Write-up: This is a 57 year old male, received the Janssen COVID-19 vaccination on 5/4/21, at clinic in Jail, approx 10 minutes post vaccination, patient developed throat tightness, difficulty breathing and swallowing. He was immediately assessed by this writer, EMS activated as code. Given epi-pen to left thigh at 11:23, 50 mg of IM Benadryl to right deltoid at 11:25. Patient reported improvement in his throat tightness, shortness of breath and difficulty swallowing post epi-pen administration. Patient was sent to the ED

Write-up: I originally had breathing problems in January. I couldn''t catch my breath. My Oxygen was very low and I was hospitalized overnight. I was treated and released the next day. It reoccurred two weeks before the vaccine and COPD was ruled out. Two weeks after my vaccine I had breathing problems again. I went to Urgent care again with breathing problems and released after treatment. I am scheduled to see a pulmonologist and I am not sure if it is related to the vaccine. I was sick in March of 2020 and I am not sure if it was Covid-19. It seemed to be a mild disease. I have three episodes since I got the vaccine and all tests have been negative for definitive lung disease.

Write-up: extremely painful menstrual cycle with fatigue for one week began on 3/20/21 on 3/23/21 hands began to have pins and needles, difficulty with coordination when standing and walking and limited endurance. Moved to cramping in hands and feet. Needed a wheelchair to see neurologist. No peripheral nerve damage. Pt used tricycle to get around the school where she is employed. Pt continues to have limited endurance, poor balance and numbness in feet. Treatment has been limited to pycnogenol (natural blood thinner), she has been referred to a second neurological specialist. Finally able to get her in for an MRI next week.

Diagnostic Lab Data: They took my blood work - I don''t know the results; they did finger thing with my oxygen - and that was below normal when I got to the Urgent Care. By the time the epi-pen was done the danger zone was over - I was transferred to a Hospital after the URgent Care gave me the the IV. I don''t remember what Hospital. I was fine by then.

Write-up: Within 5 minutes, I felt like I couldn''t swallow. My tongue got so big that it filled my entire mouth; throat closed. I started choking on my own spit. My hands got really big. I couldn''t talk. The pharmacist told my friend to take me to the ER. I could kind of spit out stuff. She took me to Urgent Care. They gave me an epi-pen and instantly my tongue got small. Through an IV they gave me: Prednisone, Benadryl and fluids - by the time the IV was done I felt 90% okay. Four about 4 or 5 days - I would get fat fingers and itching. I would still have trouble swallowing - kind of choked when I swallowed. About 5 days after the event the allergy part was gone. I had three days of an oral steroid I was suppose to take and two weeks of Zyrtec. I did have flu-like symptoms also on the night of the vaccine. I had COVID in December 2020.

Write-up: Thursday late night started have pain in both feet. Friday pain getting worse. Saturday could hardly walk. Saturday night was getting unbearable. Temperature went from 97.6 to 100.3, then 100.8 and had chills and fever. Extremely rare for me to have chills, etc. Sunday could not walk on feet. Used wheelchair to get to bathroom, etc. Sunday night, I called the urgent care who recommended me to ER. 03/14 admitted to ER. ER did xray of foot to eliminate break. Concluded POSSIBLE cellulitis. Treated with antibiotics. kept overnight for observation. Monday 03/15 pain was severe but could walk to bathroom. Released Monday 03/15 at @ 5:00 pm. Pain subsided after 4 days.

Write-up: At first on April 23rd, my tongue felt like it was burned and tingling. By 4/26 I woke up with right side of face dropping, swollen, and paralyzed. Went to the ER and was diagnosed with Bells Palsy. ER doctor also performed CT scan which was normal. Was given prednisone and valacyclovir, instructed to protect and dress my eye. The right side of my neck under my ear is very swollen and tender. I have pain in the back of my neck on the right side. I had a follow up with PCP 5/05/21. She told me to use an eye ointment. It has been two weeks and my face is still paralyzed. Some of the swelling has gone down in my check.

