Write-up: I woke up one morning will getting out of bed. I stepped on my left foot and noticed a lot of pain in my left foot. I went to an apple urgent care center. They did blood work. I also had pain in my left arm as if someone was pressing on my nerve.

Write-up: The day after I experienced extreme muscle and joint pain that lasted about a week. And it started to get better but the following Saturday I got a very terrible headache. And over the course of that week I started bruising very easily and all the veins all over my body were very prominent. The headache wouldn''t go away and I started to have chest pain by that monday. And at that point all the muscle and joint pain came back and it was very very painful. At that point I called my primary care provider and they scheduled me with someone else in the clinic because i was having the chest pain. And they did a test for covid and urged me to go to the emergency room, which i did. After that I went to the emergency room and sat for 5 hours, they did bloodwork to check clotting factor, basic blood panel and also a CT of my head. According to them, everything was normal and they sent me home with a terrible headache. My biggest complaint was the bruising all over my body but no one ever looked at that.

Was then sent to the Emergency room for evaluation and treat for DVT. Complained of Shortness of breath. CT Scan Angiogram completed and found to have bilateral Pulmonary Embolism

Diagnostic Lab Data: In the ED on 5/25/21, the physician did not see rashes, swelling or other signs of skin infection. Lab results were within normal limits. ED physician recommended that patient follow-up with primary physician.

Write-up: Per patient via phone call on 6/1/21: Woke on Sunday, 5/23/21 with high fever, sweats and swollen veins on hands. Went to ED on 5/25/21. At that time, he thought he had an infection "eating him away from the inside". Now he believes, after thinking about it, that it must be a reaction to the COVID vaccine he received on 5/21/21. He stated that he still has weak, cramping hands and swollen veins in his hands.

Write-up: I had a rash that formed on all four of my limbs that looked like a poison ivy rash. The rash was under my arm and legs. It wasn''t spreading but it became very painful. It is slowly starting to go away with the medication that the doctor provide.

Write-up: Feeling lack of oxygen and felt faint and high, passed out later. Still feel issues including various levels of inebriation. Prescribed potassium pills. Much better but still dealing with event. Also, had chills, heart racing, and short of breath.

Write-up: On 04/21/2021, 12 days after I received the vaccine, I woke up with my entire left side feeling numb and I had a headache. I felt like I had pressure on my chest and I was really lightheaded. I got up for a little while and I felt like I was going to faint. I felt like I needed to eat something and I was very weak. I drank some juice and ate an orange. I was having a hard time swallowing and at times slurred my words. I realized that something serious was going on because I had pain like when I went through chemo. The base of my spine has been hurting every day since I received the vaccine and I have had headaches since as well. I was having heart palpitations and felt super weak. I didn''t go to work for 2 days. That was 04/21 and 04/22. On 04/23 I had to do a zoom conference for work, for an hour, and my eyes starting hurting really bad like my eyes had been open for 24 hours. The light from the computer screen was really affecting me. At 5pm I did a tele-video appointment with the NP. She told me that she wanted me to go urgent care because it was the closest urgent care to me. After going to Urgent Care on 04/24, the doctor wanted me to go to the Hospital by ambulance. At the hospital the performed an EKG, CTA and MRA and they said that everything looked fine. They couldn''t validate that my pain was from Johnson and Johnson vaccine. I was release at 10:30pm PT, I called Dr., my Oncology surgeon, and he referred me to Neurology doctor. I got an appointment with Dr., 04/27, and she ordered a CTA and an MRA. I had the scans completed on 04/28, with and without contrast. The radiologist found something. I was sent back home, but I was told that right vertebral artery was completely excluded. In January of 2020, there was a CTA performed, and I didn''t have any numbness and my vertebral artery still have a steady flow even though there was a tear. It also showed that I had more stenosis in my carotid artery. I have now been referred to a new Neurosurgeon. Dr. never saw me because they couldn''t view my records. The doctor told me to take Crestor and 81 mg aspirin. I did not feel that those medications helped at all. If anything, I felt worse. on 05/15, I was in the passenger seat of my grandson''s car and I ended up throwing up and there was blood coming out with the vomit. He took me the Hospital. I had a video appointment on 05/11, with an NP, and she told me to up my aspirin to 325mg and continue to take Crestor. She referred me to The Stroke Neurology Group and she referred me to get a CTA. She also referred me to a GI doctor. On 05/15, against medical advice, and was taken by ambulance to another hospital. I stayed there until 3AM 05/16. They performed an MRA, a CT, Stroke Angiogram, they MRI with and without contrast, a chest x-ray and a lot of blood work. They also performed are lactate test and at first it came back as 2.5, which made them think I had an infection, but they re-tested and it came back as 1.5 which they said to be normal. The results from the hospitals. On 05/19 I went to see a Cardiologist, and she took me off of Crestor and put on pravastatin. She order an echocardiogram and a treadmill test and an EKG. I did see a GI doctor and they told me that the aspirin that I was told to take is what was making me throw up blood. I do have a neurology appointment on 06/11 and I have an appointment with the neurosurgeon on 06/16. My last heart test came back normal.

