Write-up: After patient received the vaccine of Janssen I saw a hand shake. When I was going over side effect patient''s hand was shaking more. I asked the patient if hand shaking was normal she stated no and she passed out. I was able to her upper body from falling. I asked her husband to hold her and I got an epinephrine 0.3mg Lot # 026a20aa exp 02/22. I injected epi into her left thigh. patient woke up after 20 second but she didn''t have any memories what happened

Write-up: PULMONARY EMBOLISM; PAIN IN CALVES; TIRED; This spontaneous report received from a patient and patient''s mother and concerned a white, not Hispanic/Latino female of unspecified age. The patient''s weight, height, and medical history were not reported. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 043A21A expiry: UNKNOWN) dose was not reported, administered on 05-APR-2021 in the right arm for prophylactic vaccination. No concomitant medications were reported. On 12-APR-2021, 7 days after receiving the injection, the patient experienced pain in calves, felt tired and had hard time breathing. She went to the doctor, and unspecified tests were run. On 20-APR-2021, the physician called and told her to go to the ER (Emergency Room) right away, which is when the patient was diagnosed with pulmonary embolism, further described as right below the right lung. The patient''s mother took the patient back to the ER a few days later. The patient now "seems better", and is able to breathe better again, but was still having a hard time breathing. Treatment medications (dates unspecified) included: XARELTO (rivaroxaban). The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient was recovering from tired, and had not recovered from pulmonary embolism and pain in calves. The patient''s height, and weight were not reported. The patient''s past medical history included right leg amputated below the knee, and concurrent conditions included varicose veins in legs. The patient was previously treated with acetylsalicylic acid for right leg amputated below the knee. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 041A21A, expiry: UNKNOWN) dose was not reported, administered on 07-APR-2021 for prophylactic vaccination. No concomitant medications were reported. On 10-APR-2021, the subject experienced superficial thrombosis of leg. Write-up: Brief HPI and Hospital Course: 67 year old male with unknown past medical history was brought in by EMS as post-cardiac arrest. Patient was found on unresponsive on sidewalk, possible collapse. ACLS started by EMS, approx 10-15 minutes. Initial rhythm was junctional PEA. Was given 2 epi and intubated by EMS and had ROSC, Initial ROSC rhythm was SVT which slowed to 90s on its own. On arrival to ED, left femoral central line placed. Tox screen was positive for cocaine and ETOH. Was placed on ACS protocol for possible NSTEMI. Tested positive for COVID19 on admission. Patient required pressor support on admission. Hypothermia protocol was initiated. Patient was rewarmed per protocol, normothermic as of 5/5. Brief Narrative of Events leading to Patient''s Death: Patient remained normothermic with persistent septic shock, multi organ failure, ARDS secondary to COVID19. Multiple attempts to reach family members made by ICU team and palliative care team daily without answer. Patient made DNR by two-physician consent. Patient''s BP low despite maximum levophed support.

Write-up: Woke up with heart racing/pounding on 04/05/2021, went to ER where blood pressure was 193/110, heart rate 108, given IV meds-metoprolol and discharged with instructions to see PCP and cardiologist. Patient states he saw Dr. who said he "didn''t know what to do", sent to cardiologist who put patient back on metoprolol 12.5mg. Patient reports he continues to have extreme fatigue with minimal exertion.

Write-up: Received shot at 10:00am on 5/5/2021. On 5/6/2021 Patient began to feel ill with fever, chills and body aches. Spent most of that day in bed. On 5/7/2021 presents to Clinic, fevers, chills and body aches had improved but noticed "rash" - right lower extremity was red, hot to touch, swollen, pain with "tightness" and "burning" sensation at site. Denies itching. Right Lower Extremity well demarcated area of erythema, hot to touch and remarkably TTP, particularly at anterior surface of leg. Erythema extends on anterior surface of leg from ankle up to 8cm inferior to the knee cap ;the erythema on the posterior surface of the leg extends from ankle up to about half way up the calf. Erythema is confluent. Anteromedial surface of the skin has diffuse petechiae. Negative Homan''s sign and no calf tenderness, 2+ pitting edema from ankle to midway up the shin.

