Write-up: Woke night up terrible pain in left hand, palm and fingers. Next couple days, left arm ached like flu, went across shoulder and down right arm to hand-HA''s not been normal to this date 3/30/04. Left fingers are numb and hurt like have been too cold. Right fingers not as bad as left-after 4 mo some better. Went to Dr 12/8/03. Ibuprofen for pain. Still not normal. Constant pain and numbness sometime better/ worse-has never gone away. Where shot given-still sore. Information from Annual Follow-up report states not completely - still numbness in fingers all time. But not as bad and not near the pain.

Write-up: FLU VACINE GIVEN ON NOV.6, 2003. PATIENT HAD TWO MECHANICAL HEART VALVES. WAS ON AN ANTIBIOTIC AT THE TIME THE VACCINE WAS GIVEN. FOUR DAYS LATER WENT INTO CONGESTIVE HEART FAILURE. HOSPITALIZED FOR 4 DAYS. CONTINUED TO GET WEAKER OVER THE LAST THREE MONTHS. LEG PAINS, WEAKNESS. PATIENT PAST AWAY MARCH 16TH 2004. SEEMED TO BE IN GOOD HEALTH UNTIL THE VACCINE. CARDIOLOGIST FEELS ONE OF THE VALVES FAILED. I BELIEVE THE FLU VACCINE IS WHAT THE PROBLEM WAS. Per death certificate, cause of death was acute MI valvular disorder.

Write-up: A report was received from a health care professional on 2/9/04 concerning a female vaccinee of unspecified age who experienced ear pain within a few days of vaccination and neck pain, chills, headaches, post-nasal drip, muscle spasms, an increase in blood pressure, an increase in heart rate, fluid in her ear, sinus congestion, a sensation of her body being on fire, diarrhea, palpitations with shortness of breath, loss of appetite, hair loss, night sweats, pain in the right deltoid, pain in the right shoulder joint, numbness in the fingers, muscle cramps, fatigue, cough, tingling in the head and jaw and was unable to turn her head an unspecified length of time after receiving Fluvirin on the right side on 10/23/03. On an unspecified date the vaccinee experienced right ear pain within a few days of vaccination and right-sided neck pain, chills, headaches which lasted approximately 1.5 weeks, severe post-nasal drip (on the right side only), muscle spasms in the jaw and neck, an increase in blood pressure, an increase in heart rate, fluid in her right ear, severe sinus congestion, a sensation of her body being on fire lasting for about 10 hours, diarrhea, palpitations with shortness of breath, loss of appetite, hair loss, night sweats, pain in the right deltoid, pain in the right shoulder joint, numbness in the fingers, muscle cramps in her right calf and thigh, fatigue, cough, a sensation of tingling in the head and jaw and was unable to turn her head. The reporter indicated that all symptoms began shortly after receiving Fluvirin. The vaccinee indicated that most of the symptoms had subsided but continues to have problems with sinus congestion, nasal discharge, fatigue, blocked ears, and muscle pain but not as significant as when experienced initially. The vaccinee also experienced some symptoms on the left side but were less intense. The vaccinee was unable to exercise when the symptoms were most intense. At the time of reporting she had commenced exercising again and noted that the muscles on the right side of her body were significantly more painful than the muscles on the left side. Laboratory tests including thyroid function and head CT were normal. It was suggested that the symptoms experienced by the vaccinee may be due to a reaction to the immunization or may be due to a type of flu virus. There was insufficient information to determine the reporter''s causality. Update 1, 4/20/04: Further information was received from a health care professional who is also the 49 year old consumer who experienced neuropathies and paraesthesia after receiving Fluvirin. The vaccinee''s symptoms were considered to be disabling. The vaccinee has undergone numerous tests. However, no diagnosis has been confirmed and she continues to experience neuropathies, paraesthesias and muscle spasms. The vaccinee has a medical history of normal childhood illnesses. The vaccinee was treated with gabapentin, cyclobenzaprine hydrochloride and physical and massage therapy. Follow up pending.

