Write-up: Pt came in for the Jansen COVID shot around 11:00, after receiving it he went to the waiting area and a few minutes later seemed to momentarily pass out in his chair, his wife called us over and Pt described feeling warm and dizzy. He drank some water and ate a candy bar over the next few minutes. Pt continued to feel dizzy and was sweating. After a few minutes Pt appeared to continue having extreme syncope and we called 911. Paramedics arrived a few minutes later.

Write-up: Pt''s husband came in today (6/26/2021) and reported that ten days after receiving the shot (6/15/21), his wife (the patient) started feeling weak and felt numbness in her legs. She is currently in the ICU being treated. They believe it may have been because of the shot. Blood clot has not been confirmed.

Write-up: tongue swelling that patient felt was onset of anaphylaxis, began about 5 minutes after vaccine was given, pt was given one dose of EpiPen and went to ER where they received IV steroids, benadryl, pepcid and observed for 45 minutes. Upon discharge they were prescribed a 5 day course of prednisone

Write-up: RN to J&J vaccine about 3-5 min after delivery. Assuming Vasovagal response. His brother reports that patient was sitting there and his eyes rolled back in his head and he fell off his chair and hit his head/right eye. Employee got out there and he was a bit combative and saying "whats going on, whats going on". I asked him to sit down and employee called 911. We put pressure on his eye area to stop the bleeding. He was sweating, shakey, pale, and nauseous. Paramedics arrived quickly and eventually took him away on a strecher. Update: mom came back a couple of hours later to thank us and let us know that he is home and resting. He did get 3 stiches by his eye and the reason they took him via strecher was because his blood pressure was staying so low while they were here.

Write-up: The first day I?d say about 3-4 hours after getting the vaccine I noticed a headache, was also fatigue, and slightly dizzy (pretty common side effects.) as the days went on I noticed that the headaches would not go away, they did with taking Ibuprofen and Tylenol, but would return 4 hrs after taking. Monday (6-14-21) I really noticed that my neck was stiff feeling, I was getting really bad random bruises on my legs, I would get really ?airy? feeling in my head, dizzy, my heart would start racing, and my hands would get really sweaty, I also felt hot even in colder temperatures. Made an appointment Monday (6-14-21) to get bruises checked out thinking it was from something unrelated, Tuesday (6-15-21) I made an appointment to make sure I didn?t have the blood clot they did a full work up, and noticed that my blood sugar was abnormally low (58) I do not have a history myself of diabetes or ever been suspected of being prediabetic. Tuesday (6-15-21) was definitely the worst of it. Later on in the day while I was working I was seeing black spots out of the corners of my eyes, I could hardly stand, I could hardly breathe (felt like a horrible panic attack,) my heart was beating so fast I felt like it was going to burst, my hands were extremely sweaty and I felt like my body was shutting down, I had a horrible headache, I could barely drive, and when my mom was taking me to the ER, I could barely lift my arms up. Went home as my blood sugars were then fine and I was feeling okay. From that day forward I noticed that at 6:30pm every night I would get these same side effects. Saturday (6-19-21) the pattern finally broke, I no longer felt like I was dying (I wish I was exaggerating.)

Diagnostic Lab Data: Blood tests and MRI''s on back and head - all normal. MRI on back did show a few degenerative disks, but normal for my age. MRI on head showed incidental meningioma.

Write-up: Intense headaches, back and hip pain and right arm pain. The pain got so bad and lasted for a few months ultimately landing me in the Emergency Room for 3 visits in 2 days. I also had Bells Palsy. They were pumping me full of morphine and it did not take away the pain. Treatment was pain management - vicodin and prednisone for the bells palsy. Severe pain is gone , but now have joint pain, weird skin sensations and dealing with fatigue and some depression. This is vaccine related - I had no issues with any of this prior to the shot.

Current Illness: I fainted on 04/02/21 about five days after the J&J vaccine. Fainted in the gym while exercising, was dehydrated. I never fainted before in my life. Fainted again on 05/23/21 while walking the dog (i.e. not doing strenuous activity). Broke out in full body hives, then fainted on the sidewalk. Needed stitches in my head both times from fainting.
Preexisting Conditions: Occasional gout - I used to take Allopurinol (300mg) daily but recently stopped taking at the recommendation of my primary care physician.