Write-up: Almost immediately after receiving the vaccine pt went rigid and was unresponsive. He came to within a minute. He was very panicked and stated he thought it was a dream. He was extremely pale and very sweaty. We gave him some water and an ice pack to cool down. We checked his blood pressure. He then lied down on the floor and we elevated his feet. He then stated he could not move his hands and his lips were tingly. We then gave him some liquid benadryl and continued checking his blood pressure and called EMS. They arrived and he left with them. I tried contacting him later without success. Will try again tomorrow.

Write-up: 33-year-old woman presented to the emergency room on May 7, during the second week of receiving J&J vaccine. She had decreased sensation/numbness in the left side of face in the cheek and jaw area, with an inability to speak. No pain. Symptoms lasted for couple of hours and resolved. On May 8, felt like she had swelling in her lymph nodes in the neck, making swallowing and talking difficult. The symptoms also resolved in couple of hours. Write-up: Regular symptoms from the shot, EXTREME back pain (lower) headache, felt cold and exhausted, on first day. The second day I had blurry vision and all those symptoms so I went to doctor. They said I was having a reaction and also gave me Toradol for the back pain. I started to feel better symptom wise but the following day I started to poop blood. Write-up: 2 events : 1st-4/15- Hives on both arms. Just watched them and kept an eye on them and used Benadryl and anti-itch cream, postponed allergy shots from Friday 4-16 until Monday 4/19. Hives went away in the next day or two. 2nd event-Lower right back pain achy off and on and eventually wrapping around to the right side. Went to urgent care on 4/29. They did a CT with contrast and found a large kidney stone that I would not be able to pass without surgery, referred to neurologist. Saw neurologist on 5/3, kidney X-ray confirmed an 18 mm kidney stone in my right renal pelvis. Scheduled lithotripsy with stent placement on 5/10. Had that done on 5/10. Had follow up kidney X-ray and stent removed on 5/17. Then on 5/24 another follow up kidney X-ray. Results weren?t easy to see due to gaseous distention but he thought I had passed most of the larger particles. Will see him in 6 months. May see him sooner because I?m still experiencing achy pain in the same areas off and on that same side.

