Write-up: Pain on side of torso of mid abdomem near lower last rib starting on Saturday, 5/8. Felt like a side stitch similar to a long run but much more intense. Pain steadily increased as to Monday, 5/17, in the AM went to urgent care. They misdiagnosed and said I must have twisted wrong. Went to primary care phisician Wednesday, 5/18, and was diagnosed with, Shingles, Herpes Zoster. Prescribed anti-viral Valacyclovir 1000mg 3x daily, Naproxen 500Mb 2x daily and lidocaine cream as needed.

Write-up: Four days after the vaccine I felt a little chest tightness but the next day it was very scary like a weight on my chest. I also had a cough that was an uncontrollable tickle and then I had trouble catching my breath. I went to the Urgent Care they just thought I had Acid Reflux.. Seven days after my cough was worse and I was very short of breath and I had very bad night sweats. I went to the doctor he gave me Asthma medicine. Two weeks after the vaccine I went back to the Urgent care and they thought Bronchitis. I went back to the ER and I saw a Covid-19 doctor and he thought I had Covid-19 symptoms. He gave me Prednisone and Azithromycin. I took the Prednisone for ten days and I finally started to feel better. I was trying to chew some fresh ginger helped the cough. I have a Heart Echo scheduled next week and I am still a little short of breath. Before the shot I used to walk two or three miles a day and go fishing every day but I can?t do that right now. At least I am back to 80% now which is better.

Write-up: pt presented at store today with a bruise on her upper left calf. she did not remember hitting her leg. she noticed it after her janssen covid vaccine. I advised pt to seek medical attention due to the risk of low platelets/blood clots- under advisement of the EUA. Pt was going to get looked at Med Express.

Write-up: I have MS and I was having some stomach issues and they said it wouldn''t be a problem but I ended up in the hospital for 5 days then in rehab. I woke up about 2am after vaccine and when trying to go to bathroom my legs gave out and I hit the floor, we called EMS to get me up and I was really weak. My legs were gone the rest of the night and I couldn''t stand my husband had to help me . On the 24th, we called the EMS to take me to the hospital and they sent me to rehab and I got out of there on the 28th to home but I do not have my strength back.

Write-up: Client during TC, expressed through the telephone Spanish Interpreter, received the J & J COVID 19 vaccine, in left arm, on 3/14/2021. After receiving vaccine had slight soreness at site, and on 3/27/2021 started having left sided facial droopiness, difficulty swallowing, talking, fatigue, SOB and weakness, around 10:00 am. Write-up: I started feeling kind of puny and sat down for supper - no taste or smell. After that I felt cold symptoms - and I had no energy whatsoever. I talked to the Dr online and he told me take Zinc Sulfate and Hydroxychloroquine; nebulized a medication but I can''t remember what it was. I got pretty sick for about three days. I got to the point that I was so weak I could hardly move. I went to ER and was there for 1/2 day. They gave me IV fluids. They said that my lungs still sounded good The illness ran it''s course of about 10 days. I still feel winded two months later. I have a little nagging cough yet. I can still go to work though.

Write-up: Kaleidoscope vision twice, followed by 5-10 minutes of severe confusion and inability to remember words and son''s date of birth. The fogginess with feelings of a headache coming on for about an hour afterwards. I was working out (as I do 3 times a week with weight training) when the kaleidoscope vision came on, but I didn''t initially feel any other effects and since it went away within 5-10 minutes, I continued with my workout. When I was driving home, it returned and the episode of confusion occurred as soon as I reached my home. I went to the ER after looking up symptoms following the confusion and decided to get checked out since a mini-stroke seemed possible. I have no history of migraines or other health conditions. At the ER, they ran some bloodwork, but I have not received the results of those tests. They also performed a CT scan and said that looked clear, but I have not seen any official results for that either. I have also been advised by a physician that a CT scan won''t show signs of a stroke and that I should have received an MRI. I will be following up with a PCP to request an MRI and will provide those results when I have them. I have had one other episode of the Kaleidoscope vision since the original event, and I was sitting down watching a movie at the time. It went away within the same 5-10 minutes and I had what felt like a mild headache coming on, but no other symptoms. And I have never had any migraines or regular headaches/vision events such as these before.

