Write-up: Information has been received from a 24 year old female with citric allergy who on an unspecified date was vaccinated with a single dose of MMR II . Consumer who is now 38 year old, reported that she received MMR II when she was young and then when she was 24 years old she went to the hospital for 2 days because of a stomach virus and while she was in the hospital she had some upper and lower gastrointestinal tests done and was diagnosed with Crohn''s disease. At the time of reporting the patient had not recovered. This is one of several reports received from the same source. Additional information is not expected.

Diagnostic Lab Data: LABS and DIAGNOSTICS: Sigmoidoscopy - Abnormal. Gram stain of stool - many white blood cells and several types of bacteria. WBC 10000 (H). Stool culture (+) for Salmonella pullorum. Sed Rate 66 (H). Adominal X-ray - Abnormal. Proctoscopy - Abnormal. Biopsy consistant with ulcerative colitis. Upper GI series with small bowel follow through - Normal. Lower GI series - Abnormal.

Write-up: Received the typhoid vaccine on 26 May 1982. He was seen in the clinic the following day for a reaction to typhoid with a temperature of 100.6; diagnosed with a serum reaction and ordered bed rest. He was seen for follow up and repeat CBC on 01 Jun 82 and was noted to have stomatitis. On 02 Jun his stomatitis improved with MAALOX and his CBC was returning to normal. He was seen again on 16 Jun 82 with a three day history of abdominal pain and bloody diarrhea. He returned to clinic on 17 Jun and was admitted for five days with an initial impression of Ulcerative Colitis. He was readmitted again on 02 July 82 for increased symptoms following a Barium enema. He was discharged on 20 Aug 82. During this hospitalization he was treated for toxic megacolon requiring hyperalimentation and six blood transfusions. In Nov 1984 he required a total colectomy with an ileostomy. In 1994 he was diagnosed with Crohn''s disease and currently remains under treatment. Symptoms: Diarrhea, Abdominal Pain, Fever. 8/18/09 Hospital and Medical records received DOS 5/27 to 7/29/2008. Inpatient DOS 6/17/82 to 6/22/82. Assessment: Ulcerative colitis. Patient presented with 4 day history of cramp lower abdominal pains followed by diarrhea. Blood in bowel movments. Outpatient DOS 7/2/09 to 8/20/82 Assessment: Probable Salmonellosis. Patient presents with episodes of high fever, chills, sweats. Inpatient DOS 2/7/82 to 8/20/82. Assessment: Toxic megacolon secondary to severe ulcerative colitis with universal involvement of the colon. Patient presents with 5 days of blood diarrhea, developed fever. Abdominal tender with some enlargement. TPN. Calcium and magnesium deficiency. Inpatient DOS 11/9/84 - 12/6/84. Assessment: Severe Ulcerative Colitis. Total colectomy. Ileostomy. Surgical Consult DOS 12/13/84. Assessment: Severe exacerbation of ulcerative colitis. Inpatient DOS 1/16/85 to 1/26/85. Closure of loop ileostomy. Drug sensitivity to Flagyl. Inpatient DOS 4/19/85 to 5/3/85. Assessment: Diarrhea and ulcerative colitis, chronic rectal pain, history of anemia. Laparotomy and creation of end ileostomy. Inpatient DOS 11/30/94 to 12/16/94. Assessment: Abdominal pain secondary to intraabdominal adhesions. Patient presents with intermittent episodes of bloating and right lower quadrant discomfort. Exploratory laparotomy and lysis of adhesions. Surgery and surgical pathology reports DOS 7/28/2008. Assessment Crohn''s enteritis with distal ileal stricture. Write-up: Information has been received from a physician concerning his 76 year old wife with Crohn''s disease and a history of pancreatitis with the use of PURINETHOL who on 07-JUL-2009 was vaccinated with a dose of ZOSTAVAX (Merck) (dose, route and lot number not reported). Concomitant therapy included prednisolone (5mg every other day). It was reported that 14 days after receiving the vaccine, on 23-JUL-2009, the patient had a flare of her Crohn''s disease. She had had mild diarrhea and the physician was treating her with PEPTO BISMOL. Upon rechallenge with PURINETHOL, the patient again experienced pancreatitis. At the time of reporting, the patient had not recovered from flare of her Crohn''s disease. The outcome of the pancreatitis was not reported. Upon internal review, pancreatitis was considered to be an other important medical event. Additional information has been requested.

