Write-up: Patient received the J&J COVID vaccine around 10:00 a.m. In the evening began experiencing subjective fevers, chills, nausea, headache. He took ibuprofen around 4:45 a.m. in the morning and went to work. Symptoms do not improve and patient presented to ED for evaluation. Patient denies chest pain, shortness of breath, vomiting, diarrhea, dysuria. No Hx anaphylaxis. Denies sensation of upper airway stridor or other respiratory symptoms. Past medical history is unremarkable. He is not on home medications. Denies smoking or illicit drug use. Patient was given 1 gm Tylenol, 10 mg metoclopramide, and 1 L NS. Patient improved, and was discharged

Write-up: ED COURSE & MEDICAL DECISION MAKING Patient is a 20 y.o. male who presents emergency department for evaluation of fevers, chills, nausea, and shortness of breath. Symptoms occurred approximately 3 to 4 hours after his Covid vaccination today. He has not felt symptomatic for symptoms related to COVID-19 prior to his vaccination. Nausea was noted without vomiting and he was given Zofran prior to arrival. No objective evidence of anaphylaxis or angioedema was noted at urgent care nor was this noted on physical examination. He does have a fever on arrival with Tylenol taken a little more then an hour prior to arrival. Chest x-ray negative for findings of COVID-19. There was no evidence of pneumonia. His blood counts were otherwise reasonable though he did have relative lymphopenia that can be described in COVID-19 infection. Metabolic profile was otherwise benign. Differential diagnosis of his fever include the possibility of a vaccine reaction though the timing is rather quick for this reaction. Certainly this remains in his differential. His he has history of asthma and felt short of breath he was given Solu-Medrol. He was given Toradol with improvement of symptoms as well as IV fluid. With differential diagnostic considerations that he may have Covid, he was swabbed and tested at bedside. With his well appearance he was ambulatory in the emergency department. He was discharged stable condition with recommendations for Tylenol and ibuprofen in the short-term. Procedures MDM CHIEF COMPLAINT Fever and Chills HPI The history is provided by the patient, medical records and the EMS personnel. Patient is a 20 y.o. male who presents emergency department with fevers, nausea, and shortness of breath. He has generally not felt well over the course of the last 8 hours after receiving his first Covid vaccination this morning at 9 AM. Fevers have been noted. He took Tylenol with 2 tablets an hour prior to arrival. He has had shortness of breath with a history of asthma. He denies productive cough, or chest pain. He denies abdominal pain, or diarrhea. He denies dysuria or frequency or hematuria. Seasonal allergies are noted without significant medication allergies. He denies a sensation of throat closing. He denies tongue or lip swelling. He was seen in urgent care and sent to the emergency department for further evaluation.

Write-up: I got a really bad migraine within 10 minutes and they transported you to the ER. I have had a Stroke in the past so migraines do scare me a bit. In the ER they gave me numerous medications and nothing worked, I was there for about 5 hours before they discharged me. When I got home I went straight to sleep and when I woke up in the morning I was feeling better.

Write-up: I had redness that started small then spread throughout the arm. The diameter of the redness at its worse was about 4 inches. I also had itching, swelling, I have a hard lump on the injection site, the area of the lump is very tender, and the area is warm to touch. I called the advice nurse line and was told it was covid arm. It was treated with warm compresses, Tylenol for pain, antihistamines, and Triderm cream for the itchiness. Now most symptoms have resolved, except for the hard lump on the injection site and mild tenderness.

Write-up: Patient recieved IM injection of the JNJ COVID vaccine in the right arm. Shortly afterwards she became diaphoretic, felt tightness in her throat ''like when I eat shellfish,'' experienced tachycardia and left-sided chest/neck pain. The Diaphoresis and throat symptoms resolved with zyrtec and benadryl. The tachycardia persisted for 4 days with even slight exertion. HR was 119 bpm upon follow up with PCM on 26Mar2021.

