Write-up: @1247 once client?s 15 minutes of observation was completed, she stood up to leave and felt dizzy. LPN assisted client to sit down and notified site managers. Client than reported feeling nauseated and dry heaved. RN and Site Supervisor and EMT arrived to client area. @1248 vital signs: 174/100, 82, 97% RA. Client AAOX4. NAD. Skin WNL. HA. EMT called 911 @1250. Fire Department Engine #1 arrived @1258. EMS repeat vitals @1300: 98.2, 187/102, 73, 20, 99% RA. EMS assessed BP/HR while standing, 218/123, 77. Stroke assessment negative. LAC 20g PIV inserted by Fire Department. BGL 170. County Rescue# 425 arrived @1307. Client showed no signs of improvement and no changes from prior assessment. Client transported to ER via ambulance @1310 in stable condition.

TECHNIQUE: Axial imaging along with 2-D and 3-D postprocessing performed from the level of the thoracic inlet to the kidneys following the administration of IV contrast and utilizing CTA protocol. Write-up: 3/18/21 ER Triage Nurse Note: "To er via EMS for eval of weakness and dizziness. States he started with sore throat and head congestion last week. Seen PCP Monday 03-15-21. Started amoxil. Took it for 2 days and became dizzy so he quit taking it. Dizziness got worse and came to ER." 3/18/21 ER HPI: 76 y.o. male who presents with c/o weakness cough and fever for the past seven days. Pt report sx are mod better with rest and worse with movement. Transferred to Hospital, dx: pneumonia dt covid-19 virus

Write-up: Patient collapsed after vaccination, hitting her head. She remained unconscious while vomiting. We administered epi-pen 0.3mg once and contacted 911. Patient was transported to nearest hospital.

Write-up: I had general fatigue and headaches. The after I woke up my left arm was red and puffy. Overnight the area had doubled in size, itchy and painful. The area was circular and 2-3 inches in diameter. I went to Urgent care and they gave me Benadryl and a Steroid to take. The area is still painful and swollen and is starting to improve. It has been seven days and the area is still red swollen, warm and itchy. I have three more days of Steroid medication left. I just had a headache and fatigue for the first couple of days.

Diagnostic Lab Data: Ekg, lab work and a bag of fluid. They sent me home because everything checked out fine and the doctor told me he believed it was a reaction to the vaccine. This happened March 17, 2021.

Write-up: I usually have normal blood pressure of 120/80. The blood pressure went as high as 160/84 and 155/92. My heart rate was resting at 115 for hours at work, which is abnormal for my heart rate. I checked into the ER around 5:30 pm. The ER gave me a bag of fluid and an EKG. The EKG came back normal. This all happened on March 17, 2021.

Write-up: Immediate(3-5 seconds) and rapid injection site swelling and redness, bleeding more than usual from a shot, dizziness, full body sweating, rapid heart rate, hives, as well as general body swelling that caused great difficulty in starting an IV. I did not have any significant issues with breathing. They treated me with a IV fluids, an intramuscular injection and IV injection of Benadryl and a steroid that I cannot recall the name of, I apologize.

There is no occlusion or significant stenosis involving the right MCA. The left MCA and bilateral ACAs are within normal limits. The intracranial vertebral arteries, left PICA, basilar artery and both PCAs are patent. There is no aneurysm or AVM. The evaluation of the venous structures is limited on this CTA but there is no opacification of the right transverse and sigmoid sinuses, suggestive of dural sinus thrombosis. Large right hemispheric hematoma is demonstrated with significant right-to-left midline shift measuring approximately 16 mm. Effacement of the right lateral ventricle and dilation of the left lateral ventricle. Right-sided transtentorial herniation is noted. IMPRESSION: 1. Suspect right transverse and sigmoid sinus dural sinus thrombosis. This can be confirmed with CTV if clinically necessary 2. No evidence of aneurysm or AVM to account for the right hemispheric intraparenchymal hematoma. Significant right-to-left midline shift and downward transtentorial herniation is noted."

