Write-up: Janssen vaccine administered at 10:15 AM. Patient complained of heavy tongue and "feeling funning back of the throat" after a minute or two of the vaccine. On or about 5 minutes after, patient continued to complain of heavy tongue and back of throat dry and "getting worse". Nurse Stat called then transferred to ED. Speech became slurred and had a fearful disposition stating her throat was getting worse. She also complained of feeling itchy.

Write-up: The following day after vaccine employee went to the ED for complaints of SOB, all over body aches so severe she had difficulty walking. Right arm pain and redness at site.. States had rash on inner thighs and itching. She was off work for the rest of the week. Employee came to occ health for evaluation with the same lingering complaints on the 7/28/21..employee was a no show for her final eval at occupational health on 8/5/21. She then resigned the following day.

Write-up: It started the same day at 7pm. Aches in my joints particularly on all my fingers, Fever 101, body aches and headaches and unable to sleep. It all lasted 15-18hours. The first week of 3 June 2021, my stomach was irregular, every 5 to 6 days I would have multiple bowels movements and urge to go and a burning sensation after each bowel movement. My butt was achy. June 28 reported to doctors. Was told to lay off certain foods, I would get cold sores ongoing off and on until today and in tongue. 2 episodes on June 2021 - lightheaded. Twice at lunch time for 10-15 seconds. On 1st July lightheadedness was severe. I had diarrhea and vomited violently and had vertigo. I went to the ER 1 July and had to be transported in the ambulance.

Write-up: 10 minutes after receiving vaccine pt started to have tingling down her left arm and left side of her face. Vitals taken Pulse - 80, blood pressure 140/110. Pt state BP was very high for her. Within 20 minutes of vaccination pt states tingling continued down are but started spreading across face and mouth began to tingle. Emergency services called.

Write-up: Patient received COVID 19 vaccine at the Health Department on 7/27/2021. She noticed a diffuse petechial rash on her legs on the morning of 8/9/2021. She was started on clonidine for anxiety about 10 days ago, but stopped because of hypotension and has had no other medication changes. She has multiple tiny petechiae diffusely scattered over both legs. CBC with platelets showed a platelet count of 1 thousand/uL. She presented to our office for evaluation on 8/10/2021 and her platelet count was <2 thousand/uL. she has no anemia, decreased renal function or decreased hepatic function. she also reports bruising of her right upper arm as well.

Write-up: Flu like symptoms occurred on 8/2 (fever, body aches, chills, headache). On 8/3 flu like symptoms decreased in severity, but severely painful headache remained. Severe throbbing headache was a pain level 10. Throbbing pains worsened when bending over or moving head in any direction. Throbbing pain location was felt all over head, but mainly felt behind the eyes. No vision changes. Severe headache did not respond to medications. Two 500mg acetaminophen were taken on 8/3 around 3:30am, and the pain did not resolve. The throbbing pain occurred for the duration of the following dates: 8/2, 8/3, 8/4, and 8/5. On 8/5, there was an onset of lightheaded/dizzy sensation. Healthcare team was contacted on 8/5, and recommended going to the emergency room. Lightheadedness is felt for the entirety of the day, with random spells of dizziness. Legs feel slightly sore. Was advised to go to the emergency department on 08/05. Given 1L IV fluids. Blood test results: abnormal result -- $g d-dimer level was elevated. Because d-dimer level was elevated, healthcare team was concerned about a blood clot in the brain. CT scan was necessary to check for clots in brain. Brain CT scan came back normal, displaying no blood clot in brain. Discharged, still feeling lightheaded and fatigue, and it has been over a week since receiving the vaccination.

Write-up: Janssen COVID-19 Vaccine EUA Patient received Janssen Vaccine on 3/10/2021. Presented to ED on 7/10/2021 with hip pain of two weeks in duration. CT Angiogram showed a right inguinal hematoma and active extravasation and was medically managed. Patient subsequently developed shortness of breath and pneumonia and treated with 7 days of cefepime. Patient tested positive for COVID-19 using the Cepheid GeneXpert platform on 7/30/2021, which was the anticipated day of transfer to skilled nursing facility. On 8/4 she developed abd pain prompting KUB whihc showed SBO but patient deferred NG tube.Later she had CT chest neck and abd which showed worsening PNA with aspiration of bowel content and SBO and poss oropharyngeal perforation. Patient did agree after detailed discussion with family about CT findings and IR able to place NG tube but shortly after she began throwing up bile and needing high flow oxygen. She was not able to maintain oxygen saturation and family changed her status to AND, and she expired on 8/6 at 0415 AM.

