Current Illness: no
Preexisting Conditions: Hashimoto''s auto immune condition - thyroid - have had it for 15-20 years
Allergies: Allergic to doxycycline - just found this out in the last couple of weeks (Dr put me on it after the vaccine and I had a bad reaction to it - I redness all on my face and hoarseness with my voice; itching on my body and palms of hands.)
Diagnostic Lab Data: no
CDC Split Type: vsafe

Write-up: First 48 hours, fever and chills on and off. Starting after the first 24 hours, I had a headache on and off since. I have had fever ever since the shot and the highest it got has been 100.0 - on the 29th - 02:41P MST . On May 12, I went to see my Dr. and she prescribed an antibiotic for sinus infection- Amox-clav - I took that for 5 days as directed. She encouraged me to contact her after 5 days for follow up. May 17, I called her back and told her I didn''t feel any different and she changed me to Doxycycline and she had Methylprednisolone- and I started both of those on the evening of May 17. I continue those until Wednesday evening. But I noticed bright redness on my face in the morning, and itching and hoarseness in my voice and I called her Thursday morning and she advised me to stop taking both medications the doxycycline and the Methylprednisolone. She advised I go to Urgent care center - . I went on May 23, at 09:00 MST and I saw a physician assistant there -- and she examined me and I told her about my fever and headaches. She didn''t know what to make of it - said she couldn''t run a blood culture at the time because I had taken the steroids. She did still see signs of sinus infection. She gave me Cefdinir antibiotic and I''m still on that until this coming Sunday. I am still having fever and headaches - not as severe (as far as the headaches). My temperature is relatively low in the morning - starts out 98.3 and goes up to above 99 every day. It usually runs at 94.5 (my normal body temperature). That''s what it was when I came home and tested it right after the vaccine when I started documenting for v-safe. I still haven''t had a normal body temperature for me since April 28.

I responded that it was only on my right leg and I didn''t see any blood in urine or stool but that I had got my vaccine 4/11 she did call me around 10:15am and wanted to walk through some questions and asked about stomach pain and I had been having some pain and diherra and he also asked about vision changes and migrains and he said at that point because of the recent concerns with the vaccine he did want me to get checked out considering my history petechiae . He advised me to go to the er because of vaccine induced trombotic thromboctopenina (VITT) and he wanted to run some blood test and diagnostics. I arrived at the Medical center around 11am and I was seen by Dr and explained everything that was going and he knew that they were concerd about VITT and they ran a CBC, and Estimated Glomerular Villtration Rate, Lypaz, Liver profile, collected urine and did a culture with an analysis and a pregnancy''s test, Vitals were normal. the test came normal. All blood work came back normal, plateness was at 300, urine should a little bit of epidilla cells. They didn''t feel like they needed to be concern about the VITT. I was discharged and then told to follow up with my family doctor and have had some communication, they asked that I followed up with a dermatologist and too look at genetics. 4/28 in the evening my left leg closer to my knee also started to itch it also developed a rash looking like strains but not as bad as what was on my right leg. but since then I have not had any more pop ups but I do still itch.

Write-up: There were no symptoms until much later in the evening. Around 8:30 PM, I felt fatigued. A little later I had chills for about 30 minutes. Then I felt nauseous and went to the restroom down the hall (I was living in a dorm). I did not throw up, but I had a pounding headache which only got worse. I also felt very hot and the cold floor was welcoming. I laid on the floor in the bathroom but felt too awful to sit up. Someone found me in the bathroom around 10:00 PM and called campus safety, which came about 20-30 minutes later. She determined that she could not give me medication on the spot and that she had to take me to the hospital. We arrived at the ER around 10:45 PM. The nurses and doctor took my temperature and found I had a 103-degree fever. Because of my pounding headache, I couldn''t hear very well. The doctor gave me a shot of 60mg of Toradol at 10:56 PM. He then gave me instructions to alternate tylenol and ibuprofen every 3 hours, increase my fluids, and only do activity as can be tolerated. I left the hospital around 11:25 PM with a huge headache, but better than when I initially walked in.