Write-up: ELEVATED BP; "SPACEY"; MUSCLE CRAMPS; SORE ARM; EXTREME CHILLS; TIRED; This spontaneous report received from a patient concerned a 74-year-old white and not-Hispanic or Latino female. The patient''s weight was 150 pounds, and height was 61 inches. The patient''s concurrent conditions included high blood pressure, allergy to formaldehyde, paint, nail polish, raspberries and pepper. Patient was non-smoker, and non-alcohol user. Patient had breast cancer 15 years ago. Patient had allergy to new carpeting with fire protection coating. Patient did not have any drug abuse or illicit drug use. The patient experienced drug allergy when treated with Tylenol, sulfa drugs, ibuprofen, loratadine, and hydrochlorothiazide/losartan potassium for drug used for unknown indication. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: Unknown) dose was not reported, 1 total administered on 28-MAY-2021 to Right Arm for prophylactic vaccination. The batch number was not reported and has been requested. No concomitant medications were reported. On 28-MAY-2021, couple hours later vaccination, patient was tired and spacey. At around 10:30 pm, she noted extreme chills and muscle spasms. She could not warm herself up, and kept getting colder and colder. She went ER (emergency room) at 1:30 am because her BP (blood pressure) was high (176/110 mmHg). She left the ER at 5 am it was 150/80 mmHg. At the hospital, they gave her Tylenol, which helped her within an hour. They noted to add some Advil with the Tylenol for relief, and to continue for another 48 hours. Her arm was little a sore. It sometimes cramps in the area about every 8 hours. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient recovered from spacey, and tired on 28-MAY-2021, was recovering from extreme chills, elevated blood pressure, and sore arm, and the outcome of muscle cramps was not reported. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 041A21A, and expiry: UNKNOWN) dose was not reported, 1 total, administered to left arm, on 05-APR-2021 for prophylactic vaccination. No concomitant medications were reported. On 05-APR-2021, at 18:00, vaccination provider gave shot on side way as she could find the muscle, immediately felt at 3 sites severe pain, at injection site, front and back of the shoulder, top of the shoulder. On the same day, after taking shot, around 14:30 minutes, patient had nausea, dizziness, and felt fainting. The vaccination provider gave her some orange juice and after 2-3 minutes later after drinking orange juice, patient felt better. Patient reported that arm remained sore for two weeks and had no other symptoms. Patient usually wore sock during night on a daily basis. On an unknown date in APR-2021, in the morning, patient noticed swelling (calf swelling) up to the sock at the line where elastic, around mid-calf area. On 28-APR-2021, at evening, patient felt heart beating out of rhythm, heart palpitation, and unusual feeling in the chest. On 29-APR-2021, she went to urgent care at 8:00. Patient was checked with electrocardiogram (EKG) machine, took blood test. Physician Assistance advised patient to go to emergency room (ER). On the report, it said vital signs noted as afebrile and cardio no rubs, no peripheral edema, heart rate was showing a normal rhythm with a rate in the 150s. They gave IV (intravenous) and bunch of fluids. On the same day, patient went to emergency room around 10:00. Patient was connected with EKG machine, had blood test and covid test, and monitored heart around 11:00, arterial fluttered atrium beating 2 times faster than ventricles high based on chest and no other issue. Patient was mildly sedated and was performed cardio version. Patient woke up around 13:30 and did not feel heart beating unusual way. Patient was discharged on same day from emergency room referred to cardio electro physiologist. Patient was given prescription medication metoprolol tartrate 25 mg take 1 tablet 2 times a day dispensing quantity 30, blood thinner xeralto 20 mg take 1 tablet by mouth once daily 30 tablets but patient did not buy medication. After 3 weeks, patient had discoloration on the right arm and it turned blue and swollen. On 03-MAY-2021, the patient was hospitalized. Write-up: At 12-00pm the pharmacist gave patient a J & J Covid Vaccine. He asked him to sit down for 15 minutes. At approximately 12-10PM Patient yelled for assistance. At the time the pharmacist ran over to the chair. Patient siaf that he was going to pass out. His eyes rolled back and it seemed like a seizure, at that point he began to pass out. The pharmacist assisted him to the floor. He was unresponsive on the floor for 10 seconds. After 10 seconds he regained consiouness. He said he felt fine. We called the ambulance. They came 15 minutes later. The pharmacist did his vitals pulse: 60 respirations:16 blood pressure 115/70. At that time the ambulance came and assisted him to the hospital.

Write-up: Five weeks (05/16/2021) after receiving the Johnson and Johnson vaccine, my left leg just started feeling numb. I went into my kitchen and all of a sudden I started feeling dizzy. I lost all strength in my legs and in my left arm. I really couldn''t move my arms or my legs and I felt really light headed. I had to have my 5 year old boys get me some water to drink. My son noticed that my heart was beating really strong and really fast. I saw water boiling over in the pot on the stove and I couldn''t even get out of a chair to turn it off. I couldn''t stand up. I finally managed to go back by my bed from the chair and slowly moved around. I tried holding my phone and it dropped out of my hands a couple of times because I couldn''t hold it. A couple hours later, I felt a little better, but I had to fight to get around. We fell asleep and when I woke up, I still felt bad. I took my children to school and told them that I was going to the hospital to get checked out. At the hospital they performed multiple tests and they back showing I had stroke. I was treated for suffering from a stroke.

Diagnostic Lab Data: Blood work was performed and my blood systemic PH was notably off because it was at 8.5 (urinalysis test performed as well). They gave me Zofran for the nausea and told me to stop drinking reverse osmosis just in case it was causing my PH level to be off. I was then released and sent home.