The patient''s height, and weight were not reported. The patient''s past medical history included congestive heart failure, and atrial fibrillation, and concurrent conditions included diabetes, and other pre-existing medical conditions included patient has no allergy or illness. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 1802070, expiry: UNKNOWN) dose was not reported, 1 total, administered on left arm on 08-MAR-2021 for prophylactic vaccination. Concomitant medications included ergocalciferol, magnesium, and mecobalamin. On 06-APR-2021, the patient woke up with lost of vision in left eye for 4 to 5 hours. Patient went to her ophthalmologist; the sight came back but very blurry. Retina specialist had seen patient on 13-APR-2021 and diagnosed transient ischaemic attack of left eye. The patient was guided to go to ER on 13-APR-2021. On 06-APR-2021, the patient experienced blindness unilateral, vision blurred. The patient was admitted in the hospital on 13-APR-2021 and was hospitalized for 3 days. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: Unknown) frequency 1 total, dose not reported, on approximately 08-APR-2021 for prophylactic vaccination.The batch number was not reported and has been requested. No concomitant medications were reported.The patient was not pregnant at the time of report. Patient had the Janssen Covid-19 vaccine on approximately 08-APR-2021 and started experiencing side effects on 14-APR-2021 (six days later) which included severe headaches, nausea, left arm numbness, cramping in one leg, and chest pressure. Patient went to Emergency room several times for these symptoms and was discharged each time. Patient also had numbness under left eye and was admitted to hospital on or about 16-APR-2021 or 17-APR-2021 to rule out blood clot on MRI and was diagnosed with arachnoid granulation and released on 20-APR-2021. Patient was hospitalized for approximately 3-4 days. Write-up: First night had chills, very slight nausea and low grade fever (99.3) the next two days. On the morning of April 7th I woke up with a swollen lip. Throughout the day the swelling continued on the left side of face, then on bottom of right foot and later on top of both feet. Went to emergency room and was treated for hives. Over the next five days, the hives kept moving around, the right side of face, and then to upper lip. I followed up with my Primary Care Doctor, who did extensive allergy testing. All came back negative. I felt that it was a result of my body reacting to the vaccine. In addition, I was feeling weak muscles and joints that kept moving around.

Write-up: Within 5 minutes of receiving vaccine, patient was hot and covered in a full body sweat, seeing spots, kept losing consciousness, nauseous. I couldn''t get a reading on the BP monitor at that time as it was too low to read and she couldn''t move her limbs. While waiting for EMS she could breathe fine, I could feel a faint pulse, but I had to keep taping her leg and talking to her to keep her awake. She was able to tell us her name and how many fingers I was holding up. I kept trying to get her to squeeze my hand, but I could only feel the tips of her fingers twitching and the whole time she was unable to move her feet/toes. I was able to look at her HR on her watch right before EMS arrived and it was at 62 and by that time her body temp seemed to be back to normal as her Mom said she felt cool and clammy. The BP reading by EMS was 100/60 and they tried to get her to stand but she couldn''t and she still kept wanting to roll her head back and sleep.

Write-up: I received the vaccine on 4/2/21. I started feeling tightness, soreness and throbbing pain in my right calf on 4/28/21. On 5/4/21 the pain was unbearable and there was swelling in my calf/ankle. Write-up: Within 10 minutes patient requested Benadryl. She felt a strong tingle sensation. Her skin was warm to the touch. Blood pressure elevated at 152/98. Two doses of Benadryl 25mg given. One at 1:18 pm and one at 1:55pm. Benadryl worked well and her skin was a normal temperature but she described a tightness in her chest. She took two puffs of albuterol 90 mcg and the nurse at the Dr. office stated that she needed to go straight to the emergency room for evaluation. at 2:30 pm she should no more signs of an allergic reaction or any signs of anaphylaxis but reported a headache. Husband picked her up and was instructed to go to emergency department for evaluation.

