The patient''s weight, height, and medical history were not reported. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, and batch number: 1805029 expiry: unknown) dose was not reported, 1 total administered to left deltoid on 14-MAR-2021 for prophylactic vaccination. The patient received Benadryl unspecified (form of admin and route of admin were not reported, batch number: unknown expiry: unknown) dose and frequency were not reported from MAR-2021 for itchy rash. The batch number was not reported and has been requested. No concomitant medications were reported. On 14-MAR-2021, after eight hours of vaccination, the patient woke up with intense muscle cramping which she had anticipated but also had joint cramp. The patient had temperature of 103.5 (unit unspecified) and the patient''s right side of face was itching down into under her ears which also started to swell. The patient had a rash on her legs, arms, face and everything was itchy for her. The patient took a diphenhydramine (Benadryl) and then she had headache, dizziness, ringing in the right ear, nausea, and her fever jumped to 104. The patient laid down her head into floor because the dizziness was so bad and she ended up in more muscle cramping. The patient''s fever never went away but went down and did not go for about a week and three days; the fever was just hanging out at 100 (unit unspecified) the whole time. The patient''s dizziness and ringing was going on and also had GI (gastrointestinal) problems and really bad headache. The patient called a healthcare company and ordered meclizine (Meclizine) but did not specified she administered it or not. On 26-MAR-2021, the patient could not take the stomach pain, cramping, diarrhea, high fever, rectal bleeding and mucusy and bloody stool. The patient had intense abdominal pain and GI (gastrointestinal) issues after three days of her fever and was taken to ER (emergency room) and was confirmed that the patient had renal insufficiency and medication induced colitis. The patient had colitis and was given with metronidazole (Flagyl), however she had low grade fever, dizziness, ringing in ears, really bad headache and was admitted to hospital on 28-MAR-2021. The patient''s CRP (C-reactive protein) was up, lactic acid was elevated, hemoglobin went from 14 down to 12, WBC count elevated, occult blood was positive and also had rectal bleeding, a foggy headache and dizziness feeling. The patient felt something was way off in her body at the side, also had a small orange halo on arm at the injection site which got resolved after four days. The patient had undergone unspecified tests and the results was negative. The patient liver enzymes were too off which was hand in hand with medication induced colitis. The patient had a small internal hemorrhoid and removed a polyp. The patient''s heart rate experienced some issues as well. The patient was hospitalized for 6 days. The patient was discharged on 02-APR-2021.The patient had a follow-up with her physician on the next day of this report. The action taken with covid-19 vaccine ad26.cov2.s was not applicable; and action taken with Benadryl unspecified was not reported. The patient recovered from had polyp on MAR-2021, generalized itchy rash on 16-MAR-2021, muscle tenderness on 06-APR-2021, and redness at injection site on 18-MAR-2021, was recovering from ringing in the right ear, rectal bleeding, medication induced colitis, joint tightness, lump at injection site, bloating, swelling at injection site, and small internal hemorrhoid, had not recovered from fever, headache, and dizziness, and the outcome of intense muscle/joint cramping, nausea, unable to eat or drink and renal insufficiency was not reported. The patient experienced drug allergy when treated with cyclobenzaprine hydrochloride, and tetanus vaccine for drug used for unknown indication, drug allergy when treated with clarithromycin, diazepam, and hydrocodone bitartrate/paracetamol. Write-up: 7 minutes after the injection of my Janssen injection for COVID-19, a heat enveloped my body from deep in my chest and I went pale (according to the objective findings of my nurse) and I began to feel lightheaded and almost became syncopal. I was asked to remain there for 30-minutes, and began to feel better shortly thereafter. The next several days, the lightheadedness came and went in waves, which became stronger and stronger. Two days post-injection, I developed severe joint pain where I''d otherwise previously had none - for instance, my low back ached with 7/10 pain which only went away after the first week. I had thoracic spinal pain which radiated to my chest, and by day 3 I was having serial panic attacks (which I don''t have), such that I visited the local emergency department to rule out sinister cardiorespiratory sequelae. I was discharged with a ddx of GERD, though I do not believe this to be true. Since then, I have largely recovered, however the lightheaded spells have dissipated only a little but are largely still plaguing me. Thankfully, the panic attacks have decreased to the point of almost being gone altogether.

