Write-up: POSSIBLE BLOOD CLOT ISSUE; ELEVATED HEART RATE (OVER 100 AT REST); KIDNEY PAIN; NO APPETITE; FLU LIKE SYMPTOMS; SWOLLEN GLANDS IN NECK; SERIOUS BRAIN FOG; DIARRHEA; SHORTNESS OF BREATH; This spontaneous report received from a patient concerned a 52 year old female. The patient''s height, and weight were not reported. The patient''s concurrent conditions included allergy to contrast dye. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 203A21A, expiry: unknown) dose was not reported, 1 total, administered on 10-JUN-2021 for prophylactic vaccination. No concomitant medications were reported. On 10-JUN-2021 after getting vaccination, the patient experienced elevated heart rate of over 100 at rest for four days. On an unspecified date in JUN-2021 after vaccination, the patient also experienced shortness of breath, headache, kidney pain, diarrhea, no appetite, flu-like symptoms, swollen glands in neck, aches and pains and had severe fatigue. On 14-JUN-2021, after four days of vaccination, the patient heart rate decreased to a healthier range but shortness of breath, headache, swollen glands, extreme fatigue and diarrhea continued. The patient also had serious brain fog. On 21-JUN-2021, after 11 days of vaccination, the patient had a severe shortness of breath and the patient''s PCP (primary care physician) directed patient to visit an emergency room in a hospital. The patient took blood test and showed an elevated d-dimer indicating possible blood clot issue. The patient could not perform lung CT (computed tomography) or angiography due to a severe allergy to contrast dye. The patient also had an MRI (magnetic resonance imaging) of brain and ultra sound of the legs and the results were normal. The patient was on day 16 post vaccination at the time of this report. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient recovered from elevated heart rate (over 100 at rest) on 14-JUN-2021, had not recovered from flu like symptoms, swollen glands in neck, possible blood clot issue, serious brain fog, and diarrhea, and the outcome of shortness of breath, kidney pain and no appetite was not reported. On 28-MAY-2021, the next day after vaccination, the patient saw doctor for respiratory issues and heavy chest and was sent to emergency room. They kept her 8 hours and put her on machine with a mask and gave her rescue inhaler. On JUN-2021, after about 1 week later, went to doctor, did blood work, the patient went back a couple weeks after that and did leg ultrasound and found nothing. On 24-JUN-2021, the patient went to doctor because lost all mobility in arm and right arm was bigger than left, and patient was admitted in hospital. They did ultrasound in leg and arm on right side and found blood clots. Her primary care physician (PCP) Dr. had put her on blood thinners before this, but hospital doctor changed to a different blood thinner. Sent for ultrasound, found four blood clots (DVT''s) in left leg (3 upper thigh and one behind left knee). Sent to ER, started on Eliquis, will follow-up with primary care next week. Write-up: I started feeling dizziness and thought I would give it a week or so, since I thought it was just a side affect that would go away. It feels like I am intoxicated, but not having fun (how the dizziness feels). Heart palpitations, thumping on my heart. Write-up: I had a steroid shot in my left knee on 6/10 at 8:00 am. By 10:30 am I was feeling off and having heart palpitations. I kept checking my pulse and blood pressure. My pulse was lower than normal (in the high 50''s and ''60s. It''s normally 80-90.) I felt heaviness in my arms (both) and my chest but no pain. After eating lunch at 12:15 pm, I tried to just rest but continued to feel worse- more heaviness in my arms and chest with dizziness at that point. My mom took me to the emergency room. I was admitted around 3:30 pm. After tests and monitoring me, they determined it was a reaction to the steroid shot and that I should speak to my orthopedic doctor regarding future doses.

Write-up: Heart Palpitations starting on the 28th of May. Chest pain starting on the 3rd of June. And a sick feeling that I have described as a flu or hangover feeling starting on the 5th of June. All of those palpitations and chest pains have continued at present. Sick feeling lasted until the 30th of June. On about midnight of 22nd of June - I felt extremely faint and went to Hospital ER. Nothing out of the ordinary was found in all the testing for ER and at the urgent. No treatment other than the IV Saline at ER visit.

