Write-up: 1st day of vaccine; I felt very tired and fatigued and my joints started to hurt and I could not walk due to the pain in my joints. 3rd and 4th day of vaccine; I started to experience numbness sensation to my hands. I also had numbness sensation to my face, right side and lips and also to my head. I went to Urgent Care where lab work was performed as well as a CT scan of the head to check for blood clots and results came back normal. Also an U/S of the right leg to check for any blood clots was performed which came back normal. I then went to see my PCP who performed more tests and exams, again did lab work to check for blood coagulation and still continued with the numbness sensation to my hands and all to my body. After about 10 days of my vaccine, I noticed that my oxygen level was 87, 96, 86, 92 and where my oxygen level has always been around 98 or 99. I will be also having heart studies in a few weeks. I have not recovered from my symptoms and feel very fatigued and tired all the days.

Write-up: Patient began sweating and feeling lightheaded shortly after vaccination at approximately 1125. Patient then fainted in his chair and was initially non-responsive to the nurse. At this time I called 911. The nurse administered a single epi pen in patient''s quad and he woke up. The nurse then took patient''s vitals at 1138: BP - 109/73; P-84; SPO2-99%; T-97.4; R-16. Patient stating he was still feeling dizzy and fatigued. The paramedics arrived and took patient to hospital for continued evaluation. I called patient later in the afternoon and he had already been discharged and was at home resting and feeling better.

Write-up: Almost immediately after injection was given, patient complained of chest pain, light-headedness and sweating. He admitted to a previous history with past vaccinations although inconsistently. Chest pain resolved, per patient. Patient transported by wheelchair to paramedic area with brief loss of consciousness, spontaneously resolved. Patient able to transfer from wheelchair to cot, reporting improvement when lying down. Paramedic and EMT monitored vitals, paramedic advised transport to hospital to rule out cardiac event, patient consented. Tranported to Hospital

Write-up: High pitched constant tinnitus primarily out of left ear starting about two days after vaccination. The tinnitus lead to generalized anxiety and difficulty sleeping. ER prescribed sleeping aide when it first started. ENT prescribed a round of methylprednisone which did not fix. Still ongoing and has not resolved.

Write-up: In first 24 hours, at 03:00 am, I woke up feeling chills and with headache and I got dizzy and feeling weak and tired. My muscles hurt and a body felt very fatigued. Also, a little bit nauseous. I think my fever got to 100 degrees. By noon, I broke my fever. By three pm I was fine again. The following days, I had soreness at injection site. I had a couple of headaches. There was one episode where two weeks later, I did get a panic attack and went to urgent care. I went because it lasted longer than normal. This panic attack lasted for 30 minutes that is why I went to Urgent Care. Usually my panic attacks last for 3 minutes - but I also had personal things going on that could have caused it. No treatment, they just said to relax.

Write-up: On 4/5, I started with upper stomach pain and then two days later I had a fever and a very rapid pulse. I went the walk-in clinic on 4/7 and they performed COVID (negative), an EKG and CT Scan of abdomen (both were normal). The fever went away, the stomach pain eased but I still had SOB, fatigue and rapid pulse. Three days later, the symptoms remained, so I saw a gastro doctor and he said it could be COVID or blood clots, then sent me to hospital on Monday, 4/12 and 4/13. They did 2 more EKGs, COVID tests and bloodwork. I still had SOB and pain in my upper stomach towards my chest. On 4/14, I woke up with a massive headache and saw the news about the vaccine, so I went back to the ER to get checked for blood clots. They did an EKG, CT Scan of the abdomen and head, along with more bloodwork. Everything was normal, but my bloodwork showed an elevation of something that indicated that I have an infection, but they weren''t sure what. They sent me home with no diagnosis. Over the next two days, I rested and I gradually started to get my strength back. I went back to work that Friday for the first time since 4/7.

Diagnostic Lab Data: The paramedics were called out to me twice for nearly fainting at work. The first time was on April 6,2021 and the second time was on April 11, 2021. On the first time, I refused to go to the hospital but the second time I had too. While in the hospital, my blood pressure still stayed high the whole time. Write-up: Received shot on 4/7. Felt fine three days after on 4/10 and went for a standard run. Morning of 4/11, woke up with three massive bruises on my knees. On left knee, bruises were located on the inside and outside of my knee. On the right knee, bruises were located on the inside of my knee. These bruises were at least 3-4 inches in diameter. News of blood clotting from J&J vaccine was released on 4/13. Days after 4/11, knees and ankles started to swell. By 4/14, the swelling in my knees and ankles continued and I became fearful of blood clots that would become life-threatening. Evening of 4/14, admitted myself to the ER.

