Write-up: Fever, body aches and I couldn''t sleep. It progressively got worse; didn''t sleep well that night. I ached everywhere. Next day, I felt lethargic and fatigue. All day the 6th, I laid around with a fever off and on. I felt better on the 7th. The following week after the vaccine, I noticed pressure in my head. The sinus pressure has progressively gotten worse and the postnasal drip. I have been taking Flonase every day and OTC allergy medicine and I was very lightheaded, so I went to Urgent Care about a month ago. They gave me Prednisone. I was on that for five days, but I still didn''t clear it up. I went to the doctor last week, wanted to change OTC medicine to see if body had built up an immunity to the allergy pill, I had been taking. I stayed on the Flonase. The pressure is crazy, it gives me headaches. It gives me postnasal drip. I still have the symptoms now and it didn''t make a difference to change up the OTC med. I have an appt next week again with the doctor to see what to do next.

Write-up: Around the first week of May I was experiencing a stuffy nose. Around the middle of May I felt like it had moved to my throat and something was gagging me. I went to Urgent Care and they tested me for strep. They gave me steroids and a Z-pack. The steroid was injectable. The next morning you feeling okay, but in the evening I went back to Urgent Care because it felt bad again. They gave me 3 more days of prednisone, 20 mg per day. One week later I went to my GP. There was a rash on my neck and my doctor gave me 10 days of Bactrim. On the last day of the medicine, I developed a painful sinus headache. I went back to see my GP 2 days later and they treated me for the sinus pressure/headache. They gave me a prescription for 5 days of another antibiotic. I went back a week later and they stopped giving me antibiotics because it had been 20 days. They told me to get over the counter nose spray and told me to clean my nose with saline. Last week I went to an ENT. They told me to continue with the saline and the nose spray.

Write-up: I felt fine for the first week and the half. All of the sudden, I felt dizzy. The room started spinning and I felt nauseous. I called my husband and we went to the urgent care near us. I couldn''t even sit in the chair, because I felt like I was falling. I had to be taken away, because I began vomiting everywhere. They took me into a room and I continued to vomit. I kept my eyes closed, because the room was spinning and I was sensitive to light. They first gave me an oral anti-nausea pill. They waited about 45 minutes. The pill didn''t help. I was then given an IV of stronger medications. I stayed for about 4 hours. They were able to stabilize the vomiting, but not the vertigo. I went home and never got out of bed for 2 weeks and never got out of bed except to see doctors. I could not walk on my own. I was referred to a new ENT and I took 10 days of antibiotics. They thought at first I might have an inner ear infection. The 10 days went by and there was no improvement. I was still bedridden. I still had random vertigo attacks that lasted from 1 hour to 4 hours. I was given an anti-nausea pill for vertigo and Valium (which I didn''t really take). I was given a 10-day dose of prednisone, which did not help. After the prednisone did not work. I continued having continued pressure, ringing, fullness, and deafness in the left ear, which started on 4/22, and this is what caused of the vertigo. After I finished the prednisone didn''t help, a second ENT suggested I undergo a direct injection of steroids in the left inner ear on 4/29. The deafness, ringing, and pressure got worse, and the vertigo became more frequent. I was then sent to a cardiologist to make sure I didn''t have a virus. An EKG was done, and I went home with a wearable heart monitor. My heart is fine and has nothing to do with my symptoms. I was then sent to an ear specialist. I also saw a neurologist, because I had vestibular migraine in the inner ear, which contributed to the vertigo attacks. The neurologist put me on Inderal to try to calm the brain stem to calm the loop in my inner ear. I still have constant ringing and fullness in my ear and nearly complete deafness in the left ear, but the vertigo and vomiting are gone. I''m on a low-sodium diet and take diuretics to keep the fluid down. The neurologist did 50 Botox injections on my scalp in late April or early May, and this didn''t help.

Write-up: Patient was experiencing symptoms after 10 minutes of receiving J&J vaccine. He passed out for 5 minutes without any verbal response; with signs of seizure. He has a history of the same reaction with another vaccine. He got epinephrine 0.3mg IM Lot OFM625 Exp. Nov. 22. The ambulance was called and the patient''s care was transferred to the paramedic. The patient was transferred to the ER.

Write-up: I received my shot on 04/12/2021 in the afternoon and started to feel the effects immediately, It started with sharp shooting head pain and through my neck. I stayed at the clinic for like 30 minutes and on the drive home extreme fatigue started to set in. By the time I got home the fatigue was the main symptom that lasted about 24 hours. The next morning I noticed edema in my right fore arm where I had a prior injury from 2012. The extreme fatigue lasted for about 2 weeks before I was able to do my job again. The edema kept getting worse until the day I went to the emergency room. I also had sharp shooting head pain, it felt like electricity going through my head and that continued for about 7 weeks. At the emergency room they just gave me fluids and that was it. The hospital did bloodwork and did an EKG. Nothing abnormal was found. I was told if things got worse that I should come back. The fluids helped and it took about 3 weeks for the edema to go away. At some point in the process of my events, I had eye palsy that also lasted about 7 weeks. It has been about 8-12 weeks, I think I am better, but still feel fatigue, but I am not certain that is from the above mention events, it could be from the sleep apnea.