Write-up: On vacation in, just got to hotel and fell asleep. Wife said I lifted my head with a strange look on my face, laid back down. Then one minute later had a grand mal seizure. Called 911 and was sent to ER via Ambulance. ER conducted EKG, CAT Scan and other tests to determine the root cause. Found, no tumors or other explanations for the seizure. Write-up: Customer did not feel right within 7-10 mins after receving the Janssen vaccine. He complained of vision changes, "seeing dots". When he tried to walk, his legs buckled, but customer was slowly assisted to the floor. While on the floor, my technician heard grunting noises coming from him and his left arm was rigid and bent at the elbow. But within 5-10 seconds, he slowly sat up and said that he "doesn''t remember being on the fall" and complained of being sweaty and "seeing dots". Paramedics were contacted and took him the hospital. Contacted the customer later in the evening and was informed that he was resting at home after spending half of the day in the hospital. He mentioned that the paramedics treated him with Epipen and bendadryl and then he felt better. He was treated "other things" but don''t recall.

The patient''s height, and weight were not reported. The patient''s concurrent conditions included diabetes type II. The patient experienced drug allergy when treated with acetylsalicylic acid (aspirin), and ibuprofen. The patient received Covid-19 vaccine ad26.cov2.s (suspension for injection, intramuscular, batch number: 1805020, expiry: unknown) dose was not reported, 1 total administered to left arm, on 18-MAR-2021 for prophylactic vaccination. Concomitant medications included ascorbic acid/calcium/minerals NOS/retinol/tocopheryl acetate/vitamin B NOS/vitamins NOS/zinc (centrum silver), fish oil, glipizide and lisinopril, all for unknown indications. Patient had acute onset of low back pain 1.5 weeks prior to presentation on 03-JUN-2021. On 09-JUN-2021, this pain and weakness (asthenia) progressed to complete bilateral lower extremity paralysis (diplegia) with loss of bowel function (anal incontinence) and bladder function (urinary incontinence). He was seen emergently and was found to have critical, bilateral lower extremity ischemia. Computerised Tomogram angiography of the abdomen and pelvis revealed: 1. Right runoff: abrupt vessel cut off suggestive of acute thrombotic occlusion of the proximal common femoral artery with propagation into the superficial femoral artery and the popliteal artery. On the delayed imaging a small amount of contrast was seen within the severely stenotic popliteal artery and very small amount of contrast was seen in the peroneal artery extending to the ankle suggesting proximal stenosis and extremely slow flow. 2. Left impression: abrupt cut off suggestive of acute thrombotic or embolic occlusion identified at the origin of the left common iliac arteries (iliac artery embolism, iliac artery occlusion) with propagation throughout the left lower extremity down to the popliteal artery. 3. On the delayed phase imaging, tiny wisp of contrast within the severely stenotic popliteal artery (peripheral artery stenosis) identified suggestive of very slow flow. The peroneal artery is also faintly patent on the delayed phase imaging with flow contrast extending to mid-calf level. Additional events were reported which occurred on an unspecified date in JUN-2021: peripheral arterial occlusive disease, peripheral artery occlusion, peripheral artery stenosis, peripheral artery thrombosis, poor peripheral circulation. The patient was hospitalized for 6 days. Due to severity of the ischemic limbs, he was offered bilateral above the knee amputations. He declined and would discharge from the hospital to hospice on 12-JUN-2021. Report stated "He is going to die." This case, received from VAERS, concerns a 71 year-old male patient with diabetes, who presented with back pain, weakness, lower extremity paralysis and bowel/bladder incontinence, and was found to have critical lower extremity ischemia, 83 days after receiving the Janssen COVID-19 vaccine. Angiography revealed occlusion of the left iliac and bilateral femoral and popliteal arteries. Events included iliac artery occlusion and embolism, and peripheral artery occlusion, stenosis and thrombosis. No platelet counts were provided. The patient was offered amputations but declined; the events did not resolve and he was discharged to hospice. The vessel stenosis is longstanding in nature and most likely related to his age and diabetes; the embolism/thrombosis may also be related to his chronic condition, and the latency between the events and the vaccine is long. However, a relationship with Janssen Covid-19 vaccine cannot be ruled out and thus the relationship is considered indeterminate.