Write-up: Both sides of my neck were very swollen. There was extreme pain on my right arm. The pain was severe and numbness in my finger. I went to the ER and they said I have radial thrombus, a clot in the radial artery. Before the shot, my heart was checked multiple times and I was fine. After the shot, my coronary arteries are now very small. There are now luminal changes inside my coronary arteries which are still now very small. I now have been diagnosed with CAD. Write-up: On May 2, 2021 I started to feel weird. I started to feel weepy and emotional. My body started twitching. I kept having involuntary movements in my legs, arms, and head. Write-up: Two weeks after receiving the vaccine my legs started swelling. I went to my PCP and he measured my legs and sent me to hospital because he said that it looked like I had blood clots. While in the ER, they started performing tests, and they found a DVT in the left leg and to PE''s, one on each lung. I was under observation because I was given Lovonex to thin my blood. While in the hospital the swelling started to go down and that is when I was discharged. I am now seeing a vein specialist and a pulmonary doctor. I was given Eloquis 5mg 2x per day. I am still having pain and swelling in both legs, but the leg with the DVT is worse. It is very hard to walk around because my left leg will swell a lot.

Write-up: When I received the injection I immediately felt an electric current flow up to the right side of my neck. It passed quickly though. Around eight hours later I developed stiff muscles in my right neck. It became twice the size and was very swollen. I called my PCP and I was told to use moist heat. I used a warm washcloth and kept heat to it. I did not take any medication until the following morning. I took Motrin at 6:00AM. The next day I still felt sore and it took a few days for the muscle ache to go away. Two weeks after the vaccination I started having numbness in my hands and feet. I figured it would just go away and didn''t pay much attention to it. 05/25/2021 my right hand was very numb and my right arm was very achy. I went to urgent care because of worry for a blood clot. While at urgent care, I saw a physician, he ruled out a blood clot, but he said that I may have Peripheral neuropathy. I was told to follow up with my PCP and it was suggested to see a neurologist.

Write-up: Patient is a 48 y/o female who speaks only Spanish. Patient had interpreter present during client registration, vaccination and post vaccine occurrence. Patient was given Janssen COVID vaccine Lot # 1805031 in the left deltoid by this nurse at approximately 2:00PM. Client left the event without c/o after 15-minute observation. 2:55PM Client returned to the event and according to the RN client c/o drowsiness and dizziness. RN reports client HR 80 bmp (strong & regular); client B/P in right arm 152/100. Client c/o a headache at #8, on the pain scale of 1-10. Client also c/o pain in the front of her head. Client was alert and oriented skin warm and dry, color WNL and no gross dyspnea noted; client c/o left side chest pain traveling up the left side of her arm. 2:57PM Explained to client 911 will be called and 911 will decide if emergency room visit was warranted. 3PM Client remains alert and oriented with some diaphoresis and change in color noted. 3:02PM Client color returned diaphoresis improved 3:04PM Client reports she is ?feeling better?. 3:05PM Paramedics arrived. Client evaluated by paramedics. Paramedics applied oxygen and checked client vital signs. According to the paramedic client breathing was normal with good oxygenation. Client reports to paramedics she had COVID last year. The paramedic conducting the client assessment reports he felt client is hyperventilating and has no s/s of an anaphylaxis reaction. Client relayed to paramedic her headache is gone but her head, left arm and leg feels ?numb?. Client agreed to go to the emergency room. 3:25PM client left with via stretcher for ambulance transport to Emergency Room (according to paramedic ED was on diversion). 5/26/21 1:00PM Spoke with RN who reports client was discharged from the ED 5/25/21 to home and is feeling better. Per RN client plans to return to work 5/27/21.

