Write-up: Immediate severe pain at site, whole left arm feeling heavy and noticed numbness and tingling in my left hand same day, following day entire arm swelling including forearm bulging up. I am having numbness & tingling in both hands as well as muscle weakness (grip strength reduced) numbness in lower extremities and intermittent tingling in feet and legs as well. Fingers were pruning and swelling up and down visibly for several days, I still feel I am retaining fluid but visible swelling reduced. Overall severe exhaustion 6 days post injection, my blood pressure is up which is not typical.

Write-up: The patient deliberately checked that she had not received a prior vaccine- she checked "no" on the question, " have you received previous Covid 19 vaccine". She received 2 doses of Pfizer vaccine on 12/18/2020 and 01/11/2021. On 08/25/2021 she requested Janssen vaccine as her one and only vaccine.

Diagnostic Lab Data: 8/28/21 D- Dimer 1.41, CT Pulmonary angiogram - on 8/28/21 which showed a nonocclusive thrombus in posterior descending branch of right pulmonary artery. CBC , Pt. PTT, INR with in normal limits

Write-up: Patient developed chest pain 7 days post first dose of Moderna vaccine administered on 8/17/21. DEVEPOPED CHEST PAIN ON 8/25/21. Was seen in TTA on 8/28/21 for chest pain evaluation. Had CT angiogram on 8/28/21 which showed a nonocclusive thrombus in posterior descending branch of right pulmonary artery. He was started on Eliquis in the ER and is currently on it. Inamte was examined and , we do not see any risk factors for PE other than getting the vaccine recently. Tested negative for covid all the time since last year. Never had covid infection. Patient is currently in stable medical condition and discharged back to the facility.

Write-up: Increased off feelings in chest -- resting heart rate increased from 62bpm to a range of 90 - 107bpm, for roughly 5 or so days. No treatment given. Resting heart rate returned to normal after about a week.

Write-up: Patient reported experiencing arm swelling, redness, warmth, and soreness around the area of injection site after receiving the 2nd dose of Moderna vaccine. Photo showed picture of deltoid area, redness around the deltoid , extending approximately half-way down to elbow. Patient developed a bruise at injection site but reported the bruise is fading, and the swelling has gone down. She experienced fever and fatigue after the injection, but reported not having to seek medical attention. She said fever escalated to over 103 deg. F. She used OTC treatment to help with fever and arm pain. She said she has recovered from all fever/fatigue symptoms as on 8/31/21. Arm swelling has gone down and redness around injection site is decreasing. She added that she experienced a similar local injection site reaction after first dose, but it was smaller and not to the extent of the reaction she experienced after the 2nd dose. She reported having no systemic symptoms after the first dose.

Write-up: The first night after the injection, I slept straight through the night and did not get up to urinate. When I woke up in the morning, my urine output was low. Write-up: About 3-4 hours after receiving vaccine, developed itchy red rash on both sides of abdomen and neck. Then i began having tingling and numbness on left side of body along with nausea and dizziness. My neck became tingly and then my bottom lip went numb. Lasted several hours, almost thought my throat was going to began closing. Took multiple Benadryl, rash did not get better. Had to call doctor next day to get steroids. Day 6 rash is clearing up. Write-up: ED to Hosp-Admission Current 8/26/2021 - present (6 days) Patient presents with ? Shortness of Breath 1. COVID-19 infection. Patient is severely hypoxic and currently on high flow oxygen at 6 L, 50% FiO2. Patient is fully vaccinated for COVID-19. Will obtain ABG on admission due to her mental status changes. I did discuss about treatment options with the patient as well as her family over the phone. We will start patient on IV Decadron x10 days, remdesivir and plasma. We will consult infectious disease due to her rapid progression of respiratory decline. I have also discussed this patient with the pulmonary team. Will try proning position. Consent obtained and given to the bedside RN in the emergency department. Will obtain D-dimer and trend inflammatory markers. 2. History of COPD with acute exacerbation. Patient is on steroids. Scheduled DuoNeb as well as as needed DuoNeb therapy. Continue home inhalers. 3. History of stroke. Continue patient on Plavix and atorvastatin. 4. Hypertension. Continue patient on Coreg. 5. AKI on CKD stage III. We will hold oral diuretics. AKI likely prerenal etiology. 6. Hyperlipidemia. Continue patient on atorvastatin. 7. GERD. Continue patient on Protonix daily. Disposition: Pending clinical improvement. Patient will need OT/PT evaluation for safe hospital discharge. Write-up: Patient Received Vaccine at approximately 1305. Patient was talking. At approximately 1307 for about 10-15 seconds patient lost consciousness. PHN immediately supported the client, called for help, and assessed patient. Patient was Diaphoretic and Pale. BP taken and noted to be 124/40 upon startling awake. Pulse in the 60?s Respirations 18. Ice pack placed under arm. Patients coloring returned. Soda given to patient and patient stated feeling better. At approximately 1315 patient lost consciousness for a second time. PHN again called for assistance while supporting the patient and the patient was lowered to the floor and feet put up on chair. Patient was unconscious for approximately 15-20 seconds. Once on the floor patient came too. Another BP was taken and noted to be 122/80 with a pulse in the 60?s and Respiration rate at 18. Ice pack placed under patients neck. Patient stated immediate relief and was already feeling better then before the second loss of consciousness. Another PHN called 911 while this was happening. Patient remained conscious until Paramedics arrived. He declined transportation to the hospital and left the clinic.

