Diagnostic Lab Data: May 24th I was seen in emergency room where a ct scan and bloodwork was done to rule out a possible stroke. No stroke was found. No abnormal blood results. They could not find anything wrong. Was seen at on May 25th where numerous testing was done and they could not find any damage, infection or disease to my eyes. The doctor said that he had never seen a case like mine before in his 30 years in practice. I was given a prescription for glasses that made me see better, but the doctor was still in shock because there was no evidence of anything wrong with my eyes.

Write-up: Received the vaccine on May 15th and on May 24th at 5am I woke up and my vision was completely blurred. My vision is normally perfect. I could not see 2feet from my face. This lasted for four days straight. I could not drive or go to work.

Write-up: Shortness of breath and chest pain starting on 7/29/21, prompting ED visit on 7/31/21 where she arrived hypoxic, tachypnic, and tachycardic. She was found to have bilateral pulmonary emboli and was hospitalized until 8/1/21 for further evaluation and treatment. Patient was placed on 4L of oxygen via nasal cannula and given Lovenox in the hospital. A transthoracic echo was done and revealed no right heart strain. A 6-minute walk test prior to discharge was performed and resulted in patient requiring 2L/min of oxygen with exertion but not at rest. She was discharged with Eliquis, which she was advised to continue for 3 months, and home oxygen. It is important to consider patient''s sedentary lifestyle and recent 4-5hr road trips as potential provoking factors as well. She was stable upon discharge.

Write-up: On 6/17/21 patient was brought to the hospital by ambulance with what had appeared to be signs of a stroke. Numbness is extremeties slurred speech and when brought to e.r. stroke was ruled out. He was not able to move legs and this continued up his body. He was eventually intabated all test run on him came back negative. He was in ICU for 3 1/2 weeks where he remained til he got his trachea and was stable to move to the next step down in the hospital. An is now in rehab slowly getting some strength back and started to be able to speak being weened off the ventilator. Recover is going to be a long road

Write-up: Started blowing large blood clots out of my nose, particularly on my left side. At first I believed this to be simple sinusitis, but it has continued and increased in severity to the point that I have been screaming in pain. I sought urgent care after getting mass swelling behind my left ear in the mastoid area all the way up to my temporal lobe/skull, and blowing a large, red blood clot out of my nose.

Mon 7/19: MRI cervical with contrast. Cervical MRI showed degenerative cervical spondylosis w/multilevel stenosis and no spinal cord lesion.

Write-up: On Wed., July 14, my left hand started tingling. As the morning progressed, my hand and arm started feeling numb. Mid-afternoon, my left foot began feeling numb; the numbness spread up my entire leg. By the time I got home (6 pm), I needed assistance to get into the house, because my left leg wasn?t working and my foot was dragging. On Thursday July 15, I went to a local clinic; they immediately sent me to the ER. I was then transported to the ER, where I was admitted. Over the next five days I had CT scans of head, neck and spine; MRI?s of the same; a lumbar puncture; and an echocardiogram. I was given acyclovir; solumedrol; and heparin and amlodipine because my BP was elevated (180/100 Thursday afternoon). With the help of a physical therapist and a walker, I was able to get around, and my balance and stability improved, although the numbness in my arm and leg remained. At discharge, I was given a walker and an AFO brace to assist my walking. I was unable to return to work until August 4, 2021.

Write-up: Patient received Janssen vaccine and within 3 minutes felt faint, slouched and was assisted to a position laying on the floor, 911 was called.. Patient had a seizure lasting approximately one - two minutes and slowly revived to nearly full attentiveness. EMT arrived within 5 minutes and assumed patient care. Write-up: Fever, General Malaise. Could not control fever on my own. I went to the emergency room where I received another gram of Tylenol and 30mg of toradol. Over the course of my stay I revived 2L normal Saline. I had fever induced tachycardia as well. Fever didn''t break until fans were placed on in the room to cool me down. I received 4mg of zofran and 5mg of compazine for nausea and vomiting. I received 1mg of Ativan for heart rate control to which it did not respond. Repeat EKGs and Troponins were done. EKGs showed Sinus tachycardia with ST depression in leads II, III, aVF, V4, V5, and V6. Troponins came back negative. I was given 5mg of IV metoprolol and my heart rate responded and decreased. Repeat EKG showed improvement with ST depression only showing in leads II and III.

