Diagnostic Lab Data: EKG, Neuro evaluation, Head CT with contrast, urine test. The only clinical detail to come from the tests was dehydration. 1 L Saline given through IV port. The ER physician could not difinitively determine that the event directly correlated to the vaccination, but the symptoms (they type and feeling of dizziness and fogginess) were similar to that of my covid infection in Dec 2020.
CDC Split Type:

Write-up: 103F fever, full body tremors, body aches and headache, 4 hours past injection. Fever lasted 9 hours from 4/5 pm until 4/6 am and returned approximately 20 hours after first starting. Second fever round was at 101F and lasted from 4/6 afternoon until 4/7 am. Visit on 4/8 to the emergency room for severe dizziness and disorientation. Dizziness was in waves and lasted for most of the day into the next day and has tapered off over the last 2 weeks.

Write-up: on March 16 went to ARNP to complain of breathing issues and provider was dismissive saying will refer to cardiologist. Told provider unable to sleep and was given RX for Xanax. Continued to struggle for two more days and ended up in ER then admitted with 6 days on a ventilator, 10 days in ICU and 12 day hospital stay. Discharged with O2 at night. Not sure of long term prognosis. Initial PCR was negative for COVID.

Write-up: Came to ED on 4/15 with c/o anginal symptoms, found to be in rapid afib. EKG showed ST elevation in later precordial leads, went for emergent left heart catheterization. Went into V-fib arrest requiring defibrillation (x2) in addition to CPR in cath lab. Angio showed acute thrombotic occlusions of LM, LAD and RI, underwent mechanical thrombectomy of LM and LAD with balloon angio of RI (recent cardiac cath in March 2020 showed clearly patent stents). Placed on Impella bypass. Currently in CVICU care, intubated with Impella.

Write-up: Pain in lower left bicep in first week, experiencing fatigue/tiredness. After 10 days I experienced severe pain at the site of injection, very small red spot appeared around injection site. Pain was so severe, I took one capsule hydrocodone 5-325 TB at 2 AM. The medication did not cure the pain, and I slept less than two hours. I called my nurse practitioner''s office, and they offered an appoint late in the morning. And I couldn''t wait that long so I drove to urgent care. Prompt care examined and saw no swelling, ruling out possible blood clot. Prompt care did not have ultra sound. They told me to use ice, heat, and pain medication. Extreme pain continued for about three more days, pain medication did very little to help. Unable to speak directly to the Nurse Practitioner on the phone, due to my disability and alexia, communicating online is very difficult without an assistant present. The pain slowly subsided over the next week.

Write-up: is a 39 yo M with no significant PMH. He presents for symptoms following Johnson and Johnson vaccine. He had one dose of Tylenol 4/12/21. At around 4pm on 4/14/21 he reports that he had SOB, more bilateral leg pain, and dizziness. He states he went home and slept. Woke up Thursday and all his symptoms had resolved. Denies chest pain, numbness, tingling, swelling in his legs. Patient does endorse that he does have a stressful job working for airport and he had 3-4 cups of coffee daily. He also endorses that after the new of the side effects of the Johnson and Johnson vaccine appeared, he did notice he had more anxiety. Write-up: Patient started having extreme pain and weakness in hands and feet on 3/20/21. Patient has been to UC/ED and her own family provider multiple times since receiving the vaccine due to debilitating joint pain. She states that her joints have been significantly inflamed making it hard for her walk, type, hold a pen. Symptoms have been most significant in her hands and feet. She has had to be increased to Prednisone 100mg po to be able to function. Pt reports acute onset of LLE pain the evening of 4/16 with progressive pain over the weekend and development of LLE edema/redness 4/18. Left leg mildly more edematous than right. Patient was being transferred to a Medical Center by helicopter. Five minutes after takeoff, the helicopter turned around and came back with patient coding. She had been talking and suddenly stopped. Attempts at resuscitation were not successful. The patient expired on 4/16/21.

Write-up: 5 minutes after injection my forearm and hand began to tingle and have pressure, nerve type pain. No breathing issues so was sent home. Reported the continued arm pain the next day to the doctor and she ordered an ultrasound. Report negative on blood arm clot. Got a fever, body aches, headache, tired, triggered a migraine per doctor, localized reaction of 2 3/4" at injection site with redness, swelling, pain. Almost passed out at a store - Became dizzy, tingling hands and sweating and taken to ER by ambulance. ER ruled out blood clot but found I had a UTI without any symptoms-prescribed an antibiotic. Contiue to have minor headache and minor discomfort at injection site. Awful experience and adverse reactions!

