Write-up: Red patchy face, felt like a sunburn later severe headache, chills, fever, rapid heartbeat, stopped14 hours later, slight headache 72 hours later left eye pain, 84 hours later left eye swelling, went to PCP. Put on antibiotics and eye drops returned to PCP no improvement. The swelling got worse on Thursday, April 15 was admitted to hospital for orbital cellulitis. Lab tests, CT scan and recieved iv antibiotics and steroids. Discharged diagnosed as periorbital cellulitis on Saturday April 17 was put on oral antibiotics and eyedrops.

Write-up: After one week I suffered a TIA and have to be hospitalized From April 11 to April 15. After I was discharged I went back to the hospital April 18 with a second TIA. I was hospitalized from the 18 to the 21. Both times at same facility.

Write-up: Pt did not voice she received Vaccine (Janssen) in county jail (04/08/21) prior to coming to Prison. pt was asked if she had a vaciine within the past 14 days and she denied. Pt received Mondera vaccine @ 1222 and a few minutes after voiced tightness in her throat @1228, at this time the pt inmate voiced she received the Janssen injction. Pt. was administered epinephrine 0.3mg IM as ordered by unit MD, approx. 30 seconds after injection pt. voiced issue had resolved - pt was sent to local ER via 911 call/ ambulance.

Write-up: Patient had known fear of needles and despite mitigation strategies, once he knew the vaccine was administered, he began to feel weak and look pale. He lost consciousness twice but only for a few seconds and then he "came to." I helped along with the mother to lay him down to prevent injury. Patient was alert, breathing, and responding, but still appeared pale, "clamy" shaky and sweaty. Code was followed with store management responding, ambulance arrived to take care of patient within minutes.

Diagnostic Lab Data: April 13, 2021 - At ER - CT scan; EKG; bloodwork up and they kept me overnight. The doctor said she found some kidney issues that she wanted me to follow up with the doctor. I hadn''t had kidney issues before. Everything else was normal result.

Write-up: Immediately that evening, I started to get headachy and nauseous. My arm in the injection site was very sore. The next morning, the other arm was sore. It was difficult for me to raise my arms above my head without feeling extremely weak. I couldn''t lift anything above my head for the fear I would drop them. Knee replacement surgery on both knees about 4 years ago - I felt pain in my knees; all my joints; all of my teeth; my vision was blurry. My head hurt so badly. Tylenol dulled the headache and joint pain but it didn''t help it to go away. My chest also hurt quite a bit. I felt extremely lethargic. I didn''t feel like getting out of bed. I could only get the meals made (I''m a caregiver for my mom and mother-in-law) and then get back into bed. I usually walk five miles a day. I''m still not there yet ( to walk that much) but I''m getting better. The nausea lasted until the 17th. The headache just left finally on April 23. The body aches, teeth ache and joints subsided around the 17th also. The blurry vision - I still have a a little bit of that but it''s starting to improve. The weakness in the arm - that is starting to improve - I do still feel kind of weak when I raise my arms above my head. The fatigue I still have but I''m feeling a bit of improvement. I have to keep checking my bed to see if I accidentally put the massage setting on (and I hadn''t) because my body was shaking in entire body- That started on the second day after the vaccine and stopped about the 19th. I called to the advice nurse to register my health complaint and she had me come in for a full workup. And she asked me to go to ER. They kept me in the ER overnight.

Write-up: Vaccine was received at 11:30 am, 4/27/21. Very minor headache started a few hours later. Fatigue, aches and chills started around 6pm. At 8pm, temp was normal at 97.7. Around 11pm, while sleeping, heart rate increased to above 100, ranging from 105-140. At 12:30am, after no decrease in heart rate, I went to ER. After receiving 2 bags of IV fluids and multiple tests later, heart rate was down to around 105 and I was discharged at 2am. Heart rate returned to normal (about 61 for me) around 3pm on 4/28/21

