Write-up: 3-19 vaccination 3-20 I started to have hives, low grade fever. 3/21 I went to church. I left between services due to being dizzy, fever chills, achy, body aches. 3/22 didn''t go to work. But by midday felt better. *Every week for a month, the hives would disappear and reappear in another place. *6th week I went to urgent care as the hives were around stomach. Prednisone. Rash/hives went away. 5/11/2021 I went to work first part of the day. I left work at 11:00am. The cough started. 5/12 my results showed second strain positive for COVID. 5/18 Cough persisted and boss advised to go seek medical attention as been sick for a while. I went to urgent care. He put me on an antibiotic and pill that makes me stop coughing . The dr. didn''t seem to think that I had COVID as I had been vaccinated. I had a low grade fever. He did the COVID test. I was able to access the records online the following day. *Off from work, could go back after my fever had left. My fever left on 5/22 *I missed approximately 6.5 days of work. I missed another day after vaccination, as well.

Write-up: Pain in right shoulder blade that radiated to chest and tingling in finger. Went to ER around 4/17/21- d diner was done and was not elevated. Followed up with doctor a couple days later. With same symptoms and more back pain. EKG Showed a slight inversion. Blood work including Troponin, liver, and platelet count all came back normal. Tried to work out but felt like I injured myself after only 10 sit-ups. This pain still exists. My anxiety grew like nothing I have ever experienced. Pain in my ankles, both feet began about a week later. Stress test done on May 4 with normal results. Followed up with doctor on May 5 for ankle pain and hip pain. Blood work tested Inflammation, Autoimmune, Rheumatoid, Thyroid all came back normal. Another d dimer came back elevated at. 65. Was prescribed buspirone for anxiety. Had a CT Scan and Venus Doppler- all came back clear. I still have pain in rib area where I feel like a pulled something. I am afraid to exercise, I still have pain in back and side. Will follow up with one more d dimer then hematologist if still elevated.

Diagnostic Lab Data: EMT responded to 911 call. Checked patient BP and said it was normal. Said it seemed like an anxiety attack, patient was still not breathing steadily. Patient was taken to free standing ER

Write-up: At 15 minutes past vaccination patient is crying. When asked if she is ok, she doesn''t speak just keeps crying. Then her eyes roll back in her head and she proceeds to pass out. She comes in and out repeating this process for 5-10 minutes. Patient is non verbal during event. Once Patient came to, she knew her name, year, and president but did not know what day it was.

After about 7 mins pt came to counter saying she had a pain in her neck and her chest was tight. Write-up: Received shot at 1:10pm and was asked to wait 15 minutes before leaving. At 1:20pm I became very lightheaded and anxious. I stood up to look for a drinking fountain then woke up on the floor surrounded by people. Blood pressure was 80/40. Spent next two hours in ER on an IV. I was released once BP came back up. After getting home, I developed chest pain on the left side which I still have after three days so far.

Write-up: I am still experiencing pain at the injection site from when I received the vaccine. I have been experiencing heart palpitations that have been waking me up in the morning as well. Write-up: With in 10 minutes of the injection my thoart closed, my lips swelled and I was having trouble breathing. Benaydyl was given and an Epipen was used, I was taken by ambulance to the local hospital ER where an IV was started, steroid was injected. I was kept several hours for observation and sent home. The following day I had attacks of itching and rashes as well as occasional numbness in my lips, by Thursday evening it was worse, returned to a different hospitals ER at 2am Friday morning, more steroid was given and I was observed, and sent home with an prescription for an epipen as well as steroids. Continued to take Benadryl often taking two with in a half hour, Monday May 24 went to my own docotor, as I had a new reaction of pressure in both ears with pressure in my cheeks and around the back of my head. She give me a prescription for additional steroids and is still monitoring me as I am still reacting.