The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 043A21A expiry: UNKNOWN) frequency 1 total, dose was not reported, administered on left arm on 09-APR-2021 for prophylactic vaccination. No concomitant medications were reported. On 09-APR-2021, after receiving the vaccine, the patient experienced a sharp pain running down left arm. Patient stated that her hand was clenched in a claw position and blood pressure began to drop. On 09-Apr-2021, the patient went to emergency room for lost consciousness and was sent to the hospital around 11:15 AM. Patient experienced some nausea and was given something with benadryl inside but the patient wasn''t sure exactly about it. Electrocardiogram (EKG) was performed, but the results were normal. Write-up: Extreme pain occurred in left thigh on 4/25/2021, went to ER. Was diagnosed with spontaneous bleed in gluteus muscle. Sent home from hospital 4/28/2021. Pain reoccurred 5/5/2021 and was again admitted to the hospital. Doctors unable to determine exact site of bleed. Stopped blood thinners to allow bleeding to stop and leg to heal. Inserted IVC filter on 5/9/2021. Had to go to to a rehab facility from 5/11 to 5/19. Saw doctor on 6/21 who ordered an MRI to be sure of no blood clots. Found out on 6/23 that there were blood clots in the lungs and was sent to the ER. Admitted to the hospital again. Had to stay until they could take out the IVC filter which was done on 6/28/2021 and she went home on 6/29/2021.

Write-up: Immediate pain in left arm. Pain continued to get worse over the weekend with burning and tingling in arm and front and rear shoulder. Sought treatment at an urgent care. Later sought treatment with primary physician. By this time I was experiencing what felt like a deafness in the forearm as well as some tingling. Write-up: She had a syncopy. She was sitting on the chair after vaccination. I checked her about 5 mins later, sitting still but her eyes were closed and her phone was on the floor. Write-up: Patient received dose of Janssen Covid vaccine in right deltoid. He chose the right arm because his left upper arm is fully tattoed. Patient sat for about 5 minutes and then he got up and fell forward on our counter in the waiting room. After about a minute he got up and sat on the bench for a few minutes. We discovered him after about 10 minutes lying sideways on the bench. First BP reading was 86/63mmHg and patient was clammy and sweating. His major complaint was fatigue. He took a sip of soda and ate a peanut butter cup. EMS arrived his BP had gone up to 120/68 mmHg. Oxygen levels were normal but patient refused to allow EMS to check his blood glucose.

Write-up: Patient was given a Janssen Covid 19 shot on 7/5/21 around noon. After the shot we talked about different side affects and he had a few questions about adverse reactions. I had him sit down for 15 minutes while I finished his paperwork. As i was walking back from the immunization room toward the pharmacy I saw him drop his phone and he fell to the ground. I tried to reach for him but was not close enough to break his fall. I saw his eye roll back and his face turn very red. He was breathing very fast and loud. He regained consciousness and I told him 911 way on the way and to please try to relax his breathing. His brother was with me the whole time talking to him and reassuring that everything was ok. The paramedics came and hooked up a monitor. They were asking all sorts of questions to everyone in the area. I went back into the pharmacy and he walked out to the ambulance with his family and was taken to the hospital.