Write-up: I received the vaccine at 1:30 pm in the afternoon on 3/22/21 and immediately afterwards I felt fine. I waited my 15 minutes and had no signs of a reaction and was free to go. Later in the afternoon, around 3:15 pm, I began to feel a little fatigued and my arm was sore. Around 6:00 pm that same night, I developed a low grade temp of 99.8, I was very fatigued, had achiness all over, as well as my arm was very warm and tender. I was unable to sleep through the night of 3/22/21 into 3/23/21 and got out of bed around 4: 30 am on 3/23/21 and had to call into work due to the pain. The pain was not an achiness but sharp throughout my whole body. Around 9:00 am, I took a pain reliever and was able to sleep for a few hours. Upon waking up around 1:30-2:00 pm, I was still hurting and was then nauseated as well. I tried to drink water and ate a few crackers to settle my stomach with no relief. At 5:15 pm on 3/23/21, I went to the bathroom and got in the shower to try to help with the pain. After a few minutes in the shower, I became extremely lightheaded and felt like I was going to be sick. I sat down in the bathtub and hollered for my mother to come help me because at this point, I felt like I needed to go to the hospital. Upon sitting down in the bathtub, I began to profusely vomit and started to lose feeling in my legs. My mother entered the bathroom and I explained what was happening and soon after my hands contracted and were stuck in an abnormal position. The pain was unbearable and I was unable to move them from that position. I was unable to get myself out of the bathtub so my mom then called for an ambulance. These symptoms occurred somewhere around 5:20-5:21 pm and EMS was called at 5:22 pm. After a few minutes, I was able to regain movement of my legs but my hands and fingers were still contracted and unmovable. I was able to help my mother enough to put my clothes on and to make my way outside to the front porch to get fresh air. After getting outside, I was accompanied by my mother and my neighbor whom is a nurse. My neighbor looked at my hands and noticed they were discolored and she attempted to straighten out my fingers to no avail. The pain in my fingers was sharp and excruciating but there was also a pins and needle type pain as well. I remained outside until EMS arrived. I was then transported to Hospital where I was seen in the ER for this. Upon arrival to the ER, the physician ordered labs and fluids and stated that apparently he had seen at least 2 other cases that day with a similar reaction to the vaccine. He explained that I was having an adverse reaction to the vaccine and that my hands should return to normal within 24-48 hours. I was then discharged home. As of 3/24/21, I was able to straighten my left hand out and my right hand, my fingers would straighten out, but they would return to a curled resting position. As of 3/25/21, I returned to work and I have some weakness in both hands with a slight discoloration of each hand as well. I am a nurse and work in an office and the physicians in my office commented on the color of skin upon seeing them. I was still very achy and fatigued. My arm was still sore and tender as well. As of today, 3/26/21, I am still achy as well as tired. My right hand has improved some but still returns to a curled resting position. The color of my hands has improved as well. I am a nurse and have seen many things throughout my career. This experience has been absolutely terrifying and I still have no answers as to why this happened to my body. I did my part by being vaccinated and was terrified that I was going to lose my life to it. I pray this does not happen to anyone else.

Diagnostic Lab Data: March 21, 2021 - I was given an IV, and CAT Scan. Blood was drawn and a urine sample taken. The CAT scan nor the blood work showed any abnormalities. Urine results showed dehydration.

Write-up: March 20, 2021, at 9:07 PM - I began having chills that evening, then aches and pains. My husband suggested I take Theraflu Nighttime for the pain and to sleep, which I took. Around 4:42 AM, I woke up to use the restroom. I was drenched so I guess I sweated out a fever. Went back to bed. My husband woke me up and suggested I drink some water. I went to the restroom, still feeling out of it, then made my way to a chair where I sat and down and collapsed. According to my husband he tried several times to revive me and finally I began gasp for air. The paramedics arrived and I was taken to Medical Center.

Write-up: Six days after receiving Janssen vaccine at approximately 4:00 pm, left hand experienced numbness. As the evening wore on, weakness spread through the left side of body and speech began slurring. The next day, after no improvement, went to hospital to find from CT scan that he had suffered a stroke. He had no precursors that would lead to being prone to strokes.