Write-up: I had 3 SEVERE epistaxis episodes 4/16-18/2021 requiring ER admissions. #1-4/16 resolved nose bleeding with nasal clamp, #2-4/17 controlled with nasal tampons and 1 unit of platelets, #3-4/17-18 ENT placed bilateral nasal packing. The packing was removed on 4/23 with no more bleeding so far. I''ve NEVER had nose bleeds! I was diagnosed with storage pool disease in 1992 which was reconfirmed in 2004. I had excessive bleeding during child birth in the late 80''s. I bruise easily and need medication or platelets with major surgery or trauma. No issues with dental or minor procedures. Next morning on 07-APR-2021, patient woke up with sever cramping in same shoulder which he got the shot in, most pain was in the upper left quarter of his torso to down half way to chest up to the shoulder till the neck . By the end of the day it spread to his whole chest and upper body. Also he had shortness of breath ,could not breath .The cramping and shortness of breath came at same time. He took Advil and went along with his day . He took 3 tablet of Advil 200mg every 4-5 hours all day ( total of 3 dose that day) . By night time the case was progressively worse as the whole chest had a lot of pain . He thought that he had heart attack . He thought that he was about to pass out . He could not stand on his own . So his wife took him to emergency room at mid night at 12:30 am on 08-APR-2021. He stayed there for 5 hours and they did bunch of tests , EKG which rolled out the heart attack .They did Chest X-ray which found things then did D-dimer which its level was very high . They found bunch of clots in the bottom sides of both lungs and on the top of one of the sides. They did ultra sound on the legs which had no evidence of blood clots . They discharged him from the emergency room as they felt that he was ok to be out . After couple of hours after discharge , patient''s health care professional wrote for him Eliquis, antibiotic and pain reliver. He was given antibiotic because he had beginning stages of pneumonia on both lobes of the lungs (collapsed) . It was found air sacs in lung with signs of pneumonia in both lobes . He came home and started to monitor his blood oxygen . He stayed in the same position till recovery . He was better each day since last week but still had sinus and cramping in the chest ( very minor but was still there). The lung capacity was very limited to take some steps. Patient stated that he needed a specialist to understand his condition. He had no medical history before that incidence . He was wondering regarding which type of blood thinner was good for this type of blood clots because he read that the regular blood thinners are not good for that type of blood clot which his HCP gave (regular blood thinner). Laboratory data included: Total lung capacity decreased (NR: not provided) lung capacity is very limited. The symptoms reported were severe cramping of arm that spread to his whole chest and upper body and shortness of breath; the patient thought he had a heart attack, about to pass out and could not stand because of the progression of pain. EKG done at the emergency room ruled out heat attack, D-Dimer was reported to be very high, clots were found in the bottom sides of both lungs and on the top of one of the sides, x-ray showed beginning stages of pneumonia on both lobes of the lungs (collapsed). The patient was prescribed with Eliquis, an unspecified antibiotic and pain reliever. Outcome of the events were not reported. The patient had no known allergies. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, intramuscular, batch number: 1805029, and expiry: UNKNOWN) dose was not reported, 1 total administered on 16-MAR-2021 for prophylactic vaccination. Concomitant medications included Centrum Multi-Gummies for Women. On an unspecified date in MAR-2021 the patient began to experience slowly progressive shortness of breath; on 01-APR-2021 she had left upper quadrant pain. On 03-APR-2021 the patient presented to the emergency room. Laboratory data included: Pulmonary Angiogram/CT Chest - abnormal and computerised tomogram abdomen/pelvis at 09:37, showed segmental and subsegmental pulmonary emboli in the lower lobes of both lungs. Small volume perisplenic hemorrhage and small volume pneumoperitoneum in the pelvis or findings concerning for spontaneous splenic rupture and moderately enlarged spleen. Additional CT abdomen and pelvis with IV contrast at 23:05 showed: Small to moderate amount of hemoperitoneum, slightly increased since the earlier study. There is definitely new blood adjacent to the liver. The hemorrhage adjacent to the spleen appears similar to the earlier study. Due to the timing of the scans with relation to the contrast injection, the physician could not confirm a splenic laceration, although it was suspected to be the source of hemorrhage. Small amount of mesenteric blood was slightly increased. Continuous heparin infusion was started for bilateral pulmonary embolism. On 04-APR-2021, an inferior vena cava filter was placed and patient was vaccinated with Acthib (HIB Vaccine), Bexsero (meningococcal vaccine) and Prevnar (pneumococcal vaccine) in anticipation of splenectomy. Concurrent conditions included irregular menstruation, schizoaffective disorder, panic attacks, anxiety attacks, depression, incontinence, gastritis, constipation, sciatica nerve pain, asthma, obese, and doesn''t comprehend well. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 1802070 expiry: UNKNOWN) dose was not reported, 1 in total administered on 06-MAR-2021 at left arm for prophylactic vaccination. Non-Company suspect included ethinylestradiol/ferrous fumarate/norethisterone acetate, "for two months" for irregular menstruation. On 17-MAR-2021, the patient''s physician ordered the patient to stop taking. Concomitant medications included salbutamol, lactulose for constipation, clozapine, gabapentin (noted as the doctor was trying to wean patient off medication), oxybutynin hydrochloride, pantoprazole, risperidone, valproate semisodium, vilazodone hydrochloride, and unspecified breathing treatments. No concomitant medications were reported. On10-APR-2021, the same day after vaccination the patient was doing fine. On 11-APR-2021, the next day after vaccination started to not feel so great and got worse as the day went on. The patient could barely reach her husband after shower and felt like she was going to pass out, heart racing, felt like pins and needles (tingling) in mouth and head felt very swollen, hard to breath, had a headache, night sweating (2-3 times per night) and weight loss (lost 16 pounds). The patient''s husband called the emergency services almost 24 hours post vaccination. The patient did not feel herself and was sweating. The patient''s heart was checked and was normal. The patient looked pale but decided to stay home. The same night remained on and off and patient felt of having a heart attack and her neck felt funny. On 12-APR-2021, the patient did not felt well and did not eat much, but was not as bad as the previous day even though it would come every 3-4 hours. On 13-APR-2021 at 03:00 AM in the morning, the patient experienced vomiting, and throwing up bile, diarrhea until 08:30 or 09:00 AM. On the same day the patient went to the hospital and had to spend most of the time in out-patients section with ongoing vomiting. The patient administered Zofran (ondansetron) and then experienced a worsening headache. The patient experienced high blood pressure on the same day. The patient''s laboratory data included blood test and computerized tomogram (CT) brain scan without contrast which was normal and was given intravenous morphine. The patient was sent home with prescription for Zofran (ondansetron) 4 milligram and Butalbutal/acetamin/caffeine tablets and came home on the same evening by 4/5 PM. The patient experienced on and off headache and fell asleep at 03:00 AM and on 14-APR-2021 she woke up with diarrhea and started to experience nausea and vomiting. The patient''s diarrhea resolved on 14-APR-2021 or 15 APR-2021, but she her nausea persisted. On an unspecified date in APR-2021, the patient experienced bruising everywhere, ran into wall one night and fell out of bed. On 15-APR-2021, the patient experienced tremors in legs (currently worsening) and arms (new onset) and had weird dreams. On 22-APR-2021, the patient experienced equilibrium off and ringing in ears and her blood pressure as normal. On 23-APR-2021, the patient again experienced ringing in ears. Write-up: Started experiencing extreme chest pains (stabbing feeling) between my lower right breast and upper rib cage on Saturday afternoon, especially when trying to take a big inhale. I went to the emergency room the next day after pain meds (ibprofen) did not help, and they did an EKG and X-Ray and they said it was a muscle strain. I went home and alternated between 3000 MGs of Tylenol every 24 hours, 2400 MGs of Ibuprofen, 8 hours of 4% lidocaine patch and then off for 8 hours + and a heating pad when the patch was off. Write-up: I had to go to the emergency room, high d-dimer (.95), elevated Pro B Type Natriuretic Peptide (152 pg/mL) ---Days 1-2 (12-22hrs post-vaccine) were horrific - fever, body aches, extreme pain everywhere, my eyes hurt so bad I thought I''d lose my vision, I was in so much pain that I couldn''t talk to ask for help. Write-up: a few days after the shot i was having a hard time breathing, made appointment with Pulmanologist he ordered a ct scan but no ultra sound. ct scan showed no clots in the lungs at that time. Woke up a day or two later with severe back and leg pain was transported to the emergency room. I was then transported to facillity because i could not move my left leg and was in severe pain. During my stay i coughed up a blood and requested the doctor to do blood work. The blood work showed high levels of clots. I was not taken to the hospital or treatment for the clots at that time. Write-up: starting have shortness of breath/difficulty breathing and back and chest pain. light headed, pain unbearable. Saturday April 23rd Went to er was treated for kidney stones but they could only find one small one, Was sent home with pain medication. Shortness of breath persisted as did bloody cough and pain in chest and all over. Write-up: The first night I got the vaccine, and I had a fever of 103 degrees for about six hours and 101 degrees at the 24 hours. I had all over body aches for two days but my hands still ache after tree weeks. I woke up in the morning an noticed a big bulging blue vein in my right eyelid right above the outer part of my eye. One week later my thumb started to throb and then a big bulging blue vein in my left thumb. It felt like it had been injured but it looked better within hours. I called my doctor and he advised that I go to the ER. I also had trouble with my head throbbing for a week after the shot. My eye was painful and lasted a little over a week and my eyes were a little blurry. Both problems have resolved but I still have an ache in my hands after three and a half weeks. It is a deep bone ache which makes it difficult to work with my hands.