Write-up: Patient received Janssen vaccine on 5/14/21 around 3:35pm. After receiving her vaccine, she did not remain seated and fell to the floor immediately upon standing. She was unconscious but still breathing and regained consciousness about 30-45 seconds after her partner and I went to her. She was alert and was able to question what happened. She said she felt "tingly" and nothing else. She remained on the floor until EMT arrived. She was taken to a local hospital after vitals were taken by EMT unit.

Write-up: Patient came to received his Covid-19 shot. After the shot, he was instructed to wait 15 minutes nearby for observation. The patient was standing up reading the EUA given to him, felt dizzy, and fell to the ground. This happened approximately 5 minutes after the shot. He was able to get up by himself and sit down to a chair, where he requested water. He proceeded to vomited as he was sitting down.

Heterogeneous large anterior mediastinal mass which is partially calcified measuring approximately 90 x 136 mm with mass effect on the aortic arch and extending to the anterior chest wall. There is mass effect on the left pulmonary artery which is likely encased. Additionally, there is mass effect on the left mainstem and upper lobe bronchus. Write-up: On 5/13/2021 patient developed a cough in the evening. Patient woke up not feeling well on 5/14/2021, and also had nausea and vomiting. Patient was driving a vehicle when he had a seizure. No prior history of seizures. Drifted off the road, no significant damage to vehicle. Patient post-ictal following seizure, seizure activity was witnessed by bystanders. CT brain showed small subdural hematoma. A CTA chest performed due to hypoxemia and tachycardia at Hospital showed Multifocal groundglass type nodular infiltrates throughout the right lung involving the upper lobe, middle lobe, and lower lobe, minimal left lower lobe patchy infiltrate, as well as patient''s known thymoma. These multifocal groundglass infiltrates were not previously mentioned on a CT chest performed on 4/10/2021. Patient was transferred to this facility for evaluation by neurosurgery. Patient was mildly tachycardic, hypoxemic to 88% on room air, and tachypneic. Patient reported decreased sense of taste & smell for 1 month, which he attributed to his losartan use. A covid/flu/rsv PCR on 4/10/2021 was negative, and a viral respiratory pathogen panel with covid PCR was negative on 5/14/2021, as was an antigen & NAAT for COVID on 5/14/2021. Patient was treated with antibiotics and admitted for further management.

The patient''s height, and weight were not reported. The patient''s past medical history included coronary stent, and concurrent conditions included alcohol use (a beer or two a day after work), and non smoker, and other pre-existing medical conditions included the patient had no known drug allergies and no history of drug abuse or illicit drug usage. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 041A21A, expiry: UNKNOWN) dose was not reported, 1total, administered on 05-APR-2021 to left arm for prophylactic vaccination. Concomitant medications included cefalexin for infection, acetylsalicylic acid, atorvastatin calcium, lisinopril, and metoprolol. On 05-APR-2021, within the first hour after vaccination, the patient felt like crap (feeling sick) and took a bath and relaxed after that he felt better. On 14-APR-2021, while he was taking bath he yelled at his wife and also the patient experienced profuse nose bleed which lasted for 20 minutes. These nose bleeds occur at least 4 times daily lasting about 20 minutes each time. On 20-APR-2021, the patient went to emergency room and he had a plug procedure and was discharged. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, batch number: 202A21A, and expiry: 31-DEC-2069) dose was not reported, 1 total, administered on 12-APR-2021 for prophylactic vaccination on left arm. No concomitant medications were reported. On 12-APR-2021, the patient experienced heart beating fast, cold sweat, throat closing up, hands and feet swollen, blood pressure up a lot, body ache, dizzy and felt very bad shortly after vaccination approximately within 5 to 10 minutes. In the 15 minutes of post vaccine observation, emergency services were called and the ambulance/EMTs came and helped stabilize the patient. The patient conveyed when she had arrived home that day after receiving emergency services she did not felt well at all and felt very bad. The patient was still had same symptoms at home that experienced shortly after vaccination, the symptoms did not resolve. The symptoms had lasted for two weeks. Write-up: WITHIN 5 MINUTES OF ADMINISTERING VACCINE, PATIENT WAS SEATED AND COMPLAINED THAT HE COULD NOT BREATHE.. Write-up: Patient became sleepy, light-headed, and confused within minutes after vaccination. He was nodding off in the observation area. He was difficult to wake up and confused when he did. We called 911 and paramedics came to check him out. They monitored his blood pressure and pulse. His pulse was high (120-130bpm), so they thought it would be best to take him to the hospital so he can be checked out.