Write-up: Diagnosis: Cortical vein thrombosis, massive intracerebral hemorrhage with tentorial herniation, thrombocytopenia. Clinical Presentation and Course: 1 week after receiving Janssen COVID19 vaccination, patient developed gradually worsening headache. On March 17th, patient presented to Hospital with dry heaving, sudden worsening of headache and L sided weakness. Evaluation with head CT revealed a large R temporoparietal intraparenchymal hemorrhage with 1.3cm midline shift. She ended up getting intubated for worsening mental status. On evaluation at arrival in Medical Center, she was noted to have extensor posturing. Repeat imaging revealed worsening midline shift to 1.6cm. CTA showed cortical vein thrombosis involving the right transverse and sigmoid sigmoid sinus with tentorial herniation. Patient developed brain herniation and brain death was pronounced on March 18th, 2021.

Write-up: Symptoms began little after ten hours of receiving vaccine. Symptoms included flu like symptoms of aches , joint pain and fatigue with fever up to 101 lasting 3 1/2 days. Write-up: Patient was seen in our ER on 3/8. She stated that she had received her Johnson/Johnson COVID Vaccine on Thursday (3/4/21) and since then, had been experiencing body aches, chills, fever, fatigue, headache, n/v/d. She was admitted for treatment with IV fluids and supportive meds for the n/v/d. On 3/9, she was still feeling very ill. Fever and chills had improved but she was still not tolerating PO. BP was also elevated ranging from 142/82 to 206/80. She was also diagnosed with AKI. BUN was 51, Cr 3.26. By 3/11, she was feeling a little better but kidney function continued to deteriorate. BUN was 53, Cr 4.39. The physician decided she should be transferred to a higher level facility for her AKI on CKD. However, there were no beds available. So she remained at our facility awaiting transfer. Around 1900 that night, she experienced a bout of hypotension and decreased urine output. Chest xray was normal. EKG was NSR. She was eventually transferred to another hospital on 3/12 at 1300.

Write-up: Presented to the ED for evaluation of hallucinations. Received the Johnson & Johnson COVID vaccine on 3/8. She had been feeling fine following it, until early this morning. She awoke and went to the bathroom when she noticed a "paisley pattern" that was "all over everything", noting that it was even present when her eyes were closed. While she was in the bathroom, the pattern began to move, causing her to feel nauseated and eventually vomit. She has never had a similar episode to this. All of her symptoms completely resolved after droperidol. Observed in the ED and was ambulating independently and returned to her baseline. She had no recurrence of any of her symptoms. She was comfortable going home and discharged with her sister.

Write-up: On Thursday, March 18, I had some slight dizziness during the day. After going to bed that night I woke up and the night light in the smoke alarm on the ceiling appeared to be spinning. I sat, then stood, grabbling walls for support and attempted to reach my recliner. At that point I was severely dizzy, completely lost any sense of balance and collapsed into the chair. I could not stand or walk. After 20 minutes, I asked my wife to call an ambulance. I did not think she would be able to get me into our car. The paramedics ran an EKG and tested me for a stroke. Both were normal. I was transported to the ER. The doctor gave me Meclizine. Had a CT scan of my brain, and blood work. All results normal. Dizziness resolved. They tested my balance and then released me at 3:30 a.m. on March 19. I was tired that day, but did not need further medication. During that night, as I turned over in bed, the vertigo returned. I took a Meclizine and went to the recliner for the remainder of the night. I am feeling somewhat fuzzy today, March 20. Write-up: Shortness of breath, sinus swelling, swelling in face, minor irritation of throat, warm, red skin on face and neck, dizziness Onset roughly an hour after injection, slow, progressive intensity of symptoms until roughly 11:30pm, at which point arrived at Emergency Department Treated with benadryl and prednisone in ED. At about 10:30am the following day, breathing clear and even,, no swelling or irritation in face or throat, some lingering redness and heat in face, second dose of prednisone administered at ~11:30am, symptoms continued subsiding.

Write-up: I had awful nausea, loud ringing in the ears, I fainted. The medics provided help, gave me oxygen. I started having convulsions. After some time I felt like the lower part of my face went numb and it took tremendous effort to move my lips to talk. At that point, the medical called the ambulance and sent me to ER, where I was under supervision for some time. I was very weak and nauseated. After I was released, I was very weak could not keep the vertical position. The next day I was very nauseated, with awful tinnitus, lightheadedness, and weakness. Staying vertical for more than 15 minutes brought fainting symptoms. I was not able to get out of the house to work. The next day the fainting symptoms disappeared, everything else continued. Too weak to go to work. I started moving about only on the 4th day after the shot. During this event, I did not have any fever or muscle ache, no virus0tyle symptoms.