Write-up: My feet were very cold starting about a week after the vaccine. I noticed twitching and numbness in my feet three weeks after the vaccine. I started taking a new medication Nortriptyline at the same time. I stopped the new medication and the symptoms persisted. I went to the the ER to rule out Guillen Barre syndrome. The numbness and tingling were working its way up my legs. It has been ten weeks and the feeling of numbness and tingling is as far up as my knees on both legs and I am having symptoms in my wrist. My feet are burning at night and it has been getting worse. I have seen specialists and I have a neurology consultation. My doctor suspects Chronic Inflamed Peripheral Neuropathy after ruling out RA, Lupus Multiple Myeloma and Diabetes.

Pt received the Johnson and Johnson (Janssen) COVID vaccine on 04.02.21 at the Pharmacy (lot# 042A21A). The pt was on warfarin prior to admission for diagnosis afib and therapeutic (INR 2.2) at time of admission so he was placed on heparin high intensity infusion. Pt subsequently developed hemoptysis so heparin infusion was held and pt was transferred to the hospital for hematology/pulmonology/oncology consult. At accepting facility heparin was held on admission and was restarted on 05.02.21.

Diagnostic Lab Data: Her initial EKG showed NSR with right axis deviation and TWI in V1, V3. Her initial troponin was 5.5, BNP 100. Her TTE showed severe concentric LVH, grade I diastolic dysfunction, normal LV/RV size and function, no significant valvular pathology and small pericardial effusion without tamponade physiology, IVC with $g50% collapse with inspiration. She was started on heparin drip initially, aspirin, and atorvastatin. On 4/20 PM, she experienced worsening chest tightness with two episodes of emesis. Troponin continued to rise and peaked at 31, CK-MB 67.7. EKG at this point showed new STE in I and aVL, STD in leads III and aVF and an incomplete RBBB. Repeat TTE with normal LV size/function but possible hypokinesis of the apical anterior and apical lateral walls, stable small pericardial effusion. Decision was made to transfer to Hospital (from sister hospital) for further management on 4/21/21. Admission EKG showed sinus tachycardia, right axis, RBBB, 2 mm ST elevation in I and aVL, TWI in V2, V3. On 4/21, she underwent LHC/RHC and RV Biopsy: Normal coronaries, LVEF 25%, LVEDP 20, RA 7, RV 25/7, PA 25/16/19, PCW 16, CO 2.8/ CI 1.47 by Fick, PVR 1.1 WU. Congestive heart failure team was consulted for new heart failure. Cardiac MRI 4/21 showed extensive late gadolidium enhancement as above with a non-ischemic cardiomyopathy such as infectious or immune myocarditis, sarcoidosis or, less likely, burnt-out hypertrophic cardiomyopathy or an infiltrative process. EF 40-45%. She was started on pulse dose steroids steroids (4/23-4/24). EMB 4/22 RV showed myocardial tissue with mildly increased interstitial fibrosis, highlighted by trichrome stain. No inflammatory cell infiltrates or granulomas are seen. On 4/23, symptoms began to improve as did the ST elevations and ST depressions. GDMT was initiated. On 4/26, she underwent a PET-MRI to evaluate active inflammation. Colchicine started for possible pericarditis. She had a negative respiratory viral panel.
CDC Split Type:

Write-up: Initially presented with 5 out of 10 chest pain (substernal, no radiation, no exacerbating or relieving factors), nausea, vomiting, diarrhea, dyspnea on exertion, and fatigue. Further history obtained included decreased exercise tolerance since four weeks prior to presentation (noticeable during runs). She was found to have an abnormal EKG at an urgent care and was sent to an outside hospital emergency department. She ultimately was found to have heart failure with preserved ejection fraction (56%), Stage C, Class III, and myocarditis. The highest suspicion for the etiology of her myocarditis is viral vs. autoimmune (given response to steroids) vs. genetically-mediated myocarditis. She was discharged on aspirin, a prednisone taper, colchicine, metoprolol, and Entresto with cardiology follow-up. Patient discharged with 4 week monitor to monitor for arrhythmia as per EP service. Infiltrative cardiomyopathy evaluation pending as well- plan for SPEP/UPEP, serum/urine protein electrophoresis, immunofixation, and alpha-galactosidase (not sent due to specific tube not available).