CDC Split Type:

Write-up: pt states around 6 pm on 3/28/21 she developed a stomachache. This lasted until 4/2/2021. She developed a migraine headache on 3/29/21 which continued to get worse. She then started getting dizzy along with a feeling of pins and needles on her head and her feet became numb. She reached out to her Neurologist and she was prescribed chlorpromazine. After taking one dose it caused her heart to race. She was also prescribed duloxetine by her PCP. After one dose her blood pressure went up. BP was 163/110. On 4/11/21 she went to the ER. They did blood test (platelets were 154) and CT that was normal. She was given dexamethasone steroid which helped w/ the heart rate. She was released to go home. Her headache came back the same day. On 4/26/21 she woke shaking and her heart was pounding out of her chest. Her rate was 169. She went back to the Medical ER. Her heart rate was 180. They did EKG. She started shaking more, her feet were tingling and going numb, she had a vibration in her head and heat from her neck going up to her head. Her heart rate was going up to 200. She felt like she was going to die. She laid down and her heart rate would go up and down. They gave her an IV and sent her for an MRI. She was admitted. She was getting ready to have an ECHO she had a vibrating and burning sensation go through her body from her neck to head, back down to the left arm and then down to leg and feet. Her heart rate went up. This episode lasted about 15 minutes. After getting the MRI she had another shaking and vibrating episode that lasted about 10 minutes. She had an ECHO and then had another shaking vibrating episode that lasted about 5 minutes. She was given a beta blocker metoprolol. All test were negative or normal. She stayed in hospital to 2 to 3 days and released to FU w/ multiple doctors including psychiatrist, cardiologist and PCP. She was diagnosed w/ having general anxiety and panic attacks. She saw her PCP and she was taken off metoprolol to propranolol. Pt saw a Cardiologist on 5/14/21. They did an EKG and she was given a heart monitor to wear for 3 days resulting in tachycardia 166 episodes. Overall heart 80 bpm to 119 bpm. Total beat count 340,832. Pt still has dizziness and migraine headaches. Her heart is now enlarged and has pain on her left side under her breast and on top of her breast. She has these episodes daily and feels like she is still going to die. She said she never had anything like this before getting vaccinated. Her Cardiologist is sending her for more tests.

A couple of weeks later I woke up with my right eyelid all swollen and bruised and went to the ER where there they did a CT scan to make sure I didn''t have a blood clot and I did not. Also my platelets were in the normal range. It took a good week for the swelling and bruising to go down and the bruising even went down my face somewhat. I had an appointment with my eye doctor and he didn''t know why it happened, but may have had something to do with my taking low dose aspirin. I did not have any contusion to the eye and it was not hit by anything.

Diagnostic Lab Data: Following the evening of the 5/25, on 5/26 I scheduled an appointment with at 11:40am. They performed an ultrasound and found bilateral deep vein thrombosis to both the common femoral and femoral veins. Immediately following the diagnosis, I went to the ER where they performed an EKG which came back OK.
CDC Split Type:

Write-up: 11 days following the single shot I developed severe pain in my right calf muscle that felt like a charlie horse and on my leg from knee to my ankle felt a squeezing sensation like I was wearing a very tight sock that I couldn''t take off. After a few days the pain subsided and the similar pain began in my left leg however not as severe. The pain would seemingly move around and come and go. Some days it was so bad I couldn''t walk or balance. I believed I had a muscular issue. I then began having swelling on the top of my left foot and around the ankle of my left foot. After about a week, my right leg began to swell from the knee to ankle fairly significantly. New veins appeared on the front of my right leg and other veins on the back of my right leg began to bulge. Pain on both legs started to subside on the evening of 5/23, however swelling continued. On the evening of 5/25 I was experiencing severe squeezing sensation and bulging veins in my right leg.