Write-up: Twelve days after the vaccine I got a severe headache with blurry vision. I was also short of breath and my legs were swollen. I was dizzy and had body aches mostly on my left side. My left kidney was inflamed. The pain went all the way on the left side of my body from my ribs to my legs. My left leg was the most swollen. Many of the symptoms lasted for two and a half weeks. The headache lasted for three days. The first week was the worst. When I went to the ER after a week they did not find any blood clots. I had a doctor appt - yesterday but that was for arthritis pain that is in my knees, wrists etc - he gave me arthritis medication and I noticed the positive affects of it already. Write-up: First felt pressure on right side of head on Tuesday May 4th. Took Vaccine on Sunday May 9th. Experienced fever, chills, and dizziness until Tuesday May 11th. Noticed head pressure increase and spread to left side of head and top side of head on Thursday May 13th. Noticed head pressure spread to back of head on Friday May 14th. Pressure and tenderness on all sides of head, with pulsating superficial veins make it difficult to sleep, as pain increases in conjunction with amount of time resting head on pillow. ER doc persribed fioricet on Wednesday May 19th. Felt tingling and numbness down left side of body in reaction to it on Friday, May 21st. Visited ER on May 21st, and received migraine cocktail. Helped relieve pain so that I could lay my head down to rest, but did not stop pulsating throbbing of superficial head veins. May 24th, neurologist prescribes steroids for pain and inflammation, and sleep medication so I could sleep. Sleep noticeably improved. Still testing out 50mg dose of steroids. Steroids noticeably do nothing for pulsating throbbing superficial veins. *Note: I worry this vaccine may have exacerbated the headache I was feeling prior to my vaccination, or that the vaccination was fine, and what I am experiencing now is merely a extension of the headache I was feeling prior to vaccination.

Write-up: 12 hours later after I had the shot my heart was pounding so hard it woke me up. Tried to look up information and couldn''t find information about a speeding heart rate. The more I moved around the higher my heart beat got. I ended up calling 911 and they suggested I go to hospital, admitted into the ER. Write-up: Person who administered hit a nerve, headache, dizziness, nausea, and a metal taste in my mouth. After 45 minutes. resting heart rate jumped to 140 and normally 70, after 45 minutes, it went down to 130 and I had my husband take me to the car. After 10 days I continued heachache, ear ache, extreme fatigue, nausea, my resting heart rate was still at 130 it would not go down, I had swollen lymph nodes under both arms. On the 10th day i called the doctor and went to ER my left leg began swelling up and I had extreme pain from my hip all the way down to my ankle. and plus all the prior symptoms. The doctor did a vein ulta sound didn''t find anything. The doctor laughed at me like I was crazy. I have high pain tolerance and I was in extreme pain. And the doctor did not take me seriously. I followed up with the physician the next day. and asked her to report it, but as far as I know she didn''t report it. I just started feeling better, May 20 th, from april 6. for me to start feeling normal again. Write-up: Covid-19 Janssen vaccination administered on 5/24/2021 at approximately 1230. At approximately 1247 patient noted to have flat red rash to left side of neck moving towards chest and same rash on left antecubital space. Patient reported feeling hot. Patient self administered albuterol inhaler, two puffs. Lung sounds per auscultated expiratory wheezing bilateral upper posterior lobes and right middle lobe, no stridor. Post inhaler lung sounds very faint expiratory wheezing upper posterior lobe, right middle lobe cleared. At approximately 1400 noted rash to bilateral underarms started to return, rash to left side of neck started to raise, noted dermatography to bilateral sides. EMS activated. Perioral edema noted, patient reported starting to feel short of breath, epi-pen 0.3mg IM administered, oxygen at 2L per NC applied. Ambulance on prison grounds at approximately 1412, left grounds at approximately 1429 to Emergency Room for a higher level of care. Treatment at ER not available. Discharged from ER on same date at 2246. On 5/25/2021 at approximately 0215 patient reported pain in jaw, neck and chest. EKG obtained, normal EKG. Follow up scheduled with medical provider.