Write-up: Within a minute of administering the vaccine. The patient fainted. She was conscious within a minute of fainting but was visibly confused. Her vital signs were normal and was able to speak and answer questions about her breathing and pain. EMT was on site within 5-10 mins of the incident and took her to the emergency room. Write-up: I gave a healthy 42 year old female a Janssen covid shot on 7-7-21. she was taken to the ER on 7-29-21 after experiencing dizziness, severe headaches and numbness and decreased motor movement on her left side. she was complaining to her mother on the phone that she didn''t feel right and she passed out. She woke up to emergency assessing her in her home. she declined transport to the ER and had a family member take her. She was assessed her and determined she may have had a TIA. She was prescribed Hydroxyzine HCl, Fioricet, lipitor and aspirin. A consult with a cardiologist and neurologist in a few weeks. On 8-4-21 she reports a sluggish feeling and swelling in her left leg and feels her leg might have decreased circulation and a cold feeling as well.

Write-up: Anaphylaxis. Swelling of the lips, mouth, throat and hives over 50% of her body. Started 5 minutes post injection and progressed over the next 5 minutes. Diphenhydramine 50mg po was administered on site and paramedics were called. Patient was calm and aware with a good airway and o2 sats. Paramedics arrived apx 15 minutes following administration. They evaluated her and decided that she was stable enough for the patent''s caregiver to transport her by private car to the nearest ER (2 miles away).

Write-up: I received the shot on Friday, June 25 at about 1630 hours. At about 1500 the next day, Saturday, June 26, I felt a fever coming on. By the next morning it had dissipated, and other than a sore arm and some general fatigue, I felt better. I went to work all the next week, but on Saturday, July 3rd, I slept until almost noon, which is very unusual for me; also I awoke with a bloody nose. The only thing of note during the week was that my right calf and thigh started hurting in a couple of places, but I figured that I must have strained it slightly and that the pain would go away soon. I did take some naproxen during the week and it seemed to help. I started doing some yard work and as I was mowing I felt like it was getting hard to breathe. I finished the front lawn, which is not very big, and sat down for about a half-hour and rested before tackling the back. Usually I do the whole thing at once and it doesn''t take me very long. I found that as I mowed the rest of the lawn that I was really struggling to breathe and I couldn''t take a deep breath at all. I felt like I was going to collapse but i managed to make it back inside and sit down. I must have sat for a couple of hours before laying on the couch in search of a place where I felt like I could take a breath. I didn''t feel like I could pull in all the air that I wanted to, no matter how hard I tried. After about four hours I got up and went to bed, where I slept for about 12 hours and again awoke with a bloody nose. I basically spent all of Sunday, July 4 and Monday, July 5 in a recliner resting and trying to breathe. My right leg was still hurting and I still couldn''t draw a complete breath. I made myself get up and go to work on Tuesday, July 6, and made it through despite having to stop and gasp following every exertion (even walking a few feet). I took more naproxen and pushed through the rest of the week, having decided that maybe I had a touch of pneumonia and that maybe it was getting better. My right leg continued to hurt and I still couldn''t take anything resembling a deep breath, no matter how much i wanted to. On Saturday, July 10, I laid down on the couch in the early afternoon and passed out for several hours. My wife, who had been out of town for most of the prior week (else I would no doubt have been to the ER sooner) got worried and checked my temperature and O2 levels and found my oxygen to be low and my temperature to be high. She insisted that we go to the emergency room. We went, and following an EKG, a CT scan, an X-ray, and an ultrasound I was diagnosed with multiple bilateral pulmonary embolisms and was admitted into the hospital on July 11, 2021. I started a course of Eliquis and as I was responding well in terms of no pain and having passed an echocardiogram, I was discharged at about 1700 on July 11. I have been on Eliquis since, and have had two follow-ups with my PCP as well as two different blood work done, both of which show improvement. I also have an appointment with the hematology clinic on August 17. I am feeling better now, can breathe easier now, and do not have as much fatigue as my stamina returns. My right leg does not hurt any longer and the swelling is mostly gone.