Write-up: Fever night of getting the shot. Severe abdominal pain for weeks. GI Dr. visit on April 7, 2021 and the PA ordered an abdominal CT with contrast. CT exam on April 13, 2021 which showed total occlusion of the superior mesenteric artery (SMA) and a clot in the thoracic aorta. Went to emergency room on April 14, 2021. Doctor determined that a SMA embolectomy is needed via exploratory laparotomy. Post operation had an event and returned to the ER but was released with all normal blood and heart values.

Diagnostic Lab Data: At the emergency room, they stitched up his lip on the outside but could not stitch the inside due to not having the proper medical supplies to do so, so they told him to let it heal. I do not know if any other tests were administered. He will probably need to get his tooth repaired at a later date.

Write-up: Patient was given the vaccine. After sitting in a chair for a few minutes post vaccination, he said he felt dizzy and then fell over and hit the floor face first but had blacked out and did not remember fainting or passing out. He was bleeding because he had hit his face and his lip was split and his tooth was chipped. He also continued to be very dizzy, was extremely pale, and was sweating profusely. The EMT arrived about 20 minutes later and took him to the emergency room.

Write-up: Patient received the vaccine and while sitting down within one minute he fainted and fell on the floor. He remained on the floor until paramedics arrived. On Thursday, April 15th, I went to the ER due to pain and selling in my leg and it was discovered that I had 2 clots in my leg (on at my calf and a second one on the right side of my knee). Write-up: Patient received vaccine, appeared to be fine, I went back into pharmacy and about 1-2 minutes later, patient fainted and fell onto the floor out of his chair. Write-up: She presented to Hospital for Johnson and Johnson Covid vaccine on 5/14/2021. Janssen vaccine was given at 1330. She was instructed by LVN to wait 30 minutes in the waiting area to be monitor for any allergic reactions. At approximately 1550, she presented to LVN reporting tingling sensation in the mouth, lips and tongue, stating "feeling funny". Then she was evaluated by an RN and Benadryl 50 mg was given by mouth at 1553. Her symptoms were increasing. 911 was called at 1606 and she was injected with EpiPen at 1610. Paramedics arrived approximately at 1618 and transported her to the Emergency Department. Patient remined alert during the entire encounter.

Write-up: Patient has hx of anaphylactic reaction to Amoxicillin ( difficult breathing) arrived at the site with her mother, was informed about the risk of getting vaccinated at non-clinical setting due to her condition. Patient verbalized understanding, agreed to stay 30 minutes in observation area, proceeded with vaccination. After 5 minutes in observation ( 13:35 PM), patient complained of feeling nauseous, dizzy and tingling/ numbness in the upper extremities. Patient vomited x 2, 40 mL of greenish output, noted with pale complex. EMS presented at site was activated. Vital signs initiated, noted with low BP ( 60~/UNK), HR: 70, O2: 99, R:18 in room air. Patient was taken to the ambulance for further assessment by EMT. Patient received an Epinephrine shot to the L thigh by EMT at 13:52 PM. Blood pressure improved to 110/70, HR: 81, R:18, O2: 99%. Patient also received 1 albuterol treatment 6 LPM. Patient was taken to Hospital at 2:04 PM via Bus.