Write-up: A report was received from a consumer on 05/04/04 who is a 54 year old female vaccinee who experienced influenza, which was considered disabling, approximately 2 weeks after receiving Fluvirin and Pneumonia vaccine (unspecified) on 10/20/03 into seperate arms. The vaccine also experienced pneumonia, which was considered disabling an unspecified length of time after receiving Fluvirin and an unspecified pneumonia vaccine on 10/20/03. The vaccine had received influenza vaccine for several years without any issues. On 11/03/03 the vaccinee experienced a sore raspy throat and felt unwell after receiving Fluvirin and a pneumococcal vaccine on 10/20/03 in different arms. The vaccinee''s sore throat became extremely painful and she visited the ER on 11/09/03. A throat culture was taken, which concluded that the vaccinee did not have a streptococcal throat. The vaccinee was subsequently diagnosed as having the flu and was prescribed Levofloxacin and an unspecified cough medication with codeine to help with the painful sore throat. On 11/1/03, the vaccinee had a follow up appointment with her treating physician as she had developed severe chest congestion. The vaccinee received an unspecified steroid injection at the clinic and was prescribed a course of Prednisone therapy. On 11/19/03, the vaccinee returned to her treating physician''s office and complained of severe head congestion and was subsequently prescribed amoxicillin and albuterol. The vaccinee developed diarrhea from the amoxicillin. Throughout this period of time the vaccinee was unable to work. The vaccinee visited the physician''s office again on 12/15/03 and between this appointment and the last visit she reported that her lungs felt like they were on fire. the physician stated that her lungs sounded clear and prescribed montelukast sodium and stated that she was to continue albuterol therapy. On 01/15/03, the vaccinee saw a lung specialist and had an X-ray which confirmed that her lungs were clear, however there was evidence of residual scarring that may have occurred cue to the pneumonia. The vaccinee returned to work on 02/16/04, however she continued to experience fatigue. Due to her continued fatigue she quit her job on 03/08/04. On 03/24/04, the vaccinee visited her physician due to bouts of wheezing and returning symptoms of a sore throat. The vaccinee was presribed albuterol and another course of amoxicillin. On 04/05/04, the vaccinee returned to her physician with continued symptoms and was prescribed a course of azithromycin that relieved her symptoms. At the time of the report the vaccinee continued to experience periods of wheezing and shortness of breath. The vaccinee is unsure if she just contracted flu or pneumonia, as anyone in contact with her has not contracted any illness from her. However, this was her first exposure to the pneumonia vaccine. Follow up received 31Aug04 from a healthcare professional. The vaccinee was confirmed as a 54 year old female (height 5ft 4 inches; weight 161 pounds). The vaccinees medical history included allergies to Tetracycline, Doxycycline, Zyrtec, Celexa and Bellergal. Cardiac catheter in 1997, total abdominal hysterectomy in 1993 and anxiety disorder and mitral valve prolapse. Her concomitant medications included Buspar 15mg twice daily for depression; Ambien 5mg at bedtime for sleep disorder; Zovirax 20mg Prn, caltrate 600mg, Depakote 500mg twice daily for mental disorder; Nexium 20mg for influenza vaccine on 15Oct02. On 20Oct03, the vaccinee received 0.5ml Fluvirin IM. Ten day later (on 09Nov03) the vaccinee was seen in the ER for sore throat, purulent drainage and cough, fever and hoarseness. She was diagnosed with acute bronchitis and tonsilitis. She was seen in follow up by her physician on 12Nov03, 19Nov03 and 15Dec03. The outcome was not reported. The reporter commented that none of the events were due to vaccination. The reporter does not indicate that these events were serious. A report was submitted to the authorities on 07Apr04 by the healthcare professional.