The patient''s height, and weight were not reported. The patient''s concurrent conditions included depression, and penicillin allergy. The patient had no current illness. The patient had no heart problems, no pulmonary problem and diabetes. The patient experienced drug allergy when treated with paracetamol. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, intramuscular, batch number: 206A21A expiry: UNKNOWN) dose was not reported, 1 total, administered on 11-APR-2021 to left arm for prophylactic vaccination. Concomitant medications included levomilnacipran hydrochloride for depression, and quetiapine fumarate for depression. On an unspecified date in APR/2021, little more than a week after single-dose coronavirus vaccine at hospital facility the patient felt general malaise and minor pain in her right side. The patient did not had any illness at the time of vaccination, no similar event in the past. He never had any adverse event in past for vaccination. The patient''s past medical history included syncope (when she had blood draw), and concurrent conditions included fear of syringes. Patient was not pregnant at the time of reporting. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 205A21A, and expiry: 23-JUN-2021) dose was not reported, 1 total administered on 19-MAY-2021 for prophylactic vaccination into Right Deltoid. No concomitant medications were reported. On 19-MAY-2021 HCP (Health Care Practitioner) reported that the patient experienced syncope after receiving the vaccine. Patient fell face down for about 5 seconds from a sitting position seconds after receiving the shot. HCP called 911. The patient experienced cut on lip from the fall. The patient''s height, and weight were not reported. The patient''s concurrent conditions included breast cancer. The patient experienced drug allergy when treated with hydrocodone, and oxycodone for drug used for unknown indication. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 1805029, and expiry: unknown) dose was not reported, 1 total administered on 20-MAR-2021 on left arm for prophylactic vaccination. No concomitant medications were reported. On an unspecified date in 2021, the patient felt numbness on both arms. Numbness got worse and patient went to emergency room. Write-up: A few hours after the vaccine, began having a headache. The next day had severe body aches and chills, which continued for another 4 days. I missed 4 days of work. I began feeling better a week after the vaccine. 24 days after the vaccine, I was awaken with excruciating pain in my left abdomen, and had severe diarrhea. The diarrhea lasted most of that day, the pain continued. I went to an Emergency Room on 4/1/21, and a CT scan revealed what appeared to be a stomach ulcer, a kidney stone and a post-menopausal ovarian cyst, all of which were causing the pain. I have been through many tests and seeing a variety of doctors, and currently am awaiting results and treatment plans for the stomach problem and the ovarian cyst. The cyst has grown and is causing continued pain. The stomach issue is being treated with Sucralfate 1 g 4 times per day and Pepcid 20 mg.

Write-up: The Tuesday after vaccination, I had physical discomfort and pain inside like period cramps. It felt like there was a tampon inserted weirdly, I tried a yeast infection solution with yogurt on a tampon. Later that week I had pelvic bone pain like I had strained to hard using the bathroom. Friday I got a hyper sensitive spot on upper right thigh, diagnosed as paresthesia, the swelling and discomfort around pelvis was still there. Saturday the swelling and discomfort on the insides started going down, both legs experienced pain, It felt as If I had done a workout and pulled my muscle. I typically have a high threshold for pain, No change on Sunday. Monday the back of right knee stated aching, sensitivity in left thigh. Both muscles on outside of legs felt like I had done a workout and needed stretched, On the 13th the aching traveled from the back of knees to calves I went to the Hospital. Myalgia and paresthesia and acute vaginitis. Shot of Toradol in left arm, which did nothing. Did ultrasound with normal results, no blood clots. The 14th my legs were comfortable as long as I was moving, started having a sharp stinging in left calf. The soles of my felt felt blistered but no visible blisters. Significant headache. I wore compression socks. I got a weird Charlie horse in left foot toes, top of right foot got really sensitive also. Volotoral is a topical pain cream that I had I used it on specific areas, of pain. Thursday the 15th the left outside of my foot was still sore, my calves had gotten better, the outside of the calf still had hypersensitivity. The muscles on the back of my knee''s and calves were still sore. While driving my Hip and thighs for aching. Ankles are popping a lot. Fri-Sun following I had leg stiffening and muscle soreness mainly in the left side, went for a walk that weekend and while walking they felt ok, until I stopped walking. Mon-tue after I had a headache that felt like my brain was burning near the left temple area.

