Diagnostic Lab Data: I went to the ER on June 9th 2021 with all of these side effects. They were in no hurry to see me. I sat there for 3 hours before they seen me. The triage nurse said I was one of the lucky ones.... The Dr. Gave me a shot of toradol (which only helped with the fever) and tested me for covid19. And sent me home. Test results were negative.

Write-up: Background:41-year-old male reported that he awakened with right lower leg pain intermittently since approx. May 10 2021. Woke on June 10 with increased pain and swelling to RLE. He tried to "run it out" but the pain worsened. In the ER he was noted to have elevated D-Dimer- 1980, ESR of 27, labs otherwise normal. Ultrasound showed DVT. He was started on hydrocodone-APAP and Xarelto. Returned on 11 June with SOB. PE was confirmed. Admitted to ICU Note today states continued swelling and pain to RLE. Considering transfer to hospital for vascular consultation and further management.

Current Illness: A month prior, I had ear infections in both ears that took almost a month to recover from.
Preexisting Conditions: Migraines with no aura, asthma, costochondritis, hiatal hernia, cyst in left ovary and long-term pain in lower back (L5/S1) due to car accident in 2010 were I was hit while crossing a side walk.
Allergies: Doxycycline, cigarettes, bug bites and fur.
Diagnostic Lab Data: 4/13: ER -Influenza A/B & SARS-COV-2 PCR combo for rapid response lab -CBC w/ Diff Blood -Comprehensive Metabolic Panel Results : Normal Diagnosis: Weakness. Need further consult and MRI. 5/12: ER -US Venous Doppler Lower Extremity Bilateral Results: Normal. Diagnosis: Potential Baker''s Cyst or Meniscus Tear. Need MRI. 5/13: Radiology -MRI of full cervical spine with and w/o contrast Results: Normal Diagnosis: Need further consult. Waiting on further neurology consults and referral for nerve conduction test. Need MRI for left knee to double check diagnosis.

Write-up: 4/8: Immediate injection site numbness and dizziness. As day wore on, I felt fatigued and later developed a fever ranging around 100.5?F. Throughout the night, I had body chills, muscle and joint pain as well as skin sensitivity. 4/9: Fever ranged from 100.5?F to 101.1?F. Dizziness, fatigue, headache, chills, muscle and joint pain, skin sensitivity and nausea. 4/10: Fever ranged from 99.8?F to 100?F. Dizziness, fatigue, headache, muscle and joint pain with mild nausea. 4/11: Fever ended. Mild dizziness, fatigue, muscle and joint pain. Around mid-day, both arms went numb with tingling/prickling pain from shoulders to fingertips. Could not open and close hands well. Could not hold, grasp, text or type/weakness in hands. Pain: 7-8/10. 4/12: Severe numbness. Noticing pain radiating from behind both elbows and going up to the shoulders as well as down to fingertips. Weakness in hands. 4/13: Went to school clinic and referred to ER for testing. Was given ibuprofen and told to take OTC painkillers. 4/19: Issues with insurance/unable to schedule follow-up with neurology in home town due to insurance based in nearby city but I am unable to drive and school is still in session. Telehealth call with PCP to go over options. Left arm is slightly improving but still going numb/tingly/prickly occasionally with pain radiating from behind elbow. Pain: 5-6/10. No improvement in right arm with pain and numbness being constant. Noticing slight pain behind left knee. Pain: 3/10. Purchased hand brace and arm sling for right arm. Taking Advil but noticing little to no relief. Use of warm and cold compresses as well as massage instead to help alleviate pain. 4/29: Attempted performing everyday tasks and school assignments for ceramics. Exasperated both arms. Severe numbness and tingly/prickly pain. Pain: 8-9/10. Left knee pain has not dissipated/becoming more and more noticeable. Pain: 3-4/10. 5/4: Intense pain decreased back to usual pain levels. Noticed swelling behind left knee and pain going down to calf muscle. 5/6: Appt w/ school clinic again. Referred to ortho doctor on 5/13 and prescribed Celecoxib 200 mg. 5/12: Appt w/ school clinic. Saw ortho doc. Diagnosed with potential Baker''s Cyst. Sent to ER to double check for blood clots. There was none. Diagnosis is potential Baker''s Cyst or Meniscus Tears. Was prescribed Diclofenac Sodium 1% Gel and told to wrap knee. 5/13: MRI of full cervical spine with and without contrast done at Radiology at another city. 5/26: Left arm is almost completely recovered. Pain: 2/10. Still slight pain behind left elbow. If overused, however, more pain and numbness. Right arm is constantly in pain but it is not as severe as previous month. Pain is 6/10 and runs from shoulder to fingertips but more prevalent around shoulders to behind left elbow and wrist. Shoulder pain feels like burning pain instead of usual tingly/prickly pain like the rest of the arm. Left knee gives out randomly and I feel pain from behind knee to calf whenever standing for long periods of time or walking for more than 20-30 minutes. Pain 2-3/10. 6/1: Telehealth appointment with PCP to go over test results. Referred for neuro consult. Was prescribed medication for neuropathy but currently meds are with mother in a city. Unable to drive from city to city to pick it up. 6/8: Telehealth appointment with neuro consult. Waiting on referral for nerve conduction study. Pain has not changed for right arm or behind left knee. Decreased walking due to more frequent moments of knee giving out. Overall, due to constant arm pain in dominant arm (right arm), I am unable to do most activities without assistance such as cooking, cleaning or driving. Right arm/shoulder get exhausted fairly quickly or nerves become inflamed when attempting to use it more. Shoulder feels like it''s burning slightly. Sometimes hand/arm will shake and feel a spike of pain when overused. Must do things more with left arm and can not walk far without falling down randomly due to left knee giving out.

