Write-up: Onset of shortness of breath and cough on 1/3 that progressively got worse. Clinical diagnosis of pneumonia without fever was made, patient started azithromycin on 1/5 and albuterol treatments every 4-6 hrs. Initially he improved, but then worsened. chest xray on 1/6 was negative for pneumonia, PCR covid test was negative, albuterol treatment did not bring much relief. He started respiratory distress on 1/10 and was taken by car to the local ER where another covid test was negative and chest CT revealed multiple bilateral pulmonary emboli. The leg US revealed blood clots in both of his legs. He had an emergency catheter-delivered thrombolysis and was discharged home from the ICU on 1/12 on oral anticoagulants. He is gradually improving, but very weak. He tires easily and gets a drop in oxygen to 90- 93%, as well as an increase in the heart rate to 120 when walking less than half a mile. He runs out of breath with exertion.

Write-up: Unusual pattern of bruising at injection site - bruising with 3 reddish/purplish small round circles underneath the skin. Circles are all approximately the same size, and all are on the same horizontal "line". Bruising started appearing evening of the injection. Left arm was very sore, especially the evening of the injection. Bruising and circles still visible two days later, but pain is negligible.

Write-up: Patient had just recovered from COVID and ended quarantine on 1/1/21. On 1/4/21, she received the first COVID vaccination. While driving home on 1/6/21, she experienced an MI diagnosed in the emergency room 30 minutes later. She was admitted to the hospital and underwent a heart catheterization and received a STENT. She had previous cardiogenic shock in 2003.

Write-up: "Moderna COVID-19 Vaccine EUA" It has been reported to me that pt. had gone into hospital for a heart catheterization on 1/12/2021. It was found during this procedure that pt. had suffered a MI. She was release to home the following day and passed away at her residence on 1/15/2021.

Diagnostic Lab Data: Since 1/14/21, I have been tested for PCR panel, covid negative x 4. Hepatitis B and C and HIV negative. Many tests have been run. CT brain negative. I am still in the hospital today. Unsure of discharge date yet.

Write-up: On January 14, 2021, I noticed generalized petechiae all over my body. I went to seek medical care and was found to have platelet count of 2. I was hospitalized for idiopathic thrombocytopenic purapura. I was given platelets which increased my platelets to 4. Next day, given IVIG dose. Also receiving 4 doses of decadron. Day after IVIG, platelets to 20. I am still in the hospital getting treatment today.

She has a history of stable ITP with her last required infusion of IVIG 12 years prior during pregnancy and monitors her CBC every 6 months. Her baseline platelet count is ~50-60k. She received treatment with dexamethasone 40 mg IVPB x 3 doses and IVIG 95 grams (1 gram/kg) IVPB x 2 doses.

Write-up: Evening after receiving vaccine, felt "fuzzy, full feeling in tongue and throat". No breathing issue. Fever (101 degree) and rash (all over body, red, flat petechiae) developed 24 hours after vaccine with diaphoresis and general skin flushing, body aches and, severe local arm reaction. All symptoms resolved after 48 hrs.

Other Medications: None at the time. I had gotten routine bloodwork done which showed iron deficiency anemia and vitamin D deficiency. I started taking supplements Friday evening. The rash worsened the next day and spread over a large majority of my trunk are
Current Illness: I had a left-sided headache that developed about two weeks prior. I went to the ER on 12/17 where I was given Benadryl, Toradol, and Reglan - the "migraine cocktail". Otherwise no issues. The routine bloodwork I had done showed high cholesterol (I have a history of this), slightly elevated platelets at 509,000 and my provider said my ANA titer was slightly elevated but I have no family history of autoimmune disorders and no other symptoms of any autoimmune disorder. I also read low vitamin D levels can alter ANA results and mine are pretty low at 20.
Preexisting Conditions: History of iron-deficiency anemia and high cholesterol, HSV-2 diagnosed January 2013 after hospitalized with viral meningitis during the primary infection.
Allergies: Amoxicillin: hives, Acyclovir: hives, Valacyclovir: mouth tingling, tongue swelling, Famciclovir: hives
Diagnostic Lab Data: Skin biopsy was done showing ?spongiotic vesicular dermatitis with eosinophils. These changes are fundamentally those of dermal hypersensitivity eruption, such as which can be seen secondary to a drug.? Microscopic description ?there is a perivascular mixed cell infiltrate including eosinophils. There is epidermal spongiosis with spongiotic vesiculation.?

Write-up: Developed severe itching starting on my forearms overnight on Thursday going into Friday that week after I received the vaccine. The itching progressed to my stomach/trunk and back. The more I itched, the itching worsened and pink bumps would immediately appear. The left side by my rib and over my left breast were the worst impacted. The bumps later appeared to join. I went to the ER that Saturday 1/9/21 after two doses of Benadryl didn?t help. I was prescribed oral antibiotics starting at 60mg a day for two days, then tapered down 10mg every 10 days. I?m currently on day 1 of 20mg. Also took Benadryl as the itching was keeping me from sleeping. I was also prescribed clobetasol cream to administer twice a day. Symptoms have markedly improved. The rash is now the same feeling as my skin, not bumpy. Hyperpigmentation remains. I?m being followed by my dermatologist whom I saw Monday 1/11/21. She performed a punch skin biopsy.