Write-up: AFTER PATIENT RECEIVED THE VACCINATION, HE SAT DOWN IN THE WAITING AREA. AFTER APPROXIMATELY 1 MINUTE THERE WAS A NOISE AND I NOTICED THAT HE WAS SLUMPED TO THE RIGHT SIDE AND STARTED TO CONVULSE. I RAN OUT TO THE PATIENT AND ENSURED THAT HE DID NOT FALL AND HELD HIM IN PLACE UNTIL HE STARTED TO COME TO. AFTER ABOUT 30 SECONDS THE PATIENT STARTED TO BECOME AWARE AND COULD HEAR ME TALKING BUT WAS STATING HE COULD NOT SEE. AFTER ABOUT 30 SECONDS HIS SIGHT RETURNED. DURING THE TIME THAT I WAS FIRST WITH THE PATIENT, THE STORE MANAGER CALLED 911 AND THE EMS WAS ON SITE AFTER APPROXIMATELY 5 MINUTES. I HAD THE CUSTOMER SIT DOWN AND GAVE HIM WATER UNTIL THE PARAMEDIC ARRIVED. THEY TOOK A HISTORY OF THE PATIENT AND AFTER A FEW MINUTES TOOK HIM BY AMBULANCE TO THE LOCAL HOSPITAL FOR ADDITIONAL OBSERVATION.

Preexisting Conditions: I have a condition called POTS. Postural orthostatic tachycardia syndrome.
Allergies: I am allergic to keflex, anything in the keflex family and penicillin. I am also allergic to peanuts, tree nuts, fish and shellfish.

Write-up: The night/next morning after receiving the vaccine I was awoken by a racing heart rate that I measured at a resting heart rate of 200 bpm. After 5 minutes of trying to assess what was happening, I recognized that my heart rate was not slowing down. I woke my wife because I was worried and had difficulty breathing. It continued to be in the 180-190''s, I called an ambulance. On the way to the hospital my resting heart rate was 180. After I got to the hospital they ran blood work, a CT scan and a chest x-ray. I was in the hospital until 10 am. My heart rate steadily fell to about 110 beats per minute after 9 hours in the hospital. They released me because my tests had come back normal. Upon being released, I came home, had lots of fluids and rest. After 1 week, my heart rate went back up to 160''s. My chest started to feel tight again and I had difficulty breathing again. I called my PCP emergency number and they advised me to go back to the ER at 10 am on 05/15/2021. They did another EKG, a D-Dimer and I received saline. My heart rate dropped to 120 and they released me. I was given an all clear from all tests performed. Sunday-Wed that week, I had a slightly elevated resting heart rate, have had difficulty getting a full breath, general stomach pains and I had felt fatigued. Thurs-Sat, my resting heart rate returned to normal (60 bpm) minimal difficulty breathing, but continued pressure in my abdomen and chest. Saturday I fell asleep because I was having a migraine. I woke up at 10 pm and the cycle started again. My heart rate went back up into the 160''s. I called the emergency line for my PCP and was told to go back to the ER (05/22/2021). They performed a chest x-ray, an EKG and some blood work. My resting heart rate went to 90 bpm around 3 am and was sent back home. The rest of Sunday, I had difficulty getting a full breath and my resting heart was higher than usual. I had an echocardiogram on Wednesday and it came back normal. I wore a heart monitor for 24 hours, but I have not received the results yet from my doctor. I have never experienced any sort of cardiac episode in my life.

Write-up: I was in the grocery store and I became dizzy. When I got to my car, I was throwing up and I could barely walk. Everything was spinning. I went to the ER and had a MRI. Then I was diagnosed with Vertigo Syndrome. I was sent home. I went to see my doctor a couple of days after that. The doctor told me I will gradually get better. A few weeks later, I went to a physical therapist. I was given some exercises to do to speed my recovery. I am still not completely over it. I still get dizzy and I get very tired.

Diagnostic Lab Data: All facilities tested for covid & flu all negative Urgent care, 2pm referral to er immediately for rare allergic reaction 5/18 Referral to hospital [ I was refused treatment and concerns with covid vaccine as was there over 2 hrs and never had any vital signs after referral from urgent care. Hospital was told by my family that we are leaving as I had to leave car at urgent care center for my safety and concerns with dizziness [doctor recommended family to take me] Hospital was at 445pm and signed paper that I was leaving to go to hospital 2. They were surprised I did not get called for vitals. Another hospital was around 7pm and immediately ekg, cbc, covid flu and paperwork brought from urgent care doctor was concerned. Cat scan and lungs.. ultrasound and with and w/o contrast. Liver and pancreas levels high caused concern as blockage at bile duct. Fever still present. They immediately gave me before all test a shot of steroids and benadryl. Emergency Surgery for ercp was discussed with 3 other MD and awaited bed. Liver levels and wbc extremely high. Transport 6am to another hospital for surgery.