Write-up: Headache, fever, chills, migraine, nausea, vomiting, stomach cramps, muscle ache, muscle spasms... got vaccine at 10:00 am-symptoms appeared around 6:00 pm with tiredness/uncomfortableness/achyness... ramped around over the next few hours, becoming severe around 10:00 pm. At about 11:00 pm, Ambulance was dispatched and patient was brought to ER at 11:30 pm under the care of doctor. Patient received 1000 mL IV fluid plus Zofran, Reglan, IV Tylenol and Toradol over the course of 4-5 hours. Patient was then released around 5:00 am on the morning of 3/22. Patient felt much better and was stable and improved.

Write-up: Received TC from patient''s grandmother today. She stated that about 6 weeks after pt received her J&J shot she started having trouble hearing out of her left ear. On memorial day her grandmother took her to a walk in clinic and they "washed" her ear out and retrieved a small amount of ear wax. She continued to have trouble and her hearing continued to deteriorate. She was then Taken to center where they did a hearing test per grandmother report and were able to determine that pt had a "dead left ear". She also underwent an MRI and a brain scan to rule out a brain tumor. Pt''s grandmother was prompted by Medicaid to call to report that she had been given vaccine and then had this reaction.

Write-up: I had no regular symptoms, but a few days later I sat down in my chair and it felt like someone shot me in the stomach, I took oxycodone, it kept getting worse and I dealt with it for a week. I started getting pain in my kidney and went to the ER because of the pain and the pain level on a scale of 1-10 was a 8-9 at all times. I couldn''t'' breathe and was rushed to the hospital. I have portal vein thrombosis and have three blood clots in my stomach. I tried to figure out what to do to relieve the pain, I couldn''t eat or during and was on IV''s. I was in so much pain to even eat a cracker, they kept me at the hospital. While in the hospital they did a CAT scan to see if it was diverticulitis and found out that I had 3 blood clots, they also did the dye or contrast. I spent days in the hospital and was transferred to the hospitals skilled nursing facility so that they can watch me because of the clots. I was told that the blood clots could burst in the vein.