Write-up: ER- TWO VISITS- 4/10/2021 4/12/2021. I''ve seen a couple doctors to get follow ups and get stronger allergy medications, I can''t recall how many I have seen. Write-up: Within 15 minutes of the shot, I became extremely weak in my muscles. In the middle of the night I had a fever of 103 and couldn''t stop shaking, it was so painful. That was the weirdest thing I ever experienced in my life. I had three blankets and a robe on and couldn''t stop trembling. There was pain all over my body, I had to wear a dress to the hospital because I couldn''t put on pants because of the pain. Write-up: pt stated about 4 hours after taking the vax she had a terrible headache. pt laid down but when she got up she had very sharp pains shooting from her toes up to her legs. pt said she had a stabbing pain in her left mid thigh. This episode lasted about 15 minutes. the next day she went to a routine exam but her PCP did not seem to think there was anything to worry about. pt had to have blood test 10 days later. She stated she was so tired she could barely get to the lab to get the test. The following weekend on 5/17/2021 she started having SOB and a coughing spell. On 5/19/21 she called to make an appt w/ PCP on 5/21/21. She saw a different physician and she was ordered a D-Dimer test. On 5/22/21 the pt got a phone call stating she needed to get to an Emergency Room since the lab test proved she had a blood clot. She went to ER. They did a US Doppler and CT of lungs. She had 2 pulmonary thrombosis. One in left lung and one in right lung. She had one DVT in left calf. Pt was admitted. She was given heparin IV for 3 days. She was then put on Xarelto and discharged on 5/25/2021. she has to take Xarelto for 90 days and scheduled to have another US on 7/19/2021 to see if the clots are dissolved.

Write-up: I received the Janssen vaccine on 4/8/21 at 12:00 pm. By 5:00 pm that day I began having flu-like symptoms (fatigue, body aches, nausea). These symptoms carried through to the next day (4/9/21). When I woke up at 6:00 am on 4/10/21 I had a rash on the backs of my upper arms and the front of my thighs. As the day went on, the rash progressed to cover my entire body. I went to the ER around 2 pm where I received a steroid injection and was prescribed oral steroids. I was fatigued for the next two days, but was able to return to work on 4/12/21. The rash began subsiding, but overall lasted 2-3 weeks. I completed my seven day prescription of oral steroids on 4/17/21. On 4/18/21 I was very fatigued. On 4/19/21 and 4/20/21 I was extremely fatigued and unable to get out of bed, sit up, or stand up. On 4/23/21 I woke up at 7:00 am with a swollen face, throat, tongue and lips. I returned to the ER where they prescribed me another round of steroids, but a lower dose and a longer duration. I experienced fatigue for 6 weeks after receiving the vaccine.

Write-up: About a week after the vaccine I had welts all over my body. They were like the welts that I had when I had Covid-19 in July of 2020. The welts came and went for a while and they were mainly on my left side. I woke up with them and then they would come and go during the day. They were hard raised welts and it lasted for three weeks. I also seemed to have allergic reactions to some foods. I went to the ER on 04/29/2021 three weeks after the vaccine. The welt on my wrist was very red and swollen and I was concerned. I was prescribed an antihistamine. It helped with the rash and other symptoms.