Write-up: I have an extreme fear of needles and we thought that the dizziness came from that and the nauseous came from that as well, that night I had a tough time sleep because my arm was so painful and over the next several days my arm had became inflamed and red we noticed that my sugar had started climbing. So I was kinda okay Monday still kinda sick and nauseous at 6:00 am that morning I started to throw up and I threw up at 6 times my mother had to call 911 and I was taken to hospital in the squad my sugar was 501 I was dka, I had to go without food and water for over a day. I had complications with the insulin and the shot was causing my sugar to rise and there were complication. I was not able to lay on my left for a week longer maybe a little longer. I think my shot was infected it started to leak white goop as well. I was on an insulin drip for a while, my pump was not working so I had to stay longer until a new pump came. I went into the er Tuesday morning and came out Thursday morning

Write-up: The participant woke up with a severe migraine headache. He couldn''t focus on anything everything looked blurry, He went to work and couldn''t focus on numbers, every time he tried to read everything was blurred, He had a constant headache for 2weeks and his eyes are still experiencing blurred vision. He heard that several others also experienced reactions who were vaccinated at his job. He missed 2 days of work and went to urgent care, where he was advised to go to the ER immediately. Write-up: About 2 weeks after the vaccine I developed SOB and went to the Urgent Care where I received an irregular EKG so I was transported to the ER and was admitted into the hospital for 3 days. On 5/1 I developed severe chest pains again so I went so I went to another Hospital and was informed that my D-Dimer was elevated and was transferred to a different hospital where I was monitored for 24hrs and was released. Write-up: More than 30 days after receiving the Janssen vaccine, I tested positive for COVID-19. Symptoms began as runny nose, sneezing, and sinus pressure/burning on April 6, 2021. Tested for COVID-19 on April 11, 2021 (nasal swab, PCR test) after a family member tested positive. Symptoms worsened on April 12, 2021, including shortness of breath, chest pain, complete loss of taste and smell. Sought treatment for breathing difficulty at ER on April 13, 2021 (Vidant Medical Center and was instructed to rest at home. Sought treatment for breathing difficulty at PCP on April 14, 2021 ( Medical Center) and prescribed Prednisone. Symptoms worsened on April 15, 2021, including shortness of breath, chest pain, numbness in face/arms/legs, dizziness, loss of balance, memory and speaking difficulty, near fainting. Sought treatment at ER on April 15, 2021 (Vidant Medical Center, Greenville, NC). Held for three days, treated with Remdesivir (3 doses) and supplemental oxygen. Discharged from hospital on April 18, 2021. Symptoms did not improve following discharge. Sought treatment for ongoing symptoms at ER on April 21, 2021, instructed to rest at home. Symptoms continued to worsen on April 24, 2021, including shortness of breath, chest pain, numbness in face/arms/legs, dizziness, loss of balance, memory and speaking difficulty, near fainting, and slurred/incoherent speech. Write-up: 22 year old received Janssen vaccine at 10:50 am, and while in recovery area, reported that he became light headed, dizzy, and passed out. He slumped out of the chair hitting the right side of his head on the floor. Sustained a cut near his right eye. At 10:56 am blood pressure @140/65, pulse @ 53, 97% O2 sats while sitting on the floor. EMS was called 10:57am. When interacting with staff and friend accompanying him, eye was cleaned and ice applied. EMS arrived 11:05am. EMS reported vitals to be 86/49, 59, 100% O2. EMS recommended transport to client. Client accepted. EMS transported to Medical Center at 11:12 am.

Write-up: Patient looked normal and relaxed before and during vaccination. About 5-10 minutes after vaccination, patient passed out (fainted) while sitting down in a chair. Moved patient on to floor lying down flat and observed him for a few seconds. Patient was un responsive and appeared to struggle to breath. Asked my staff to call 911 for emergency services, and to bring an Epipen just in case, and started to perform CPR on the patient. The patient gained consciousness in about 15 chest compassions and became responsive. He answered the questions like his name, where he lives etc. As the patient became conscious, responsive and appeared to breath normally, I did NOT administer Epipen. Kept patient in observation and kept him engaged in talking until EMS personnel have arrived. They checked his vitals and blood glucose. In the conversation with EMS personnel, they said he is hypertensive and hyperglycemic. Ems took him to the ambulance by walking him (stretcher wasn''t necessary)