Write-up: Eight hours after the vaccine I got a Migraine and my Asthma kicked up. I had achy joints and muscles and a slight fever. I had a rash on my shoulders, ankles and forearms. It was little spots about six inches. Those symptoms lasted for 36 hours. After that point I had very low Oxygen levels and my Asthma was very severe. My Nebulizer and rescue inhaler were not very effective. It has been almost nine weeks and my Oxygen levels are still lower than normal at 96 or 97. Before the vaccine my Asthma was controlled and I rarely used the rescue inhaler. I use my rescue inhaler once daily. I am using my Nebulizer once daily. Immediately after the vaccine I was using my Nebulizer four times daily. It is still not near the level of control I had before the vaccine.

Write-up: On or about April 28, I began feeling what felt like a charlie horse no bigger than the size of a fingertip behind my right leg knee. It came and went until it was steady about a week later. In the following weeks, the feeling changed to a burning or jolting pain and spread from the knee both upward and downward the right leg. Because the event on 4/28 was so close to the 3-week mark after my vaccine and because it was strange but not painful, I didn''t report or associate it with the vaccine. As it grew, I became more concerned that it wouldn''t go away on its own. Around the week of May 24th, the pain spread down to the ankle and up to my thigh becoming more painful over the days. After speaking w/ nurse line and my Pharmacist, I drove myself to the ER on June 5th. A sonogram revealed an engorged main vein but no clots and a CT scan revealed no evidence of Fiskars. I was released with the advice to take Tylenol/Ibuprofen and to follow up with the vascular surgeon. By June 7th, I was unable to get out of bed drinking water and taking both meds to relieve pain. On 6/8, I called 911 and went back to ER. I had overdosed on aspirin and was treated for 2 days. An MRI revealed a bulging herniated disk. I am getting physical therapy and seeing Dr. on July 15th who will be the best contact after that date.

Write-up: Left arm started hurting. It hurt for 2 days. Then my right and left arm started hurting. this went on for another 2 days. then both my arms ,across my back and both hands went numb. Initial information received on 07-JUN-2021 was processed together with additional information received via 2 telephone calls to and from the patient''s husband on 11-JUN-2021. The patient''s height and weight were not reported. No past medical history or concurrent conditions were reported. The patient had no family history of any blood/clotting disorders and that she did not travel anywhere prior to receiving the vaccine. The patient received COVID-19 vaccine Ad26.CoV2.S (suspension for injection, route of administration not reported, batch number unknown), dose not reported, administered once in total on 13-MAY-2021 for prophylactic vaccination. The batch number was not reported and has been requested. On an unspecified date, the patient experienced feeling dizzy, and had no energy for anything. On 14-MAY-2021, the patient started feeling confused, her mouth started feeling like it was drawing, and her speech started slurring. On 28-MAY-2021 she went to bed and got up and she couldn''t feel her left side, with paralysis. She was also drooling and her husband took her to the hospital. The patient was flown by helicopter to another hospital. During the telephone call to the reporter, he reported that his wife''s laboratory results were all within normal ranges at the second hospital, including her platelet count at 189,000 (units and normal range not reported). On 29-MAY-2021, a magnetic resonance imaging (MRI) scan of her brain revealed a 12 mm blood clot in the thalamus. The patient was treated with Aspirin (acetylsalicylic acid) and Plavix (clopidogrel bisulfate). It was reported that the clot could not be cleared before she was discharged after 4 days hospitalization on 31-MAY-2021. As of 07-JUN-2021, the patient reported being highly agitated and could not hear out of her left ear, had no feeling or strength on her left side and was very unsteady walking. As of 11-JUN-2021, the patient''s husband reported that the patient was still experiencing weakness of the left side of her body with no energy for anything (reported as being very active prior to vaccination), trouble walking, feeling dizzy sometimes, trouble hearing with her left ear, was very agitated and was frustrated that these events had happened to her. However, a relationship with Janssen Covid-19 vaccine cannot be ruled out and thus the relationship is considered indeterminate.