Diagnostic Lab Data: Both times I had MRI, EKG and blood tests. Nothing irregular recorded.

Write-up: Fainted twice after receiving the vaccine. First time 5 days after receding the vaccine, the second time about 6 weeks later. First time I was dehydrated, and fainted at the gym while exercising. The second time I fainted while walking the dog. Both times I broke out in hives, the second time more severely (all over my body). Both times I fell, hit head head and had to get stitches or staples.

Write-up: Severe rhabdomyolysis with peak CK ~750,000, myoglobin $g5000. Severe life-threatening hyperkalemia, K up to ~7.5. Renal failure including ~2+ weeks anuria, still on hemodialysis 5 weeks post onset with uncertain prognosis. Respiratory (muscle) failure, on ventilator for ~18 days with endotracheal tube, followed by tracheostomy ~1 week. Hospitalized for 26 days including 19 days in Intensive Care Unit, ~11 days in medically induced coma. Loss of swallowing function requiring retraining. Profound diffuse muscle damage and weakness, ~20 lb. loss of muscle mass, initially unable to move arms, legs or support head. Will require extensive physical therapy to achieve ability to care for self and function somewhat independently.

Write-up: 69 YO female received one dose of J&J vaccination on 3/12/21. 4/8: Patient complaint of epigastric abdominal pain almost nightly since March 12 (coincided with her COVID vaccination) described as 10/10, crampy and without radiation. CT abdominal and pelvis with contrast showed infiltrating mass in the left hepatic lobe concerning for intrahepatic cholangiocarcinoma with reactive adenopathy. Additionally, left mid ovarian vein thrombus found. No thrombus in the renal veins or IVC. 4/28: Venous duplex demonstrated bilateral leg acute DVT. Eliquis was started. 5/7: Pt underwent laparoscopic biopsy/paracentesis. Eliquis was held 2 days prior to laparoscope...5/11: Pt presented to hospital with complaints of generalized weakness, fatigue and leg pain. CTPA showed bilateral PE. Pt received heparin drip and was switched back to Eliquis on discharge. Echocardiogram on 5/12 noted mobile round mass in RV junction, right atrial mass; presumed to be a thrombus. Pt additionally with elevated troponin, determined to likely demand ischemia related to PE; EKG without any acute changes. Discharged on 5/24. 5/27: Patient started on chemotherapy with gemcitabine/cisplatin for metastatic intrahepatic cholangiocarcinoma. 6/4: Presented to the ED again due to worsening dyspnea and home pulse ox readings in the 70s. CT chest showed moderate burden of bilateral obstructive pulmonary emboli with no overt RV strain. EKOS and clot retrieval was contraindicated d/t thrombocytopenia. Was noted to have demand ischemia, and transitioned from eliquis to heparin gtt. Vascular was consulted for eval of her ischemic right foot, and deemed that it needed amputation, but would be a high risk surgery. 6/8: Patient was transferred to this reporting institution for second opinion requested by the family. Upon arrival, patient was found to be obtunded and hypotensive requiring heated high flow nasal cannula and vasopressor with norepinephrine. Code status was deemed DNRCCA/Do not intubate per family. Bedside ultrasound was concerning for an LV mass. Cardiology and Vascular surgery were consulted and recommended that the foot be amputated as well, but requested further workup with ECHO and CTA chest/abdomen/pelvis to further evaluate the LV mass. During admission workup, she was incidentally found to be COVID PCR positive. She was started on DEXA-ARDS prednisone dosing, remdesivir, and placed in enhanced droplet isolation. Her hospital course was complicated by acute hypoxemic respiratory failure due to pulmonary emboli, COVID-19 infection, and underlying malignancy requiring heated high flow nasal cannula. Empiric cefepime was started to treat any underlying superimposed pneumonia. Due to extensive clot burden with limb ischemia, as well as + COVID, she was systemically anticoagulated with a heparin drip despite her thrombocytopenia. She was intermittently given platelet transfusions in order to keep platelet count $g30. TTE was obtained on 6/11 which showed large masses in right ventricle and right atrium, vegetations on mitral valve, aortic valve, and tricuspid valve, and probable mass in IVC. She also developed AKI with oliguria related to shock and critical illness. Medical oncology was consulted and she unfortunately was not a candidate for further chemotherapy at the time due to critical illness, as well as ischemic limb. (If further treatment was pursued, she would need a left leg amputation, then would require 4-6 weeks of recovery before additional chemotherapy could be given.) She slowly became more encephalopathic and had increased oxygen requirements, requiring continuous heated high flow nasal cannula. Goals of care were discussed with her family. Given her advanced cancer, it was decided to transition to comfort-focused care. Her code status was changed to DNRCC. Symptoms were controlled with IV pain and anxiety medication. She expired on 6/13/2021 at 0225.