Write-up: Vaccine given to me at 10:00 am on 05-04-2032 at the Volunteer Fire Dept. At approx 12:05 am 04-05-2021, I woke up with every , EVERY. joint in my body on fire, I realized that it was a reaction and took 2 aspirin which did no good. The patient experienced drug allergy when treated with ibuprofen. The batch number was not reported and has been requested. No concomitant medications were reported. Next day after getting vaccination (on 07-MAR-2021), the patient got a mild headache. On the same day around 10:30 pm, while sitting in a reclined position, he experienced palpitations in the neck. He put on his wife watch and it showed atrial fibrillation. Write-up: For background - I have ITP -and have to get my blood drawn usually weekly and platelets were down the middle of March - saw an oncologist - take me off blood thinners and increase the Prolacta. Sometimes platelets go up and down for no reason. I went in to get a steroid injection in knee for knee pain on May 18th. I had one three months earlier in my other knee with no problems at all. Before I got the injection, my blood pressure was really high - they told me to watch it. Which is very abnormal for me - usually it''s low and very stable. First time in the history of my life that my blood pressure was ever like that. When they gave me the steroid injection - I got a pounding headache the next couple of days. It was the worst pain I''d ever had. It was a headache I have never had before. That Sunday, I went in to the Urgent Care because I couldn''t take the pain anymore. The doctor sent me home to wait for bloodwork results. Got results on Monday, 24th. My platelet count was through the roof - 700,000 PLUS. I would have had blood clots if I wouldn''t have been on Blood thinners. I would have had a blood clot on the brain. They have never been that high. This was with no medication change - just had had the vaccine and the injection. I called my oncologist on the 24th after that and they had me go to go to ER. They took me off Prolacta and put me on blood thinners. They were having me get my bloodwork at least twice a week and it took about three weeks for platelets to go back down. Now they are low. The headache has stopped. Have spoken just on the phone with my primary care doctor. I feel like this had to do with the combination of the vaccine and the steroid injection.

Diagnostic Lab Data: 1. Allergic reaction Consider adverse reaction to COVID vaccination (J & J). Was treated with Solu-Medrol IV, famotidine, benadryl, and zyrtec with improvement in symptoms and was eager for discharge. Case discussed with Dermatology (6/29/21) and this was felt to be urticaria / angioedema, possibly related to COVID vaccination. Respiratory pathogen panel recommended (6/29/21) and this was negative. ANA, dsDNA antibody, Complement Activity Total, C3 and C4 were all negative / in range (6/29/21). Lengthy steroid taper recommended. BMP, CBC, and TSH were also drawn (6/28/21) and were unremarkable except for slightly elevated WBC (11.2) and eosinophils slightly low (0.4). 2. Abdominal pain Pt reported abdominal pain and was very worried about tumor recurrence. CT (6/29/21) abdomen / pelvis reassuring. 3. Crohn''s disease Stable.

Write-up: The patient is a 35 year old female with PMH remarkable for Chron''s disease in remission, seasonal allergies, s/p R adrenal schwannoma s/p resection who presented to the ER with ongoing problem with urticaria for the past 3 weeks. She reports feeling well when she developed scalp itching which progressed to diffuse intermittent hives and angioedema 2 days after receiving Covid J&J vaccination. Write-up: After vaccination that day, patient was too tired to get out of bed to do his evening walking exercises. On 6/10/2021 3:00 am, patient unable to support himself on the commode and was put back to bed. Next couple of days patient was too weak to get out of bed. 6/14/21 1:00 am, patient became very congested and unable to remove fluid in his lungs, wife called medics and patient was admitted to hospital for 2 days with pneumonia. 6/16/21 7:00 pm, patient discharged from hospital. That night, patient became very congested despite continuous suctioning through the mouth, patient experienced respiratory distress and lungs congested with fluids, 2:30 am medics was called and paramedic performed invasive deep suctioning of lungs via his nose on way to the hospital, upon arrival at hospital they performed another deep suctioning of lungs before sending patient to the ICU. 6/21/21 4:00 pm, patient discharged from hospital in stable condition, doing physical therapy and occupational therapy at home.

Write-up: Tired and sleepy - for the next couple of days right after the vaccine. June 14, I started getting on my right side - from cheek to toes - and my whole leg and is still in pain today. For the last three weeks - the pain level was from 9 or 10 out of 10. I missed work for three weeks. I can barely sleep at night - maybe for three to four hours because the pain would wake me up. Today was my first day after three weeks that I was able to go to work. Today the pain is 5 out of 10. No treatment at the hospital. They prescribed me with 800 mg of Ibuprofen - two pills at night. The medicine was not strong enough. The prescribed me cream on my neck. The day I went to pick up my prescription to the pharmacist advised me to take another brand - so found a generic brand for $25 instead of $525. The cream doesn''t numb my leg or make me fill better. I was applying hot water in the shower on my neck and that helped a bit. Told me to watch videos to stretch and exercise my neck every day.