Write-up: At the time of vaccination I had a feeling of numbness in my lips and tongue. Woke up at 4am with severe vomiting and diarreha. The ER stated to follow up with my PCP. Follow up visit my heart rate was 120. I was put on a heart monitor which results came back inconclusive. Been referred to see a cardiologist. Awaiting appointment for a future date.

Write-up: after 5-6 minutes post vaccination while sitting in waiting / monitoring area, patient got very dizzy and fell off her chair on to the ground. she immediately got up with help from staff and sat back on the chair. paramedics were called on her request and after monitoring and running diagnostics patient asked to be taken to emergency room to be further monitored because she felt dizzy and had bumped her head when she fell off the chair. she returned a few hours later on her own after getting discharged from the hospital to get her vaccination card.

Write-up: Twitching, convulsing for couple of seconds. Then patient fainted for about a couple of seconds and twitched again for another seconds (less than first onset). Patient was confused and disoriented for a couple of seconds after waking up. We had emergency allergy kit ready/ benadryl injection ready/ and called 911 as soon as she started twitching. Patient was sitting already, so we elevated her legs onto another chair. EMS came and took control of the situation.

Write-up: I experienced on 04-07-2021 after the J&J vaccine, my blood pressure went up to 180/100. I had a bad headache, ringing in my ears so I went to the ER. They put me on the Amlodipine medication. My BP was still going up. My headache and ringing on my ears would not go away. I called my cardiologist to increase the Metoprolol medication. Then he added Spironolactone daily for me to take and my blood pressure came down to 120/70. I was then referred to a kidney and hypertension doctor and he has decreased the Amlodipine and so far, my BP is still stable.

Write-up: I received the vaccine and within the next few days became pregnant about 1 month later - I found out I was pregnant on 5/10. I was hospitalized with an ectopic pregnancy and had emergency surgery to remove the pregnancy and the entire Fallopian tube on 5/12. I would have only been 5.5 weeks pregnant but was showing all other signs of a healthy early pregnancy when I went in for the test on 5.10. When I see the date of conception and date of covid vaccine - they are within a day of one another. I would have died had I not gone into the hospital the day of the pain - the Fallopian tube was rupturing.

Write-up: For the first few days, I had fatigue and I just rested. No medical attention for it. June 7th - severe leg cramping - both calves in the back for about five days. June 21, 02:00 am, I woke up and it felt like something was sitting on my chest and I couldn''t get a regular breath or deep breath. I sought medical treatment at 07:00 - my partner took me to ER - got there at 07:30. They gave me tests to check for blood clots and heart attack. Write-up: From the April 4, 5, 6 I felt very dizzy and out of it. When I got home on the 4th of April I had a metallic taste in my mouth and I started to feel pain in my chest. On my right side, center of my chest and the left side of my chest is where I felt pain. It was the worst pain I felt in my life, I took Tylenol. I called my doctor the next day 4/5/21 and she told me take off for 2 days per protocol for J&J vaccines. I did have a fever on 4/5/21On the 7th of April I had to stop working because my headaches were so bad. The doctor ordered new nebulizer kits for me, I was just using those. My doctor told me not take anything else except for Tylenol. I was having trouble breathing so I went to ER on the 20th of April, they ran tests (xray, blood). Since the ER visit, I have not been able to work full time. I have been feeling very unwell, breathing issues and a bad cough. I have been sick again since 6/2/21, I was given antibiotics and I have also been prescribed prednisone. The prednisone has made a huge difference, I was able to start working more. Once off the prednisone, I started to feel bad again. Since getting my COVID 19 vaccine, I have not been able to work a full work week.

Write-up: Approximately 15 seconds after administration of the vaccine the patient passed out. The patient almost immediately regained consciousness and shook a couple of times. The pt. reported feeling dizzy so I moved her to the waiting area, with her mother, to wait to make sure she was feeling ok and that there were no more adverse reactions. After 5 minutes the pt. reported that her legs felt like jelly, that her head and legs felt like they were burning and that her throat was feeling dry/tight. I gave the pt. 25mg of oral diphenhydramine and immediately called 911. I monitored the patient and her breathing until the paramedics arrived. Upon arrival the paramedics took over the patient care, the pt. was eventually transported to the hospital.