Write-up: The day within 10 minutes I started feeling hot and feverish. Then I started to feel lumping and cramping in my left breast and it persists and it turned in to nausea, vomiting, and dizziness. I also felt body cramping and both of breasts became extremely hard and was not comfortable. I also had muscle aches along with a mild fever for about a week. The pain was persistent. Then on day 2, the headaches started and I couldn''t sleep because every time I moved the headache would cause me to vomit. So, I couldn''t. My vision was blurry and I was not sure if it was due to the headache or the vaccine. I ended up going to the ER on 05/16/2021 for cramping in my left leg which was down by my ankle and was bruised. I''m still experiencing heart palpitations, tiredness, cramps still happen in my breasts, and there are still moments when my vision goes in and out.

Write-up: I got my Covid vaccine on Wednesday, 5/5/21. That night I experienced the typical side effects (Chills, Aches, Pains, Soreness in the injection site on my right arm). By Thursday afternoon I was feeling significantly better. However, on Friday evening I started to feel a tightness and pressure in my chest and left arm and neck. The chest pain felt like I was having a heart attack but my heart rate was mostly normal. I felt shortness of breath where I couldn''t get a good deep breath in and I was also nauseous and had no appetite. At times I felt lightheaded and weak. These symptoms continued over the next couple days. On Monday 5/10/21 I decided to got to the ER to get checkout since I was still having chest pains and tightness. They ran a series of tests and concluded that I was just experiencing typical aches and pains from the vaccine. My symptoms continued for another full week and at times the shortness of breath and chest pains felt really severe. By the following week I was starting to feel a little better on and off. I got my appetite back and and I was able to start exercising again. After about 2 full weeks after receiving the vaccine most of chest pain and pressure + shortness of breath has disappeared and I feel better.

Write-up: patient has developed blood clot after getting the J&J shot on 4/6/2021. The patient has had a sore arm since the day he got his vaccine. According to his wife, he started to feel weak on Sunday 5/30/2021. She said his hands were cold that day too, then by Tuesday his fingers turned white, this is when she took him to the hospital and found out about the blood clot. After 5-6 minutes of observation, pt reported feeling faint and appeared diaphoretic. Per facility staff, pt just admitted into rehab facility and may be going through alcohol withdrawal. Pt then started slumping over the chair and became unresponsive. Facility staff called 911. Upon waiting for paramedics, nurse and staff laid pt down onto the floor. Pt became responsive and started talking again. Respiration and heart rate within normal limits. Monitored pt until paramedics arrived. Pt taken to ER for further evaluation.

