Write-up: Itchy all over face and body around twenty minutes after injection. Went home and took benadryl which relieved itching for six hours then symptom returned and will take more Benadryl and see how it goes from there.

Write-up: He felt nauseous, got up to go to kitchen sink and passed out for a few seconds. When I got to him, he was on the ground, eyes open, but seriously disoriented. He got up to the couch. He was very, very clammy and his heart rate was 44 bpm. I held three fingers for him to identify, he correctly answered. He has a small gash on his forehead. His heart rate came up to 70, 20 minutes after episode. Refused to go to urgent care.

Write-up: Complaint of dizziness immediately after vaccination. Waited a few minutes at the vaccination station and still complained of dizziness. Vital signs obtained x 2. She was given something to drink and rest. VSS. Escorted to waiting area by Nurse and patient''s husband. Monitored for 30 minutes. Client stated she felt better after drinking. Client able to walk unassisted and after 30 minute observation she was discharged to home without further incident

Write-up: After the vaccination, I experienced pain and tingling/numbness in upper arm radiating to under the arm. Throughout the day and into the night I had pain, redness, itching and swelling at injection site. The next day (21st) a hard lump developed under the skin where I had the injection. I also had a headache and muscle aches. I didn''t take any medication for the pain and discomfort as I was told it was beneficial to let the reactions occur without drug interaction or interference.

Write-up: Five days after receiving the Pfizer vaccine, I had a sudden onset of neck & shoulder pain on my left side, radiating up into my left ear & across my chest. My chest was tight. It hurt in my neck and shoulder and chest to take a deep breath. Shortly after, I felt jittery inside & started getting chills. The backs of my legs were quaking against the chair and I got really cold. My blood pressure went up to 149/78. I also had one bout of diarrhea.

Write-up: Sore arm, headache, blood pressure on meds normally 110 over 65, a few hours after vaccine blood pressure 145 over 64, fatigue, all subsided after a few days, 1 week later hard lump at injection site about size of silver dollar that is red and itches.

Write-up: A 31 yr/o female who presents with palpitations and left upper back pain. Patient reports she has been told she has a high resting heart rate on prior healthcare encounters, especially when she has been seen here in our facility for asthma exacerbations. She states because of this she has worn a Holter monitor with no evidence of arrhythmia. Over the past year or so she has had a few episodes of heart palpitations described as a flip-flopping or fluttering or irregularity of her heartbeat. She had a few hours worth of palpitations after receiving her first Covid vaccination a few weeks ago. She received second Covid vaccination yesterday and has had off-and-on palpitations both last night and throughout the day today. Beginning this morning she developed a sharp constant pleuritic discomfort in the left upper back near the shoulder blade, which is worse with deep breath. No fever, cough or dyspnea. No anterior chest pain whatsoever. No leg swelling. No history of DVT or PE.

Write-up: Fever of 100.2, headache, all my chronic pains such as back and neck pain was intensified. The fever, headache and back pain started 12 hours after receiving the vaccine and got worse 24 hours after getting the vaccine. My fever and pain was not getting reduced with Tylenol and Aleve. I had to use an ice pack on my head and back. 29 hours after the vaccine the fever went away but the pain was still there. I took Tylenol and CBD edibles and went to bed. I slept fine. Write-up: One week after vaccination my arm started to itch. When I looked at it it was pink, about an inch around. Also at the same time the lymph node under my arm, on the same side as the vaccine site, started hurting. Over the next 24 hours the spot on my arm grew to about four inches around and got brighter red. It alternated between itching and burning, much of the time feeling like a sunburn. One day after it started I did an online doctor visit. She said it was most likely "Covid arm" and after reading about it online I agree. Now almost 48 hours after symptoms started there is improvement, it has gone back to a dark pink. Lymph node still hurts but it has also improved.

Patient states on April 8th she went to her dentist for cleaning and do not expiring any gum bleeding at this time. Patient states 2 day ago (April 19th) her gums were bleeding so bad that she could not see her teeth and patient states she had a nose bleed as well. Patient also states she was experiencing headaches. Patient presented the the Emergency Department on April 21st . Patient was found to have thrombocytopenia (PLT = 2). Patient was admitted to Hospital on April 21st (as of April 23rd, patient is still admitted). Hematology was consulted for further management of thrombocytopenia. Patient received 2 platelet transfusions, IVIG x 2 doses, and is receiving corticosteroids.

Write-up: I''ve developed a tic on my left eye that makes my eye keep closing about twenty times in a minute. Write-up: I had a severe headache starting in the evening. The pain was constant, dull, and felt like it was coming from the center of the head. I don''t have a history of headaches, so this was unusual. After going to bed at 11PM, I woke up around 2AM from the intense pain. I could not sleep for a couple of hours in the night and I took Nyquil, which helped a little bit. I considered going to the emergency room, but decided to wait until morning to make the decision. I was able to go back to sleep and then slept until 11AM. I woke up feeling weak and groggy, although it may have been due to the Nyquil. The headache was improved and gradually diminished throughout the day. The next morning, it was completely gone and I felt back to normal.

Write-up: I now have mini little eczema patches on my face - two or three of them; I use Fluticasone (steroid) on them which I was using for the original adverse event with red patches on/below my neck. I had a health report today primarily related to a different health issue but I asked her if I should use the Fluticasone cream on them. She cautioned me to use the steroid cream minimally on my face. The eczema I have on my hand - that is what I always there in dry, bad weather - seems worse than it normally gets. The rash on my neck is gone. Also, two weeks after the Vaccine, I have noticed that my eye sight is different. I am going to the eye doctor tomorrow because of this.