Write-up: To begin, I have always been a healthy, middle-aged male, with no history at all of neurological problems. What follows is a comprehensive summary of all that happened to me shortly (7 days) after getting the Janssen COVID vaccine and the supporting documentation to support the diagnosis of my having developed Guillian-Barre'' Syndrome (GBS) as a direct and obvious consequence of the vaccine. THURSDAY, 5/13/21: Received the J&J vaccine at 10:00 AM (LOT NUMBER 205A21A). FRIDAY - THURSDAY, 5/13 - 5/20/21: No obvious reaction to the vaccine, at all. FRIDAY, 5/21/21: During the day, I noticed deep-muscle pain/soreness in both shoulders (deltoid muscles) that increased throughout the day. It felt as if I had been doing heavy-weight presses with the resulting soreness. I attributed it to a delayed reaction from the vaccine (as I''d heard muscle pain, particularly in the shoulders, is a common reaction symptom), as I had no injury or other possible cause and chose to ignore it. It may have even started a day or two before Friday, but wasn''t of such consequence as for me to recall it; the only vivid memory I have is the discomfort on Friday, 5/21/21. SATURDAY - SUNDAY, 5/22 - 5/23/21: I had continued shoulder pains, but ignored them because they were not acute and just attributed them to a short-term vaccine reaction. MONDAY, 5/24/21: In the evening, while sitting in the bleachers attending a sporting event, I noticed my buttocks was pretty sore. The bleachers are not normally comfortable, but this was a dull, serious ache across my entire buttocks. I didn''t know to what to attribute it (not yet at that time associating it with the shoulder pain), so ignored it. In retrospect, I realized later that the soreness I was feeling at that time was similar to the shoulder pain and was the pain felt from a deep numbness across the buttocks, akin to when a body part (hand, foot, etc) "falls asleep" so deeply that it''s numb, but is still very, very sore. Later that night, my upper back began to feel sore, from shoulder to shoulder, down to my mid-back. As the night progressed, this pain increased exponentially to a "Level 10", leaving me unable to sleep but in 15-minute increments, wandering the house to find a spot -- sofa, chair, floor, anywhere -- to give me some comfort to sleep; I slept 2 hours, total, that night. The pain across my back felt equivalent to someone beating me with a baseball bat all over my upper back -- it wasn''t an acute, localized pain, but very deep and broad across the area. Normally, Aleve quickly resolves any pain issues I have, but overnight, it did nothing to reduce the pain. TUESDAY, 5/25/21: I began to piggyback Aleve and Extra Strength Tylenol, and that seemed to help abate a little of the most intense pain, but it was still painful and precluded sleep and much of my working. I think it was at this time that I began to notice the pain and numbness going beyond my buttocks, down the backs of my legs, and into my feet, leaving my feet (and toes, in particular) numb and tingling. I also began to notice a problem with balance and gross motor controls (stumbling when walking, nearly falling several times in the shower) -- all symptoms I have never experienced in any capacity. Additionally, I had considerable pain down the backs of my legs, leading me to feel as if my calves and hamstrings had been through a heavy workout the day before and desperately needed stretching -- however, any attempts to do so led to excruciating, stabbing pains in those muscles. I also noticed that the pain in my upper back was becoming more acute and localized in my cervical vertebrae area. Having taken care of a father who has had many back surgeries and suspecting I have my own degenerative cervical disc issues (I''ve had past neck pains), I began to believe that all the pain and numbness throughout my body might be due to a cervical issue (e.g., bulging disc, stenosis, etc), and so tried to treat the area as such, with hot shower, heating pad, better neck support, etc. I continued to piggyback the two pain meds that day, but the upper back and neck pain and numbness and gross motor issues came back in full that night, again leaving me no relief for sleep -- I could only doze in 15-minute increments and found no position or place to sleep that was comfortable; I was in tremendous pain 10) across my entire back and down my legs. By 2:00 AM, I knew there was going to be no relief, and I couldn''t wait until the morning; I drove myself to the nearby ER, again still believing/expecting at the time that this was a spinal issue. In the ER, I was met by a PA who reviewed my situation, determined that cervical imaging was unnecessary, and prescribed Percocet. My blood pressure was recorded at 160/94. I was released after two hours and returned home. Despite the meds, I was still in considerable pain that night and found myself unable to sleep in more than 10- to 15-minute increments. WEDNESDAY, 5/26/21: During the day, the Percocet seemed to abate the pain, somewhat, although it tended to flare up much more at night during the prior 2 nights, anyway. However, I noticed I had an upset stomach by midday, and as the day progressed, the pain in my abdomen grew considerably. By the evening, the pain across my abdomen was unbearable, as bad as it was in my back and legs. Late in the evening, I researched Percocet and saw that abdominal pain and nausea are two prominent side effects. I stopped taking the pain medication immediately, knowing that my back pain would return in full through the night; again, I slept in 15-minute increments in considerable back and abdominal pain for a total of about 3 hours. It was also during the day Wednesday that I noticed considerably worse gross motor skills (e.g., walking, ascending/descending stairs, standing up straight, writing), balance, and increased numbness and tingling in my legs, feet, and hands. At one point, I tried to walk by my daughter in a hallway in the house, and in trying to go around her, I fell against the wall. She thought I was joking; I wasn''t. It happened several more times that week. Also, this day or the next, I started noticing additional problems with urinating and defecating. It felt as though I had no control over my abdominal muscles, so could not strain or push when going to the bathroom. When trying to have a bowel movement -- and I knew needed to -- I felt as though I had no control to push; when urinating, I had no muscle support for pushing out the urine. THURSDAY, 5/27/21: Still believing these pain, numbness, and other neurological, problems to be related to cervical vertebrae/disc issues, I had an appointment with a spine orthopedist. He ordered cervical X-Rays and confirmed that I do have disc compression/degeneration, but not excessive and nothing that could explain the pain and numbness persisting all over my back, down to my hands, across my buttocks, and down my legs to my feet. The message: the cervical discs were not the problem. This is when I reconsidered all I had been through, tying back to the initial pain I felt in my shoulders the prior Friday. This is when I realized that my body had been reacting to the vaccine. Never before in my life had I had neurological problem. Never before had I experienced pain, numbness, and tingling across my entire body that way, so until that moment, I hadn''t considered the vaccine to be a possible vector for all that was going on with me. The coincidence of these events was FAR too great to be anything but a causal relationship. The likelihood of these symptoms arising on their own is infinitesimal in comparison to a causal effect. The vaccine had triggered a serious and painful neurological response in my body. The orthopedist, knowing I had responded poorly to the Percocet, prescribed oxycodone. I took that pain medicine that day and into the evening, but once again, my stomach and abdomen began to bother me and the abdominal pain returned. Once again, I looked at the side effects of the medicine, and the first two I saw were stomach pain and nausea. I stopped taking the pain medicine immediately. That night, I slept a total of 2.5 hours. FRIDAY, 5/28/21: I was still in considerable pain, and that combined with very little sleep (and none of it good) for 4 days led me to having trouble thinking clearly. My wife insisted I needed to go back to the ER in the morning to see what could be going on. My blood pressure upon being