Write-up: Got a flu shot in early January 2001. Got the flu about third week in February 2001. Took two weeks to recover from severe flu. Mid-March (about a weeks after symptoms were gone) arms and legs started tingling and going numb with increasing severity. Hospitalized and diagnosed with Guillain-Barre syndrome. Write-up: Two weeks after flu-pneumonia shots I got flu like symptoms and skin rash. Gradually the rash on my legs turns in vasulitis and foot drop. In December of last year I contracted unusual and extremely severe skin rash and flu like symptoms, just two weeks after my flu-pneumonia shots. One week later, I got typical flu like symptoms, with joint pain, fevers with temperatures over a 100 deg. F. This condition went on for five weeks. I spent five days in a local hospital. I was treated with antibiotic Augmentum and eventually my temperatures came down to normal by 1/20. However, the rash which started before the flu gradually spread all over my body. It was of very unusual character, extremely irritating burning and itching to a point where I was not able to sleep. Eventually the rash spread all over my body. The dermatologist ordered a biopsy, which was done and came back negative, so no pathogen was found responsible and that condition was diagnosed as eczema. A variety of cortisone ointments were prescribed with little or no healing effect. In early February, I noticed swelling on my ankles and some strange pinching sensation when I pressed with my finger the soft tissue. MD, who was subtitling for my primer physician did not make any conclusion and simply said it is common for older people. Later the same month, I again went to see my primer physician MD and I was started on ointments Triamcinolone Acenonide cream 0.025%. This was the first cream which started to reduce the rash and itching. However, the rash persisted to stay on both legs. Under the skin patches of very painful strips developed along the tibia bone. In late May 2002 this condition worsened even more and then started a pinching and stabbing sensation. Also both legs and ankles got more swollen and the swelling gradually has spread further up to the knee. The reddish spots were inflamed and sensitive to touch and it started to appear around the front leg bone and some dark blue spots occurred on the right leg. Write-up: Information has been received from a physician concerning his wife, a 64 year old white female with hay fever and a pcn allergy who on 10/18/02 was vaccinated with a dose of pneumococcal vaccine 23 polyvalent (lot 641532/1353L). Concomitant therapy included Lipitor, Niaspan, Volmax, Mavik, Allegra, influenza virus vaccine and Foltx. Within six hours after the administration of her first dose of pneumococcal vaccine 23 polyvalent. On 10/18/02, the pt experienced pain and her right upper arm had swollen two to three times its size. She also develped erythema and heat with immobility of her right arm. The pt felt warm and may have had a low grade fever but measurements were not taken. The physician also noted that a blood test following the injection indicated that the pt''s serum creatine kinase (CPK) was elevated to 512. Subsequently on 10/18/02, the pt was treated with rofecoxib 50mg tablet, 50mg once a day for two days. Thereafter, the dose of rofecoxib was decreased to 25mg once a day along with Vicodin. On 10/21/02, a complete blood count, "SMA-20," serum creatine kinase test, urinalysis and a serum immunoglobulin E test were all within normal limits. On 10/22/02, after four days of oral therapy, a heating pad four times a day and elevation of the right arm, the pt recovered from the pain, swelling, erythema, and heat with immobility of her right arm as she had remobilization of her right arm and the swelling was 80% down. The pt sought unspecified medical attention. The pain, swelling, erythema, and heat with immobility of her arm were considered to be disabling for 96 hours and other important emdical events (OMIC) by the physician. Write-up: Given Td/Influenza (same syringe)on 11/6, already had flu like symptoms at time of vacination. 11/9 showed increased signs of flu like symptoms, pain in neck and chest. 11/10 began vomiting, complained of chest pain rapid heart beat, had fever. 11/11 was given antibiotics at job corp center and sent home w/complaints of chest pain and coughing up green and black "stuff". Continued to vomit thru 11/12, had appointment to go to private doctor on 11/13. 11/13 0500 am collapsed. Taken to emergency room. BP low and continued to decrease. Showed enlarged heart and liver, kidneys shutting down. Dignosed with myocarditus, inflamation of heart muscle. Placed on ventilator for three days. Survived illness at this point heart functioning at 30%. Continued treatment indefinite at this point in time. |

Write-up: A report was received from a regulatory authority on 11/25/02 concerning a 54 year old male vaccinee who experienced a cold (3 days post vaccination), muscle pains (7-10 days post vaccination), decreased urinary output (9 days post vaccination), diagnosed as rhabdomyolysis with acute renal failure after receiving the Fluvirin vaccine on 10/18/02 which involved or prolonged hospitalization, involved persistent or significant disability or incapacity and was medically significant. The vaccine had a history of deep vein thrombosis, inferior vena cava obstruction, hypertension, renal allograft and end stage renal failure. The vaccinee was receiving ciclosporin, prednisolone, sirolimus, atenolol, doxazcsin, simvastatin, omeprazole and warfarin concomitantly. On 10/21/02, the vaccinee experience cold symptoms. Seven to 10 days later the vaccinee had muscle pains and on 10/27/02, decreased urinary output. A renal biopsy was performed and nothing abnormal was detected, that is, no rejection of the allograph. A diagnosis of rhabdomyolysis with acute renal failure in the transplant kidney was made which required dialysis treatment. The vaccinee had not recovered. There was insufficient information to determine the reporters causality assessment. |
