Write-up: Approximately one week after vaccination patient experienced a flare up of RA. Rheumatologist increased Prednisone with only minor improvement. On 6/3/21 patient started experiencing radiating chest pain. Presented to ED and was later admitted. He was diagnosed with pericarditis and treated accordingly.

Write-up: Patient conceived 2 weeks after being administered the vaccine. Patient is now 20 weeks and on June 3rd the patient found out the baby has no kidneys and also has an enlarged heart. Patient was informed that there are no fluid around the baby and the lungs are not developed. Patient was informed that chances are very slim for the baby to survive.

Write-up: 36 hours after the shot (my second - and my arm did not hurt at all as it did with the first), I passed out on the bathroom floor. Went to ER, they determined no concussion, no bleeding in brain (CAT scan), no clot in lungs. Wore a Holter monitor for 24 hours; unremarkable. Since then, I''ve experienced diarrhea (mostly resolved), but what is concerning me now is lingering numbness and tingling, mostly on my right side (arm and toes) but also on left. Also heartburn (not resolved). Visited doctor yesterday; doesn''t think I need to see a cardiologist, but I really wish I had not gotten the second shot. I had Covid in February, and the first shot April 6th.

Write-up: The vein in her neck and it started itching; She also was waking up with sweats; little headache; she could only lay on her right side. When she laid on her left side she felt like she couldn''t breathe. She woke up in the middle of the night and moved to her left side and started breathing heavy; Felt like a bite where she received the vaccine; hurts when she eats; Where her glands are under her arm swelled up a little bit; gums on the top and bottom are really red like they are bleeding but they are not bleeding; Her gums are swollen; She developed a twitching of the left eye; This is a spontaneous report from a contactable consumer (patient, reported for herself). A 67-year-old female patient received BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, Solution for injection, Lot number: EL00185), via an unspecified route of administration in the left arm on 01Jun2021 at around 1:39PM as 2nd dose, single for COVID-19 immunization. Medical history included ongoing high cholesterol. She had been doing stair master for an hour each day. In 2020, when the Covid 19 virus was new she had a dinner with friends before they quarantined and one of those friends contracted the Covid 19 virus and died. Patients concomitant medications included atorvastatin (LIPITOR) for high cholesterol (lab value 225 units unspecified), twice a day (taking it more than that causes her back pain and sciatic pain). Patient had undergone lab test on unspecified date for Blood cholesterol and resulted as 225. Historical vaccine included flu vaccine on an unspecified date in 2014 with that she had that tightness and was told to drink a lot of hot tea and it worked and first dose of BNT162b2 (PFIZER-BIONTECH COVID-19 VACCINE, Solution for injection, Lot number was not reported), via an unspecified route of administration in the left arm on an unspecified date as 1st dose, single dose for COVID-19 immunization over which she just had a sore arm. After the second dose of the vaccine on 01Jun2021, she had some symptoms. She was wondering if there is anything she can do to prevent her symptoms. With the second dose her left side, where her glands are under her arm swelled up a little bit. It wasn''t a big deal, but her biggest concern was that on 03Jun2021, five days after receiving the vaccine, she developed a twitching of the left eye. She called her eye doctor and he suggested that she should call Pfizer to see if it had been reported. She enquired if anything she could do before going to see an eye doctor. She wanted to know if eye twitching was reported with the Pfizer COVID vaccine. She mentioned that her friend (69-year-old female) went blind for two weeks after receiving the COVID vaccine. Although, her vision came back but she did lose it for two weeks. She has also experienced her gums in her mouth at the top and bottom looking as if they were bleeding but they were not they are just really red since 10Jun2021. She massaged her gums with turmeric liquid. Her gums had been swollen and super, super red. It was not excruciating pain and it had been hurting when she would eat. She wanted to know if she was at risk of losing her teeth and if this has been reported before and if there is anything she can do for her gums. She stated that in doing her own research she read that people who had COVID-19 virus some people lost their teeth. She was enquiring as if she''s at risk of losing her teeth. Her gums are swollen and super, super red. It''s not excruciating pain, it only hurts when she eats. She was wondering if this could be associated with the vaccine in people who did not have COVID-19 virus. With the second Covid 19 vaccine it felt like a bite where she received the vaccine and the vein in her neck and it started itching. She also was waking up with sweats. She reported that yesterday that she noticed her gums, 10Jun2021. The eye twitching started last Thursday, 03Jun2021. She reports she used to be an importer for Pfizer. She further added that she could only lay on her right side. The next day after she received her second vaccine, she was fine. She did have a little headache. When she laid on her left side, she felt like she couldn''t breathe. She woke up in the middle of the night and moved to her left side and started breathing heavy. That was only the evening after she received her second Covid 19 vaccine. She expected things to happen with the vaccine. When probing adverse reactions to other vaccines she reported she had a reaction to the flu vaccine. Her throat started closing. When she got the Covid 19 vaccine she took hot tea with her. That''s what she was told by the medical department by Pfizer years ago when she got the flu vaccine. She told her doctor she wasn''t going to get the Covid 19 vaccine. patient had one daughter who isn''t going to get the vaccine but her other daughter won''t let her see her grandkids unless she''s vaccinated. Patient had not visited emergency Room and physician office because of the events occurred. Prior Vaccinations within 4 weeks patient had not received any list of other vaccinations within four weeks prior to the first administration date of the suspect vaccines. Before she got the Covid 19 vaccine she checked to make sure she didn''t have the Covid 19 virus. In response to the patient query it was provided that there is no information in PI regarding eye twitching, red gums, or loss of teeth. Serious and unexpected side effects may occur. Pfizer-BioNTech COVID-19 Vaccine is still being studied in clinical trials. Additional adverse reactions, some of which may be serious, may become apparent with more widespread use of the Pfizer-BioNTech COVID-19 Vaccine. Diagnostic Lab Data: Extensive testing done including MRI brain (6/3/21 and 6/4/21): "Multifocal T2 hyperintensity within the left centrum semiovale, left middle cerebellar peduncle, subcortical right superior frontal gyrus near the motor cortex and right posterior periventricular white matter adjacent to splenium of the corpus callosum. Questionable T2 hyperintensity of the left optic nerve although exam is not dedicated for optic nerve evaluation therefore finding is equivocal. Largest left centrum semiovale lesion is likely more acute and demonstrates diffusion restriction with faint contrast enhancement. Overall, findings are primarily concerning for a demyelinating process, such as multiple sclerosis, anti-MOG disease or NMO. Acute toxic leukoencephalopathy is also a possibility but considered much less likely. Similar findings on MRI orbits and cervical spine on 6/7 as well as repeat cervical spine MRI 6/23.
CDC Split Type:

Write-up: 6/3/21 at 1230 pm: Sudden onset of right arm numbness progressing to transient blurry vision, difficulty walking, right lower extremity weakness, difficulty moving his mouth, stuttering, imbalance, and right sided facial droop. He was admitted to hospital on 6/4/21. Found to have a demyelinating process (see imaging report below). He received IVIG treatment and IV steroids with some (but incomplet)e improvement and was discharged to Rehab on 6/16/21 on oral steroids. However, he had recurrence and progression of symptoms and was sent back to hospital on 6/19/21 with worsening right sided hemiplegia. He underwent repeat high dose IV steroids and initiation of plasmapharesis. He has currently (as of 6/25/21) completed 3 of 5 runs of plasmapharesis with marked improvement in symptoms but still has significant right sided motor deficits. Neurologists here now feel comfortable making a diagnosis of multiple sclerosis. He will likely be discharging home for ongoing therapy and treatment after he completes plasmapharesis on 6/28/21.

Write-up: Left leg numb from the whole left butt cheek down to his knee/ left arm feels kind of numb and a little heavy; Left arm feels kind of numb and a little heavy; Fatigue; Was sleeping a lot; Pain from the shot at the injection site; This is a spontaneous report from a contactable consumer (patient, self-reported). A 32-year-old male patient (age at vaccination: 32 year) received second dose of BNT162B2 (PFIZER-BIONTEC COVID-19 mRNA VACCINE, Solution for injection, lot number: EW0217 and expiry date was not reported), via an unspecified route of administration, in Deltoid Left (upper arm), on 03Jun2021, as a single dose for COVID-19 immunization at Pharmacy. The patient''s medical history included dysautonomia (he was born with it) and herniated disc. Concomitant medication included acetylsalicylic acid, caffeine, paracetamol (GOODY''S) (powder for headache or something like that, or back pain powder, in the week or 2 before the vaccine. It was like a pain powder that comes in a little pouch instead of actual pill like Tylenol. No further information provided). There was no family medical history relevant to adverse events. Historical vaccine included first dose of BNT162B2 (PFIZER-BIONTEC COVID-19 mRNA VACCINE, Solution for injection, lot number: EW0182 and expiry date was not reported), via an unspecified route of administration, in Deltoid Left (upper arm), on 14May2021, as a single dose for COVID-19 immunization at Pharmacy and no reaction on previous exposure to vaccine. The patient did not receive any vaccine within 4 weeks prior vaccinations, he had never even had the flus shot before. He had never even had the flu either. Investigation assessment was not done. The patient stated he was calling regarding the Pfizer Covid vaccine. He got the second shot 03Jun2021, about 2 weeks ago. The back side of his left leg was completely numb from the whole left butt cheek down to his knee and it has been since he got the second shot. Also, his left arm felt the same, kind of like he has been holding it over his head for a long time, it felt kind of numb, but not really, but kind of heavy. This probably wasn''t a common thing, so he wanted to report it. He had some other side effects, but he wanted to give it a little more time to make sure it didn''t go away. The patient''s left leg numb from the whole left butt cheek down to his knee which started within the next day or 2 and it hadn''t gone away. It felt about the same. It felt like it fell asleep, the whole back side area. It was like holding the arm above the head too long where it loses a little blood. It felt kind of cold and stiff and felt a little heavy. It started around the same time as the numbness in the leg, but it may have taken a little bit to notice because he did have the fatigue and was sleeping a lot the first couple days. That might have been why it took a couple of days to really notice what was going on with his arm. It was after the second dose and lasted a couple of days. It has stayed the same. Numbness was 2 inches higher where he has a herniated disc. The patient''s had pain from the shot at the injection site on 03Jun2021, which was normal and went away, the heaviness of the arm was still there and was just right at the injection site. It didn''t feel any worse than a tetanus shot. It was kind of a longer needle and all, so he expected