Write-up: 15:50pm ?patient was injected Janssen-J&J COVID-19 vaccine to Lt. deltoid muscle ? 25gauge-1? safety syringe used w/o difficulty. Side effects/adverse reactions explained to client, verbal acknowledgement and consent obtained. Written consent signed prior to injection. 16:20pm ? Patient was carried to clinic area by 3 fellow patient. He presented obtunded, grey pallor, face noted to be profusely diaphoretic with gasping attempts at respiration. P-110. Unable to respond to verbal commands, unable to speak or open eyes. Brisk taps to his face and calling his name elicited a slight optic response by visible attempt to open his eyes. At this time, he was observed to grimace, grab his throat and jerk himself upright, proceeded to projectile vomit a moderate amount of clear fluid. Immediately slumped off chair and was limp and non-responsive to commands. EpiPen Auto Inject administered-0.15mg Epinephrine to Lt. anterior quadriceps. Pulse-126, gasping during respiratory attempts. Skin frigid to touch. 16:22pm- B/P-150/101- P-58 per portable wrist B/P monitor. Respirations -22-rapid and shallow, appearing to hyperventilate. He attempted, but unable to follow instructions for calming and deeper breaths at this time. Fellow seamen and the 2nd Officer, carried him to the ship ?hospital? area accompanied by me RN, monitoring continued. Patient responded rapidly to administered Epinephrine. 16:23pm-B/P-148/108-P-68-per monitor-R-20 - Responsive to name, LOC x3, visible muscle twitching and tremors. Remains diaphoretic, pallid, and skin is cold to touch. Speech is slurred and weak, client continues to be lethargic and slow to verbal responses. C/O of ?bad HA?. 16:30pm- two-500mg Paracetamol tablets PO with H20 administered by 2nd Officer for increasing HA pain per patient request. Lights dimmed in room. Clear soup and a coke ordered for patient due to suspected hypoglycemia based on reported pt. hx of limited caloric intake in the past 12 hours. Attempting to locate a glucose meter on ship at this time. 16:38pm ? B/P-139/87-P-69-per monitor-R-20- C/O severe HA and feeling weak. C/O muscles twitching in extremities. Visible twitching noted. Reassurance provided. 16:43pm- B/P-139/87-P-69-R-20 ? Pt. fed clear broth soup and 12oz Coke PO without difficulty. Resting quietly. Appears to be tired, with continued slow responses to questions and weak speech. Pt. continues to be concerned about his exaggerated leg muscle twitching. Reassurance provided and informed of effects of epinephrine. 16:52pm ? B/P-133/86-P-65-per monitor-R-18- Personnel informed of need to transport by EMS to the Hospital for extended monitoring and evaluation by Physician per CDC adverse response to COVID-19 vaccination Protocol. Report given to personnel ? Technical Superintendent- who began transport requests through EMS services. Report to RN Supervisor. 17:07pm- B/P-126/83-P-85 per monitor-R-18- Monitoring pt. at BS. VSS. C/O severe HA and feeling ?weird?. Exhibiting pain with HA despite previous Paracetamol PO by squeezing his head and stating it hurts. Write-up: After administration, patient did not report any reaction and was asked to wait 15 minutes for monitoring. A few minutes later, he started feeling lightheaded and called his father. His father notified me that he was "spazzing out/seizing" with his head jerking backwards "for a couple seconds". When I saw the patient, he was sitting and unresponsive to verbal and physical stimuli with his father patting his cheek. After a minute or so, the patient was able to respond, but his voice was faint. He reported having a "severe headache" and feeling "sweaty". He had no fever or difficulty breathing. Father said there was no anaphylaxis, and asked for a bucket since the patient felt very nauseous. I then called 911 for emergency services. When I returned to the patient after the call, the father said he was feeling much better and that the "color had returned". Patient was unwilling to stand, and remaining symptom(s) was headache. Emergency services came and checked blood pressure, which was normotensive (SBP 111). Father insisted ER visit was not necessary.

Write-up: The vaccine overstimulated my thyroid, which produced chemicals, creating sever tachycardia producing 170 BPM''s. When my bloodwork was tested, my thyroid levels were over doubled the limits. I also had high blood pressure. Four days after receiving the vaccine I went to the hospital because I was experiencing a fast heart rate, I was extremely shaky (to where I could hardly stand) and I was having trouble breathing. After multiple tests at the hospital, the conclusion is that I have to let all of the thyroid hormones leave my body. So, basically, I''m just waiting.

Write-up: Patient was waiting in waiting area after receiving Jenssen vaccine. After about 15 minutes, another patient who also received Jenssen came to inform me that her friend was having difficulty swallowing. Patient was not able to speak English so her fried translated to her. She was able to talk and responding to her friend''s questions. I gave her Benadryl liquid to see if it resolves. After 5 minutes or so she complained of having chest tightness and having hard time breathing. I immediately gave her EpiPen 0.3mg injection in left thigh. After few minutes patient said he is feeling better with chest tightness but still having feeling of throat closing. at this time, I had one of my tech call 911 and provided another EpiPen injection (about 5 minutes after the first one). Police showed up in approximately 5 minutes. EMT showed up 10 minutes after them. Patient was conscious the entire time. When EMT took her, she was feeling better but still complaining of difficulty swallowing. Management was informed about the incident

Write-up: I went to Pharmacy on Aril 10 2021, after the vaccine I got fever for three days in a row, then four days with pain in my left leg. Diagnostic Lab Data: Went to ER at 11:00 p.m. on 4/21/21. Nurse who asked me for the reason for my visit told me that they had seen vaccinated patients coming in with hives. The nurse took my temperature and blood pressure. We waited for over an hour - by this time my vision was back to normal - and it was clear I wouldn''t be seen for several hours. I decided to check myself out of ER and see my doctor later in the day - it was now close to 1:00 a.m. My doctor wouldn''t see me in person because of my upper respiratory symptoms, they told me I had to go to urgent care, which I did on 4/22/21. The NP confirmed my tonsillitis/strep and he told me that the hives were likely due to something I had contact with - which I disagree with as I had not introduced anything new externally (detergent, soap, etc.). My vision was fine at this point so he really couldn''t comment on that.