CDC Split Type:

Write-up: My shot hurt very bad, and my IMMEDIATE reaction was ?wow she shot me really high on my shoulder?. Thought that was strange, but didn?t think anything of it. Pain increased daily, and 3 weeks after my vaccine (after taking advil/tylenol daily) I was in such excruciating level 10 pain that I was losing sleep at night. I then found out about SIRVA from a friend, and after doing some reading, I was sure that?s what it was. I went to urgent care, and they knew nothing about it. They gave me muscle relaxers that didn?t do anything. I woke up a few days later with such intense throbbing/sharp pain that I was unable to move my arm/shoulder above my elbow. I went to the emergency room with a friend because I couldn?t even drive, and they also didn?t know what SIRVA was. They gave me oral steroid medication & an orthopedic referral for it it got worse. The meds helped a little, brought my pain down to a 6/7. Once the meds were complete, my pain increased, so I went to the ortho, who KNEW WHAT I WAS TALKING ABOUT, and diagnosed me with SIRVA after doing an x-ray. He told me to let him know how it was feeling a week later, since the steroids did help a bit. Unfortunately, the pain only got worse, so I went back in for a cortisone shot. That helped more than anything, and my pain is now down to a 2/3. I have been trying out some PT, but am honestly reading some crazy stories about chronic pain online that are scaring me and am afraid I will never have my arm at 100% again. The reason I am bothering with this form, is because I would love to be considered for any compensation for the nearly 3 grand in medical bills sitting on my desk. Of course on top of it, my last day at my last job was 3/31 (few days before getting vaccinated) and I just got a new job this week. These past few months have been a really really stressful time period, and I?m super angry that it?s just because I got vaccinated. Sorry for the rambling, but I really hope they raise awareness of this and alert untrained health care workers of the possibility, so that no one has to go through this again.

Write-up: 3/26/21 After Pfizer vaccine noticing hives on chest and left arm as well as facial tingling and slightly left eye itchiness. Received 2 dose of Epinephrine during ED visit. 5/13/21 After Janssen vaccine he received at this morning around 8:10 AM. After that he had almost the exact same symptoms with a shooting pain in the left arm and neck some left hand tingling and swelling to the point where he can''t get his wedding ring off and a sensation of pressure/tingling/fullness in the left side. Given Benadryl and steroids, initially there is no airway concerns or compromise. However after a period of time, he thought his voice was different. He did sound just slightly raspy, though I did question whether this could be related to dryness from the Benadryl. Regardless given that he did note a change, we opted to go ahead and give a dose of epinephrine, which did resolve his symptoms

She said, I think I threw up because my head hurt. I asked her where it hurt and she said, in the back. I took her to the ED and she had a brain bleed and 5 blood clots in the major veins and sinuses of the left hemisphere of her brain. Her medical team said this is a very rare part of the brain to have blood clots. I would like further investigation into whether this was related to the vaccine.

Diagnostic Lab Data: THE FOLLOWING IS GENERAL PER DISCUSSION WITH PATIENT: 5/24/21 - PRIMARY CARE DOCTOR - PLATELETS NORMAL, RBC''S AND HEMATOCRIT SLIGHTLY ELEVATED
CDC Split Type:

Write-up: THE PATIENT CAME TO THE PHARMACY TO BE VACCINATED FOR COVID-19, WITH JOHNSON & JOHNSON, ON FRIDAY, MAY 21, 2021 AT ROUGHLY 2PM. I GAVE HER THE VACCINATION IN HER DELTOID MUSCLE ON HER LEFT ARM. SHE ALMOST IMMEDIATELY COMPLAINED OF DISCOMFORT AND ASKED IF I HAD A COLD PACK. I PROVIDED HER WITH ONE, WHICH SHE USED WHILE WAITING THE 15 MINUTE POST-VACCINATION OBSERVATION TIME. SHE LEFT THE PHARMACY WITHOUT INCIDENT. ONLY A COUPLE HOURS LATER, SHE CALLED THE PHARMACY TO REPORT ERYTHEMA AT AND AROUND THE INJECTION SITE. I DID NOT KNOW AT THAT TIME WHETHER THE ERYTHEMA WAS DUE TO THE COLD PACK OR IF IT WAS A VACCINE-INDUCED RASH, SO I INSTRUCTED HER TO TAKE A DOSE OF ANTIHISTAMINE. AS SHE HAD CETIRIZINE IN THE HOUSE, SHE TOOK THAT. SHE WAS HOME WITH HER HUSBAND. I RECOMMENDED HE KEEP AN EYE ON HER AND IF SHE DEVELOPED ANY ADDITIONAL SYMPTOMS OR WORSENING RASH, SHE SHOULD GO TO THE HOSPITAL FOR EVALUATION. ADDITIONAL SYMPTOMS INCLUDED SWELLING, TROUBLE BREATHING, WHEEZING, CHEST OR THROAT TIGHTNESS, OR HIVES. THOUGH SHE DID NOT EXPERIENCE ADDITIONAL SYMPTOMS, SHE DECIDED TO GO TO UCONN HEALTH CENTER ER IN FARMINGTON. THEY DID BELIEVE THE RASH WAS VACCINE-INDUCED BUT DID NOT FEEL SHE REQUIRED ANY ADDITIONAL TREATMENT AND DISCHARGED HER (I.E., NO EPINEPHRINE). THE PATIENT NOTED THAT HER BLOOD PRESSURE AT THE ER WAS HIGHER THAN "HER NORMAL" AT 152/95. THE FOLLOWING TWO DAYS, THE RASH STAYED THE SAME AND WAS DESCRIBED BY THE PATIENT AS THE "WORST SUNBURN SHE''S EVER HAD". SHE ALSO HAD NORMAL SIDE EFFECTS OF BODY ACHES AND CHILLS/SWEATS. ON MONDAY, 5/24/21, SHE FOLLOWED UP WITH HER PCP. HE AGREED WITH THE HOSPITAL THAT THE RASH WAS VACCINE-INDUCED AND TOLD HER THAT HE DIDN''T WANT HER GETTING ANY COVID-19 VACCINES IN THE FUTURE. HE DID BLOODWORK TO RULE OUT PLATELET ISSUES, WHICH, PER PATIENT, WERE NORMAL; SHE NOTED THAT HER RBC''S AND HEMATOCRIT WERE SLIGHTLY ELEVATED. HER PCP GAVE HER A MEDROL DOSEPAK FOR THE RASH. THE PATIENT IS RECOVERING. I BELIEVE IT IS WORTH NOTING, HOWEVER, THAT THE PATIENT APPLIED THE COLD PACK DIRECTLY TO HER ARM FOR AN EXTENDED PERIOD OF TIME AND DID NOT PROVIDE THIS INFORMATION TO EITHER THE ER OR HER PCP. SHE SENT ME A PHOTO OF HER ARM WHICH SHOWS GENERALIZED ERYTHEMA (THE SIZE OF THE COLD PACK) - THE ONLY PART THAT IS COMPLETELY NORMAL IS THE SKIN BELOW THE BAND-AID. I BELIEVE THE PATIENT CREATED AN ICE BURN BY USING A COLD PACK TOO DIRECTLY, FOR TOO LONG, AND THAT THE UNDER BAND-AID AREA WAS NOT RED BECAUSE IT PROTECTED THE SKIN FROM COLD BURN. I SUGGESTED TO THE PATIENT THAT SHE RUN THIS THEORY BY HER PCP, AS I BELIEVE IT IS IMPORTANT "MISSING INFORMATION", THAT COULD POTENTIALLY CHANGE THE DIAGNOSIS AND ALTER HIS RECOMMENDATION TO AVOID FUTURE COVID VACCINATION. WHETHER OR NOT THE PATIENT CONTACTED HIM AGAIN IS UNKNOWN.

