Write-up: patient received the Janssen injection and at one minute became vagal and began vomiting at 2 minutes had a temporary loss of consciousness, EMS arrived at 4 minutes and patients heart rate was alternating between 20bpm-40 bpm. EMS transported to ED. Patient was discharged home at a later time.

Write-up: Immediately after the shot my arm started hurting. The next day I started getting a headache and nausea, chills and muscle ache. My bottom lip was swollen by the third day and I was not feeling right. I had numbness going up to my right eye and I could not speak clearly.

Write-up: I visited the urgent care the following Tuesday. I felt very flushed and dizzy. 24hrs after I had major migraines which lasted two days. I had a flare up from my lupus. I also had extreme stomach pain which is why I went to the urgent care.

Write-up: I immediately felt sharp pain and burning in my arm while being injected. The burning in my arm lasted for about 20 minutes. I got a severe migraine about an hour after the vaccine that lasted for about ten hours. I broke out in hives all over my body later that night, the same day as the vaccine. I also had intense heartburn. I never get heartburn so this is not normal for me. The following day, I was still having hives and started also having chest pains. I was consistently taking antihistamines to keep the hives under control. On Saturday morning, 2 days after the vaccine, I was still breaking out in hives. My throat also started to swell and I was having a hard time breathing. My heartburn was also getting more severe. I ended up going to urgent care. There I was prescribed 2 stronger antihistamines and prednisone to help with my reaction. I was also given an epi pen. Later that night, I ended up going to the ER as I was having severe chest pain. I have a history of DVT so I was concerned about blood clots. I had several tests done along with a chest CT scan. Thankfully everything came back clear for blood clots and heart attack. The ER doctor could not explain why I was having chest pain and advised me to monitor it and to come back in if it got worse. The following day, I was still needing antihistamines for hives and difficulty breathing. I woke up in the middle of the night with severe stomach pain and cramping. I had intense vomiting and diarrhea. The diarrhea started to have a lot of blood in it. I went back urgent care and they sent me for a CT scan which showed I now had colitis. The bleeding lasted for about 3 days.

Write-up: To ED via ambulance for chest pain and shortness of breath. Decompensated during transport, hypoxic, cyanotic, hypotensive. Intubated in ED. CTA of chest positive for saddle embolism. 1/2 TPA/heparin given. Admitted to ICU. Change in mental status, CT of head showed intracranial hemorrhage. Patient had reportedly been sick about a week or 2 earlier, covid positive PCR on admission to hospital.

Write-up: I experienced neck puffiness, eye puffiness, chills, chattering teeth, neck pain, red face and skin as well. I also experienced scalp discomfort it felt itchy and skin felt different. I also had face and head flushing with hot flashes. Didn''t feel well the full weekend duration of 3 days.

Write-up: 4/6 vaccination 4/12 HA 4/13 HA turned into migraine, legs started to tingle, achy. Zero sleep due to pain. 4/14 Both extremities very swollen. Rotating between Tylenol and Ibuprofen (started on Tues). I went to my PCM. BP 130/98. Prescribed water pill but only to take on Friday if symptoms got worse, not better. 4/15 Sick all day. By 5pm, I couldn''t even focus, my legs were so swollen, my ''head was ready to explode'' and I ''didn''t feel right. I went to urgent care; 160/98 BP and referred to the ER. I went to the ER, BP was still elevated, legs still swollen. Checked my heart, ultra sound on my legs and x-ray. I was kind of ''out of it''. They did everything to check for blood clots and heart; everything came back clear. They gave me some medicine for my head thru IV. I still left with HA but didn''t feel as bad. That helped my BP went down. I was at the ER 2-3 hours, drinking lots of water. The next day the swelling started to go down on the legs and migraine was subsided. By Sat., I felt fine.