Write-up: 30 YO Male patient received injection of Janssen (lot#202A21A) at 4:31 PM PST, at 4:35 he complained of being dizzy and within a few seconds became incoherent and had a seizure that lasted about 10 seconds. He awoke lucent yet confused about what occurred. I told him he had a seizure and took his blood pressure at 90/45. He started to complain about having difficulty breathing. Staff called 911. EMR arrived within minutes and took his blood pressure again. I observed his BP on the EMR monitor to be at 98/68. The patient reported he was feeling somewhat better and able to breath better. He was transported to Providence ER at 5:02 PM lucid and alert.

Write-up: patients arm started to swell right away after given injection, applied ice, swelling started to decrease, after about a minute patient complained of feeling lightheaded and felt he was going to pass out/never did stayed seated entire time, then a few minutes after that patient had cold sweats, called 911, stayed with patient entire time until rescue arrived. no tightness in chest or breathing issues, patient started to feel a little better when they arrived, took them about 10 minutes to arrive. patient did go with rescue to hospital to be checked.

Write-up: HEART RATE IN THE 50''S; PAIN IN STERNUM/ ENTIRE CHEST HURTS; PRESSURE ON THE RIGHT SIDE OF CHEST; HEART PALPITATIONS/SENSATION OF HEART TUMBLING; PAIN IN ARM DURING INJECTION DUE TO VACCINE BEING COLD; This spontaneous report received from a patient concerned a female of unspecified age. The patient''s height, and weight were not reported. The patient''s past medical history included: anemia, and concurrent conditions included: weak immune system, hay fever allergy, and allergies to cat, dogs, dustmite. The patient had no allergies to any medications. The patient used inhalers in her 30s for four to five years. The patient was hospitalized for anemia when 2 years old and again at 4 years for anemia. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: Unknown) dose was not reported, 1 total administered on 28-JUL-2021 for prophylactic vaccination. The batch number was not reported. The company was unable to perform follow up to request batch/lot numbers. No concomitant medications were reported. On 28-JUL-2021, the patient experienced pain in arm during injection due to vaccine being cold. The patient felt pain in sternum, pressure on the right side of chest, heart palpitations and heart tumbling. On 29- JUL-2021 patient had talked with the nurse and instructed to go to emergency room. On 30-JUL-2021, the patient went to emergency room because the pain was not getting better and the heart rate was in 50''s. The patient had undergone blood test, lung X-ray, heart scan, electrocardiogram(EKG) for which results were not reported and oxygen saturation was 98%. The patient''s physician stated that at that moment patient was fine, but if experienced again or long term need to come back and later patient was discharged. The patient was hospitalized for 1 day and was discharged on 30-JUL-2021. On 04-AUG-2021, the patient again contacted the nurse and the nurse clarified everything about the symptoms. The heart rate went back to 60''s. The pain was much better. On 05-AUG-2021, the patient''s pain got better to 1 and 2 instead of 4 or 5. On 06-AUG-2021 patient reported that heart rate was never in 50''s only in upper middle 60''s. Last month pain in stomach got really bac and started severe nausea. went to ER, had blood work. Scheduled abdominal scan and ct. Had gall bladder removed August 29th. Have appointment to see my nurse practioner on august 18th to set up referral to eye doctor.

Write-up: woke up with 104 degree fever, severe headache and weakness and very high blood pressure.. went to the ER . Had CT scan, fluids and x ray of hip. Given pain meds and sent home. Went tprimary doctor one month later for ongoing high blood pressure, stomach pain. Have since been put on 2 blood pressure medications! and had to have gall bladder removed. NEVER had either problem before. Now my vision is getting worse, stays blurry and double.. Have an appointment to see my nurse practitioner for THAT on 8-18th.

Write-up: first 24 hours felt run down and flu like, next numbness and tingling in hands, arm, legs, feet and face. Went to ER due to numbness and tingling on 7/24/21. Feeling worse each day. History of Postural Orthostatic Tachycardia, but has been off medicine (metroprolol) for years. Heart rate is elevated again - 150''s and even 160''s. Difficulty standing. Now like when first diagnosed with POTS when has been controlled for years. Interferring with activities of daily living, unable to go to work as must be on feet and not sitting. Write-up: On the afternoon of May 11, 2021 I noticed blood in my urine; later I saw blood in my stool & my gums bled when I brushed my teeth. My husband took me to a nearby ER, where I spent 6 hours. They did a CT; Covid test; and urine culture which showed I had a UTI. Finally at midnight an ambulance arrived to take me to a hospital. As I''m being wheeled out of the ER I notice my right hand is becoming purple splotched. I was told that''s petechia & it continued to spread over my body. The EMT with me in the ambulance told me I had low platelets; he said I had "6", which I have learned meant 6,000. I was put in the "Progressive Care Unit" at the hospital. Blood tests were taken & I was hooked up to an IV which delivered steroids; later more IVs were added, so that I had 3 at once delivering steroids, whatever they give for "nutrition" and insulin (they would not let me take my regular insulin (U500) thereby letting my blood sugar get all the way up to 578 at one point! At some point early on I was told I had ZERO platelets! A hematologist/oncologist was called in and he ordered a bone marrow biopsy on Thursday morning. I have never heard a definitive answer on what they found, but they have not contacted me again. My blood sugar was checked every hour for days making some very sore fingers! Everything they did with needles resulted in purple/ red bruises all over my upper body, including one huge hematoma on my right forearm. I have pictures to show my "bruising" if you need to see them. I was finally released on Sunday morning. I have struggled since then with constant backache; & sore tailbone, I assume from the biopsy.

