Write-up: On 11/20/01, the pt exhibited brachial plexus palsy, 12 days after receiving the Flurvirin vaccine. The pt had received previous Influenza vaccinations but it is unknown if Fluvirin was administered. It is unknown if this developed on the same arm as the site of the Fluvirin injection. At the time of this report, the pt had not recovered. The reporter assessed the relationship of the study medication to the event as possible. Following internal medical review on 2/20/02, this case was upgraded to "Serious". OMIC

Write-up: The pt had quarter-sized raised area of erythema at injection site about 1 hours, post vax, according to pt. She had a headache and occipital visual loss and congestion. MRI on 11/24/99 is consistent with ADEM. Treated with steroids. This is an addendum to the original form that I sent in on this pt on 5/16/01. It has been brought to my attention that the vaccine lot number (10163AA) written by me, may have been a misinterpretation of the actual number. The injection was given at another healthcare facility and I have been unable to talk to the nurse who actually gave the injection, to determine which of these numbers is correct. Discharge summary states brain lesion.

Write-up: The 54.5 year old male pt developed vertigo and vomiting. After brain tumor surgery 8 week coma with meningitis, stroke sepsis, respiratory and kidney failure, permanent residuals-gait ataxia, peripheral nerve damage, bronchiectasis with ehmophysis episodes, speech and swallowing difficulties, hypotension after meals, hearing and vision damage. Per follow up letter pt did not recover. Galt ataxia, chronic bronchitis, partial swallowing paralysis. Very disapponited in how my original report was handled. The interviewer was either blindly bias or severly handicapped by lack of training as an interviewer. This is being made just in case the VAERS program is not the sham it appears to be. From top to bottom, CDC and HHS should be overlooked so as not to appear to be anothe PAC for the pharmaceutical industry.

Write-up: On 10/30/01, the pt received Fluvirin into his left upper arm and 3 day later, on 11/2/01, the pt experienced an inability to move his right arm and right leg for 70 minutes. The pt''s right arm and leg went numb also. The reporter indicated that the reaction involved persistent or significant disability or incapacity, due to the numbness and paralysis of the right arm and leg. The pt recovered on the same day. Write-up: A report was received from a regulatory authority on 3/6/02, concerning a 25 year old male pt who had been enrolled in a (non-company clinical trial) who experienced vertigo, which was considered disabling after receiving a Fluvirin vaccine on 10/20/95 which (was not part of the clinical trial). The pt (a helicopter pilot) had received adsorbed tetanus-diphtheria vaccine (reduced dose) and a poliomyelitis booster on 9/26/95. Since 2/01, the pt had experienced paroxymal vertigo approx. once a week, lasting between 30 seconds and a minute. A full exam was performed which was normal, including ENT/neurological and psychiatric assessment and MRI evoked potentials. Further investigations are planned to rule out migraine. At the time of reporting, the pt had not yet recovered. There was insufficient information to determine the reporter''s causality assessment.

Write-up: Pt developed complete paralysis and she was hospitalized for a month with physical therapy and one month in a nursing home for paralysis treatment. The discharge summary states GBS, and congestive heart failure. Annual follow up states right and left foot totally numb up to the ankle. No individual toe movement on either foot. Unable to walk properly. Contracted Guillian-Barre Syndrome after receiving the flu shot and has the above symptoms since then.

Write-up: The pt contracted Guillain Barr? syndrome and was treated for three weeks at the hospital. Has been in full rehab facilities since that time. Pt is not able to walk or sit up. On the annual report received on 04/22/2003, the reporter states, "Patient is still unable to walk unassisted (uses walker). Needs assistance to dress, undress, toilet and food prep. Is homebound (90%). Has spent all savings on aides and can no longer afford them. Severe arthritis in right arm, hand, knee and ankle. Suffered hair loss due to stress."

Write-up: In 10/01, the pt received Fluvirin, after which, the pt experienced a painful, burning sensation and edema in the injected arm, finding it difficult to sleep on that side. The pt then received physiotherapy and an injection of betamethasone. At the time of reporting, it was not known whether or not the pt had recovered. The reporter has assessed the relationship of Flurvirin to the event as possible.
