Write-up: I experienced extreme vertigo on the in the middle of the first night after the shot. When rolling over in bed from flat on my back to my left side my body felt like it kept moving when I had in fact stopped. It feels like I am rolling out of a hammock. Right side has the same sensation. Also happens if I bend over when standing. It is not always accompanied with dizziness. Severity increased over a few days, then gradually decreased over the next 5 days or so and is now increasing again. I HAVE NEVER HAD THESE SYMPTOMS IN MY ENTIRE LIFE. My pre existing tinnitus has grown very much louder. I have had occasional low pain / pressure deep in my sinuses.

Write-up: 56 years old male called to report an adverse reaction after he received his jhnson and jhnson vaccine on 04/11/2021 @1230. patient reported 30 mins after receiving his vaccine he developed arm pain, diarrhea, and chills. patient reported sinus allergies. patient reported that he takes advil for his allergies. and amlodipine for his high blood pressure. patient was instructed to take advil to help with his symptoms. patient was instructed to seek medical attention if the symptoms got worse.

Write-up: I woke up three days after the shot with a headache. I was running and I got a sharp pain in my hamstring muscle in my right leg. I had to stop running and the leg pain persisted for two days. I went and saw my doctor and my tests were normal. The headache lasted for several days and was dull at first and then concentrated at the back of my head. The leg pain was present for a couple of days. All of the symptoms are gone after two weeks. I still haven''t returned to running daily as I normally do.

Write-up: Vaccine was administered on 4/8/2021 and patient stated on vaccine questionnaire that she has had a severe reaction to a vaccine in the past. When interviewed, she indicated that her physician did recommend she receive the vaccine. We confirmed that she should wait 30 minutes, which she did. She was accompanied by her husband at the visit as well. It was later reported that Sunday, 4/11/2021, she began having a rash (hives) and some breathing difficulty which caused her to go to the ER. Her breathing continued to be an issue and she was hospitalized on Wednesday 4/14/2021 (admitted to PCU per husband).

Write-up: Menstrual cycle came on 10 days early, extremely heavy and lasted for twice as long as normal. Also severe dizziness started 48 hours after vaccine and has not subsided.

Write-up: Approximately 2 hours after the vaccination I felt very minor side effects (headache / soreness on injection site). The next morning I felt the more intense side effects (fever of 101.1, chills, fever, extreme sweat, loss of appetite, body aches and fatigue). While the majority of the side effects have passed I still have no appetite. I am a very healthy adult that does strength training 4x a week, I typically eat and snack every 3 hours. Since the vaccination day I have no desire to eat and I am down to 2 meals (breakfast and dinner). I am also experiencing headaches and dizziness but this could be from the lack of nutrients. Because I feel dizzy I am forcing myself to eat as I almost fainted during a morning workout and had shortness of breath. This particular morning I had coffee, a protein shake and a banana prior to my workout. I have not read or seen any additional information on the loss of appetite lasting this long and I am getting concerned

Write-up: On Sunday morning i woke up to my left arm being swollen, red, hot to the touch. Over the course of the next few days the swollen part got bigger to the size of a baseball Tuesday I called my doctor and they wanted me to get seen right away at the ER. They did an ultrasound right away and they ruled out a blood clot and an abscess. They figured it would just be an infection. They put me on an antibiotic for 7 days up to 4x a day. Today 4/21 I took my last dose of the 7 days. Called my doctor and they are starting me on another 5 days of antibiotic to get rid of this infection in my arm. It is now down to a quarter size or a little bigger. Hoping that in the next 5 days it will be completely gone. I don''t have the redness and heat anymore just a smallish lump.

Write-up: Patient was nervous and shortly after injection reported that she had vision changes and got diaphoretic. While assisting the pt into a reclining position she briefly lost consciousness; total duration 3-5 seconds. She regained consciousness immediately once she was flat and her feet were elevated. I monitored her blood several times and it was normal. Over the course of 30-40 minutes she recovered and left the pharmacy.

