Write-up: Day 10 post first shot I developed a small red swollen area near injection site that was accompanied by slight tingling in my fingers in the same arm (this was bearable). However, I also developed a petechial rash at the same time on upper back which spread to lower back and had a low fever of 100.5-101 and felt unable to function. I did a video visit with a doctor who confirmed it was a petechial rash, she prescribed antihistamine and a topical steroid cream. The rash continued along with fever for 3 days, it spread slightly to my stomach area. By day 4 it began resolving and I no longer had a fever. Prior to this reaction occurring on day 10, I had typical symptoms of sore arm and swollen lymph node.

Write-up: Patient daughter notified me that patient started Stivarga on 2/25/2021 (appx). She states that on 2/26/2021 her father started having bloody diarrhea. She states he was "more fatigued than normal" leading up to this event. Labs were drawn and it yielded throbocytapenia. She states the oncologists states he does not believe it was from the medication but feels it was the vaccine that produced this. The patient is in the hospital receiving blood and platelet transfusions.

Current Illness: had local reaction with arm redness and swelling after first Moderna vaccine on 12/30--that occurred about 9 days after vaccination and resolved on its own. Had febrile reaction after second dose Moderna on 1/28 with temp 103.4, body aches, chills and fatigue. Developed arm swelling and tightness the week of 2/22 and was evaluated in the ED 2/26/2021 and found to have axillary vein thrombosis
Preexisting Conditions: distant history of papillary thyroid cancer 2011, no evidence of recurrence, had blood work and neck US in January before this incident
Allergies: cephalexin and penicillin
Diagnostic Lab Data: Ultrasound doppler of the Right arm/axilla, CT angiogram of the chest, routine labs in the ED, then hypercoagulable work up and referral to hematology

Write-up: developed fever and chills within 24 hours of vaccine and at 4 weeks out had unprovoked acute DVT in the axillary and subclavian vein on the side of the vaccine.