Diagnostic Lab Data: Bloodwork all normal, except a heart value that was elevated. I had chest rads which were normal. My blood pressure was normal. CT of my lungs was consistent with pulmonary embolism. I had ultrasound of my legs which did not show any signs of blood clots or deep vein thrombosis. Echocardiology showed increased pressure on the right side of my heart due to blood clots. Otherwise, my heart was normal. All tests were done 03/18/2021, except echocardiology was done on 03/19/2021.

Write-up: Pulmonary embolism with acute Cor Pulmonale/hypoxia I had shortness of breath, dizziness, coughing. I had Covid test on 3/12/21 that was negative. Went to Urgent Care on 3/12/21. Gave me an albuterol inhaler which was not effective in relieving symptoms. I wasn''t improving, so went to emergency room at the hospital on 03/18/2021. I was admitted to hospital and placed on oxygen and kept overnight. I was also given Xarelto. I am at home and on Xarelto.

Write-up: Patient had vaccine on 3/18/21 at 1600. She presented to the ED on 3/21/21 @ 2202 for chest pain that started at ~1600. She states the chest pain is in the center of her chest, feels like pressure, non-radiating, with occasional shortness of breath. It felt similar to when she had COVID in January. She denies other symptoms including fever, URI symptoms, palpitations, pain with deep inspiration, leg swelling, recent travel, nausea/vomiting, diarrhea, dizziness, feeling like she was going to pass out, rash or other symptoms. It is not exertional and was better with walking. Denies heavy lifting or injury. She was hypertensive in the ED BP 151/99 but all other vitals within normal limits. EKG shows NSR without evidence of arrhythmia or ischemic changes. She did have U waves seen so labs were drawn including CBC, CMP, cardiac enzymes. Chest x-ray showed no acute cardiopulmonary process. She received 30 mg Toradol IV for pain. Labs were all within normal limits except mild hypokalemia. She felt better on reassessment and BP was still elevated but improved 137/90. She was discharged and instructed to take OTC Tylenol as on the bottle as needed for pain and given return precautions and follow up with PCP on return to stateside.

Write-up: The day the vaccine around 8pm Patient had chills and fever 100 degrees. Around midnight he started vomiting, had chills, fever 101, headache, body aches, extreme fatigue, racing heart 140 bpm. I called ER and told them symptoms. They said to bring him in. We arrived at ER at 1am. At ER they gave EKG, Tylenol, Toradol injection and IV fluids. They also gave him covid19 test. He started to feel better and he was sent home at 5am. Two days later he had rash going down his left arm where he received the vaccine. It was gone the next day.

Write-up: I started to get uncontrollable shakes the night of my vaccine for 10-15 minutes. I had chills for about an hour afterwards and a sleepless night. The next morning I woke up with a headache and was achy/sore throughout my body. While I was eating I passed out without any signs. I was unconscious and do not remember the episode. Once I was alert, I felt nauseas and was heading to the restroom when I passed out again and was unconscious. I do not remember the episodes. There were no signs of passing out before I did either time.

Write-up: Started at 2:00pm with itching and redness on left arm at the injection site. By 6:30pm rash/hives spread to legs, arms, feet, stomach and back. Severe itching. Took 25mg of Benadryl at 6:30pm. Rash appeared to subside. By 10:30pm rash and severe itching returned on legs, feet, back, stomach and arm. Felt tingling and numbness in tongue. Took additional 25mg of benadryl and called 911. Paramedics arrived and assessed vitals. Rash began to subside. Sunday 3/21 had residual internal itching all over body.