Write-up: After patient received Vaccine she started immediately to look very pale and stated she was not feeling well. Write-up: 4/13/2021 From patient c/o of left leg pain and swelling s/p J&J vaccine on 4/03/21 from her local pharmacist. Patient states the the pain and swelling has increased over the last couple of days. She states she does have a history of varicose veins. Advised patient that due to receiving the J&J vaccine within the past 3 weeks, she should proceed to the nearest ER for evaluation. Patient was resistant to going to ER due to her uninsured status and asked that I contact her PCP for a second opinion. A message was sent to physician who agreed patient should be evaluated in the ER. Write-up: om 5/8/21 at 9:00 PM, felt light headed, stepped outside for air, returned to closest chair, calling to significant other, I feel dizzy. Sat in chair, staring blankly, not responding to calling, or physical touch. breathing shallow, facial and lip color pale, pasty white gray. weak pulse. 911 called, and responded 8-9 minutes later. By the time they arrived, patient had aroused and aware of his surroundings,. Although color was slow on returning to normal. EMS arrived, BP 120/80. Patient refused to go to ER . On 5/10/21 at 2:00am, patient woke up and had a nosebleed lasting 2 hours, pouring from nose. 911 called and transported to medical center for cauterization, nasal packing and observation. 5/11/21 developed a reddened rash on right hand, lasted 5 days. Nasal packing removed on 5/13/21. No further complaints at this time.

Write-up: J&J Dose 4/10/21 (1808980) COVID Positive 4/27/21 4/29/21: Presented to ED and admitted. 4/30/21: Pt. is a 49-year-old female with history of persistent asthma who was diagnosed with COVID-19- 5 days ago. The patient tested positive for COVID-19 infection on 04/25/2021. Since then she had dry cough, on and off wheezing, worsening shortness of breath. Since yesterday she became extremely short of breath for which she decided to come to the emergency room. Earlier yesterday she was evaluated by an ED physician and she was advised to be admitted because of hypoxemia and pulse oximetry of 87%. Patient declined admission and went home. She came back with worsening shortness of breath. On exam patient was comfortably breathing on 2 L with oxygen saturation 95% ; chest examination was significant for bilateral extensive rhonchi with good air entry and no wheezing. Blood work shows sodium 133, potassium 2.4, BUN 5, creatinine 0.5, albumin 3.4, D-dimer was elevated at and follow-up with CT of the chest with contrast was done. It showed bilateral extensive multilobar pneumonia . There was no pulmonary thromboembolic disease. 5/4/21: Patient is a 49-year-old female who was diagnosed COVID positive on 4/25/2021. She presented to the emergency department on 4/30/2021 with worsening shortness of breath and was noted to be hypoxic. She was admitted to the medical floor with acute hypoxic respiratory failure, secondary to bilateral COVID-19 pneumonia. She received 1 unit of convalescent plasma. She was started on Rocephin, Zithromax, dexamethasone, and Remdesivir therapies. She had a persistent dry cough for which codeine/guaifenesin was effective. Her potassium was also noted to be low, and it was supplemented. She was started on oral therapy and this has maintained it. She is no longer requiring supplemental oxygen. She reports she is feeling much better. She has completed her Remdesivir therapy today and is requesting to go home.