Write-up: 11:30am-Pt was given Tdap, HPV, & Meningococcal Vaccine while sitting on exam table. Pt stated she felt "lightheaded" following injections and was instructed to lie down, but pt declined. Very shortly after the pt received immunizations, she fainted. As she was moving from sitting to lying down, she actually slumped forward. The drawer holding some books on the exam table was open. She apparently landed onto the drawer first and then rolled off the drawer landing on the floor. The pt awakened immediately upon becoming supine, as Dr. rushed into exam room upon hearing the loud noise. Initially pt denied discomfort but then did discover that she had some discomfort on the dorsal aspect of her R wrist. The pt was observed closely for quite some time and then allowed to sit up. Even when standing after 15-20 min., she was a little lightheaded. Further questioning revealed pt had no PO intake for the entire day. She was given a couple of glass of apple juice and was brought to parent vehicle via wheelchair. Pt mother was instructed to push fluids for remainder of the day and continue to monitor.

Write-up: Information has been received from a physician concerning a 21 year old female who in July 2007, was vaccinated with a first dose of GARDASIL. Last week the patient was diagnosed with Crohn''s disease. The patient was placed on an unspecified dose of steroids. The patient sought unspecified medical attention. Follow up information received on 21-JUL-2009 from a registered nurse indicated that on 30-JUL-2007 the patient was vaccinated with the first dose of GARDASIL (lot # 657737/0522U). The patient had not received the second or third dose of GARDASIL. The patient was recently diagnosed with Crohn''s disease. Follow up information was received from a certified medical assistant and a physician who indicated that on 30-JUL-2007 the female student was vaccinated with the first dose of GARDASIL (lot # 657737/0522U) in the left deltoid at 11:00 am. The patient developed Crohn''s disease in 2009. The patient had a family history. The reporters wanted to know if the patient could got the second dose of GARDASIL while on treatment for the Crohn''s disease, which was immunosuppressive. It was reported that there was no adverse symptoms from GARDASIL. Additional information is not expected.

Write-up: Initial report received from the patient''s parent on 10 January 2011. This case is not medically confirmed. A 10 year-old male patient (also reported as a 17 year-old) with unknown medical history had received on an unknown date a dose of MENOMUNE (lot not reported), and a dose of Cholera, and Tetanus Diphtheria vaccines (trade names, manufacturer names and lot numbers also not reported) in preparation for travel out of the country. Two months after receiving the vaccines while the patient was overseas, he developed bloody diarrhea and was treated with antibiotics. The patient returned home (date not reported), and a year after his return he again experienced bloody diarrhea and had wheezing and shortness of breath. It was reported that about a year after receiving the vaccinations the patient was diagnosed with Crohn''s disease. Recovery status for the wheezing, shortness of breath and bloody diarrhea were unknown. The patient has not recovered from the Crohn''s disease. Documents held by sender: None.

Write-up: Information has been received from a licensed practical nurse concerning an approximately 11 year old female patient with irritable bowel syndrome and no drug reactions or allergies who on 06-MAY-2004, was vaccinated with the first dose of VARIVAX (Merck) (Lot# not reported, expire date 27-OCT-2010)). On 31-AUG-2010, the patient was vaccinated with the second dose of VARIVAX (Merck) (Lot# not reported). On an unspecified date, the patient was diagnosed with Crohn''s disease after receiving VARIVAX (Merck). There was not concomitant medication. It was also reported that the patient''s varicella titer was found to be negative. At the time of the report, the patient had not recovered. The patient sought unspecified medical attention. On follow up information received on 22-NOV-2010, the medical assistant reported that the patient''s Crohn''s Disease was unrelated to the dose of VARIVAX (Merck). Follow up information has been received from the medical assistant concerning the female patient who on 06-MAY-2004, was vaccinated with the first dose of VARIVAX (Merck) (Lot# 647259/1137N). On 31-AUG-2010, the patient was vaccinated with the second dose of VARIVAX (Merck) (Lot# 662450/1385X). It was reported that there was no adverse reaction. The GI doctor wanted to know if having Crohn''s disease was going to effect the VARIVAX (Merck) from working. Patient had Crohn''s had nothing to do with VARIVAX (Merck). GI doctor did not feel Crohn''s was from the VARIVAX (Merck). No further information is available.