that to be normal anyway. The soreness went away the second day. The adverse events did not require a visit to emergency room or physician office. No relevant tests were performed. The patient did not receive any treatment. When asked if he had any other concerns, patient stated, not unless Pfizer was going to help with the copay for going to the doctor and clarified that this was not an actual request, says he was joking. Outcome of the event pain from the shot at the injection site was recovered on 04Jun2021 and for the events fatigue and was sleeping a lot it was reported as unknown and for all other events, it was not recovered. The patient''s weight was 315 pounds, and height was 167.64 centimeters. The patient''s pre-existing medical conditions included patient had no known allergy, no history of hospitalization in last 30 days, no pre-existing acute illness 30 days prior to vaccination, no illness at the time of vaccination, no past history of similar events, no adverse events after any previous vaccination. she was not currently breastfeeding. The caller had tons of bumps all over the place under her arms, on her trunk and buttocks. Some of the bumps are quite large and some are smaller. The Reporter''s bottom left wisdom tooth, the gum around it, was inflamed and tender and hurt and she couldn''t eat on that side of her mouth or brush her tooth. The Reporter took acetaminophen and got on with the day. The caller''s throat hurting got worse because there were swollen glands on Saturday. Saturday evening the reporter had a dizzy spell which was strange and she had to sit down. This improved the next day. Then the caller couldn''t taste her tea. On 06Jun2021 the reporter''s throat was still sore and the glands were still swollen, her eyes were still sore, her head hurt and she still had the dizzy spell. The reporter had a sudden stabbing pain first in her left arm near her underarm but a little lower then in the left lower leg, then the left eye. The reporter has had broken toes and a broken foot so she wears a compression stocking on her left toe and she has a lymph flow residual its improving every year slowly. On 07Jun2021 the reporter head and throat hurt, the glands are tender and the petechia and red cherry angiomas still present. On 08Jun2021 the inflammation around the gums subsided. The caller thinks it might have been because she took CoQ10. The caller started taking CoQ10 around that Friday or Saturday because she remembered it is good for teeth. On 09Jun2021 the reporter was itchy all over, all day. It was like prickly almost like little stabbings and itching. This improved the next day. On 10Jun2021 the reporter feels better but the petechia is still present. On 11Jun2021 the reporter feels like herself, she still has petechiae and red cherry angiomas and she has a new body. The reporter stated that these symptoms did not happen right away and that they were delayed in onset possibly because of her immunodeficiency disorder. The reporter also added that after she returned home from receiving the vaccine and was cooking later that evening she noticed her upper lip was tingling and felt "full" but she did not look in the mirror to see if it was swollen. Reporter wanted to know if she should receive the second dose of the Pfizer COVID-19 vaccine. The decision to receive the second dose for any other reason cannot be made by Pfizer. Write-up: The flu like symptoms dissipated very quickly. I was sent to the Foot Hill Presbyterian ER after a trip to urgent care. They took an X-ray and blood work. They ruled out myocarditis. I think my heart was elevated for 2 days after that. It would skyrocket for 120-130bpm. My heart rate would respond oddly depending on sitting down or laying down. No stimuli and it would still be odd numbers. I was getting weird readings each time I looked up my heart rate. I thought it was normalizing. I have been getting these tachycardia heartbeats as I am sitting down. It will be up to 120bpm after 2 hours. Now it stays at 90-100 bpm and palpitations keep happening. Sometimes when I get up, it will start skyrocketing. My BP jumped to 150/120 and my ER doctor prescribed me beta blockers (metoprolol 25mg). I am scheduled to see a cardiologist on Monday. I have had multiple EKG readings. I have noticed a strange cycle pattern for the past few days and no reason for it. It''s almost like its playing its own beat.