Write-up: The evening of the vaccine I had severe chills, headache and I threw up. The next day the chills were gone but I still had a headache which I had to take off work, it lasted a week. One week after the shot I experienced shortness of breath and my Reynaud?s symptoms were bad. My fingers and toes were blue. I had leg pain and I was concerned for a blood clot. I returned to the ER a week later because I had shortness of breath and chest pressure. I continued to have sharp chest pain for four weeks. Diagnostic Lab Data: ER performed chest x rays, D-dimer test (to look for blood clots), complete blood count w/differentials and platelets, and heart enzyme test. Everything was normal. At a follow up appointment with family physician, I had slightly low WBC (3.6) and elevated Bilirubin (2.3). Over period of two months (Late April to June) we retested my blood count and WBC went from 3.3 to 3.4, Bilirubin went up to 3.5. Neutrophils in range of 1160-1560ish over that time. Doctor also ordered metabolic panel, liver panel, rheumatoid panel. Positive ANA test, 1:80 titer, and dense, fine, speckled pattern. ESR was within normal range. During most recent test, bilirubin was at 1.8. (My grandfather has Gilbert''s Syndrome and my mother also consistently has high bilirubin counts that fluctuate).

Write-up: About 11 days after receiving the shot, I started experiencing chest tightness/pain. The pain persisted for four days and I went to urgent care. Urgent Care referred me to ER to screen for blood clots. After returning from ER, I started feeling sharp pains in multiple places all over my body?hands, fingers, toes, feet, calves, shins, forearms, back. Chest pain also continued. The chest pain has subsided, but I still experience shooting pains in arms, legs, hands, feet, toes, fingers, and occasionally my back. Pain is a 3-4/10 on average, but persistent. Some days are better than others. Some days the pain is mostly in a forearm (sometimes both) and the next day it''s in a leg (sometimes both). Some pain seems to be in/along muscles, some in/along bones. It sort of feels like there''s a small electrical storm in my body. Movement does not seem to have any effect on the pain.

The patient''s weight was 123 pounds, and height was 67 inches. The patient''s concurrent conditions included sulfa allergy, nonsmoker, non alcoholic, and narrowing of artery in neck The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 205A21A, and expiry: 23-JUN-2021) 1 total dose, administered on 21-MAY-2021 around 11:00 in right deltoid for prophylactic vaccination. No concomitant medications were reported. On 26-MAY-2021, during the flight patient started experiencing symptoms, she had vertigo which lasted the entire trip and progressed ever since then. She believed that on 26-MAY-2021 injection might have speed up the process of narrowing of artery in neck. On 15-JUN-2021, she started experiencing nausea, stronger than it had ever been, she got tongue twisted, tingling on whole body from head to toe, because of this she had difficulty grasping items and sometimes sliping out of hands, difficulty sleeping for 3 days, difficulty swallowing. She mentioned she was not able to swallow at times and would need to regurgitate the food and re-swallow. On 15-JUN-2021, she also difficulty breathing, it was not continuous She would just need to take a time out and catch her breath, headache on the middle of the head all the way to the back of the head. It was intermittent. Lightheaded, balance was completely off and that balance made her nauseous, tightness in the throat related to the difficulties with breathing at time. She had to use her daughters nebulizer past weekend due to breathing issue, which was relieving and made her relax, for 2 nights, for a split sec, she could not talk or breathe, she had to remind herself to relax and breathe, she felt wasted and stoned all day. All the symptoms were just at once. Healthcare professional (HCP) was aware of symptoms. She went to the emergency room (ER) twice because of all the symptoms on 18-JUN-2021 and 21-JUN-2021. Bloodwork was done twice, no results reported. As per CAT Scan (Computerised tomogram), there was no blood clot. HCP wanted her to see neuro for her vein in her neck, as they saw a narrowing during the CAT scan. As per patient narrowing may have been there at an earlier age, but she never had any symptoms. As per patient vaccine shot made all the symptoms because never felt like this before, it was really odd. She felt perfectly fine before shot. Write-up: Chief Complaint Patient presents with o Shortness of Breath Chest Pain 36-year-old male without significant past medical history presents with concerns for vaccine reaction. The patient reports that he got the J&J vaccine yesterday, while at work today he developed some dizziness. He went home, while resting at home he developed some chest pressure, shortness of breath. He reports the symptoms have largely resolved at this time, he just feels mildly dyspneic at this time. No known fevers. No recent illness. Patient does report some anxiety related to the vaccine. Well-appearing 36-year-old male presents 1 day after J & J Covid vaccine. While he did experience some dizziness, dyspnea and chest pressures earlier, at this time the symptoms have resolved. He appears well, is breathing comfortably, stable vitals. Patient is PERC negative. Will check an EKG, but otherwise recommend symptomatic management at home. Differential diagnosis: Vaccine reaction, viral illness, arrhythmia Low concern for PE given that the patient is PERC negative EKG Interpretation: normal sinus rhythm, no ischemic changes ED Course and Disposition: Patient reassured and discharged home.