Write-up: Several hours after the vaccine my head started to itch, the next day I had a full body rash and my skin was red and painful. My underarms, genital area, head, and eyebrow area was severely itchy. I went to the hospital where they gave me a 12 day steroid taper, steroids in the hospital and monitored me until my skin calmed down. They also gave me hydroxozine and Pepcid. Two days after the steroid taper was over the rash returned and my primary care gave me 7 days of steroids. The day that ended I felt itching in my head and eyebrows and my skin began peeling, and I saw a dermatologist that day who prescribed a 21 day steroid taper. I have just finished this medication today. I have missed work due to symptoms. The second time the rash came back my face looked like I had shingles and was painful to the touch.

Write-up: Extremely Severe Headache, Fever, Body Ache during the first 24 hours after vaccine injection. Extreme mental fog during 24-48 hours after vaccine injection. After initial 48 hours after vaccine injection, experienced tightness in chest with a slight bias to left side. Also still experiencing very faint feeling of headache since injection. This seems to cause slight mental fog and has remained as of writing this 2 weeks after injection. At 72 hours after injection, tightness or pressure moved across entire upper chest collar area, and down left arm and up left side of head behind left ear and into the back of the head, and then finally to the top of the head. Write-up: initial - 20 minutes after shot - 1 quick shot of vertigo but I was already in the car. Next day - headache, tired, low grade fever lasted 1 day. On March 30 & 31 I would have massive nausea. On 4/2 after dinner I started to have chest pain. This lasted into the night and I had cold sweats, nausea, vertigo and went to the ER. CTScan w/contrast was done, chest Xray, EKG was adnormal. Was admitted to the cardiac unit for observation. Was there 24 hours. On 4/4, I was still having the chest pain, went to another cardiologist where I had another EKG, Stress test, blood work. Everything looked good, couldn''t figure out why I was having chest pain. Echo was scheduled. The night of 4/6 the vertigo increased drastically where I couldn''t close my eyes and go to sleep, went to the dr on 4/7 and that is when it was thought that it was tied to the vaccine. Was put on meclazine for the vertigo as needed. It was then that the anxiety and depression started. By body broke out in Hives, was put on allergy medicine to help with those. Unexplained bruising was occurring on my legs. Was put on a Mediterranean diet which is supposed to help with inflammation. Stopped drinking caffeine and alcohol. The vertigo, anxiety and depression continues. I had an MRI of my brain to see if there was anything showing. Bloodwork was done. Started to experience breast pain, went to my dr, had a mammogram and ultrasound to see if anything was there. Lymph nodes are still swollen, have to go back in 6 weeks. Went to an ENT for the vertigo and he found some of the crystals in my ears, I''ve never had that before. On 4/29 I had a hair appt where 1 minute after she put the hair dye on my hair, I experienced more vertigo. Still experience the depression and anxiety, hoping that subsides very soon.

Write-up: Within 30 minutes after the injection, my face became flush and my heatrate was racing. I went back into the clinic where I received the injection and was checked by EMT''s. I was experiencing chest pain and my bloodpressure was extremely high (195/125) An EKG was done which showed irregularities and I was given Baby Asprin and Nitrogylicern. I was then transported by ambulance to Hospital and kept overnight. Several Tests were conducted including EKG''s, Ultrasound, Nuclear stress test, and Enzyme Test to determine if I had a heart attack. In the end, all tests were negative and I was released from the hospital the following evening (8:00 pm). I was extremely fatigued and weak for the next two weeks and eventually recovered. The chest pain was determined to be a pulled chest muscle wwas exacerbated by the high blood pressure.

Write-up: Six weeks after the vaccine I started to get headaches. It is a dull headaches at the back of my head where my neck connect. I have not been able to find any medication that helps. Write-up: On April 06,2021 I took the Johnson and Johnson vaccine the next day I threw up and my leg would fall asleep I advised my doctor she sent me to the emergency room I went to the hospital. They did a ultrasound they didn?t find and blood clots I also advised them that I had been having headaches and they told me take over the counter pills. The head aches continued I was with the pain for a month in half my doctor sent me for a MRI and to a neurologist. The headaches still continue finally my neurologist sent me to the hospital. They conducted MRI MRV MRA I was admitted to the hospital for 14 days I haven?t been able to return to work I am not 100% I am on blood thinners. They ended up finding a blood clot on the right side of my head I am still not able to return to work still having headaches and still having to go to follow ups.