Write-up: Reason for admission: This 83-year-old gentleman was transferred to our care from a Hospital because of right lung pneumonia, positive coronavirus assay, and ulceration noted in the distal portion of the patient''s aortic arch. History of present illness: Patient is 83 years old, and he lives with his wife and two of their daughters. He has a history of paroxysmal atrial fibrillation. He is chronically on Amiodarone therapy and Apixaban. The patient also has peripheral vascular disease; the patient had bilateral carotid endarterectomies performed in 2018. The patient also has a history of chronic obstructive pulmonary disease. From reading the last note from his pulmonologist the patient''s FEV1 was 37% of predicted. The patient also has a history of congestive heart failure. The patient''s left ventricular ejection fraction was "at the lower limits of normal" on his last echocardiogram from May, 2020. However, the actual ejection fraction could not be measured because the patient was in atrial fibrillation with a rapid ventricular response. The patient had a nuclear medicine perfusion scan done July, 2020, and this showed that the patient''s left ventricular ejection fraction was 46%. No perfusion or reperfusion abnormalities were noted. The patient states that on a normal day that he gets short of breath walking to and from the mailbox. The patient states that it is about 100 yards from his house to the mailbox. The patient can walk to the mailbox at a slow pace. He stops and rests when he gets to the end of the driveway. The patient states that he will usually have to stop and rest about halfway between the mailbox and his house when he is walking back to the house. The patient sleeps on one pillow. He does not have paroxysmal nocturnal dyspnea. The patient states that he has had edema in his feet, ankles, and lower legs for about one year. The patient has not had any chest pain, be it retrosternal or pleuritic, whenever he exerts himself. He is only having dyspnea. The patient states that he mostly went into the emergency department at the Hospital yesterday evening because of worsening shortness of breath. The patient states that he had a cough about a week ago. This cough lasted for about three days, and it was productive of small amounts of white sputum. It was not productive of purulent sputum. The patient states that after the cough resolved on Wednesday or Thursday of last week he has noticed that he is more short of breath with less exertion. The patient''s daughter checked the patient''s temperature yesterday afternoon, and it was 101 ?F. This constellation of symptoms is what led the patient go to an urgent care facility. The patient was subsequently transferred directly to the emergency department at the Hospital. The patient was tested for coronavirus, and his PCR assay is positive. The patient states that he and his wife both got coronavirus vaccinations in July. The patient was given 6 mg of IV Dexamethasone at the Hospital. The patient also had a CT angiogram of the chest performed. The patient did not have any evidence for pulmonary emboli. However, the patient does have evidence for small right pleural effusion and lung infiltrate primarily in the right lower lobe. There does appear to be some honeycombing in the lateral portion of the right lower lobe. Also noted was an area of ulceration in the distal portion of the patient''s aortic arch associated with aortic wall thickening. There is also a possible intramural hematoma present. The patient was going to be admitted to the Hospital, but they did not have a bed available. The patient was subsequently transferred here to treat his pneumonia, his coronavirus infection, and for cardiac surgery consultation because of his aortic arch ulcer. The patient is not having any retrosternal chest pain or pleuritic chest pain. The patient quit smoking cigarettes 6-7 years ago. The patient states that he normally does not have a cough. The cough that he had last week was out of character for him. The patient has been anticoagulated