Current Illness: Had DVT and PE after the first dose of the Moderna Covid vaccine. This was previously reported to VAERS. Write-up: Had DVT and PE after the first dose of the Moderna Covid vaccine administered at First Health. This resulted in a two day hospitalization and was previously reported to VAERS. Pulmonary and hematology specialists were consulted and recommended that the second dose of the Moderna vaccine be given because the patient was anticoagulated with Eliquis. After the second dose was given at First Health, the patient had obvious worsening of the swelling in his affected leg three days later. We are currently reporting this reaction to the second vaccination. As a result of the recurrent, obvious worsening of the swelling in his left leg (known site of the DVT after the first vaccination), the patient was sent to the Emergency Department. There, he had a second doppler study which showed no worsening of his DVT in comparison to the previous study. He was advised to follow up with hematology and vascular surgery as an outpatient. He was also told to wear compression stockings and to elevate his left leg.

Diagnostic Lab Data: Troponin I peak 16. 30 on 06/04/2021 Cardiac MRI 06/04/2021: Impression: 1) Normal left ventricular size with mildly reduced global systolic function. Diffuse global hypokinesis. 2) On delayed gadolinium enhancement imaging, there is patchy midwall hyperenhancement of the basal inferior, mid lateral, and apical anterior, inferior, and lateral walls and midwall hyperenhancement of the basal septum. These findings may be consistent with acute myocarditis. Cardiac sarcoidosis can also be included in the differential diagnosis but probably less likely.

Write-up: The patient is a 46 yo F who has a history of pemphigus vulgarisms in 2019 s/p one cycle of Rituxan completed in August 2019. She had significant improvement and was last seen in my dermatology clinic 11/2019. She had not followed up until this current visit. Patient then again presented to me on 6/14/2021. On this date she presented with a severe flare of her pemphigus vulgaris affecting her scalp, trunk, extremities, mouth. The patient reports that she had essentially been in remission from the pemphigus from when I last saw her in 2019. However, she says she received the first dose of the Moderna vaccine on May 23, 2021 and then approximately 1 week later (aprox June 3) she developed this acute flare of her pemphigus. I have asked her several times if she had any blisters or erosions prior to the Moderna vaccine. She states that she had zero blisters prior to the Moderna vaccine.

Write-up: First noticed double vision and blurry vision while driving. A few days later, someone noticed that my left eye was not moving outward and along with my report of having double vision it was decided that I should go to the ER. Tests conducted included CT scan of head with and without contrast, MRI of head, eyes, and neck, ultrasound of neck, EKG, and blood work. Also had two separate series of tests performed by a neurologist. Everything came back normal. Was sent home and visited an ophthalmologist a couple of days later. Several tests were made and nothing was out of the ordinary, but doctor suggest a left 6th nerve palsy. Referred to neuro ophthalmologist. Appointment is still a few weeks away. Double vision has largely gone away as well as blurry vision. Left eye movement outward is still limited.