Write-up: On the morning of Monday April 19, 2021, 1 1/2 days from receiving the Johnson & Johnson shot, I walked upstairs after coming down for breakfast and felt something off with my heart and I felt my heartbeat and noticed that I was in AFib. My heart had an irregular heart beat. I was taken by ambulance to the hospital where they had to shock my heart to get it back to a normal rhythm. They were able to get it back to a normal rhythm with one shock. Since then I have still noticed more heart palpitations, but have been alright since. I am a healthy 32 year old (now 33), with no health issues.

Write-up: Please note patient did not receive vaccine at this organization. Submitting to VAERS following ED visit and admission. 23 y.o. male with no previous past medical history presented 6/10 with complaints of dizziness, presyncope and syncopal episodes. Initial fainting episode was in January 2021 upon receiving sutures the laceration on his his right index finger. At the time he reports that although he passed out he did not really lose consciousness as he was aware of his surroundings. Things seem to go fine for a while until the day after he received his J &J vaccine in April 28, 2021. Reports having the usual post vaccine symptoms such as myalgias and malaise but the following day while at the barbershop he reports having another syncopal episode. He reports a prodromal phase that includes shortness of breath, numbness of the lips and a generalized warm feeling. During this episode he denied any loss of consciousness, as he was able to unlock his phone and respond to the people around him. He denies any myoclonic movements or jerks. He denies any postictal symptoms. Patient has been seen in the ED 3 times within the last week. On 6/10/2021 while at the base clinic, he reports feeling like he was about to pass out and was brought to the emergency department for further evaluation. He denies any nausea, vomiting, headaches. He endorses some chest pain when he lays down flat that is relieved by leaning forward. He denies any recent illness. Denies any new medications or changes to his diet or hydration status. The day prior to admission, his ED visit for weakness and presyncope occurred shortly after PT eval at the base. He felt very lightheaded and he attributes that to not eating and drinking prior to the PT eval. Work-up in the ED included EKG which showed ST elevation in leads V3 and V4 in a biphasic pattern. CT chest which showed no PE or consolidation pneumonia or pleural effusion. CMP was remarkable for potassium of 3.4 serum bicarbonate of 21. CBC was normal. CK was slightly elevated at 913. Troponin was negative. Lactic acid was 1.9. UA with no concern for infectious process. UDS was negative. Due to EKG findings reported above, patient was discussed with cardiologist who recommended that patient be admitted for observation. Upon further discussion with patient, it was clear that his chest discomfort improved when leaning forward, and his EKG was consistent with diffuse ST elevation. With a clinical diagnosis of acute idiopathic pericarditis, patient was initiated on colchicine 0.6 mg twice daily and ibuprofen 800 mg 3 times daily. He is to continue his colchicine therapy for 3 months and his ibuprofen therapy for 1 to 2 weeks with a gradual de-escalation of that ibuprofen by 200 mg/week. He has follow-up with cardiology scheduled. Cardiology mentioned in their notes that they would obtain an echocardiogram to rule out structural heart disease and recommended a 2-week event monitor upon discharge with a 3-week follow-up with tilt table testing and exercise stress testing. Cardiology had high suspicion for possible vasovagal. Echocardiogram was unremarkable without pericardial effusion.