Write-up: At first, I had joint pain, nausea, diarrhea and fatigue. Each day got a little bit better, lasting a few days. The headache started on the second day and that kept getting worse. Headache, every time I went to swallow anything, the headache would be excruciating hurting at these times. It was unbearable and I had to go ER. They checked to see if I had a blood clot. I didn''t. They gave me a pain injection, Dilaudid, which made me feel disgusting and didn''t help. No medications to take home. Had a phone visit with my doctor. The headache continued, and it lasted me a full two weeks. It has eased up now.

Write-up: Received vaccine on 6/5/21, next morning at 7:00 am mother heard a loud noise and found PT face down lying half way in bathtub unresponsive, attempted to lift him observing his eyes were rolled back in head, was unconscious for approximately 1 min trying to lift him out, then he blacked out a second time hitting bathtub a second time. Lifted him out and placed on floor, he was unresponsive - called 911, he came to slightly after 3 minutes. Ambulance took him to ER and we stayed all day receiving fluids and chest XR (clear). We were discharged in the afternoon with the doctor saying he has a syncopy episode. At 4:00 PM PT was going to the bathroom and he stated he didn''t feel good. I stood in front of him because he turned white and eye rolled up into his head and he blacked out and stopped breathing for approximately 2 minutes while I got him on floor and gave CPR. Called 911 and he began breathing again . Back in ambulance to hospital and we stayed in hospital for 2 days.

Diagnostic Lab Data: I have had multiple tests completed. There was a lot of blood work drawn, an EKG, an echocardiogram and an ultrasound. It was found that I have a blood clot in my arm and that is why I have been prescribed Eliquis.

Write-up: I started to suffer from a numbness in my left hand that radiated into a deep pain and increasingly became intense. That pain radiated up to my shoulder and into the left quadrant of my chest. I started experiencing shortness of breath and a hypersensitivity to the heat. I went to Hospital ER and they ran a bunch of tests. I was diagnosed with a blood clot in my arm. I was given Eliquis. I was on that medication for about 2 weeks and after my second week of being on Eliquis, I started to suffer from the same symptoms. I decided to go to the Hospital. When I went there they admitted me and ran through blood tests to check for a heart attack or stroke or anything relating to the pain I was experiencing. Now I am back home and I am still taking Eliquis. As of today, I am still experiencing the symptoms. The symptoms are mild, but it''s still scary. I would like to add that I have had an increase in dietary intolerance.

Write-up: Patient reported to the administering pharmacist today that he has been experiencing dyspnea and shortness of breath, which began approximately two days prior to date and time that the event was reported (i.e. beginning on or around July 4th, 2021). Patient states that he has been experiencing dyspnea and shortness of breath both upon exertion (and, even upon minimal exertion ? for example, when walking from car in parking lot into the pharmacy today) and also while at rest. Write-up: Noticed Petechiae, bruising and blood blisters in mouth 6/15/21. Went to urgent care and had CBC done and platelets were at 2,000 and was instructed to go to ER. Got second opinion from primary care physician an instructed to go to ER. Went to ER on 6/15/21 and was immediately admitted to hospital. Began steroid IV treatment and had completed blood profile done. Underwent bone marrow biopsy and abdominal ultrasound 6/18/21 - all normal. Was given platelet infusion and human immune globulin infusions. Release from hospital on 6/21/21 with platelet count at 32,000. Continued steroid treatment and regular CBC tests. Diagnostic Lab Data: I had an EKG, echocardiogram, head CT, chest CT would die, and an MRI on June 9th. The EKG showed an issue with my heart. The echocardiogram showed that my right ventricular was larger than my left. My head CT was fine as well as my MRI. My chest CT showed a massive saddle pulmonary embolism. I also had a d-dimer test done on June 9th that came back negative. Throughout the rest of my hospital stay I had additional blood work done along with another echocardiogram and EKG.