Write-up: Dose 1, 7:05pm June 3, 2021: - Tight throat / minor swelling and elevated heart rate within 5 minutes of vaccination, lasting for about an hour - Moderate level of dizziness / lightheadedness (needed to sit down and stay sitting) beginning 1 hour after vaccination, lasting for about 3 hours. - On waking the next morning, body aches and somewhat intense exhaustion lasting for half the day. - On waking the next morning, pain and tenderness at injection site (left arm) and pain lifting or moving arm. (steadily improving over 2 to 3 days) Dose 2, 6:50pm June 24, 2021: - Tight throat immediately following vaccination, lasting about an hour (but somewhat less so than after first dose) - Pain and tenderness at injection site the same evening of vaccination (continuing for a day or two, less pain than after first dose) - Within 48 hours: Flare up of shingles symptoms from virus case end of Jan - Feb 2021 (with no other such occurrences of symptoms happening after resolution of the virus). Symptom 1: Allodynia under left armpit/upper arm (along the same path it manifested previously) lasting around 48 hours in total and lessening over this time. Write-up: I had my 2nd Dose of Pfizer on 03/25/2021 and on 06/03/2021, I noticed blisters on my forehead and in my scalp. I was diagnosed with shingles through a video doctor. Pt is oriented x 3 but a poor historian. Pt reports chills, night sweats, and severe frontal headaches that have become progressively worse over the last 3 weeks. Pt reports blurry vision but denies extremity weakness, nausea, or vomiting. Pt reports intermittent fevers after her craniotomy but has not taken her temperature since being home. Pt denies nausea, vomiting, diarrhea, abdominal pain, dysuria, cough, congestion, travel, or known sick contacts. Pt tachycardic in 100''s and febrile 101 upon arrival to the ED meeting SIRS criteria. Pt without leukocytosis. CTH shows a low-density region within the left parietal lobe with overlying craniotomy changes of the calvaria that may represent postsurgical changes, recurrence, or known mass without an intracranial hemorrhage. CXR and UA were unremarkable. NSGY consulted in ED, recs pending. However, pt reports she does not want to continue with chemotherapy or radiation at this time. Current Illness: Chief Concern, Brief History of Present Illness, and Hospital Course 68yo M with PMH including Parksinson''s disease, oligodendroglioma (s/p chemo, radiation, resection), seizure disorder 2/2 brain cancer, BPH, CKD who presents for evaluation of progressively worsening weakness in lower extremities . Was evaluted by Neurology on presentation and dose of Sinemet updated. CT lumbar/Thoracic spine without acute abnormality. Plan for discharge to acute rehab which was denied by insurance and which wife is appealing. Pt has been finally accepted to a rehab facility and has an ambulance set for 1 pm today. Has CKD with Cr stable On room air Rest of the hospital course is as follows: ACUTE: #Acute hypoxic respiratory failure: resolved #Possible obstructive sleep apnea: -pt was hypoxic overnight at sleep on 6/15-6/16. Doesn''t use oxygen at home -CXR on 6/16 was stable in comparison to CXR from 6/5. Changes in CXR likely represent old COVID exposure. As he is no longer hypoxic, and has no other evidence of pneumonia, further imaging is not pursued.