Write-up: I started feeling pain in my arm and shoulder, but I didn''t pay much attention to it. The next day, the area above my breast was hurting and tight. My husband took me to a healthcare facility and they took xrays and blood. They wanted to admit me, but I didn''t want to stay. They gave me Tramadol for pain and told me to relax when I got home and to see my PCP. Write-up: Thirteen days after the vaccine I got a sudden sharp pain in my right leg after the vaccine. I am predisposed to blood clots and the staff could not find pulses in my foot. The pain started in my groin area where the femoral artery is located. I have been diagnosed with Raynaud''s Syndrome which could have caused the lack of pulse. Write-up: because they were unable to use a bandaid they used a cotton ball to put pressure, within two minutes it was a sense of pain up and down her arm from shoulder to elbow where she was vaccinated. after 5 minutes it calmed down, after the 30 minutes she felt all her joints hurt and felt swollen, prior to vaccine her whole body was sore from the day prior, the doctor came in and she was given an icepack for her shoulder. She was then getting very confused and just couldn''t focus when the doctor asked for her medical condition and needed to call in her mom to assist because she was beginning to panic, blood pressure did go up and was then given Benadryl, within this time she had drank 2 big cups of water, after the ice pack and benadryl the bones felt better but not okay. Write-up: By 7:30, I felt drunk. My brain was not functioning normally and had a fever all night. By 7:15 I felt fine, with the exception for random joint pain. The following Friday, April 9th, I had bloodwork done. It felt like I had hit my funny bone. I had shooting pain up my arm and into my neck and down my spine. Write-up: About 2 hours after receiving shot, I was feeling very run-down and sore. My neck had pressure building at the base. Left work early to get rest. My next day entire head and shoulders were pressurized and shooting pains down neck, through arms, numbing left side of body cramping feet and arms, all triggering anxiety /panic attacks. Write-up: Patient received the Janssen COVID vaccine on 5/17/2021 around 1513. Prior to vaccine administration during prescreening questions, the client reported "I have a history of passing out after vaccines because I have to have monthly vaccines in my leg for headaches and I hate them" Client was taken to another room with a reclining seat to lay down during vaccine administration. She remained in the supine position for several minutes afterwards and after reporting she felt fine was changed to the sitting position for several minutes and again reported feeling fine. Patient was then changed to standing, reported she felt fine and ambulated to the waiting area, escorted by an RN assistant to be observed for 15 minutes by the EMS provider on site. Soon after, patient appeared to be having a syncopal episode. EMS provider on site called 911 to transport patient to the nearest hospital.

Write-up: On March 28th, started noticing symptoms. There was pain in my entire right leg, shin swelling; calf- swelling and skin discoloration; and right foot - swelling. On 4/1, I went to Urgent Care where I was seen, evaluated and treated by Dr. She ordered blood tests and an ultrasound within 20 minutes. I had 4 blood clots in 3 veins in my right leg/DVT. My blood platelet value was 267 and the standard range is 130 to 400. They didn''t consider it low, but it''s still an issue. I was given an injection of Enoxaparin Sodium Injection--1 ml injection twice a day for 5 days, which was started in the ER. I then switched to Pradaxa 150mg pill twice a day. I took that until around the 6th of this month when I noticed my right leg had gotten worse. I was instructed to go back to the urgent care. Another ultrasound was ordered and it showed I still had the 4 blood clots in 3 veins that seems to have worsened, so the meds were not working. He consulted with a vascular surgeon and they put me back on the injection once a day, which I am currently still on. I was told to follow up with my PCP in two weeks and that appt is on Thursday with Dr.

