Write-up: Had flu shot on 10/08/2003. Got sick 10/10/2003. Headache, heavy coughing, Drowning sensation. Spine felt like hit with ice cold axe. exhaustion, and extreme shortness of breath. Shortened my life. Follow Up: Pt is a 59 year old woman who experienced intermittent chest discomfort and profound shortness of breath over the last couple of weeks since she got her flu shot. She has a history of dyslipidemia and hypertension. She had a symptomatic but electrically uneventful adenosine phase with a lot of flushing and symptoms associated with vasodilation. She had no EKG changes and no arrhythmia. She had a left bundle branch block as the baseline EKG tracing which precipitated the adenosine test. She was recovered to baseline and stable on transfer to nuclear medicine for follow up imaging. Medical Records states pharyngitis. msv

Write-up: Pt presented with SIADH and muscle weakness and was coincidentally diagnosed with Guillain Barre. She was treated with PO demeclocycline and IVIG and physical therapy. 04/20/05 Involved case final diagnosis of GBS, with URI Hx prior to flu vax, with onset of S/S 10 days'' post flu vax. Pt presented w inability to stand, walk, myalgia and some worsening of low back pain. On exam strength noted to be 5/5 in all extremities but w absent DTR''s in lower extremities. Pt''s labs were consistent w SIADH and initial intervention focused on correcting the electrolytes. The following day, the chemistries became increasingly altered and mental status changes of hallucinations and disorganized speech. Work-up at that point included CT''s of chest and head (which had unsure clinical significance or negative), LP with Protein level 159 and EMG which showed slowing of nerve conduction in lower extremities and changes in both motor and sensory nerves of upper extremities. Pt in MICU due to Hyponatremia & MRI of head at this time showed tiny scattered areas of deep white matter with a high C2 signal(of doubtful significance). Questional Dx of viral meningitis and lupus cerebritis was ROed and after being transfered back to floor, pt given 4 rounds of IVIG, pt''s mental status cleared dramatically, her sodium level trended upward with increased dose of Demeclocycline. She did have increase in BP, controlled w metoprolol; With these changes, pt transfered to rehab.

Write-up: A report was received from a female vaccinee in the USA on 2/11/05. No details were provided regarding the vaccinee''s medical history or concomitant medications. The vaccinee has stated she has no allergies to eggs. In December 2001, the vaccinee received an influenza vaccination. Within 10 days after vaccination, the vaccinee experienced many symptoms including dizziness, ringing of the ears and total imbalance. The vaccinee was referred to a neurologist who diagnosed her symptoms as a severe reaction to the flu vaccination. The vaccinee underwent physical therapy and it was explained that the symptoms might never completely disappear. The symptoms reoccur when the vaccinee is tired. Follow up information will be sought. A follow up report was received from a female vaccinee in the US on 2/23/05. On an unspecified date in December 2001, the vaccinee received Fluvirin (batch # E10801LA). The vaccinee was treated for what was thought to be an inner ear infection because of extreme dizziness, nausea, ringing of the ears and other symptoms. The vaccinee was seen by a neurologist who diagnosed a severe reaction to the flu shot. The vaccinee was extremely limited in her every day activities. The vaccinee''s symptoms have not diminished and often re-occur when she is extremely fatigued. Further information is expected.

Other Medications: 08 Dec 2004- Began daily dosage of malaria chemophophylaxis (doxycycline). 02 Jan - 15 Jan 2005: As part of smoking cessation class, took Wellbutrin for about two weeks before stopping due to difficulty walking.
Current Illness: None. Initial symptoms of neurological distress began with patient tripping overself and twisting right ankle while walking on level smooth surface. Due to ankle injury and large gravel surface throughout camp, patient had difficulty identifying the fact that motor neurons were being destroyed and attributed difficulty walking to rough surface and ankle injury initially.
Preexisting Conditions: None. Perfect condition prior to deployment.

Diagnostic Lab Data: Upon returning home, the patient underwent an EMG and Nerve Conduction Velocity test along with MRI''s of the head, cervical, thoracic, and lumbar vertebrae. Patient also had a spinal fluid draw along with many other tests to rule out all other diseases.

Write-up: Patient was in excellent shape prior to deployment (no complaints, no medications, running a first class physical fitness test). Patient was preparing to deploy for three months. Patient received six vaccinations during deployment (30 Nov - PPD; 03 Dec 2004 - Yellow Fever and Smallpox (15 jabs); 14 Dec 2004- Flu and Typhoid; 16 Dec 2004- Meningococal). On 30 December, patient began to develop neurological symptoms which were eventually diagnosed upon patient''s return home as Amyotrophic Lateral Sclerosis (ALS- Lou Gehrig''s Disease) on March 15, 2005.

EMG did show significant changes, slowing of responses and denervation in upper and lower extremities.

Write-up: I was given flu shot 10/09/04. Within 7 days began developing a rash within 2 weeks had leg weakness, balance issues, falling, loss of reflexes. SS lasted 2 1/2 months, went away then returned in Feb. following removal of appendix removal. Now Dx. CIDP. 07/25/05 Neurologist''s statement GBS ''or initial episode of CIDP'' ; EMG done 12/13/04 positive for diagnosis. Notes from 03/15/05 visit includes information of pt''s reporting being ''back to normal'' in January, having surgery for a ruptured appendix late February and pt''s perception of ''starting to go down-hill again''. Visit of 04/19/05 includes details of exam confirming clinical symptoms with changes in vibratory sensation, continued areflexive, slowed gait. Pt''s diagnosis ws revised to CIDP and the decision to treat with IVIG is recorded.