Write-up: COLLAPSED RIGHT LUNG; RIGHT LUNG FILLED WITH FLUID; FUNGAL INFECTION IN LUNG; DID NOT FELT RIGHT; This spontaneous report received from a consumer concerned a 48 year old female. The patient''s height, and weight were not reported. The patient''s concurrent conditions included alcohol use one in a while, once a week, non-smoker. About twenty-five years or more ago, she had an episode of fluid in cavity of right lung due to fungal infection and that the way they knew she had it because she coughed one day and there was blood. She was treated at that time and she did not have any other problems like it after. Other pre-existing medical conditions included no known drug or non-drug allergies, no history of drug abuse or illicit drug use. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 041A21A, expiry: unknown) dose was not reported, 1 total administered on 08-APR-2021 at 05:00 for prophylactic vaccination. No concomitant medications were reported. Patient received the Janssen COVID-19 vaccine on 08-APR-2021. Patient''s husband stated that patient had a cough for about seven days after receiving the vaccine. She told him she felt like it was just a normal side effect and that they did not pay much attention. She also stated that she did not feel right at the time. She then developed a cough with phlegm. On 16-JUL-2021, patient and her husband went to an indoor whirlpool and while there she stated that she could not breathe. Husband stated that at that time, they started to take it seriously and made some appointments for her on 22-JUL-2021. On 23-JUL-2021, Laboratory data included: X-ray (NR not provided). Right lung was filled with fluids and noted fungal infection. At that time, the physician told her that she needed to go to the emergency room since she had a collapsed right lung. On 23-JUL-2021, She went to the hospital where physician admitted her. The physicians noted that her right lung was filled with fluids and that they noted a fungal infection which they tried to treat. The physicians said that her right lower lobe was stuck and that physician had to relieve the pressure and fluids around it. She had tubes in her that physician had placed to try to help her breathe and that she received morphine every four hours. The physicians noted that they did not knew what caused the lung to get collapsed and the treatments the physicians had tried to get rid of the fungal infection, had failed. The physician stated that she needed more extensive care from possibly a surgery where the infection could be removed along with the fluids but that the physician there did not had any experience of doing such a procedure and also because patient had high chances of mortality. Her husband also did not had insurance. The hospital had tried to contact other physicians and hospitals to see if there was anyone who could take patient''s case. She was discharged on 30-JUL-2021. Write-up: Symptoms began roughly one month after administration. Symptoms began with numbness and tingling in the feet, moving to the calves, upper legs, arms, hands, and face. Weakness developed in the extremities shortly thereafter. Patient consulted with primary care provider (?PCP?) on July 21, 2021. PCP ordered various laboratory blood tests and recommended patient see a rheumatologist. On July 27, 2021, patient contacted PCP for a neurology referral. Upon receiving referral, Patient contacted Neurology for a consult. Patient described symptoms to via a web portal. On July 28, 2021, ,clinic advised patient to go to the emergency room. Patient visited emergency room at Hospital and Health center on July 28, 2021. Patient was admitted to the hospital with a diagnosis of Guillain-Barr? syndrome on the same day after a clinical examination, MRI of the entire spine, and a lumbar puncture showing elevated protein levels. Patient lost almost all strength in legs and developed saddle paresthesia shortly thereafter. Patient was treated with Immunoglobulin therapy over the course of five days. Patient was discharged from the hospital on August 4, 2021.