Write-up: Severe chest pain; fever; This is a spontaneous report from a contactable consumer (patient). A 15-year-old male patient received second dose of bnt162b2 (PFIZER-BIONTECH COVID-19 mRNA VACCINE, Formulation: Solution for injection, Lot number: EW0179), via an unspecified route of administration in the right arm on 03Jun2021 at 10:00 (at the age of 15-year-old) as single dose for COVID-19 immunisation in public health clinic. Medical history included Tourette''s syndrome from an unknown date and unknown if ongoing. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. Concomitant medications included pimozide taken for an unspecified indication, start and stop date were not reported. A 38-years-old female patient received bnt162b2 (PFIZER-BIONTECH COVID-19 VACCINE, Solution for injection, Batch/Lot Number: EWD187), dose 1 via an unspecified route of administration, administered in Upper Arm Right on 03Jun2021 09:45 (at the age of 38-years-old) as dose 1, single for covid-19 immunisation. The patient''s medical history included, she was being seen by a rheumatologist to see if there are any autoimmune issues, she did test positive for something autoimmune. she has a pulmonary embolism. She has a list of conditions but declined to provide further details, felt like internal inflammation in her upper body, she experienced like a gurgling and squeezing of diaphragm like it was contracting and felt like fluid was in her left knee in last year (in 2020). The patient had no family medical history relevant to adverse event. The patient''s concomitant medications were not reported. No previous immunization with the Pfizer vaccine. No additional vaccines administered on same date of the Pfizer suspect. No prior vaccinations (within 4 weeks). The patient had tetanus shot two months prior to first dose of the covid vaccine. No adverse events following prior vaccinations. The patient was calling to report a side effect after the first dose which she received on 03Jun2021. She experienced the side effects the day of the vaccination. She received the vaccine about 09:45 in the morning. At 10:30 she started to have what felt like internal inflammation where her upper body, chest, back and arms and everything tightened up. It lasted about a few hours; it resolved that day. It was like muscles and everything just tightened up in her upper body and it stayed like that from 10:30 am till 15:00 pm. She experienced like a gurgling and squeezing of diaphragm like it was contracting. She felt like fluid was in her left knee which came and went pretty quickly. She also experienced little pains in her rib areas on left and right side. Everything was scary and unexpected. States she had these same symptoms when she got sick last year, she did not know if she had covid or not at the time, but she tested negative for covid and flu. States these symptoms did not occur for several months and then all of these symptoms came back in full force after she received the covid 19 vaccine. She spoke to her primary and was advised to report it. States she also experienced the fairly normal side effects such as nausea, fatigue, and dizziness. The nausea kind of lingered for the following two days. Fatigue recovered by the following day and the dizziness recovered the night of 03Jun2021. She declined any treatment. Her second dose scheduled for this week and she was asking if she should get the second dose. The adverse events required visit to physician office. No emergency room visit required. No relevant tests done. Her doctor could not advise and said it could just be here immune system reacting. The outcome of all the events was recovered. Information on Lot/Batch number was available. Write-up: a little over 24 hours exact same symptoms as first vaccine, except worse. Started with headache, that worsened and was on the couch non-functional at about 3. Fever/chills started about 6 and continued throughout the night. Next morning no fever/chills - just weak and tired for the next 24 hours. I missed 1/2 of the afternoon. Worked in the AM but still fatigued. These symptoms morphed into a cold with coughing sneezing, severe congestion. I ended up going to the DR 1.5 weeks ago and it had developed into sinus infection. I was treated with antibiotics and a steroid. I would say right now most symptoms are gone except fatigue and congestion (which could be allergies). I don''t feel completely well and have not felt really good since I had COVID. :(

Write-up: Toes tingling on left foot less than 24 hours after vaccine. 3days later itching between toes on left foot. Write-up: I started itching within 30 minutes of receiving the vaccine. I had weird reaction that felt like burning through my veins in just my right arm. I could not move my arm for 7 days after the shot. I was progressively feeling a bit better. I had severe headaches for 3 whole days. I had to take my medication for migraine for days until June 13. I definitely spent most days sleeping if not taking my medications. I did experience some more mild headaches but nothing like the migraine of hell from days 7-10. It was mostly near the injection site that it hurt the most. I tried laying in bed for several days to rest.