Write-up: Rapid decline in ability to perform respiratory gas exchange and build up of carbon dioxide in the blood to life-threatening levels, ultimately leading to death. Rapid decline began about 2 weeks after receiving vaccine. Death would likely have occurred on 5/6/2021 or 5/7/2021 but for hospital intervention via use of a BiPAP machine. Patient had been exhibiting more mild, non-COVID respiratory distress for some time, back to at least late 2020, and likely at even lower levels for several years prior. Signs and symptoms included shallow breathing, extreme fatigue, difficulty or inability to lie down, occasional lightheadedness, etc. Oxygen saturation levels in April, as measured by a pulse oximeter, ranged from about 87%-96%. On May 6, it measured 65% just prior to ER visit.

Write-up: Patient received vaccine on Thursday evening. He began to feel very tired Monday and Tuesday. Wednesday he woke up in alot of abdominal pain. We went to healthcare facility at 4:30pm. He was diagnosed with Acute Pancreatitis. He was sent home at 10:30pm We went back to ED at 1:30 am Thursday. He was admitted during the day. In the early morning of Friday May 28th his blood pressure tanked and he became unresponsive. His heart stopped and they did compressions for 8-9 minutes. He was put into ICU. By 3am we were told he may not survive. By 5 am we were told his organs were shutting down. He passed around 7:45am. In researching the vaccine we discovered the SARS virus used in vaccine can attack the pancreas. We believe he died as a result of the reaction to the vaccine and his body and organs could not fight.

Write-up: April 17th, 2021 - Feeling jaw and ear pain in AM. Progressed to severe chest pain, shortness of breath, chest tightness. Went to ER. Elevated Troponins(hi specificity) x 2 over 2 hrs then improved. Pain persisted for hours, Medrol dose pak for 6 days did not improve symptoms. Continued symptoms for over greater than 6 weeks continues to come and go intermittently lasting 15 minutes at a time 5-6 times per week. Not associated with activity or eating. Write-up: After I received the vaccine and was waiting 45 minutes, I had a weird taste in my mouth and I texted my mom to tell her that I wasn''t feeling good. While texting her, I started experiencing color distortions and the next thing I knew was the Pharmacist was asking if I could breathe and they laid me down on the floor and called 911. The Fire Department arrived and took my blood pressure and I tried to walk out and the next thing I knew was that I was weak; therefore, the Ambulance transported me to the hospital. Write-up: PATIENT RECEIVED VACCINE. WAS WAITING IN THE LOBBY FOR THE REQUIRED 15 MINUTES WHEN HE STARTED TO LOOK REALLY PALE. PATIENT LOST CONSCIOUSNESS AND TWO EMPLOYEES AND HIS WIFE LOWERED HIM TO THE GROUND SAFELY. ANOTHER EMPLOYEE CALLED 911. PATIENT HAD A PULSE AND WAS BREATHING, ALTHOUGH BREATHS SEEMED IRREGULAR. EMTS ARRIVED AND TOOK HIS VITALS. AT THIS POINT PATIENT HAD REGAINED CONSCIOUSNESS AND WAS RESPONSIVE. THEY SAT PATIENT UP AND HE FAINTED AGAIN. THEY TOOK PATIENT TO THE HOSPITAL FOR EVALUATION. Write-up: Client was waiting in observation 30 mins when she asked EMTs to check her blood pressure as she was feeling palpitations. First vitals at 1512: blood pressure 158/100, pulse 82, oxygen 98%. PHNs arrived shortly. Client sitting in chair, alert and oriented x4. Per client had reaction to Pfizer first dose and medical team had recommended Janssen as second dose instead. Per client feeling nausea, headache, and palpitations. PHN gave client emesis bag. Client assessed for swelling, flushing, per client no shortness of breath at this time. Retake of blood pressure at 1513: 150/100. Per client history of hypertension, diabetes, thyroid, and took her medications today. No other allergies except Pfizer vaccine. 1515 client reported feeling warmth sensation to throat area and feeling itchiness. Per client it was same reaction to Pfizer vaccine. PHN asked client if she drove, per client had family outside waiting. PHN offered client Benadryl and client accepted. PHN administered 50mg IM on right deltoid at 1519. Vitals: pulse 107, oxygen 98%. At 1522 client stated stronger itchy throat and felt like it was starting to close. No swelling noted, per client feeling tingling around lips. Client stated shortness of breath, unable to take deep breaths. PHN Ae called 911. 1524 pulse 104, oxygen 99%. Per client feeling very dry mouth. PHN kept speaking with client, alert and oriented x4, able to answer all questions in short answers. 1527 pulse 103, oxygen 99%. Client stated feeling warmth in throat area again, began coughing intermittently. Per client new onset of strong headache to occipital area, dizziness and chills. 1531 paramedics arrived and assumed care. Client transported to facility via gurney at 1540.