Write-up: Severe migraine headaches, dizziness, painful canker sores on roof of mouth, numbness on right side of body from toes to head, brain fog, light sensitivity, intermittent ringing in ears, nausea, vomiting, fatigue, chills/fever, severe body aches. Write-up: Within 17-20 minutes of receiving the vaccine, I felt a burning, hot sensation in my throat. I let paramedics know and sat for about 10-15 more minutes. My mouth and throat became extremely dry and I had a tingly feeling in my feet. I also experienced high anxiety. An ambulance came and proceeded to bring me to the nearest hospital. In the ambulance, I began coughing, which caused tightness in my lungs and moments of needing to cough. My vitals were normal in the ambulance and I was given IV fluids. I arrived at the hospital around 4:30 pm. When checked by triage about 2 hours later, the nurse reported my vitals were normal, and that I shouldn''t need epinephrine or lab work. She reported that "hopefully a triage doctor would be able to see me soon." I continued to cough into the evening, and began wheezing from coughing. I waited in the ER waiting room until just before 10pm. Triage could not give a time estimate of when I could be seen. I needed an inhaler or nebulizer treatment long before. I left the hospital after the nurse removed the IV port. At this time, the nurse reported my "rash on my chest was gone," although she did not mention a rash earlier in the evening and I did not notice it (had a jacket on). I did not need to sign or receive discharge paperwork because I was never seen by a doctor. I took Benadryl and gave myself a nebulizer treatment once I got home and all adverse reactions were gone the next morning. I experienced flu-like symptoms the next day.

Write-up: 1st few days expected to be sick. Diagnostic Lab Data: The ER team put me on an IV for 2 hours during which time they injected into me a bag of saline and high doses of Ibuprofen and Tylenol to lower my temperature. They put me on a regimen of prescription level Tylenol and Ibuprofen for the next 48 hours, which I ingested every 3 hours. Two days later I felt back to normal with no additional side effects.

Write-up: Six hours after receiving the vaccine shot my teeth started chattering with such intensity I could hardly speak, and my arms started to shake as if I were having a mild seizure. Write-up: For 2 days, typical arm soreness, flu-like symptoms with significant muscle pain and general weakness and malaise. However, on the 5th day, EXTREME abdominal pain, left side. Pain level up to 9. Incapacitating. Two visits to urgent care. This was in March before any info came out about adverse blood clot events for the J&J vaccine, so the doctors were not really interested at the time that I had just gotten my shot. They gave me antibiotics, thinking it may be diverticulitis (diagnosis based on symptoms only - no diagnostics - it was the doctor''s "best guess"). Pain did not subside, only got worse. 4 days later I could barely move, abdominal pain was extreme. Went back to clinic and they did a PET scan to check for diverticulitis. There was none; PET scan revealed nothing out of the ordinary. So they told me they had no idea what it was and sent me home. PET scan reading by the doctor notes specifically that blood/vascular was not able to be observed because no contrast or dyes were used. Bottom line was over 10 days of extreme abdominal pain 5 days after receiving the shot. I''ve never experienced anything like that before.

Write-up: On 4/30/2021 I recieved the Covid-19 vaccine and on 5/1/2021 I started with upper abdominal pain and nauesa. It went away and came back on 5/3/2021, went away and came back on 5/5/2021 with the pain and nausea intensifying. I went to ER on 5/6/2021 at 1 am. When I arrived to the ER they proceeded to do bloodwork, also ordered an x-ray to ensure that my lungs were not causing an issue due to where pain was located. After the labs came back they saw my liver numbers were extremely high, so then they ordered a CT scan with contrast. CT scan showed everything to be normal. They then determined I needed to be admitted due to believing I had a stone in my bile duct, Gallbladder was removed in 2019. The MRI was done at 7pm on 5/6/2021. The results were recieved on 5/7/2021 and showed my bile duct was clear of obstructions. They continued to monitor my liver numbers and I was released today 5/9/2021 due to my numbers continually decreasing with instructions to follow-up with my PCP to have labs run again to ensure my numbers remained on decline to the normal range. I was also instructed to do the adverse event reports as to them not having enough data to determine if the vaccine may have been the reason for my high numbers. My PCP had just had my yearly labs done on 3/16/2021 and my liver numbers were normal.