Write-up: I started with fever, vomiting, bodily pain, bone/joint pain and head pain the early morning after my J&J vaccine which these symptoms lasted two days. Write-up: I started to have an allergic reaction to my face after my vaccine. I noticed it at night that my face was getting swollen. Write-up: 48yoM (h/o childhood asthma, GERD, lipomas, preDM, obesity BMI 30) who had Covid J&J vaccine 3/12/21, then 4/4/21 had dizziness and a little SOB, seen in ER with neg brain MRI/MRA and trop/EKG neg. and labs. Later had DDIMER 560 on 4/15/21 due to h/o J&J vaccine and CTA lung 4/29/2021 shows Right lower lobe pulmonary emboli. Patient is clinically much better. No more SOB. Pt read about some blood thinner making clot worse and doesn''t want to to start a blood thinner without checking with hematology first. Write-up: At three AM I woke up with the room spinning, I couldn''t focus, felt like I was falling. I had horrible pain in my back. I was sweating and vomiting. My husband carried me to the car and brought me to the ER. At the Er I was given a patch for my back pain, medicine for the dizziness and nausea. The dizziness didn''t subside. I couldn''t walk. Write-up: On and off headaches since vaccine. 4/28 right calf pain/ cramping, 4/29 11am quick onset severe pressure headache followed by left side face, arm, and leg numbness/tingling/weakness. Within 1 or 2 minutes heart felt like it was racing combined with body shaking/shivering. Ambulance to the hospital by 11:45am, and she was treated with tPA Alteplese after a CATscan came back negative. Symptoms improved over a few hours. I am writing this 23 hours after the ambulance transport and patient continues to have outside and inside mouth numbness and throat numbness all on left side. Blood work hass been normal except low potassium, leg ultrasounds negative. MRI results pending. Patient is still hospitalized as of 10am on 4/30.