Write-up: The next day after the shot, I had a small rash on my wrist. It disappeared during the day. Saturday evening, my right thigh felt like it received a trauma like a hit, but I did not have an injury. An area the size of my hand is slightly pink on the thigh. On Tuesday, I tried starting ibuprofen, but it did not really remedy the pain. I went to urgent care Wed. evening. They sent me for an ultrasound to rule out blood clots. No blood clots, but lymph nodes were visible in ultrasound. They suggested I follow-up with PCP. On Thursday and Friday, the leg felt crampy but less painful. Friday night the pain traveled down my right leg to ankle as well as right lower lung had dull ache. Returned to urgent care. They are treating the leg for cellulitis (1000mg daily of generic Keflex.) They said if the leg felt better in 24 hrs, probably cellulitis, but if not, it might be a response to vaccine. There is no relief after 3 doses and 24 hr + later.

Write-up: At about 23 minutes broke out in a cold sweat. At 26 minutes, became hoarse (always my first symptom of anaphylaxis). Progressed to coughing and feeling a lump in my throat and then tightness in my chest. My blood pressure elevated to 152/100. At that point I self administered epi pen and paramedics called 911. This provided relief but after about 5 to 10 minutes the tightness in my chest and the feeling I couldn?t get a deep breath began to return and the paramedics administered two puffs of an albuteral inhaler and transported me to the hospital. At the ER, they did an EKG, started an IV and administered fluids, prednisone and Benadryl. I was discharged about 90 -120 minutes later with a prescription for 3 days of prednisone (50mg) and instructions to continue to take 25 mg oral Benadryl every 4 hours. The next morning my face and chest were red and looked like I was sunburned. My face was also swollen.

Write-up: Approximately 15 minutes after receiving the Janssen vaccine, the patient complained of numbness and tingling around her mouth and fingertips, felt flush/warmness on her face with noticeable redness, and became hoarse while speaking. The patient was given 25 mg of diphenhydramine at the vaccination event and her hoarseness improved but returned a few minutes later. She was taken to the Emergency Department for further observation. In the ED, she was given famotidine, dexamethasone, and 1L of fluids, and her symptoms improved. She was observed for 2 hours and discharged home with diphenhydramine and cetirizine.

Diagnostic Lab Data: EEG on 3/18/21: It is suggestive of severe diffuse encephalopathy of nonspecific etiology. Bisynchronous rhythmic slow waves with a wide distribution (GRDAs), are usually associated with a diffuse encephalopathy. GRDAs occasionally occurs in the setting of widespread structural damage involving subcortical and cortical gray matter to a great extend than white matter and in the setting of circumscribed structural lesions involving mesial frontal, diencephalic or brain stem structures. Therefore clinical and neuroimaging correlations are strongly recommended. MRI brain without contrast on 3/20/21: Asymmetric scattered regions of predominantly right-sided subcortical signal abnormality of a posterior predominance. This is a somewhat nonspecific appearance which could be seen in the setting of posterior reversible encephalopathy syndrome/acute hypertensive encephalopathy, though could also be seen with other etiologies including infectious processes such as progressive multifocal leukoencephalopathy or other encephalitides.
CDC Split Type:

Write-up: Patient received vaccine in the AM of 3/16/21. Patient was in his usual state of health when he went to bed the evening of 3/16/21. He ate dinner normally and his PD was started as usual. There were no overnight complications with his PD and he got up shortly before 8 AM. Mom reports the dialysate from his bag was clear and normal and none of his dwell was retained. She heard a loud thud from the bathroom around 8 AM and the patient was found by his mother having generalized convulsions (bilateral upper and lower extremities) and he appeared to be foaming at the mouth. He did not respond to her and she called 911. The seizure stopped by the time EMS arrived and his initial GCS was 12, and 15 by the time they started transport. He had another seizure en route around 9:30 AM and was given 2.5 mg versed and required bagging. Patient was loaded with Keppra and now has been on maintenance Keppra. Patient has not suffered any additional seizures and is currently undergoing an extensive workup by inpatient neurology team. Vaccine adverse effect currently remains on the differential for inciting event leading to seizure however additional labs are pending to rule out other causes (eg. viruses causing PML). Patient was also diagnosed with multifocal pneumonia and started on antibiotics at time of admission.