seen was initially approximately 185/140, but what was finally recorded after a second test was 191/112. I waited to be seen for about 2 hours, but I finally met with the ER physician and described my history of problems that week, including my then-belief that this was a vaccine reaction. She ordered full bloodwork, chest X-Ray and CT scans (without contrast) of lumbar and thoracic spine and CT scan with contrast of chest/abdomen. Nothing strikingly abnormal was found in these scans, but doctors (ER physician and neurologist) noted bliteral weakness in lower and upper body. The doctors wanted to order MRIs, but I am unable to have an MRI due to a piece of metal in my left eye. The morphine I was administered helped reduce my pain and my blood pressure upon leaving had abated, somewhat (don''t recall my final BP). Both doctors seemed not to believe that these symptoms could be the result of an adverse vaccine reaction, but they were unable to explain a cause. On my inquiry, at my friend''s recommendation I ask to verify, the neurologist disagreed that these were symptoms of a presentation of Guillian-Barre'' Syndrome (GBS). The ER doctor asked if I was interested in being admitted, but I felt some of my more extreme symptoms (blood pressure and pain -- thanks to the morphine) had abated sufficiently to allow me to leave. After 8 hours in the ER, I left and returned home. My numbness, tingling, some pain, balance, and motor skills issues persisted, but were blunted somewhat (probably from the morphine?). It was either this day or the next, that I was standing outside, and knowing I had problems walking and with balance, I decided to try to run a few steps to see how weak my legs were and bad my motor skills and balance were. Immediately, it was clear that things were worse than I had considered. I had very poor control of my legs -- they flew off to the sides, out of control, as I tried to step; I stumbled a couple times and almost fell; and I couldn''t proceed in a straight line, veering off at an angle. I earnestly tried several times, with the same results each time. That night, while the pain was still there, it was tolerable enough that I was able to sleep for a total of about 5 hours. SATURDAY, 5/29/21: Early in the AM, I noticed I was unable to smile. Having a friend who had had Bell''s Palsy, I recognized the signs immediately. The left side of my face sagged, I was unable to smile (left side of my mouth sagged), and I was unable to close my left eye. My wife took me to the ER, and we were met by the same ER physician who had seen me the day before. She was shocked about my return so soon after my prior visit. My blood pressure, on being admitted, was 151/102, but as with my other visits, asymptomatic other than the problems described before (pain, numbness, and tingling, in my back, arms, legs, and abdomen; gross motor movement; lack of coordination; and lack of clarity in thinking); no other signs of infection or illness. The Dr ordered a CT scan (without contrast) of my head and other bloodwork. Of note, the CK came back at 712 U/L. The CT scan came back normal. Again, I was administered morphine, which abated my pain and allowed me to relax. After 3 hours, I was released, with prescriptions for Amlodipine for the high blood pressure and Prednisone and Valtrex (Valacyclovir) for the Bell''s Palsy. The morphine helped reduce my back and abdominal pain and give me some comfort, such that I could sleep another 5 hours that night, although all other symptoms persisted. SUNDAY - WEDNESDAY, 5/30/21 - 6/2/21: I continued to experience the numbness and tingling in my back (primarily), abdomen, legs/feet, and arms/hands, but my severe pain began to abate, such that I could sleep a bit more easily. I taped my eye shut at night, as my Bell''s Palsy eyelid would not close fully, and it teared constantly. WEDNESDAY, 6/2/21: Not having a GP, I finally found an Internist to review my case and symptoms, but primarily to extend my meds (Blood Pressure and antiviral and steroid for the Bell''s Palsy), as the ER doctor had only given me a 5-day prescription. The doctor review my situation and continued the meds. Having been prompted regularly by my friend to confirm a GBS diagnosis, I asked this doctor about a possible GBS-vaccine link and after a cursory review, he said he did not think so. THURSDAY, 6/3/21: With the weakness in my left eyelid, my desire to find a resolution to it, and my concerns about its effect on my vision, I had an appointment with my opthamologist, who recommended eye drops until the eye improved and instructed me to stop taping the eye shut. She noted no other problems with my vision. FRIDAY - MONDAY, 6/4/21 - 6/7/21: During this time, the medicines for my Bell''s Palsy seemed to take effect, and the paralysis symptoms abated mostly (with some residual weakness in my left eyelid). I continued to experience the other neurological symptoms, however. TUESDAY, 6/8/21: Having been unable to get a timely appointment with a trusted neurologist, a friend with a contact at medical school head of neurology was able to get me an appointment with a neurology faculty member. I met with Dr. that afternoon, and he was the first to really listen to the symptoms I had and understand some of the possible triggers and consequences of them. I started the conversation explaining that I''m a reasonable, rational, and science-based person, an engineer by training and career, and that I firmly believed all the symptoms I was presenting were a direct consequence of my getting the vaccine. Additionally, at my friend''s continued urging, I asked if these symptoms could point to a Guillian-Barre'' diagnosis. He was at first skeptical of my proposition of the vaccine cause, but was quick to say he was glad I had raised the question of GBS, because he had that diagnosis in mind, as I discussed my symptoms. He believed that the symptoms I had and the way and speed with which they had progressed pointed to a likely GBS diagnosis. He scheduled a follow-on appointment to have an Electromyogram (EMG) and Nerve Conduction Study to assess the extent of the neurological damage and prognosis for resolution, as I am unable to get an MRI and he seemed to feel a lumbar puncture was unnecessary. FRIDAY, 6/11/21: I had my scheduled EMG and Nerve Conduction Study with Dr. The results showed some abnormal neuropathy, but mostly normal in my extremities and indicative of some recovery from symptoms. Although I still had considerable numbness, tingling, and some pain down my legs into my feet and down my arms into my hands, my most prevalent issues at that point were numbness and tingling across my entire upper back and abdomen, but the doctor indicated these tests don''t identify issues very well, that they are better designed for testing neurological issues in the extremities. We agreed to reconvene over the phone in several weeks, unless problems worsened. TUESDAY, 6/29/21: During a follow-up appointment with the doctor, I indicated that some of my neurological issues were abating and that my blood pressure had returned to near-normal range. My energy was returning to a degree and my motor skills and balance and control had improved. However, I continued to have significant numbness and tingling across my back and abdomen, and of greater concern, I had increasing tingling and pain going down my left arm to my index finger when I stretched out my arm. This was a new symptom and increased in pain in frequency in the following days and raises the question of what other symptoms might arise. AS OF 7/8/21: CURRENT STATUS: I have continued numbness and tingling in my feet and hands and back and abdomen, although they are reduced from their earlier state. However, I have increased shooting pain in my left index finger when stretching out my left arm. Additionally, I have a mild increase in weakness in my left eyelid, after prior improvement from the Bell''s Palsy. My dangerously high blood pressure at the early onset of these symptoms has returned to near-normal levels, although my GP continues to keep on blood pressure medicine (Amlodopine) for now. Finally, a lot of the issues I had thinking clearly and regularly making mental mistakes, all noticeable and commented on by my family, have lessened, although I still find myself making mental mistakes and forgetting information that would have been MUCH less likely prior to this adverse GBS event caused by the J&J vaccine.