Write-up: After like two weeks, before going to bed, I started having SOB and then a full-on chaotic panic attack. Chest pain, stomach was visually twisted and hurt, felt like I had a knot in my throat and full on anxiety. It took about a week for it to start going away after one Urgent Care visit, two ER visits, and a PC visit who prescribed me with clonazepan and Prozac. When I took both of them two weeks later, my symptoms and pain finally got better, but I am still having SOB, a little fatigue, my joints hurt once in a while, digestive problems, weird chest feeling. Write-up: I was taking a shower the night of 04/28/21 and I had a bad pain in my right leg. It felt like it could be a blood clot. The area was swollen and I was in a lot of pain. My blood pressure started getting high and it kept going up. On the way to the hospital, I started getting a headache and shortness of breath. I tried to elevate my leg on the way there. I took an aspirin. I had to sit in the waiting room for 2 1/2 hours and my pain in my leg and headache continued. Once I was taken back, they ran a few tests. I was told that my D-Dimer was elevated which indicates a blood clot. I was checked for a pulmonary embolism and that was negative. I was given a blood thinner because an ultrasound technician was not on duty. I went home at 4 am. I went back for an ultrasound the next day for my entire leg. Everything came back normal. The pain slowly went away.

Write-up: 4/10 Vaccination. Within 4 hours I had body aches, fatigue, chills. 4/11 Migraine upon waking up. I took Excedrin. I had been out of my migraine medications. Rested all day. Pattern continued. 4/13 My BP had increased (200/140), racing heart, not well. I went to urgent care. They gave me migraine cocktail. I was having all over global pain and concentrated pain. Lidocaine Q-tips in nostrils as a nerve block to help with pain. The rest of the night I was ok but the next day the migraine was back. The medication was for onset of migraine but it was already there and didn''t work. 4/13-4/18 migraine persisted 4/18 contacted PCM. They told me they needed to see me. I was having high BP due to the increased pain per dr. They did the same migraine treatment that day. The BP medication stayed same as they felt it was a migraine issue. Write-up: The day I got the vaccine I had been having health problems. I had been having withdrawal from one of my medications and I had lost weight. Right away after the shot I passed out and when I came to, I threw up a couple of times. I was taken to the ER and I had low potassium levels were low, but all other tests were normal. I had some aches pains and feverishness for a couple of days. Five days after the vaccine I had bad diarrhea several times a day. I was nauseous and feeling bad. I had an appointment with my GI doctor a week later but I had to go to the ER before that. They did not find anything in most tests. The next day I tested positive for CDIFF. I was prescribed antibiotics. I went back to the ER a third time because the Vancomycin prescription had caused severe nausea. I am still feeling bad after six weeks but it has gotten better.