Write-up: At approximately 10:00 p.m. on 4/21/21 I noticed that my vision was blurred. I removed my glasses, put them back on - and still had blurred vision both near & far. Write-up: This is a 28 Years old Female presenting sent from urgent care to rule out stroke presenting with headache, twitching and tremors of the right upper and lower extremities, and tongue numbness. The patient mentions that yesterday they received the first dose of the Johnson & Johnson vaccine. She received the first dose of the modern vaccine in April but had an allergic reaction to it (hives, wheezing, cough, chest pain) so she did not receive the second dose. She received the first dose of the Johnson & Johnson vaccine at 3:15 PM yesterday and her symptoms began at 5 PM yesterday. Since their onset the symptoms have remained the same. Write-up: The first day, I was in a lot of pain all over my body, the normal aches and flu-like symptoms. I was in so much pain that I cried. Write-up: About 2 weeks after receiving the J&J Covid-19 vaccination, I became ill with confusion, lethargy, and disorientation. Taken to hospital in town to rule out possible CVA. MRI, CT, Carotid sonograms, were all negative. While waiting on lab, the preliminary DX was metabolic Stroke. After all lab returned, the diagnosis was Hemolytic Uremic Syndrome (HUS). Transferred to ICU in different town for dialysis and plasmapheresis. Hospital in another town had closed their department for holiday. I spent about 2 weeks at hospital. My Hgb on admission was 7.0 and kidney function was 20% . the platelets were being destroyed in the glomeruli. Blood transfusion and platelets were contraindicated.

Write-up: Patient developed numbness and weakness in left arm, left leg and left side of abdomen. Cervical MRI with and without contrast showed enhancement . Lumbar puncture had high protein levels. Neurology consulted and think process is either cervical myelomalacia or cervical transverse myelitis/vaccine associated inflammation. Patient had never had a problem like this before. He was given high dose Solumedrol X 2 days and sent home on a prednisone taper. His numbness and weakness had improved at time of discharge from the Hospital on May 18, 2021. Write-up: The day after, I woke up with a bad headache and I had the headache for a good 19 days. The doctors thought I was over-reacting. It was a bad headache on left side of head. I was just not feeling well. On the 19th day, it went away. I still from time to time I do still feel pain in the head on the left side. I had a lot of tightness in my left leg. In previous years, I have had blood clot in my leg and after birth in my stomachs - so with the pain in calf I called a doctor and went. They did a visual. After a lump appearing in my leg, an ultrasound was scheduled. I went Urgent care on May 10 - she ordered an ultrasound but her office didn''t ever follow up with me. On 17th, I saw my Primary care and she sent me straight to get an Ultrasound - on the 18th, it revealed I had a blood clot.

Write-up: While in observation after vaccine was given, 18 y/o male had a tension of body and fainted out of chair from sitting position. Head did hit the concreate floor. Male immediately jumped up once he hit the floor. Syncopal event occurred. HE was brought back into treatment area and placed on EKG along with vitals. Normal sinus rhythm, BP and HR stable. Small laceration to right side of temporal lobe about 1cm long, minimal depth, no stiches required, bleeding stopped immediately and ice pack placed. Neurological intact, pupils equal and reacted to light. AAOX4, No nausea, vomiting or dizziness. Male is stable condition with brother. Write-up: The evening after the shot I experienced very high fever, 104.8, chills, shaking, body aches. The next two days the fever stayed at 101 and by the third day I felt better. Then a week later I developed dizziness, falling sensation. A few days after that I started to feel numb on the left side of my face. Then on April 22 I went to the doctor because the numbness had moved to my hand and foot on left side. It was determined that I wasn''t having a stroke and that my iron levels were low after blood test. I was given Ferrous Sulfate tablets to start taking. My symptoms persisted and the dizziness and numbness continued to worry me. I called the doctor again who said to take steroids (prednisone) for one week. He thought I might have Balls Palsy even though my face wasn''t drooping. The steroids did not change my symptoms. On May 31st the dizziness felt more spinny and I had awoken with severe headaches the past two mornings. I went to Urgent Care where they performed and EKG and blood glucose. All was fine. She sent me to the ER to rule out brain tumor, MS, or aneurysm. I had four MRI''s done and all were fine and normal.