Initial information was received on 13-APR-2021 and processed with additional information received on 20-APR-2021 via telephone log. The patient''s height, and weight were not reported. The patient''s medical history included flu shot in OCT-2020 without any side effects, gestational diabetes during pregnancy 6 years ago, and gave normal birth. The patient is a non-smoker and does not consumer alcohol. No surgery or injuries were reported. The patient did not have any prior clotting disorder or family history of clotting disorders. The patient did not have vericose veins. No allergies were reported. The patient received vaccination with JANSSEN COVID-19 VACCINE (covid-19 vaccine ad26.cov2.s) (suspension for injection, route of admin not reported, batch number: 206A217, expiry: UNKNOWN) dose was not reported, 1 total, administered on right deltoid muscle on 11-APR-2021 for prophylactic vaccination. No concomitant medications were reported as it was reported that patient did not take any medications. On 11-APR-2021, the patient received the vaccine and within 3 minutes, she felt dizzy and fainted and was taken by Emergency Medical Services to a hospital. Her vital signs were taken (pulse rate, blood pressure, and oxygen saturation) and patient was found to be hypotensive (the husband did not recall blood pressure level). When blood sugar was being tested using a needle for a prick, the blood was not coming out. Her blood sugar was normal at 129 and electrocardiogram was normal. The patient was given intravenous fluids and released home after 6 hours without any other treatment. The patient had no history of allergic reactions or reactions to a flu vaccine (last one received OCT-2020). No concomitant medications. Post vaccination the patient felt dizzy, could not sit on the chair and fainted and was taken to Emergency Room. Her blood pressure was low and she experienced hot flashes and watery eyes. Write-up: 10 days after the shot, my blood pressure rose to 198/112 and my pulse went to 161. and i called 911 and went to hospital. i had a severe headache for 3 days leading up to calling 911. finally on the April 1st, i went to hospital. i had a lump on my right shoulder never there before and i felt like the lump was causing my right side of my face to be numb. The hospital refused to believe me that i had some blood clot, they downplay the incident and instead gave me a brain CT SACN and brain MRA which was negative, they kept me for observation overnight and let me go noon the next day. since i have had follow ups with my regular doctor, a cardiologist and my nephrologist. I know that reaction was due to medication, either way glad i got the shot. BUT Since this time, I am retaining water, i know this because my rings do not fit anymore, rings i had for 20 years, never ever once had a hard time to put on or remove. I know my body, thats not the worse of it. i test my blood pressure every day at requesting of my kidney doctor, since the incudent of my pressure 198/112 my diastolic on average is raised 20 points. I log this daily. here are some examples from before the medication 111/67 , 109/63 , 129/64 , 105/62 120/81. now my pressure is totally different for example, 155/88 150/91 157/90 , 134/88. nothing else changed in my life, I am exactly the same as before only now i have covid shot.. yesterday i saw the 4th doctor since i am shot up and she gave me a new pill olmesartan medoxomil, only on it one day but my diastolic pressure is still much higher than before. i dont want to die, i dont want a stroke, or a heart attach, i have a lot to live for still. nobody is helping me, doctors are just throwing pills at me and not treating root cause which i feel strongly to beleive it is from covid shot.

Write-up: Three days after the vaccine I started to get a feeling of pressure on my chest. I woke up during the night because it was hard to breath. I had leg pain in both my sides. The leg pain lasted three days. I took it easy for the next two days. I called the doctor and she advised me to go to the ER. I was given an EKG and Chest X-Ray and blood work. I still have the feeling of pressure on your chest after two and a half weeks. It is a similar feeling to when I had the Covid-19 in October of 2020. Write-up: Patient states her joint pains were markedly worse the day after her vaccine dose, including new pain in the knees. Of note is that her rheumatoid arthritis has been worsening for the past 2 months since she stopped taking Xeljanz 3 months ago. (Her rheumatologist closed his practice and she was unable to establish with new one till 4/9/2021. Exam then showed now inflammatory changes in joints. Exam today by me also showed no inflammatory in joints, including knees. Labs ordered on 4/9 are just being drawn today.