Write-up: She got the vaccine, she was fine at first, sent her home at 11:45, by 11:50 she started getting itchy around her lips, then she got tingling and numbness in her left cheek about 5 minutes later. She then started to see slight swelling in the left cheek by 12:00. She then took 2 Benadryl. By 12:10 she had tightening of the throat, and by 12:20 she was in the ER at Hospital, and at 12:40 she was seen and they gave her Pepcid at 1:00 PM on top of the Benadryl. She is currently in observation waiting to see the progress of the reaction. She is feeling better now, she was not intubated.

Write-up: I experienced had recurrent headaches 04-02-2021 lasting to 04-22-2021. I did not work 04-02-2021, 04-03-2021, 04-04-2021 and also on 04-14-21 and 04-15-2021. I also had some nausea that came and went in the time I experienced the headaches. n 04-08-2021 had a telehealth visits about my headaches and recommended I take two Benadryl tablets to break the cycle of the headaches and try it for 3 days. Write-up: The day after the vaccine I had a low-grade fever and felt bad. I was extremely fatigued and I felt weak. It almost felt like the flu. I had nausea at night and I felt flushed. I consulted with Urgent Care and they asked me to come in and get a COVID-19 test which was negative. Those symptoms lasted a full week. My period is also out of sync and it lasted longer than usual for a total of two weeks. It kept coming and going. It normally only lasts five days and it is consistent. Write-up: Five days days after the vaccine I pain in my chest and all the way around my back on left side. The pain was intermittent and it seemed to move. I went to the the emergency room and they did some tests and the results were normal. I have been taking some Tylenol and it helped a little. Write-up: Was diagnosed with a DVT 3.5 weeks after vaccination. Started experiencing symptoms of DVT 20 days after the vaccine. Symptoms were not atypical and were almost not checked for DVT at ER. Started with twinges in my right leg for several days. Then pain in my back right behind my heart. Then developed sporadic chest pain and heart palpitations. I was afraid to go to the ER because of COVID because my 12-year-old daughter has a heart condition. Then on April 30 mid-day my right thigh began to hurt like a pulled muscle although I had not injured it or pulled it. I couldn''t walk on my leg the pain was so intense. My family was very worried about a blood clot because of the Johnson and Johnson vaccine. That night I had really sharp chest pains and my thigh pain was no better by the next morning. I went to the ER told them my symptoms. After the ER doctor examined me he said he was 99% sure I didn''t have a blood clot because my pain was in my thigh and there was no redness or swelling. He told me my chest pain was probably due to anxiety. I asked if he would check my leg anyway. Once the ultrasound of my leg was done the DR came back and said I had a DVT. Then I had to have an MRI of my chest which he told me was a lot of radiation. I did not have a PE. They did not prescribe Heparin because they felt the DVT was from the vaccine. So I was giving Eliquis. Since then my family DR has run more tests and I do not have any genetic reason to have the DVT. I do not and have not taken birth control in the past 20 years. I am now on a blood thinner for at least 90 days if not longer and I am continuing to have twinges and chest pain twinges.