Write-up: On 04-07-2021- I experienced a lump on my neck and developed pain to both of my shoulders. I went to the ER where x-rays and lab work was done. X-rays showed that my lymphatic glands inflamed. I also experienced a cramps from my neck that radiated upwards to the base of my head. My pain was so intense that I took OTC Tylenol to help with my pain. I went again to the ER eight days later because I was having pain to my heart, chest, neck and shoulders and x-rays again demonstrated lymphatic glands inflamed and was told to take Ibuprofen for pain. To this day I continue to have pain to my shoulders, neck and head but the pain is not as intense as before although the pain is still present daily. Write-up: I did have leg injury in April - and ever since the vaccine, I now have an issue with swelling that won''t stop - still have swelling. I was checked with blood clots - and it was elevated so they gave me an ultrasound in leg - no blood clot. Swelling on right leg was enlarged after the vaccine. Two missed menstrual cycles; now I have had the cycle but it''s a longer cycle than I normally would have. It''s abnormal - the length and flow is off. Spotted for a week before I started bleeding. Write-up: Patient felt dizzy less than 5 minutes after vaccine, patient became light headed and pale, had difficulty opening eyes. She remained sitting, but was on the verge of passing out. Write-up: The patient walked in to the pharmacy for a Janssen Covid vaccination along with family. Patient was advised post vaccination administration to sit in observation for 15 minutes but did not comply and as he was walking away from the pharmacy fainted. Parents reported he did not hit his head and pharmacy was alerted post fall of what had happened. We approached the patient as he was getting to his feet and we advised again for him to come back to pharmacy to sit in a chair for further observation. His parents told us he always has fainting issues post any vaccination or blood work (no prior issue reported to vaccinations on consent form). The patient continued to decline going back to pharmacy and as he walked fainted again, he was able to hold on to shopping cart which lessened severity of fall. Pharmacy staff advised him to remain on floor and after he regained consciousness he was able to answer accurately his name, where he was and what day of week it was. At that time a nurse not employed by Company was shopping near by and began to help care for the patient. The patient and his parents continued to decline calling an ambulance, and after several minutes sat against a wall. He was starting to sweat profusely and heart rate was rapid (approximately 145 bpm per nurse) so we had him remove his coat and provided a cool compress for his head and neck. He was breathing rapidly but was able to slow his respiration upon our direction. The patient did not seem to be improving and the nurse helping advised calling EMS which we did. The patient continued to lose consciousness for brief periods awaiting and during EMS aid. EMS ran the patients blood pressure, blood sugar nd ran an ECG prior to taking patient away to hospital (results unknown to pharmacy as we stayed in area to provide information on patient vaccination administration but had no active assistance care at that time.

Write-up: I started having a lot of pain in my right leg. I could actually feel and see that my veins were swollen. It progressed and started moving around. I started having chest pain and headaches. I ended up at ER. They gave me pain meds and did a CT Scan and ultrasound, which were both clear. The next day, I was walking around and my hand and arm started hurting and then went numb. My right wrist was swollen to twice its size. I went to care and they told me to go back to the ER. They eventually did another ultrasound, but the swelling had gone down by then. A few hours later, my left arm started to swell and lasted for about 4 hours. From there, I had pain in my arms and legs for a few days after that, but then I was fine.

Write-up: I received my Janssen vaccine on 3/31/21. On 4/29/21, I had a virtual appointment with a naturopath about a completely separate health issue (mold illness), and she asked me to check my blood pressure. I checked my blood pressure using my father in law''s at home cuff, and it was 149/99. I continued to check over several days and had persistently elevated readings, ranging from 122-165/93-107. My last known normal BP reading was in February of 2021 and was 110/70, which is my typical BP. I had not checked my BP at any time between February and April because I had no reason to believe it would be elevated. On 5/2/21, I had a reading of 165/107, which prompted me to go to the ER. At the ER in triage, my BP was 188/144. EKG was normal and troponin I was normal. I was monitored for a few hours with BP around 155/100, and after a dose of Nadolol, my BP came down to 122/79. I also had chest pain/tightness during this experience, which may have been due to anxiety. My BP has continued to be persistently elevated (now coupled with anxiety about what is going on, which doesn''t seem to be helping the numbers!), and I had to return to the ER on 5/5/21 after getting an at home reading of 180/122 (this was about 1 hour after taking a dose of Nifedipine). At the ER triage, my BP had come down to 140/109, so they sent me home. My BP has now remained elevated at around 135-140/95-100, while taking 30mg Nifedipine ER daily.
