Write-up: Serious headache starting Friday (4/9) evening and lasting through Sunday (4/11) morning. Woke up Saturday am (4/10) with temperature of 101.6 - fever was gone by 8 am. Overall did not fee well Friday (4/9) through Sunday (4/11). Slept on and off. Charley horse in right leg Sunday afternoon, which is rare for me. Woke up Sunday (4/11) with pain in left arm.

Write-up: Bilateral Pulmonary Embolism: Symptoms began acutely at 8pm on 4/12/2021. She developed chest pain, shortness of breath and nausea. Sharp pain. She was seen in the emergency department and at that time D-dimer was elevated to 1338. She had a subsequent CT scan that showed Subsegmental pulmonary emboli in posterior basal segments of lower lobe bilaterally

They believe it was because it wasn''t a clean catch. Culture was for Steph - came back as flagged...Today I am having another urinalysis and they will probably do another culture from that. Write-up: Chills and body aches - within two or three days after the vaccine and they resolved pretty quickly. I became symptomatic with general UTI symptoms on or about the 4th or 5th of April. I haven''t had one of these for about 7 years. I went to Urgent Care on 8th of April - I was put on two meds - antibiotic (Nitrofurantoin) and the other was for something to do with my bladder (Phenazopyridine). My symptoms have cleared up.

Write-up: Symptoms began evening after receiving vaccine. Patient reports headache which progressed into the next day to fever, fatigue, intense chills, chest pain. Patient had these symptoms over 2-3 days and had began to experience arm tingling in arm where vaccine was administered, which then spread to both arms. Patient reports arm tingling will come and go. Patient reports taking OTC medication (ibuprofen). Patient called a nursing line and was told to go to the ER. Patient went to ER on 4/14 and was admitted for observation. Chest X-ray was done with no remarkable findings. According to patient, additional testing was done. Patient was discharged on 4/15 and was told to return if arm tingling returns and does not go away.

Write-up: 4/8 Vaccination 12:30 By 8PM, chills, fever 101.4. Took Tylenol at midnight. 4/9 I noticed baby was moving less when I woke up. I had fatigue. My watch at 10am alerted me to high HR alert; inactivity but HR was at 130''s. I felt off. It happened 4xs thru the day with the apple watch during the day. Write-up: I am not sure if my event was caused by the vaccine I received on April 8, but I have a very strong feeling it is, as never in my life have I experienced anything like it. My day began normally, and I just had returned home from an appointment. I had some lunch, and a few minutes later as I was standing in my kitchen, I began sweating terribly, heart was racing, and I felt like I was going to pass out. I managed to make it to a large living room chair, and once I say down and put my head back, I passed out. My daughter witnessed everything and said when I passed out, my arms were trembling and my eyes rolled back. This lasted for about 30 seconds. She was unable to arouse me for a bit, but when I came to my speech was slurred. I did not know what had happened. My daughter called 911. By the time medics arrived, I was feeling much better and aware of everything happening. The checked for signs of stroke, vital signs, blood sugar, and everything was normal. I was transported to the ER, and my work up there was normal, including EKG and blood work. The physician that I saw diagnosed me with vasovagal syncope and did not feel the event was related to the vaccine. I completely recovered, and like I said I?m not sure if it was related to the vaccine I received or not. I just feel that it may be worth noting since I?ve never had an experience like this and I?m pretty healthy with really no medical history.