Write-up: I went to the hospital with severe stomach pain that was radiating to my back. It was causing me to have diarrhea, nausea, vomiting. At some point a stool had turned black so they thought I had a GI bleed, but they did a CT scan and they said my colon and my intestine were severely inflamed. I was hospitalized for 3 days so they could give me round the clock antibiotic treatments. When I was discharged they put me on a 7 day antibiotic. I followed up with my PCP and made him aware of the situation and now I am just waiting for September 2nd to get a colonoscopy performed. I am still experiencing the same symptoms. No vomiting, no black stools, stomach and back pain are not as bad.

Write-up: Looked over patient''s consent form and saw that the patient had multiple allergies such as Keflex, Amoxicillin, and Bactrim. And everything else did not prevent her from getting the vaccine. Pt did not mark that she had any health issues on the consent form. Prepped the vaccines and called both husband and the patient into the room to get their shots. Verified both patients date of birth and confirmed that they were getting the JJ vaccines. Gave the husband the first shot and went over the side effects of the JJ and the rare occurrences of GBS in men. Gave him the shot and we discussed the differences of the vaccine and why they were getting the vaccine because at their son''s school nobody was masked and up in each other''s faces and they wanted to get vaccinated. The husband said that a few doctors told him that the Moderna and Pfizer did not stay in one place and how the JJ does and it is like the flu shot. Then the patient got into the chair and went over the side effects and how it effects women about risk of clotting and it affects women that are smokers and on birth control. The patient confirmed that she''s good and that everything will be fine. I discussed her allergies and that bc of that we ask that all patient stay and be observed for at least 30 minutes. And then gave the shot to the patient and everything seemed fine. Then the husband and I continued our discussion about how they developed the Moderna and the Pfizer vaccines and then the patient said that her heart was racing and how she was feeling dizzy. I advised the patient to stay down and sit and asked her husband to look over her and I grabbed her a cup of orange juice and rice krispy treat and she ate it and drank the orange juice and she said that she was feeling a little better, but then progressively looked worse. Staff continued to watch over her as they took her blood pressure and pulse ox and the first blood pressure reading was 144/89 and 81 pulse, then a second blood pressure reading was 151/90 and her pulse ox was 99% but she was trying to breathe in and out slowly. At that time 20 minutes had passed and I had to go to lunch and other staff members were looking over her. Staff member brought up POTS syndrome but the consent form didn''t indicate any about dizziness or fainting or health condition for before and after shots. Staff member and I went over shot technique and everything was fine in our eyes. Patient experienced dizziness, rapid breathing, and chest pain

Write-up: Patient received J and J vaccine July 31st around noon. Pt was waited 15 minutes and was fine when left the pharmacy. Patients symptoms started at night. Pt vomitted, had diarrhea and dehydration and numbness on the legs. Monday, pt was given 2 iv bags. Platelet count was low. Tue: pt was at ER for having dehydration, diarrhea and was given 1 iv bag of fluid. On Wed, was briefly was without symptoms, except being tired. Since then, patient continue to have dehydation, and vomitting up until the following Monday (August 9th, 2021). Pt was admitted to the ER. Oxy level dropped to around 60%. Pt was given oxygen. CT scan did not indicate any blood clot. Cardiac enzym was elevated. Scan also showed cardiac inflammation. Doctor determined patient might have experienced minor heart attack or cardiac inflammation. Pt has been transported to Cardiac clinic for further evaluation.