Write-up: BLOOD CLOTS (RIGHT HAND AND LEFT LOWER LEG); BLOOD CLOT IN THE LUNGS; This spontaneous report received from a patient concerned a 70 year old male. The patient''s height, and weight were not reported. The patient''s past medical history included lung cancer, tumor removal surgery, four chemotherapy sessions, and surgery to remove the blood clots, and concurrent conditions included non-smoker, and other pre-existing medical conditions included patient had no known allergies. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 203A21A, expiry: unknown) dose was not reported, 1 total administered on 11-MAY-2021 for prophylactic vaccination. No concomitant medications were reported. On 11-MAY-2021, the patient developed blood clot in the lungs was reported. On 14-MAY-2021, (three days after) the patient experienced shortness of breath was reported. On 20-MAY-2021 went to Emergency room (ER) and was subsequently admitted to Intensive Care Unit (ICU). On 21-MAY-2021, the patient underwent through surgery to remove the blood clots on the lungs along with this on same day patient developed swelling of left leg and right arm was reported. On 22-MAY-2021, the patient developed blood clot right hand and left lower leg. On 29-JUN-2021, patient underwent through laboratory test included: Computed tomography (CAT) scan revealed negative for blood clots. It was also stated that patient was on Eliquis (apixaban) for anticoagulation due to blood clots. Treatment medications (dates unspecified) included: apixaban. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient recovered from blood clot in the lungs on 21-MAY-2021, and had not recovered from blood clots (right hand and left lower leg). Write-up: So around the 27th of April, that week, I started having some symptoms. It started out with a posterior headache that became bad and debilitating. I also described the next as a sore throat but it was pain at the front of the neck. I had fever and chills, kind of flu like symptoms. I went to the ER on May 8, 2021. Once the results came back on May 12, 2021 and they said I had group c strep so I got antibiotics - Amoxicillin. I was still feeling terrible so I went to my PCD and she switched me to a z-pac. So around the 18th of May I went in to see her again and she told me to finish the Z-pac and they ran blood tests. A couple days later it showed I was in hyperthyroidism and I lost about ten pounds and that is significant for me. On June 16, 2021 I saw my endocrinologist. I got another blood test on June 29, 2021. Now I am clearly in Hypothyroidism, I feel a little bit better. My HR was 124 resting so I was also put on 25MG of Metoprolol, but now my heart rate is under control. I mainly talked to my Endo about this, but he said it may have better caused by the Janssen vaccine.

