Write-up: Received second dose of Moderna COVID vaccine on 7/2/21. 7/3 developed fatigue, dizziness, and decreased hearing. Symptoms progressed to difficulty breathing and chest pain. 7/4 chest pain localized to left side of chest. He was seen in local ED with elevated troponin of 8154, EKG with diffuse ST segment elevation. Transferred to another ED for further evaluation. He received IVIG on 7/5 and did develop V-tach lasting about 7 beats around 0500 on 7/5 but otherwise hemodynamically stable.

Write-up: On July 10Th the patient had small rash on her abdomen thigh and down her leg. On July 13th the patient woke up and was covered from head tp toe in the rash. under her arms, behind her ears and neck and private area as well.

Write-up: PT is a 17 y.o. female who presents to the ED with complaint of chest pressure that is generalized. She states that this began about 30 minutes after receiving her second Covid vaccine. She states that she does not have any shortness of breath currently but reports a sensation of shortness of breath. She states that she has had a headache for "weeks" she states that this headache has been unrelieved with medication including ibuprofen and acetaminophen as well as allergy medication that she has taken 2 doses of.

Write-up: 8 days after vaccination, developed allergy/cold like symptoms with labored breathing. Tested for COVID on 5/28 but continued breathing difficulties have continued. Have had chest X-ray/clear, prednisone with no impact, allergy testing. Urgent care, pediatrician and allergy specialist all agree breathing isn?t related to asthma or allergies. Still continues.

Write-up: Patient developed right eye lid swelling and bilateral eye redness when he got home. Patient''s mother called to inform and brought patient back to the clinic. Patient was given Benadryl 50mg IM. Approximately 15 minutes after Benadryl administration, the patient''s right eye lid swelling went down and bilateral eye redness went away.

On 01-May-2021, the patient experienced PRODUCT ADMINISTERED TO PATIENT OF INAPPROPRIATE AGE (vaccination clinic accidentally gave her the Moderna vaccine and her daughter is 17 years old), PAIN IN EXTREMITY (arm hurt) and FATIGUE (fatigue for a few days). On 03-May-2021, the patient experienced DYSPNOEA (could not breath well in her cloth mask) and OROPHARYNGEAL PAIN (throat hurt). The patient was treated with PARACETAMOL (TYLENOL) at a dose of 1 dosage form. In May 2021, FATIGUE (fatigue for a few days) had resolved. At the time of the report, PRODUCT ADMINISTERED TO PATIENT OF INAPPROPRIATE AGE (vaccination clinic accidentally gave her the Moderna vaccine and her daughter is 17 years old), PAIN IN EXTREMITY (arm hurt), DYSPNOEA (could not breath well in her cloth mask) and OROPHARYNGEAL PAIN (throat hurt) outcome was unknown.