Write-up: 3/15/21: Within a few hours of the vaccine, I felt fatigued, followed by chills, severe body aches, fever, and dehydration (in that order). Improved by Day 5. 4/5/21: Lump on left thigh appeared. Raised up, hard, red, and warm. Went to Clinic ER immediately. Ultrasound of area and blood work showed no obvious abnormalities. Symptoms were diagnosed as "insect bite". Lump became painful several hours later. Antibiotics, Benadryl, and Corticortisone prescribed for 10 days. Took medication as instructed. Improvement by Day 7. Lump shrunk but did not completely resolve. Swelling and pain gone. Completed medication on 4/15/21. 4/22/21: Lump returned on left thigh, quickly turning into a large, solid mass. Red, warm, more swollen than the first time, and more tender to the touch and painful. 4/23/21: Went to Walk-In Clinic , Nurse Practitioner ordered MRI and more blood work. 4/25/21: Lump appeared on right thigh, same characteristics as the left thigh, also quickly turning into a large mass. Very painful. Hip size increased from 40 to 42 inches within a few days from the swelling. Unable to wear underwear due to how excruciatingly painful the area(s) were. Painful to sit, painful to stand up, painful to remain standing. Sharp shooting pains. Legs would feel as though they would give in if I stayed standing up too long. 4/28/21: Returned to Walk-In Clinic due to unbearable pain and nightly fevers. Antibiotics were prescribed on this visit, and a referral to a general surgeon and a Hematologist were made by the Nurse Practitioner who tended to me, . 5/3/21: Went to see Orthopedic Surgeon Dr. . He reviewed the MRI with me and showed me the areas where fluid was all over in the area affected (thigh). Referred me to Dr. , Rheumatologist. 5/4/21: Dr. examined both thighs and determined a biopsy under general anesthesia was needed in order to do deep tissue removal. He also ordered more blood work. 5/4/21: Initiated surgery preparations: Lab work, COVID testing, EKG, Chest X-Ray, Visit with General Practitioner to be cleared for the surgical procedure, Visit with surgery department. 5/6/21: Surgical biopsy under general anesthesia performed. 5/13/21: Visit with Dr. to remove surgical gauze, and examine the area. 5/14/21: Visit with Dr. for pathology / biopsy results. I was informed that inflammation levels in my body were at 45%, where they should normally be at 8%. Pathology concluded that an external cause was to blame for the inflammation, further stating that it was a foreign substance consistent with silicone injected in the area. It is noted that I have no silicone implants in any part of my lower body. Nor have I had any silicone injected in my thighs. 5/17/21: While using a pair of scissors and exercising normal pressure to cut a paper tag from new clothing, the area below my right thumb suddenly became swollen, purplish, and extremely tender. After two hours of no improvement, went to Health where they took an X-ray and determined it was a Sprain. I did not exercise any force to justify a sprain in that region. I was prescribed cortisone for 6 days. 5/20/21: Follow up visit with Dr. , Rheumatologist, for second opinion on pathology results. Dr. ordered blood work done in-office, as well as ordered additional blood work to be done at the lab. Results pending. After also examining all of my blood work done up until that point, she deduced that the issue with my hand was also likely caused by inflammation in my body, and whatever is causing the swelling in my thighs. The area of the hand turned black and blue the likes of severe trauma, yet there was no blunt force to the area. At the time of this reporting, I continue on blood thinner and anti-inflammatory medication for another 10 days to complete the 30 days post surgery. Additional blood work has been ordered to measure the inflammation levels and proceed accordingly. Currently I feel throbbing in my legs mostly at night when I lie down. Prior to the Johnson & Johnson COVID-19 Vaccine, I had no medical issues. My only visits were routine control visits annually. I do not drink or smoke; and exercise regularly (until this happened).

The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 1805025, expiry: unknown) frequency 1 total, dose was not reported, administered on 31-MAR-2021 to right arm for prophylactic vaccination. Concomitant medications included blood pressure pill, cholesterol pill and anti-depressant. One week after vaccination, on 07-APR-2021, the patient was feeling sick and dizzy. She said that the ambulance picked her up that day she felt really dizzy and took her to the hospital. There they performed tests on her and kept her overnight. Doctor of Medicine (MD) diagnosed her with vertigo and told her to take some over the counter medications for it. She reported that her runny nose and sick feeling went away after about a day. One to two weeks after that emergency room (ER) visit, she again went to the ER. This occurred at around 1-2:30 AM she went to the ER and MD said she had vertigo but she just stayed until the morning. MD prescribed her medicine for the vertigo, but the vertigo had not gone away. Write-up: UPPER LIP GROWING AND MOVING UP ON FACE; TERRIBLE REACTION; This spontaneous report received from a patient concerned a female of unspecified age. The patient''s weight, height, and medical history were not reported. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: Unknown) dose was not reported, frequency 1 total, administered on 04-MAY-2021 for prophylactic vaccination. On 06-MAY-2021, two days after vaccination the patient experienced top lip started to swell on the right side. Write-up: I had flu like symptoms for 24 hours afterwards. My arm swelled up from shoulder to elbow. I had a bright red rash that started to go away and then came back and got worse over two weeks. I have a half dollar size lump at the injection site that showed up 24 hours after the vaccine - hard like bone. Not hard like a muscle knot or something. Hot to touch - the whole rash area and itching. I went to Urgent care on the 21st -after it didn''t go away in two weeks. They prescribed Cephalexin to take 1000 mg twice a day for a week. The rash is mostly gone and heat is gone but lump is still there. And followed up on Sunday 29th. The doctor said that the hard lump might be permanent. Doctor said the rash part looked like Cellulitis. Rash and fever are gone but lump is still there.