Write-up: Pt called today asking about blood clots. Stated her foot swelled up "like a balloon" a few days after receiving the shot. She did not give specific dates, but we are over 2 weeks from date shot given, and over 1 week from when she claims to have swelling, and she called today after hearing about clots in news. Write-up: Johnson and johnson covid-19 vaccine eau March 20th - allergic reaction to covid vaccine Johnson & Johnson. Allergic reaction started approximately 2 minutes after receiving shot. It started with itching and redness on my arms and upper chest. After approximately 10 minutes, I knew something was wrong and I signaled an attendant. They immediately brought me over to the EMTs on site, where they looks me over and started drawing up IV Benadryl. In the time it took for them to draw up IV Benadryl and insert the IV, the hives on my chest started to turn into burning and ice sensations intermittently. As soon as they administered the IV Benadryl, the itching redness and hives on my arms went away within a couple minutes, and I was left with only redness on my upper chest. They felt comfortable with me getting a ride to the ER from my dad and not getting emergency transport. At the ER, they gave me pepcid and prednisone. About an hour and a half later, I started developing chest tightness and hives and redness on the side of my face. They decided to administer more Benadryl and start epinephrine. I was discharged a couple hours later. March 22nd - I went back to the hospital, with complaints of severe stomach pain. The stomach pain lasted approximately 24 hours. March 20th to march 26th - I experienced extreme fatigue. I was exhausted to the point where on Wednesday I took a ten minute shower and by the end of it I had to lay down again because I was shaking from exhaustion. Symptoms improved slightly until April 5th where it started taking a turn for the worse. April 5th - I started noticing black spots in my vision that wouldn''t go away. They didn''t move and they stayed in one spot for hours until I fell asleep and then they''d be gone the next morning. They usually started around 8:00 or 9:00 at night. Sometimes a couple hours earlier. They came every day until Friday April 9th when they did not come. April 5th to present - I also started noticing light-headedness, motion sickness when I stood still or sat in a chair, nausea, neck pain, and periods of muscle weakness in the neck. These symptoms have not stopped. Friday April 9th around 6:45 p.m. I started having a bad migraine, I noticed in the bathroom at work my eyes were dilating over and over and over again rapidly. The pain was very high. I ended up leaving work early and going to the emergency room. While at the emergency room they treated my migraine with a cocktail of Benadryl, toradol, and compazine. It reduced the pain at the time, however it did not take away the migraine. Friday April 9th to present - My migraine has continued from April 9th and is still happening. I have been having severe light sensitivity, sound sensitivity, and trouble reading. The rapid dilation of my eyes stopped April 10th, however my eyes stayed dilated until April 12th. In addition to all of my other symptoms, there are others that I am not as positive are relevant. I have asthma and it has been worse over the past couple of weeks, it it is not usually worse this time of year. I''ve been also been having quite a bit more light bruising than normal over the past two weeks and over the past one and a half weeks I have noticed red spots all over my skin. I was not sure if these red spots were normal or not because I do have them on occasion however these are darker than normal.

Write-up: No symptoms until 10pm when I started to experience achey joints, sensitive skin, and sore muscles. 12am is when I started to experience a fever and took my temperature which was 101''F. The fever and muscle aches lasted until the following morning. Around 12pm is when I began expecting some stomach discomfort/sharp pains on the upper left hand side of my stomach. It began to increase in intensity around 4pm when I began to get nauseous. 5-6pm is when I began vomitting, and decided to go to the ER after the pain reached a level 8-9 and my face and limbs felt numb. Write-up: feeling of closing of the throat. Pt came in for covid vaccine. Answered no to screening questions. Was given Janssen single dose. Pt asked MA, what an allergic reaction looks like. I stepped in and told pt. hives, swelling/ closing of the throat, and feeling like you can''t breathe. Pt stated @ 1535 that she felt her throat was closing up a little and that breathing was getting difficult. Pt was speaking in clear full sentences w/o difficulty. I told her to take deep breathes and drink water. I assessed her, she was warm and dry and no apparent swelling of the throat, no itchiness or dryness of the throat. MA brought in BP machine, vital signs were stable. O2 was at 95%. Pt has asthma. I wanted to seek a second opinion on pts well-being, MA went to seek provider that had seen pt prior to vaccination. As MA went to get nurse, I continued to talk to pt about her status. I asked if the feeling was getting better or worse and pt said it was staying the same. Pt was not in distress and speaking in clear full sentences. I told her if her status worsens I have an epi pen to help her breathe better. MA came back 5 minutes later w/o provider and pt stated her throat closing as she had her hands on her throat and was worried. MA did not realize the urgency of the situation and went back to get the nurse. During this time I asked pt if the feeling was getting worse and pt said yes. I told her I have the epi pen and thats when pt requested I administer it to her. I administered the epi pen at 1554. One minute later Nurse comes in to assess pt. Pt is calm and speaking in clear full sentences. MA takes vital signs and BP is elevated and O2 is at 99%. I inform the nurse that I administered epi and she advised to keep pt for observation, she left to see another pt. Pt was calm and speaking in clear full sentences A&O X4 and stated that the feeling was better. At 1610 Pt stated that her throat was feeling the same closing feeling as before the epi pen. I messaged my nurse supervisor, at 1612 about pt and she proceeded to bring Dr. for assessment. Dr. assessed pt and told me to call 911. Pt was not in distress and speaking in clear full sentences. Fire dept. came and took over the situation approximately 8 minutes later. Pt requested to be taken to Hospital.