Write-up: On 3/7/2021 my body was aching and very sore. Write-up: Two weeks after the vaccine I noticed that I had a strange feeling in my eye. When I looked in the mirror I saw a little line of blood. Write-up: About a week and a half after receiving the first and only dose of the Johnson and Johnson Covid-19 vaccine, I developed small bruises (Petechiae) on my chest and arm. I decided to consult my doctor just in case. My doctor did a blood draw and ran a CBC panel. The next day I was rushed to the ER as my platelet count was reported at 7. By the time I got to the ER my platelets had dropped even further to a count of 2. I was treated inpatient for 3 days 2 nights. At this time I was diagnosed with IPT (Immune Thrombocytopenia Purpura). It was treated with two rounds of IVIG (Intravenous immunoglobulin) and 75 mg of Prednisone daily (which I have been slowly tapering off of since). My platelets restored to a non-critical level and have continued to hold steady ever since.

Write-up: Within 2 days, the lymph nodes at my collar bone/neck on the left side began to swell. By the following Tuesday, I went to the ER for the enlargement. Routine blood tests showed nothing unusual (lymphoma). Was sent home to see my doctor if the swelling doesn''t subside. Scheduled an appointment with my PCM on 23 Apr 2021 (virtual - they are not seeing in-person patients yet). By this point, the swelling had not subsided and there was new pain in my left elbow shooting dow nto my hand. The doctor prescribed Naprosyn, lidocaine patches, and a wrist brace to keep the nerves stable. Was told it was "pain management" until the swelling from the lymph nodes go down. It is now 6 weeks post-vaccine and the pain and swelling is still the same. I have been unable to secure an appointment with my PCM. For the past 6 weeks, my pain level has been at a steady 8 out 10.