Pt received his single dose of Janssen IM Shot in the lt. deltoid. pt., who was a 32 y/o M had a 20 second witnessed syncopal event with no traumatic event pre/post syncopal. pt. did not complain of any symptom other than a bit of minor dizziness. Write-up: The night after receiving the vaccine I started to develop a fever and headache. It lasted 3 days for the fever. On Tuesday I felt something different in my body. On Thursday I went to see an Optometrist. While there he found that there was something going on in my eyes. He noticed there was blood clot in the back of my right eye. I did go to the ER because of the blood clot. At the ER I had a CT scan and bloodwork completed. I was told that everything looked normal and I was told to go to a different hospital. I was transferred to Medical Center. While I was there they performed an MRI, CAT scan and Lumbar function test. I was told that everything looked fine from the neurology point of view. I need to follow up with an Ophthalmologist. I have gone completely blind in my right eye and my left eye is not clear anymore. On 05/20 I had an appointment with the ophthalmologist. There was something wrong with my optic nerve as well. There was a test performed 05/21. I went back to the doctor on 05/26 he said that the reports he received are not making sense to him. My next f/u appointment with Dr. is 06/09 because the doctor asked to have2 weeks to read through my hospital reports. I saw a different doctor instead on 05/27, Dr. , and he said that this can happen from the vaccine. He said that there is a swelling and I need to see a different doctor about my retina. I was told that I need to see another physician. I saw Dr. . on 05/27 and he said that there are fluids there, but no one can confirm that this is because of the vaccine. He gave me the option to take prednisone 10mg and I feel that it is working, but it is very slow. It was moved to 3x per day. I can see objects now even though it''s still not clear. I have a f/u with Dr. 06/03.

Write-up: Right after receiving the vaccine my thumb started to swell. I took a picture of it because I couldn''t believe the size. For the next three mornings I woke up with a headache. I took Tylenol and it helped get rid of the headaches. I started to see unexplained bruising on my arms and legs a week after the vaccination. To this day, I still have bruising, but it is going away. I noticed that I was getting Petechiae (tiny red, flat spots that appear on the skin). I noticed a bruise on my breast ( I am a breast cancer survivor and have had reconstructive surgery on my breasts.) After the bruise started to disappear, my skin was already thin from radiation, but the skin started to open up and expose my silicone inside my breast. I had to go to the surgeon and my implant had to be removed. Now I am flat chested on that side. This is going to take several weeks of recovery to get through this. The surgery for the removal of the silicone was 9PM PST, Emergency Surgery.

Preexisting Conditions: Slight abnormal EKG. Coming from low blood pressure. 2 syncope episode arising from severe upper stomach pain caused by Azithromycin one time and some restaurant food another time. 1 syncope in childhood arising from sudden stop after running.
Allergies: Several reaction to Azithromycin.
Diagnostic Lab Data: This is the precise problem as none of the doctors are proactively investigating the issues and addressing issues I am dealing with. Thereby I reached out to my Primary Doctor, to enlist her help on 5/24/2021. Dr. wants the inflammation marker tests which I communicated to the Primary, Dr., but yet to get the script from Dr. messaged saying that will issue after talking to Dr. and they are playing a phone tag. But it has been already a week.