Write-up: On 7/9/2021, the patient received her dose of Janssen COVID-19 vaccine a little after 0900AM. A couple of minutes after receiving the shot, the patient began to complain about dizziness. The patient was given some water which alleviate some of her symptoms but she remained dizzy. Then the patient was given a lollipop after she stated she had not eaten breakfast and did not have any food allergies. Meanwhile, her vitals were taken: Blood pressure 143/90, respiratory rate 16 breaths/min, heart rate 68, and oxygen saturation 100% on room air. The site lead contacted emergency medical services - her symptoms progressively worsened - she was experiencing worse dizziness, chills, palpitations, and she felt tired. A second set of vitals were taken and her heart rate was 76 and oxygen saturation 100%. The emergency medical services arrived at the site and the patient was taken to the hospital.

Write-up: Right after you received the vaccine on 03/31/2021, it was painful, but I thought it was just a shot pain, put I thought it was extremely painful. On my drive home it kept hurting. Finally on 04/17/2021, I was concerned because that was the time the media was mentioning blood clots from the vaccine. Because my arm, my shoulder hurt, and under my arm still continued to hurt I went into the ER. The doctor did not feel any abnormality in my arm. Till this day I still feel soreness in your arm and shoulder and under my arm. I can''t extend my arm or use my arm. I use it cautiously, if I use it. I have had 3 or 4 visits with my doctor and she put me on methylpred 4 mg/ triage pack. It helped somewhat with the pain, but I still had soreness. So she prescribed Methocarban 500 mg. I just took it for 3 days. and it upset my stomach and gave me diarrhea . Now I have an MRI that is scheduled, but I don''t know when at this point. I have to see if my insurance will cover it. I''m still pursuing to see why my soreness is persisting.

I GAVE HER SOME WATER AND AT THAT POINT SHE LOST CONSCIOUSNESS FOR APPROX 30 SECONDS. 911 WAS CALLED. SHE COMPLAINED OF HAVING BLURRY VISION AND FEELING MALASE. EMTS AND FIRE DEPT. ARRIVED. HOOKED HER UP TO SEVERAL MONITORS. THE ONLY ONE THAT WAS OF CONCERN WAS HER BLOOD PRESSURE OF ABOUT 84/43. IT WAS RECOMMEDED THAT SHE BE TRANSPORTED AS SHE LIVES ALONE. SHE HAD ONLY EATEN EGGS AND POTATOES AT 11AM AND WAS FOSTERING 3 KITTENS. RAN ERRANDS ON A HOT DAY AND SAID SHE WAS HYDRATED.

Write-up: I woke on 4/14 at about 7AM. The initial feeling I had was a little pain in the upper left joint between the hip and leg. It wasn''t severe, but it was uncomfortable. I thought that maybe it was a side effect that would go away. The very next day, it graduated to a very tingly feeling throughout my entire body. My skin became very sensitive, tingly. I felt some numbness throughout, all the way from the soles of my feet to the top of my head, and even on my lips and tongue. This has continued to this very day. I then started to get inflammation all over my body the day after the tingling started; the inflammation started in my thighs. I still have both the tingling and numbness; the inflammation comes and goes and moves around. It has become painful. The inflammation is almost constantly present in my thighs and arms, though some times are worse than others. When I move around or walk around, it can kind of dissipate. As soon as I try to relax, though, the tingling and numbness gets really bad. It''s especially bad when I try to sleep. It feels like my whole body is going to sleep, and it''s scary. Last week, it was the worst ever. My torso began shaking uncontrollably during my sleep, and it woke me up. We went to the ER. It''s gotten better this week, and I have been able to get some sleep, though.

Write-up: Patient reported to my office complaining of eyes sporadically tearing, feeling dry, and reported that she had been having trouble since being hospitalized for her Hb dropping to 5 and that she has been having trouble with Hb stability ever since. Write-up: Immediately 10 minutes after receiving the vaccination, I started sweating really bad, I got a metal taste in my mouth. The next morning, I woke up with a huge migraine, couldn''t concentrate, pain in my arm, and my whole body was just very hot. Over the next few months, I had developed swelling from my toes to my mid-calf, which went into my thigh the next week. It got to where I could no longer fit into my jeans. My whole body turned red. I went to the urgent care, which they immediately sent me to the ER. They did an ultrasound all over my body to test out blood clots, they took blood work. They ruled out having a heart attack and blood clots. They sent me home. A Few weeks later, I was at work and the pants I was wearing were cutting into my abdomen. I went to my PCP; I was so swollen head to toe. He put me on a steroid. PCP did blood work and everything is still going on and they still can''t figure out why and having more symptoms on top of swelling. I have been on many different kinds of steroids. Had hallucinations, my PCP told me to go to the ER again. My breasts got super enlarged, the doctor I got at the ER still didn''t figure anything out. They did a CT, Blood work, EKG, more ultrasounds. They then sent me to another ER, they did another CT, my brain was smaller than normal, my eyes were in the wrong place. To this day I am still experiencing all of these symptoms.

