Write-up: It was reported that a 45 year old female pt received a Fluzone vaccination administered on 10/28/2001 two possible lot numbers were provided U0445AA or U07008AA. The pt reported experiencing flu-like symptoms in January 2002. Physician saw her; numerous tests done-possible diagnosis of chronic fatigue syndromes. Symptoms are unresolved. From follow-up report returned by the pt on 08/02/02, it was reported that she received a dose of Fluzone on 10/28/01 at a walk-in clinic. On 01/23/02, she experienced shortness of breath, fever, sore throat, swollen glands, chest pain, muscle aches, joint pain, jaw pain, and extreme fatigue. Pt also requested a copy of the report to be sent to her physician. Upon receipt of this info this report was upgraded from non-serious to 15 day. A 15-day follow up report received 09/30/2003 adds: From additional info received on 09/15/2003 from an office administrator, the reporting physician''s new address and fax number were provided. No additional relevant info was obtained. Adverse event coding was updated, and the provisional diagnosis (not confirmed) - Chronic Fatigue Syndrome - was coded. Follow up on 11/05/2003: "Case closure, on 10/27/2003. A sixth attempt for follow up information was made on 09/15/2003, at which time the responsible physician was contacted via fax. No response has been received. No further information is anticipated, therefore, this case is closed."

Write-up: Report from pt''s husband. Pt experienced joint pains (knees, ankles, and elbows) after receiving Fluvirin (for the first time). Pains reported as often quite severe. Pt''s husband also experienced adverse event after receiving Fluvirin. Case linked to MPU2000-00086. Follow up above case received from a practice nurse. Pt (female 69 years old, height: 155cm, weight: 62Kg) experienced pains in ankles, knees, shoulders, and wrists (onset three to four weeks), cold sore inside nose and clicking of all joints after receiving (intramuscular dose) of Fluvirin. Pains unresolved. Outcome of other AE''s unknown. Medication to treat AE: paracetamol and Ibruprofen spray. X-ray taken of knees and ankles. Medical history of painful spondylosis reported to be ongoing. Pt has not received previous vaccinations for flu. Further info received from the pt''s husband stated that the pt continues to suffer acutely with pains in her ankles. She has consulted a rheeumatologist, who prescribed physiotherapy and a course of drugs (details not provided), and considered that the symptoms were consistent with a reaction to the vaccine. The reporter stated that the pain has reduced but has not gone completely, and that catching a cold still brings on the same symptoms. A follow-up report from a health professional was received on 8/19/02. The 69 year old female pt experienced pains in ankles, knees, shoulders, and wrists, clicking joints and had a cold sore in the nose, which was considered to be incapacitating three to four weeks after receiving a Fluvirin vaccine on 10/19/99. The pt received betnovate cream concomitantly. The pt had a medical history of spondylosis. The pt was diagnosed with osteoarthritic change in both knee joints on 2/8/00. The pt had not been given a flu-vaccination before. Pt also treated with oral ibuprofen since November 1999. At the time of reporting it was unknown if the pt had recovered. The reporter has assessed the relation of study medication to the event as probable.

Write-up: Non medically substantiated report received from a male pt, who experienced pains in joints (knees, ankles and elbows) after receiving Fluvirin. Pain reported as often being quite severe. Pt''s wife also experienced an adverse event after receiving Fluvirin. This case is linked to MPU-2000-00089. No further details reported. Follow up report received from doctor. Male pt (DCT, 60 years old, 77Kg, 175cm) experienced pain in joints-ankles, wrists, knees, shoulders (onset 3-4 weeks), cold sores inside nose and clicking of all joints after receiving single dose of Fluvirin IM. No previous flu vaccinations. Pain in joints not resolved. Outcome of other AE''s unknown. AE reported to be incapacitating. No cattarrh-no cold/flu symptoms as reported. Medical history insignificant. No concomitant medication. Causal relationship as per reporter is probable. Further follow up received from pt. Pt reports that the pains he has been experiencing have eased considerably, though some tenderness is still present in the ankles. A follow-up report from a health professional was received on 8/19/02. The 60 year old male pt experienced pain in ankles, knees, wrists, and shoulders, clicking joints and had cold sores inside the nose, which was considered to be incapacitating, approximately three to four weeks after receiving a Fluvirin vaccine on 10/19/99. The pt did not experience cold or flu symptoms. At the time of reporting the pt had yet recovered. The reporter has assessed the relation of study medication to the event as probable.