for a number of years because of his history of atrial fibrillation. He is also chronically on Amiodarone therapy Urinalysis is normal except for 1+ albumin being present. Pro-beta-natruretic peptide is 2128. CMP reveals the following: sodium 140, potassium 4.1, chloride 101, bicarbonate 28, BUN 25, creatinine 1.36 (creatinine was 1.55 four months ago), glucose 113, albumin 3.4, calcium 7.8, AST 35, ALT 20, alkaline phosphatase 61, total bilirubin 1.0, total protein 6.0, and ionoized calcium 4.3. CBC reveals a white count of 4.14, hemoglobin 11.3, hematocrit 35.2%, MCV 109.7, and platelet count is 162,000. Hemoglobin level was 13.0 three months ago. Coronavirus assay via PCR is positive. Problems: 1. Right lower lobe pneumonia 2. Positive coronavirus assay 3. Severe chronic obstructive pulmonary disease 4. Aortic arch ulcer 5. Suspected congestive heart failure 6. Peripheral vascular disease 7. Proximal atrial fibrillation 8. Hypothyroidism 9. Glaucoma 10. Chronic renal failure; the patient''s creatinine is currently 1.36 11. Anemia Discussion: The patient has been admitted because of his myriad of problems. We will treat his right lower lobe pneumonia with IV Azithromycin and IV Ceftriaxone for community-acquired pneumonia. The patient does not have any infiltrate in his left lung. Most of the infiltrate is present in the patient''s right lung on chest x-ray. I suspect this is probably from a community-acquired pneumonia. I do not think the patient has significant coronavirus pneumonia since most of the infiltrate is located in one lobe of the lung. We will send a sputum specimen for Gram stain and culture. We shall send a urine specimen to strive for Streptococcus pneumoniae antigen. Because of the patient''s hypoxemia and his positive coronavirus assay, the patient needs to be treated with IV Remdesivir and IV Dexamethasone. We will treat the patient''s COPD with the following medications: Tiotropium two puffs daily, Advair 230-21 two puffs twice daily, and Albuterol 2 puffs every 4 hours as needed. With regard to the patient''s aortic arch ulcer, we will consult cardiothoracic surgery for their opinion about what, if anything, needs to be done about this. Regarding the patient''s probable congestive heart failure, we will get an echocardiogram on this gentleman to determine what his left ventricular ejection fraction is. The patient is in sinus rhythm on his EKG right now. We will continue the patient on Metoprolol XL 75 mg daily to treat his congestive heart failure. We will start the patient on Losartan 25 mg daily because he is hypertensive and because he apparently has a depressed ejection fraction based on the results of his last nuclear medicine stress test in July, 2020. I will stop the patient''s diuretic therapy for the time being while we sort things out. With regard to the patient''s paroxysmal atrial fibrillation, we will continue the patient on Amiodarone 100 mg daily. We will continue to anticoagulate with Apixaban 5 mg twice daily. We shall continue the patient on his usual dose of Levothyroxine to treat his hypothyroidism. We shall continue the patient on his usual Brimonidine eye drops to treat his glaucoma. The patient''s chronic renal failure is stable; hjis creatinine is currently 1.36. With regard to the patient''s anemia, his hemoglobin is 11.3. Three months ago, the patient''s hemoglobin level was 13.0. The patient''s MCV is elevated. The patient just had blood work done earlier this month. The patient''s percent transferrin saturation at that time was 45%. Serum iron level was 111, and total iron binding capacity was 244. Ferritin level was 310. Vitamin B12 level was 684, which is within normal limits. Serum folic acid level was 15.0, which is also within normal limits. I am not certain what might be causing the patient''s mild anemia. Obviously, the patient is not iron deficient. He also does not have B12 or folic acid deficiency. It is hard to say what might be causing the anemia at this point in time. PATIENT REMAINS HOSPITALIZED AT THE TIME OF THIS REPORT