Write-up: 3/11/21 Left side of face went numb, jaw pain, felt like electric bolts through head, involuntary muscle contacting in face (both sides), Jittery, Eye twitching, headache. Ear pain. 3/12 -3/13/21 Face numbness got worse, left side started to droop. Muscle soreness in left arm, headache, tired. 3/14/21 ER visit to rule out Stroke. Diagnosed with Bells Palsey. 3/15-3/17/21 Left side of face still numb, pain/muscle spasms come and go. 3/18/21 Woke up with skin hypersensitive- feels like I have a severe sun burn from my neck to waist area, swollen glands, eye twitching, lips, tongue and throat swelled up. Severe jaw and ear pain, trouble breathing. Face still tingly/numb. Legs became extremely sore and heavy feeling, hard to walk. Called Dr- put on Prednisone (60mg and decreasing) and an anti-viral. 3/19/21 Same symptoms as previous day plus stomach area began to hurt/burning sensation- like all my organs were inflammed. Uncomfortable pain in my right side. Took first dose of meds this am. 3/20/21 Extreme jaw/ear pain, clenching feeling, loss of strength on the left side of my face Symptoms tapered off fir a few days, face numbness decreased and swelling seemed to improve. 3/25/21 Woke up with Skin burning again and glands in neck swollen. Called Dr- placed back on a higher dose of Predisone . At night, throat and tongue swelled up 3/26/21 Tongue and throat swelling, still facial tingling and numbness

Write-up: Patient went home asymptomatic 15 minutes after receiving the vaccine. She returned to the vaccination center 30 minutes later with complaints of repeated loose to watery bowel movements. Upon arrival she appeared pale and complained of dizziness and of being nauseated. No vomiting occurred but she was retching and burping loudly. After arriving to the center she had one more episode of diarrhea. She was afebrile, pale, had a strong regular pulse, a regular heart rhythm, clear bilateral breath sounds and complained of feeling dizzy. No abdominal pain.

Write-up: Client lost consciousness and became unresponsive after vaccination in observation area of clinic. Staff nurse in observation area responded, states client was diaphoretic and pale, had weak pulse, and did not observe chest rise. RN administered epi-pen 0.15mg in left thigh, 911 was called. Lead nurse onsite and nurse manager responded after epi-pen administration, client was rousable and responding minimally to questions. BP 124/60, HR 50-60, o2sat 98-100%. Client was intermittently drowsy, remained diaphoretic and pale, oriented x4. 911 arrived 7 minutes after call, assessed pt and transported to hospital.

Write-up: Pt was being obs starting 5:25pm, post J7J vax. After 15 min obs, at 5:40pm, she fell out of her chair and onto floor. Pt''s eyes were closed, not responsive, not breathing, and stiff. Immediately 911 initiated, VS initiated, Pt was turned to Right side-lying position. Stimuli initiated (sternal rub, verbal communication). Pt opened eyes after 10 secs and started breathing. at 5:41pm/ Pt was able to answer all questions Alert & Oriented x4. Pt supine, ice packs applied to neck and chest. 5:42pm: 1st VS = BP 110/60, HR 58, RR 18, O2SAT 98%. 5:44pm: Pt vomited into trashcan, yellow emesis. Pt said that last time she ate was 3:30pm. 5:47pm: 2nd set VS = BP 115/60, HR 75, RR 18,, O2SAT 99% 5:50pm: Pt sitting upright position on floor. Pt said she "feels warm" 5:53pm: Fire dept arrived 5:55pm: EMS arrived. 3rd Set VS = BP 118/60, HR 88, RR 18, O2SAT 99%. Pt discloased that she has "fainted once when I was little at a concert". 5:58p: Pt left ambulatory with EMS & her friend.