Write-up: Eleven days after getting the shot, my mother in law was experiencing severe back pain and was not able to get comfortable or sleep at night. Before we could take her to see a doctor, she fell while trying to get out of bed at 7am the next morning. She had trouble getting up and exerted a lot of energy trying on her own. She called for my husband and I to help her get up. I grabbed her around the waist and my husband held her walker for her as support. Upon rising (mostly of her own ability) she was so exhausted that we had her sit on her walker and rest a moment. My husband went to check on her twice over the next 15-20 minutes, but when he returned again she was showing classic stroke signs and EMS was called. They arrived quickly and agreed. She was taken immediately to the ER, where they also agreed that it looked like a stroke. She was admitted as they continued to run tests, but she continued to decline more and more each day at the hospital losing motor skills and ability to speak. She was not able to move her right side at all and the left side was fidgety and had uncontrollably flailing that left her bruises on her arm and leg. She was also excessively sleepy. After loads of testings and images, the MRIs finally showed that the brain stem was extremely inflamed and it was an auto-immune response. By this time, she was already a vegetable in the ICU ward under incubation, IVs and a feeding tube. They continued to run more tests and brought in infectious diseases, but they ultimately said her body was fighting an infection that she did not have. They attempted a hemoglobin treatment with an oncologist to shut down her immune system but never consulted with her renal doctor, even through they were very concerned about renal failure. She could not complete the treatment and only received 4 out of the 5 doses they wanted to give her because of her kidney counts. They said that the auto-immune attack was very aggressive and they could not shut it down. In the early morning of 4/28/21, they finally contacted her kidney doctor and she was put on dialysis as a last effort to try to save her, but it was too late and all her organs were failing. She passed at 1:56 pm. Patient was homebound during the pandemic and did not leave the house unless she was visiting her doctor''s office. My husband and I have limited contact at our jobs, as we work in small offices, and our children (that also lived with her) were being schooled virtually. There is nothing that I know that could have caused her immune system to go into overdrive, except for the vaccine that was administered less than 2 weeks (which is considered the time frame for vaccine immunity) prior to her symptoms.

Write-up: A lump appeared shortly after the vaccination was given and remained for six weeks. After this time the lump started to decline and pain in the arm started. Write-up: Ran a fever for about 1 week. Had extreme pain in left leg beginning the day after vaccine. Visited emergency room 1 week later based on guidance from physician but little was done- they seemed unsure of what to do. They ran blood tests but since I had no swelling, they assumed no blood clots and did no more testing. Released after about 3 to 4 hours. Pain continued so contacted my doctor a 2nd time. She indicated other patients were reporting the same and suggested continuing aspirin or tylenol. After about 4 weeks pain resolved.

Write-up: Bilateral facial paralysis - symptoms started on the right on April 21, 2021. Patient was seen by PCP on April 22, 2021 and started on Valtrex and steroids for the treatment of Bell''s Palsy. On April 24, 2021 the left side developed paralysis as well. Patient was seen in the ED. Labs, MRI and CT of the head were performed. Lumbar puncture was attempted but unsuccessful. Patient was discharged home and had lumbar puncture as outpatient on April 29, 2021 as well as additional blood work. Tests showed no malignancy or infectious etiology. He was started on another round of steroids on May 7, 2021. Started physical therapy the following week. Still has minimal facial mobility at the time of this submission.

Write-up: On .4/19 I started to feel fatigue in my legs 04/20 it got worse in the legs 04/21 I came to work I worked through out the day with pain in my legs it hurt to walk it hurt to sit down the pain in my leg was getting worse 04/22 the pain got really bad I got up in the middle of the night to use the bathroom and I could feel my legs , my legs got really heavy and it felt like I had Charlie horses from my ankles up to the top of my thigh and I could not stand up. So I went to the ER and I got blood work done and I was told something about my CK levels were extremely high and that''s when I was admitted. The day that I got the vaccine I got really dizzy during that 15mins that they make us wait and I felt very sick While I was admitted my CK levels kept going up the doctors told me that I had something rhapdomysosis , the pain got worse and the doctor increased my level of fluids and they did and ultra sound on my legs to check for blood clots and they kept increasing my levels of fluids and once I was let go from the doctor I did have to keep going back to keep getting my blood check. I did continue to feel muscle weakness in my legs while I was home. I did go back to the doctor 05/14 for muscle weakness I was given as bag of fluid.. After that any time I stand too long or I am working out I can no longer work out for as long as I can. I can no longer work out for longer then 15mins because of the pain in my legs. I suffer at night time now with muscles spams now they happen all throughout the night. I had very low potassium