Write-up: I had the vaccine and around midnight I had fever and I thought that was normal but a week later I had a very intense headache and I still thought that was normal but I was worried because that was the time they had discontinued the Janssen vaccine. After that I had a very intense pain on my legs and some big and purple veins appeared from my knees down to my legs and some red veins on my thighs. The pain was unbearable, and I could not walk. I bought some creams and medicine but they did not help relieve the pain and I felt like my legs were burning and I decided to go to the ER. They did some vein exams and said I did not have thrombosis; they also did a blood test and a urine test and everything was normal. My legs still hurt until today and I really regret having the vaccine.

Write-up: after 12 hours that I took the Janssen vaccine, I woke up around 12 am and sweating a lot. my 2 hands and 2 feels are numb and even my tongue was numb and I can not control my body and my finger can not bend. Write-up: Patient has been suffering from headaches, rashes, chest pain, and numbness to her hands and fingers. Patient was order a CT scan of the chest. CT scan indicates mild pericarditis as well as post inflammatory lung changes. Echocardiogram is pending to confirm the diagnoses of pericarditis. Write-up: About 7 hours after, I developed a fever of 103 that lasted about 12 hours. On 5/25, I woke up with a rash/hives that covered my chest, stomach and back, which I still have. I went to the urgent care and was given steroids, Benadryl and Pepcid. The meds stopped them from spreading, but they didn''t go away. On 5/31, I went back to urgent care due to it spreading more onto my neck, bottom and starting to do down my thighs. It looks like little burns all over my body. They are thinking I got it because my body was trying to fight off the vaccine due to my deficiency. I was put on 12 days of steroids to prevent it from continuing to spread, the inflammation and itching. She diagnosed it as Pityriasis Rosea. She said that I will have this rash for approx 4-8 weeks.

Write-up: 25 year old female with prior history of syncope was at public vaccine clinic when she syncopated 9 minutes s/p receiving her 1-time IM dose of J&J COVID-19 vaccine in her L deltoid. When she syncopated, she fell forward while sitting in a chair and hit her head on concrete. She suffered a 1cm superficial laceration on her L frontal scalp. There was a brief moment of generalized seizure-like activity that lasted for about 10 seconds. After this, she woke up and was able to sit up. Patient appeared anxious. She was alert and oriented x 3 once she was up with a GCS of 15.

Write-up: Felt numbness in the tongue as well as taste of anesthetic for 5 days. Had 3 minutes after the injection redness of the face and chest. Had bodyaches 7 days after and persisting since that time for now nearly 2 months.

Write-up: TWO BLOOD CLOTS; LEFT CALF PAIN FOR A COUPLE OF DAYS/FELT LIKE MUSCLE STRAIN; This spontaneous report received from a patient concerned a male of unspecified age. The patient''s height, and weight were not reported. No past medical history or concurrent conditions were reported. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 042A21A, expiry: unknown) dose was not reported, 1 total, administered on 03-APR-2021 to left arm for prophylactic vaccination. No concomitant medications were reported. On 25-MAY-2021, the patient experienced left calf pain for a couple of days which the patient believed that it was a muscle strain and was increasingly painful. On 28-MAY-2021, the patient was sent to the hospital to do an ultrasound and the HCP (health care professional) suggested for the patient to go to the emergency room visit because the patient had two blood clots. On 28-MAY-2021, the patient was hospitalized and was discharged on 28-MAY-2021. The patient was hospitalized for 1 day. Laboratory data included: Diagnostic ultrasound (NR: not provided) 2 blood clots. On the day of reporting, it was informed that the patient was taking treatment medications (dates unspecified) which included: Eliquis (apixaban) for blood clots. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient was recovering from two blood clot, and the outcome of left calf pain for a couple of days/felt like muscle strain was not reported. Write-up: Systemic: Allergic: Itch Generalized-Medium, Systemic: Allergic: Rash Generalized-Medium, Additional Details: 45 minutes after patient recieved the vaccine , she came back complaining aboout generalized mild to mederate rash and redness specially in the upper extremeties. advised patient that a call to 911 will be required and possible emergency visit. patient approved , i called 911 and while on the phone briefing the operator i was advised not to give the paient any medications or food or drinks and the emt will be on the way now