Write-up: After Janssen vaccine shot, patient was asked to sit and wait for 15 minutes. During the wait time patient was walking around and felt fine. About 10 minutes later patient looked a little pale. Told him to sit down and gave some water and a power bar. Patient had a seizure and past out for a few second. Patient regain consciousness and we lay him down on the ground. Breathing was normal. We called 911 and when EMT checked his vitals, his blood pressure was low. Patient was then taken to hospital for further monitoring.

Write-up: 6/25/21 Received vaccine. 6/26/21 Started feeling sick, unwell. Patient stated he almost fell over. EMT''s at game stated it was because his immune system was working in overdrive trying to create immunity to pneumovax and Covid. On 6/27/21 in am, Patient went to hospital because his shoulder hurt. He was given some medication cocktail by hospital and went home same day. On 6/27/21 in pm the redness in his arm started and got worse and he started sweating really bad. Write-up: BRAIN ANEURYSM; BRAIN DEAD; BLOOD CLOT; FALL; NEEDLE BROKE OFF THE SYRINGE; NEEDLE BROKE OFF THE SYRINGE UNSURE OF THE FULL VOLUME THAT WAS INJECTED; This spontaneous report received from a consumer concerned a 57 year old White Hispanic/Latino male.. The patient''s height, and weight were not reported. The patient''s concurrent conditions included blind, diabetes, cancer, congestive heart failure, and penicillin allergy. Nurse came to patients house to administer the vaccine due to patient disability, patient usually does have a home aid. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: Unknown) dose was not reported, 1 total, start therapy date were not reported for prophylactic vaccination. The batch number was not reported and has been requested. No concomitant medications were reported. Upon administering the vaccine, the needle broke off the syringe, unsure of the full volume that was injected. On 21-MAY-2021 patient experienced dizziness, vomiting, and fall. Blood pressure and sugar was fine after the wife had checked it (BP 120/75, glucose 112). They called the ambulance because the symptoms were unrelenting and a headache began on 21-MAY-2021. Patient required and emergency surgery to relieve pressure on the brain, drilling of the skull where ultimately patient was pronounced brain dead. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient died of brain aneurysm due to blood clot and brain dead on 25-MAY-2021, and the outcome of fall, blood clot, needle broke off the syringe, needle broke off the syringe unsure of the full volume that was injected was not reported. She was not pregnant at the time of report. She had no drug abuse or illicit drug usage. The patient was healthy and not taking any medication regularly. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 202/ZA21A, expiry: unknown) dose was not reported, 1 total, administered on 28-JUN-2021 at left arm for prophylactic vaccination. (The patient was not sure about 3rd letter of batch number as ?2'' or ?Z''). No concomitant medications were reported. On 28-JUN-2021, within 30 second after receiving vaccine the patient did not feel good. She asked for water to her son while waiting she fainted. She urinated. She woke up after a while, felt confused or could not figure out what was going on. She was lethargic. She felt ?so hot'' for 30 minutes. She was sweating profusely. She sat down on the floor and felt sick, retching several times and finally vomiting/ puke. She could not hold her body weight and could not open eyes. Patient was taken to the hospital via ambulance. In ambulance patient received IV anti-nausea medication. In emergency room (ER) bloodwork up and electrocardiogram (ECG) was done and both were normal and she had really low blood pressure (did not recall the exact measurements). She felt freezing cold and needed warm blankets until her body temperature finally regulated, an hour later. After 3-4 hours she felt better and went home. She has not sought any further medical care after this occurrence. She was hospitalized for 3 hours. She was feeling better at the time of this report. Write-up: Twelve hours after vaccine, he developed severely sore arm, shoulders and down back. This was followed by severe vomiting, to which he lost two pounds in 4 hours. Arriving at the ER at 5:45 AM, he had horrible stomach pain, (we could see his stomach move), nausea, vomiting. His hands, arms and face became fuzzy then numb. He was given multiple medications to ease symptoms, along with IV. He had tests of blood, urine and a CAT scan. This was followed by morphine. Discharge around noon on 7-8-21

He went to the ED and was found to have multiple bilateral pulmonary emboli with signs of right heart strain. It is not clear if the vaccine was the cause of the embolism, but there are no other provoking factors that could be identified. He has been started on anticoagulants with improvement in symptoms, but he has persistent dyspnea with exertion.