Write-up: Received the covid19 vaccine at 11am. Started feeling very cold around 2pm (nail beds were turning purple). By 4pm I had a headache, body aches, upset stomach, tired, fever, and chills. My fever would go up and down, highest was 103.2 reached that temp 3 times. By 2am my mother drove me to the ER. I was diagnosed with a migraine and was given IV fluids and an IV migraine cocktail. Write-up: Really bad hurting in chest pain, swollen lips and a headache. Had to go to ER. Forgot to mention the vaccine shot. They finally decided it was an allergic reaction. Gave a steroid shot and Benadryl. Sent home on steroids for 5 days. The next day had rash and itching. The next day it was worse looked like lace pattern rash over most of his body and really bad itching. Like this through the 20th then he took Claritin and put on anti -itch cream. The next day the rash had mostly cleared but still a little itchy. Today the 22 I have had a really bad headache.

Write-up: Client has previous allergies to cows milk and aloe. Reported her face was hot and swelling after vaccine. Family member in car with client confirmed her face was swollen as not visible initially. Client tearful and panicked. Reclined in car seat and Dr and Epi Pen called for. Dr spoke with client and examined the swelling, stating her Uvula was swollen. He administered an epi pen to left thigh muscle. 0.3ml . Blood pressure was 135/75 with pulse of 82 RR22 and oxygen saturation of 100%. EMS on site. Epi pen was administered on 12.10 pm. Client responded quickly and stated the she felt much better. second vital signs in 5 minutes were blood pressure 133/77 pulse 78 RR 18 and oxygen saturation of 97%. Transported by EMR to hospital.

Write-up: upon initial injection, pins and needles down left arm. Immediate onset of high blood pressure. Few hours later developed typical "flu like symptoms". Next day woke up and both arms, hands, legs and feet and side of face had pins and needles and felt numb. Heart rate sky rocketed and went to ER. Felt like I was going to pass out. Took blood work at hospital and doctors reassured that the feeling should come back in my legs and arms. When I left 6 hours later my legs were still pins and needles and my joints began to hurt. Write-up: Client complained of feeling dizzy with chest discomfort following vaccine. She also complained of a lump in her throat with swelling. and a headache. Write-up: I got sick to my stomach an hour after getting the shot and it lasted all night. I didn?t have much appetited. I had vomiting and diarrhea for seven hours. I had hives about an hour and a half after the shot all over especially and my feet. The hives were extremely itchy, and my skin felt like it was on fire. The next morning, I went to urgent care and gave me an anti-itch medicine/antihistamine Hydroxyzine. The prescribed medicine knocked me out, but it only helped a little with the itching. The hives stayed for another four days. I have also had severe joint pain that is still lingering in my feet and hands. I don?t normally have joint pain, but it started the day after the shot.

Write-up: first 2 days slight headache and body aches, third day through 10th day, nausea, vomiting on the days 9 and 10, stomach pain, hives. I went to the ER on March 19 for nausea and vomitting, was dehydrated and received IV fluids. I also had hives which I was prescribed Steroids for. The next day, March 20, I went to urgent care for severe hives and was given a steroid shot and epinephrine shot. Write-up: pruritus Narrative: 61 y/o female received COVID19 vaccine (Janssen) on 03/18/21 @0906. Has prior history to Shingrix vaccine on 02/2020 documented as causing diarrhea/facial numbness, but also had been exposed to Shingles prior to vaccination. Prior ADR was reviewed with RN and pharmD; was given the vaccine and observed for 30 minutes. @0930, was observed to be itching to back, legs, and left side of neck. @0935, diphenhydramine 50mg PO was given. @0944 itching continued and reports lips felt funny. @0947 patient reports feeling better and itching resolving, was observed until all symptoms resolved. Patient denied any shortness of breath. Patient stated symptoms of numbness/tingling in lips resolved. Patient left @1007. Instructed to seek emergency care if symptoms worsen or have any difficulty with breathing.

Write-up: 5:40pm - vaccine given 5:50pm - patients arm and legs begin itching, slight tingling of the tongue 6:00pm - Diphenhydramine injection given IM/RA 50mg/mL. BP 135/95, O2 Sat 96-97%. Dr was contacted via telephone (and continued on the phone until patient left the facility.) 6:25pm - Patient states that itching of extremities seems to be getting a little better, but tingling of tongue seems to be getting worse and now throat is tingling, voice sounds "raspy". Epinephrine 0.3mg was administered via EpiPen to the left thigh, at the direction of Dr. 6:40pm - Patient was transferred via ambulance to ER for observation.