Write-up: Around the 08/18/2021 I thought I was having allergies, I started feeling like my chest and throat were congested. I started having a mild to moderate headache. I started feeling weak and tired. After that I lost my taste and smell and the abdominal pain came on. I went to a mobile clinic and tested positive for COVID 19 on 08/25/2021. On 08/27/2021 I went and had another test done with a positive result. My symptoms have gotten better.

Write-up: We found out after we gave his shot we realized that he had a first dose of the Pfizer vaccine but told us he never had any shot before. We called him immediately and hew told us he wanted to get the shot but changed his mind and never got the shot. We checked with the clinic where he was reported as receiving the dose and they told us he actually received the shot, we asked them to send us a copy of the consent for he signed via fax or email but never got such a document sent to us. We are not really sure of who is telling the truth. Several attempt to contact the individual has been unsuccessful.

Write-up: I was fine immediately after the vaccine until the night of the 25th and my throat started to hurt. I started drinking liquids because it felt dry. And then the 26th, I woke up and my throat was killing me and it was hurting so bad. I ran to the mirror to see if I could see if my tonsils were infected. They weren''t infected but throat was flaming red and swollen. They took a strep test. It wasn''t strep. He gave me a prescription for amoxicillin and I didn''t take it since I wanted the booster to do it''s job. I started gargling three times a day with salt water. By Saturday, 28th, I couldn''t talk at all and it hurt so bad to swallow. I started taking ibuprofen for the pain. 29th, the swelling went down a little bit but it still hurt to swallow and I couldn''t talk still. I thought it was Laryngitis. Thirtieth - on Monday - when I woke up in the morning, I could swallow with no problem except it felt like something was still there and I went to the mirror and I saw that there was a pocket of pus - a little one. I kept gargling with salt water all day on Monday. On Tuesday it was all gone. Today, I still have a raspy throat but as of yesterday swelling and redness is gone, etc. I am tired, though.

Write-up: It was like having COVID all over again. Aside from the soar arm by the back day I felt fatigue and achy in my ankles, feet, and toes. The fatigue continued the next day and by Saturday (3 days) I had chills, then even more fatigue by Sunday and a cough. By Tuesday my throat and chest burned. Wednesday the congestion was overwhelming a s finally I broke and was completely overcome and had to go to bed as my head felt like it was going to explode and my ears ached. I had very little contact with any person prior to the second vaccine dose and no one I knew had experienced COVID recently or even symptoms. I have been holding off getting vaccinated because others I?ve talked to said they felt this way too, especially those who had COVID. So I decided to set report my experience thinking perhaps these cases just aren?t being reported.

Diagnostic Lab Data: informed primary care physician of extreme dizziness and mild neck pain severe headache. Rested after collapsing twice. August 27 (three days later) I was taken to the ER. Fluids were administered MRI was conducted Benadryl was advised MRI showed no signs of stroke. Benadryl helped the stiff neck subside. Stamina and blood pressure still frequently bothersome.

Write-up: Serious issues with blood pressure stability disease blackouts, serious headache initially, my headache subsided three days later, Extreme stiff neck and joints of hands, some light sensitivity, and feet relieved primarily by Benadryl. (Sixth day vomiting and diarrhea, but I am uncertain if that was related to the vaccine or not). Still having issues with stamina but overall dizziness has subsided. Some pain and hands and feet still occurring this is not uncommon with an inflammatory response. Most of these are controlled by diet it under normal circumstances

Current Illness: Hospitalization 7.27.21 - 8.5.21: HOSPITAL COURSE: Patient is an 87 y.o. man with PMh: CAD, s/p PCI, Dm2, HTN, hypothyroidism who recently had bilateral SDH s/p left Burrnhole on 7/22/21 and Right MMA embolization on 7/23/21 who was admitted from 07/20 02/07/2020 7/21 to neurosurgery and discharged to a rehabilitation specialists presented back on same day for change in mental status and left-sided weakness consult for acute CVA. Patient was initially admitted to neurosurgery with neurology consult. He was found to have a cryptogenic right MCA territory stroke with seizures. Aspirin was not given, due to his recent subdural hematoma and risk for re-bleeding. He was started on Lipitor 40 mg daily. He was placed on Keppra for epilepsia partialist continua. Neurosurgery added Provigil due to daytime somnolence, and dose was titrated up to 200 mg daily during admission. Neurology recommended 14-day ZIO patch at discharge, and clinic follow-up in 3 months. Care was transferred to Internal Medicine service. Patient''s course was complicated by encephalopathy, likely multifactorial due to left hand cellulitis and hospital delirium. He was treated with cefazolin, and left hand cellulitis resolved. Due to his encephalopathy, patient was not eating so a Corpak was placed. Once patient received nutrition, his mental status gradually improved. His appetite and oral intake improved, such that Corpak was able to removed on 8/5. Both SLP and nutrition followed, and he was tolerating mechanical soft diet with nectar-thickened liquids. He will need to be followed by PT, OT and SLP following discharge. He was felt stable for discharge to subacute rehabilitation on 8/5/21. He will require outpatient Neurosurgery follow-up in 2 weeks. He will continue on Lipitor 40 mg daily and Provigil 200 mg daily. No antiplatelet agents/Aspirin due to recent brain bleed. He should follow-up with stroke clinic in 3 months and PCP in 1 week.

