Write-up: Vaccinated 1/6/2021. No issues reported per client until day 5, 1/11/2021. Reports lymphadenopathy, axillary area, left side, left breast area and chest. Reports some swelling noted lymph nodes right axillary and sore to touch. Report fever onset 1/11, 1/12, 1/13. Then Thursday 1/14/2021 contacted PCP and was told on 1/15/2021 to see medical evaluation. Reports on 1/15/2021 went to ER and was evaluated. Reports lymphadenopathy and "small P.E." found. Unsure if incidental as history of P.E. 4 years ago. Also reports over 25 years ago had lymphadenopathy from Flu Vaccine and has not had any vaccines prior to this COVID-19 vaccine. Reports discharged same day to home with Topical Benadryl Cream for redness and itching at site of injection which per client also did not start until day #5 after vaccination. Reports no issues prior to day #5 post vaccination. Home and doing better but with continued lymphadenopathy left side axillary, breast and chest area. Denies any follow-up appointments other than a request to talk w/LHD Doctor to see if she should get vaccine #2. This RN will relay to MD for advisement/consult.

Write-up: Dark reddish-purple rash on neck that started to spread to the chest area about 1 hour after the vaccine was given - took 1 Claritin and 1 Benadryl and slept. I woke up a few hours later with chills, headaches and body aches. I took another Benadryl and slept for the rest of the night. The next day, I woke up with the same symptoms, but worse. I was very drowsy and slept all day. I had no appetite. That evening, I took some Tylenol and began to feel better a few hours later. The next day, the symptoms had mostly resolved. A little bit achy and sore but overall, I felt good.

Write-up: Developed chest tightness around right side of chest into back and SOB 50.5 hours after vaccination. Went to local ER and found to have a right lower lobe pulmonary embolism. Treated with Xarelto and sent home with outpatient follow up.

Write-up: Patient had first dose of Moderna vaccine on 12/26, and developed a diffuse red/raised petechiae rash on lower abdominal/groin area and underarms down sides and up near her shoulders. Rash began one week later on 1/3. She called her PCP who recommended to take Benadryl and OTC allergy medicine. Later that night the rash continued to spread so she did go the emergency room at received a prescription for a Medrol taper regimen. Denies any resp distress at that time. Has reported her symptoms

Write-up: Injection sight had remained tender and warm since vaccination on 12-31. After scratching lightly on 1-9 noticed significant increase in heat, redness, itching and swelling. By 1-11 had formed a firm lump slightly larger than a tennis ball - approximately 4 inches in diameter that was hot to the touch, tender and angry red with a purple center. On Tues 1-12 I contacted the local health dept who advised cold compresses and benadryl and if it did not started to shrink to see my pcp. On Wednesday 1 -13 2 more significant misshapen lumps had formed, 1 towards the inside of my arm and another towards the other. The irritation, swelling and redness had also spread while the color was no longer so angry. Due to that on Thursday 1-14 I saw my PCP who diagnosed cellulitis. I was prescribed 2000mg of amoxicillin a day for 7 days and a 5 day course of prednisone 20mg.

Current Illness: Patient was actively being treated for Bipolar disorder with most recent episode depression, Anxiety disorder, ADHD, Oppositional Defiant disorder, Autism Spectrum Disorder, Fetal Alcohol Syndrome, Intermittent Explosive Disorder, a history of Pseudo-seizures, and insomnia. She has Alopecia Areata, and allergic rhinitis, and constipation. She has a history of left eye strabismus and uses glasses. She has not been acutely ill prior to vaccination.
Preexisting Conditions: See above, history also of right radial neck fracture in 2019 with ORIF repair. Living at Center for behavioral management and stabilization

Write-up: One week after the shot (1-14-2021) Patient (19 y.o.)reported side pain and appeared constipated, Laxatives given along with Tylenol, on further assessment Patient was noted to have left leg redness and abdominal fullness. Dr. was updated and we had orders for close monitoring, the next day when she got up, her leg appeared better, and she had passed a small BM, but by lunch she had developed significant pain and edema in her left leg, and the color of her leg was reddened again. She was sent to the emergency room with her symptoms. She was admitted back to our facility yesterday, her diagnoses included Acute provoked left external illiac, femoral, popliteal, and peroneal DVT. Elevated Factor II levels, Elevated APC resistant, May-Thurner Syndrome, history of developmental disabilities, fecal impaction and urinary retention - suspected related to her fecal impaction. Vascular surgery was consulted, and pt. was started on a heparin drip, and mechanical thrombectomy was needed for both legs due to multiple clots. She was started on Eliquis and Plavix, and thigh high compression stockings were ordered, ace wraps being used until these are supplied. Her Fecal impaction was addressed also and the urinary retention resolved.