Write-up: The patient called the pharmacy on 4/7/2021 and said that he had vomited and had been experiencing an excruciating headache that he said was worse than any headache he had ever had and was concerned about how he was feeling since he had just had his Covid vaccine on Monday. I told him that headaches were a very common adverse reaction and that many people had experienced headaches after vaccination. I told him that from my experience it usually didn''t last more than a day for others and I assured him that it should go away soon. I later talked to him and found out that he had went to the Emergency room that Wednesday evening (4/7/2021) because the headache was so bad. He was seen in the ER, observed overnight, released the next morning, and then had a follow up appointment with his primary care physician on 4/15/2021. He also had a very high blood pressure reading he told me in the Emergency room that evening he was evaluated.

Write-up: After the injection about 15 minutes afterwards I was getting dizzy I was having trouble breathing. Was given a Benadryl. An ambulance was called. I do not remember being transferred to the hospital. I was given solu- Medrol (methylPREDNI solone succinate 125mg IV and 20 mg Pepcid IV for an allergic reaction to the vaccine. I was prescribed an EpiPen. I am still having side effects from the vaccine. Migraines, dizziness, hives, swollen lymph glands.

Write-up: On the day after vaccination I had extreme dizziness and faintheadedness, and heavy brain fog. I could not drive or stand up for long periods of time. I was having issues expressing thoughts and having my normal conversations. This was accompanied by strong headache on right side of my head and slightly stuffed sinuses. The day afterwards all symptoms disappeared. However, they reappeared at milder levels intermittently through the next several days. I am now at day 14 with the dizziness and brain fog still occurring intermittently and last night had short episode of abdominal pain. I decided to call the doctor''s office this morning when the dizziness started again, for advice and to schedule a brief exam. They told me to go straight to the ER instead, so I went and had blood drawn. Blood came back normal and likelihood of clot very low, so was released but referred to my primary doctor and a neurologist for follow up over the next week. The intermittent dizziness and brain fog persist ever since my shot 2 weeks ago. Additional symptoms that I believe to be normal are dull leg pain behind the knees, slight tightness in chest sometimes, and recurring mild headache.

Write-up: Patient started noticing left arm swelling and pain about 2-3 weeks after the vaccine. Write-up: Within 5 minutes of the shot, I started to have difficulty swallowing. I notified the staff at the clinic, They put me in a separate room where I took 50mg Benadryl. I continued having difficulty swallowing along with tingling in tongue and lips. I then decided to give myself an Epipen 0.3mg shot. I had this with me because I had a severe allergic reaction in January to Pfizer COVID 19 vaccine. 911 was called. In the ambulance, IV Benadryl and Solu-Medrol was administered. My face was starting to itch on the way to hospital even after IV medication administered. I was monitored for 2 hours in hospital. I was also given normal saline IV in the hospital. I was discharged with Pepcid 20mg daily for 4 days and Prednisone 50mg daily for 3 days. I consistently had to take Benadryl 50mg for three days to keep itching and swelling at bay.