The patient''s height, and weight were not reported. The patient''s concurrent conditions included asthma (as a child), non-alcoholic, and cigarettes smoker. The patient experienced drug allergy when treated with Tamiflu (oseltamivir phosphate). The patient received Covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: unknown, expiry: unknown, NDC number: 59676-0580-15) dose was not reported, 1 total administered on 14-MAY-2021 at 10:00 for prophylactic vaccination on left arm. The batch number was not reported and has been requested. No concomitant medications were reported. On 14-MAY-2021, as soon as the patient got the shot (within the 15 minutes of wait at the pharmacy), he had an anaphylactic reaction. It was further reported that he could not breath, his lips and face started to swell and then he passed out. The patient was taken to ER (emergency room) and his doctor gave him EpiPen (epinephrine) shot, Medrol (methylprednisolone), Benadryl (diphenhydramine hydrochloride) and IV (intravenous) fluids. The patient''s lips were drooping to the right side of his face even after taking the medications. The patient stayed at the ER for 6 or 7 hours then was sent back to home on the same day. Write-up: Thursday, 05/20/2021 02:05PM received Janssen 0.5mL to left deltoid IM injection. 04:55PM Noticed a pin needle like rash on my chest, stomach, and neck. 05:00PM Took 50mg of Benadryl. 05:40PM Took another 50mg of Benadryl due to rash being more present on my chest, stomach, and neck. Itching began on the neck behind the ears. 05:50PM Noticed difficulty/restriction to swallowing. 06:20PM Checked into a facility. Waited an hour to be seen by a provider, Dr. He states only an acute reaction without examining my body of the rash; only examined my back & inside my mouth. I informed him that my rash was on my chest, stomach, neck, now my arms, and top of my thighs. He provided Prednisone and dismissed me. (I will be filing for malpractice) 08:00PM- Got home, still have rash with some difficulty swallowing. Friday, 05/21/21 06:00AM- Rash seems to subside and throat is fine, but swollen. 09:00AM While at work, coworker mentions my face is red. Rash came back to my arms and is now on my face. Write-up: 8 hr post vaccine, typical side fx (fever, headache), but also significant bi-lateral leg cramping and spasms that continued for 6-8 hours. Spasms were prominent in inner thigh/groin and down entire legs. Legs were not movable (heavy), difficult to get out of bed or walk. Very strange and nothing like this ever experienced before shot. Fine next day. Around 4/9, pain began in ball of R foot in the AM, hard to walk. Over next few days, pain extended up to medial posterior calf (very painful), back of knee and groin. Unexplained bruising on lateral calf. 4/20 woke to swelling of R leg (ankle, posterior knee, groin), went to Urgent Care that night and was sent directly to ER for DVT symptoms. ER Ultrasound showed no clot, was sent home. Pain continued to increase, with burning and difficulty walking. Second Ultrasound done 4/22. No clots found. Pain continued to worsen, with swelling/aching on front of calf and severe foot pain. Veins/skin tender. Painful groin spasms happen again, same as night of shot. PCP ordered ABI and spine Xray. ABI was normal. Xray showed same issue as 29 yrs ago (prior known L5S1 anterolisthesis; have not had back pain for many years...when I did, it was localised to spine and sciatic in the L hip, was not at all similar to these current issues in R leg. perhaps related if inflammed, but is vastly different in symptoms and unlike past history). Saw Vascular Surgeon 4/30, ordered abdominal CT Abdomen Pelvis w IV Contrast. results normal. arm bruising at IV and blood draw site. Saw second Vascular Surgeon 5/4 who felt pain related to inflammation response and ordered CBC. Platelets 178. (patient had requested this test starting on 4/20, this was the first doctor to order it). Pain continued but some relief found with compression socks. on 5/10 US Bilateral lower extremity venous duplex with reflux study was done on both legs. No DVTs, but damaged valves and reflux in the external iliac common femoral vein and femoral veins proximal segment (worse in R leg) and great saphenous vein at the sapheno-femoral junction (R only). These areas are where the spasms are happening but doctor not able to conclude relation, wants neuro consult, could be nerves causing spasms. Doctor prescribed compression socks for venous insufficiency concerns, to be rescanned annually for life. Vascular test results don''t explain severe pain in calf and foot and referred to Neurologist for 5/28 visit. As of today, 5/22, pain is persistent. Pain/swelling is reduced with use of compression socks but returns without them; walking is difficult in the AM and PM hours; night time spasms. Have also had shooting pains in L calf, both forearms, and ribs (back). These occur mostly in the evening (nerve?). Taking magnesium & extra vitamin D seems to help some. On strict anti-inflammatory diet. I''d really like some guidance for treatment and what to do about boosters - do I switch to an mRNA vaccine? I am a very healthy active 49 yr old woman who felt fine before the shot, and now I''ve been in a lot of pain, working greatly reduced hours for almost 4 weeks. Please advise on what can be done to help. Concerned about permanent damage and how to handle future vaccines. (also of note: first menstruation period was skipped after the shot, second period came a week late and was very heavy with bright red blood. I am normally extremely regular, mod-heavy flow; also, the J&J shot I had was a "last shot of the day" extra dose. I was offered it while having a routine physical. Write-up: Patient - received the Johnson & Johnson vaccination (Lot # 1808609) on 05/22/2021 at 1131 in the left arm. Upon walking to observation patient fell, it was observed that he hit the back of his head by nurse at nursing station #4, nurse immediately cradled patient?s head and turned patient on his side for safety. Patient was assessed for open wounds, hematoma and bleeding. Negative findings. EMS called by RN Patient assessed RN. Patient is a 21-year-old with no past medical history per patient?s grandfather. Patient is laying on the floor, with eyes wide open, not verbally responsive, and unable to follow commands. Patient was pale and diaphoretic. Patient is awake, but not oriented to time and place. Patient also does not recall having breakfast, but his grandfather stated that he did have eggs earlier this morning. Vital signs as follow: Time 1134: BP 147/78, pulse 43, 98% room air, temp 97.1 Time 1138: BP 147/82, pulse 47, 98% room air, patient started to become response asking questions. Patient denies any lightheadedness or dizziness, he only felt a burning sensation on the back of his head. Patient?s grandfather at his side. Patient?s wallet, vaccination card and glasses given to patient?s grandfather. Time 1142: BP 125/79, pulse 64, 98% room air Privacy screen in place for confidentiality from other patients. Awaiting EMS. Incident aware 1144 EMS arrived. 1146 Patient placed in wheelchair and left the building with EMS. Patient was awake and alert upon leaving the building.