Write-up: Hospitalized; COVID-19 positive 8.25.21 (fully vaccinated) Admission Date: 8/25/2021 Discharge Date: 9/1/2021 PRESENTING PROBLEM: Patient is a 87 y.o. male with history coronary artery disease, hypertension, hyperlipidemia, hypothyroidism, insulin-dependent diabetes, obesity, and recent subdural hematoma status post burr hole as well as recent MCA stroke with seizures who presented to the ER from his rehab facility with concern for aphasia/confusion. It is reported that there is a COVID outbreak on the floor he resides on at the nursing home. His wife spoke with him around 9 am the morning of presentation and he reported feeling ill and had a fever. He tested positive for COVID and shortly after he was brought to the hospital with concern for abnormal speech/confusion. The patient was evaluated by Neurology in the ER, loaded with Keppra 20 milligrams/kilogram. In the ER, vitals remained stable, afebrile, normotensive, O2 sat 98% on room air. Labs showed mild AKI with creatinine 1.23, stable hemoglobin 11.1, no leukocytosis. EKG showed normal sinus rhythm, 63 ppm, first-degree AV block. Confirmed COVID positive. CT head, CTA head/neck, and CT brain perfusion did not show acute pathology, fairly stable interval imaging. Neurology recommended admission to Medicine for MRI brain, EEG monitoring, and workup for stroke. The patient was admitted to Internal Medicine for further evaluation and treatment. HOSPITAL COURSE: Multiple small embolic subacute ischemic CVAs of both hemisphere CTA head/neck: multiple areas of narrowing/stenosis without large vessel occlusion CT brain perfusion: large, chronic left holohemispheric cerebral convexity subdural hematoma, and a small, resolving, chronic right PCA territory. There is apparent ischemia within the bilateral cerebellar hemispheres, which is favored to be artifactual. No acute core infarction is apparent. -MRI brain w/o contrast: 1. Stable bilateral subdural hematomas and stable associated mass effect. 2. 6 mm focus of early subacute ischemic infarction in the right putamen without hemorrhagic transformation. 3. Small noncontiguous foci of subacute ischemic infarctions in the right PCA distribution without hemorrhagic transformation. 4. Stable late subacute/early chronic infarction in the right temporal occipital region. There is stable petechial hemorrhagic transformation without space-occupying mass effect of this infarction. 5. There are numerous tiny chronic infarctions throughout the cerebellum. 6. Focal and more confluent areas of hyperintense T2 signal are noted in the periventricular and in the subcortical white matter. This is nonspecific. Statistically however it is most consistent with mild chronic small vessel ischemic change of the white matter. 7. Age-appropriate brain atrophy. -Echo: left ventricular ejection fraction is 65%. No clear evidence for regional wall motion abnormalities noted but not all walls are seen well in all views. The mid and distal right ventricular free wall appear to contract well. The base is not well visualized. Today''s study was compared to one performed on 7/28/21. The LVEF was 63% on the prior report. -LINQ implanted on 8/30, if having A. fib than he can be started on Amiodarone per EP -continue statin -aspirin 81 mg started on 8/31 after discussion with patient and wife, who are in agreement to start it [Neurosurgery: Recommend holding ASA indefinitely if able, but if pt must continue ok to restart] -PT/OT recommended subacute rehab, and he was transferred on 9/1/2021 -he should follow up with neurosurgery in 2 weeks and with neurology clinic Seizure - cEEG: The inpatient video EEG reveals epileptiform abnormalities involving the right temporal and left frontocentrotemporal head regions, consistent with an increased risk of focal seizures. There is a severe degree of multifocal bihemispheric slowing, consistent with a focal cerebral dysfunction involving the indicated regions. There is also a mild degree of generalized slowing, consistent with a diffuse cerebral dysfunction or encephalopathy - continue Keppra 750 mg bid, when taking 1000 mg bid, he was too sedated -follow-up with epilepsy clinic in few weeks Recent bilateral SDH (L$gR) with MMA embolization (7/21/21) SDH stable on interval imaging Continue daily Provigil Asa 81 mg daily started on 8/31, watch closely Neurosurgery recommending to hold ASA indefinitely if able, but if patient must continue okay to restart. Plan to repeat imaging in 6-8 weeks (First week Oct). Patient to follow-up in neurosurgery clinic in 2 weeks. Write-up: Moderate pain in the back of right leg/calf area beginning the night of Aug 25 2021, around 8 pm. Increasinly more painful with a feeling of tightness, especially upon walking. I elevated and iced area and after 2 hrs called nurse hotline on back of insurance card. Took a Tylenol and Flexeril. No improvement by Thurs morning. Went to an Urgent Care clinic by 2 pm Thurs. FNP said blood flow was good but noticed swelling and discoloration. Pain upon touch but no warmness, fever, or severe pain upon flexing, only moderate. Was prescribed 10 mg of prednisone for 4 days. Was told likely inflammation. Improvement after 3 days of the steroids.