Diagnostic Lab Data: At the ER (April 13th), I was given an EKG (Normal) and CT scan. The CT scan showed mild inflammation in the left upper quadrant of my colon and was diagnosed with mild diverticulitis and acute pancreatitis. I was given a prescription for antibiotics and put on a 3 day liquid diet and scheduled an immediate appointment with my GI doctor. Upon visiting Dr. (GI doctor) on Thursday April 15th, I was informed that it was a case of Diverticulitis and based on its location (upper left quadrant) he wanted to do a colonoscopy and biopsy, due to the unconventional location of the diverticulitis. I was told to continue on my medication and start easing into foods, like rice and mash potatoes. He was surprised as am healthy and have no other symptoms that may have caused the disease.

Write-up: Note this is in reference to initial report filed 03/31/2021 - Received Janssen vaccine for COVI-19 in mid-March 2021. Over the next couple of days, became very ill, spent five days experiencing severe headache, disorientation, confusion, neck, shoulder, and back pain together with generalized arthralgias and myalgias. Around the 2nd week post-vaccination began to experience ?numbness and tingling? initially, affecting all fingers and toes; symptoms gradually evolved into ascending paresthesia affecting the upper and lower extremities. (worse in the upper extremities). Neurological symptoms are accompanied by painful electrical-like impulses running along my spine and elicited by flexing my neck (Lhermitte neurological phenomenon). Neurological signs and symptoms prompted me to go to the emergency room. Currently, I am under the care of a neurologist and undergoing extensive diagnostic workup. Present treatment: Glucocorticoids (tapering) and Gabapentin. Among the initial rule out diagnosis, were transverse myelitis, and Guillain Barre. Present preliminary r/o diagnosis: Post-vaccine immune reaction ? No definitive diagnosis yet.

Went back to Urgent Care on 4/14 and was again treated for dehydration. Outcome is that no one knows why I have been feeling this way except for that it is a direct result of receiving the vaccine.

Diagnostic Lab Data: The patient is a 64 y.o. male evaluated for a few days of right sided neck pain wrapping around to his right upper back. No recent trauma, no objective neurological deficits or other indications for further imaging studies. No risk factors for spinal epidural abscess. No clinical evidence of cauda equina syndrome. Will treat symptomatically. I feel that he is appropriate for discharge at this time and the patient is comfortable with this plan. He has been provided with prescriptions for Medrol. The patient has also been advised to follow up at the Provider for further evaluation. Home care instructions and return precautions were discussed prior to discharge.

Write-up: The patient is a 64 y.o. male who presents to the Emergency Department with a few days of right sided neck pain wrapping around to his right upper back. Patient reports a history of similar pain prior to his cervical laminectomy hardware placement. Patient also reports that his sciatica on the right has been exacerbated recently and he is having BLE numbness. He is ambulatory. Patient was seen in the ED for this complaint on 4/5 and was prescribed ibuprofen and Robaxin. He denies any alleviation with these medications. He denies any recent falls, injuries, or other concerning MOI''s. Patient notes that he has been trying to get into his PCP, but they are on vacation. He recently quit smoking.

Subtle, multifocal T2/STIR hyperintense signal abnormality seen at the thoracic spinal cord. Exact etiology uncertain, but findings could reflect demyelinating disease. Longer segment involvement spanning T2-T5 would be somewhat unusual for multiple sclerosis. Neuromyelitis optica is known to cause longer segment demyelination lesions. Consider correlation with CSF analysis. No enhancement seen to suggest active demyelination or other inflammation. Other causes of cord signal abnormality such as metabolic derangement and transverse myelitis not excluded. Neoplastic etiology thought unlikely. 2. No significant spinal canal or neural foraminal narrowing.