Write-up: On March 13th, 2021 I was given the opportunity to receive the Janssen Johnson & Johnson Covid 19 vaccine so I decided to go ahead and get vaccinated. Within 1.5 hours of receiving the vaccine, I began to feel unwell. It started with my left leg going numb. From there symptoms continued to increase. I got very tired could hardly stay awake. Then the chills hit. I could not get warm. By the next morning (Sunday day 2) I had developed a low fever, headache, body aches and whole body weakness. I could still hardly stay awake still. When I was awake, I was very dizzy and I felt like everything around me was moving but I was not. My left leg still felt numb but I thought it would pass on its own.. Because of how I felt, I just laid around and rested. My injection site was swollen, Red and extremely hot to touch. Next comes Monday (day 3), I got up that morning, still not feeling well. I made my way to work. That did not go well. The more I moved, the worse I felt. I was completely unbalanced and looked like I was drunk. I couldn''t walk straight, I had to randomly grab onto chairs and sit down. I was feeling weaker by the minute. I left work at that point and went to see my primary physician that afternoon. At this point I could see straight to drive myself, could hardly walk as my left leg now dragged. I had no balance and having lots of dizzy spells at this point. He suggested I head to th ER but eventually decided we could wait until morning to see if things improved at all. If no improvement, I was to go to ER. Tuesday Morning (day 4), my husband took me to the emergency room. I had reached the point of not being able to walk unassisted. At this point, the weakness and numbness I had on my left, had spread to my right side. We had no idea what was happening. We got right in and discussed what was going on. What lead up to me being there, etc. The ER ran blood work, xrays of my chest and 3 different MRIs trying to figure out if something happened like a stroke. NOTHING was present on any test they did yet there was something obviously wrong with me. My arm was still very Red and hot.. I had developed a lump in my arm where the vaccine was given. After ruling out everything the doctor could think of, he consulted with the on call neurologist since it was obvious to him something was happening with my neurological system. The diagnosis...my body was taking the vaccine as a toxin causing my neurological system to react and shut down. They felt that I should be starting feel better in the next few days as the vaccine settles into my system. I was told to follow up with a neurologist and my primary. I was then discharged from the hospital in the condition I had arrived. Wednesday (day 5) I spent the day at home, not able to do anything. I napped most of the day. I still felt like crap and had no idea when I would start feeling better. No improvement that day. Thursday (day 6) I woke up feeling a bit better. My right side was beginning to feel normal again but I was still feeling weak and unbalanced. This is also the day I meet with a neurologist to follow up. I don''t know that this appointment had accomplished much as she didn''t have access to all my medical records. She ended up prescribing me a medication to help with nerve pain I was having in my left side and referred me to do physical therapy. I am to follow up in 2 months. Friday (day 7) my right side is back to pre-vaccine stage. My left is not. I feel pretty good except the occasional dizzy spell and the fact my left isn''t working yet. Day 8 and beyond...I''m feeling ok. I went back to work but still have issues with my left side and residual dizzy spells. I began physical therapy 2 days a week to work on strengthening my left side however it has yet to help. I am now just over 4 weeks post covid vaccine. I am now using a cane to help me walk as my left side has still not returned to normal. It is unknown if it will. I''m in Physical therapy 2x a week still. The lump at injection site is now the size of a pea. I still have moments of unbalanced feeling and dizzy spells.