Write-up: Patient previously had a Moderna (dose #1) vaccine on 1/15/21. Patient had difficulty swallowing, tingling on tongue, and got the jitters about 10-15 mins post dose. She was recommended by her PCP to try the Johnson and Johnson vaccine because of this allergy. We had discussion with an ID specialist and pharmacy manager to go ahead and give the vaccine after weight the risks/benefits. A pharmacist was there to monitor the patient and about 15 minutes post dose, patient had difficulty swallowing and noted that this was a very similar onset to when she received the Moderna vaccine. A code blue was called, patient was sent up on stretcher to the ER. Patient received steroids (methylprednisolone) , famotidine, and Benadryl while in the ER. Currently being monitored in the ER.

Patient had premedicated herself with Symbicort, Montelukast, diphenhydramine 50 prior to coming in. But 5 minutes after receiving the Covid vaccination she developed throat itching itching all over her skin/lungs, and chest tightness with intermittent coughing. Code Blue was called, and patient was taken to the ER. Patient received epinephrine, famotidine, benadryl and methylprednisone. Patient was monitored in the ER and eventually discharged home.

Write-up: Developed deep lowerchest/upper abdomen pain on right side 4 days after vaccine was administered. Pain got progressively worse and a rash developed on right thorax in the Thoracic rib 5 region; Visited Army doctor on 12 MAY. Based on visual exam of multiple, tiny water-filled blisters only on right side of thorax + description of pressing pain in chest similar to feeling of a heart attack, both doctors on shift diagnosed shingles. Forgot I received COVID vaccine 4 days before symptoms started so I never mentioned it to the doctor. TRX= 800mg acyclovir 5x/day x 7 days. Rash continued to get worse and spread more before finally subsiding and starting to crust over. Nearly 3 weeks later, rash is now a combo of scabs and open wounds. Pressing deep pain in right chest is persistent and only relieved partially with 800mg ibuprofen q 8 hours.

Write-up: About 30 seconds after the vaccine was administered the patient collapsed in her chair and began to seize. Prior to the collapse she had asked to sit for a moment and said that she was feeling strange. I stabilized her in the chair until the seizure finished, it lasted about 30 seconds. I then helped her to the floor and had her sit against the wall. She was pale and confused but conscious. She complained of head and neck pain, as well as oral pain. She denied seizures in the past, but states that she may have previously had "seizure-like activity". I asked my technician, to call 911. Once emergency services was on the way I kept her in the immunization room with the door propped open and got her an apple juice. Then I stayed with her until EMS arrived. EMS determined that she was stable and stayed with her until family arrived to take her home

Write-up: The injection did hurt quite a bit more than other injections I have had in the past but it began to feel better and I sat in a chair and was on my phone to send a message, spoke to a stranger who was looking for where to go, and was ok until the feeling of fainting began 10 minutes after the shot. I put my head between my knees but it did not help so I called for help and eventually was helped onto the floor to lie down which began to help. I had vision trouble and hearing trouble and could not speak. I was told my neck became bright red as my face grew white and my hands twitched and my eyes rolled around. I found out later that I had also peed my pants. I have fainted only once before in 1989, and found this reaction pretty frightening although I do not believe it happened as a result of anxiety or fear, the adverse effect itself was frightening and unexpected- not listed on any paperwork I received and was not aware it could occur.