Write-up: The night following my vaccination I ended up with a fever of over 100 degrees Fahrenheit and also suffered from a severe headache and sore throat. The fever and headache subsided after 24 hours, but the sore throat and ulcers on my gum line remained for the next 3-4 days. 9 days after I had gotten the vaccine, I started suffering from what I thought was a significant MS relapse. The left side of my face went numb and started drooping, my legs went completely numb, and my vision went blurry.

On an unknown date, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (unknown route) 1 dosage form. On 11-Apr-2021, the patient experienced PYREXIA (Fever), CHILLS (Chills) and HEADACHE (Severe headache). On 14-Apr-2021, the patient experienced MYOCARDIAL INFARCTION (Heart attack) (seriousness criterion medically significant). At the time of the report, MYOCARDIAL INFARCTION (Heart attack) outcome was unknown and PYREXIA (Fever), CHILLS (Chills) and HEADACHE (Severe headache) was resolving. Write-up: Pain from head to toes and feels like her body is wrap in rubber band and a lot of tingling, sore throat. PCC f/u s/w caller states she feels better. Before being vaccinated She has had" steroid shots and had some antibiotics" doesn''t remember the name of the antibiotics and she was feeling restless before getting the vaccine. She has had body myalgia but has resolved by now. Adv: Restlessness unlikely to be related to the vaccine. Myalgia is common to occur within first 24 hours and usually resolves on its on. Write-up: dizziness started 4/11 - 11 days after the vaccnine - followed by nausea and headache. this has gotten worse since then. was seen in ER on 4/13 at ER was then seen at my PCP on 4/12 and again yesterday, 4/22 - had full blood workup , and all blood work came out good / normal ranges, including WBC count - had ultra sound of left leg and normal and ekg as routine, also normal. went back to Dr. 4/23 because of bad dizziness lasting all day in waves and headache in temple area. now being treated with augmentin and prednisone for possible sinus infection, but i have never had this with any other sinus infection i ever had in the past. next step is MRI of the brain, which i really dont want to do. any help would be appreciated to rule out if this is due to the vaccine. very nervous. Write-up: Client fell while waiting in line to enter the POD. Client states that he got hot while waiting in line a long time. Client states that he does not think he lost consciousness but may have hit his head. 911 was called, EMS evaluated client and found him medically stable and client declined to go to the hospital. Write-up: Started having muscle pain; I didn''t have chills. I was very fatigued - in bed all day. I had a headache and the muscle pains went into my left side and into my left lung and every time I breathed I would get a sharp pain. It went into both of my breasts but worse on left side of breast. This lasted about three or four days. I went to the doctor on Thursday, the 15th. But now everything is cleared up and I feel good. Write-up: Patient, called on 04/22/2021 stating that she was going to have a colonoscopy today, but got cancelled due to her high glucose level of 400+ . Her colonoscopy is being performed for requirement of an upcoming weight loss surgery. Patient wanted to know if the blood sugar is high due to the recent covid vaccine she received, Johnson & Johnson by our office on 04/10/2021. Upon questioning patient stated she has had recent increased blood sugars since she has had the vaccine from her normal 160 after meals to 270 after meals. Patient c/o headache, run down, diarrhea, nausea and lower abdominal pain (Lower Middle Quadrant), which started the day after her vaccine and has not gone away. She takes Tylenol ES 500 mg twice daily in AM and PM, but only takes the edge off for her headache. Her arm was sore without redness or swelling at vaccination site, right deltoid. Patient did confirm with me that she does have an appointment with her family physician, today at 1:15 pm about an hour from now. I notified patient that she should inform her doctor of her recent J& J vaccine, we are reporting to Vaers, and to follow up with our office if she could. Patient verbalized understanding that this may or may not be attributed to her recent vaccine, needs to immediately contact and follow up with her physician, Dr.