Write-up: Hour 2: numbness in right jaw, numbness in right leg, and abdominal cramping Hour 5: continued numbness in right leg, groin pain Hour 8: chills, shakes lasting 45 minutes), followed by fever for several hours, highest recorded temp of 103.8, took ibuprofen to reduce fever, numbness on right side continued Day 2: numbness on right side, low grade fever, patient stayed in bed and continued taking fever reducers Day 3: numbness on right side, now including arm and back, and abdomen pain brought patient to primary care doctor; strength, pinprick, and vibration tests came back normal. Patient was instructed to return home. That evening patient began experiencing numbness on left side in her ankles, and left calf. Numbness is described as electric pulsation, tingling, and dull feeling. Patient can walk, lift limbs, and control muscles. Patient then experienced breathing difficulties, tightness in chest, sharp pains in chest. Abdomen pain intensified. Mouth numbness and difficulty swallowing. Dizziness grew stronger when patient sat or stood. Day 4: breathing issues and chest pains resolved. Numbness throughout body continued and is described as moving in intensity from one area to another. Often intense in the legs, ?feeling like weights?. Day 5: patient goes to ER seeking answers. ER preforms routine lab work and collects a urine sample to test hydration and electrolytes. Patient shows some signs of dehydration. IV is given. Referred to neurologist.

Write-up: Patient had 5 days of bruising and left leg swelling prior to presenting to the emergency department 3/26/21, where she was found to have an extensive occlusive DVT of left lower extremity as well as thrombocytopenia of 15,000. That evening she was taken to the Cath Lab and had an IVC filter placed. The next day patient began to have paresthesias and discoloration of the right lower extremity. Ultrasound showed high-grade occlusion of the right proximal superficial femoral artery. Patient was pre-treated with platelets and taken back to Cath Lab emergently. In addition to the right SFA there is also thrombotic occlusion of bilateral iliacs. Patient had bilateral thrombectomy and bilateral common iliac stent placement. The following day developed gross hematuria.

Diagnostic Lab Data: I sought medical treatment at Emergency immediatelyand was treated with a steroid shot. By about 7pm my symptoms were resolving and left the ER about 8pm. At 8:30 pm I needed more benadryl as my symptoms returned. I continued to have mild swelling in tongue and mouth for 8 days. During that time I continued on the steroids and benadryl. I did not seek further medical care.

Write-up: About 35 minutes after injection had swelling of tongue and face, tingling lips and pressure felling just above collar bone on throat. I took benadryl as soon as I felt tongue swelling , but it did not stop the advancement of more swelling in my tongue. Eventually I got throbbing jaw pain from the pressure of my tongue.

Write-up: On Saturday 3/20 I experienced the usual symptoms that were noted. On Monday 3/22 I began to get very dizzy with a heart rate of 150bpm. My blood pressure went up to 210/138 which is extremely abnormal for me. I began to tremor uncontrollably. I thought it was an anxiety attack so I took my Xanax. .50mg. That did not work at all. I experienced nausea and stomach pain. I called 911 and was taken to the hospital where after 4 hours I started feeling better. Tuesday I continued to have tremors. Wednesday the rapid heart rate, stomach pain, and nausea returned. The rest of the week did not get any better. On Saturday 3/27 I slept all day long which again is not normal for me at all. At this point we are 1 full week into the vaccine administration. On Sunday 3/28 I bagan to have the same feeling as the prior Tuesday. Sweating, couldn''t breath, heart rate hit 170 this time. BP was the same, tremors, stomach pain, nausea, dizzyness, and this time my Carbon Dioxide level dropped to 24. This morning 3/29 I woke up with extreme tremors. No dizzyness.

Write-up: Suspected pneumonitis. Patient presented on day following vaccination as shortness of breath, nausea, and diarrhea were not abating with home management. They had started later in the day after the vaccination. At ER admission - SpO2 = 80% room air. Nasal cannula was applied and SpO2 recovered quickly. BNP was elevated so acute exacerbation of heart failure was suspected and treated. However, patient did not improve with diuresis. Viral workup and bacterial cause workup negative. No acute cardiac events discovered. Patient continued to decline. Transferred to ICU on 3-19-21 to use BiPAP with 65% FiO2 and able to take breaks to 15L nasal cannula. Borderline blood pressures at this time. Infectious disease, pulmonology, cardiology, nephrology, and critical care anesthesia services evaluated and ruled out multiple possible diagnoses and patient continued to decline. Diagnostic bronchoscopy on 3-22-21 with BAL lead to patient ventilator dependent with profound hypotension. Aggressive ICU management ensued. Bronchoscopy very clean with no real secretions - in line with CT evidence of pneumonitis. Despite aggressive care and maximized pressor support, patient expired on 3-29-21 when family chose to withdraw care as blood pressure declined again.