She noted that a couple of days ago she initially felt very chilled and fatigued. She had a cough. Her baseline oxygen in 4 L She is fully vaccinated against COVID-19. Patient was admitted and treated with Decadron 6 mg daily. She was also a candidate for Remdesivir and got 5 doses before her discharge. Patient was stable on her baseline oxygen of 4 L NC. Of note patient was confused the day after her admission. She could not remember why or how she got to the hospital. She kept insisting she was at her home. The following day the patient was back to her baseline and she was aware of her previous confusion. She was given steroids in the ED which could have contributed to the confusion. Write-up: While playing casual basketball I suddenly lost the feeling in my arms, over a few minutes I collapsed to the ground twice with a racing heart. The workload was light. A few days later the same thing happened while I was mowing the lawn. Both times I was plenty hydrated. Nothing like this has ever happened to me. HISTORY OF PRESENT ILLNESS Patient is a 71 y.o. female with past medical history significant for Crohn disease on chronic anemia depression, anxiety, subdural hematoma with chronic right-sided weakness, epilepsy, hypertension, history of DVT/PE who presented emergency department complaints of cough, shortness of breath and weakness. Patient states that her husband was found to be COVID positive recently. She was seen at urgent care a few days ago and was negative but states that she has had worsening symptoms. Cough has been productive and she has been having intermittent shortness of breath. Has been having fevers at home with most recent being this morning. She also notes a decreased appetite and minor sore throat. Patient notes that she is fully vaccinated with Pfizer. Per patient and sister at bedside, they noted patient had 3 mechanical falls in the last 3 days. Last fall was yesterday. Patient states that these falls were secondary to ?getting tripped up? and not necessarily from weakness. She does have chronic right-sided from her prior stroke. Patient states that she had to call her sister for assistance getting off the floor has her husband is currently weak with COVID and was unable to help. Patient denies having had any of her falls. Patient is also not currently on any anticoagulation. Denies any chills, lightheadedness/dizziness, loss of taste or smell, chest pain, abdominal pain, nausea/vomiting, diarrhea urinary symptoms. Upon arrival to emergency department, patient was tachypneic. Baseline blood work was obtained showing minor hyponatremia of 132, slight bump in her creatinine to 0.99, minor leukocytosis of 12.8 with lymphopenia, stable chronic anemia. COVID PCR was positive. CXR was obtained was negative for acute process. Patient was stable on room air for most of her stay in the ER. She did have 1 episode of desaturation to 88% but did recover quickly. Patient was then started on Decadron. She was then admitted to General Medicine under observation for further evaluation and management.

Write-up: Hospitalized; COVID-19 positive 8.28.21; admitted for COVID (fully vaccinated - 3 doses of Pfizer vaccine recevied) Admission Date: 8/29/2021 Discharge Date: 08/30/2021 Admitting Diagnoses: COVID-19 PRESENTING PROBLEM: COVID-19 HOSPITAL COURSE: Patient is a 87 y.o. man who presented to the emergency department with complaints of cough and shortness of breath. He has past medical history of chronic obstructive pulmonary disease, OSA with CPAP use, hypertension, hyperlipidemia, SVT, and CKD. Patient received a COVID-19 booster vaccine on Tuesday, 08/24/2021. Shortly after that time, he he developed a cough with shortness of breath. The cough had been worsening which prompted him to present to the emergency department for further evaluation. He additionally complained of fever, chills, weakness, and sputum production. In the emergency department, patient was febrile, tachycardic, hypertensive, and required supplemental oxygen to maintain a saturation greater than 90%. Complete blood count was unremarkable. BMP showed CKD. Lactic acid was 1.3. COVID-19 was positive. Chest x-ray showed no acute cardiopulmonary disease. Patient has been transferred and admitted to the hospital for further evaluation and management of COVID-19. He was started on decadron and remdesivir. His procalcitonin was mildly elevated and was started on rocephin and azithromycin. He was initially requiring supplemental o2 and successfully weaned off. He was discharged home on decadron and azithromycin.