Write-up: On Wednesday May 12th around 615 pm I started having symptoms of a stroke. I was scrambling my words, couldn?t talk right and my left side of my face wouldn?t move when I spoke or smiled. When I smiled my whole smile shifted to the right. I had my girlfriend call 911 because I knew I was having a stroke. The symptoms lasted about 6 or 7 minutes. By the time the emt got there I was talking fine and my face was back to normal. I went to hospital where a cat scan was done on Wednesday night and came back fine. The doctors decided to admit me because they wanted me to get an MRI in the morning, which I did and came back that I had a minor stroke. The doctors think the stroke had something to with me being on an estrogen birth control as well as some family history but my primary care doctor recommend me report this because I did get the Jansen vaccine and it could have triggered what happened. I was discharged on Friday 5/14/2021 and I know have to take aspirin every day for the rest of my life, plavix for 21 days, and Lipitor until my doctor thinks I can come off.

Was also told to report this side effect. I still have the heaviness and pressure in my chest as well as a bit of breathing problems.

Write-up: About 30 minutes after the shot, I didn''t feel right, my face, my arm and my chest was broken out in hives. The arm that I got the shot in felt numb. Write-up: 4/11 around 8am my nose just out of the blue started bleeding profusely. I tried about 40 minutes to stop it but wasnt successful so I was transported to hospital ER where they had to cauterize it just to get it to stop. I was advised I could not take anything but Tylenol to try to ensure the bleeding did not start again. It never really got any better and I began developing a weird head pain in my head, which caused severe headaches which the pain being felt in my face and behind my eye area. After the pain continued to get worse on 4/22 I had a virtual visit with Dr. who prescribed a steroid and antibiotic and advised I see an ENT. I havent had any more nose bleeds but I do still get intermitten headaches from time to time

Write-up: Approximately 12 hours after receiving the vaccine, I woke up around 2:00 a.m. or so with extreme body aches, fatigue, headache, nausea, dizziness, and slight fever. Around 9:00 a.m. or so that same morning, I went to Emergency room because I was so sick I didn''t know what to do. They said I was just having a reaction to the vaccine, and it should go away in a day or so. They gave me Ibuprofen and Tylenol and a shot of toradol. I was then discharged and went back to my living quarters. I had covid already back on January 20th 2021. But the vaccine was five times worse than the virus.

Write-up: Vertigo within 15 minutes, nausea. Subsided quickly. Within the hour, itchy, swollen throat, flushed, very intense vertigo, shortness of breath. Visited Emergency Room (04/06/2021), treatment was IV fluids, and Benadryl. Told to take Benadryl to within 24 hours, and symptoms did subside. After 24 hours, body aches and chills.

Write-up: Patient received the Janssen vaccine at approximately 11:30 AM and was asked to stay sitting for a 15 minute observation period. Write-up: After the 15 minute waiting period, I felt fine. I did my grocery shopping while I was at the store, went home and worked the rest of the day at my desk. We ran errands in the evening like normal. I hadn''t really noticed because we were busy, but my heart rate had been steadily increasing all afternoon and into the evening. My watch tracks my pulse rate and reports on screen. After I took a shower at 9:30, I noticed that my pulse felt high. I laid down, thinking it was just from taking a hot shower and it would go back down to normal. That''s when I looked at my watch report and it looked like a stair step, with my heart rate increasing steadily every few hours, starting at 4 p.m. It was currently 110-120 (my normal RHR is 60, and drops into the 50s when I''m asleep). I went to bed, hoping I was just in my own head about this and it would be fine. I woke at 2 a.m. and my heart rate had not dropped below 100 while I was asleep and was at 130. I went to the ER at 2:00 a.m. because it was alarming. There were points where it spiked to 150 while sitting still. The ER ran an EKG, full blood and urine labs, monitored my heart rate and BP - my BP was in the normal range the whole time. The doctor who saw me in the ER was awesome. Her husband is still dealing with lingering neurological damage from having COVID last year. She informed me that I was not in a-fib or any type of cardiac distress and my labs were all normal, but was likely having an immune response to the vaccine. They did a chest x-ray and CT to check for blood clots and both came back negative. My platelet count was well within normal range as well. I returned home about 6 a.m. and my heart rate was still higher than normal, but had already started to work its way down. It stair-stepped down just like it went up and by 4 p.m., my RHR was back down to 70 and it was down in the 50s overnight. Aside from that, I had the typical headache, low-grade temp and overall body aches. Everything had subsided by Sunday afternoon. I did make an appointment see my PCP as instructed by the ER doctor on Tuesday, April 13. She had read all reports from the ER and was pleased with how my heart rate had come back down to normal. This was the same day that the authorities stopped use of the Jannsen shot due to the blood clot concerns. Dr ran another platelet count for my own peace of mind and it was a little higher than it was when I was in the ER, so she was confident that I was not in any danger, but did review the symptoms/warning signs of blood clots with me just in case. I have not had any issues or symptoms since.