Write-up: After receiving her shot, the patient was walking the store aisles when she felt weak and dizzy. She was helped to a seat by a nurse and a paramedic who happened to be walking in the aisles at the same time. she responded fairly well to a mental state exam and was asked if she has a history of illness or chronic health conditions to which she replied no. Write-up: PAIN IN UPPER BACK; CLOT IN LUNG; CLOT IN CALF; DIZZINESS; FLU LIKE SYMPTOMS; JOINT PAIN; STOMACH ACHES; SWELLING IN CALF; This spontaneous report was received from a patient and concerned a 55 year old male. The patient''s height and weight were not reported. The patient''s past medical history included fractured ribs and T6, and concurrent conditions included drug allergy to Vicodin (causes rash). The patient was healthy before the vaccine. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, intramuscular, batch number: 042A21A expiry: 21-JUN-2021) dose was not reported, administered on and unknown date in APR-2021 (approximately "middle of April") in the left arm for prophylactic vaccination. There were no concomitant medications. The day after receiving the vaccine the patient started experiencing fever (no actual temp given), stomach aches, chills, joint pain, "flu-like symptoms" which lasted "4 to 5 days after getting the vaccine" (no actual dates given). He stated those symptoms went away. On the "10th day after getting the vaccine" he experienced pain in his calf and had a hard swollen spot which he assumed was cramps. This lasted until Sunday, 02-MAY-2021. On Monday, 03-MAY-2021 (also reported as 02-MAY-2021), the patient started experiencing pain in upper back which he felt "in his lungs". On Tuesday 04-MAY-2021 , the pain was still present. On Wednesday, 05-MAY-2021, he started experiencing difficulty breathing, dizziness, and shortness of breath and went to the emergency room (ER). This pain was unlike anything he had ever experienced. While at the hospital, they performed an ultrasound and found a clot in his calf. They also performed a CAT scan and found a clot in his lungs. The patient was treated with heparin, but after he expressed concern with heparin, the hematologist changed it to Eliquis (apixaban). The patient was discharged on 05-MAY-2021 with Eliquis 20 mg, one tablet by mouth twice a day for two weeks. He will be on it for the next 6 months. Write-up: I experienced acute onset of dizziness with chest pain, shaking chills, muscle spasms in my legs, headaches and nausea. I went to hospital where they did lab work, 1280 EKG, gave me fluids, and gave me Zofran medication. They also gave me a prescription for meclizine. They also did a Covid test which was negative. I saw my PCP on 04-11-2021 who ordered a chest x-ray and lab work. I also saw my PCP 05-11-2021, because I had a TIA on Sunday 05-09-2021. I still being worked up on lab work, TIA and an MRI has been ordered for next Monday 05-17-2021.

Write-up: Friday night, at 04:00, I got a fever and it was low grade until 10:00 pm and it spiked to 103 and it lasted to 01:00 am. I took Tylenol for it. The next morning, at 08:00 am, I woke up to abdominal pain right under my sternum - and it lasted until 08:00 pm that night - it came and went throughout the day but was pretty much over by that night.. Write-up: So the 28th, early in the morning, I started getting a fever until 06:00 pm that day. Then I felt fine. Monday, the 29th, I had a small rash on my jaw area. It was pretty small but it kept spreading - face, ears , neck and then on to my chest, arms - it was very itchy. Thursday, I went to Immediate Care. I was prescribed prednisone. On Friday, my face was swollen to double the size. They gave me five days worth of doses which I got from the pharmacy on Friday, the 2nd.. I started April 2 with the prescription. It didn''t offer immediate relief. I was still taking Benadryl in addition to prednisone. My rash still lasted for another week and a half to two weeks. I think there are still sometimes traces of it - it appears in small patches - I still get a rash on my neck. It has not fully gone away.

Write-up: 10 minutes after getting the Johnson and Johnson vaccine, while patient was waiting in the waiting area for 30 minutes due to history of bee allergy and anaphylaxis, she reported shortness of breath and dizziness to nurse. Assisted her to the floor with her legs up- obtaining pulse ox and BP/HR, which were stables. Patient reported shortness of breath and dizziness, worsening symptoms, she was tearful, and stated symptoms are the same as her previous anaphylaxis reaction. I informed her I was giving her a dose of epi, and injected 0.3 ml of epi, adult dose to left thigh. Patient reported immediate relief, EMT arrived, At 15 minutes, patient was crying and laughing, reporting worsening SOB again. EMTs arrived and took over the scene. Patient was transported to hospital.