Write-up: The patient called the pharmacy to report that she begin breaking out in an itchy rash about one month ago. She said it started on her chest and spread quickly from neck to groin to legs and arms. She visited her primary care physician who first diagnosed her with poison ivy. It continued to get worse so she went to an urgent care clinic. There was still symptoms, so she contacted her insurance and found a dermatologist. The dermatologist (who she saw today) said that the rash was caused by the vaccine. The dermatologist gave her a "stronger cream" (she did not know the name) and is hopeful that it will resolve.

Write-up: I had fatigue 2 days after the vaccine. My fatigue was getting to the point where I couldn?t work. The next day I had pain on my left side in my chest and arm. I started to have severe chest pains. I went to the ER; they did an EKG. They gave me heparin due to maybe having a blood clot. They ran some other test, and they found an infusion. I was admitted for 5 days to run tests. My pain level was severe. Once my inflammation went down evenly. I was discharged to go home but I have to follow-up with a pulmonologist. I have had high blood pressure and tachycardia in the hospital, I was also on oxygen. I may have to go on FMLA due to not being able to go back to work at normal capacity. I still have pain I my left shoulder and my left side of my chest. I had a course of antibiotics that I took which was 10- days. My bladder was inflamed and both my lungs. I was diagnosed with partial lung collapse on the left and right due to fluid on the lungs.

Write-up: 8/1/21: injection- normal pain, with muscle soreness all over. 8/2/21: woke with intense pain in all joints, muscles spasms in back and legs. Difficulty speaking and using lips to produce words. Feeling of mild paralysis all over body difficulty walking. Write-up: Inflammation in brain stem causing Internuclear Ophthalmoplegia, Bilateral. Eyes unable to focus together, resulting in blurred vision, vertigo and nasea. Hospitalized 5 days and treated with three day course of solu medrol steroid and blood thinners. Condition improved 5 days after discharge from hospital and daily thereafter. A recurrence of this adverse event is a distinct possibility.

Write-up: As pt was waiting 15 minutes after vaccine, tech looked out and noticed pt had head between legs and was breathing heavy. Mother was paged overhead. I went out to see if she was okay and when I brought a water to pt. Seemed like an anxiety type of reaction but mom noticed pts fist where clinched and request we call 911. EMTs thought this was more of a reaction from anxiety . Pt was pale but seemed ok when EMTs arrived. They took her to hospital just in case.

Write-up: Tuesday July 20 I had a severe headache and loss of vision in my right eye. I thought it was a migraine. The two days that followed my headached got better but I still didn''t feel right. On Friday the right side of my body started going numb. I went to the ER where they did a CT scan and an EKG. CT scan showed I had a stroke. They transferred me to another hospital where they did a MRI to confirm the stroke and did an echocardiogram to check my heart. Everything with my heart checked out perfectly fine, my blood pressure was good. They could find no reason for me to have a stroke at 34.

No concomitant medications were reported. On 03-APR-2021, the patient experienced severe headache, extremely fatigue, right leg hurt after sitting too long. This got resolved without taking any medication. On 09-APR-2021, the patient experienced blood clot above the right knee, on same day patient had red inflamed rash with itch and blisters on front of left leg above the shin, on 15-APR-2021 patient has massage his primary physician to get steroid cream for rash. Patient had not come up in contact with new clothing or laundry detergent or outside exposures, or changes in diet, the rash did not get better even after using the steroid cream (triamcinolone), for about 3 days, but the cream actually seemed to increase the flare up. He stopped using the cream, and on unspecified date in MAY-2021 the rash eventually fully recovered on its own about 2 weeks after discontinuing the steroid cream (triamcinolone), patient had blisters but not with too much fluid in it. Write-up: BROKE OUT RASH ON NECK, AND ALL THE WAY BY THROAT TO ALL UNDER ARM, BREAST, BEHIND LEGS, CREASES OF ARM, AND CREASES OF GROIN AREA; This spontaneous report received from a patient concerned a 7 decade old female. The patient''s height, and weight were not reported. No past medical history or concurrent conditions were reported. Write-up: Extreme joint pain in ankles, feet, knees, wrists, and hands. Could not hold a pencil or walk up stairs. The symptoms began about 3 weeks post-vaccine. After several days, they were unbearable and I went to a walk-in clinic. I was prescribed a 5-day pack of prednisone which alleviate the symptoms. The week of July 26th, the symptoms returned but were more focused on my hands, wrists, and fingers. Ankles & feet were also sore but I could, again, not hold anything in my hands, or bend my fingers. Extreme fatigue accompanied this flare. Provider prescribed another course of prednisone and maloxican.