Write-up: SWELLING IN LEG; BLOOD CLOT IN LEFT CALF; ITCHING; This spontaneous report received from a patient concerned a 43 year old female. The patient''s height, and weight were not reported. The patient''s past medical history included old fracture in tibula or fibula, and concurrent conditions included social alcohol drinker and non smoker. The patient had no drug abuse or illicit drug usage. The patient experienced drug allergy when treated with levofloxacin. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 041A21A and expiry: UNKNOWN) dose was not reported, 1 total administered on 02-APR-2021 for prophylactic vaccination. No concomitant medications were reported. After receiving vaccine immediately the patient was fine but about a week later on 9-APR-2021, the patient experienced itching. The patient spoke with her doctor who said it could or could not be related to the vaccine so the patient took allergy pills, Zyrtec 10mg a day, and itching got stopped. The patient reported that she had an old fracture in her tibula or fibula that she has had for 14 years that swells occasionally. The patient recently traveled and when came back, swelling in her leg had not gone down so the patient spoke with doctor who advised her to go to the ER (emergency room) for imaging. On 28-JUL-2021, the patient was diagnosed with a blood clot in left calf. For treatment the patient was on Eliquis, 5mg twice a day, for a total of 90 days. After completion of the 90 days treatment, the patient will receive a new scan and it would be determined if she needed any further treatment. The patient also noted that she traveled before receiving the vaccine but had never experienced a blood clot before. The action taken with covid-19 vaccine ad26.cov2.s was not applicable. The patient recovered from itching, had not recovered from blood clot in left calf, and the outcome of swelling in leg was not reported. Write-up: EXTRAPYRAMIDAL EFFECT (TREMORS); FACIAL SWELLING; EXCESSIVE DROOLING; INABILITY TO SWALLOW; This spontaneous report received from a parent concerned a male of unspecified age. The patient''s weight was 140 pounds, and height was 68 inches. The patient''s past medical history included: psychotic episode (patient''s parents were unsure if it was from marijuana, he had this episode or from mental breakdown), and aggressive behaviour (patient showed aggressive behaviour with the hospital staff. They tried to give him medication but was uncooperative thus he was sent to mental facility and he got discharged on 08-JUL-2021). The patient received haloperidol decanoate (form of admin, route of admin, and batch number were not reported) dose and frequency were not reported from JUL-2021 to JUL-2021 for drug used for unknown indication. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 202A21A, expiry: unknown) dose was not reported, 1 total, administered on 08-JUL-2021 to left deltoid for prophylactic vaccination. No concomitant medications were reported. On 10-JUL-2021, patient''s mother noticed he was having tremors, so she asked medical doctor to stop haloperidol decanoate, so they discontinued the haloperidol decanoate and pushed fluids to get it out of his system. On 27-JUL-2021, tremors got worse, so his parents took him to the emergency room. On 28-JUL-2021, the patient was discharged (as per his mother, the discharge note stated extrapyramidal effect and they also suggested to visit psychological medical doctor.). On 01-AUG-2021, the patient was not able to swallow, and the tremors were really bad. His parents were got panicked and they again took him to the emergency room. On 02-Aug-2021 around 17:00, he got discharged. Psychologist gave him lorazepam and another medication via intravenous (IV) and his tremors stopped. When patient came home, the tremors reoccurred again (full body tremor which occurred all day). On an unspecified date, his mother also stated that he experienced excessive drooling and facial swelling. The patient also took diazepam, but it was not working. Write-up: Pt received Johnson & Johnson vaccine 8/9/21 @ 1000 at a pharmacy out in town. At approximately 1400, pt reported shortness of breath and throat tightness to his corpsman. Pt received unknown amount of Tylenol and Benadryl with no improvement. Pt brought to Hospital ER at 1700 c/o worsening shortness of breath, throat tightness, myalgias, nausea, headache, chills. Pt found to be tachycardic to 115bpm, mildly tachypnic at 22RR/min, and febrile with TMAX 38.6. Pt''s lung sounds clear, tolerating PO secretions, speaking in full sentences, satting 100% on RA. Pt was treated with additional anti-pyretics, IV steroids and fluids with improvement. Pt discharged to home after a few hours in the ER with recommended PCM follow-up.

He never had swelling of face or tongue and did not have trouble breathing. Pharmacist and store manager remained with patient until EMS took over. Write-up: Approximately 5 minutes post vaccination, the patient displayed shaking features and called out for help. He continued to shake wildly for approximately 30 seconds to 1 minute. He asked for help throughout and was visibly very upset. Staff implemented an emergency response, contacted EMS who responded in approximately 5 minutes from the time of the initial reaction. Prior to EMS arriving the patient was slowly lowered the the floor while shaking and gradually calmed down. He said "I think I just had a seizure." He injured his leg during the episode by hitting it off of one of the shelves in the pharmacy waiting area. He did not hit his head or appear to lose consciousness during the episode. Patient was examined by EMS upon their arrival. Vital signs were examined and noted by EMS (I do not have those numbers). Patient was slowly moved to a seating position by EMS and monitored for approximately 10 more minutes upon which time the patient experienced a similar episode to the first. At this point the patient was transported by EMS to the Hospital Emergency Department. The patient contacted the pharmacy later in the afternoon indicating that he had been examined and discharged to home.