Write-up: Patient started to feel itchy, had "burning" sensation shortly after injection. Patient was given initial dose of Benadryl and walked to the on-site urgent care clinic. In urgent care following IVs were administered: Pepcid, Benadryl, Solumedrol. Patient was monitored for about 2 hours. Symptoms mostly resolved in that time, still had some residual itching but felt comfortable for discharge. Discharged with outpatient medications of prednisone, famotidine, and Benadryl.

Write-up: Woke up with discomfort/pain in my breastbone area. It spread to my esophageal tube (extremely tender), a couple hours later my throat became strep-like sore, could not take a full breath, and throat swelling impacted my ability to talk clearly. About 12 hours later, I had facial swelling. One full day later, a chain-like rash appeared on my arms, chest, thighs, and feet. Each subsequent day is spread to a new body part (but never occurred on my face), I had to take Benadryl EVERY 4-5 hours to keep everything at bay. Three days into the event my neck and torso skin were extremely painful to touch. Most days since, I am exhausted and get winded easily. Today is 30MAR21 and it''s happening again as it began on 21MAR21. I took two Benadryl in the hopes I will not have to go to ER again.

Write-up: Patient started to experience dizziness a couple seconds after the shot. Provided patient with water and he waited another 30-45 minutes and he started to experience nausea and chest pain that was rated a 3/10. 911 was called and his blood pressure was measured at 210/110 (previous blood pressure was 190''s/110. Nitroglycerin was given but it did not help with his chest pain and he began to have a headache. He was taken to the hospital. He stated that he was not feeling good a couple days before today. Write-up: No appetite for days. The smell of food made patient sick. On week two after vaccine, oxygen stayed at 84%, patient couldn?t stand as he had no energy. Patient began to eat after lots of convincing and seemed to be doing at least 20% by week 3, but oxygen was still in mid 80%. On day 20 after vaccine patient had to be rushed to the ER due to sepsis. All vital organs began to fail and on transport patient suffered a mild heart attack. Patient was a healthy 78 year old male that lived alone and drove and had no major health issues before the date of the vaccine.

Write-up: Called to lobby where client was sitting upright in chair after receiving J&J vaccine. She had brought her own ice pack from home and stated when she gets shots she sometimes feels like the room is going black and feels faint. Patient requested to lie down. Myself and nurse assisted patient to wheelchair and assited her to exam room 2 where she got on the exam table with our assistance.Patient did not lose consciousness but was pale and clammy. Patient then became restless and began to complain of not being able to feel her tongue and her fingers, noted decorticate posturing and patient complained of feeling like she needed to vomit. Write-up: patient stayed for 15 minutes after the shot and then left. She said she tasted metallic and tingle in lips/mouth so she drove back to the store and came inside. We gave her 50mg diphenhydramine PO. She had BP of 178/96 and pulse 107. She seemed to calm down but we had already called paramedics. They arrived and she became frantic and asked for EpiPen. They monitored her EKG and asked her many questions. She demanded to go to the ER so they put her on a stretcher and left. Her pulse did get up to 150 when she was most frantic however her oxygen saturation stayed 100% through entire time. She was instructed to slow her breathing and remain calm.

Write-up: On the day of vaccination I was treated at the site for a reaction which started 40 minutes after the shot. I was hypertensive and Tachycardia and I had chest tightness . I was monitored for 90 minutes and I had taken some Prednisone and they gave me Allegra. I left there and six hours later I felt worse. I had been having intermittent chest pain. My chest pain got worse and my heart rate was elevated so I went the ED. I was given Benadryl and Pepcid. I was discharged after two hours but I still had some chest pain. I took another dose of Steroids after four days I felt better. I have a scheduled Stress Test at Hospital for follow up. I still have some burning chest pain on occasion.

