Write-up: Post vax, the next morning (12/16/01) the pt had a cramp in left shoulder with pain. He had flu-like symptoms and left extremity pain on 12/20/01. Leg were weak and had difficulty walking. On 12/23/01, his legs gave out and he fell to the ground. Went to ER and evaluated by MD who conducted nerve conduction velocity: finding consistent with Guillain-Barre Syndrome. He was started on IVIG on 12/23/01 and also on 12/24/01, 12/25/01 and 12/26/01. The pt was not improving so he was transferred to another facility on 12/27/01. On 1/2/02, dx''d with GBS; not moving anything at all but can open eyes. He will be on prolonged ventilation support.

Write-up: The pt''s left arm experienced sensitivity after flu shot was administered and have received 2-3 months of physical therapy and has still not regained all sensitivity. The doctor felt a nerve or tendon was hit.

Write-up: 3 days post vax, the pt developed progressive bilateral leg weakness, fever and cough. 5 days post vax, she developed paraplegia and sensory impairment to the T6-T7 level (pin prick, light touch, temperature). The pt was treated with IV steroids and discharged to a long-term care facility without any improvement on 12/11/01. Pt with PHH sign for MS and an episode of acute transverse myelitis and paraplegia.

Write-up: 1 week after flu shot the pt had complaints of severe pain in both legs and feet. Also started to have painful nodules on legs and bottom of feet. Had flu shot for the last 2 years; administered by another co. Presently on valium, prednisone and pain med. Pt was hospitalized for 4 days from another facility. Annual follow up states pt had painful nodules that developed on his legs. "Took 2 months to go away." He did not take a flu shot in 2002

Write-up: A report was rec''d from a hlth professional on 1/30/02 regarding a female pt who experienced injection site reaction, acute viral illness and arm weakness which was considered disabling after receiving a Fluvirin vaccine in 10/01. The pt has had a previous influenza vaccination (brand not specified). In 10/01 the pt rec''d a Fluvirin vaccine and was on an unknown date experienced painful arm at injection site which resolved. Two months later the pt had an acute viral illness which included swollen glands. The pt''s arm also became swollen and painful, her arm still remains painful especially around the site of vaccination. The pt feels her arm is weaker and is receiving physical therapy treatment. The pt had not recovered at the time of reporting the event. the reporter did not provide a causality assessment. Follow up 01/20/05: A follow up report was received on 04/16/02 stating that the patient, a 50 year ol female who had received Fluvirin on 11/02/01, had recovered from her general lack of wellbing, but was stil experiencing should pain in the left shoulder, which the reporter felt may be possible tendonitis, arm pain-tender over deltoid and bicept tendon, and exhibited a decreased range of movement. The patient was being trated with physiotherapy and NSAIDS. The painful swollen arm from the injection site reaction was still present. The reactions were still painful, although improved, on 04/10/02. The reporter has assesd the relationship of Fluvirin to the reactions as possible.

Write-up: The patient states that, "I have experienced persistent bilateral foot problem of unclear etiology since Feb of 2000. I do not recall the exact date of my flu shot, but I believe that I received a flu vaccine, through my employee health office, about 1-3 weeks prior to the onset of my symptoms. I have attempted to obtain the exact date of inoculation through my employer. However, the employee health and medical records staff are unable to provide me with documentation re:date of my flu shot. I have seen many physicians/specialists and have undergone many diagnostic tests in the past 2 years. Many diseases, such as multiple sclerosis, several types of neuropathies, Lyme diseases, autoimmune diseases, etc.., have been ruled out through diagnostic tests, including foot/ankle/brain/spine MRIs, x-rays, and blood tests. I still do not have a clear diagnosis. Many physicians have suggested that my symptoms and subsequent physical limitations may be related to a virus, but it is only in the recent month that a "new" neurologist has recommended that I formally file a VAERS form. My symptoms are localized to the bottoms of both of my feet. Prior to pain management with medication, I experienced extremely severe hot knife-stabbing pain, which restricted my ability to tolerate standing (even for 5 mins) or walking. I also experienced paresthesias/numbness even when I was not weighbearing. I also lost motor function of 2 of my toes and developed ankle weakness. I have been severely limited in function since onset of these symptoms. I was essentially housebond the first 3 months of these symptoms. I was gradually able to tolerate more walking once I started taking neurontin and with the use of crutches (which allowed me to walk maybe 200 ft before my symptoms would increase). After about one year, I was finally able to walk very short distances (and not on a daily basis) with a cane. Presently, my walking and standing tolerance is still very limited as compared to my previously active lifestyle. Because of unclear etiology of my symptoms, I have essentially been "medically managed" with medication. Throughout the last 2 years, I have continued to make very slow progress in terms of increased walking/standing tolerance, and my physicians are unable to offer clear explanations for the onset of symptoms and my "natural" progress in the absence of a clear diagnosis and treatment plan. As mentioned above, some of these physician have suggested a "viral" cause. I have attempted to provide you with a brief description of my situation." per doc #213154, 60 day follow-up, incomplete, but continuing recovery from foot pain, loss of left 4th and 5th toes, ankle weakness.

Write-up: A 10 year old boy presented with a complaint of not hearing from left ear. He failed a hearing screen on that side (right is normal) and had a normal exam. ENT referral obtained and dx''d with sudden left sensorineural hearing loss. He had improvement with oral steroids but has residual drop in highest frequencies between 2000 to 8000 Hz. He has partial recovery. F/UP report on 3/21/03: By phone conversation w/PT''s mother (He has transferred out of reporting office''s practice) he has regained "90%" of his hearing and does have some tinnitus at times.