Write-up: I experienced a week later after receiving my J&J vaccine, inflammation to my legs and hands, I have water retention, feel very sick, have kidney pain which I did not have before getting the J&J vaccine. When I was getting my vaccine, I told the nurse that was putting the injection, that I have history of high blood pressure but I do not take any medicine, the nurse said it was fine that I should be okay. I also experienced swollenness to my face. I went to the ER on 05-27-2021 due to how I was feeling. AT the ER they performed lab work and did an echocardiogram. I recently got a type of medical insurance. I am also not working at this time due to my symptoms of retaining water in my body, where I get so inflamed to my legs and hands and face. At the ER I was told I needed to see a PCP but I do not have one at the moment since I did not have insurance when I received my J&J vaccine. Write-up: Four days after the vaccine I lost feeling on the left side of my face and tongue. I was also having trouble finding words. I visited my doctor and they assessed me visually. I was also getting Migraine headaches. I had ocular issues on the left side with the headaches which were a daily event. They were consistent for the whole day. I went back the next day after being called and I went to the ER. My D-Dimer was elevated but my other tests were normal. I felt very bad, and I was having flashes of pain across my body. Those continued for four to five weeks. The Migraines also persisted during that time. Over the counter Tylenol and Advil did not work. A growth in my right thigh was discovered during that time. It was about the size of an Almond. It was identified as a Hematoma. I feel better now after eight weeks and I am starting to work out again. I am scheduled for a follow up on the Hematoma in my thigh.

Write-up: Upon receiving her first shot of Janssen COVID-19 vaccine on April 7, 2021 at a clinic, the patient experienced shortness of breath, body numbness and near syncope. The RN called 911 and patient was taken to the hospital ER. She was discharged after 3-4 hours, some shortness of breath persisted. On April 9, 2021 she experienced increase shortness of breath mid-day and went to the emergency room and was there for 6-7 hours and discharged without diagnosis. On April 13, 2021 in the evening, she had multiple symptoms including shortness of breath, headache, abdominal pain and leg pain. She was watching TV where there were talking about cerebral venous sinus thrombosis associated with the J&J vaccine and cautioned anyone with headache to see MD but she could not get an appointment at her regular clinic. Her husband then took her to the ER again and head CT was negative. She is still not feeling 100% with some shortness of breath on exertion.

Write-up: Patient came to Pharmacy today for his J&J vaccine along with his wife who also received her vaccine as well. After I administered the vaccine, the patient got light headed and I asked him if he would like to lay down. Before he could respond, the patient collapsed off his chair hitting the side of the plastic trash bin in our patient care room. I caught him before he hit the ground. I asked if he could hear me after saying his name a few times, no response. I assessed his subjective vitals and could tell he stopped breathing. Called for Pharmacy resident to grab and administer an epi pen. Patient responded within 30 seconds of epi pen administration and started breathing again. Proceeded to call emergency services, first responders arrived within 5 minutes and administered IV fluids and oral ondansetron. Patient left with the ambulance and will follow up tomorrow to make sure everything is ok.

Write-up: Patient presented to ED on 6/14/21 with 4 day history of cough, malaise and 1 day history of right flank/pleuritic pain. He was found to have a left leg DVT and bilateral pulmonary emboli. He was started on a heparin drip which he tolerated well. He was transitioned from heparin to Eliquis. He has no family or personal history nor other risk factors for DVT or PE; no long travel, no surgery, no immobilization. Hypercoag workup was initiated. He continued to have right sided pleuritic chest pain which improved during his stay. There was some question of possible community-acquired pneumonia, which he was empirically treated with CTX and azithromycin. He had normal WBC, low procalcitonin, and no fever. Patient was discharged home on 6/15/21 on Eliquis with follow up scheduled with primary care provider.

Write-up: describes symptoms beginning 11 days later as swelling to bilateral medial elbows, without pruritis or erythema. The swelling on the right arm was associated with digital paresthesias of the 4th and 5th digits. About two days had macular erythema and pruritis on medial arms, trunk, back, legs, starting small and coalesced to grew, accompanied by intense pruritis and sought care in the ED x 2. With complaint of arm swelling, had negative DX, determined not to be a venous thrombotic disorder. On 22 Apr patient presented again to clinic and diagnosed with acute generalized urticaria. Reported rash lasted less than 24 hrs. No airway involvement. Arm swelling lasted about 1 wk. Did report low grade fever and slight site soreness the day of vaccine that lasted less than 24 hrs. Symptoms: URTICARIA, UNSPECIFIED(L509), EDEMA, UNSPECIFIED(R609).