Write-up: On June 19, 2021 I woke up around 6:00 a.m. having extreme chest pains. I went to the Emergency Room to be treated. They did an EKG, blood work, and an ultrasound of my heart. My EKG was negative for a heart attack, but my blood work was positive because of my Troponins were elevated. They gave me some aspirin and the chest pain went away. I was admitted into the hospital where I stayed until the end of the day on June 21, 2021. On June 20, 2021 they continued to watch my vitals, took tons of blood for blood cultures and for pretty much every virus you can get along with urine samples as well. All of the test for any virus and the blood cultures were negative. The urinalysis and urine culture were negative as well. Once all test came back negative, the cardiologist ordered a Heart cath. The heart cath was performed on June 21, 2021 and there was no blockage in any of my arteries and everything was clear so I was discharged that afternoon from the hospital. I was diagnosed with Myocarditis or Pericarditis. The doctor prescribed me 25MG Captopril to take 3 times daily and 25 MG Metoprolol Tartrate to take 2 times daily. I have only experienced any other affects a few times since being released which was a burning sensation in my check. I spoke with the doctor and they told me it could be indigestion or heart burn.

Write-up: First noticed double vision and blurry vision while driving. A few days later, someone noticed that my left eye was not moving outward and along with my report of having double vision it was decided that I should go to the ER. Tests conducted included CT scan of head with and without contrast, MRI of head, eyes, and neck, ultrasound of neck, EKG, and blood work. Also had two separate series of tests performed by a neurologist. Everything came back normal. Was sent home and visited an ophthalmologist a couple of days later. Several tests were made and nothing was out of the ordinary, but doctor suggest a left 6th nerve palsy. Referred to neuro ophthalmologist. Appointment is still a few weeks away. Double vision has largely gone away as well as blurry vision. Left eye movement outward is still limited.

Write-up: Gross motor weakness distally in Left Upper Extremity within 1-2 minutes after receiving COVID 19 (Janseen) injection in left bicep muscle. Pain in biceps muscle with passive elbow flexion. Sensation intact to light touch across all limb. Motor function in Anterior Interosseus nerve, Posterior Interosseus Nerve and ulnar nerve intact in Left hand although weak (relative to normal contralateral limb). Vascular exam normal distally, with strong pulses in radial/ulnar arteries. Patient was evaluated by onsite physician and sent to the Emergency Room via EMS.

Write-up: First night, I had severe chills and sweats off and on throughout the night; including muscle and joint pain that was severe. Fatigue, joint pain and an occasional chill (better then the next day). Monday - Joint pain and occasional chills - those continued for the rest of the week and Wednesday I also had a migraine headache. Friday - severe migraine and vomiting and my doctor sent me to ER. Write-up: I received the Johnson and Johnson covid vaccine on 7 April 2021. Within 6 hours of receiving the vaccine, I experienced typical flu like symptoms. Though the majority of the symptoms subsided within 24 hours, I continued to experience headache symptoms for several days. On 12 April 2021, I woke up with a broken blood vessel in my left eye and red and black eyelid. The blood from broken blood vessel continued to get worse. I went to the ER to have tests run because the J&J vaccine was removed from use during this time.