Write-up: On March 21, 2021 at around 5:30 I had fever, headache, nausea, fatigue, dizziness. The next day, March 22, I had heart palpitations, dizziness and still fever. On March 24, 2021 at approximately 8:00 pm, my right leg became heavy and felt numb. On March 25 I woke up to my mouth locked with Bell''s Palsy like drooping on the right side of my face. My mouth remained unable to open fully for the next day and the numbness and pain in my leg increased. On March 26 I went to the ER because I was afraid I was having a stroke. They gave me tests and said I did not have a stroke. They gave me steroids and they helped with my jaw but the pain in my leg increased. The pain in leg continued with more pain in my knee. I followed up with my family doctor and they sent me to have an ultrasound of my leg to check for a blood clot on April 9th No blood clot was found. Pain continued and increased so the doctor sent me for an MRI on April 14. MRI showed moderate effusions to the knee joint and I have an appointment with a orthopedic doctor on Tuesday April 20th. I continue to have stabbing pain in my knee and swelling and discomfort.

MRI demonstrates large enhancing sellar and supraseller mass extending into the left sphenoid sinus and causing mass effect on the optic chiasm and inferior left hypothalamus likely representing a microadenoma, no hemorrhage. Write-up: Pt''s mother called, and stated that her son in autistic so she was calling on his behalf. She stated that her son has been experiencing some adverse reactions since receiving his Janssen Covid vaccine on 4/3/21. She states the the first three days he experienced sleepiness, pain, and flu like symptoms; which she treated with Tylenol. Over the course of the next couple of days the symptoms have gotten progressively worse, now including stomach pain, disorientation, body aches, and unusual behaviors. She states that the symptoms are intermitted, may stop for a day or two then returns. She states her son does not have a fever. Write-up: 4 days after vaccination, beginning at 3AM on Wed 04-15-21, began uncontrolled and unending concurrent watery diarrhea and vomiting and abdominal pain, and sweating. Taken to hospital by ambulance and admitted for 2 days. Write-up: Patient went to the hospital Monday. He received the Janssen single-dose Covid vaccine on Thursday, April 8 and by Saturday, April 10 he was starting to develop fairly significant peripheral edema without any breathing difficulties. This persisted and was worse by Monday, April 13 EKG showed global nonspecific T-wave abnormalities and ST segment changes so I had him sent to the ER. As it turned out he had a pancytopenia (mostly neutropenia and thrombocytopenia) so he was admitted to the hospital. This seemed to resolve pretty much on its own and so they attributed everything to his hepatitis C. Indeed his LFTs are elevated in the 300 range and imaging showed hepatosplenomegaly. they also did a bone marrow biopsy for which there are no results yet

Write-up: Itching started around 8:30pm on 4/15, Hives started on inner thighs at 11:30pm on 4/15, took Loratidine at. midnight, hives were gone at 4am on 4/16. Hives and swelling itching returned on 4/16 worse and spreading on legs armpits and neck-other patches spread all overs, took Benadryl at 5:00pm no 4/16, worsening and went to a local urgent care for treatment around 7:00pm on 4/16, told it may be related to the vaccine as it did look like a medication allergy. Given liquid oral steroids at urgent care and prescription called in for prednisone and hydroxyzine to be started the next day on 4/17. Hives did not go away at all, able to sleep until about 5:30am 4/17 and woke to worse itching and hives spread even worse, all over legs, hips, arms stomach back and neck. Pharmacy not open until 10am 4/17, to pick up new prescriptions, so was not able to take the hydroxyzine and prednisone until 11:15am today, 4/17, which was 1 hour ago, still extremely red, itchy, swollen hives all over as of right now. Will give the medicine more time to work before going to emergency room, since my breathing has been fine.