Write-up: It was reported by a female nurse that she received a dose of Fluzone (Trivalent types A & B). The last week of October 2000 for the first time, she has developed a "very severe skin condition called Bullous Pemphigoid and Linear IgA disease, which is an autoimmune disease". Currently, both of her hands and feet started to have more lesions. She has been dermatologists and also went to a hospital as well. Biopsy was taken to confirm Bullous Pemphigoid and Linear IgA disease. Follow up # 1. From the pt indicated that she has been visiting her dermotologist every 4-6 weeks in th past 2 years. the rash, which started around Oct 17 (no year specified) on her face, is still present and spreading. She is experiencing severe itching, is extremely uncomfortable and has redness, swelling, and excessive tearing in her eyes. The lesion on both of her hands which started in 18 dec 2002, has not resolved. the center of her palms are raw, itch, and tight (contracture). She also reported that her whole body is now scar from the rash and lesions that started since Nov 2000. The Fluzone was administered IM n the left deltoid. The pt considered her skin condition resulted in permanent disability due to unable to perform duty. The follow up received on 2/27/03 from the pt reporting that since she is a nurse herself, and due to her ongoing condition since the vaccination, that she is on restrictive duty at work and to see no patients. She also reports that she has been taking many creams and ointments-prednisone, to ease her condition. Both of her hands are still affected and have gotten worse, they are oozing and itching. She has seen at least 3 dermatologists and she states that all of the doctors agree that she has autoimmune disease. Her biopsy results showed linear IgA and possible famkphogoid; pt has had to use all of her vacation time and her scheduled time off because of this condition.

Write-up: Affected muscle in my neck across shoulders back, swollen right ankle. Treated for rheumatoid arthritis said to have been brought on by vaccine. had broken right arm-affected left arm where shot was given the year before. Couldn''t get clothes on or off. MD gave me Sulen Dx which helped soreness swelling but I dropped everything couldn''t sleep-coughed and coughed all night I had horrible headaches. Couldn''t take Aspirin. My pharmacist said to get off it for 2 weeks and then take Aspirin. I had fallen three times bending my left wrist back twice and I twisted my right ankle. The third time I went down on my back and shoulders and neck. My left arm (the inside) was solid black and blue. The regular doctor had me on Celebrex and Prednisone she then sent me to a rheumatoid arthritis doctor 2/26-3/26. Last Sept 2001 I helped my neighbor take up his old deck I lifted 39 6ft x 4in" deck boards. I am an outside working person. Thanks to the prednisone I went from 126lb to 113lb. All muscles. As one doctor put it about a year before, I''ve never seen some your age with so much muscle now I have hanging skin and ouch ouch ouch-can''t and aged 10-15 years. I was walking two miles a day plus what I did around the house mowing the yard etc. They can put a man on the moon but you can''t have a solution to contract the flu shot. Please help. Next day pain from right arm across shoulder to left arm Celebrex Sulindac fell fractured wrist 3/21/02, 3/23/02 fell on 3/21/03 fell fractured ribs, ankle, elbow and back. Prednisone back muscle 13of 113 rbs Oct 2002. Bextra had rash, 3/21/03 fell fractured ribs.

Write-up: I got sick in the year 2000, I believe as a result of the flu shot. I remained severely ill from the end of the year 2000, through to the month of October 2001. At that time, the severity of the symptoms began to subside a little bit therefore I thought that I was on the road to recovery. Then in November, for and through work, I got another flu shot. Consequently the severity of symptoms has increased, and health has been a significant struggle. This include severe muscle and joint pain, headaches, burning pain in the spine, centered around the base of the skull, persistant right sided weakness, especially in the arm and leg, right side of face is drooped and lacks muscle tone and strength of the left side. Right eyelid is also droopy. I am only able to work part time instead of full time, due to persistent pain and fatigue.