Write-up: J&J COVID Vaccine EUA J&J Vaccine 4/10/2021 4/13/21: Believes she is having a reaction to her Johnson and Johnson COVID-19 vaccination. This vaccination was given on 04/10/2021. Patient is 39-year-old female with a past medical history significant for ulcerative colitis that has been in remission since the patient was 21 years of age, epileptic seizures on Vimpat and clonazepam. Patient comes in as a transfer with a chief complaint of possible reaction to Johnson and Johnson COVID-19 vaccine. Patient presents with multiple complaints. The patient states that upon receiving the vaccine on 04/10/2021 she suddenly developed presyncopal episodes where she felt like she was about to pass out but that subsided later on. 12 hours following the shot she had intense headache which was frontal and then later on travel to the back of her head and traveled down her spine. Patient described the headache to be the worse headache of her life, stated that it was worse than going through labor. Patient stated that she received morphine which helped her with the headache. Patient also complained of chest pain which was left-sided with no radiation, constant. No history of previous myocardial infarctions. Her troponins that were conducted were negative. Patient also complained of experiencing blood in her stool. She stated that when she wiped herself on the morning of 04/13/2021 she experience blood on the toilet paper and blood in the bowel. However, her stool occult blood that was conducted was negative. Patient is being followed by Neurology, she contacted her neurologist and they recommended to come to the ED with concerns of thromboembolism. Patient''s D-dimer was negative. Patient denied any recent travel anywhere or coming into contact with anyone sick. She denied developing any fevers cough shortness of breath abdominal pain nausea vomiting or any urinary concerns. 4/15/21: Patient is a 39-year-old female with a past medical history significant for ulcerative colitis that has been in remission since the patient was 21 years of age, epileptic seizures on Vimpat and clonazepam. Patient comes in as a transfer with a chief complaint of possible reaction to Johnson and Johnson COVID-19 vaccine. Headache: MRI brain and MRI venogram was ordered. MRI brain did show concerning signed for idiopathic intracranial hypertension. Neurology was consulted. Patient had high volume LP. Patient responded well post LP. Patient will be discharged on topiramate with a taper instructions per neurology. Patient will make appointment with Ophthalmology. Patient will follow up with Neurology as well. Chest pain concerning for ACS: Negative troponin. EKG normal sinus rhythm. Concern for vaccine reaction: Patient has J&J vaccine. No clots were noted. Patient''s symptoms are most likely are secondary to idiopathic intracranial hypertension and history of migraines. 4/16/2021: 39-year-old female presents emergency room with chief complaint of headache. Onset this morning. Headache is generalized. Headache is constant. Headache is worth with position change. Such as moving head side to side or with changing from lying to sitting and sitting to standing. Headache improves with lying supine. To has tried pushing fluids, taking over-the-counter medication, and also increasing her caffeine intake to offset the headache. She denies any of these helping with her symptoms. Patient''s pain is right an 8 out of 10. The patient denies anything additional alleviating and or aggravating factors to chief complaint. Today when the patient''s headache started. She contacted her neurologist. She was told to come to the emergency department for a blood patch. The patient added that her neurologist felt that she needs a blood patch not only because of the recent lumbar puncture but because there was at minimum 25 mL of spinal fluid removed at the time of her lumbar puncture. This lumbar puncture occurred under fluoroscopy. However, they had difficulty getting the pressure gauge on thus, more than 25 mL was removed.. Pain Pain Scale: 8 /10. Timing: constant. gradual. Character: mild. 4/19/21: patient reports intense headaches she attributes to the spinal tap. Ketorolac, DHE, ondansetron given.

The patient''s weight, height, and medical history were not reported. The patient received covid-19 vaccine ad26.cov2.s (suspension for injection, route of admin not reported, batch number: 1805018, expiry: 25-MAY-2021) dose was not reported, once total, administered on 27-MAR-2021 at Left Arm as prophylactic vaccination. No concomitant medications were reported. On 11-Apr-2021, the patient experienced blood in stool (bleeding in feces). It was reported that bleeding was almost like hemorrhage, a lot of blood came out and patient felt like patient was bleeding internally. Patient described pain like stomach was cut inside. The patient felt bad on the side of his stomach all over midsection. He mentioned that it started all of sudden, on same day an hour later he went to the restroom and went 3 times after in a row, on the day of report patient did not use the restroom and the pain was less.