Write-up: I had - as soon as she gave me an injection - I felt like it was a little bit hot. I told her that. I knew I had a lot of allergies so I was being careful. Within a minute of sitting down, my right arm became swollen and bright red and felt burning and itchy. And it spread across my chest and went to my left arm. The rash or hives continued up my neck up to my face and that all happened within first five minutes. Immediately after that, I had excessive coughing - couldn''t cough up anything. I couldn''t catch my breath and my throat started swelling a bit. they gave me 25 mg of Benadryl - capsule over the counter - and the 9-1-1 operator advised not to use the epi- pen. Paramedics came - gave me addition 50 mg of Benadryl - and they took me to the ER across the street and I was administered another 25 mg of Benadryl and also some steroid - Solumedrol - and after receiving these - then these symptoms went away. I continued on Benadryl and prednisone for a few weeks - the very next week - I had a large streak that went from injection site to left chest and I had a second red streak from left chest that ran down to my left arm. For two weeks, that burning fire rash would just come. No rhyme or reason as to when or why - arm, neck, chest, legs and sides. And I''d have to take extra Benadryl every time it would happen and I was still on the high dose of steroids as well. I also had - the next day - 24 -48 hours after vaccine - I had a severe headache and it lasted all day long. And about 24 hours after the vaccine- extreme fatigue, body aches and headache and weakness and that lasted for about three days. Fatigue lasted three to four weeks. Joint aches lasted three to four aches. Headaches have still continued. They have almost been worse actually. I had two headaches that I was so concerned about that would not go away - worst headaches I have ever had: one on May 1 and one on May 9 - and I took Maxalt and some Benadryl - the Benadryl did help a little bit (enough for me to go to sleep). Each headache lasted 12 -15 hours and they were the most severe headaches I had ever had. I didn''t have anyone to take me to take me to hospital so I just tried to sleep. I saw my primary care doctor - virtual visit. I took the prednisone for two to three weeks. I still contend with the headaches.

Write-up: Patient asked if she could get an inhaler for her asthma filled while she was waiting after her vaccine. It was determined that the patient had not had any medications for asthma filled in over 3 years. We contacted two pharmacies that she normally used. Patient then said she was having asthma like breathing - and that she''s been having issues but had not seen a PCP in a long time. I then called the Police dept and asked for EMT to come to our site; patient kept saying she didn''t want us to call 911. EMT arrived, gave her oxygen, and patient was refusing to go to the hospital. She then changed her mind and decided to be transported to the Hospital. While these events happened after vaccination, it is likely that patient''s issues are from not treating her asthma.

Write-up: 15 to 20 minutes after the shot I got very hot/clammy, weak and confused the employee health nurse came to check on me and told me to come to her office and I passed out. I was taken to the ER and kept there approx. 5 hours. I was extremely weak, could not lift my head and had trouble forming thoughts and words. They gave me medication via IV''s and observed me. I started to feel better after about 4 hours after the vaccine, but have had more headaches than usual after receiving it. Write-up: I experienced after my J&J vaccine at about 8pm, felt very fatigued with pain at the injection site, felt like drunk feeling which lasted one week. The main thing is that I slept a lot and was unable to work which lasted 9 days. The thing that I went to urgent care for pelvic pain, body aches and pain, developed an UTI and my symptoms cleared at home with medications. At the urgent care lab work and urine culture was performed. When I went to the urgent care they said I also had tachycardia. My symptoms have resolved.