Write-up: The day after I had flu like symptoms. Write-up: The patient states that symptom onset started at 8 AM. She awoke at 5 AM and felt fine. She complains of bilateral weakness in the distal upper extremities and in her bilateral grip. She states that she has never had this weakness, which is worse on the left than on the right. MRI C-spine showed an Abnormal cord signal from C3/C4 to C6/C7 involves both halves of the spinal cord with a predilection for the central gray matter. No pathologic enhancement. Write-up: That next Tuesday, I felt a swelling in right armpit, I went to the urgent care. It was warm and swollen and looked like the start of the infection. They prescribed sulfameth/trimethoprim 800/160mg 2x a day which didn''t help anything and I went back 2 days later. The abscess had swollen to golf ball size. I also had swelling at injection site, hot, red, & painful. Doxycycline 100mg. That started helping but the arm was still painful. Hidradenitis suppurativa was the diagnosis. Dermatologist says that it was caused by a autoimmune response. Armpit was injected with a steriod/lidacaine injection. I had to have wound treatment. Clindamycin lotion, 1% to help would site clean. The issue was the injection site area that took 2 weeks to resolve. It was hard, red, raised and hot. They were not sure if it was an allergy or infection. I also felt like I was in a brain fog during this.

Write-up: On 05/25/21 at 03:23PM patient received the Janssen Vaccine Covid-19 and 10 minutes post administration she had 2 anxiety induced seizure (pseudo-seizures). The first seizure lasted about 2 minutes, followed by a second seizure that lasted approximately 30 seconds. The patient stated she''s gotten anxiety induced seizures in the past, but never after a shot administration. She was fine for 10 minutes, then said she felt a seizure onset. We lowered her and placed her flat on the ground. Her heart rate was around 100bpm and blood pressure 140/90. The paramedics came to the scene and took patient to Hospital to get checked out.

Write-up: Within 15 minutes, I started to feel dizzy, then numbness in my face, hands and legs. I then started to feel sweaty and my hands were ice cold at the same time as feeling sweaty. The pharmacist called 911. I was taken by ambulance to the hospital. An IV was started in the ambulance, and I was given oxygen because I started to feel nausueous. Write-up: I am currently lactating and breastfeeding for 11 months. The afternoon that I got the vaccine I got a fever, lethargy and extreme fatigue. I was nauseous and I had diarrhea. My fever was 102. I started to feel better after three days. I also got aa headache that was similar to the Migraines I used to have. I hadn''t had a Migraine for 18 months and I had a Migraine daily for five weeks. I went to the care facility eight days after the vaccine because I thought I had a UTI. The UTI was positive and I was told I had some Streptococcus B at the time. I was also experiencing confusion, and tiredness. One month after the vaccine I went back to the care facility because I was have having cramps in my lower abdomen and circulating to my lower back as well as my legs. All tests were negative but I was given multiple antibiotics. After eight weeks I still have lower abdominal cramping, vaginal discharge and headaches. Some of the symptoms were relieved by the antibiotics but they have returned.

Write-up: I have heard, and saw recently that a small group of primarily male adolescents and young adults have developed difficulties with their hearts after taking the vaccine. Write-up: Janssen COVID-19 Vaccine EUA Patient had her Janssen vaccine on May 10th at a Pharmacy. On May 18th the patient presented to the ED with complaints of abdominal pain and chest pain. At that time troponin was negative, ECG was without acute ischemia or ectopy, D-dimer = 1.94. CT was negative for acute pathology, Ultrasound negative for hepatobiliary etiology. Patient requested discharge home and was referred to GI and cardiology. Patient presented to ED again on 5/19 with worsening symptoms. Repeat CT consistent with bilateral adrenal hemorrhage. Heparin Induced Platelet Antibodies PF4 Absorbance elevated at 2.138 and interpreted as positive. D-dimer greater than 20, and platelet count less than 79.

Write-up: Initially I experienced nerve pains across my body, including under both of my armpits, in my groin area, in my arms including my triceps, forearms and they were very sharp pains. Then I started to get a severe headache that was pretty constant. After multiple days, my face became paralyzed. I also experienced severe fatigue. This was over 10 days. Once paralysis happened we went to the Medical Center. I was treated with IVIG for many days and oral steroids. I was in the hospital for 6 days. I also experienced my BP spiking. I had an abnormal EKG. I''m still unable to chew properly. I was told that now I have Encephalitis and GBS (Gillian Barres syndrome).