Write-up: The first 2 days had awful headache and muscle aches, could not leave the bed. Then a week later, I got a horrible rash all of my body, worse at my hands and feet. The rash included many blisters. Loss of appetite and cannot sleep. I still have the rash now, and it is even getting worse in some parts, while other parts are better due to a steroid cream prescribed by my dermatologist. On April 26, I felt very dizzy and weak and went to urgent care. I have since recovered from those symptoms.

Write-up: Woke up April 25, 2021 at 9:00 am with left shoulder pain. Throughout the day the pain became intense and unbearable. It had also swelled considerably. I could not lift my arm at that point. At approximately 10:00 pm I went to the hospital. I was given an Ultrasound on left arm and shoulder to check for blood clots which was negative. I was also given an Xray on left shoulder to check for bone abnormalities which was negative. I was diagnosed with "COVID shoulder", released and given pain medication. Was told to follow up with family practitioner. Went to Dr. on Tuesday, April 27, 2021 who confirmed the diagnosis. Blood was taken which showed elevated white blood cells.

Diagnostic Lab Data: I went to urgent care 4/12 in which I was put on prednisone. I was at The hospital 4/23 and had ultrasound done for blood clot in left leg because of leg swelling. I was givin a cream for rash. I was at my doctors office for follow up with doctor in which I was checked over an now they are sending me to skin doctor for rash an welts caused from vaccine. Write-up: Injection given at 2PM I had neck pain and headache at 9PM that evening. At 3am woke up with chills, full muscle aches, headache and felt flu like. At 9am following morning still a very bad headache along with chills and extreme fatigue. By noon headache gone but fatigue an chills continued for several hours. Exactly 3 days later I developed an awful rash started on hands. This rash is extremely itchy an painful at times. The rash is on hands, arms, legs, belly, neck an back pretty much everywhere. Write-up: Patient developed cold symptoms with heavy drainage on 4/15/21. Patient took a whole package as directed but continued to get worse. Used Vicks Vapor Rub and took a couple doses of sinus medication after the Coriidin was gone. Started severely hurting with difficulty breathing and chest pain on 4/27/21. Went to The Hospital on 4/28/21. Was diagnosed with pneumonia, pulmonary embolism in left lower lobe pulmonary arterial branches. Patient was admitted. Patient placed on Rocephin 1 gram IVPB daily and azithromycin 500 mg IVPB daily. Patient was started on therapeutic dosing of Lovenox and then transitioned to Eliquis 10 mg BID for 7 days then 5 mg BID. See hos.

Current Illness: UTI 3/9/21 Ciprofloxacin 500 mg 1 twice daily for 6 days. Vitamin D insufficiency 3/9/21 ?vitamin D 3 125 mcg 1 p.o. daily. Anxiety- No medication issued.
Preexisting Conditions: Generalized anxiety, multiple sclerosis, persistent anxiety, spinal stenosis of the cervical foramina, overweight, status post bariatric surgery, adjustment disorder with anxiety, hyperlipidemia.
Allergies: Latex and morphine derivatives.
Diagnostic Lab Data: CBC, CMP, C-reactive protein, erythrocyte sedimentation rate That were drawn on 4/15/21 at 12:19 PMwere all within normal limits. Patient was notified of the results but stated that she was still not feeling well so she was instructed to proceed to the emergency room and did so at 17:46 on April 15, 2021. She was seen and evaluated by emergency room personnel. Given epinephrine 0.3 mg IM x1, dexamethasone 8 mg IV x1 diphenhydramine 25 mg IV x1 and famotidine 20 mg IV x1. She was diagnosed with drug rash and angioedema. She was instructed to have close follow-up with her PCP. The patient has not returned to clinic or called expressing any additional concerns.

Write-up: The patient began to notice that her face/eye was swollen/finger tingling. She then proceeded to have an itchy and scratchy and burning sensation. Her fever increased to 101.6. During her telehealth evaluation on April 14, 2021 8 macular papular rash that was not deemed to be proper was noted on the lower extremities. They were nonblanchable and no pain was elicited with palpation by the patient under my direction.

Write-up: A few days after the vaccine my leg became complete cramped which caused me to go to the ER where several testwere performed and a vascular X-ray was also performed where it revealed a DVT was present in my lower left leg. I am currently taking Eliquis and various additional testing has been done to see why this has occurred but are pending the results. I still have edema and the minor cramping in my leg and foot also.