Write-up: I came home from work on the 14th and I was freezing cold. I laid on my bed and bundled up in 5 0r 6 blankets even though it was 110 degrees outside. I got up and took a couple of Tylenol and went to sleep. Got up the next day and went to work and worked through my day and took my grandchildren somewhere and I got home and I was freezing and miserable. I took a couple Tylenol. On Wednesday, my husband said "This isn''t normal" and he took me to Urgent Care - and they tested for COVID it came back positive for COVID. Gave me a cough medicine prescription - but it was starting to upset my stomach. Had my husband to give me bananas and applesauce and when I eat that then I take the medication so it won''t upset my stomach. And I can''t breathe, I cough and have fevers and pain. I am pretty sure right now I have blood clots in my legs. I get up every once in a while to get something to eat and I''m really light headed and dizzy. Have to hold on to walls - don''t feel stable in walking. And then I lay down in bed again. I did get my doctor to call in a large dose of Zofran. I''ve got Zofran now and two different cough medicines. Other than the cough medicine (which makes me dizzy when I call back) - it does prevent me from coughing. Symptoms ongoing. I''ve tried to force myself to get up more today and move around more to do a few things. I have had to take my work in small portions to fold three or four pieces of clothing and then rest a while. I don''t have the energy to hardly do anything. I''m nervous to going back to work. I''m in the process of recovery.

Write-up: The day following my vaccine, I became extremely nauseous and fatigued. I was unable to lift my head or open my eyes because of extreme dizziness/nausea for 24 hours. Over the next 2 months, ending around June 11th, I had migraines almost every other day, constant nausea, and a few migraines that lasted 3-4 days. At one point, I needed to go to the ER to get relief from the migraine. This helped for about 3 days before the migraines returned. At 2 months out, the migraines have improved, but I still have very tight muscles and stiffness throughout my body, a symptom that I often get along with migraines.

Write-up: Flu symptoms 12 hours after the vaccination that lasted for another 12 hours. Fatigue the rest of the week. Intermittent fatigue for the next few weeks. Write-up: Patient complaint of rash to arms and legs starting after receiving vaccination on 6/16/21. Patient seen at ER on 6/18/21. Labs unremarkable and treated with oral Benadryl, oral Pepcid, and oral Motrin. He was sent home with prescription for Benadryl and Pepcid and to f/u with PCP. The patient''s height, and weight were not reported. No past medical history or concurrent conditions were reported. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 1805018, and expiry: UNKNOWN) dose was not reported, 1 total administered on 21-MAR-2021 for prophylactic vaccination. Concomitant medications included iron for anemia. On 21-MAR-2021, shortly after vaccination patient became very tired and felt like getting sick and felt like as it was the expected. Patient stayed on couch next day. On 23-MAR-2021, bleeding was started which was not normal bleeding. Patient knew that she had to see hematologist as oxygenated blood was leaving her body. Patient experienced very heavy vaginal bleeding. Patient called gynecologist and was advised to go to emergency room, if patient bleed more than a pad an hour. Patient went to doctor shortly because patient felt this was not normal and something else was going on. When doctor took patient''s blood, she fainted. Patient was healthy before shot and never had this before. Patient had intravenous (IV) so that patient could drive home. Patient was doing regular things at slower pace. Some weird things happened at 17.00 daily, patient cried for no reason. Blood test was received with something low. Patient went to the emergency room where they gave her blood, which patient never needed before in her life. Patient needed 2 units of bloods. Patient was slowing dying but did not know it. Patient was now anemic and needed 4 iron infusions and maybe needed 5th also. Patient had been told to follow-up in august. Patient was not sure that her body was making her own blood again. Patient felt almost died. Patient could not speak a full sentence even after the emergency room visit. Patient was still bleeding. Patient also experienced inflammation in chest which was very painful. Patient started birth control on 14-MAY-2021 and 15-MAY-2021 and stayed on until 24-MAY-2021. Patient started bleeding again on 25-MAY-2021 which continued till 29-MAY-2021. Patient had a lot of shortness of breath and thought may be a blood clot but on 20-MAY-2021, X-ray showed no clots. Cardiologist said this was most likely from the COVID Shot and likely inflammation. Patient chest was still painful to touch and it was better than before. Write-up: I got bit by a deer fly in two spots and it got infected by poison ivy and I also got shingles (the doctor diagnosed) and it all happened at the same time. I still have the rash for the shingles and I can still see the bite infections. The poison ivy has dried up but can still see it on my leg. I got an anti-biotic for the deer fly bite. I got a Steroid Prednisone for the poison ivy. There was nothing they could do for the shingles because it was too late. First I went to Urgent Care - and that''s where I got the antibiotic. Write-up: I woke up very nauseous and woozy. I walked into my sisters room and fell down due to weakness. Write-up: I had a number of strokes between April and May. As many as 9 of them. Memorial Day, 31st, I went to the hospital ER at 9:00PM. I had a PFO, which is something like Patent Forman Ovale, heart condition, hole in atrium of heart that causes the blood to go from heart to brain. Blood clots to the brain, venous blood gets into the arterial blood. The clots could have formed everywhere but they go straight up the brain. They went through an artery and put a mesh screen over my heart opening and eventually it will help that. It was done through an artery, 15 minute process. I left the hospital the following Saturday. This week (two days ago) I have really bad tinnitus starting in both ears. My hearing is quite disrupted. I don''t know how serious it is right now. I am being treated with an anti-cholesterol drug (Lipitor) and aspirin. I still have a little bit of cognitive impairment and physical fatigue. My coordination is messed up. Physical therapy starts the 15th and Speech therapy on the 23rd. I had just gotten a job two weeks before the strokes.

