Write-up: I still felt the burning sensation and pain where the shot was given. Soon after, I started developing cysts on my scalp. No treatment received.

Write-up: My reactions with previous shots had gotten and I developed new ones. Lost 50 pounds, dizziness at least 10 times per day, ears began ringing periodically and got the chills often. Developed 2 new bumps on my penis and have muscle and stomach pains.

Write-up: Patient presents to flight Surgeon April 19, 8-15 cm area of erythema & warmth over olectanon process. Pt was hospitalized on IV antibiotics for 2 1/2 days, released to home on oral Augmentin for one week. 60 day follow-up dated 10/18/00 provided no additional data.

Write-up: Pt states that after receiving 3rd Anthrax shot, a few weeks post vax he experienced a mild fever (a little over 100) and it remained for about 6 weeks. Pt states that after that time there were no symptoms. Approximately on 6/28/99, he experienced a fever, no energy, increased bowel movement and burning upon urination. On 7/1/99 he saw an MD and had some tests run. He states that the MD concluded that it was the flu and sent him home. On 7/3/99, the fever persisted and the pt went to the ER. The pt was given some medication and an IV, the fever then lowered and the MD sent him home. On 7/4/99 the symptoms returned and this time they admitted him and started running test on him. This time his temperature peaked at 104.5. The pt states on 7/8/99, he was transported to another Medical Center where they continued testing. Approximately on 7/12/99 the fever broke and two days later they sent him home. About a month later he went to see an MD about an unrelated issue and was told that he would be on a temporary waiver pending investigations. Pt states also between the 1st and 3rd of July, 1999, he was experiencing discolored urine which was found to be bile.

Write-up: Within 20 minutes of injection, the pt began feeling tightness in his chest, difficulty swallowing and swelling under chin. Also complained of pruritus and tingling. Pt was given epinephrine and Benadryl with no change in symptoms. Second injection of epinephrine and oxygen started with IV fluids. Pt responded to treatment within 15 minutes of second epinephrine dose. 100% recovery.

Write-up: Received injection in 2 stages. On May 13, 2000 hands, forearms swollen, joints ached, shoulder sore, very tired. Used Benadryl also was given Cicsorex for pain. That did not help. Was given Advil 800 mg. As of 6/4/2000 pt still has fatigue, swelling and pain.

Write-up: Eight days post vax, the pt had viral syndrome symptoms and fever. One month post vax, the pt developed tinnitus and hearing loss in left ear.