Write-up: A report was received from a healthcare professional in the USA on 6/25/04 concerning a 45 year old female vaccinee. No details were provided regarding the vaccinees medical history or whether she was taking any other medications. In October 2003, the vaccinee received an IM injection of Fluvirin. At an unspecified time after vaccination, the vaccinee developed a frozen shoulder syndrome, which she attributes to the Fluvirin. She is now receiving physical therapy for this problem. Further information will be sought. Follow up received on 15Nov04 from a health care professional in the US. The vaccinee has been identified as a 59 year old female who experienced frozen shoulder syndrome and injection site oedema after receiving and Fluvirin vaccination (batch 765753) in her left deltoid on 22Oct03. The vaccinee still experiences frozen shoulder syndrome, despite being provided with long term physical therapy treatment. The vaccinee has continued reduced mobility and pain 13 months after vaccination. The vaccinee''s medical history includes C-section in 1982 and an allergy to anaestetic during the operation. She also has an allergy to penicillin since childhood and macrolides since the age of 55. Her concomitant medications include Hydrochlorothiazide, 25mg daily, for high blood pressure. The reporter considers the reaction serious due to disability and the vaccinee has been advised against influenza vaccination in the future.

Write-up: Sore chest and butt muscles. Then sore feet and ribs; hurt to turn in bed. Then swollen foot with rash at 2 weeks; could not walk. Something in my tailbone or nerves? Now it is in the hands, feet, and left knee. Per annual report- Still have swelling on my left hip (per MRI), 3 fingers and right foot. I''m using crutches. Soreness moves around in my right shoulder and neck, knees. This past week I can feel it will be leaving. I was told by my doctor this will be a slow recovery in August 2005. Write-up: On Thursday, 10/30/03, I had received a flu vaccine while on the job. The injection was given IM by the nurse. Immediately after I had received the injection, I began to feel very ill. A cold chill went through my body and I had to sit down in the pharmacy. The next day, 10/31/03, I was not able to get off the sofa at my home. I was not required to go to work until 11/5/03. I had then began to feel somewhat better, however, I still had a persistent headache. Then, on 11/6/03, I had such a severe headache I could not hold my head up. My friend drove me to the emergency room. The diagnosis at the ER was that I had the flu and a urinary tract infection and I was sent home with Levaquin 400mg for 4 days. My friend had informed the hospital that I have had a history of genital herpes, and that I did not have an outbreak at that time. I was then sent home from the hospital in the care of my mother and brother. The severe headache gradually got better, however, I developed double vision for part of the next day, 11/7 and was severely vomiting. I gradually began to feel better from 11/8 to 11/14, and returned to work on 11/16. I continued to have a dull headache and some neck pain and tingling in my fingers. On 12/10 I went to see the doctor. I explained my symptoms to the doctor and he suggested that I see a neurologist. The doctor told me that he though I had viral encephalitis. While I was at work on 12/14/03, I was transposing numbers on the computer. Suddenly, the telephone rang in the pharmacy but I picked up the stapler and was talking to it. I was immediately taken to the hospital. After a spinal tap was done, I was diagnosed as having herpes simplex encephalitis. While in the hospital for 4 days, I was given Acyclovir 550mg IV every 8 hours and was then sent home on this same medication through 12/24. I have enclosed documentation from the hospital. I am currently seeing a doctor. I am also seeing a neurologist at the hospital. Currently, I am going to speech therapy 3 times weekly. Soon I will begin to do work simulation at the rehab center. I hope to return to my job as a pharmacist soon. Per the annual follow up report I am still taking Depatkote ER, I am taking Zovirax daily also. I truly believe that anyone who has active cold sore virus or herpes simplex should not receive a flu vaccine. Also whether the herpes is active or not active do not receive a flu vaccine. Once you have had herpes virus it is always in your system. I believe the flu vaccine activates a dormant herpes virus and causes a terrible reaction. When I received the flu vaccine I became very ill afterward. I then had a seizure and was hospitalized for 3 days. I had one year of speech therapy for memory loss. I am presently working 30 hours a week as a pharmacist.

Write-up: Soon after vaccine, my arm that injection given became uncomfortable, like there was a tournaquet around my arm, then my arm became heavy and sore, I had tingling in my right arm and my right leg, I went to the emergency room that night, I had a CVA. I reported this to the nurses at Occupational medicine, but I''m not sure that an adverse event form was filed. Follow UP: Pt states left arm and leg affected. Nurse follow up on 10/30/04 states: "cerebellar infarct; Subcortical ischemic change." Follow up 01/26/05: I continue to have numbness and tlingling along with imtermittent "heavey feeling" in left arm and leg. My balance is also affected, I loose my balance very easily.