Write-up: I had no symptoms up until May 3rd. I had high blood pressure - it was through the roof. - It was 193/101. I was experiencing dizziness and wobbly - not sure on my feet. ER. Changed dosage to - higher dosage - Lisinopril and Amlodipine. Primary care doctor decided to give me Prednisone - I did the series of it and I no longer take it. Blood pressure is still high and racing with the heart issues and working with the doctor to still get it under control and taking the meds. They did issue me Meclizine - in case I have a vertigo feeling. May 26th - I told her I am not going to use her services anymore and so I''m seeing Dr. now. Remicade infusion every 8 weeks. Write-up: The vaccine administrator didn''t listen when I told him he stuck the needle into the wrong place, and proceeded to insert the injection despite my protest. I felt an immense pressure in my shoulder, and the sensation of something cold trickling down my arm. The administrator was standing over me during this process while I sat in a metal chair and I''m not sure he wanted to even touch me. My fiance and I received the shot together, however only he experienced a slight fever. I only experienced increasing pain when moving my arm. No other health changes. The pain was immediate and progressively got worse. My primary care provider assessed me, and agreed that the injection was indeed pumped into my shoulder joint, instead of the muscle. My PCP is also worried about the efficacy of the vaccination, and whether or not absorption was successful. There is possible rotator cuff damage, and chronic pain when performing NECESSARY routine daily activities. I''ve been ordered on pain medication, as well as physical therapy and have been advised that many vaccine related shoulder injuries never heal, or require surgery.