Write-up: One week after receiving the vaccine I woke up on 5/21 with a mild rash. As the day went on the rash progressed and swelling of lips and uvula occurred around 5pm. I was taken to an urgent care who transferred me to a hospital via rescue. I spent the night at the hospital receiving multiple epi pen injections, and fluids. I was discharged 5/22 with a dose of prednisone. I was fatigued all day. On 5/23 I woke up to swollen eyes and lips and covered in hives again. I went to the ER where I was given steroids and fluids again. The hives went down but did not go away. One 5/24 I took multiple doses of Benadryl and dose of prednisone and in the evening time the hives progressed. Today 5/25 I woke up with swollen lips and checks and hives. I went to my doctors office to be evaluated.

I was having it on all joints and not just leg joints. It was moving to different joints around the whole body. The doctor said, If it lasted for more than three weeks that I should come back. The second week the pain 4; The third week pain was between 2 or 1. At end of third week it kind of cleared up.

Write-up: The side affects: fever, body aches, headache, and flu-like symptoms (chills, sweats) and that lasted between 11:00 am - 03:00 am Tuesday morning. What continued was the extreme body and joint pain. That was probably about a 6 or 7 on the pain scale. That lasted from Monday and then on Thursday and Friday I couldn''t sleep because of the pain. I called the doctor because there were things on the news about calling dr. with leg pain. I couldn''t bend down and pick up anything. It felt like I had horrible arthritis. Dr said to take Advil and Tylenol if I needed help - that helped when I slept. I have a very physical job that probably made it worse - lifting and pushing - it would be worse.

Write-up: Around 4pm, I started feeling nauseous, along with stomach pain, chills, headache, diarrhea and two of my fingers went numb. Write-up: Patient received Janssen COVID19 vaccine (LOT#201A21A) in his left arm at approximately 14:43 on 5/25/21. During the 15 minute observation period, he expressed "I don''t feel well" and endorsed dizziness/ lightheadedness, slight tingling feeling in fingers, and nausea. Shortly thereafter at approximately 14:50, he started to slouch and appeared faint while sitting in a chair with eyes closed. He did not fall or hit any part of his body. He was easily aroused with a touch on his arm and responded to verbal questions immediately. He denied trouble breathing, chest pain, palpitations, headache, muscle weakness, vision changes, skin reactions, swelling. He was oriented x3 and not exhibiting verbal or swallowing problems. His vitals were normal- HR 80 bpm and BP 105/80 mmHg. He was offered a receptacle to vomit in if needed; however, he did not vomit. This initial reaction was consistent with potential vasovagal reaction/ near-syncope after vaccination. Patient was encouraged and instructed to sit on the floor to prevent injury in case of fall but he refused. Staff offered him water, a fan, and opened windows while monitoring. Later while waiting for a ride at around 15:10, he endorsed headache (first above his eyes and then toward the back of his head) and clarified this did not feel like a typical migraine that he has had before. Then at around 15:20 he endorsed vision changes in his right eye- he could not see a portion of vision out of that eye except peripheral vision and described not being able to see writing on the right side of a whiteboard in front of him in the room. Later he described a resolution of that gap in vision but then had blurry vision out of that eye. He denied any vision changes in the left eye. Due to alarming symptoms of vision changes and onset of headache, patient was recommended to go to ER for further observation and evaluation. He was not in urgent distress and patient did not wish for EMS to be called but agreed to go to ER when his emergency contact picked him up. Patient''s emergency contact came to pick up patient at approximately 15:40 and was recommended to take him to the ER. Patient and emergency contact were amenable to this plan. He was able to stand up and walk to the car without issues.

Write-up: I had about 20 min of being fuzzy headed. I couldn''t remember the names of people I''ve known all of my life, including my best friend and boss. I couldn''t think of a town I had vacationed to. My daughter could tell by looking at me that something was wrong. My nephew took me to the ER. They CT Scan, EKG, COVID test, blood work and took my vitals. My BP was 196/84. They gave me IV meds to decrease my BP. They needed to do an MRI, so I was sent by ambulance. Write-up: Administered about 3:30pm and sent to observation area. At roughly 4:40pm client started getting dizzy and closing eyes, color gone from skin with lips getting grayish-purple. At 4:45pm client was going in getting "clamy" skin, sweating some. I had him recline in a chair and administered 1 dose (0.3mg) epinephrine using an auto-injector pen and called for emergency services. At 4:50pm blood pressure was 125/80. Client still coherent. Had mentioned that he had not eaten that day so gave him some saltine crackers and juice. Color started to return and emergency services arrived to take over.

Write-up: After about 20 minutes of observation she started to dry heave/spit up white bubbles. Write-up: After vaccination, patient fainted and by the time got over to him he was alert. He was able to sit up and then was able to get into the chair.