Write-up: Received Vaccine on 4/8/2021 and had Immediate Vertigo 5 minutes after shot. I drove home scared because I was very dizzy. It is 1 month today and I still cannot drive. Left side of face went numb for a few hours on that day as well. I now have left side weakness, dizzy, off balance and lightheaded. I get occasional bouts of nausea. Write-up: Approximately 8 minutes post-vaccination patient started to feel dizzy. Patient then got out of his chair to come tell the pharmacist (myself), during which time I witnessed him starting to sway and attempted to approach the patient. Before I could make it to the patient he passed out and fell to the floor, his bottom hit the floor first followed by his torso. I told the intern to call 911 and stayed with the patient. He never stop breathing and a pulse was always present. He attempted to get up moments after falling and I requested he stay on the ground. Write-up: SEVERE CHEST PAIN ON RIGHT RIBCAGE AND LEFT HAND SIDE; This spontaneous report received from a consumer concerned a 71 year old female. The patient''s weight was 120 pounds, and height was 64 inches. The patient''s concurrent conditions included rheumatoid arthritis since approximately 14 years, penicillin and IV contrast solution allergy, no alcohol use, and non-smoker, and other pre-existing medical conditions included the patient was not pregnant at the time of reporting and had no history of drug abuse or illicit drug use. The patient received covid-19 vaccine ad26.cov2. s (suspension for injection, intramuscular, batch number: Unknown, and expiry date: unknown) dose was not reported, 1 total administered on APR-2021 for prophylactic vaccination. The batch number was not reported and has been requested. Concomitant medications included adalimumab (Humira) for rheumatoid arthritis. On APR-2021, four to five days after the vaccination, the patient experienced mild soreness or discomfort initially and progressively worsened over time. Later the symptoms developed into a severe chest pain on right hand side of the ribcage, and it hurt to breath and move; the patient felt relieved only while lying flat. The patient had visited emergency room and physician room for severe chest pain on right ribcage and left-hand side. The patient went to the hospital after 6 to 7 days after the vaccination and got admitted. The patient was put on blood thinner immediately after the admission as the staff thought the patient had blood clot. On performing D-dimer test, the levels showed 3.49 (units unspecified) and was identified that the patient had no clots. The patient was discharged from the hospital on 28-APR-2021. The patient was pain free for 2 days; However, she started experiencing severe pain on left hand side of the chest on the day before reporting. The patient stated that the pain moved over. Write-up: During 15 minute observation period, patinet fell from chair to carpeted floor with seizure lasting 4 mintues, airway maintained, EMS activated. Write-up: Later during the night after the vaccine I developed a high fever, body aches for about 24hrs which caused me to sleep the next day. 2 weeks later I developed a bacteria infection in my right eye and possibly bronchitis so I went to the Urgent Care and was treated with Z-Pack and steroids. I feel alot better but I still have a little lingering cough. This past Friday I woke up with a pain in the right side of my neck which got worse so I went to my PCP on Sat who diagnosed me with Carotidynia where he states my corotid artrery is inflammed and was prescribed an antiinflammitant Diclosenac with the intents of decreasing the inflammation.

Write-up: Patient developed acute onset bilateral anterior thigh pain that radiated to the lower back and buttocks, as well as in the posterior hamstrings. The pain was 10/10 and started suddenly over 1/2 days, described as sharp and stabbing in nature. He was able to ambulate but had extreme pain with hip flexion. Associated with this had unintentional weight loss of 10 lbs over 3 weeks, loss of appetite, drenching night sweats for 4 days prior to admission. He presented to the ED on 4/16 with negative work up and was discharged. He re-presented on 4/24 with the same symptoms and was admitted until 5/3. His pain was managed with oxycodone and extensive work up was pursued given concern for systemic inflammation with leukocytosis, elevated CRP, ESR, but normal CK. Eventually pursued thigh muscle biopsy which showed ". At last point of contact at 5/7, he described having two more episodes of bilateral thigh pain after discharge that lasted 30 min in the mornings, but had not had any symptoms the day prior to the call.