Write-up: On 06/28/2021 i received the vaccine. I was sitting in a chair at the pharmacy, unsupervised. Within 5 minutes of receiving the shot I began to feel light-headed, lost consciousness and fell onto the floor and hit my head (according to a witness). Write-up: One or two minutes after administration, patient said we was feeling light-headed, and then that he didn''t feel well. He was instructed to focus on his breathing. He did this for a moment (maybe a minute) and then his head fell forward and he slumped forward in the chair. He was held stable in the chair by the pharmacist. After a short time (less than a minute) the patient''s upper body (bilateral arm and hand) muscles contracted (maybe a seizure?) and then he relaxed and sat up. He was alert, a little disoriented initially and had no recollection of his reaction other than feeling light-headed and unwell. He then became sweaty and felt a little nauseated. Write-up: BILATERAL LOWER EXTREMITY PAIN; MULTIPLE ACUTE BILATERAL PULMONARY EMBOLI; PLATELET COUNT DECREASED; DEEP VEIN THROMBOSIS; VACCINE INDUCED THROMBOTIC THROMBOCYTOPENIA; ULTRASOUND DOPPLER ABNORMAL; FULL BLOOD COUNT; METABOLIC FUNCTION TESTS; MUSCLE SPAMS; SWELLING; MILD HEADACHE; This spontaneous report was received from a health care professional via a Regulatory Authority and from literature: Vaccine-induced thrombotic thrombocytopenia following Ad26.COV2.S vaccine in a man presenting as acute venous thromboembolism. 2021 Jun 03;1-4., and concerned a 48 year old white male (not Hispanic or Latino). The patient''s height, and weight were not reported. The patient''s concurrent conditions included exercise induced asthma, and no tobacco use (both current and historically), and other pre-existing medical conditions included the patient had no known allergies. The patient was not obese, denied recent procedures, prolonged immobility, family history of bleeding or clotting disease, etc. The patient was up to date on cancer screenings based on age/family history. He was reported as being an unusually physically active and fit 48 year-old male who has enjoyed good health and exercised regularly. In 2019, his platelet count was 177 10*9/L. The patient reported flying out of town and back on 30-MAR-2021 and 04-APR-2021 respectively with the longest continuous flight lasting around 4 hours. The patient at age 48, received covid-19 vaccine ad26.cov2.s (suspension for injection, intramuscular, batch number: 201A21A, expiry: UNKNOWN) dose was not reported, 1 total administered on 07-APR-2021 for prophylactic vaccination. Concomitant medications included finasteride, and salbutamol. On an unspecified date in APR-2021, the patient experienced mild headache. On 18-APR-2021, 11 days after receiving the vaccine, the patient experienced deep vein thrombosis, vaccine induced thrombotic thrombocytopenia; new pain in his big toe, bilateral leg cramping (muscle spasm), bilateral lower extremity pain, and swelling. The patient attributed soreness in the large muscle groups of his thighs to Peloton riding and exercise. The patient travelled again via airplane to an unspecified location 22-APR-2021 and 25-APR-2021 (both1 hr long flights). The patient administered Advil for the leg pain when he returned on an unspecified date. On an unspecified date he felt a leg vessel that "didn''t seem right." On 26-APR-2021 the patient went to emergency department for evaluation presenting with leg pain and chest discomfort. He had normal vital signs and oxygen saturation. Additional laboratory results: Platelet count (NR 140-440 K/uL) 74; fibrinogen (NR 150-430 mg/dL) of 254; D-dimer (NR 0-499 ng/mL FEU) 15,109 ng/mL FEU; activated partial thromboplastin time (aPTT) (NR 25.1-36.5 s) 31.8. Venous duplex ultrasound of the lower extremities revealed non-occlusive DVT in the bilateral popliteal veins extending to the gastrocnemius veins, an occlusive DVT of the left posterior tibial vein, as well as occlusive superficial venous thrombosis in the bilateral saphenous veins. The patient was discharged from the emergency room on rivaroxaban 15 mg twice daily with two-day follow-up with his primary care physician. He had no known exposure to heparin before the onset of symptoms and no exposure during his emergency room visit. On 28-APR-2021 (also reported as the next day), the patient complained of a new onset of chest pain (pleuritic) and presented again to the emergency room. Chest CT showed presence of acute pulmonary embolism, with emboli present in segmental and more proximal arteries. He had already taken his rivaroxaban for the night (1800). The patient was hospitalized for further evaluation and work up. A peripheral smear showed thrombocytopenia without schistocytes. The hematology service was consulted, and a presumptive diagnosis of VITT (vaccine induced thrombotic thrombocytopenia) was made. His anti-PF4 enzyme-linked immunosorbent assay (ELISA) (LIFECODES PF4 IgG), performed on a sample collected prior to initiating IVIG therapy, demonstrated a strongly positive result of 3.323 optical density (OD) units (reference interval < 0.399), consistent with