Write-up: On May 4th, I fainted shortly after waking up. Light headed and unable to sit upright, stand, or walk. Had a fever. Called out of work. Headaches, chills, nausea, pain at injection sight, bedridden for most of the day. Around 3/4 in the afternoon I called the doctor''s office (phone kept ringing) so I called the nurse through the insurance company. Nurse advised me to stay home for now but to seek medical care if symptoms persisted for more than 3 days. Symptoms continued but were less severe on Wednesday May 5th causing me to miss another day of school. On Friday May 7th at about 12:50 AM I awoke to severe abdominal pain-the kind the pharmacist had warned me of when getting my vaccine. I could not stand or move. I never take aspirin and yelled for a family member to give me Tylenol. I called the nurse from my insurance company and she asked me a series of questions and advised me to seek immediate medical attention. My family called 911 and an ambulance was sent to the house. They checked my vitals and suggested a family member take me to the ER. My family took me to the Emergency Center. I was wheeled into the ER. ER did test but put "Abdominal Pain, Unknown Cause, (Female) on my paperwork. I missed another day of work due to my ER visit. Symptoms lessened but persisted for several days.

Write-up: My shoulder was sore for the first day or two after the vaccine, which seemed normal. But instead of getting better, it got worse. It wasn''t exactly at the site of the injection, but higher and closer to my neck. It became painful to lift my shoulder to my desk to type. On April 15, my shoulder was hurting a lot and my fingers became numb during the day. It was significant enough that I went to an urgent care facility . The NP I saw indicated the thought that the inflammation in my shoulder was compressing on the nerves, leading to the numbness I felt and by then had spread up most of my arm. I also indicated that moving my arm was painful. She instructed me to take ibuprofen, 600 mg, 2x per day and see if that helped. I did that for about 2 weeks, but that much ibuprofen in my system actually caused me issues and I had to stop taking it. My shoulder becomes tired and sore, making it nearly impossible to function on a daily basis. It causes immense pain and OTC pain medications do not provide relief. I have used heat and cold, which provide temporary relief at night, but by about 1 or 2 p.m. each day I am in immense pain. Using the shoulder to type at a desk or sitting in my car with the arm pushed forward accelerate the pain each day.

Write-up: 1. 24 hours of chills, body aches, fatigue, and low grade fever on the day after my shot. 2. On March 20, 2021 I woke up with swollen lips and face, and hives on my arms, legs and trunk. I also had a cough, watery eyes and itchy throat, like typical allergy symptoms. I went to the emergency room and was treated for an allergic reaction. No one, including myself, considered the COVID shot as the reason. The hives were gone within 24 hours, but the other allergy symptoms lasted for one week exactly. They had not gotten any better with a variety of over the counter allergy medications. When I went to my primary doctor for a follow up visit on March 29th all of the symptoms were gone. They were the ones that recommended I sign up with the vaers system, and they suggested the possible/likely connection to the vaccine.

Write-up: I had a severe cold, with a harsh cough for over a week, extreme shortness of breath, and exhaustion. Slight fevers. I tried mucinex, but had to go to the hospital for an inability to void, with resulted in a catheter. We couldn''t figure out what was wrong, so I stopped the mucinex. Then, I took doxycycline, and that seemed to help significantly. After four or five days on the doxycycline, the symptoms were almost gone.

Write-up: I woke up at three AM feeling like I had a fever. I had body aches. The next morning when I woke up I felt like I had a tourniquet on my arms and legs. I was also short of breath. I went to the ER and they checked for blood clots. I started to get lower extremity cramps and muscle spasms. Two days after I started to have some shortness of breath and I was out of breath if I walked any length. One week after the vaccine I ended up wit an ear infection in my right ear. The muscle spasms continued especially in my right lower legs with twitching which lasted for five weeks. Two weeks after the shot my Asthma, which is normally controlled got very severe. My inhalers didn''t seem to work. Three weeks after the vaccine my Asthma got worse and I was having trouble breathing. I went to the ER again and there were more tests done. I was prescribed Prednisone, and I took one week off work. After multiple treatments I was prescribed antibiotics and switched my inhaler which helped. I have been trying to do some Cardio activity this weeks and I feel better after six weeks. I don''t have to use my Nebulizer as much as I had been.