Write-up: On Wed May 19 at approx 11:45 am my son was given the Johnson and Johnson vaccine as he was due to go to work on June 1st however patient had a seizure as a reaction to vaccine he was taken to vaccine site ER by ambulance where he had a second seizure around 3:55 pm he was released from ER after several hours and was told to follow up with PC within 2-3 days. As a result of reaction my son was disqualified from going to work on 6/1, due to seizure -this changed his whole life plans. To add his drivers license was medically suspended as well. Which we are now working having to submit medical test results in order clear him of having medical conditions.

Write-up: Had her Johnson and Johnson covid vaccine on 4/8/21 LMP 4/14/21 Pt began spotting 5/9/21 which was day 25 of her cycle Day 2-5 very heavy bleeding Day 5 passed clot 11 days of bleeding Pregnancy test positive on 5/19/21 and 5/20/21 Presented to ED w/ intermittent RLQ abdominal pain and vaginal bleeding and spotting since 5/9 with positive hCG, 52 on 5/20 and 209 on 5/27 with OB ultrasound on May 28 that showed no IUP and small hyperechoic nodule adjacent to the right ovary that could represent an ectopic pregnancy and patient opted to avoid methotrexate treatment at the time. Returned to the ED on 5/30 with ongoing symptoms of RLQ cramping but no vaginal bleeding. OB ultrasound shows right adnexal structure measuring 2.6 cm, larger than structure 2 days ago. Write-up: fWithin the first 24 hours I had a low grade fever, headache, and chills. About 35 days later I noticed serve pain in my leg and a skin rash which was diagnosed as shlinges.

Write-up: 2 weeks after the shot I had a scary night where I had severe chest pain. I sat up for a few hours hoping I wasn''t having a heart attack and taking water and tums. I have never had chest pain before or since. I was very concerned and wondered if it was the vaccine. I didn''t have anything unusual to eat or drink. I was happy when it finally resolved that same night. I also experienced Bell''s Palsy 2 months after the vaccine. (~June 1st) I have never experienced Bell''s Palsy. I am not sure if either of these events were related to the vaccine, but I have no intention of getting a yearly covid or flu vaccine now that I have Bell''s Palsy. It is terrible, and I''m not taking ANY chance of getting this again even if that means I get the flu and covid yearly. I placed my trust in science, and not I am not sure I made the right choice. Although I hope my facial movement and ability to use my lips and face come back, I realize that some never recover.