Write-up: I was thirsty starting about 1hr after shot. Could not quench the thirst. Then around 0130-0200 I had a massive headache. Headache was debilitating, along with sever joint pain. I could not even stand up straight. I had a fever that would come & go. Also, the skin at the hip area was hot/warm to the touch. The side effects lasted about a day & a half. Write-up: When I received the first Pfizer vaccine I had mild acid reflux. I did not know it was related to the Pfizer vaccine. Then when I received the second shot, later that day the acid reflux was severe. I''ve had extreme symptoms for the past 10 days and I''ve lost 3 lbs and it''s difficult to eat and sleep. I''ve never had a history of acid reflux. I am not overweight. I eat very healthy. I do not eat processed foods or spicy foods, I don''t drink coffee or alcohol, so diet does not play a role in these symptoms. My doctor prescribed me Famotidine 20mg for the symptoms. As of September 3rd, I''m still suffering from it. I also asked a group of 12 friends who also received the Pfizer vaccines and out of those 12 friends 3 people also had acid reflux after getting the Pfizer vaccine. So that told me there''s definitely a correlation.

Write-up: After receiving the vaccine I contacted my husband and he said that my speech sounded very slow. I did feel a little tired after receiving very fatigue. I then went to the grocery store and I felt very tired and felt my vision get very blurry. I then decided to go home and was putting my groceries up and I felt very cloudy and felt as if I had to faint, also was experiencing heart palpitations at the same time, I grabbed the counter and decided to go upstairs and lay down. I then slept from 4pm-6am the next day and when I woke up I went to work and just was not understanding anything my coworkers were saying and they advised me to go to the emergency room. There they then gave me a Covid test which came back positive, and also an EKG and everything seemed normal. So I was sent home and advised to follow up in a week days, when I arrived for my appointment I was given another Covid test which came back negative, and the EKG came back a little different from the first scan but nothing to be worried about. I am currently still experiencing fogginess, chest pains, and heart palpitations, but they''re not as bad as they were after the vaccine. Write-up: On August 25th, the patient began to develop hives on his arms, legs, and neck. With benadryl the hives went away but came back once the medication wore off. From the 25th to the 29th the hives began to get worse and became painful, spreading to his face back, hands, feet, legs, and arms. On the 29th, he was taken to the Clinic and was prescribed prednisone for five days. After three days of taking the prednisone, the hives finally stop coming back. The five days of doses was complete on September 2.

Starting at 10A shortness of breath began by noon I had a severe headache, shortness of breath, chest pain, shooting pain in my lower arms, thighs and calves, bad back pain, chest tightness, abdominal pain, low grade fever and heart palpitations. Was admitted to the ER at 6PM on 8/25. Was treated with electrolytes, breathing treatment x-rays and blood work. Chest X-ray was clear, blood work was in normal ranges except for lymph auto level being low (12%). It was determined that inflammation was causing the shortness of breath and a course of prednisone was prescribed but was advised to be taken only if not improving in a few days. I avoided taking the prednisone due to its effect on the vaccine. I instead used 6 puffs of Proair HFA into a spacer in substitution for a home nebulizer four times a day and continue to at this time (9/3/2021). Over the course of 2 more days many of the side effects subsided. However, I still fight with shortness of breath, random shooting pains throughout my body, head pressure, occasional abdominal pain/cramps and occasional heart palpitations. I returned to the doctor today, 9/3 for a check up on the ongoing issues and was given a script for an x-ray and blood work. This time the X-Ray has revealed an opacity in the lower right lobe of my lung. I was given a Z-Pak script while awaiting blood work results and have to return for a 6 week X-Ray appointment.