Write-up: About 2 weeks after I received my Covid vaccine I experienced a sharp pain in my chest. I wasn''t on going so I didn''t worry about it but another week after that I began to feel palpitations and decided to go to the emergency room at Hospital. I was given an EKG and an X -Ray of my chest. They didn''t notice any abnormalities so they told me to follow up with my physician. My doctor made an appointment for me to have a stress test. I was on the treadmill for 6 minutes and my blood pressure dropped so low that Dr. called an ambulance to take me to the hospital. I was there from the 27th to the 30th of May taking test. I also had a cardiac cath test which came back inconclusive. Another symptom that now is troubling me is vertigo

The patient''s height, and weight were not reported. The patient''s concurrent conditions included pre-diabetic. It was unknown if patient was taking any prescriptions, over-the-counter medications, dietary supplements, or herbal remedies at the time of vaccination. No allergies to medications, food, or other products was reported. The patient was not pregnant at the time of vaccination. The patient did not have an adverse event following any previous vaccine. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 1805020, and batch number: 1805020 expiry: UNKNOWN) dose was not reported, administered on 20-MAR-2021 (also reported as 27-MAR-2021) for prophylactic vaccination. Anatomical vaccination site were not reported. No concomitant medications were reported. On 03-APR-2021, one week after vaccination, the patient had headache and vomiting. On 05-APR-2021, the patient had seizure due to hemorrhagic stroke. On an unspecified date, the patient experienced brain compression, brain stem haemorrhage, brain stem syndrome, cerebral haemorrhage, hemiplegia, possible cerebral venous sinus thrombosis, biopsy soft tissue, and brain scan abnormal, and was hospitalized. On 06-APR-2021, the patient had a surgery to remove pressure on brain and brain stem (craniectomy). The surgeon noted alarmingly low platelet counts (results unspecified). The patient''s height, and weight were not reported. The patient''s past medical history included hypertension, hypothyroidism, and intermittent pancytopenia, and concurrent conditions included metronidazole allergy, hashimoto''s disease, and microscopic colitis. The patient was previously treated with metronidazole. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, intramuscular, batch number: 1805018, and expiry: UNKNOWN) dose was not reported, administered on 09-MAR-2021 for prophylactic vaccination. Concomitant medications included amlodipine, levothyroxine, losartan, rosuvastatin calcium, and ubidecarenone. On 09-Mar-2021, the patient got the vaccine and after few days she had aches and pains (very mild reaction) and went to bed early. The patient thought she was fine. On 18-Mar-2021, 04:00, she woke up with an excruciating headache above her left eye in her eyebrow. The patient wondered if it was an aneurysm and took 2 tablets of extra strength Tylenol and it helped. The patient stated that after every 5-6 hours, excruciating headache came back. The patient had the same headache for over a week. The patient got tested for Covid-19 and it was negative. The patient thought she had sinus problems but had no other symptoms. On 30-Mar-2021, the patient woke up with headache. Her eye was killing her, was sore and swollen, and felt like it was going to burst and her left eye was swollen shut in a couple of hours. The patient narrated it was excruciatingly painful. The patient''s husband drove her to urgent care, where ophthalmology tested the pressure: right eye was normal (she thinks it was 18- normal) and the left eye was 51 (normal is 14-19), units unspecified (intraocular pressure decreased). The eye doctor started prednisone and the patient had a CT scan and lab work done. MRI was also ordered but they were unable to do it right away. On unspecified date in Dec/2020 the platelets were 349 in December and came down to 64, which was very low (low threshold was 150) (thrombocytopenia). The CT scan was performed, they thought she had a cavernous carotid fistula and she was scheduled for an angiogram for a further diagnosis on 01-Apr-2021. On the 13th, she was watching television and saw that the vaccine was being put on hold due to the women with clots whose progression matched hers exactly (severe headaches, low platelets, and required attention). She was on prednisone for a month. The patient''s height, and weight were not reported. The patient''s past medical history included tendonitis in right shoulder, surgeries on right shoulder and concurrent conditions included arthritis in right shoulder, non-smoker, abstains from alcohol, and seasonal allergies, and other pre-existing medical conditions included the normal wear and tear on her body at the time of this report. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: Unknown) dose was not reported, one total, administered on 04-MAY-2021 at the right arm for prophylactic vaccination. The batch number was not reported and has been requested. No concomitant medications were reported. The Patient reported that on 04-MAY-2021, after vaccination, she began feeling unwell with a low grade fever, her highest body temperature was 102F. She experienced headache that she initially felt on the right side of her head but could not determine the source for sure. On 14-MAY-2021, she coughed up blood, but she didn''t think anything of it and continued her normal routine then on 15-MAY-2021, she coughed up more blood but it had some phlegm in it, so she went to the ER (emergency room) on 18-MAY-2021. She received a Computerized axial tomography (CAT) scan of her chest which showed blood clots in her right lung. She got a secondary CAT scan and blood clots were found in her stomach, brain and in her neck. She was admitted to hospital on 19-MAY-2021. The Patient was reportedly hospitalized for 6 days and was supposedly discharged on 24/May/2021. For treatment, she was receiving an Intravenous (IV) blood thinner but later changed to an oral blood thinner. She was also receiving Tramadol for pain. She was receiving other medications when first admitted but could not recall specific names. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient had not recovered from blood clot in brain, blood clot in right lung, and blood clot in stomach and neck. The patient''s height, and weight were not reported. The patient''s pre-existing medical conditions included patient had no known previous medical conditions and had no known allergies. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 041A21A, expiry: 21-JUN-2021) dose was not reported, once total administered on 06-APR-2021 15:00 in Left Arm for prophylactic vaccination. Concomitant medications included amlodipine, hydrochlorothiazide and losartan for which indication was not provided. On 06-APR-2021, the patient experienced headache while driving back to home and was not sure if it was due to vaccine or traffic conditions. At 18:30, patient got up to walk across the room and felt a strong sharp pain in her right and left leg described as incredible cramping type of pain which her husband had to help sit down to a chair. Patient states that although the pain was in both legs, the left leg felt as though someone had took a knife to her thigh, the pain lasted approximately 15 minutes. On 07-Apr-2021, patient states that she went for a routine doctors visit to her family care physician who wrote her a prescription for routine blood work. On 14-Apr-2021, patient went to get the blood work done and states at this time, she felt extremely exhausted and that due to a bloodwork mix up which required her to drive back to the lab, she was not able to drive back so her husband had to drive her. Around 17-Apr-2021 to 18-Apr-2021 patient had to chase the dog and found that she couldn''t catch her breath and that she started coughing. Her daughter who happened to be there asked her why she was coughing and thought that it was not normal and that she should contact her doctor.On 19-Apr-2021, patient called the doctor. On 21-Apr-2021 patient saw the doctor and during examination, patient was instructed to run a D-Dimer test to rule out blood clots. On 22-Apr-2021, doctor stated patient to go to the Emergency Room as the result for the D-Dimer test came positive. The patient was immediately hospitalized where Doppler and CT scan was performed on her and was diagnosed with Deep Vein Thrombosis and Pulmonary Embolism on both the lungs. Patient mentioned that she was treated with Heparin drip from 22-Apr-2021 to 24-Apr-2021 and then she was given Xarelto. On 25-Apr-2021, the patient was discharged from the hospital after 4 days of hospitalization. Patient reports that she was still experiencing extreme exhaustion and that the pulmonologist states that this is the normal course of blood clots and it could take anywhere from 60-90 days for the body to clear up the blood clots. She states she still has bouts of shortness of breath but showing signs of improvement. The patient''s weight was 285 pounds, and height was 56 inches. The patient''s concurrent conditions included high blood pressure, depression, diabetes, flea bites allergy, peanut allergy, alcohol consumer, non smoker, heart disorder, nerve pain, mood disorder, fluid retention and pain. He had no known drug allergies. He did not have any drug abuse/illicit drug use. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 207A21A, expiry: 23-JUN-2021) dose was not reported, 1 in total administered to right deltoid on 09-APR-2021 for prophylactic vaccination. Concomitant medications included hydralazine 25mg BID (twice a day) for blood pressure, metformin hydrochloride 500mg BID for diabetes, Baby Aspirin (acetylsalicylic acid) EC (enteric coated) for heart disorder, escitalopram 10mg daily for mood disorder, gabapentin 100mg TID for nerve pain, and celecoxib 200mg 1 tablet a day as needed for pain. olmesartan medoxomil 40mg daily for blood pressure, amlodipine besilate 10mg daily for blood pressure, furosemide 40mg daily for fluid retention, potassium chloride ER (extended release) 20 Meq TID (thrice a day) and metoprolol succinate ER 50mg daily for an unknown indication. Write-up: FELT LIGHT HEADED; DISORIENTED; SWELLING OF STOMACH; SWELLING OF ARMPITS; SWELLING OF HANDS; ANXIETY; FEELS LIKE VERTIGO; This spontaneous report received from a patient concerned a 58-year-old male. The patient''s height, and weight were not reported. The patient''s past medical history included covid-19 (took 6-7 months to clear), and concurrent conditions included smoking, fatty liver, high triglycerides, dieting, and seasonal allergies. Patient had no known drug allergies. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 204A21A, and expiry: UNKNOWN) dose was not reported, administered, 1 total to left arm on 09-MAY-2021 for prophylactic vaccination. Concomitant medications included loratadine for seasonal allergies. On 09-MAY-2021, 30 minutes after vaccination the patient felt light-headed and disoriented and he drove to the hospital where he consulted in emergency room and did blood work, and did X-ray of heart and lungs. They put him on IV (intravenous). Patient was admitted for 4 hours and discharged. On 09-MAY-2021, the patient also experienced swelling in both hand, stomach and in his armpits. Since after discharge patient felt disorientated sometimes. The patient felt nervous and also felt like vertigo. Patient was completely disoriented and felt dizzy when he was walking (prior day of reporting). He still had swelling on hands and recovered from swelling on stomach and armpits. Patient felt anxious sometimes and could not walk and he mentioned it could be partly due to unemployment and overwhelming medical bills. The patient mentioned that he had covid-19 in OCT-2020 which was pretty bad that made him decided to take vaccine. He was worried how long these symptoms will persist at a time and unemployment added to his anxiety. He also mentioned about seeing doctors to rule out other rational other than the vaccine. Patient had lab test during his emergency room visit. Laboratory data included: Blood test (NR: not provided) as Not reported, and X-ray of heart and lungs (NR: not provided) as Not reported. Write-up: approximately 13min post vaccination patient fell to the ground, medic went over and then checked for injuries. Within minutes patient was answering questions and was oriented to person, place and time. Patient then started vomited, vomited for about 2 minutes stated he then felt better. Dad came and medic recommended that we call 911 and have the paramedics take him to the hospital - Dad agreed.