Write-up: Facial numbness on left side of face including forehead, cheek, jaw, ear, lips, and tongue. I had a slight droop on the l eft side of my face, mostly noticeable around the corner of my mouth. I also experienced itchiness around the injection site. Write-up: I was 9 months pregnant at the time of the vaccination. My due date was April 3, 2021. The actual date of delivery was March 20, 2021 by C-section. I gave birth to a son weighing 8 lbs 6 oz. I had no complications during this pregnancy. I left the hospital on March 22nd. On March 25, I was experiencing shortness of breath and a low pulse (35 bpm; my usual pulse is in the 50s-60s). My OBGYN told me to go to the emergency room, where they did a CT scan. The CT scan showed a small pulmonary embolism. I was in the hospital until March 27th, where I was treated with Heparin and then was released with Lovenox injections. I am currently on the Lovenox. Write-up: I got the vaccine on 4/7/21 at approx. 10am. Soreness at injection site started about an hour later. At approx. 3pm that day I began to have body aches and chills and had a low grade fever and a very bad and continuous headache. Those symptoms lasted approx. 24-36 hours. By Friday 4/9, I was feeling better and fever was gone. Starting approx. Sun 4/11 I began to have a dull headache wrapping around the back of my head/neck and began having muffled hearing (bilateral, but more prominent in left ear). On 4/15 at approx. 1:15pm I was sitting at my desk and suddenly my heart rate went from resting heart rate to 150. I could feel the pounding in my head and ear and also placing my hand over my heart. I tried to stand up to walk around thinking that might help my heart rate, but I felt faint, so I immediately drove myself to urgent care. My heart rate came down and was back to normal approx. 5-10 min from onset. EKG and CT were done in the ER. I was released approx. 5-6 hours after arrival and told to treat headache with Tylenol or Ibuprofen (which I have not done so far because I normally try not to take medications), and follow up with my primary doctor in a week. Headache and muffled hearing are still present today (4/17), predominately affecting left ear.

Write-up: Immediate muscle tension in left arm following vaccine shot in left arm. The soreness persisted for several days. Painful to lift left arm higher than my shoulder. The morning of day 3, I woke up to feel a pronounced skin irritation under my left armpit. Later that day, I noticed a small welt on my left arm and the irritation had grown to include my upper left arm front and back. Over the next few days, the number of welts increased and the pain was increasing from the skin irritation. The tension in my shoulder started to give way to feeling tightness in my left chest. I reported the symptoms. I tried to report to the Health Department, but they wouldn''t take a report stating that the symptoms I was reporting did not match "known symptoms". On 4/13/21, I went to the Urgent Care to have the rash examined. The doctor tentatively diagnosed as Shingles. However, because of the chest pain, he referred to the Emergency Room for blood tests to rule out a possible Pulmonary Embolism. The blood tests came back normal. I was prescribed 12 Hydrocodone-Acetaminophen for the pain and discharged. On 4/15/21 I returned to Urgent Care because the welts were continuing to spread and the pain was increasing. Including sharp pains associated with the shingles. At this visit I was prescribed 21 tablets of Hydrocodone-Acetaminophen and 21 tablets of ValACYclovir.

Write-up: On Thursday around noon, pt. had a headache, chest pains, numbness in left arm, the ER checked his heart rate and it was 215, they did blood work which came back normal & took more blood 4 hours later to check for heart damage, They admitted him to the hospital and did an EKG the next day that did not show any negative results (you''d have to get that from the hospital) They completed an Angiogram and found no blockage. Today, April 17th, I believe they did a cat scan of his lungs to check for blood clots and I am not sure of the results.

Write-up: after approximately 10 minutes of receiving Covid-19 vaccine, the patient fell out of chair in which he was seate. patient was unconscious for approximately 1 minute while I rolled him over and checked for pulse. his eyes were wide open and he seemed to have had an absensence seizure lasting upto a minute before regaining consciousness. he was quickly cognizant and I asked if he had any history of seizures or if he might be hypoglycemic. He did say that he had not eaten and it was 3:00pm. paramedics soon arrived and removed the patient from the scene. I did text patient the next day and he said only that he had a sore arm and symptoms consistent with having a vaccine administered.

