Syndrome. 5/11/05 Sore throat, fatigue. 5/14/05 Numbness and tingling to hands and fingers. 5/15/05 Numbness/tingling to feet. 5/15/05 Deep throbbing, pain in arms up to elbows, pain in shoulders, increased NBC. 5/18/05 Difficulty with gait. 5/19/05 Hospitalized. 5/23/05 Transferred to another hospital and placed in ICU; Condition critical for 4 days. 5/24/05 Lumbar puncture. 5/25/05 LP findings, Guillain Barre. 5/30/05 Step down from ICU. 6/3/05 Admitted to physical therapy rehab facility. 7/28/05 As of this date, discharged planned for 8/2/05. Received hospital medical records for 5/19-6/3/2005. FINAL DX: acute inflammatory demyelinating polyneuropathy Records reveal patient experienced bilat ext numbness & tingling w/progressige arm/neck pain x 10 days prior to admit. Had sore throat & fatigue approx 3 wks prior to onset of s/s. Transferred from outlying hospital. On admit to ICU, had weakness of all extremities, urinary retention, constipation, respiratory insufficiency. Neuro consult done. Tx w/IVIG x 4. Slow improvement & transferred to inpt rehab. Per follow up report-Continued muscle weakness, numbness, tingling and pain to lower extremities. Muscle instability. Unsteady array gait. Per annual report-Continues to have numbness and tingling to lower extremities (feet & toes), balance is offset by this (numbness and tingling). Physical stamina has not returned to pre-illness level. Has decreased muscle strength in thighs and those muscles fatigue more easily.

Write-up: Upper arm shoulder and eventually neck ached continuous for 1 year like when you get the flu shot. Then the spasms that lasted until 2004, Then weakness in the biceps and shoulder and neck. Now I have cramps in them so bad I have to straighten it out which hurt tremendously.

Write-up: Pt woke me up about 11:30 pm and it scared me because I did not recognize his voice. He sounded extremly hoarse, he was very pale, skin was cold and clammy and wheezing. He said he was having much trouble breathing (getting his breath). I called the emergency dept. and spoke with them relayed Pt''s symptoms and I asked if this was a reaction to the flu vaccine I was told they did not think so that any symptoms occurred 10 to 12 hours after the shot. We then made the pt as comfortable as possible and it seemed to start to give him some sort of ease. About two this morning he finally dosed off into a fitful sleep this was on and off for the rest of the night, extreme chills, sweats, nausea persisted. This morning he was still having the occasional chill, very weak, lightheaded, and the breathing problem was much better but uneasy feeling was still here. His eyes are very red, his skin is a little pale and his head hurts. He states he still feels nothing like he did last night. The pt is a owner operator and we had to cancel todays load and Friday''s load. It is impossible to drive in this condition. I called and was told to fill this form out.

Write-up: The child was found unresponsive in her crib. The child was experiencing a seizure (tremors) which involved her right upper and lower extremity and right side of face. The child was foaming at the mouth. The seizure lasted approximately 15 minutes and paramedics were called. Pediatrician was paged. By the time paramedics arrived, the seizure had ended. We were advised by pediatrician to take the child to the hospital emergency room. After that first incident, the child had 11 seizures in the last 3 years. The child is currently under the care of pediatric neurologist and several anti-seizure medications were tried. The child is currently on her 3rd anti-seizure medication. Per follow up 01/03/2006 she now has a seizure disorder and is on anti-seizure medication. Follow up received 04/05/2007. Patient has been on seizure meds for over 3 yrs now.