Write-up: A report was received on 10/2/02, concerning a 50 year old female pt who experienced capsulitis of the shoulder and a small sensitive lump in the armpit after receiving Fluvirin vaccine in November 2001. On an unknown date, the pt initially complained that the right arm at injection site was tender several weeks after receiving Fluvirin in November 001. On an unknown date, a small sensitive lump developed in the right armpit. More recently, the pt complained of restricted movement in the right arm and in September 2002 approximately the pt was diagnosed with capsulitis of the shoulder. At the time of reporting the pt had not recovered and was receiving physiotherapy. There was insufficient info to determine the reporter''s causality assessment. Follow up on 02/12/2003: "Follow up report was received from a health care professional, concerning a 52 year old vaccinee which stated that the vaccines had no history of trauma. Within 24 hours of receiving the vaccination there was pain at the site of the injection and the vaccinee had an inability to throw objects. There were small painful lumps in the right armpit. Over 10 months there was increasing severity of pain, which the reporter considered to be temporarily disabling. The pain increased particularly when the muscle was jarred and used as in, for example, attempting to throw or in chopping actions. There was periodic throbbing pain, which was apparently spontaneous. The vaccinee experienced sleep disturbance due to pressure on the upper arm when lying on the right side. There was increasing, though not very severe, loss of power in the arm. there were increasing difficulties over 10 months with the shoulder, although the reporter was unsure if this was directly related as the vaccinee had had a history of difficulties with the other shoulder. Examination of the vaccinee on 09/16/2002, showed there was limitation of abduction of the right shoulder to 70 and some limitation of external rotation, other movements were normal. The health care professional felt that this indicated a degree of capsulitis resulting from a chronic painful shoulder which had stiffened up because it was not being used due to pain. It was presumed the injection was the cause of that pain, which may have become self-perpetuating as the shoulder stiffened up. The health care professional did not feel that the vaccine would have had a direct toxic effect on the joint capsule, as the injection would have been administered in the muscle some distance from the capsule. On 12/01/2002, the vaccinee was still unable to throw, although the pain had lessened. There was a small indentation in the upper arm at the injection site and the vaccinee experienced sensitivity in this immediate area. The shoulder pain and range of movement had improved due to targeted exercise. On 01/22/2003 the vaccinee''s condition had improved greatly, and her recovered almost 90% of the range of movement. The vaccinee was still unable to throw objects without pain, but this pain was less. There was no loss of power to the arm. Write-up: This is a time course of events that occurred after her influenza virus vaccine was administered 12/7/00. Pt was admitted into the clinic from 12/28/00, through 12/31/00, for extensive heart tests for extreme pain in chest and back. Nothing was found to be wrong with her heart. She returned home only to have further deterioration in her condition. 1/2/01, pt was admitted into hospital for loss of mobility as she could not walk at all. It started with drop foot and ended with total paralysis. She had no control over her extremities with excruciating pain in her back and chest. She was placed into intensive care after a full body MRI was done a few days after her admission. There she laid in pain, kept alive until the specialists visited approximately one week after admission. At this time it was determined by the specialists that they were treating an extreme case of Guillain-Barre Syndrome. The paralysis that had begun in her feet, crept up to the top of her head, leaving pt totally immobile but with excruciating pain. She remained in hospital until 3/19/01. 3/19/01, pt was admitted into hospital under the care of the original physician whose nurse had administered the influenza virus vaccine and pneumonia vax on 12/7/01. It was under the subsequent care of MD it was mentioned that the flu shot might have been the cause of reaction to the flu shot that was responsible for pt''s Guillian-Barre Syndrome. It was also mentioned at that point that there was a vaccine adverse event reporting system available to injured parties. Pt was hospitalized at hospital until 4/19/01. Her hospitalization would have been longer had her private insurance not run out. Pt''s physical therapy after hospitalization was continued within a month of her discharge down at the local swimming pool until June of 2001. All of the above has documentation behind it from the various hospitals, available upon request. Pt''s current condition is confinement to a wheel chair and slight use of her hands and upper torso. Per 60 day follow up: Pt'' records were released to VAERS on 11/22/2002. This is a time course of events that ocurred after her influenza virus was administered Dec. 7, 2000. Pt was admitted into the clinic, from Dec. 28, 2000, through Dec. 31, 2000, for extensive heart test for extreme pain in chest and back. Nothing was found to be wrong with her heart. It was under the subsequent care of MD it was mentioned that the flu shot might have been the cause or reaction to the flu shot that was responsible for pt''s Guillian- Barre Syndrome. It was also mentioned at that point that there was a VAERS available to injured parties. Pt was hospitalized at hospital until April 19, 2001. Her hospitalized would have been longer had her private insurance not run out. Pt''s physical therapy after hospitalization was continued within a month of her discharged down at the local swimming pool until June of 2001. This report is the result of an inquiry to VAERS several weeks ago. All of the above has documentation behind it from the various hospital, available upon request. Annual follow up on 11/18/2003: "Vaccine recipient did not recover from the adverse events that were reported for this vaccination. Chronic weakness lower extremities wheel chair bound for life." Annual follow up on 11/25/2003: "Vaccine recipient did not recover from the adverse events that were reported for this vaccination. Chronic weakness lower extremities wheel chair bound for life."