Write-up: My sister, received the J & J vaccine on March 5th. I now know that she was not feeling well for a few days afterwards. She then felt better until day nine after the vaccine. On the evening of the ninth day, she began having abdominal pain, vomiting & diarrhea, headache. This was Sunday evening. Unfortunately I wasn?t aware of how she was feeling and she didn?t go to the emergency rm. until Tuesday morning. Tests determined her platelet level was low and abdominal CT showed bleeding from her adrenal glands. She was given platelets, pain meds and admitted into the hospital. Later that night staff felt she wasn?t responsive and a head CT was performed. It was determined she had a brain hemorrhage and emergency surgery was performed. It is my understanding that there was brain damage at that point. During the early morning hours of the 10th day, post surgery, a repeat CT scan was done showing more hemorrhaging and blood clots. She also developed a DVT and PE and was kept on a respirator until the 13th day post vaccine to allow family to say their goodbyes. On the morning of the 13th day patient was removed from life support and she passed.

Write-up: NAUSEA; NUMBNESS OF FACE; TINGLING OF FACE; INJECTION SITE PAIN BUT PAIN UNDER RIGHT ARM; ANAPHYLACTIC REACTION ( FEELING HOT, AND THROAT FELT CLOGGED); This spontaneous report received from a patient concerned a 54 year old female. The patient''s height, and weight were not reported. The patient''s height, and weight were not reported. The patient''s past medical history included anaphylactic reactions with other vaccines, and concurrent conditions included antibiotic allergy (except ciprofloxacin), non-alcoholic, non-smoker, cancer, cerebral palsy, and pheochromocytoma. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 1808978, expiry: UNKNOWN) dose was not reported, 1 total, administered on right arm on 27-MAR-2021 12:15 for prophylactic vaccination. No concomitant medications were reported. On 27-MAR-2021 12:21, the patient experienced anaphylactic reaction (feeling hot and throat felt clogged and closing). The patient was taken to the emergency room and given epinephrine. Anaphylactic reaction continued during the transport to the emergency room. On 28-MAR-2021, the patient experienced injection site pain but pain under right arm, numbness of face and tingling of face. On an unspecified date, the patient experienced nausea. Patient also received a rapid COVID-19 test on 07-APR-2021 and waiting on lab results. The patient was worried about the cancer surgery planned after 5 days. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient recovered from anaphylactic reaction ( feeling hot, and throat felt clogged) on 27-MAR-2021, was recovering from injection site pain but pain under right arm, numbness of face, and tingling of face, and the outcome of nausea was not reported. This report was serious (Life Threatening).; Sender''s Comments: V0: A 54-year-old female experienced anaphylaxis 6 minutes after vaccine. Past medical history included anaphylactic reactions with other vaccines, and concurrent conditions included antibiotic allergy, cancer, cerebral palsy, and pheochromocytoma. Six minutes after vaccine she reporting feeling hot and throat felt clogged and closing. The patient was taken to the emergency room and given epinephrine and recovered the same day. Given the close temporal relationship, history of anaphylaxis to other vaccines as well as antibiotic allergy, the event is considered related.

Diagnostic Lab Data: EKG was performed on arrival to ER, all results normal, including PR and QTC. Blood tests were all normal: Sodium 142 Potassium Glucose 145 BUN 14 Creatinine 0.67 D dimer 0.29 Doctor suggested it could be an acute stress response, or spontaneous panic attack. Unfortunately, another attack (dizzy spell) occurred spontaneously today (4/17) and I was able to slow my breathing enough to prevent further symptoms.
CDC Split Type:

Write-up: Sudden dizziness without any trigger or prior condition, immediate occurrence of numbness in left hand, spreading up to left elbow , and heart palpitations / chest tightness all within 30-60 seconds onset. In the 5 minutes waiting for paramedics, the numbness spread to the right hand and up to the right elbow. Dizziness became intense. Once in the ambulance (5-10 minutes after onset), legs experienced light numbness, and chest tightness increased, with what felt like spasms / clenching of the heart muscle. Chills and out of body feeling occurred as well. Paramedics examined heart, pulse, oxygen, with all being normal. IV was inserted and Ativan was given. After 10-15 minutes symptoms subsided.