Write-up: FELT PARALYSED; DIAGNOSED WITH GUILLEN BARRE SYNDROME; INJURED RIGHT FOOT; NUMBNESS IN FACE; LEFT LEG WEAKNESS; SWELLING IN LEGS; TIREDNESS; FEELINGS OF WEAKNESS; NUMBNESS IN LEFT HAND; This spontaneous report received from a consumer concerned a 46 year old female. The patient''s weight was 113 kilograms, and height was 66 inches. The patient''s pre-existing medical conditions included the patient had no known allergies. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, intramuscular, batch number: 201A21A, and expiry: 07-AUG-2021) dose was not reported, administered on 01-MAY-2021 to left arm for prophylactic vaccination. Concomitant medications included immunoglobulin human normal for guillen barre syndrome. On 01-MAY-2021, the subject experienced feelings of weakness, numbness in left hand. On 28-MAY-2021, the subject experienced tiredness. On 29-MAY-2021, the subject experienced left leg weakness and swelling in legs. On 02-JUN-2021, the subject experienced numbness in face. On 09-JUN-2021, the subject experienced diagnosed with guillen barre syndrome, and was hospitalized. On an unspecified date, the subject experienced injured right foot, and felt paralysed, and was hospitalized on 08-Jun-2021. On 01-MAY-2021, the patient developed numbness in her hand on the left side and felt weak. The patient travelled overseas and had a connection to a country and her husband stated that she started to have swelling in her legs. When she reached her destination, she was admitted to the hospital and was diagnosed with Guillen Barre Syndrome by the neurologist per her husband''s recollection. In the hospital she was given Immunoglobulin G for 5-7 days and she did regain some feeling in her legs. Based on patient''s husband recollection this may have occurred "maybe tired due to travelling," but when she arrived to the country that was when her left leg weak and requested a wheelchair. The patient then injured her "right foot" in another country, but "she didn''t feel it and she requested help because she couldn''t walk." per her husband. After she reached her final destination, she felt tired and then she completely fell. It was reported that she could not "control herself and felt paralyzed." On 08-JUN-2021, she entered the hospital and was still in hospital at the time of this report. Her husband states that he "does not know any side effect from any vaccine" that caused this in the past. Lastly, he mentioned "she may have numbness in her face when she reached original country". He also stated that the on 04-JUN-2021, doctors conducted a computerized tomography scan and did not find a blood clot. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The outcome of the feelings of weakness, swelling in legs, diagnosed with guillen barre syndrome, numbness in left hand, tiredness, left leg weakness, numbness in face, injured right foot and felt paralysed was not reported. This report was serious (Hospitalization Caused / Prolonged, and Other Medically Important Condition).; Sender''s Comments: V0: This is a spontaneous report received from consumer concerning a 46 year old female, unknown ethnicity, who experienced weakness and numbness of the left hand on the same day after receiving the Janssen Covid-19 vaccine and was diagnosed with Guillain-Barre Syndrome 39 days after. Patient''s height was 66 inches and weight was 113 kilograms. Past medical, previous surgery, current illnesses and medications taken as well as history of allergy, smoking, drug abuse and alcohol abuse were not reported. On the same day as the vaccine was given, patient developed weakness and numbness on the left hand. Patient still travelled. Twenty-eight (28) days post vaccine, patient experienced left leg weakness and swelling on the legs. Patient was noted to feel tired the day before. Thirty-two (32) days post vaccine, patient developed numbness on the face. Computerized Tomography (CT) scan was done and there was no blood clot found. Thirty-eight (38) days post vaccine, patient felt ''paralyzed'' and injured her right foot. Patient was brought to the hospital the following day and was subsequently admitted. Patient was diagnosed with Guillain-Barre Syndrome and was given IVIG. Information regarding other potential etiologies was insufficient. Considering the temporal relationship, the event is assessed to have an indeterminate relationship with vaccination.

Write-up: SEIZURE LIKE ACTIVITY; BP 87/64; DIAPHORETIC; PULSE RATE 50; NOT FEELING WELL; This spontaneous report received from a health care professional concerned a 49 year old male. The patient''s height, and weight were not reported. The patient''s pre-existing medical conditions included patient had no known allergies. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 204A21A, expiry: unknown) dose was not reported, 1 total, administered on 12-JUN-2021 for prophylactic vaccination. No concomitant medications were reported. On 12-JUN-2021, health care professional (HCP) reported that patient received Janssen Covid-19 vaccine. About 2.5-3 minutes after receiving the injection, the patient began having seizure like activity. HCP described that as full postured seizure like in the upper extremities and chest, and the eyes had been nystagmus. This activity lasted approximately 45 seconds. Emergency medical services (EMS) staff was on the scene and when they came to, the patient self-reported that he was not feeling well and the HCP described him as diaphoretic and was appearing shaky. EMS took his vitals initially (Blood Pressure, BP 87/64 mmHg, Pulse rate 50 unit unspecified and oxygen saturation 100 percentage). Vitals at 5 Min (Blood Pressure, BP 124/80 mmHg, Pulse rate 76 unit unspecified, oxygen saturation 100 percentage and blood glucose level 125 unit unspecified). An ambulance was called to transport the patient to the ER, and upon arrival vitals were taken again at about 10 Min. Post-ictal (Blood Pressure, BP 115/76 mmHg and pulse rate 76 unit unspecified). Patient was taken to the ER for further evaluation. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The outcome of the seizure like activity, not feeling well, BP 87/64, pulse rate 50 and diaphoretic was not reported.
