Write-up: Three days post vax, the pt''s skin erupted in rash over most of the body with extreme itching. Treated with prednisone; which took the rash away but it returned within a few weeks. Information received from medical records state anemia, hyperlipedemia.

Write-up: An attorney reported that a 54 year old male, received an injection of Flu Shield (1998-1999 Formula) on 11/17/1998 and within 10 days, developed vomiting and diarrhea. He was seen by the MD on 11/30/1998. The reporter indicated that "it was believed at that time that he had some type of virus". On 12/6/1998, the pt was admitted to the hospital due to general paralysis; he was unable to move his hands, arms or legs. The pt was paralyzed for months and was dx''d "as suffering from Guillain-Barre Syndrome". He was transferred to a rehabilitation center on 12/22/1998 for long term rehabilitation. He was discharged on 6/30/1999 and received physical therapy at home for 4 to 6 weeks. The pt continues to be weak and disabled; he is unable to walk in a "normal fashion" and is only able to ambulate with the assistance of a "stand-up" walker for very short periods of time. The pt is only able to lift a few pounds of weight with either arm or hand. This report of a serious, labeled event is being submitted in a 15-day time frame, as requested.

Write-up: Report received from a neurologist concerning a pt who experienced peripheral neuropathy 5 days post vax. Reported date of vax and reaction start date were approximate. Case reassessed to be serious on receipt of follow-up information form the neurologist. This pt developed neuropathy and loss of coordination and reflex strength. The vaccinee was also reported as ''dysesthetic poor''. The vaccinee was treated with steroids. Symptoms were stated by the reporter to be disabling/incapacitating. No stop date was given for the symptoms. Numerous labs and tests were consistent with neuropathy.

Write-up: Optic neuritis in right eye, numbness in legs, and blurry vision in right eye. Treated with Solu-Medrol. Numbness was gone in a week but blurry vision has not changed on 5/13/00. Annual follow up states blurred vision in right eye. Per F/U 7/24/00; also treated with Methyl Prednisone. Pt also had a headache and pain with eye movement.

Write-up: Employer sent me to Health Dept for Hepatitis shot but Health Dept gave me a Flu shot, instead. I got very ill and had to spend 7 days in the hospital. The doctor''s dx is Guillain-Barre'' Syndrome (GBS). I have not recovered; lung and breathing problems.

Write-up: Spontaneous report received, 10/30/00, from the son of a pt who experienced an adverse event possibly coincidental with receipt of Fluvirin. Additional information was obtained in a phone call to son on 10/31/00. This 74 year old male received his vaccination on 10/11/00. On 10/12/00, he was noted to be unable to raise his left arm. His PCP referred him to an orthopedic physician, who dx''d atrophy of the shoulder and neck. Pt has been referred to a neurologist; appointment has not yet been confirmed.