Write-up: I went to the ER because I had a very bad headache that was in the front of my face, behind my eyes, but it usually starts at by my neck and head, either one side or the other. I also had this squeezing under my ribcage. They did an EKG, which was normal and then told me to see a cardiologist. Write-up: 42-year-old female with history of bipolar disorder presents with allergic reaction. Patient states she received the Johnson and Johnson vaccine and subsequently developed itching and scratchiness in her throat on 4/11/21 and developed hives. Felt as though throat was closing. . Denied ever having any shortness of breath or difficulty breathing. Patient was given steroids epinephrine and Benadryl and transported to the hospital. Proceeded to ED and given Epi 0.5mg and Benadryl 50 mg PTA. Discharged and instructed to go to ED if ASAP if develop new/worsening symptoms and follow-up with PCP within next 24-48 hours. Patient denies any complaints at this time. States she never had a rash and denies any shortness of breath difficulty breathing. Denies any chest pain. Denies abdominal pain nausea vomiting or diarrhea and denies any urinary symptoms including urgency frequency or dysuria.

Write-up: I had the vaccine and about a month later I was diagnosed with Campylobacter which is a bacterial infection and they told me it was definitely not related to the vaccine. I took Azithromycin.

Write-up: PATIENT WAS ADMINISTERED THE JANSSEN COVID-19 VACCINE AT 11:03AM ON 5/27/21. LESS THAN 10 MINUTES LATER, PATIENT STARTED COMPLAINING OF SEVERE ABDOMINAL CRAMPS, LIGHT HEADDEDNESS, AND BLURRY VISION. SHE RUSHED TO THE RESTROOM WHEN THE CRAMPS OCCURED, WHERE SHE VOMITED AND EXPERIENCED DIARRHEA. WHEN PATIENT CAME OUT OF THE RESTROOM AROUND 15 MINUTES LATER, SHE WAS GIVEN WATER AND A SNACK BAR TO SNACK ON. SHE REPORTED FEELING BETTER, NO NAUSEA OR STOMACH CRAMPS, BUT STILL VERY WEAK. SHE WAS ESCORTED TO HER CAR VIA MOBILITY CART, WHERE HER DAD WAS ADVISED TO TAKE PATIENT TO URGENT CARE FOR FURTHER EVALUATION AND POSSIBLE FLUID THERAPY FOR DEHYDRATION.

Write-up: I experienced after 24 hours, I felt chills, fever, fatigued, horrible headaches, left sore arm and then my symptoms went away. On 04-22-2021, I experienced nasal congestion, pain in my nose, cheeks, ears, pain to the back of my head, lost my tense of smell and taste and had real bad cough. The doctors treated me as Covid, I did got tested twice for Covid but my results came back negative. The doctors treated me as a Covid symptoms. I also got tested for flu through my nose. I went to a military station to get my first covid test and my second was at an urgent care. After two weeks my symptoms of flu-like symptoms, headaches and fatigued went away. On 4-22-2021 I can say that the same symptoms that I experienced, my daughter and husband also get the same symptoms and it was significant amount of nasal drainage, cough and flu-like symptoms.

Write-up: At 6 pm I started having some goosebumps, at 9:00pm I felt cold, I was shaking a lot. At 3am the shaking started again. At 7:00am I had headache, pain on my eyes and a lot of body pain. Felt so tired and painful