Write-up: The patient presented to medical center with stroke like symptoms on 4/8/21, the day following his J&J COVID vaccine at the medical center. He was recently admitted to medical center for stroke like symptoms and bilateral leg pain that has reappeared. Both times the patient presented to the hospital, he was given TPA and symptoms subsequently resolved, however we continue to have no clear findings on imaging regarding strokes. Patient continues to complain of fatigue that is unresolvable. He and his wife strongly feel it should be reported regarding possible side effects and to go on the CDC clinical evaluation for possible adverse reaction to the Vaccine.

Write-up: Pt was seated in the waiting area for observation. When he started to feel unwell he stood up and became lightheaded and collapsed. By the time I reached him he was alert so he could not have loss consciousness for more than a few seconds. A customer witness him hit his head on the way down so we contacted the paramedics for evaluation. We had him lay flat on his back with a pillow under his head until the paramedics arrived. He reported some head pain from the fall and was still dizzy when the arrived. His blood pressure and heart rate were elevated during the assessment. His blood sugar was within normal range. When they took his blood sugar his heart rate dropped in the 40s so they noted a vasovagal response. Because he was still dizzy and not able to drive the paramedics recommended he get evaluated at the hospital and took him to hospital. I told him to update us if there were any developments.

Write-up: Patient became ill the evening after getting the vaccine. She started feeling like a flu was coming on. The next day, 08/03, she had chills, fever, and began shaking. On 8/4/21, she began feeling better, but had canker sores throughout her mouth and a swollen lip. On 8/5/21, she noticed blood in her stool and went to the hospital. They performed blood tests (nothing abnornal) and released her. She had blood in her stool on 8/6/21 consisting of hard clumps of blood. The hospital told her her blood pressure was reading high and told her to see an endocrinologist.

What looked to be a seizure lasted approximately 20 seconds. Nurse entered room. Patient became alert but was confused and incoherent. 5:52EMS called.

Write-up: I was vaccinated without incident on March 23rd, had normal reaction of chills the night after vaccination but otherwise uneventful. On about April 14th I started developing severe hives all over my body. They would flare up in patches on my legs, trunk, arms, and back, then disappear, then reappear again throughout the day. I tried treating them with OTC antihistamines (benadryl, Zyrtec) with little to no relief. After a few days I visited an urgent care who prescribed a short duration of prednisone. The hives temporarily went away, but as soon as the course was done the hives returned in force, along with angioedema of my face (eyelids, lips). I visited a primary care doctor who gave me a shot of steroids (dex?) and referred me to an allergist. The allergist prescribed 4x daily 2nd gen antihistamines (I took fexofenadine 180 mg) and 2x H2 blocker antihistamines (famotidine 20mg). After 6 weeks with minimal improvement my doctor started me on Xolair 300mg monthly. First dose was June 3 Rd. I''ve seen progressive improvement on Xolair but still need to take 4x antihistamines + famotidine to keep hives away. I still have occasional flares.

Write-up: A few hours after I received the shot I started to get a headache and pain in my chest. My neck started to feel stiff and I started to feel under the weather. After getting home I started to feel miserable and I took my temp and it was only 99 then it rose to 102 and I started to get severe muscle pains and the headache wasn''t leaving. Felt like somebody was crushing my face. I had excruciating pain in my neck and arms and into my hands. I had fatigue and was light headed. I woke up the next morning and went to the Urgent Care.