The patient''s height, and weight were not reported. No past medical history or concurrent conditions were reported. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 043A21A, expiry: 19-SEP-2021) dose was not reported,1 total administered on 02-APR-2021 on right arm for prophylactic vaccination. No concomitant medications were reported. On an unspecified date in FEB-2021, the patient got an annual blood work done that showed normal platelets at around 217. On 02-APR-2021, after getting the vaccine the patient only had slight fever for about 24 hours. Around 21-APR-2021, patient began to not feel well and went to the emergency room (ER). The patient got tired and bruised easily. Bruising was most obvious at spots where bloodwork was done. When patient was in the hospital, platelets were normal (190) but WBC (white blood cell) count was high (13.9). Later, the WBC came down to normal. The physician then noticed an abnormally low platelet count. The tiredness and low platelets still occurred. Some congestion and lower abdominal pain also started around the same time, but both went away in about 7 to 10 days. On an unspecified date in JUN-2021, platelets were averaging around 75, but on an unspecified date in JUL-2021, it increased a little to 78. The patient was currently seeing hematologist. Write-up: Was having intermittent chest pain several weeks prior to 6/19 I had a heart attack due to a 95% occlusion due to a blood clot. Had cardiac cath and stent put in. On 7/3 i was hospitalized and found to have 4 pulmonary embolisms. Never had a cardiac issue, no family history of cardiac issues. Have had work up by Hematology, rheumatology, cardiology, oncology. All testing has come back with no underlying or contributing diseases. Now on life long medications, currently in cardiac rehab, still have not yet fully recovered.

Write-up: Went to the ER several days after shot after experiencing chest pain and difficulty breathing... tests came out fine and started to feel better. Two weeks later, started experiencing tingling and pins/needles in feet. That sensation slowly shot up my entire body and had trouble walking and coordination was off in a major way. Write-up: About 6 hours after I received the vaccine, I got a massive headache. I went to the emergency room several times and they treated me for a migraine. The headache lasted a couple months. I also started having Grand Mal seizures, one of which left me unresponsive in the ICU for a few days. I also dislocated, and eventually fractured my left shoulder after a seizure. I have continued to have seizures, even on medication, since I received the vaccine. My seizures are being treated with Keppra and Lamictal. I have now been diagnosed with Epilepsy. I have never had a seizure before having this vaccine.

Write-up: Pt completed the J&J Janssen series, with one dose on 3/28/2021. This dose was received in another state, so no lot number is available. On 8/2/2021, pt presented to a local hospital ED for a covid test after a close exposure to someone positive. Mild symptoms at the time, "sore throat, shortness of breath, cough,dizzy/sweaty.". Returned to the ED on 8/9/2021 and was transferred to our hospital for inpatient care on 8/10/2021. Required gen med level of care. Diagnostic Lab Data: Surgery Indication for the procedure: patient is a 63-year-old man who was admitted with an acute leg ischemic right leg and evidence of right iliac femoral and popliteal thrombosis on CTA I have offered and recommended the right femoral exploration with embolectomy if necessary popliteal exploration and fasciotomies I explained the alternatives risk and complications which he understands and accepts Description of the procedure: After the induction of LMA anesthesia the patient''s lower abdomen and right leg to the ankle are prepped with ChloraPrep and Ioban at the leg and groin and isolation bag around the foot. The patient had received 5000 units of intravenous heparin previously and was given another 2500 units of heparin had received IV vancomycin. Somewhat vertical incision was made over the right femoral canal and carried through the subcutaneous tissues down to the common femoral artery common femoral artery was controlled with a maxi vessel loop the profunda with a maxi vessel loop in the superficial femoral with a small vessel loop. An incision was made from the origin of the superficial femoral to the common and there was gross thrombosis at this point time I did an initial thrombectomy at the common femoral artery and restore backflow from the profunda that was quite vigorous. I got no flow in the superficial femoral. I passed initially #4 the #3 Fogarty catheter is returned with clot and minimal backbleeding. I then passed a guidewire and a flush catheter noted continue thrombus in the distal superficial femoral and popliteal arteries. Using a angled Glidewire I was able to pass the angled glide into the anterior tibial peroneal trunks and a over the wire balloon catheter was used to complete thrombectomy of the superficial femoral popliteal tibial peroneal and anterior tibial arteries arteriogram was then done from the superficial femoral and followed to the level of the ankle noting widely patent vessels. Much better backbleeding from the superficial femoral and the profunda then performed proximal thrombectomy with a #4 Fogarty and then adherent clot catheter return with basically fresh thrombus and active arterial inflow. Notably there was minimal evidence of significant atherosclerosis by the catheters or by the arteriograms. The arteriotomy was closed with a running 6-0 Prolene suture flow was restored initially into the profunda and into the superficial femoral and Doppler flow was noted to be very good in the common superficial and profunda femoris vessels. The leg did appear to be pinking up initially in the lower leg and did not appear to be developing a tight leg at this point in time I did not feel that fasciotomies were necessary at this point and he would be observed for compartment syndrome. The pledget of Surgicel was placed over the arteriotomy after checking in with Doppler then closed with 2 layers of running 3-0 Monocryl for Monocryl subcuticular and surgical glue blood loss was estimated to 100 cc sponge needle and estimate counts correct patient taken to recovery in stable conditiongery completed 8/12 to attempt to salavge leg