The patient''s height, and weight were not reported. The patient''s concurrent conditions included arthritis, penicillin allergy, and allergy to wasps. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 025A21A, expiry: UNKNOWN) dose was not reported, administered on 29-APR-2021 at right arm for prophylactic vaccination. Concomitant medications included naproxen sodium for arthritis, chlorphenamine maleate for itchy skin, cyanocobalamin, and fluticasone propionate. Patient experienced tiredness, some general malaise and slept for most of the next day of vaccination, but did not experienced other significant symptoms after the vaccination. However, on 29-JUN-2021, patient had severe allergic reaction included anaphylaxis and her hand started itching then swelling spread to the hands and face. She had put her hands under cold water and by then they were completely red, Legs then became red, then the nape of the neck began to itch, She was dripping wet with sweat at that point. She then had to immediately get to the bathroom as she voided and vomited for the next three hours. EMS was called at 10 o''clock and was taken to the hospital. At the hospital no medications were given. Vitals were fine, Blood pressure was 128/76 mmHg, pulse was 83 bpm, and oxygen saturation was 97%. Treatment medications (dates unspecified) included: epinephrine. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient recovered from slept for most of the next day on 30-APR-2021, and the outcome of general malaise, anaphylaxis (severe allergic reaction, itching in hand and nape of neck, voided and vomited for three hours, swelling in hands, swelling on face, red hands and leg and dripping wet with sweat) and tiredness was not reported. Write-up: Well we went on vacation and I was 30 weeks pregnant and I had no other symptoms, warnings or anything. I was feeling great in my pregnancy and I woke up in the middle of my water broke. They gave me fluids and magnesium. I went to ER 4/30/2021 and I was there until Tuesday 5/1/2021 in the evening, the baby''s heart rate was not recovering and they had to do an emergency C-section. I have 3 children who I all had all induced, never labored on my own. My first child was 7lbs 5 ounces, my second child was 7lbs 5.5ounces, and the third was 6lbs 9ounces. My fourth (this one) was born at 30 weeks and 6 days and he was in the NICU for 5 days, he was 3lbs 13 ounces. I had all similar pregnancies in regards to my previous ones, my DR had cleared me prior to vacation, they cleared me for travel and to go, so they do not know what caused my water to break. My child was due to be born on August 4th 2021. I followed up with a DR, because we just got back to where we live, just a few days ago. I will see my normal OBGYN on Thursday July 15th 2021.

Write-up: Received Johnson and Johnson vaccine on the 10 th of March, started having flu like symptoms early next Sunday morning, late Monday night my hands began to swell up and hurt when I was laying down. This got progressively worse and I started having pain when breathing, I called my doctor and was told my symptoms were normal for the vaccine and to go back to bed, and call after a day or so if no changes. The next day I called the advice nurse and was advised to go to the urgent care. I didn?t get any information on why this was happening to me, just a bad reaction to the vaccine, I was prescribed medication to help with my breathing, but the pain in my hands now includes my arms, legs and feet. I take a minimum of 600 mg of Advil liquid gels every 5 hours, I?m pretty miserable and cannot do normal life things.

Write-up: On May 13th, I felt my feet go numb and I could not stand. I sat down to immediately find a heat go through my bod - heat to head. I then felt my heart racing. Took my pulse and it was 134. (I have had no heart issues at all previously - normal resting heart rate in the 60s). Three more times this happened, with heat going from feet to head (numbness in feet). Each heart rate was progressively lower with each incidence. Then it stopped. This was in the afternoon and by evening felt ok. The 14th, the same thing happened but only 3 times and with hr incidents starting at 123. Then fine again. The morning of the 15th started again, so I went to urgent care appointment in the afternoon. By this time, my heart rate was up and down.. They ran an EKG and it was fine, just showing rapid heart rate. But I was sent to the ER for further tests. At ER another EKG, chest x-ray and blood work. Only item of significance was an elevated TSH. At this point, my heart rate was going up and down (I had a finger monitor so I could watch it). I''ll note at this point, the heart rate was random but I was able to walk. Over the next week or so, hr got worse and positional. When I stood up, it would sky rocket to the 140s. I became shaky, developed brain fog, nausea, feeling like I was going to fall over and felt only well laying down or sitting. I visited my primary who initially thought anxiety but as symptoms were getting worse (I could no longer do anything: walk farther than from the living room to the kitchen, do simple tasks while standing or sitting - hr would increase while doing activities even while sitting, and no longer drive).My doctor decided it was more than anxiety when I was continuing to have symptoms that looked like POTS. I have been to the cardiologist - she did a 24 holter and EKG - no significance, but decided my heart was not the primary cause of the positional heart rate increase. I am due for an echocardiogram this week - just to make sure but she did not think that was the issue. An MRI this week, recommended by my neurologist. and have done preliminary blood work/urinalysis for possible endocrine issues - all that have come back normal so far (but seeing an endocrinologist next month anyway). I currently am seeing functional neurologist who has diagnosed me with dysautonomia (so far of unknown reasons) - he is helping me with the heart rate issue (POTS) and other symptoms that have developed over the weeks like electrical zaps throughout my body - some sort of neruopathy. I was in great health until this shot. I had not even had a cold since January of 2020. I may have hypothyroidism but it has been managed with some ups and downs in TSH for the last 20 years with no other issues. Some of my doctors do not correlate the shot to my problem but some have. There does not seem to be any other explanation.

Write-up: Within minutes of receiving vaccination, pt stated he was hot and thirsty, became unrepsonsive to verbal stimuli. Pt proceeded to have tonic-clonic seizure lasting 25 seconds. Pt lowered to the ground by staff, monitored VS. No incontinence, no biting of tongue. pt regained consciousness, and oriented x3. VSS. pt reported he felt light-headed. When attempted to place in chair, symptoms returned, and pt was placed back in supine position. EMS arrived, and after VS established, EKG indicated sinus rhythm, placed in chair. Pt stated he did not feel good, and similar symptoms began with eyes rolling back and contractures. Pt taken to hospital.