Write-up: Johnson/Johnson vaccine 5/6/21 at 2 PM. About 7 hours later, body aches gradually worsening. Around 9 AM 5/7/21, developed substernal CP aching/sharp quality, 6/10, accompanied by SOB. He notes mild increased SOB/CP with exertion. EKG was abnormal with inverted t waves V1-4, III, he was admitted over night and diagnosed with pericarditis. Stress echo showed no abnormality. Negative troponin. On follow ekg changes normalized, he improved with ibuprofen alone

Write-up: Patient reported that after ~30 minutes after receiving vaccine she reported that her fingers and both her arms were tingling going up towards her shoulders. Patient also c/o her heart racing. Patient''s VS were stable, HR: 80, BP: 136/70. EMS was called @ 1139 am: at which time patient reported that her symptoms had not worsen but that she was not improving. Write-up: After waiting 15 minutes patient said did not feel well after vaccination. Patient complained of burning throat and was coughing. Advised patient we should wait another 15 minutes to monitor for allergic reaction. Patient said burning feeling progressed to ears and head. Kept saying he felt like he could breathe but would mention having trouble making a breath. Patient then said his eyesight was hazy and having trouble feeling. We decided to call 911 at this point. Patient was able to speak and breathe during the wait for EMS. EMS arrived and took patient out to ambulance to further treat. Not sure what happened after ambulance left.

Diagnostic Lab Data: 5/21/2021- Blood was taken and I was given a panel of about 50 blood testing items. While I was paritally unconscious I don''t know what other tests were taken. The vaccination was given in a hospital. After I collapsed, I was taken into their emergency room. Prior to stepping foot in this facility, I was completely fine. I had a annual physical and then a thryroid exam, after each exam had large blood tests taken and EVERYTHING WAS NORMAL before I went into Hospital to get the J&J Vaccine. It was one of the most horrifying experiences of my entire life.