Write-up: 1. After FIRST dose, I had reaction within 30-40 seconds where I found it difficult to catch my breath, felt like I might pass out, my palms were sweating profusely, & I experienced tingling in my extremities. These symptoms resolved themselves within a few minutes, with the tingling resolving in about an hour or so.(no more noticeable reactions after that) 2. After SECOND dose, ...no immediate reaction like the first time, but arm hurt, as expected for the next couple of days. I didn''t notice any side effects until, 06/03, which was about 8-9 days later. So I was running at about mile 4(I am a trail runner), when something just didn''t feel right around my heart...some tightness, even some pain across shoulder blades. I continued the run...turned into a run/walk. I continued my day, with something feeling a little off and experienced some night sweats, & poor sleep. I went a couple more days and then on Sunday I went to the local Immediate Care. They did an EKG & sent me to ER. At the ER, they did an EKG, and wanted me to stay to be monitored over nite. At that time, I did NOT feel bad enough to stay, so I left with the agreement that I would return on Monday to do a Stress Echo on my heart. HOWEVER, upon getting home and having a bowl of soup, I experienced what I would call my first episode. Upon getting off the couch, my heart started beating unusually fast, it became VERY difficult to breathe, and I thought I was having a heart attack. Long story short, I drove back to the ER, they checked me in, did another EKG, a chest X-Ray, a chest CT-Scan, admitted me overnite, where they drew blood every 4 hours, and attached me to a heart monitor for roughly 12-16 hours. The next day they also performed a heart stress-echo. Everything came back normal. Since that time, I have had roughly 4-5 of those major episodes, with some smaller ones as well. I went back to Hospital ER on 06/19/2021 with another episode that I felt was worse than the original one. Again, they did an EKG, a chest X-Ray, various blood tests, and everything came back normal. Amongst all of this, I have seen my Family Doctor twice, and a cardiologist once. Hospital performed a heart Calcium test, which came back with a score of six. I believe the range where medium risk of a heart attack comes in is around 100-200. I have also been put on a Holter heart monitor, of which I have a week left of a two week assignment. I haven''t had a major episode in about a week and a half now, but I also stopped eating chocolate and given up all caffeine about a week and a half ago. All of that said, I still have tightness in my chest, as well as just an odd feeling there. Other symptoms I am having include being light-headed, dizziness at times, and, more concerning, tingling and numbness in my extremities. The tingling & numbness are primarily in my fingers, hands, and forearms. In addition, twice my index finger on my right hand has turned blue/purple, but ultimately returned to normal color after 24 hours or so. Finally, It is very difficult to get sleep. It seems my heart tries to wake me up every 2-3 hours to let me know something is not right. I can feel an unusual feeling almost immediately after lying down. I really can''t even explain the feeling in my chest, but I have never had this feeling before, and have never had sleep issues. In summary, I WAS a 52 year old man with no history of any heart problems, a fairly avid runner with a resting heart rate between 40 and 50, and no history of dizziness, tingling, or numbness in my extremities. After the 2nd dos of Moderna, I now have stopped running, have a myriad of symptoms as mentioned above, and my life has been flipped on its head for a month now,....moreover, I am getting little relief. I need help.

A few weeks later the rash spread to smaller rashes over the body, trunk, back upper legs, upper arms. It appears I have Pityriaisis Rosea. The occurrence of additional non-serious events is detailed below. The patient''s past medical history included Thrombosis in 1999 and Thrombosis in 2017. Concomitant products included ENOXAPARIN SODIUM (LOVENOX [ENOXAPARIN SODIUM]) and APIXABAN (ELIQUIS) for an unknown indication. On 05-May-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular) 1 dosage form. Write-up: I have had Tinnitus since late 2017. Several days after the first Pfizer shot the intensity and volume of my Tinnitus began to slowly increase. This kept up for about 2 weeks and it has remained at the higher level since. Tinnitus is now present in the mornings, which was never the case prior to the vaccine. It used to increase in volume throughout the day. Now it''s loud and present the moment that I wake up. I have been a lot more sensitive to noise in general. I am not sure if this counts as "permanent damage" as listed below, so I will leave it unchecked.

Write-up: Red eyes, slightly sore, beginning June 3, thought it was possibly beginning of pinkeye infection from getting spit in eyes. Visited doctor, he thought it could be slight infection or slight irritation or could just be allergies or dry eye; was prescribed 1 week course of Tobramycin eye drops. Write-up: After receiving 2nd dose on May 8, 2021, I awoke the morning of June 3, 2021, with hearing loss in my right ear. Muffled sound, unable to hear low sound such as whispering, unable to make out certain sounds or where the sounds coming from. I thought just a flukey thing, however, to date (July 10th, 10:32 p.m.) and a visit to Urgent Care and a visit to my Primary Care and so far a 4 day course of Prednisone (1 day left), zyrtec and fluticosone nasal spray I still have hearing loss the same as it was on June 3rd when I awoke in the morning. I have never in my life had hearing loss/plugged eaustachian tube for more than a day or 2 when I''ve been sick with a cold, so this is very worrisome. Write-up: Approximately 2 - 2.5 weeks after shot #1 on 3/19/2021: Abdominal Rash on left side (red, raised, hot to touch, itchy) - Oral Steroids prescribed. Write-up: 4 days after the covid-19 vaccine 2nd dose, I have severe pain started in the left back neck. The pain started from the neck and goes up to back left head, head above the ear and left ear. Left part of neck becomes stiff. The head pain is burning. If someone touches the pain points in the head, I don''t have a pressing pain but still have a burning pain. Initially though it was due to a sleeping position(pillow) change. But as day progressed, pain increased and saw left armpit swelled. Went to a walking clinic and Dr. said it is due to muscular pain and asked me to take over-the counter pain killer. I took the pain killer and pain reduced but when I stopped the medication, pain reappeared. I also found a pattern in the pain. Neck stiffness and pain starts at around 2:00 pm and it increases to 8/9 at around 3:00 to 3:30. The pain at peak is extreme pulsive burning pain and automatically reduces at around 4:30 pm to level 2/3. This happens every day. I always have pain of 2/3 during the throughout the day. Revisited the Same walk-in clinic and other Dr. said, It might be possibly linked to the Vaccine and they have few more similar cases reported. She prescribed me the Muscle relaxer and steroid and suggested me to contact my PCP for few blood work. I didn''t take any of them before contacting my PCP. PCP ordered some blood work and CTScan. Suggested me to take muscle relaxer and not to take Steroid as It will reduce the vaccine effect. The Blood and CTScan report came normal. She also referred me to Neurologist. Neurologist also looked at report and said, everything looks normal. I took muscle relaxer. Neck stiffness and head pain is reduced but I still have pain. It repeats at the same time interval and in same manner