Proximally 2 days ago developed cramping and discomfort in the right leg that has progressively worsened he now has numbness in the right lower leg and cool right lower leg with paresthesia. Write-up: We administered the Johnson and Johnson COVID vaccine to a 31 year old male, we instructed him to remain in the store for at least 15 minutes. A few minutes after receiving the vaccine the gentleman got up, walked a little and did a few squats in order to look at some items on the shelf, he ended up passing out a few moments later. We administered the EpiPen and 30 seconds later he was conscious again. We called 911 and an ambulance showed up within 5 minutes of him receiving the EpiPen. Although the patient seemed to be okay, we advised him to go to the hospital for a check up anyway and he walked out with the EMT technicians.

Write-up: Patient received the vaccine at 9:40 am on 08/11/2021. That evening, he experienced some shortness of breath when lying down to go to sleep. He has the same experience the following evening. He thought it was anxiety, so he took a valium. He did sleep a little better, but he still felt like it was difficult to take a deep breath. He returned to Clinic on 08/13/2021 for evaluation and advice. At his visit, it was discovered that his heart rate was 150-160 bpm. He was referred to hospital.

The initial information was processed along with additional information received from the patient on 10-AUG-2021. The patient''s height, and weight were not reported. The patient''s past medical history included: triple bypass open heart surgery (3 years ago), and concurrent conditions included: non-alcohol user, nonsmoker, and pulmonary embolism, and other pre-existing medical conditions included: The patient had no known allergies. The patient had no history of drug abuse/illicit drug usage. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 207A21A, and expiry: UNKNOWN) dose was not reported, 1 total administered on 02-JUN-2021 for prophylactic vaccination. The concomitant medications included rivaroxaban (Xarelto). The patient had no side effects after vaccination between 2-JUN-2021 and 22-JUN-2021. On 23-JUN-2021, the patient experienced burning pain sensation behind the bridge of nose and then extended into pain behind the left eye. The patient had bad headache over the top left side of the head and it was accompanied with very extreme dizziness. The patient also experienced voice change. The blood pressure had spiked up to 200 diastolic (that the patient had never experienced before and his blood pressure was usually normal). The duration of the symptoms lasted for 2 hours. On 25-JUN-2021, the patient had same symptoms and lasted for 3 to 7 hours and it was intense than before. The patient went to emergency room(ER)for the symptoms as he felt like a brain stroke or hemorrhage due to his symptoms. The patient had undergone CT (computed tomography) scan and nothing found on testing. On 26-JUN-2021 and 27-JUN-2021, the patient had same symptoms. On 28-JUN-2021, the patient had very bad symptoms and went back to the ER. The ER doctor recommended the patient to consult with ENT. On 01-JUL-2021, the patient consulted with ENT and doctor stated the patient had not experienced symptoms due to vertigo. As the doctor identified the patient had left sided vocal cords paralysis. The ENT referred the patient to neurologist. The Patient was told by the neurologist, to have CT scan with a contrast and MRI (magnetic image resonance) if nothing showed up in CT scan. On MRI it showed that patient had occlusion in left vertebral artery, and the right vertebral artery was clear. On 2-JUL-2021, the neurologist explained that it might be possible that the patient might have over more than 1 episode of brain strokes, and the machine might have not detected. The patient was also advised to maintain cholesterol level below 70. On 03-JUL-2021, the patient had experienced same symptoms. On 4-JUL-2021, the patient had severe symptoms with unbearable headache, diastolic blood pressure200-220(felt like going to explode from the inside). The patient was hospitalized upon ER doctor decision. The patient was ordered for X-ray of heart and echo of the heart and which all came out good. On 05-JUL-2021 to 06-JUL-2021, the patient had worst episode of symptoms and was transferred in the middle of night to the ICU (intensive care unit) The blood pressure was 240/113mmHg and the hospital gave medication for BP then, the patient experienced arrhythmia. At the time of reporting the patient''s symptoms are not worse but the BP only goes up to 169-175. The patient had dizziness and voice changes (still get vocal cord paralysis) along with that, the patient stated 3 episodes of symptoms per day which last only for 5 minutes. The patient also stated all of his symptoms are slowly fading in terms of intensity and duration. The cardiologist told the patient that his symptoms was not related to heart. The patient consulted almost 8 to 10 doctors (internal medicine, ER doctors, neurologists, and ENT) and had 4 CT scans and 3 MRIs to look for bleeding or hemorrhage and brain stroke. The patient was referred to another hospital on January or February 2022 for consultation with radiologists. The patient has an upcoming MRI imaging to be done. Preexisting Conditions: hashimoto, pre-diabetes, gallbladder removed (no stones-hida scan 2% function), anemia, stroke (protein S deficiency), GERD, IBS-Constipation, lactose intolerance, asthma, allergies (technically sensitivities), osteoarthritis, extreme reactions to most narcotic pain medication, sleep apnea, mild high blood pressure (easily controlled by low dose medication), moderate cholesterol moderate high (easily controlled by diet and medication).