Write-up: 5/21/21 Was given vaccine in Hospital. Around 10 minutes later, became violently ill. Threw up everywhere. Passed out. Was taken into ER right away. In and out of it. Held for 5 hours. Received IV and medication. Neurological event- had hard to moving muscles in my face. Freezing, shaking and shivering, they kept putting blankets on me. I was afraid I was going to be paralyzed. Utterly and horrificly horrifying experience. Was given a turkey sandwich that had a slice of moldy bread. I told them I was perfectly fine when I went in there. I was sorry I ever stepped foot in there. Examined by doctor, nurses and who knows who else. Nurse asked me if I have anxiety. I said yes, it from what happened to me after the shot. What did she think? Duh. I needed bathroom. Was escorted to bathroom. I was able to walk on my own and use bathroom on my own. I said I wanted to leave. When nurse removed IV, arm was bleeding had to be rebandaged. I was discharged. Given scrubs to wear due to puke all over my clothes. No one even helped me out of the place. I walked out. I decided to drive home. I left and stopped off at parking lot to rest. It was a Friday night during rush hour traffic. My head was spinning. I felt nauseated. I waited half hour then selected different stopping points in case of emergency. Waited for traffic to clear up. Got home ok. When I was changing, found 4 electrodes never removed from my body. They started yelling at me demanding my medical insurance. They went through my purse and then my wallet. I never signed anything saying I would be responsible for medical bills. 5/22/21 Had severe nausea. Very tired. Headache. Pain in jaw and face. Disgusting taste in my mouth. Ate lightly and did not puke. No fever. Arm hurts a little from the shot, not too bad. 5/23/21 Exhausted in the morning. Nausea, very tired, a little dizzy. Need to lie down, rest. Had light breakfast, piece of toast. Ate very lightly all day. In the afternoon, developed very bad gas. Abdominal pain, but not severe. Hospital nurse called. Late afternoon, started to get diarrhea. Had upset stomach. Walked around complex for 5 minutes, then walked around apartment for afew minutes. 5/24/21 Felt a little better. Still have diarrhea, loose stools on and off all day. Nausea. Bad taste in mouth. 5/25/21 Very tired. Same diarrhea. Bloating, gas, abdominal pain continuing. Morning nausea. Exhausted. Bad taste in mouth all day. 5/26/21 Nausea again in morning. Loose stools on and off but becoming more normal. Gas pains here but starting to go away. Bad taste in mouth. 5/27/21 Very tired. Nausea again in morning but alittle better each day. Diarrhea in morning and on and off all day. Bad taste in mouth, seems to be slowly getting better. Tired spell on and off. 5/28/21 Nausea in morning, felt sick, upset stomach, abdominal pain again but slowly goes away. Gas pains started again in night after eating same light dinner I was eating all week (mainly rice and tuna fish). Stool is loose. Tired but perkier than previous day. 5/29/21 No morning nausea (finally). Stomach is still gassy but no abdominal pains. Very tired, slept about 16 hours. 5/30/21 Slight morning nausea. Had a semi-normal stool, the loose stool again later. Bad taste in mouth still there alittle but going away. 5/31/21 Woke up with nausea. Lost 4 pounds. Anxiety over recalling the shock of the bad reaction to vaccine. Afraid, I have been contaminated and/or given a bad vaccine or the wrong dosage. Afternoon started to feel normal. 6/1/21 Stomach feels better but still not normal in the morning. Starting to feel better. Slept partially through the night. 6/2/21 Slight morning nausea that went away after I got up. Feeling better but not myself. 6/3/21 Stomach feels better but not normal. 6/4/21 Still have nausea in morning but is improving each day. 6/5/21 Stomach still not right. Still eating light foods. Nervous about what happened to me. Don''t know what was in that vaccine, made me really sick. 6/6/21 Today, I actually feel ok. Decided to continue eating plain foods anyway. Stomach still not 100% but getting better, still a slightly bad taste in mouth. 6/7/21 Started to feel like old self. Will take it easy for next few weeks. Write-up: Adverse Event: Within 10-15 seconds immediately following the vaccine administration, patient stated she "felt funny". I asked her if she could explain exactly what didn''t feel normal, and she stated, "I just feel weird. I''m hot and getting light-headed." Patient began sweating profusely and face was flushed. I gave her water and cooled her down. After about 2 minutes, she became weak and was unresponsive for about 30 seconds. Emergency services was contacted right away. Patient regained consciousness. While waiting for the ambulance to arrive, patient further complained of signs and symptoms including sweating, weakness and tingling in the arms and legs, "heart hurting", chills, dizziness, and feeling disorientated. She remained seated the whole time. Treatment: Patient initially was able to drink water and I cooled her down with cold/wet paper towels during the first 2 minutes. When she became unresponsive, a second pharmacist dialed emergency services as I held her head up. I was told by emergency operator not to further administer food or drink and to wait for first responders. I took the patient''s blood pressure upon the arrival of EMS and it measured 108/74. EMS tested patient''s blood sugar. Total time course <30 minutes. Outcome: First Responders took patient to a nearby hospital ER by ambulance for proper treatment and evaluation.

Write-up: Starting day 4 post vaccine developed heart flutters that would last several seconds before spontaneously resolving. This heart fluttering happened off and on over the next 6 to 7 days. On day 12 developed diagnosed atrial fibrillation with a rapid response rate which resulted in a hospitalization for 24 hours and treatment of a Cardizem bolus and Cadizem drip. Discharged home with outpatient cardioversion scheduled, however; spontaneously converted back to normal sinus rhythm but remain on a daily dose of Cardizem and a twice per day dose of Eliquis. I require follow-up with a cardiologist and will require the two new medications for at least three to four weeks post atrial fibrillation event.

Write-up: 3 days after event patient had migraine, muscle ache, nausea, lethargy. On July 9th, client had seizure, with no history of seizure. Since July 9th patient suffers from migraines, headaches, ringing in ears, nausea, light headed, confusion, short term memory loss which has persisted for over a month. Patient is seeing neurologist and cardiologist, primary care, urgent care.