Write-up: I felt dizzy and had problems with breathing right away after the injection. A doctor from that site who checked my vitals wanted to call an ambulance to take me to a hospital. Diagnostic Lab Data: I went to the ER on April 14th for uncontrollable vomiting and severe back pain. Ended up going back on 4/15 due to dangerously high BP and BPM. Write-up: Same day of vaccine around 3:30pm I started to shiver I showed a fever of 99 by 4:00pm I had incontrollable shaking with a fever 104.2 fever and sever headache. I had muscle pain on my back and neck. I had violent vomiting multiple times. I felt asleep and about 2 hours later I woke up and I had an SVT episode where my heart rate was beating was up to 200 by then the fever had gone down to 99. I called the nurse line.. they told me to call an ambulance. I called the ambulance they took me in they immediately put fluids they monitored me for 7 hours and they sent me home afterwards. I slept for 24 hours. Then on 4/27/2021 I had another SVT episode where my heart rate went up to 176 beats per minute. This lasted for 2.5 hours but I did not have to go to the the ER.

Write-up: On 4/19, I woke up with severe abdominal cramping, vomiting and diarrhea. The diarrhea turned to bright red blood, so I went to the ER. I was diagnosed with severe sepsis and colitis. Although I was diagnosed with sepsis, I never had a fever and all blood cultures came back negative. Before I was released, I went into uncontrolled atrial fibrillation. Write-up: The patient developed headache approximately 2 weeks after receiving her vaccine and was found to have intraparenchymal hemorrhage (i.e. hemorrhagic stroke) and intraventricular hemorrhage in the setting of thrombocytopenia with a platelet count of 47 thousand. She was intubated and mechanically ventilated. Platelet count normalized 9 days after admission. She underwent ventriculoperitoneal shunt placement 11 days after admission and was extubated 4 days later, then transferred out of the ICU 2 days after that. Course complicated by pulmonary embolism diagnosed 4 days after she left the ICU which is being treated with apixiban. She has a right hemiparesis and continues to have dysphagia and dysarthria. She is dependent on NG feeds for nutrition. Anticipate discharge to long-term acute care within the next 2-3 days.

Write-up: The days after the shot the patient was sleeping 12-14 hours a day which was not normal. On 04/15 pt started to feel dizzy and signs of blurry vision began. They went their general physician ruled out stroke & clot, then recommended they see an eye dr since the vision was the worst symptom. By 04/21/2021 the pt lost 100% of their eye sight. They eye dr after doing a scan discovered the pt had temporal arteritis. They believe the vaccine could had caused the body to lose the fight against the temporal arteritis and pt eyesight was lost.

Write-up: The night of the vaccine I had muscle aches and I started gagging. The muscle aches were better in a few days. I went to the Emergency room the night of the shot. I was diagnosed with a bladder infection. The urge to gag was constant. I was given some nausea medicine in the ER and it helped a little I became dehydrated after a week. Write-up: After my J&J vaccine at night I experienced fever, chills and body aches. The next day when I woke up I felt a lump on my neck. I went to ER on 04-09-2021 where they examined my neck and they told me that my lymph nodes are swollen and the ER doctor told me that that was a reaction to my vaccine and that it would go away in a few days. Write-up: Pt was given Janssen shot at 9:40am. Within 3-4 minutes, pt complained that she felt tingly, lightheaded, faint, and had issues breathing. She was instructed to sit in a chair along with her husband by her side. In the meantime, RPH called for help in the store. Store PIC came over and called 911. After the PIC was on the phone with 911 for 2-3 minutes, the pt complained of trouble breathing and faintness. She was then administered an epi-pen. She started to feel a little better, less faint and could breathe a little easier (maybe 10% improvement) but not great. After 5 minutes, the paramedics had not shown up yet so she was given another dose of epipen because she started to have more faintness, lightheadedness, sweatyness, tingly and SOB. 3-5 minutes after 2nd dose of epi was given, paramedics then arrived to the scene. They took her BP and it was ~ 76/40. They took her away in the aid car.