Write-up: elevated heart rate (up to 135 while resting) beginning approximately 8-10 hours after receiving the shot. my heart also had strange, strong beats and did not feel right. I left the hospital before someone would read my EKG because it was crowded and I was worried about infection. The symptoms persisted for approximately 6 to 8 hours and mostly coincided with fever and chills. about a week or ten days later, I had another episode of strong heart beats lasting about 6 to 8 hours. Write-up: I woke up one morning will getting out of bed. I stepped on my left foot and noticed a lot of pain in my left foot. I went to an apple urgent care center. They did blood work. I also had pain in my left arm as if someone was pressing on my nerve.

Write-up: The day after I experienced extreme muscle and joint pain that lasted about a week. And it started to get better but the following Saturday I got a very terrible headache. And over the course of that week I started bruising very easily and all the veins all over my body were very prominent. The headache wouldn''t go away and I started to have chest pain by that monday. And at that point all the muscle and joint pain came back and it was very very painful. At that point I called my primary care provider and they scheduled me with someone else in the clinic because i was having the chest pain. And they did a test for covid and urged me to go to the emergency room, which i did. After that I went to the emergency room and sat for 5 hours, they did bloodwork to check clotting factor, basic blood panel and also a CT of my head. According to them, everything was normal and they sent me home with a terrible headache. My biggest complaint was the bruising all over my body but no one ever looked at that.