Diagnostic Lab Data: The symptoms that I had were greatly increased symptoms, from what I had experienced before the vaccine. The nerve related pain, unsteady gait, weakness, etc. existed before the vaccine, but it was greatly increased after the vaccine. I don''t know if it was related. Since then, I had a brain MRI, 7/9/2021, and neck spine MRI on 7/23/2021. Neurology appointments. Added medrol pack, and gabepentin. Remaining on gabepentin. Follow up neurologist in September 2021.

Write-up: I got this on a Monday. This has been a horrible ordeal. I was a perfectly healthy person until mandated by work to get this vaccine. When I got the injection, before I left the pharmacy my left arm went numb. It stayed numb for a few hours. Then at dinner my esophagus spasmed and felt like a Charlie horse. That lasted for a few hours on and off. Then that night, normal flu/COVID symptoms which lasted for 1 -2 days-fever headaches, chills, achy, run down, trouble breathing. Thought I was on the mend and only had a slight fever Wednesday. Then Thursday I started to feel light headed and numb hands throughout the day. Around 1pm I had to pull my car over and have a friend continue driving as I was extremely light headed and my whole right arm had gone numb. Within 15 minutes of that I blacked out completely and when I came to, couldn?t breathe, both arms numb and tingling and then I continued to fade as if going to pass out again. My friend freaked out. And called 911 and I went to a hospital. They ran tests that apparently came back normal. I went home and that night and all day Friday I had chest pain, numb arms, dizziness and trouble breathing. Saturday morning (today)those symptoms were still present just not as pronounced only now I have sharp shooting pain randomly in my heart area, right rear of my head, and my legs. This is terrifying and I hope you people stop recommending this to healthy people.