Write-up: paralyzed; felt like a stroke; fell on floor; could not take pants down; bowels emptied; woozy; sweating profusely; she had some soreness in her arm; This spontaneous case was reported by a consumer and describes the occurrence of PARALYSIS (paralyzed), TRANSIENT ISCHAEMIC ATTACK (felt like a stroke), FALL (fell on floor) and LOSS OF PERSONAL INDEPENDENCE IN DAILY ACTIVITIES (could not take pants down) in a 74-year-old female patient who received mRNA-1273 (Moderna COVID-19 Vaccine) (batch nos. 020F21A, 030L20A and 001A21A) for COVID-19 vaccination. The occurrence of additional non-serious events is detailed below. The patient''s past medical history included Broken ankle. On 27-Jan-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) 1 dosage form. On 24-Feb-2021, received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) dosage was changed to 1 dosage form. On 25-Aug-2021 at 2:00 AM, received third dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) dosage was changed to 1 dosage form. Her cognitive function was fine and so was her speech. Patient gave consent to be contacted by email or phone. No relevant concomitant medications were reported. Write-up: lack of efficacy- contracted COVID-19; lack of efficacy- contracted COVID-19; This is a spontaneous report from a contactable consumer (patient). A non-pregnant 38-year-old female patient received bnt162b2 (PFIZER-BIONTECH COVID-19 VACCINE), dose 1 intramuscular administered in Arm Right on 05Feb2021 (Lot Number: EM9810) at age of 37-year-old, dose 2 intramuscular, administered in Arm Right on 24Feb2021 (Lot Number: EN6202) at age of 37-year-old, both at single dose for covid-19 immunisation. Medical history included asthma. It was unknown if covid prior vaccination. Concomitant medications included bifidobacterium bifidum, bifidobacterium lactis, lactobacillus acidophilus, lactobacillus brevis, lactobacillus bulgaricus, lactobacillus casei, lactobacillus paracasei, lactobacillus plantarum, lactobacillus rhamnosus, lactobacillus salivarius (PROBIOTIC 10). The patient previously took amoxicillin and experienced drug hypersensitivity. No other vaccine in four weeks. The patient experienced lack of efficacy- contracted covid-19 on 25Aug2021. Events resulted in [Doctor or other healthcare professional office/clinic visit]. No treatment received. The patient underwent lab tests and procedures which included sars-cov-2 test (Nasal Swab): positive on 25Aug2021. The outcome of events was recovering. Follow-up attempts are completed. This non-pregnancy 35-year-old female patient received bnt162b2 (Brand=Pfizer), dose 1 via an unspecified route of administration, administered in arm left on 25Aug2021 10:00 (Batch/Lot Number: Fa7485) as dose 1, single for covid-19 immunisation. Medical history included chronic pain depression. Concomitant medication included paracetamol (TYLENOL) taken for an unspecified indication, start and stop date were not reported. Facility where the most recent COVID-19 vaccine was administered: Pharmacy or Drug Store. The patient did not receive any other vaccines within 4 weeks prior to the COVID vaccine. Other medications the patient received within 2 weeks of vaccination: paracetamol. Known allergies: No true allergies just adverse side effects like clarithromycin (MACROBID) vomiting trazadone weird dreams varenicline tartrate (CHANTIX) we. The patient experienced anaphylaxis and fever on 25Aug2021 13:00, shot throat started closing up couldn''t breathe on 26Aug2021. Fever day one day two 1.5 days into shot throat started closing up couldn''t breathe. Went to ER everything checked out except she was in anaphylaxis from an allergy. No new meds or foods eaten. No new allergens introduced except vaccine. Never had a true allergic reaction in my life only side effects from certain meds like vomiting with clarithromycin for instance. Her throat still felt like it was trying to close up about once a day and she had to take more benedryl. The adverse event result in Emergency room/department or urgent care, Life threatening illness (immediate risk of death from the event). Treatment received for the adverse event included Iv steroids famotidine and benedryl. Prior to vaccination, the patient was not diagnosed with COVID-19. Since the vaccination, the patient has been tested for COVID-19: Covid rapid at work on 19Aug2021: Negative (Nasal Swab). The outcome of event shot throat started closing up was not recovered. The outcome of other event was unknown.; Sender''s Comments: The events was considered related to suspect drug based on strong temporal association and known safety profile of the drug. The impact of this report on the benefit/risk profile of the Pfizer product is evaluated as part of Pfizer procedures for safety evaluation, including the review and analysis of aggregate data for adverse events.