Write-up: Pt states she had weakness that began about 30 minutes after vaccination. This included muscle spasms and muscle pain "all over." She states she had flu-like symptoms for ~1.5 days after vaccination and then felt ok until approximately 3/22/2021, when muscle weakness began and has not stopped. She presented to another hospital on 3/29/2021 to the emergency room and "had a full work-up including EKG, x-rays, and lab work" and was told she had a viral illness. She was given a prescription for azithromycin and ibuprofen, as she was complaining of sore throat. She presented to the emergency room on 3/30/2021 with c/o generalized weakness, back pain, myalgias and chest pain. She also c/o fever and chills. Temp in the ER was 98.5F. Physical exam revealed "diffusely tender abdomen, no rigidity or guarding" and "diffusely tender back all throughout." 3/31/2021-saw family physician w/ c/o cough and myalgias, negative for back pain and chest pain/palpitations. Sore throat and cough were improved. Was diagnosed with an apparent viral illness and supportive care. She was advised this could be a viral illness and discharged to home in stable condition. 4/11/2021-Pt reports she went to urgent care b/c felt she had a bad ear infection and eyes were hurting (mostly right); was diagnosed with sinus infection and that ears looked ok. Was told to stop azithromycin and begin amoxicillin and ibuprofen. 4/16/2021-presented to emergency room. "Patient with 3 weeks of progressive symptoms of headache, blurry vision, nausea, weakness, and poor control of her bowel and bladder. Symptoms are constant with gradual progression with a duration of 3 weeks. Timing is constant. Severity is now severe. Quality is sharp, tight headache." Vision changes got worse when she was on the flight there. "No exacerbating or alleviating factors. Not associated with fevers. Does have some nausea and muscle aches. Diffuse tremors and difficulty walking without assistance. PE revealed mild psychomotor retardation, tremulous and diffuse weakness, and Romberg present. Mild right sided papilledema. 4/17/2021- further HPI recorded-The patient is presenting for evaluation of headaches, paresthesias, numbness. The patient is a fairly good historian and tells me that symptoms began after she received the Johnson & Johnson COVID-19 vaccine. Patiet began to feel fatigued, malaise, numbness and weakness over the legs. Patient had some mild blurriness of vision in the right eye. Symptoms have been persistent and patient began to have some difficulty with walking and feeling off balance. Patient describes the headaches as primarily right-sided. According to nursing staff, the patient has also had some incontinence of urine today. Patient has had some numbness over proximal legs and "private areas". 4/19/2021 ? Assessment as per Infectious Diseases physician; Pt with continued intermittent headaches. Has continued difficulty bearing full weight due to unsteadiness. No loss of sensation at arms/legs. Has action tremor, wide gait, hyperreflexia of arms/legs bilaterally. ADEM Hyperreflexia CSF pleocytosis Fatigue Headache, new onset Patient has nonspecific findings of CSF pleocytosis as well as clinical findings concerning for ADEM. No infectious cause for this problem; temporally associated with receipt of Janssen COVID vaccine. 4/19 Assessment as per Neurologist; Question of transverse myelitis/ADEM: Perhaps induced by the Covid vaccine On steroids MRI scan of the thoracic spine shows demyelination but no contrast enhancement Continue to work with therapy services No signs of CNS infection and antibiotics discontinued. The patient continues on methylprednisolone 500 mg iv q 12 hr at this time (4/19/2021).

Write-up: On April 2, 2021 around 8:00 p.m. I begin having chills, body aches, fever ,nausea, etc. This continued until Monday April 5, 2021, minus the chills. My urine became very dark brown for a few days, then changed to a light pink color.

Diagnostic Lab Data: The patient is a 28 y.o. male evaluated for left sided body aches and 6/10 head pain that began approximately two weeks ago following the administration of the Johnson and Johnson COVID-19 vaccine. Given the patient''s current presentation on exam and history provided low concern from vaccine complication or infection. He has normal vital signs here and his symptoms were somewhat reproducible, in particular with the tenderness of the midline T-spine and left scapula. Primarily concerned for musculoskeletal strain, likely worsened from his physical labor at work. At this time I do not feel that further emergent workup is warranted and the patient is medically stable for discharge with instructions to rest and ice the area of tenderness. He has significant change in disposition once explained that this was unlikely to be the result of vaccination. The patient was informed of all pertinent findings today. They were informed of their diagnosis and home management plan. They were informed about the importance of close follow up with their primary care doctor. Write-up: I began to feel shortness of breath when walking, tightness in my chest, for the same reason any physical effort made me feel like I was short of breath. Write-up: Day of Vaccination, fatigue 4 hours after. 12 - 13 hours after I woke up with unusual joint pain very intense in my ankles, feet and knees and a bad headache that was not like my usual headaches. the next day, I felt pretty good, I told my husband I have a leg cramp tin my left leg that does not feel like a charley horse. I ate a banana, drank lots of water and took a magnesium capsule to help with the leg cramp. and by 1pm I started to develop a headache. I did not report because I have frequent headaches. It intensified as the day progressed. after Sunday, this became a daily occurrence. Early afternoon I would start to get intense headaches and i would go home take benadryl and sinus medicine to help ease the pain. Continued on daily until Saturday April 17th when my headache became so bad I could not stand it anymore. I was nauseous and shaking. No fever but profuse sweating and weakness. Went to the ER around 930pm and was released around 12am after they gave me an IV with anti nausea meds and benadryl. Sunday I was weak with headache all day. Woke up today (Monday 4/19) with a headache and felt fatigued and severe headache. It does not feel like a sinus headache nor tension, nor hormonal. It is my entire head throbbing and left side of my head is sensitive to pressure. I have not sustained any head injuries in the past year or longer.