Write-up: SUSPECTED CLINICAL VACCINATION FAILURE; METALLIC TASTE; SUSPECTED COVID-19 INFECTION; This spontaneous report received from a patient concerned a 60 year old male. The patient''s height, and weight were not reported. The patient''s concurrent conditions included seasonal allergies, and the patient have no other health history. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, Frequency: 1 total, batch number: 1808978 expiry: Unknown) dose was not reported, administered at left arm on 26-MAR-2021 for prophylactic vaccination. No concomitant medications were reported. On 27-APR-2021, the patient experienced suspected covid-19. On 01-MAY-2021, On the same day the patient underwent rapid swab test and tested positive for COVID-19 (suspected clinical vaccination failure, Stuffiness, Runny nose, Drainage, Congestion, Anosmia, Ageusia). It was reported that, the symptoms started about 4 days prior, but it was hard to discern due to allergies. On the same day, the patient went to gym for about 45 minutes and felt okay except for slight metallic taste in mouth. Later, he went home and tried smelling whiskey and realized he had lost his sense of smell and experienced loss of taste. The patient did not present with typical symptoms and had felt okay because he had been working out prior. It was reported that, the patient could smell firewood burning but nothing else. Later, all vitals checked at urgent care and all was normal. The patient''s laboratory data revealed blood pressure was normal, Oxygen level on Oximeter 99%, Smell and Taste was 90-95% gone. At the time of this report, the patient had a tiny amount of fatigue and was in quarantine for the next ten days. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient recovered from metallic taste, had not recovered from suspected covid-19 infection, and the outcome of suspected clinical vaccination failure was not reported. Write-up: SUSPECTED SINUS INFECTION; TIGHTNESS OF NECK; BODY SPASMS; UNABLE TO SLEEP; SEVERE PRESSURE HEADACHE; DIFFICULTY CONCENTRATING; LIGHT HEADEDNESS; PAIN AROUND LEFT EYE; TIGHTNESS OF FACE; NUMBNESS OF FACE AND NECK; This spontaneous report received from a patient concerned a 37 year old male. The patient''s height, and weight were not reported. The patient''s concurrent conditions included seasonal allergies, and other pre-existing medical conditions included the patient had no known drug allergies.The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 042A21A, and batch number: 042A21A) dose was not reported, administered on 06-APR-2021 for prophylactic vaccination. No concomitant medications were reported. On APR-2021, the subject experienced numbness of face and neck. On 12-APR-2021, the subject experienced severe pressure headache. On 12-APR-2021, the subject experienced difficulty concentrating. On 12-APR-2021, the subject experienced light headedness. On 12-APR-2021, the subject experienced pain around left eye. On 12-APR-2021, the subject experienced tightness of face. On 30-APR-2021, the subject experienced body spasms. On 30-APR-2021, the subject experienced unable to sleep. On 30-APR-2021, the subject experienced tightness of neck. On 04-MAY-2021, Laboratory data included: CT scan (NR: not provided) CLEAR. On an unspecified date, the subject experienced suspected sinus infection. Treatment medications (dates unspecified) included: amoxicillin trihydrate, naproxen, azithromycin, and paracetamol. The patient had not recovered from severe pressure headache, difficulty concentrating, light headedness, body spasms, unable to sleep, numbness of face and neck, tightness of neck, pain around left eye, and tightness of face, and the outcome of suspected sinus infection was not reported. Write-up: BLOOD CLOTS IN LUNGS/BLOOD CLOTTING; VACCINE-INDUCED THROMBOTIC THROMBOCYTOPENIA; This spontaneous report received via company represenative, via a social media and concerned an adult male ''below 50''. The patient''s weight, height, and medical history were not reported. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: Unk) unknown dose, in APR-2021 for prophylactic vaccination. The batch number was not reported and has been requested. No concomitant medications were reported. In social media, it was reported that a third male has been treated for rare blood clotting that may have been caused by Johnson & Johnson vaccine. The patient first showed signs of vaccine-induced thrombotic thrombocytopenia around 10 days after receiving the vaccine in APR-2021. After a week of leg pain, he went to emergency room where he was placed on blood thinners (unspecified) and sent home. The patient returned next day with chest pain which led to a discovery of blood clots in his lungs. After a few days on blood thinners, the patient''s platelet count was normal and was returned home where his condition continued to improve. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient was recovering from blood clots in lungs/blood clotting and vaccine-induced thrombotic thrombocytopenia. This report was serious (Hospitalization Caused / Prolonged).; Sender''s Comments: V0: This case reported in the media concerns an adult male under 50 years of age who experienced vaccine-induced thrombotic thrombocytopenia approximately 10 days after receiving the Janssen Covid-19 vaccine. After a week of leg pain, the patient went to the emergency room where he was placed on unspecified blood thinners for an unspecified indication and was sent home. The next day, the patient returned with chest pain which led to the discovery of blood clots in his lungs. After a few days on blood thinners, his platelet count was normal and he returned home where he is recovering. Additional information (e.g. height, weight, full medical history, social history, family history, concomitant medications, specific platelet levels, peripheral smear, D-dimer, fibrinogen, anti-platelet factor 4 antibodies, radiologic diagnostic test results such as ultrasound, CT, MRI, angiogram, or venogram) was not provided. Information is limited in this case. However, based on evolving knowledge of Thrombosis with Thrombocytopenia Syndrome (TTS, per definition) and considering the reportedly low platelet count and temporal relationship to vaccination (Criteria level 3), the events are assessed to have a plausible relationship with vaccination. Write-up: My appointment was on time, 4pm on 4/3 at the Pharmacy. The young girl that gave me the shot told me she was certified, and she gave me the shot in the presence of her manager. I have given myself shots for years with diabetes, plus many flu shots, and have never felt pain like that. I expressed this to her, how painful it was, and she just said "I know, I know..." as if it was normal for the shot to be so painful. I knew instantly something was wrong. They were more worried, it seemed, about a potential adverse reaction such as an allergy shock, since I''m allergic to a few things, but I did not experience that. I waited 15 minutes, my arm hurting the whole time, and they suggested I go buy Tylenol for the pain, which I did. My left arm has hurt since then. I got a horrible bruise from this shot (will send picture) that took weeks to heal. Since then I''ve continued to feel pain and don''t have full mobility of my left arm. I can type, but not lifting anything heavy and have to go easy with anything on the left side. I thought at first it might be what people were calling "covid arm" but I discovered that was more about soreness, not pain. I tried ibuprofin, but after three weeks I''d had enough and made a doctors appointment. Since I was also noticing my left shoulder was very tense and tight, and I was starting to get neck stiffness in same area, I started googling before my appointment. When I entered my symptoms, SIRVA kept popping up, so I wondered if that was what I now had because of the improper vaccine administration. I asked my doctor for an MRI order to find out, but she said we needed to do an Xray first (which only showed mild issues related to osteo). She offered a prescription of taking Advil 400 mg three times and day and to use a muscle relaxer when I needed it, such as at night (Cyclobenzaprine). She said to try that for a week, and if it wasn''t better, that another Dr could order MRI. Since then I''ve also added a pain spot under my left shoulder blade. Insurance called and said the order was "under review" so that''s where things stand now. If they don''t approve, I guess I will go next to an orthopedist for guidance? This has cost me health, due to lack of sleep also - I wake up with pain in the middle of the night, even when I take the meds. Praying for a diagnosis, and solution, soon.

Write-up: Janssen COVID-19 Vaccine EUA Bilateral Bell?s Palsy April 28th- I lost taste and my tongue felt burnt and had a coating on it. April 29th- Morning, I noticed my top lip was off-center. Later that evening, I felt numbness around my lip and mouth. April 30th- Lost muscle function on left side of face, including around left eye. Went to Urgent Care, they diagnosed as Bell?s Palsy and prescribed a high dose steroid and antiviral medication. May 2nd- Called doctor because right eye was twitching and muscles were feeling very tight. They thought it was my eye over compensating May 4th- Lost muscle function on right side of face. Went to Urgent Care to be re-evaluated. They suggested I go to the ER. Went to ER, bloodwork, CT scan and MRI all came back normal. They believe it is bilateral Bell?s Palsy