Write-up: I fainted in my backyard on June 9th. I was brought to the hospital by ambulance and had elevated heart enzymes and a problem with my EKG. A subsequent CT scan was done and it was found that I had a massive saddle pulmonary embolism. I was air transferred to another hospital and given a TPA to break up the clot. I was in the ICU for 3 days and then the cardiac floor for 2 days. I also had a bleed where my central line was put in in my groin and a subsequent hematoma. The PE was most likely caused by a DVT in my right leg, believed to be caused because of a dislocated ankle and estrogen-based birth control.

The patient had no adverse event following any previous vaccine. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, intramuscular, and batch number: 206A21A expiry: UNKNOWN) dose was not reported, 1 total administered on 21-APR-2021 on left arm for prophylactic vaccination. No concomitant medications were reported. On 08-MAY-2021, the patient''s laboratory data included CT (computed tomography) angiography of chest showed severe burden of acute pulmonary embolism (PE) with severe right heart strain, small early infarct posteromedial left lower lobe (LLL), hepatic steatosis and interventional radiology (IR) thrombectomy procedure showed large bilateral pulmonary emboli. On 09-MAY-2021 (approximately 3 weeks after vaccination), the patient was presented to the emergency room due to progressively worsening shortness of breath. The patient also had pain and swelling in the left calf and leg. On the same day, the patient experienced brain injury, brain oedema, acute kidney injury, hypoxic-ischaemic encephalopathy, mental impairment, pain in extremity, peripheral swelling and had lung assist device therapy. The patient was hospitalized on an unspecified date. On 10-MAY-2021, the patient''s ultrasound venous Doppler showed complete thrombosis left femoral, popliteal, posterior tibial, peroneal and anterior tibial veins, partial thrombosis of left common femoral vein. The patient underwent urgent thrombectomy, however, on 11-MAY-2021 (the next morning), she became more hypoxic and hypotensive and was intubated and was urgently placed on extracorporeal membrane oxygenation (ECMO). The patient developed multi-organ failure and was started on continuous veno-venous hemofiltration (CVVH) for oliguric renal failure. On 13-MAY-2021, CT computerised tomogram (CT) scan of the patient''s head showed diffuse cerebral edema, which was most likely a result of hypoxic encephalopathy. With no improvement in her mental status over the course of the next few days, the patient was taken off life support and died on 15-MAY-2021 due to unknown cause. The patient was in hospital for 7 days. It was unknown if autopsy was performed. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. Venous doppler showed complete thrombosis left femoral, popliteal, posterior tibial, peroneal and anterior tibial veins, partial thrombosis of left common femoral vein. Write-up: I had some dizziness, headaches, right side neuropathy. I was also tired all the time. I went to the doctor where my doctor tried looking for other things aside from the pulmonary embolism. A brain MRI was done which came back normal. A thyroid test was done which showed hypothyroidism. I went to a specialist after this where a thyroid ultrasound was done and showed a swollen thyroid. I was diagnosed with Graves? disease. I have never had this before. I am now on methimazole 20mg/day. I had labs drawn to see if dose needs to be adjusted. I was told that it will be about 2 years before it can be determined if that medication is working. I also experience brain fog. I have lost 10 lbs in the past months due to this as well.

Write-up: It was around May 3rd 2021 that I got Bronchitis out of the blue it was weird, I went into the ER for that and was put on antibiotics and an inhaler. Well I started having a sinus infection sometime in May and around June the 17th 2021 is when I had a telehealth appointment with my DR. That was happening and let me see, then I went back in to the DR on July 2nd 2021 and they had to do a check up and they put me on a lot of medications for allergies basically. I still feel the same though. I was put on doxycycline and feel no difference. I had an allergic reaction to Ellipta medication, I had a bad reaction to that one. I also got Claritin, Flonase, albuterol inhaler along with the doxycycline.