Write-up: Around 1:30pm on Tuesday April 6th, I was sitting at my desk in my home office, working. I began experiencing moderate chest pain and shortness of breath. These symptoms continued to worsen for the next hour. I had difficulty breathing, and taking a full breath was painful enough to prevent me from doing so. I drove myself to the ER. While waiting in the ER, I began experiencing chills, severe joint pain, and moderate pain generally throughout my body beginning around 4:00pm, which I advised ER staff. While being examined by the ER doctor around 7pm, there was some tenderness noted in my right upper quadrant. My blood pressure was extremely high (205/130) and I developed an intense headache. I was admitted overnight and discharged the afternoon of April 7th. While the chills and pain improved on April 7th, my chest pain and shortness of breath continued for another week or so. The night of April 7th at home, I woke up several times with difficulty breathing and ended up sleeping fitfully while propped up at a 45 degree angle so I could at least breathe mostly normally. The chest pain and difficulty breathing slowly improved over about 10 days until I was no longer experiencing symptoms.

Write-up: Pt received his vaccine and returned to the waiting room. Within 5 minutes he slumped to the floor, he lost consciousness and had a brief seizure lasting less than 30 seconds There was a sheriff deputy in the waiting area that asked him his name, year, president and where he was. He was able to answer all questions, but was still shaking. We called 911 and EMTs were here within 2 minutes and took over. After assessing him and taking his vitals they decided to transport him to Hospital.

Write-up: The morning after I received the vaccine, I started having chest pressure and acid reflux. There was a hard pressure in my chest and my heart was beating weird. I started to develop a headache. I had low blood pressure a couple days later. I went to the clinic and they said I may have a blood clot. I also went to Hospital and they took some blood and did an X-ray and they told me I was fine. I was told that it may be anxiety or stress. The chest pain started to go away, but now my teeth are hurting and are sensitive. I feel that the adverse event lasted for three weeks.

Write-up: She got her vaccine, was fine. The next day she was exhausted, but nothing unusual. Then about 4/6/21, a month later she started slowly loosing her vision, and got her vision back for about 2 weeks, and then she slowly started loosing it again. She went to her primary care who told her nothing, and told her to go to the ER. She went to two walk in clinics, the ER, an ophthalmologist, a neuro-ophthalmologist. She has been to a retina specialists, two other neuro-ophthalmologists, and a neurologist who do not know what is wrong with her. They tell her that her eyes look healthy and good but she cannot see. Her left eye she may have about 5% of her vision and 15-20% in the right eye. She was given dry eye drops by one of the earlier physicians. She has had some follow ups, and the neurologist told her to drink water eat low carbs. So, she is frustrated at this point that nobody knows what to do and treat her eyes and is depressed. She lives alone and is not able to see to read her credit card info to order food or to see what she has to eat. She was also given Xanax, which she has not taken yet.