Write-up: DEEP VEIN THROMBOSIS/BLOOD CLOT IN RIGHT LEG BELOW KNEE; BRUISING IN RIGHT HAND AND AREA IN LEFT HAND LIKE BRUISE; AREA OF BLEEDING UNDER SKIN IN LEFT UPPER ARM CLOSE TO VACCINATION SITE VERY DARK; 5 RED SPOTS FROM ELBOW TO HAND ON RIGHT AND LEFT ARM (BETWEEN THUMB TO INDEX FINGER); BLEEDING UNDER SKIN IN LEFT UPPER ARM CLOSE TO VACCINATION SITE; This spontaneous report received from a patient concerned an 83 year old female. The patient''s weight was 130 pounds, and height was 65 inches. The patient''s past medical history included ex smoker, surgery and concurrent conditions included bees allergy, insect allergy, alcohol user and non smoker and other pre-existing medical conditions included no drug abuse or illicit drug usage. The patient experienced drug allergy when treated with pregabalin, and oxycodone hydrochloride/paracetamol. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 1805031, expiry: unknown) dose was not reported, 1 total, administered on 18-MAR-2021 to left arm for prophylactic vaccination. No concomitant medications were reported. On 02-JUL-2021, before a couple of days of hospitalization, patient had a red rash on left upper arm. The area was close to where patient received the vaccine. On the same date, at the hospital, patient was told that it was bleeding under the skin and not a rash. The area turned very dark. The area on left hand was like a bruise. On the right arm, patient had 5 red spots that run from her elbow to right hand. It was unknown as to the exact date that the bleeding rash on left arm first occurred. The issue on the right hand is recent. On both hands, the affected area on both hands between the thumb and index finger. On 04-JUL-2021, the patient had swelling in both legs, was difficult to walk. On 05-JUL-2021, the daughter of the patient took to the hospital and had blood test, ultrasound, chest x ray, EKG (electrocardiogram), the physician diagnosed from ultrasound patient with a blood clot in right leg. The blood was 9.6. On the same date, the patient was admitted in the hospital for 4 days and was put on Heparin for 4 days, then stopped Heparin and switched to Eliquis. On 08-JUL-2021, patient was discharged from the hospital. The patient was just now getting the mojo back and was using a cane for a while. They were not concerned, but patient should see a nephrologist (emergency room physician at hospital said). Patient stated that hospital did not have a nephrologist during the four days of the patient. The patient had appointment with nephrologist tomorrow. Patient was concerned and lived by own. The diet of the patient was fish and fresh vegetables and numerous fruits. Treatment medications included: heparin. Additional treatment medications (dates unspecified) included: apixaban. Write-up: Patient is a 61 y.o. male with history of hypospadias/epispadias who presents to the emergency department for evaluation of urinary issues. A few days ago the patient noticed burning pain in his penis with urination and some residual pain, urinary frequency, and difficulty holding in his urine. The symptoms came on subtly, but have progressively worsened. He urinated 2 times at night last night, which was very unusual. He called a doctor and was told to come in. No hematuria, cloudy urine, fever, chest pain, scrotal pain, penile drainage, nausea, vomiting, constipation, diarrhea, abdominal pain, flank pain, or rectal pain. He denies dietary or lifestyle changes. He is married and not concerned for sexually transmitted infection. As well, the patient has had 4-5 days of sore throat and cough, and was put on a z-pack by his telemedicine doctor 3 days ago. He has taken 2 days of that. Write-up: On 5/2/21 I started experiencing pain in my upper back when walking. 5/3/21 I noticed I was short of breath going up the stairs 5/4/21 My shortness of breath got worse, when walking from room to room. I went to urgent care which sent me to the hospital. Write-up: At 4 am I awoke feeling poorly and had an alert from my watch that my heart rate was 140; I went to work (I am an ICU nurse) and felt fine until 08:30 when I felt heart palpitations, dizziness, light headedness, chest pain and pressure and I felt like I was going to pass out. I checked my pulse and it was 140. I asked my coworkers to put me on the monitor so they put me on pulse ox and checked my blood pressure. On pulse ox my oxygen saturation was 98%, my heart rate was 125 to 130 with some dropped beats that were non-perfusing. I was hypertensive at 160/90. This episode lasted about 10 minutes. After my heart rate normalized so did my blood pressure but I still felt lightheaded and dizzy and nauseated. After leaving work, I went to my doctors office. As I was being assessed by the nurse at 2:40 PM I felt another episode coming on. My heart rate was 150, I felt lightheaded and dizzy, nauseated, I felt like I was going to pass out, and I had chest pain and chest pressure. I had to lay back flat with my feet elevated so that I would not pass out. I was normotensive when the nurse checked my blood pressure. By the time the nurse got me on a 12 lead EKG, my heart rate has slowed down to 90 to 115, and appeared to be a sinus arrhythmia with increases in rate by 20 beats with any movement. For about 30 to 40 minutes I was unable to sit up because I still felt discomfort in my chest and I felt like I would pass out if I sat up. After I was able to move my heart was in a normal sinus rhythm and I was transferred to urgent care for further monitoring. In urgent care I had some blood sample sent including a CBC, BMP, D dimer, and troponin. The D dimer was normal and the troponin was negative. The BMP was normal, and the CBC was normal with the exception of A slight bump of my white blood cell count to 10.5, and a slightly elevated neutrophil count. I also received a chest x-ray which was normal. After I was discharged, I had another event at home with a heart rate of 100-115. This episode lasted about 30 minutes and I had to lie on my back with my feet elevated for the duration.

Write-up: I woke up on the morning of 07/28, with pain from arm and wrist. It was swollen and hurt when I twist it. After a couple days I purchase a brace for my wrist. I went to doctor 08/09 because the night before I woke screaming in pain. Doctor gave me a different brace to help with support.

Write-up: Immediate Severe burning with Injection, told by pharmacist normal for some...thought I was going to passout hurt so bad. Severe arm pain. Over next few days swelling rectangular shaped, showed pharmacist, again dismissed as normal reaction. Over next few weeks continued arm pain and swelling, as wll as other symptoms, numbness, tingling, burning up to shoulder with decreased range of motion. Then neck started hurting with decrease in range of motion as well. Went to Chiropractor, Xrays-assisted with neck slightly, but continued with pain and decreased range of motion in neck and right arm. Started to correlate onset of neckpain and shot as pain fromn neck down shoulder into lower arm. Went to Medical DR. CONTINUE
