the 11th. On the 11th, patient experienced extreme fatigue, sensitivity to light and sound, and nausea. Monday the 12th, at 3am, patient was woken up to severe migraines (patient has no prior history of migraines or headaches). She described her pain as radiating-- like a bright, light house was being shined directly into her eyes in her sleep. This was combined with a pounding, pulsating feeling in the frontal lobe. Patient reported hypertension, nausea, and tingles in the tips of her fingers. These symptoms persisted throughout the day. On Tuesday the 13th, the same symptoms persisted. Patient heard the new CDC warnings about the Janssen vaccine, and concerned for her well-being, called her physician. Her physician directed her to go to the emergency room as soon as possible.

Write-up: the day after the shot, started feeling unwell (approx 24 hours post shot). Slurring words, stumbling around and general unwell feeling with a fluttery feeling in chest. No previous heart issues of any kind. After it continued for an hour or so, I took him to the ER and it was discovered that he was in AFIB. 2 rounds of meds and tests, but stayed in that rhythm. They ended up doing a cardio version and it went back into normal sinus rhythm. In addition to the swallowing problem, two other major adverse events need to highlighted. First, on 27 March, while in the hospital, vaginal bleeding started. On 29 March, her doctor, performed a hysteriscopy/DNC finding " "an unexplained blood clot (hematoma)." The second major event was when patient demonstrated significant confusion and lack of memory. On 7 April, she was diagnosed with Metabolic Encephalopathy. Neurologists at the hospital suspected that her MS was reacting to her UTI and once the infection is eliminated her memory should come back. But as of 10 April, she was still having issues including hallucinations. Although she is eating some as of 13 April, she is still very weak and will likely by moved from the Hospital to a rehab facility. She still does not remember much of the last 37 days.

Write-up: Developed heart palpitations approximately 10 hours after receiving vaccine. Also insomnia making it impossible to sleep. Palpitations seemed to get better by 11 am the next morning but my heart rate was still elevated ( pulse 106-112). Shortly after that ( around 11:45 am) I became extremely light headed and dizzy and felt like I was going to pass out. I also started to feel nauseous and sweaty. I rested lying down for two hours before hearing back from my doctor''s office. I was able to control the lightheadedness and dizziness by lying down. The feeling of passing out / nearly fainting went away. I continued to feel nauseous and my heartrate was still elected. I started to get a headache as well. On the advise of a nurse at my doctor''s office, my husband took me to the ER. My EKG was normal . My heartrate was elevated. Due to the overcrowding at the ER that day I was made to wait at least an hour and half for further evaluation. I continued to feel light headed and slightly dizzy. My nausea and headache got much worse . I was finally taken back to a treatment room and received IV fluids, Zofran and Tylenol. My heartrate went back to normal and my headache and nausea improved with treatment and I was discharged around 7pm. I continued to feel very tired and slightly lightheaded that evening. The next morning I was much more nauseous again and still very weak feeling and slightly lightheaded. I have continued to feel weak, very fatigued , unwell and nauseous on and off until today, April 14, which is 6 days after I was vaccinated.

Write-up: On Monday 4/12/2021, the patient reported feeling sick and was taken to Medical Center ED but left the ED AMA before being seen by a doctor. He did not have any fever and all other vitals and blood sugar were normal. He then returned to the home facility and did not complain about feeling sick any further. On Tuesday, 4/13/2021, the staff reported that he took his noon medications. The vitals, temperature, and blood sugar was normal as reported by the facility staff. Around 4:10 PM, the patient was found unresponsive without any pulse and some foam around his mouth were reported by the staff. Two rounds of CPR was performed until the arrival of the ambulance. The patient was revived upon the arrival of the ambulance as reported by the facility staff. His O2 saturation was 10% as reported by paramedics/upon admission. He was admitted to the SICU unit at Medical Center and intubated. The hospital was informed that the patient received the Janssen COVID vaccine on the Friday before the event 4/9/2021. Additional notes: The patient is a heavy smoker (4 pack per day). Within two weeks prior to this event, it was reported that the patient missed his Symbicort inhaler for a few days.