Write-up: Patient went to COVID vaccine clinic and was administered Janssen COVID 19 vaccination lot# 1802068 at 0800. About 14 minutes following vaccination developed a diffuse erythematous blotchy rash across her chest and arms. Did complain of burning to her skin. Patient did not have any shortness of breath or difficutly in breathing. She was given 50mg of PO benadryl and transferred to the ER for further care. While in the ER the patient was given 20mg of IV famotidine and 125mg of IV solumedrol. Patient was discharged from ER about an hour later - her symptoms had completely resolved - she was advised to continue Benadryl 50mg every 6 hours for the next 24 hours by the ER provider.

Write-up: "Client received Johnson & Johnson vaccine on 3/19/21 and started with symptoms on 3/21/21, he assumed it was a reaction to the vaccine." Client admitted to Hospital on 3/29/2021. Per hospital 3/30/2021: Presented to the ER with SOB and low oxygen saturations that started on 3/30. The patient did not have any complaints of shortness of breath up until that morning. He received the Johnson & Johnson vaccine on 19 March, then developed fevers, fatigue the Wednesday after his vaccine and tested positive for Covid on March 26. Patient does complain of fevers of 103 at home. He resides at home with his wife who is also sick with Covid. He was admitted with Covid pneumonia and new oxygen needs. Tested positive on 3/26.

Write-up: Patient is a 69-year-old female who reports a history of hayfever who is coming in today after recent Covid vaccination with concerns of facial swelling also complaining of the rash and itching to her chest. On exam the patient has obvious facial swelling. She has what appears to be an urticarial rash on her chest. While she does have swelling around her lips and face otherwise she does not appear to have any obvious signs of airway involvement with no swelling of her tongue or uvula, she has good phonation with normal respirations without difficulty. Does not appear to be having reactive airway disease exacerbation at this time. Does not appear to have impending respiratory distress. She otherwise does have some skin changes in her bilateral lower extremities that she reports is baseline with her history of eczema. Primary concern at this time is for possible allergic reaction. Patient appears to have angioedema. Does not appear to have anaphylaxis. Other abnormalities are considered. Patient will be connected to cardiac and respiratory monitors. IV access will be obtained. The patient will be given a 1 L IV fluid bolus. She will be given Benadryl, Solu-Medrol, and famotidine. Patient be monitored for response. Labs have been ordered to evaluate. EKG as well as chest x-ray will be obtained. We will file VARES report. Chest x-ray obtained. There is no acute cardiac no acute pulmonary pathology on my interpretation. Laboratory evaluation has returned. The patient''s CBC is reassuring. CMP shows a glucose of 78, otherwise reassuring without significant abnormality. Troponin within normal limits. Covid, influenza, RSV testing is negative. On repeat evaluation the patient is resting comfortably. She reports that the itching has resolved. Her face feels less uncomfortable. She has had no progression of the swelling. On repeat examination she continues to have facial swelling as well as swelling around her lips however she has no tongue swelling, no swelling within her posterior oropharynx, and her phonation continues to be normal. She and her friend were informed of the initial findings. Plan at this time is to admit for further evaluation and treatment.

Write-up: On Saturday 3/20/21, Patient developed a circular rash below injection site on left arm. She had swelling and pain from left shoulder to elbow and radiating pain up to neck and down to hand with numbness in hand. Patient said the rash lasted about 2-3 days. On 3/27/21, Patient went to ER due to pain and was given naproxen. She went back to ER on 3/29/21 due to continuing to have pain. She is to follow up with PCP this week.

Write-up: Patient felt like she was going to pass out, did not want to lie on the ground as instructed. Patient passed out in her chair, provider was next to patient and lowered patient to the ground. Patient was unconscious and not alert for a few seconds. When patient came to, she was alert and oriented, answered questions appropriately. Patient''s BP was 118/82, HR 80, BG 63 (after a large lunch 2 hours prior). EMS was called, patient felt fine. Decided not to go to the hospital. Patient waited at vaccine clinic, BG 110 after an apple juice.