a diagnosis of VITT. The patient was immediately treated with 1 g/kg of intravenous immunoglobulins (IVIG) for 2 days, 1 mg/kg of prednisone, and switched from rivaroxaban to an intravenous argatroban infusion drip, during which he achieved therapeutic aPTT levels between 44 - 50 sec. The patient was also treated with apixaban. Other studies included: Magnetic resonance venography (MRV) and angiography (MRA) of the brain, performed due to symptoms of mild headaches, were unremarkable. Anticardiolipin antibodies (NR: not provided) 5 GPL, Beta-2 glycoprotein 1 antibodies (NR: 0 - 20) 0 SGU, 5 SMU, Blood homocysteine (NR: not provided) 13.4 nml (normal), C-reactive protein (NR: not provided) 2.6, Cardiolipin immunoglobulin G (NR: 0 - 12) 6 MPL, Cell marker increased was positive, EKG (NR: not provided) Normal, Hematocrit (NR: 36.9 - 52.1) 42.2 %, Hemoglobin (NR: 2.5 - 18.0) 15.0 g/dL, Lupus anticoagulant (NR: not provided) not detected, MCHC (NR: 33.4 - 35.3) 35.5 g/dL, MCV (NR: 80.6 - 97.6) 83.2 fL. On an unspecified date the serotonin release assay was no longer positive. Within a week post-discharge, the patient developed recurrent thrombocytopenia, with a platelet count that down trended to 107 10*9/L His thrombocytopenia appeared to somewhat correlate with prednisone dose adjustments, suggesting an autoimmune-driven process, although it did not respond to repeat IVIG administration. Due to the normalized D-Dimer levels and negative SRA, it was assumed that there was no further ongoing platelet activation. The authors stated the patient had no prior risk factors for thrombosis other than the 2 recent 4-hour flights. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient recovered from bilateral lower extremity pain, was recovering from deep vein thrombosis, vaccine induced thrombotic thrombocytopenia, platelet count decreased, swelling, full blood count, metabolic function tests, muscle spasms, ultrasound doppler abnormal, and multiple acute bilateral pulmonary emboli, and the outcome of mild headache was not reported. This case obtained via VAERS (ID 1264060) concerns a 48-year-old White male who developed symptoms of bilateral lower extremity deep vein thromboses (DVT) and thrombocytopenia 11 days after receiving Janssen Covid-19 vaccine. Concurrent conditions included exercise induced asthma. Patient was not obese, did not smoke, denied recent procedures, family history of bleeding/clotting disease & was physically active. He was up to date on cancer screenings based on age/family history. He had taken two 1-hour long flights 15 days and 18 days post-vaccination. Concomitant medications included finasteride & salbutamol. After 1 week of bilateral leg cramping & swelling which began 11 days post-vaccination, he presented to emergency department (ED) where complete blood count showed platelets 74 k/uL, peripheral smear with giant platelets, and d-Dimer 15K. Venous duplex ultrasound showed nonocclusive DVT in the bilateral popliteal veins extending to the gastrocnemius veins, occlusive superficial vein thrombosis in the bilateral saphenous veins, and occlusive DVT of the left posterior tibial vein. He was started on rivaroxaban & was not hospitalized. Two days later, he developed new chest pain & returned to ED, where CT chest showed numerous bilateral pulmonary emboli. He was hospitalized; empiric treatment for vaccine-induced immune thrombotic thrombocytopenia was started: IVIG, steroids, & argatroban. Anti-PF4 resulted as OD=2.9 and 3.323 (strong positive), confirming diagnosis. The patient was recovering from the events. Based on evolving knowledge of Thrombosis with Thrombocytopenia Syndrome (TTS, per definition from Brighton Collaboration), low platelet count & temporal relationship to vaccination, the events are assessed to have a plausible relationship with vaccination.

The patient''s weight was 200 pounds, and height was 65 inches. The patient''s concurrent conditions included deep vein thrombosis, hypothyroidism, hypertension, penicillin allergy, nickel allergy, wasp sting, peanut allergy, alcohol user, and non smoker, and other pre-existing medical conditions included patient had no drug abuse or illicit drug use. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, intramuscular, batch number: 1805020, and expiry: 20-MAY-2021) dose was not reported, administered on 11-MAR-2021 15:11 for prophylactic vaccination. Concomitant medications included epinephrine for anaphylaxis, cannabidiol for burning sensation on back of hand/are of foot or hand, acetylsalicylic acid for prevention of deep vein thrombosis, amlodipine for treatment of hypertension, and levothyroxine sodium for treatment of hypothyroidism. On APR-2021, the subject experienced lesion/blister on foot. On APR-2021, the subject experienced itching on left foot. On 17-APR-2021, the subject experienced discoloration of foot and hand/ finger. On an unspecified date, the subject experienced swelling occurring from time to time in left foot/swelling of left foot and right hand/finger, bruise, tenderness, burning sensation, and veins are more puffed up distended and swollen.