Write-up: 04.11.2021 12:15 p.m. Vaccination and immediate sore arm 12:45 p.m. Headache and continued sore arm 7:00 p.m. Chills, fever 100, took Advil and hot shower. Broke fever during shower. 11:00 p.m. Went to bed but chills returned. Couldn''t sleep. 04.12.2021 12:35 a.m. Went pee and felt nauseous with chills. 2:30 a.m. Went pee, drank a bottle of water and felt extreme pain in joints and chest. 3:00 a.m. Took 400 mg Advil. 4:15 a.m. Went pee and drank another bottle of water. 6:00 a.m. Woke up SO due to high fever of 104.4. checked repeatedly both ears. Checked SO temp and he was normal. 6:30: In ER. 2 L of IV, tylenol, pain meds (tramadol) and sleep. Chills went away around 7:45 a.m. when the fever broke. 8:00 a.m. Another round of pain meds. 9:00 a.m. Last round of pain meds. 10:00 a.m. Discharge with temp of 99.8. 3:00 p.m. After rest and more fluids at home, fever broke. 7:00 p.m. Chills returned, mild headache, and joint pain. 8:00 p.m. Diarrhea. Flushed 3 times. Lost a lot of fluid. 8:15 p.m. Took hot shower to help with chills. 9:00 p.m. Drank 15 ounces of water and Gatorade combined. 9:15 p.m. Fever 101.0 10:45 p.m. Fever 101.5 04.13.2021 12:40 a.m. Fever 102.5 and took 2 Tylenol 2:30 a.m. Fever 99.8 5:00 a.m. normal temp 98.7, still have headache. 11:00 a.m. normal temp 98.5, pain in leg and arm muscles and joints primarily ankles, knees and wrists. Pain in lower abdominal area. 2:20 p.m. PCP appt. Blood drawn. Temp normal at 98.9 6:00 p.m. temp 100.0 8:20 p.m. 2 Tylenol, headache, joint/muscle pain 9:30 p.m. Temp 100.0 11:00 p.m. Normal temp 98.8 04.14.2021 12:05 a.m. Temp 105.5; took 2 Tylenol. 3:00 a.m. Temp 96.8/97.4 10:30 a.m. Blood draw to test for clotting 12:30 p.m. Blood results indicate clotting potential; went to ER for head CT scan to asses clotting in the brain. Headache still persists and pains in muscles.

Write-up: Immediately after the JANSSEN vaccine I felt tenderness in my jaw that lasted for about two days. My body was sore, my Legs felt like I was having cramps ?growing pains? I also was experiencing allergy/asthma like symptoms that included difficulty breathing and shortness of breath that worsened over time I couldn''t speak in full sentences or laugh I was unable to walk 10 steps without gasping for air, after a couple of days of trying to maintain and cope with my theses symptoms. I couldn?t take it anymore, as I was preparing myself to go to the emergency room. I got into the shower the steam from the shower felt like it was suffocating me and I passed out my children took me out of the bathtub and rushed me to the emergency room where I was given several tests that include x-ray, CAT scan, artery bloodwork and vein bloodwork. I was being treated for asthma and given several different breathing treatments and steroids, unable to keep my stats at a decent level and the hard rebound even after continuous treatments, I was admitted because the breathing treatments were not effective. I am still uncertain of the ?actual outcome? the Hospital doctors treated me for an asthmatic episode and gave me directions how to handle my asthma i was prescribed prednisone and referred to my primary that would then refer me to a pulmonologist and cardiologist. At the hospital I was told that there was black spots on my lungs I was told that I was being treated for infection, but then also told it could be pneumonia no certain answer was ever given and i was given and antibiotic and was also told that I needed to see my primary for a way to maintain my asthma. The antibiotics seem to work for a couple of days and again I''m back to having difficulty breathing, shortness of breath, muscle pain soreness and mild lingering headaches. Write-up: Started having excruciating, painful headaches. On about 4/6/21, headache awoke me from sleep. Pain was through my left eye towards the back of my head down to my neck. Lasted for awhile. Took Tylenol and was able to get rest about an hour later. Woke up that morning pain was still there but it was bearable. Two days later it happened again. Awaken with excruciating pain. Didn''t think it was related to shot because it had been almost 3 weeks since I had shot. On 04/15/21, it happened again. Never had any issues with headaches. Write-up: Patient received J&J Janssen vaccine 4/8. She reported nausea, abd pain, fevers, and fatigue since received vaccine. On 4/13/2021 presented with worst headache of life, trouble speaking, and abnormal gait. Exam showed L sided motor weakness and impaired sensation. Initial CT normal, received tPA for presumed stroke. Platelet count normal. Currently admitted to critical care unit at Hospital.