Write-up: I was out of town, I had driven for about 12 hours, I spent the day relaxing, I went fishing, I got a sunburn on my knees and chins. My feet swelled up considerably and wound up with a really bad feeling that night. Feeling of dread that I felt my blood pressure was bad, I felt I was going to pass out. When I got home I bought a blood pressure cuff and it was 195/110-118 so it was really high. When I went to the doctor they gave me more medication and it has lowered a bit since then but not where it should be. I don?t know if it was because of the sunburn or for driving that long. But it happened after getting the vaccine.

Write-up: miscarriage; first pregnancy; Early miscarriage; Vaginal bleeding; Cramps; Low back ache; Maternal exposure during pregnancy; This is a spontaneous report from a contactable consumer received from the Regulatory Agency. The regulatory authority report number is GB-MHRA-APPCOVID-20210605192632, Safety Report Unique Identifier GB-MHRA-ADR 25426369. A 36-year-old female patient received first dose of BNT162B2 (PFIZER-BIONTECH COVID-19 VACCINE, Lot Number: E003143)via an unspecified route of administration at single dose on 03Jun2021 at the age of 36 years old for COVID-19 immunisation. Medical history included miscarriage, lactation decreased, ongoing pregnancy. Patient has not had symptoms associated with COVID-19. Patient is not enrolled in clinical trial. Patient is not currently breastfeeding. Concomitant medications included estradiol (ELLESTE-SOLO) taken for IVF support from 19Apr2021; folic acid (Manufacturer unknown) taken for folic acid supplementation at 400 ug; ascorbic acid, betacarotene, biotin, calcium pantothenate, colecalciferol, copper, cyanocobalamin, folic acid, iodine, iron, magnesium, nicotinamide, phytomenadione, pyridoxine hydrochloride, riboflavin, selenium, thiamine mononitrate, tocopheryl acid succinate, zinc (PREGNACARE ORIGINAL) taken for pregnancy vitamins from 22May2021; progesterone (UTROGESTAN) taken for IVF support from 06May2021. The patient experienced miscarriage on an unspecified date, early miscarriage on 05Jun2021, first pregnancy on an unspecified date, maternal exposure during pregnancy on 03Jun2021, vaginal bleeding on 04Jun2021, cramps on 04Jun2021, low back ache on 04Jun2021. The events were serious as medically significant. The clinical course was reported as follows: The day after the first dose of vaccine was administered the patient started having cramps in her low abdomen, ache in the low back, and brown vaginal spotting. The day after the patient had a more intensive, red, bleeding. The patient was 7 weeks pregnant, and the patient was suspecting that the vaccine might have caused this bleeding/aches and potentially miscarriage(?). The patient was yet to confirm if she has had a miscarriage (it is Saturday evening and the patient was unable to get hold of anyone on the phone or via email). The patient was first pregnancy via IVF. Patient was exposed to the medicine first-trimester (1-12 weeks). The patient underwent lab test included COVID-19 virus test: Negative(No - Negative COVID-19 test) on 04Jun2021. The outcome of the events first pregnancy, miscarriage was recovering, of the event Maternal exposure during pregnancy was unknown, of the events Vaginal bleeding, Cramps, Low back ache, Early miscarriage was not recovered. No follow-up attempts are possible. No further information is expected.