Write-up: Patient was complaining of being light headed, sweaty, and he stated his arms were feeling numb and that he could not feel his legs. We laid him on the floor and he started to shiver and said he was cold. We monitored his pulse and contacted emergency services. An ambulance came at 10:45 AM and they asked which vaccine he was given and took over care of the patient. Write-up: Five days after the vaccine I had recovered from the original symptoms. I woke up during the night with a strong sharp in my left Testicle. When I woke up in the morning the pain was more severe when I moved. It was painful when I moved in certain directions and when I was sitting. Write-up: Pt. requested and received the J&J / Jansen COVID 19 vaccination yesterday at 1317. While he was being monitored for any adverse reactions, Pt. asked if lightheadedness was a side effect of the shot. Writer advised that it could be and that everyone is different. Writer asked if he would like some water, and he declined. Pt. was advised to remove his mask so he could breathe better. Writer took his pulse while I gathered information. It was 96. Pt. reported that he did eat lunch, is drinking plenty of water and fluids. He relayed he is keeping food down and not having any other issues. Pt. is currently on opiate withdrawal protocol. Writer asked about his breathing and his chest. Pt. relayed he was not having difficulty breathing and denied any chest pain at all. Writer spoke with the Deputy to be sure that we kept an eye on him for a bit longer after his 15 minutes while he was in his cell, due to his lightheadedness. Pt. was talking with writer, answering questions with ease. Pt. was not diaphoretic and did have good color. His pulse was checked again, and it was 84. At 1333, when his 15-minute observation was done, writer went to assist pt. to his feet and informed him that I would escort him back to his cell because he was still feeling a bit uneasy. When he stood, pt.''s knees buckled, and he knelt to the ground. Writer helped pt. to lay down. It was a smooth transition, and he had no injuries to his head or body. Pt. had what appeared to be a short spell where his body completely tensed for a few seconds, then he relaxed. He was having a hard time keeping his eyes open, but when prompted, was able to open his eyes. The Deputy standing by was asked to call for the Public Health Nurses, who had moved on to the next POD to continue vaccinating. Once on the scene, pt.''s blood pressure was obtained. 142/80. His pulse was 82. During this time, it appeared that the tensing episodes became more frequent and a bit more severe. They were happening approximately every 30 seconds, and lasting between 5 and 10 seconds. EMS was activated at approximately 1340. Pt. was awake during these episodes, and communicated with the nurses attending to him. While attending to pt. and waiting for EMS, it was at that time that pt. admitted that this was not the first time this had happened. He admitted that about a week ago, the same thing happened. When that new information was explored, pt. reported that his wife thought he had "Serotonin Syndrome". Writer asked what meds he was taking on the outside and he reported that he only takes methadone form the clinic. He denied any other medications or street drug usage. He does not take any mental health medications at our facility. Collectively, the nurses that attended to him after this episode agreed that this is an isolated incident and not related to receiving the COVID vaccine. EMS arrived at approximately 1347, and report was given. He was transported to the hospital for evaluation. Pt. was returned from the hospital at approximately 1600. The Officer that accompanied him reported to facility that the hospital also believed this was an isolated incident, most likely related to withdrawals. They did not believe that receiving the vaccine was a factor in today''s events. Labs were drawn at the hospital, and were all WNL. Once back in custody, writer spoke briefly with pt.. He was feeling much better, and was sent back to his cell without further incident.

Write-up: I had normal side effects like the flu twelve hours after. The following day I had really bad anxiety and insomnia and my period came early. I could not sleep and I was very anxious. I was very scared and jumpy and restless for no reason. The news of the vaccine safety issues had not been released yet. I went to the ER and I was given anxiety medication. It lasted for a week and I could not sleep at all. I got my period three days after the vaccine and that was three weeks early and it was lighter than normal.. I normally have regular periods. My anxiety has been better and I am sleeping normally.

Write-up: About 30 min later, I started to swell from my neck to your chin. It went numb and was red. My husband immediately took me to the hospital. I was given Pepcid, Benadryl and an Epi. I was released after a few hours and I went home and slept for two days. The guy that gave me the shot must have hit a bone and I was in pain for weeks. I am still experiencing some issues from it. My doctor has me stretching every morning due to my shoulder feeling tight. She says it could be a nerve issue.

Write-up: For thirty-six hours after the vaccine. I had body aches and chills and my eye sockets were sore and it hurt to look up. Ten days after the vaccine I was in the shower and a large blood clot came out of my nose. It was a large clot about the size of a quarter and there was no other blood with it. The clot came out of my left nostril but it was not like a nose bleed. I went to the Urgent Care but they did not do tests.