Write-up: 12 hours after vaccine I got chills, fever, nausea. Felt better next day with just fatigue and lightheadedness. Tuesday after the vaccine I got diarrhea with constipation along side the fatigue. Those symptoms lasted throughout Thursday. Friday night after the vaccine I got small haves around my head, neck, and upper back that would not away after an hour or two. That continued throughout Saturday and midday trough Sunday. Sunday I had shortness of breath after moving some water bottle cases around and lightheadedness along with the small hives at around 2pm I went to the ER to get checked out. Doctors said it was a reaction caused by the vaccine. They gave me Benadryl for the reaction and injected me with a "Steroid" to the my upper buttock. I don''t remember the name of what they injected with me. It is now Monday April 19th a day after the ER and I feel much better. The Benadryl made the hives go away.

Write-up: Around 5:30 pm in the evening following my vaccine I started having chills, headache, nausea, and fever of 101. Chills got worse, whole body was shaking and shivering. Nausea progressed into constant dry heaving. Head was pounding with the worst pain I have ever felt in my life, worse than natural child birth and worse than running marathons. The nausea was so bad and I could feel my fever burning and getting worse. I was about to vomit so got out of bed to go to the toilet to vomit and the whole room started spinning completely out of control. My heart was racing and I could hear it and feel it pounding rapidly through my ears. I couldn?t walk and was so disoriented I crawled to the bathroom to throw up in the toilet. My husband heard me and came and picked me up off the bathroom floor and carried me to our bed. As the night progressed fever got worse, head was in almost unbearable pain, severe body aches over my whole body, heart racing and horrible nausea and dry heaving. I made it through the night. For two days after I had a headache and fever and nausea and body aches and couldn?t get out of bed to care for my children. By the third and fourth day I was able to get out of bed at times and still felt fatigue, nausea, headache and body aches. Anytime I would stand up for 10 days after the vaccine it felt like I was rocking on a boat. I am almost at 2 weeks and still feel fatigue and dizziness at times. On April 15th, 9 days after my vaccination I had symptoms of a blood clot in my leg. I went to the Emergency Room and was diagnosed with a superficial blood clot of the leg as well as swelling in the vein. I am thankful to be alive!

Write-up: 6-48h: joint pain, fever, chills, headache, nausea, sore arm and pain around injection site, extreme pain below knees 48h+: original symptoms subsided but developed numbness and weakness left leg and left arm 56 hours: left face partially paralyzed and/or swollen to the point where couldn?t move week. Prickly feeling left skull. No concomitant medications were reported. On 02-APR-2021, at night, the patient experienced fatigue but, it was not extreme. On 03-APR-2021, the patient woke up vomiting, had difficulty breathing, fast heartbeats and felt like her body was shutting down following which her mother called the ambulance. On the same day, the patient was admitted to the emergency room (ER) of a hospital where her blood pressure was found to be very high and she was also given an intravenous (IV) as she had difficulty breathing. Write-up: Immediately after receiving vaccine, pt had complete loss of consciousness for approximately 30-45 seconds. Pt was lowered to the floor and placed in trendelenburg. BP 132/71, HR 93, 98% on RA. BP 10 minutes later with pt lying down dropped to 83/49, HR 52 and 99% on RA. Each time we attempted to have pt sit up, Pt would begin to have tingling in extremities and ringing in ears. EMS called. BP eventually raised to 127/66 with pt sitting upright but EMS preferred to transport pt to Hospital. Pt evaluated in ED and notes indicate that EKG was done and revealed normal sinus rhythm. Determined to be vasovagal reaction leading to syncope. Pt discharged to home in stable condition.
